Background. Occupational therapists, especially those in education systems, are challenged to define their professional roles and identities. Purpose. This research evaluated the Role-Perception Questionnaire for Occupational Therapists in the Education System’s (RP-OT) psychometric properties, the Professional Identity Scale (PIS) internal consistency among occupational therapists, and relationships among role perception, professional identity, and demographic characteristics. Method. A sample of 147 occupational therapists in education systems completed the RP-OT and PIS. We conducted exploratory factor analysis and calculated Cronbach alpha to evaluate the RP-OT and PIS psychometric properties. Relationships were examined using Pearson correlation. Findings. The RP-OT was found reliable and valid, with medium-to-strong correlations among role perception, professional identity, and demographics. Teamwork in the education system was the major predictor (82%) of professional identity. Implications. Teamwork is part of the professional role of occupational therapists in the education system and a significant component for developing their positive professional-identity perception.

Partant des aspects démographiques de la proto-industrialisation, on examine le processus de transition qui mène soit à une industrialisation réelle, soit au contraire à un retour aux travaux des champs. Deux villages des Pays-Bas sont etudies à cette fin, situés dans la région de Twente, où domine une industrie textile enracinée dans l'agriculture. L'organisation de Pagriculture subit au début du 19e siècle des changements structurels, accompagnés d'une intervention de l'Etat dans l'économie régionale, avec pour conséquence un boom du tissage à domicile, ce qui, dans certaines communautés, conduit à l'industrie mécanisée et dans d'autres au contraire à un retour à la terre. La reconstitution des families permet à l'auteur de suivre le comportement démographique tant à l'époque de la protoindustrialisation qu'après, suivant l'un ou l'autre processus d'évolution. II s'avère que ni la proto-industrialisation, ni l'industrialisation, ni le retour à la terre n'ont, dans les deux villages étudiés, d'influence profonde sur le comportement démographique. II n'y a en effet pas de différences spectaculaires quant à l'âge au mariage ou à la dimension de la famille complète, entre groupes professionnels ou entre communautés; il n'y a pas non plus de changement majeur en ce domaine au long de la période étudiée.

Professional development in the K-12 English language teaching (ELT) classroom is an evolving entity that focuses on meeting the needs of changing demographics and latest educational trends. As a result, many texts have been published with the intention of providing the necessary skills educators need for success in their classrooms and to instruct a highly heterogeneous English for Speakers of Other Languages (ESOL) student population. Nonetheless, many of these resources focus on specific scenarios and instructional approaches that may not be applicable for all ESOL teachers. In this Perspectives article, I propose the incorporation of action research as a practice for K-12 teachers with the vision of empowering them to take control of their professional development and continue improving their instructional practices. Furthermore, I share three examples of how I have used action research in my own practice and the benefits obtained by my ESOL department, my ESOL students, and myself as a teacher researcher. Le perfectionnement professionnel dans les classes d’anglais du système d’éducation K-12 est une réalité changeante qui cherche à concilier les besoins de l’évolution démographique avec ceux des dernières tendances éducatives. De nombreux textes ont été publiés afin de procurer aux éducatrices et éducateurs les compétences nécessaires à leur réussite en classe, et également dans le but de renseigner une population étudiante hautement hétérogène dans le système d’enseignement de l’anglais à des apprenants étrangers (ESOL). Toutefois, plusieurs de ces sources mettent l’accent sur des scénarios spécifiques et des démarches pédagogiques qui peuvent ne pas convenir à toutes les enseignantes et tous les enseignants ESOL. Dans cet article de Perspectives, je recommande la pratique de la recherche-action aux enseignantes et enseignants du système d’éducation K-12 en vue de leur permettre de s’autonomiser pour prendre en main leur propre perfectionnement professionnel et continuer d’améliorer leurs pratiques pédagogiques. Je donne également trois exemples de la façon dont j’ai recouru à la recherche-action dans ma propre activité et des avantages qu’en ont tirés mon département ESOL, mes étudiantes et étudiants ESOL, et moi-même comme enseignant chercheur.

Abstract: In fall 2001, over 200 digital television channels were launched in Canada. One of those channels was WTSN (Women's Television Sports Network)-the world's first 24- hour television network exclusively dedicated to broadcasting women's sports. In the fall of 2003, however, WTSN ceased broadcasting operations. This analysis of CRTC policies and personal interview data with Canadian media members argues that while the demise of WTSN can be attributed to the unfortunate pitfalls associated with early digital television rollout and cultural policies, the network's downfall is best explained in substantially more ideological terms. From the outset, WTSN entered uncharted waters in the Canadian television sport landscape, attempting to showcase women's sports to a predominantly female audience-a demographic that has yet to materialize for mainstream sports programming.Résumé : En automne 2001 au Canada a lieu la lancée de plus de 200 chaînes de télévision numériques. Une de ces chaînes est WTSN (Women's Television Sports Network), le premier réseau de télévision au monde entièrement dédié à la diffusion des sports féminins 24 heures sur 24. WTSN, cependant, disparaît des ondes dès l'automne 2003. Cette analyse des politiques du CRTC et d'entrevues menées par l'auteur auprès de professionnels des médias canadiens soutient que, bien qu'on puisse attribuer l'échec de WTSN à certaines politiques culturelles ainsi qu'aux problèmes reliés à l'expansion trop hâtive de la télévision numérique à l'époque, on peut aussi tirer avantage d'une approche plus idéologique pour expliquer la disparition de ce réseau. RésuméDans les pays à haut revenu, il a été amplement démontré que la mortalité des adultes est plus faible au sein des catégories professionnelles favorisées ainsi que chez les personnes bénéficiant d’un niveau d’instruction élevé ou d’un revenu confortable. Dans les pays à revenu faible ou intermédiaire, les différentiels de mortalité aux âges adultes sont par contre nettement moins bien étudiés en raison du manque d'information sur les caractéristiques socio-économiques des personnes décédées. Cet article vise à évaluer si les informations fournies dans des enquêtes sur la survie des sœurs peuvent aider à combler cette lacune. Puisque les niveaux d’instruction sont très corrélés au sein des familles, les niveaux atteints par les fem­mes enquêtées peuvent servir à approximer le niveau d’instruction de leurs sœurs. Afin de quantifier les différences de mortalité, nous combinons toutes les Enquêtes Démographiques et de Santé dans un même modèle linéaire généralisé à effets mixtes. Dans la plupart des enquêtes, nous observons une relation inverse entre le niveau d’instruction et la mortalité adulte, en particulier dans les zones urbaines, mais des gradients opposés sont observés dans plusieurs pays durement frappés par l’épidémie du VIH/sida. L’hypothèse selon laquelle les femmes moins instruites auraient tendance à omettre disproportionnellement certains décès de leurs sœurs n’est pas vérifiée dans les données. Les données sur la survie des soeurs semblent plutôt bien refléter la complexité de la relation entre mortalité et éducation des adultes, en particulier dans les pays touchés par l’épidémie du SIDA. Dans l’ensem­ble, le recours à ces données ouvre de nouvelles voies pour mesurer les inégalités de mortalité aux âges adultes dans les pays à revenu faible ou intermédiaire.AbstractIn high-income countries, there is extensive evidence showing that higher levels of educational attainment, higher income and higher occupational classes are correlated with lower mortality rates among adults. In low- and middle-income countries, far less is known on mortality differentials in this age group due to the lack of information on the socio-economic characteristics of the deceased. In this paper, we evaluate whether survey reports on the survival of sisters can help to fill this gap. We find that levels of educational attainment are correlated within families, and therefore characteristics of the deceased can be inferred from levels attained by their sisters responding to the survey. Because estimates based on sibling survival histories have large confidence intervals, we pool all Demographic and Health Surveys together and apply a generalized linear mixed-effects model to capture mortality differentials. In most surveys, higher education is indeed associated with lower risks of dying, especially in urban areas, but reverse gradients are observed in several countries experiencing generalized HIV epidemics. There is limited support for the hypothesis that women with less education are disproportionately omitting to report some deaths. Instead, we argue that sibling histories truly reflect the complexity of the relationship between adult mortality and education, especially in countries affected by the HIV/AIDS epidemic. Overall, the sibling approach offers new avenues for measuring inequalities in adult mortality in low- and middle-income countries.

Abstract:
Objectives: Major depressive disorder (MDD) is associated with impairments in both cognition and functioning. However, whether cognitive deficits significantly contribute to impaired psychosocial and occupational functioning, independent of other depressive symptoms, is not well established. We examined the relationship between cognitive performance and functioning in depressed patients before and after antidepressant treatment using secondary data from the first Canadian Biomarker Integration Network in Depression-1 study. Methods: Cognition was assessed at baseline in unmedicated, depressed participants with MDD ( n = 207) using the Central Nervous System Vital Signs computerized battery, psychosocial functioning with the Sheehan Disability Scale (SDS), and occupational functioning with the Lam Employment Absence and Productivity Scale (LEAPS). Cognition ( n = 181), SDS ( n = 175), and LEAPS ( n = 118) were reassessed after participants received 8 weeks of open-label escitalopram monotherapy. A series of linear regressions were conducted to determine (1) whether cognitive functioning was associated with psychosocial and occupational functioning prior to treatment, after adjusting for overall depressive symptom severity and (2) whether changes in cognitive functioning after an 8-week treatment phase were associated with changes in psychosocial and occupational functioning, after adjusting for changes in overall symptom severity. Results: Baseline global cognitive functioning, after adjusting for depression symptom severity and demographic variables, was associated with the SDS work/study subscale (β = −0.17; P = 0.03) and LEAPS productivity subscale (β = −0.17; P = 0.05), but not SDS total (β = 0.19; P = 0.12) or LEAPS total (β = 0.41; P = 0.17) scores. Although LEAPS and SDS scores showed significant improvements after 8 weeks of treatment ( P < 0.001), there were no significant associations between changes in cognitive domain scores and functional improvements. Conclusion: Cognition was associated with occupational functioning at baseline, but changes in cognition were not associated with psychosocial or occupational functional improvements following escitalopram treatment. We recommend the use of more comprehensive functional assessments to determine the impact of cognitive change on functional outcomes in future research.

ObjectiveThe study describes the characteristics of attendances for low back pain (LBP) in the French emergency departments (ED) network Oscour®, in order to give an overview of this disease before launching a prevention campaign.IntroductionLBP is one of the leading contributors to disease burden worldwide [1]. In France, LBP is a frequent reason of general practice consultations. According to a study published in 2017 and based on 2014 data issued of the National Health Insurance Cross-Schemes Information System (Sniiram) [2], this pathology stands for 30% of thickness leave and 4 of 5 people will suffer of low back pain during their own life. Most often, LBP is a chronic pathology with acute episodes which most often require emergency care.In order to prevent chronicity, French health care insurance launched into a mainstream national prevention campaign during spring 2018. This campaign was also targeted for health professional to inform them of the best recommendations to provide to their patients. Then the French society of emergency medicine (SFMU) has been asked to relay this campaign to emergency departments (ED) where LBP is a frequent reason of attendance.Since 2004, the French syndromic surveillance system SurSaUD® [3] coordinated by the French Public Health Agency (Santé publique France) daily collects morbidity data from the emergency departments (ED) network Oscour®. Almost 92% of the French ED attendances were recorded by the system in 2017.The availability of this large ED dataset on the whole territory since several years gives the opportunity to describe LBP attendances before the potential fallout of the national prevention campaign.MethodsFor each ED attendance, the SurSaUD® system daily collects individual data containing demographic (age, gender, zip code), administrative (ED unit, date of attendance, transport…) and medical information (medical diagnosis (ICD10), chief complaint, severity, hospit.). These data are routinely analyzed to detect and follow-up various expected or unusual public health events all over the territory [3] and also constitute a large database to perform in-depth studies on specific public health issues.ED attendances with a medical diagnosis of LBP have been identified using at least one of the following ICD10 codes “M545”, “M5450”, “M5456”, “M5457”, “M5458”, “M5459”. Those data have been analyzed from 01/01/2014 to 31/12/2017 (504 ED) for the following age groups; less than 18 years old (yo), 18 to 34 yo, 35 to 49 yo, 50 to 64 yo, 65 to 84 yo and 85 yo and over, at national and regional levels. ED attendances have been also described by month, day of week and hour of day. Hospitalizations after discharge, stay duration in ED services, transport and associated diagnoses were also analyzed.ResultsFrom 2014 to 2017, 481,291 ED attendances for LBP were recorded corresponding to 1.12% of the total number of ED attendances with a coded diagnosis. 60% of annual ED attendances for LBP concern 18 to 50 years old adults. The proportion of LBP attendances among the all-cause activity remains stable between 2014 and 2017.At the regional level, LBP proportion among the all-cause activity is similar to the national value in metropolitan regions (0.8% in Brittany to 1.6% in Corsica) and is lower than the national value in overseas regions (0.4% in Mayotte to 0.8% in Guyane) except for Saint-Barthélémy (1.8%).At the national level, almost 10% of ED attendances for LBP are hospitalized after discharge. This proportion increases with age to reach 43% for the 85 years old and more. Proportion of hospitalization ranges between 5.6% (in Paris area) and 17.1% (in Brittany) in metropolitan regions and between 2.8% (Guyane) and 9.3% (Reunion island) in overseas regions.From 2014 to 2017, ED attendances for LBP remain stable by month. However, we observed a slight decrease along the week with more attendances on Monday (17.8% of LBP attendances) than the other days. The attendances are more frequent in the morning (between 6 and 12 AM).At the national level, mean stay duration for LBP attendances in ED is almost 5 hours whereas median stay duration is 2 hours and 45 minutes. Stay duration is longer for patient arrived during night hours (from midnight to 6 AM) and for those hospitalized after discharge. Stay duration is also increasing with age. At the regional level, mean stay duration varies from 3 to more than 6 hours.ConclusionsThe broad coverage of the French ED network on the whole territory since several years enables to give an overview of ED attendances for acute LBP and their characteristics.One strength of the system is its strong partnership between epidemiologists and the ED physicians.
