Abstract:
Health is determined by the past as well as the present; the health status of indigenous peoples has been strongly influnced by the experience of colonisation and their subsequent efforts to participate as minorities in contemporary society while retaining their own ethnic and cultural identities. Colonial journays may have led to innovation and adaptation for Maori, but they have also created pain and suffering from which full recovery has yet to be felt (Durie, 2001). The oral health area can be described as having considerable and unacceptable disparities between Maori and non-Maori (Broughton 1995; Thomson, Ayers and Broughton 2003). Few reports have been conducted concerning Maori and patterns of oral health service utilisation, however a lower service utilisation among Maori than non-Maori has been noted (TPK 1996; Broughton and Koopu 1996). Overall, Maori oral health is largely unknown due to a paucity of appropriate research. This research aims to provide new information by describing Maori oral health outcomes over the life course, within a Kaupapa Maori Research (KMR) methodology. In general, the basic tenets presented for KMR are: (1) to prioritise Maori - from the margin to the centre; (2) to be Maori controlled - by Maori, for Maori; (3) to reject �victim-blame� theories; and (4) to be a step towards action and change in order to improve Maori oral health outcomes. The aims of this research are to: 1. Describe the occurrence of caris at ages 5, 15, 18 and 26 and periodontal disease at age 26 years for Maori. 2. Describe self-reported oral health, self-reported dental aesthetics and oral health service utilisation among Maori at ages 5, 15, 18 and 26. 3. Compare the above oral health characteristics between Maori and non-Maori . 4. Investigate the determinants of any differences in oral health outcomes between Māori and non-Maori using a KMR methodology. The investigation involves a secondary analysis of data from the Dunedin multidisciplinary Health and Development study (DMHDS). The existing data-set was statistically analysed using SPSS (SPSS Inc, Chicago, USA). Descriptive statistics were generated. The levels of statistical significance were set at P< 0.05. Chi-square tests were used to compare proportions and independent sample t-tests or ANOVA were used for comparing means. A summary of the Maori/non-Maori analysis shows that, for a cohort of New Zealanders followed over their life-course, the oral health features of caries prevalence, caries severity, and periodonal disease prevalence are higher among Maori compared to non-Maori. In particular, it appears that while Maori females did not always have the highest prevalence of dental caries, this group most often had a higher dmfs/DMFS for dental caries, compared to non-Maori. As adolescents and adults, self-reported results of oral health and dental appearance indicate that Maori males were more likely to report below average oral health and below average dental appearance, when compared to non-Maori. However, at age 26, non-Maori males made up the highest proportion of episodic users of oral health services. This study has a number of health implications: these relate specifically to the management of dental caries, the access to oral health services, and Maori oral health and the elimination of disparities. These are multi-levelled and have implications for health services across the continuum of care from child to adult services; they also have public health implications that involve preventive measures and the broader determinants of health; and involve KMR principles than can be applied to oral health interventions and dental health research in general. Dental diseases and oral health outcomes, such as dental anxiety and episodic use of services, are a common problem in a cohort of New Zealanders with results demonstrating ethnic disparities between Maori and on-Maori. As an area of dentistry that has had very little research in New Zealand, the findings of this study provide important information with which to help plan for population needs. The KMR approach prioritises Maori and specifically seeks to address Maori oral health needs and the elimination of disparities in oral health outcomes. While the issues that are raised may be seen as the more difficult to address, they are also more likely to achieve oral health gains for Maori and contribute to the elimination of disparities.

Presents results of 2 longitudinal studies investigating the oral health of older adults with dementia, using questionnaires and clinical inspections at baseline and one year. Groups studied were nursing home residents and those living in the community, with moderate to severe dementia or no dementia diagnosis. Caries experience was related to dementias severity and not to specific dementia diagnoses. Coronal and root caries experience was higher in dementia participants with moderate-severe dementia, the socio-economically disadvantaged, more functionally dependent, taking neuroleptic medications with high anticholinergic adverse effects, with eating and swallowing problems, were not attending the dentist, who needed assistance and were behaviourally difficult during oral hygiene care and whose carers were burdened.

Despite surveys being considered important to evaluation, there is only an incipient literature concerning the assessment of epidemiological surveys´ work process. This cross-sectional study, employing quantitative and qualitative approaches, aimed to assess the perception of the 2010 Brazilian Oral Health Survey (SBBrasil 2010) coordinators of the survey´s work process. An electronic semi-structured questionnaire was sent to all SBBrasil 2010 coordinators (N=213). The investigated variables were grouped into the following topics: respondents´ characteristics, role during the survey, qualification, operational aspects, interpersonal relationship, financial aid, advertisement, work experience, usefulness of the survey, self-assessment, future oral health surveys´ participation, and suggestions for future surveys. Response rate was of 75.6% (N=161). Most respondents were dentists (90%), aged between 30 and 49 years, females (68%), in management position (75.8%), permanent workers at the public health system (59.2%) and acted as municipal coordinators of the survey (77.6%). Reported positive aspects of the survey were: planning, training and calibration workshops, logistic support provided by both the health system and the surveys´ coordination, good relationship between staff, and survey acceptability by the participants selected for the sample and by the health professionals. The transport allowance/financial aid, the perception of inconsistency between the allowance granted and the delay in payment were considered negative aspects. The respondents reported the SBBrasil 2010 experience as useful to train staff to perform data collection and showed interest in participating in future oral health surveys. Of the seventeen aspects studied, only three were identified as shortcomings of the SBBrasil 2010's work process: the transport allowance, isolated cases of conflict between staff and conflict between members of the coordination. All of the other aspects were mainly facilitators of the work process. The most frequently cited suggestion for future surveys was changes in methods (34.4%), followed by general issues related to the conduction of the survey (31.1%) and also higher pay (23.3%). It is concluded that the perception of the coordinators of the National Oral Health Survey (SBBrasil 2010) of the work process studied was mainly positive. The results may contribute to the improvement of the health professionals´ work process in future oral health surveys.<br>O processo de trabalho em inquéritos de saúde pode interferir na qualidade das informações produzidas, que posteriormente são utilizadas tanto na análise da vigilância em saúde de uma população como na tomada de decisões. Apesar do reconhecimento da importância atribuída à avaliação, a literatura se mostra incipiente em estudos que analisem o processo de trabalho na pesquisa epidemiológica. Este estudo tem como objetivo identificar a percepção dos coordenadores da Pesquisa Nacional de Saúde Bucal (SBBrasil 2010) sobre o processo de trabalho realizado. Trata-se de um estudo transversal, com abordagem quanti-qualitativa, utilizando um questionário eletrônico semiestruturado, que foi enviado para os indivíduos que atuaram como coordenadores do SBBrasil 2010 (n=213). As variáveis pesquisadas foram agrupadas nos seguintes eixos temáticos: características dos participantes, função exercida na pesquisa, qualificação, aspectos operacionais, relacionamento interpessoal, ajuda de custo, divulgação, experiência profissional, utilidade da pesquisa, autoavaliação, participação em futuros inquéritos de saúde bucal e sugestões para futuros levantamentos. A taxa de resposta foi de 75,6% (n= 161). A maior parte dos respondentes eram cirurgiões-dentistas (90%), entre 30 a 49 anos, do sexo feminino (68%), em cargo de gestão no serviço público (75,8%), com vínculo efetivo (59,2%) e desempenhou a função de coordenador municipal na pesquisa (77,6%). Os pontos positivos relatados foram: as oficinas de planejamento, de treinamento e calibração; o apoio logístico tanto do serviço quanto da coordenação da pesquisa; o bom relacionamento entre os membros da equipe e a aceitação da pesquisa pelos indivíduos selecionados para a amostra e pelos profissionais envolvidos. O custeio para deslocamento inerente à pesquisa, a percepção de incoerência do valor de custo recebido e o atraso no seu pagamento foram considerados pontos negativos a serem superados. A maioria dos coordenadores relataram que a experiência do SBBrasil 2010 serviu para qualificar a equipe de campo na coleta de dados e manifestaram interesse em participar de futuros inquéritos em saúde bucal. Dos dezessete aspectos estudados, apenas três foram apontados como dificultadores do processo de trabalho no SBBrasil 2010: ajuda de custo para deslocamento, alguns casos isolados de conflito com a equipe e de conflito com algum outro membro da coordenação. Os demais aspectos foram predominantemente assinalados como facilitadores do processo de trabalho. Dentre as sugestões apontadas para futuros levantamentos, mudanças na metodologia (34,4%) foi a mais citada pelos coordenadores, seguida por aspectos gerais relacionados à realização da pesquisa (31,1%) e por melhor remuneração (23,3%). Conclui-se que a percepção dos coordenadores da Pesquisa Nacional de Saúde Bucal (SBBrasil 2010) sobre o processo de trabalho investigado foi predominantemente positiva. Espera-se que os resultados possam contribuir com o aprimoramento do processo de trabalho dos profissionais em futuros levantamentos em saúde bucal.

Methods: This is a cross-sectional study of clinical and self-reported oral health in two samples of veterans: 538 users of Department of Veterans Affairs (VA) outpatient medical clinics (VHS) and 278 veterans from VA’s Dental Longitudinal Study (DLS). Clinical data incIuded number of teeth, tooth mobility, periodontal treatment need, an index of root caries, coronaI caries, oral mucosal status and denture stability and retention. Health-related quality of life (HRQOL) was measured by the Veterans SF-36. Oral-specific heaith-related quaIity of life (OQOL) measures included the single item seIf-report of oral health (OH1), the Oral Health Impact Profile (OHIP), the Geriatric (General) Oral Health Assessment Instrument (GOHAI), the D-E-N-T-A-L, a screening measure of need for care and the Oral Health Quality of Life (OHQOL) measure. Results: Self-reported oral problems are significant burdens on the health and function of users of VA outpatient care. Self-reported oral health, as measured by the GOHAI, is associated with the general well-being of users of VA care. VA patients have worse clinical and self-reported oral health than community dwelling men of similar age. Clinically determined need for dental care was universai in the VHS and present in two-thirds of the DLS. The OH1 and the DENTAL are useful in identifying who needs dental care in the combined population. Criterion and construct validity of self-reported measures of oraI heaIth (OH1, OHIP, GOHAI) are supported by their associations with selected sociodemographic, behavioral and dental conditions in users of VA care. Validity is further supported by the association with recency of dental care and reason for last visit in the VHS. Conclusions: VA outpatients have significant oral health needs as measured by self-report and clinical measures. Users of VA care have worse oral health than the general population. The OH1 and the DENTAL can help identify veterans who are in need of dental care. Longitudinal studies are needed to evaluate the impact of oral health care on general health and well-being and to estimate the resources needed to meet the needs of veterans eligible for care in VA.

Abstract:
Implementation of electronic health records by the Health Information Technology for Economic and Clinical Health has led to the implementation of electronic dental records (EDRs) and digital radiography in dental offices. The purpose of this study was to determine the state of the implementation and usage of EDRs and digital radiographs by the private general and pediatric dental practices in Mississippi as well as reasons why the dental practices are not moving forward with the advanced technology. A survey was emailed to 712 dental practices: 116 responded (16% response rate), and 104 consented to participate (89.66%). Results indicated dental practices in Mississippi using EDRs was 46.07%, EDRs with paper records was 42.70%, and only paper records was 11.24%. Results indicated dental practices using digital radiography was 76.40%, conventional radiography was 13.48%, and both was 10.11%. Common reasons for not advancing were cost, insufficient training, computer/software issues, and “too old.”

Abstract:
Safety net clinics across the country struggle with a lack of resources to tackle the needs presented. Screening programs set up for children and elderly have proven to be effective in triaging need, prioritizing care, and maximizing resources. These programs do not currently exist for working uninsured adults. Research was initiated to answer the question: Does the screening process improve patient care for the community clinic? During a 6-week pilot study a licensed dental hygienist performed 30 screenings in the community clinic setting. Findings were recorded and coded according to patient’s level of need identified. Pre- and posttest data for patient care factors were attained. Statistical tests showed a significant effect on patient care factors. While the evidence existed to support the implementation of screening, more research would quantify the specific impact on this population.

Oral Biology<br>M.S.<br>Abstract Introduction: Early Orthodontic Treatment (EOT) has been extensively studied, but questions still remain regarding the prevalence of its need in the United States. Without information regarding the epidemiology of EOT need, it is difficult to make determinations as to its viability as a Medicaid service. The Index for Preventive and Interceptive Orthodontic Needs (IPION) developed by Coetzee is the only index specifically meant for children in the mixed dentition. The purpose of this study was to utilize the IPION to measure EOT need in two pediatric dental populations in Philadelphia. Methods: 87 children between the ages of 6 and 9 were screened using the IPION. Overall scores, demographic information, and prevalence of specific malocclusions were recorded and analyzed. Results: A substantial proportion of children fell into the definite treatment need category (60.92% ±5.2% when including restorations and caries in the score, 31.03%±5.0% when excluding restorations and caries from the score). Site of screenings, race and sex had no significant effects on prevalence of EOT need, while IPION rubric used (IPION6 versus IPION9) had marginally significant effects on the prevalence of EOT need. Conclusions: There is a significant proportion of children in Philadelphia pediatric dental populations who have unmet EOT needs. Patients, communities, Medicaid, orthodontists and residents could all benefit from treatment of these children if policy were changed to allow EOT to be covered by Medicaid in Pennsylvania.<br>Temple University--Theses

Estudos epidemiológicos são a única forma de propiciar uma visão do estado de saúde bucal e necessidades de tratamento de uma população, e as possíveis relações entre os fatores ambientais e a prevalência de cárie. Estudos desta natureza, no Brasil e no mundo, tem sido dirigidos a grupos de estudantes da rede pública ou trabalhadores da indústria ou comércio. Estretanto, face à situação atual de alto desemprego e um grande número de crianças fora da rede escolar, usou-se uma metodologia que permitiu o sorteio de quarteirões de todos os bairros urbanos da cidade de Bauru. Todas as residências dos quarteirões sorteados foram visitadas e 1839 foram examinadas. No levantamento epidemiológico foram examinados 6.539 indivíduos de mais de 3 anos de idade, que foram classificados quanto à idade, sexo, classe socio-economica, nível de escolaridade e atenção odontológica recebida nos últimos doze meses prévios ao exame. Baseados na análise estatística dos resultados, pode-se concluir que: 1 - O padrão de saúde bucal nas faixas etárias testadas, em relação ao número de pacientes isentos de cárie, CPOD, última visita ao dentista, dentes restaurados e cariados mostrou não have diferenças estatisticamente significantes entre os sexos. Entretanto, houve diferenças estatisticamente significantes entre os sexos em relação ao número de dentes perdidos no sub-grupo de 35-44 anos, tendo o sexo feminino perdido mais dentes que o masculino. 2 - O padrão de saúde bucal na faixa etária de 3-4 anos de idade mostrou diferenças estatisticamente significantes entre as classes sociais, tendo as de nível sócio-econômico mais elevado mostrado maior número de crianças com CPOD igual a zero. Já nas faixas etárias de 5-6 e 12-13 anos, esta diferenã não foi observada. Quando o CPOD e o número de dentes perdidos foram considerados para o grupo 12-13 anos, não houve diferença estatisticamente significante entre as distintas classes sociais. Para a faixa etária de 35-44 foi observada uma diferença estatisticamente significante quando ao número de dentes perdidos, sendo que as classes mais privilegiadas perderam menos dentes que as demais. No sub-grupo de 12-13 anos houve diferença estatisticamente significante, tanto para dentes perdidos, sendo que as classes mais privilegiadas perderam menos dentes que as demais. No sub-grupo de 12-13 anos houve diferença estatisticamente significante, tanto para dentes restaurados, quando para dentes cariados. As classes mais privilegiadas mostraram maior número de dentes restaurados, enquanto as demais mostraram maior número de cariados. As comparações entre os nossos dados e as metas estabelecidas pelo OMS para o ano 2.000 apontam para a necessidade do aumento de 38% para 50% de crianças isentas de cárie, a diminuição do CPOD aos 12 anos, de 4,87 para 3 e o aumento do percentual de indivíduos com todos os dentes aos 18 anos, de 51% para 85%. Certamente estes índices não serão conseguidos dentro do modelo atual de atendimento. Atitudes firmes necessitam ser tomadas para reverter o quadro atual.<br>A sampling survey may be the only feasible method of collecting the relevant data on disease distribution for use in planning and evaluating the communities dental status, dental need and how these states are influenced by the enviroment. Epidemiological investigations in the world and Brazil have been generally directed to more accessible population groups such as school age children or organized groups like factory workers. Howevwe, with the particular situations wich face the population nowadays such as high inflation rate, unemplyment, and a great number of children out of the school system, the sampling selection as performed in two stages. In the first stage, blocks of all the urban districts that divide the city were randomly selected. In the second stage, all the dwellings in each block were selected. Of the 1,927 families visited, 1,839 agreed to be examined, representing a response rate of 95,4 per cent. Examinations were provided for 6,598 individuals from 1,839 families. The subjects were between the ages of 3 and 92 years-old. The individuals were classified according to sex, age, educational level, socio-economic status and having received dental treatment in the last 12 months previous to the examination. To assess the oral health of each individual, information about dentition status and treatment need, prosthetic status and denture requirements as well as conditions needing immediate attention were obtained according the criteria proposed by WHO. According to the experimental conditions of this investigation and based upon on the statistical analysis of the data, the following conclusions were drawn: 1- No significant difference was found by sex in the DMFT score, last visit to the dentist, caries free children, filled or decay score. However, significant differences were detected by sex in the M-D score among the 35-44 age goup, and females presented higher numbers of missing teeth than males. 2 - Significant differences was found among children aged 3-4 in the number of caries free by socio economic status. In the same way, among people of 35-44 years of age, significant difference was found in the number of missing teeth by socio economic status and the M-T is inversely related to the social class; the higher social class the lower the mean number. Significant differences were detected in both the number of filled and decayed teeth among people aged 12-13. With higher social class, the number of filled teeth was higher and the number of decayed teeth was lower. However, no significant differences were found by socio economic status in both DMFT score and missing teeth among the age-groups 5-6, 12-13. 3 - By the comparasion between our data and the WHO\'s desire for the year 2000 to achieve their goals, it will be necessary to improve both the number of carie free children from 38% to 50%, and the number of people who had all of their teeth from 51% to 85%. It will also be necessary at the same time to decrease the DMFT from 4,87 to 3 for 12 years-old people. These goals will not be achieved with the dental care system available nowadays. A change in attitude must be taken to improve the current situation.

Monte Negro é um município de pequeno porte com características rurais no interior do Estado de Rondônia, Brasil, que não contava com atendimento odontológico público e nem com cirurgião-dentista da rede particular residente no município até o ano de 2003. Buscando o desenvolvimento de um modelo de programa odontológico preventivo que não seja baseado na presença constante do profissional de saúde, com o envolvimento da comunidade, um programa baseado em escovação supervisionada por agente da comunidade e em ações educativas foi implantado na Escola Estadual de Ensino Fundamental Mato Grosso, localizada nesse município, e é avaliado nesse estudo. Foram examinadas 241 crianças matriculadas de primeira a quarta série, com média de idade de 8,77 anos. O CPOD médio foi de 1,51 e o ceod foi 2,69. Apenas 15,35% das crianças eram livres de cárie. A maior parte das crianças (82,57%) apresentava alguma necessidade de tratamento odontológico. Preconizou-se uso da técnica de Stillman modificada para escovação e a qualidade da escovação executada pela criança foi avaliada pelo índice PHP. O índice foi avaliado em quatro momentos ao longo do ano e as médias obtidas foram: 2,172; 1,278; 0,878; e 0,734. As reduções nas médias de superfícies coradas indicam que as crianças aprimoraram progressivamente a técnica ensinada. Pode-se concluir que o programa baseado em escovação supervisionada por um agente da comunidade devidamente treinado pelo cirurgião-dentista para esse fim e em promoção de saúde foi eficiente na melhora da coordenação motora da criança no período de um ano. E ainda, sugere-se que programas odontológicos preventivos e educativos sejam executados nas escolas de modo continuado e rotineiro como fator redutor de disparidades sociais que poderiam se refletir na saúde bucal das crianças, mesmo em localidades onde não se pode contar com a presença constante de cirurgião-dentista.<br>Monte Negro is a little rural town located in State of Rondonia, Brazil. There were any dentist who offer odontologial care both at particular or public systems until 2003. A program based at school supervised tooth brushing by a person living at this community was installed at local public primary school named Escola Estadual de Ensino Fundamental Mato Grosso in attempt to draw a dentistry program model that is more dependent of community involvement than on full time presence of a Dentist. 241 children coursing first to fourth degree were examined. Mean age was 8.77 years old. Mean DMFT was 1.51 and dmft was 2.69. Only 15.35% children were zero carie. Major portion of children (82.57%) had some oral treatment need. It was recommended modifing Stillman technique for tooth brushing. Quality of children toothbrushing was evaluated by PHP index. PHP was evaluated at four moments during the year and their means were: 2.172; 1.278; 0.878; and 0.734. Decline in means of stained surfaces were statistical significants at every moment (p<0,05), showing that children had been performance progressively better at teached technique. It can be concluded that the program based at health promotion and supervised toothbrushing by an communitary agent trained by a Dentist was efficient at children motor coordenation durant an year period. And it can be suggested that dentistry prevention and educative programs should happen at school in a rotinary and continuous way as a social difference redutor factor that would afect oral health of children, even where there is not always the presence of a Dentist.

The study was nested in a population-based birth cohort started in 1982 in Pelotas, Brazil. A representative sample of 720 young adults was evaluated. Dental examinations and interviews were performed at 15 and 24 years old. Use and need of prosthesis was the outcome. Exploratory variables included demographic and socioeconomic (at birth, 15 and 24 years old), oral health and dental service utilization patterns (at ages 15 and 24). Prevalence of outcomes was calculated and Poisson Regression was used to determine risk factors to needs of dental prosthesis. The prevalence of use and needs of dental prosthesis was 2.08% and 29.72%, respectively. Multivariable analysis showed that poorest family socioeconomic trajectory [PR= 1.56 IC95% (1.08-2.26)], lower mother s educational level at children birth (p=0.023), lower family income at children birth [RP= 1.37 (1.01-1.86)], no oral hygiene instruction by the dentist [RP=1.64 IC95% (1.11-2.41)] and caries presence (high DMFT tertile) (p<0,001) were associated with prosthetic treatment needs. Em 1982, todos os nascimentos hospitalares ocorridos em Pelotas foram identificados e estes indivíduos foram acompanhados por inúmeras vezes. Em 2006, uma amostra de 720 indivíduos pertencentes a esta coorte foi visitada para realização de exame bucal e aplicação de questionário incluindo questões de uso de serviços odontológicos, hábitos e comportamentos relativos à saúde bucal. Foi calculada a prevalência dos desfechos e utilizada Regressão de Poisson para estimar os fatores de risco à necessidade de prótese. A prevalência de necessidade e uso de prótese foi de 29,7% e 2,1% respectivamente. A análise multivariável mostrou que a trajetória de pobreza sempre ao longo da vida [RP= 1,56 IC95% (1,08-2,26)], a baixa escolaridade materna ao nascer (p=0,023), a baixa renda familiar ao nascimento [RP= 1,37 (1,01-1,86)], a não recepção de instrução de higiene oral por parte do dentista aos 15 anos [RP=1,64 IC95% (1,11-2,41)] e a presença de cárie pelo CPO-D (p<0,001) foram associadas à necessidade de prótese aos 24 anos. Dental caries is the most common chronic disease in children, and prevalence rates are disproportionately higher in American Indian/Alaska Native (AI/AN) populations. The Association of State and Territorial Dental Directors (ASTDD) recommends annual oral health screening for children in Head Start programs using the Basic Screening Survey (BSS). The 2014 study was a follow-up to a 2010 national survey of AI/AN children ages five and under that assessed oral health outcomes such as untreated decay, decay experience, urgent need for treatment, presence of sealants and decayed, missing and filled deciduous teeth (dmft) scores, and investigated the changes in Early Childhood Caries (ECC) prevalence from 2010 to 2014 in the Indian Health Service (IHS) Nashville Area. A gap analysis was completed comparing current recommended practices among dental clinics that participated in the IHS ECC Collaborative ASTDD Framework to Prevent and Control Childhood Tooth Decay (ASTDD Framework). Due to historical mistreatment of AI/AN populations in research, and out of respect for the sovereignty of the Tribal Nations that participated in the study, there limited data was made available for this study. In 2010, 579 children were screened in the Nashville Area; 1231 children participated in 2014. While there was a statistically significant, yet clinically small 9.36% reduction of untreated decay from 2010 (30.33%) to 2014 (27.49%), the ECC Collaborative did not reach their objective of a 25% reduction. There was also a significant increase in urgent need for treatment (3.17% in 2010 to 4.35% in 2014), and in presence of sealants (4.54% in 2010 to 10.01% in 2014). Gaps in best practices identified were related to need for increased risk assessments and enhancing policy development. Based on study findings and the limited access to data on Tribal and Area levels, development of culturally appropriate policies that are unique to individual Tribal needs, and focus on perinatal care, is recommended. Individual Tribal programs also need to be evaluated and surveillance needs to be continued to establish trend data. All program evaluations and research should be conducted in an ethical manner that is community-based and considerate of the needs of the Tribe.

In Dentistry, it became in evidence by the fact that almost twenty-five percent of the population between 65 and 74 year-old presents a completely edentulous arch. As the elderly have a higher probability of being affected by chronic oral problems they tend to demand complex treatments. The aim of this survey was to: (i) evaluate the relationship between the Oral Health Impact Profile (OHIP-EDENT) and Geriatric Oral Health Assessment Index (GOHAI) questionnaires to assess the oral health quality of life in denture wearers, (ii) evaluate the associated factors to impairment in the oral health quality of life in elderly wearing removable dentures using the OHIP- EDENT and GOHAI. Evaluation of the oral health quality of life in complete denture and removable partial denture wearers was performed according to the World Health Organization criteria. The survey consisted of applying GOHAI and OHIP-EDENT questionnaires and a clinical examination. Questions about socioeconomic conditions were applied aiming to determine which factors have an influence in oral health related quality of life. The hypotheses tested were (1) different questionnaires would provide similar results when assessing oral health quality of life in totally or partially edentulous patients wearing prosthesis; (2) age, socioeconomic profile, type and location of the prosthesis and their stability and retention would not be associated to the oral health quality of life. The results showed that the questionnaires are different tools when both the total and adjusted values were compared. The exception was for Kennedy Class I and II removable partial dentures wearers. Mandibular complete dentures caused a higher impact in individuals quality of life, except for the removable partial denture wearers. For the patients aged 60 years old or more the questionnaires showed themselves quite different. Among the subscales, the only statistically difference was found in functional limitation and the domains food catching was the one that showed difference in the removable partial denture wearers group. Using the same questionnaires the variables age, familiar income, type of prosthesis, stability and retention of the prosthesis were evaluated. Factors associated to the 14 impact in OHRQoL were familiar income, stability and retention of the prostheses. Therefore, OHIP-EDENT and GOHAI are different tools, especially when used with individuals older than 60 years old, poor denture quality was associated to the impact in quality of life while income had influence on the results<br>O aumento da longevidade nem sempre está associado diretamente a uma melhora na qualidade de vida. Na Odontologia isso é evidente uma vez que quase um quarto da população entre 65 e 74 anos tem um de seus maxilares totalmente desdentados, conforme o levantamento de Saúde Bucal de 2010. Os pacientes idosos têm uma maior probabilidade de apresentarem problemas bucais crônicos e potencial demanda de tratamentos complexos. Os objetivos deste estudo foram (i) avaliar se existe correlação entre os questionários Oral Health Impact Profile for Edentulous (OHIP-EDENT) e Geriatric Oral Health Assessment Index (GOHAI) para medir qualidade de vida relacionada à saúde bucal em uma população de usuários de próteses removíveis; (ii) avaliar os fatores associados a incapacidades na qualidade de vida relacionada a saúde bucal em uma população de usuários de próteses removíveis utilizando o OHIP-EDENT e o GOHAI. Pacientes usuários de próteses removíveis atendidos nas Clínicas da Faculdade de Odontologia da UFPel foram avaliados seguindo os critérios da Organização Mundial de Saúde onde foi realizado exame intrabucal. Foram aplicados os questionários GOHAI e OHIP-EDENT, além de questões sobre condições demográficas e socioeconômicas, visando determinar se algum fator influenciaria na qualidade de vida destes indivíduos. As hipóteses testadas foram as de que: (1) os questionários apontariam resultados semelhantes na mensuração da qualidade de vida relacionada à saúde bucal em pacientes usuários de próteses removíveis; (2) os fatores idade, condição socioeconômica, tipo de prótese e localização da prótese, bem como sua retenção e estabilidade não estariam associados a incapacidades na qualidade de vida relacionada à saúde bucal. Os resultados mostraram que o OHIP-EDENT e o GOHAI são ferramentas diferentes tanto quando se comparam os resultados brutos quanto os ajustados, exceto para as próteses parciais removíveis Classes I e II de Kennedy. As próteses totais mandibulares se mostraram causadoras de maior impacto sobre a qualidade de vida dos indivíduos, porém isto não ocorreu com as 12 parciais removíveis. Para o grupo de pacientes acima dos 60 anos o OHIP-EDENT e o GOHAI mostraram resultados bastante diferentes, parecendo ser o OHIP-EDENT mais sensível. Dentre as subescalas dos dois questionários, a única que mostrou diferença estatística foi a limitação funcional, sendo o domínio que mostrou diferença a retenção alimentar nos grupos de próteses parciais removíveis. Utilizando os mesmos questionários, quando se avaliaram as variáveis idade, renda familiar, tipo de prótese e retenção e estabilidade das próteses, os fatores associados ao impacto na qualidade de vida foram: renda familiar, estabilidade e retenção. Conclui-se que o OHIP-EDENT e o GOHAI são ferramentas diferentes, principalmente para uso em pacientes acima dos 60 anos e que a qualidade da prótese está associada ao impacto na qualidade de vida, além do fator renda ter influência sobre os resultados

A representative sample of 1,211 children with 8-12 years of age from 20 public and private schools from urban area of Pelotas, Brazil was assessed. DMFT index was obtained by full mouth examination and three partial protocols: P1 all first molars; P2 e P3 first molars from two random diagonal quadrants (1 and 3, 2 and 4). Socioeconomic, behavioral and oral health variables were also collected. Sensitivity, absolute and relative bias and inflation factor were calculated for each partial protocol using dental caries prevalence (DMFT ≥ 1).Risk factors were assessed for all protocols using Poisson Regression for both dental caries prevalence and severity (mean DMFT). Prevalence of dental caries was 32.4% for full mouth examination, 30.2% for P1, 22.2% for P2 and 21.4% for P3. In comparison with full-mouth examination, P1 showed a higher sensitivity [93.1% (CI 95% 91.5; 94.5)]. All protocols showed similar magnitude of association measures for all risk factors investigated, using caries prevalence and mean DMFT. The partial protocols showed potential applicability in epidemiological studies assessing dental caries prevalence and associated factors in schoolchildren aging 8-12 years<br>O estudo teve como objetivo testar a acurácia de três protocolos de exame parcial da cavidade bucal, avaliando somente primeiros molares, para estimativa da prevalência e severidade de cárie, assim como da sua associação com variáveis socioeconômicas, comportamentais e de saúde bucal em escolares com faixa etária entre 8 e 12 anos. Uma amostra representativa de 1211 escolares, proveniente de 20 escolas (públicas e privadas) de Pelotas-RS, foi avaliada. Através de exame bucal da boca inteira, foi obtido o índice CPOD (número de dentes cariados, perdidos e obturados). A partir deste exame, estimou-se o índice CPOD para outros três protocolos parciais de exame avaliando somente os primeiros molares: P1 avaliação dos quatro elementos e; P2 e P3 avaliação de dois molares de quadrantes contralaterais.Variáveis socioeconômicas, comportamentais e de saúde bucal foram também coletadas. Utilizando a prevalência de cárie (CPOD ≥ 1) foram calculadas a sensibilidade, viés absoluto, viés relativo e fator de inflação para cada um dos protocolos parciais em relação ao exame de boca inteira. A associação entre a prevalência e a média do CPOD com as variáveis independentes também foi avaliada para os diferentes protocolos, utilizando Regressão de Poisson. A prevalência de cárie foi de 32,4% para o exame de boca inteira, 30,2% para P1, 22,2% para P2 e 21,4% pra P3. O P1, comparado à avaliação de boca inteira, mostrou alta sensibilidade [93,1% (IC 95% 91,5; 94,5)]. Oralno zdravlje predstavlja stanje zdravlja svih anatomskih i funkcionalnih delova usne duplje, jedan je od osnovnih elemenata blagostanja i važan pokazatelj kvaliteta života stanovni&scaron;tva. Podaci Svetske zdravstvene organizacije (SZO) ukazuju da je 15-20% svetskog stanovni&scaron;tva srednje životne dobi (starosti od 35 do 44 godine) pod rizikom od gubitka zuba, a oko 30% stanovni&scaron;tva starosti od 65 do 74 godine nema svoje zube. Oboljenja usta i zuba su jedan od vodećih javno zdravstvenih problema 21. veka u svetu i nalaze se na 4. mestu po visini tro&scaron;kova. Osnovni cilj ovog istraživanja je bio da se utvrdi stanje oralnog zdravlja odraslog stanovni&scaron;tva Srbije starosne dobi 20 i vi&scaron;e godina i evaluiraju činioci koji na njega utiču. Istraživanje predstavlja deo nacionalne studije &ldquo;Istraživanje zdravlja stanovnika Srbije&rdquo; za 2013. godinu Ministarstva zdravlja Republike Srbije, koja je sprovedena u vidu studije preseka na reprezentativnom uzorku od 13690 ispitanika (53,8% žene i 46,2% mu&scaron;karci). U istraživanju su kori&scaron;ćene tri vrste upitnika koji su dizajnirani u skladu sa upitnikom Evropskog istraživanja zdravlja. Svaki ispitanik stariji za jednu godinu ima 8% veću &scaron;ansu da svoje oralno zdravlje oceni kao lo&scaron;e (OR=1.08; 95% IP=(1.07-1.09); p=0,000). Ispitanici sa blagim depresivnim simptomima i depresivnim poremećajima imaju 2,04 odnosno 3,48 puta veću &scaron;ansu da svoje oralno zdravlje ocene kao lo&scaron;e u odnosu na ispitanike koji nisu depresivni (OR=2.04; 95% IP=(1.28-3.26); p=0,003) (OR=3.48; 95% IP=(1.14-10.59); p=0,028). Žene imaju 30% veću &scaron;ansu da pripadnu povoljnoj kategoriji poseta stomatologu u odnosu na mu&scaron;karce (OR=0.70; 95% IP=(0.63-0.78); p=0,000). Ispitanici sa nižim i srednjim obrazovanjem imaju 5,70 odnosno 2,45 puta veću &scaron;ansu da svoje zube operu povremeno u odnosu na visoko obrazovane (OR=5.70; 95% IP=(4.35-7.48); p=0,000) (OR=2.45; 95% IP=(1.91-3.13); p=0,000). Najučestaliji prediktori oralnog zdravlja su žene, starije stanovni&scaron;tvo, niže obrazovanje, nezaposleno i neaktivno stanovni&scaron;tvo, najsiroma&scaron;niji i pu&scaron;ači.<br>Oral health represents health status of all anatomic and functional parts of oral cavity, it is one of the basic elements of well-being and important quality of life indicator. According to the World Health Organization (WHO) 15-20% of world population aged 35-44 have risk of tooth loss, and about 30% population aged 65-74 does not have own teeth. Oral and dental diseases are one of the leading public health problems in 21st century and on the 4th place of the health care expenditure. The main aim of this research was to assess oral health of adult population in Serbia aged 20 years and older and to evaluate related factors. The research represents a part of &ldquo;National Health Survey in Serbia 2013&rdquo; implemented by The Ministry of Health of the Republic of Serbia, which was conducted as a cross-sectional study on the representative sample of adult population in Serbia. The study included 13690 examinees (53.8% females and 46.2% males). Three types of questionnaires which were designed according to the European Health Interview Survey were used. Every examinee older for one year had 8% greater odds to assess their oral health as bed (OR=1.08; 95% CI=(1.07-1.09); p=0.000). Respondents with mild depressive symptoms and depressive disorders had 2.04 and 3.48 times greater odds to assess their oral health as bed compared to one who were not depressed (OR=2.04; 95% CI=(1.28-3.26); p=0.003) (OR=3.48; 95% CI=(1.14-10.59); p=0.028). Females had 30% more odds to be in the category of affordable dental visit compared to men (OR=0.70; 95% CI=(0.63-0.78); p=0.000). Respondents with lower and middle level of education had 5.70 and 2.45 greater odds to brush their teeth occasionally compared to higher educated (OR=5.70; 95% CI=(4.35-7.48); p=0.000) (OR=2.45; 95% CI=(1.91-3.13); p=0.000). Respondents who belonged to the poorest class had 4.88, poorer 3.73, middle 2.52 and richer class 2.01 greater odds to lose 10 and more teeth compared to the richest (OR=4.88; 95% CI=(2.95-8.08); p=0.000) (OR=3.73; 95% CI=(2.42-5.75); p=0.000) (OR=2.52; 95% CI=(1.69-3.76); p=0.000) (OR=2.01; 95% CI=(1.38-2.91); p=0.000). Former smokers and smokers had 1.43 and 2.80 times greater odds to lose 10 and more teeth compared with non smokers (OR=1.43; 95% CI=(1.00-2.03); p=0.046) (OR=2.80; 95% CI=(2.08-3.76); p=0.000). The most frequent predictors of oral health are: women, elderly people, those with low educational attainment, unemployed, inactive and respondents who assessed their financial situation as poor and smokers.

Introdução. A criação do Sistema Único de Saúde (SUS) e o processo de descentralização que lhe é inerente levaram à municipalização das ações e serviços de saúde. Esse processo vem tendo impacto sobre as políticas públicas de saúde bucal e contribui decisivamente para a reorientação dos programas odontológicos no âmbito do SUS. Objetivo. Analisar o processo de municipalização da saúde bucal no município de Itapira, SP, no período de 1991 a 2006, e verificar seu impacto sobre os níveis de cárie dentária na população escolar de 7 a 12 anos de idade e o acesso ao tratamento odontológico. Método. Trata-se de estudo de caso complementado por levantamento epidemiológico de cárie dentária, realizado em 2006 (n=704), na área urbana. Dados secundários de dois levantamentos, realizados em 1991 (n=1.159) e 1999 (n=749), foram utilizados na análise. Nos estudos de cárie foi utilizado o índice CPOD (em 1991 conforme proposto por Klein & Palmer; em 1999 e 2006 conforme preconizado pela OMS). Entrevistas com informantes-chave foram realizadas com o objetivo de captar sua percepção da municipalização da saúde bucal e reconstruir essa história, uma vez que documentos oficiais não estavam disponíveis. A técnica do Discurso do Sujeito Coletivo foi empregada. Resultados. Os profissionais conseguiram perceber na prática os resultados da municipalização, o impacto positivo sobre os processos gerenciais e a organização do serviço público odontológico, com o desenvolvimento de atividades de prevenção, saindo de uma época em que a lógica do serviço era mutiladora, com alto número de exodontias de permanentes, para tornar-se rara nos dias atuais. Os valores do CPOD aos 12 anos de idade indicaram prevalência de cárie considerada muito alta em 1991 (CPOD=6,85±1,03), moderada em 1999 (CPOD=2,79±0,53) e baixa em 2006 (CPOD= 1,49±0,38). Houve diferença estatisticamente significativa (p<0,05) entre as médias. O declínio nos valores do CPOD foi de 78,25% entre 1991 e 2006. Nesse período ocorreu polarização na prevalência da cárie dentária, com a proporção de livres da doença (CPOD=0) alterando-se significativamente (p<0,05) de 3% em 1991 para 43,8% em 2006. Os primeiros molares foram os dentes que mais contribuíram para os valores do CPOD, e essa participação aumentou no período: 49,02% em 1991, 69,12% em 1999, e 84,21% em 2006. Conclusão. A expressiva redução na prevalência e severidade da cárie dentária poderá contribuir para mudanças na estrutura dos serviços de saúde bucal em Itapira.<br>Introduction: The creation of the Unified Health System (Sistema Único de Saúde- SUS) and the process of decentralization that involves it leading to municipal administration of the health actions and services (municipalização).This process has been having an impact over oral health policies and contribute definitively to the reorientation of dental programs in the SUS. Objective: Analyze the process of transferring the administration to the municipal system in terms of oral public health policy in the city of Itapira,SP between the period of 1991 and 2006 and check it impacts on the dental caries levels in a school population of children between 7 and 12 years old and access to dental treatment. Method: This is a case study complemented with an epidemiological research of dental caries realized in 2006 (n=704) in the urban area. Secondary data from two epidemiological researches, accomplished in 1991(n=1159) and in 1999 (n=749) was in the analysis. The DMF index was used in the dental caries research (in 1991 according to the Klein &Palmer, in 1999 and 2006 according to the WHO proposition). Interviews with key informants were made aiming to collect their perception about the municipalization of the oral health public system and rebuild this history since the official documents are no longer available. The analysis used the collective subject discourse technique. Results: The professionals attained to perceive the results of the municipalization, the positive impact on the management processes and organization of the oral health public system results with development of preventive activities, leaving an age when the logic of the service was mutilating with a high number of exodontia of permanent teeth and getting into an age of rare exodontia in actual days. Values of DMF for 12 years of age indicated a very high level of prevalence of caries in 1991 ( DMF=6,85 + 1,03), moderate level in 1999 ( DMF= 2,79 + 0,53) and low prevalence in 2006 (DMF= 1,49 + 0,38). There was a significant statistical difference (p< 0, 05) between means. The decline of the DMF indexes was 78,25 % between 1991 and 2006. In this period a polarization in the dental caries prevalence occurred with the proportion of caries-free (DMF=0) changing significantly (p<0, 05) from 3 % in 1991 to 43,8 % in 2006. The first molars are the teeth which most contribute to the values of DMF and this participation increases during the period: 49,02 % in 1991, 69,2% in 1999 and 84,21 % in 2006. Conclusion: the significant decrease in the prevalence and severity of dental caries can contribute to the changes in the structure of the oral health public system in the city of Itapira.

A população indígena brasileira compreende mais de 900 mil indivíduos distribuídos por todos os estados da Federação. Diversos estudos têm apontado para a deterioração da saúde bucal dos povos indígenas ao longo das últimas décadas, embora dados epidemiológicos ainda sejam escassos. A transição para um modelo alimentar baseado em uma dieta rica em gorduras, açúcares e alimentos refinados tem sido indicada como um fator contribuinte para o desenvolvimento de doenças como a obesidade e a cárie dentária em diferentes populações. O objetivo deste estudo é analisar o perfil epidemiológico de saúde bucal da população indígena adulta residente na Terra Indígena Guarita, bem como investigar aspectos subjetivos e determinantes sociais relacionados. Buscou-se, ainda, analisar a experiência de cárie dentária nos povos indígenas da América do Sul. Foi realizada uma revisão sistemática da literatura e meta-análise referente à severidade de cárie dentária em povos indígenas que habitam países sul-americanos. Além disso, foram conduzidos grupos focais com as etnias Kaingang e Guarani da Terra Indígena Guarita. Os dados produzidos foram analisados por meio da metodologia Grounded-Theory e interpretados a partir da teoria do sociólogo Pierre Bourdieu. Adultos com idade entre 35 e 44 anos de ambas as etnias foram examinados em relação à experiência de cárie, necessidade de tratamento, e uso e necessidade de prótese dentária. Participantes responderam a Escala Brasileira de Insegurança Alimentar, a versão curta do questionário Oral Health Impact Profile, e a uma entrevista estruturada sobre dados sociodemográficos e uso de serviços odontológicos. Foram observadas altas experiências de cárie entre povos indígenas da América do Sul, contrastantes com a redução da prevalência da doença na população não indígena. Interferências nos sistemas alimentares indígenas afetam aspectos da identidade cultural, da organização social e da saúde das comunidades indígenas Este fenômeno implica em percepções de vulnerabilidades de saúde bucal e tensões entre o modelo biomédico e as práticas tradicionais de cura. Participantes (n = 109) apresentaram média de dentes cariados, perdidos e obturados de 14,45 (± 5,80). Cerca de dois terços do valor do índice foi composto por dentes perdidos. Necessidade de tratamento foi observada em 93% da população. Foram observadas altas frequências de necessidade de prótese e de experiência de perda dentária. Diferenças significativas na prevalência de dentição funcional foram observadas em relação a sexo e tempo da última consulta odontológica. Cerca de 94% dos participantes vivem em domicílios em situação de insegurança alimentar. Maiores pontuações de insegurança alimentar apresentaram associação com o recebimento do benefício Bolsa Família, maior densidade domiciliar e maior percepção dos impactos da saúde bucal na qualidade de vida. A presença de insegurança alimentar severa foi mais prevalente entre indivíduos que vivem em domicílios com mais de 5 moradores e aqueles com maior pontuação no instrumento OHIP-14. Este estudo apresenta dados epidemiológicos importantes para a compreensão das condições de saúde bucal dos povos Kaingang e Guarani, assim como para o planejamento de serviços de saúde culturamente apropriados para as necessidades desta população.<br>The Brazilian indigenous population comprises more than 900 thousand individuals distributed throughout all states of the Federation. Several studies have pointed to the deterioration of oral health of indigenous peoples over the last decades, although epidemiological data are still scarce. The transition to a food system based on a diet rich in fat, sugars and refined foods has been pointed out as a contributing factor for the development of diseases such as obesity and dental caries in different populations, including native peoples. This study aims to analyze the oral health epidemiological profile of the adult indigenous population living at the Guarita Reservation, as well as to investigate associated subjective aspects and social determinants. It was also intended to analyze the dental caries experience in the indigenous peoples of South America. A systematic review of the literature and meta-analysis regarding the severity of dental caries in indigenous peoples living in South American countries was carried out. In addition, focus groups were conducted with the Kaingang and Guarani ethnic groups from the Guarita Reservation. The produced data were analyzed through the Grounded-Theory methodology and interpreted using the Pierre Bourdieu\'s theory. Adults aged 35-44 years of both ethnicities were examined regarding caries experience, treatment need, and use and need of prosthodontics. Participants answered the Brazilian Food Insecurity Scale, the short version of the Oral Health Impact Profile questionnaire, and a structured interview on sociodemographic data and use of dental services. High caries experiences were observed among indigenous peoples of South America, contrasting with the reduction of the prevalence of the disease in the non-indigenous population. Interferences in indigenous food systems seem to affect aspects of cultural identity, social organization, and health of indigenous communities. This phenomenon implicates in perceptions of oral health vulnerabilities and tensions between the biomedical model and traditional healing practices. Participants (n = 109) presented a mean number of decayed, missing and filled teeth of 14.45 (± 5.80). About two-thirds of the value of the index was composed of missing teeth. Need for treatment was observed in 93% of the population. It was observed a high frequency of prosthodontic need and experience of tooth loss. Significant differences in the prevalence of functional dentition were observed in relation to sex and time of the last dental visit. Approximately 94% of participants live in food insecure households. Higher food insecurity scores were associated with the Bolsa Família benefit, higher household density and greater perception of the oral health impacts on quality of life. The presence of severe food insecurity was more prevalent among individuals living in households with more than 5 residents and those with higher scores on the OHIP-14 instrument. This study presents important epidemiological data for understanding the oral health conditions of the Kaingang and Guarani peoples, as well as for the planning of culturally appropriate health services to the needs of this population.

It is present at birth and is apparent by the age of three. The prevalence in the population is 1:500 though there has been an increase in the number of cases over the last decade. Data on odontologic characteristics are scarce and controversial. One example is that some authors report an increase, whilst others a reduction in respect to the rate of caries compared to normal individuals. The difficulty to perform examinations including odontologic evaluations is attributed to the behavioral disorder. The objective of this study was to evaluate the odontologic characteristics of autistic individuals and their ability to cooperate during assessment. Based on evaluation criteria for oral health created by the World Health Organization and adapted by researchers, 32 autistic individuals and 32 normal subjects from the general population, matched by gender, age, ethnic group, and social class, were evaluated. There were no significant differences in respect to the prevalence of caries or teeth submitted to any type of treatment. The autistic individuals presented with a greater number of extra-oral alterations and orthodontic problems associated with alterations in the growth of facial bones. In 90% of the cases, assessment can be achieved by dentists themselves if they adapt to the reactions of patients. Performing the exam was impossible in no cases. The results corroborate previously described reports and confirm that the oral conditions of autistic individuals are similar to the population and suggest that the greatest impediment in providing adequate oral health for these individuals seems to be in adapting the odontologic treatment to autistic behavior.<br>O autismo é caracterizado por comportamento emocional e social alterados, déficit cognitivo e fala ausente ou deficiente. Está presente desde o nascimento e manifesta-se até os três anos de idade. A prevalência na população é de 1:500, com um aumento do número de casos na última década. Os dados da literatura quanto às características odontológicas são raros e controversos. Alguns relataram um aumento, enquanto outros uma diminuição, em relação aos índices de cáries dos indivíduos normais. Ao distúrbio comportamental é atribuída a dificuldade de realização de exames, entre eles o odontológico. O objetivo deste trabalho foi avaliar as características odontológicas de indivíduos autistas e a habilidade de cooperar durante o exame destas características. Com base nos critérios de avaliação de saúde bucal, preconizados pela Organização Mundial de Saúde e com a utilização de um roteiro estruturado pelos pesquisadores, foram avaliados 32 indivíduos autistas e 32 indivíduos normais da população em geral, pareados por sexo, idade, etnia e classe social. Não houve diferença significante entre os grupos quanto à prevalência de cáries e dentes submetidos a qualquer tratamento. Os autistas apresentaram um número maior de alterações extrabucais e problemas ortodônticos associados com alterações de crescimento dos ossos da face. Quanto à cooperação durante o exame, em 90% dos casos ele pôde ser bem realizado com o dentista se ajustando às reações do paciente. Em nenhum indivíduo o exame foi impossível de ser realizado. Os resultados corroboram outros previamente descritos, reforçam que as condições bucais dos autistas são semelhantes às da população e sugerem que o maior entrave para proporcionar uma saúde bucal adequada para os autistas parece estar na adequação do atendimento odontológico ao comportamento autístico.

Background: Teeth affected by apical periodontitis (AP) that could be retained by root canal therapy (RCT) are increasingly being extracted and replaced with implant-supported crowns (ISC). Objectives: 1) To investigate preferences of dentists and patients towards retaining a tooth with AP using RCT versus its extraction followed by, no replacement, replacement with an ISC, or with a partial fixed or removable denture. 2) To investigate patients’ preferred role and dentists’ ethical responsibilities in decision-making. Methods: A mail-out survey (sent to: dental specialists, n=195, response rate=39%; patients, n=434, response rate=43%) and a web-based survey (general dentist sample, n=304, response rate =15%) were used to collect data. Statistical bivariate and multivariate analyses were undertaken with P set at < 0.05 with Bonferroni adjustment as necessary. Results: RCT or ISC were the most selected treatment choices for dentists, while the preference for RCT was reduced for teeth treated previously with RCT. Dentists’ preferences were associated with their specialty as well as clinical experience in endodontics and/or implant dentistry. Patients’ stated general preference for saving teeth was associated with higher self-rated oral health, frequent dental check-ups, and higher socio-demographic factors. Those with higher education, regular dental visits and previous experience of RCT preferred to undergo RCT for future treatment in comparison to those who never experienced RCT. Patients preferred a collaborative role with their dentists in making treatment decisions and valued the retention of natural teeth very highly. They also felt that communication with and trust in their dentist was paramount. Conclusion: Dentists should establish rapport with patients in a milieu that values and respects the patients’ autonomy. By following the highest standards of evidence-based care, and transferring unbiased information on treatment options and associated risks/benefits, it is highly probable that dental care will be delivered in a more ethical manner than would otherwise be possible.
