Abstract:
Thesis (MSc (Radiography))--Cape Peninsula University of Technology, 2018.
Introduction: Forensic dentistry plays a key role in identifying human remains that cannot be identified visually or by other means. Studies have shown that in cases of single or multiple deaths, scientific identification of human remains utilising forensic dentistry is often the most successful source of identification. Dental identification of human remains consists of a very complex procedure that makes it necessary during the investigation process to use and compare unique dental identifiers. A reliable and accurate method of identifying human remains is a positive radiological identification between ante-mortem and post-mortem images of dental radiographic images. Even if ante-mortem radiographic images may not be present during the identification process, post-mortem images may include details of dental restorations such as dental implants which cannot be seen during visual examination. The different types of dental implants vary in morphology and in conjunction with the unique appearance of dental anatomy and the placement of custom restorations such as dental implants, it has been found to accurately assist in the identification of human remains. Objectives: To establish a radiographic dental implant guide for ten commonly used dental implant types in the Western Cape, South Africa; and to identify and describe the morphological characteristics of these dental implant types as observed on pantomographs. Methods: The ten commonly used dental implant types were imaged radiographically to create a reference instrument which served as a tool for identifying and comparing different types of dental implants. The morphologies of the different dental implants, specifically the apex, thread and neck, were observed on ante-mortem pantomographs and compared to the appearance of the dental implants on the reference instrument to make a positive match. The straight tube image of all ten dental implant types in the reference instrument was used as the point of reference to positively identify the morphological characteristics of each dental implant type on the pantomographs. The morphological characteristics of the ten commonly used dental implant types used in the Western Cape were described and based on this a radiographic dental implant guide was developed. Results: A total of 384 dental implants were observed on the pantomographs. Of these, 380 dental implants could be positively identified on the pantomographs while 4 dental implants could not. A total of 350 dental implants (91%) were identified as dental implant types listed in the reference instrument while 30 dental implants were identified as a dental implant type not listed in the reference instrument. A total of 208 dental implants (54.2%) could be positively identified using the morphological characteristics namely the apex, thread and neck on the straight tube images of the dental implant type in the reference instrument. The radiographic dental implant guide was developed based on positive identification of the morphological characteristics of the dental implant types. Conclusion: This research study has illustrated that the morphology of dental implants can be used to differentiate between different dental implant types on pantomographs. Each dental implant type had unique morphological characteristics as well as similarities which enabled distinction between the different dental implant types, which facilitated dental implant identification and the development of a radiographic dental implant guide. The radiographic dental implant guide developed as part of this research study, may be useful in the field of forensic dentistry and forensic radiology.

Abstract:
Abstract Radiographic oral and maxillofacial findings were recorded in a cohort of 293 home living elderly, in Helsinki, Finland, derived from a random sample of 8035 subjects, , born in 1904, 1909, and 1912, who participated in the Helsinki Ageing Study. They were 76, 81, and 86 years old at the commencement of the radiographic study. The relationships of potentially infectious findings with increased all-cause mortality over four years were studied. During the four-year follow-up, 18.5% of the subjects died. Of the 124 edentulous subjects, 17% had condylar findings, 13% radiopaque intraosseous findings, 9% retained roots, 6% maxillary sinus findings, 4% impacted teeth and 3% radiolucent findings. Edentulous women had more arthrotic condylar findings than men. The mean number of teeth in the 169 dentate subjects was 13.9, 15.5 in men and 13.2 in women. Carious teeth were found in 75%, radiolucent findings in 41%, teeth with vertical infrabony pockets in 51%, furcation lesions in 28%, calculus in 40%, and condylar findings in 25%. Periodontal attachment loss was slight in 18%, moderate in 31%, and advanced in 46%. 21% of the teeth had been endodontically treated. Periapical lesions were found in 17% of these teeth, and in 4% of the other teeth. 75% of the rootfillings were inadequate, exhibiting periapical lesions twice as often as the adequate ones. Men had more carious teeth, periapical lesions and furcation lesions than women, indicating better oral hygiene and/or utilisation of dental services in women. Compared with the previous studies carried out in Finland, slightly more teeth and less tooth-associated pathology were found in the present subjects. In contemporary Scandinavian studies, only a slightly better oral health status in the elderly has been reported. During the four-year follow-up, mortality was higher in the subjects with moderate to advanced infrabony pockets, OR 2.2, 1.0-4.7. In the previous studies, similar associations have been found in larger study cohorts including younger subjects. Our results indicate that oral foci may be more dangerous for the elderly than it has been previously thought, as the subjects who died had poorer dental health than those who survived.

Abstract:
Variation in health care, whether it be in terms of the utilisation of resources, observed health outcomes, costs, quality or access to health care is a well recognised and ever present feature of the modern day health care system. Health care variations challenge basic assumptions about the nature of the health care economy and raise questions about efficiency, equity and where best to direct policy instruments in health care markets. Despite the vast literature documenting variation, and the many discussions around ways to reduce variations in health care markets, the field of dental care has received little interest, in comparison to that of general medical care. This thesis will address this gap and will analyse the variation observed in a specific dental care treatment (dental radiographs) within NHS Scotland, with particular emphasis on the contribution of both dentist and patient unobserved heterogeneity. The thesis takes its focus from two strands of the literature; the underlying theoretical aspect draws on the literature concerning the theory of incentives and physician agency, whilst the empirical component makes use of recent advances in micro-econometric methods, documented in the labour economics literature. Although the thesis is predominantly an empirical analysis, the estimation strategy combines ideas from both the theoretical and empirical literature. A matched patient provider dataset from NHS Scotland is used to conduct an analysis of the variation in dental radiographs, in the presence of, and controlling for unobserved dentist and patient heterogeneity. The results indicate that the remuneration structure alone has little or no impact on the treatment decision to provide a radiograph. When a dentist changes from being on a fixed salary contract to being paid on a fee-for-service basis, they are in fact less likely to provide a radiograph. This result changes in the presence of insurance (identified as being when patients are exempt from the patient charge) and indicates that when the self employed dentist can identify the patient as being exempt, they are more likely to provide a radiograph. This result provides some support for the theory that in the presence of insurance, financial incentives do influence the treatment decision. A final result of the study highlights the importance of accounting for unobserved patient and provider heterogeneity, a factor that has had little attention in the healthcare literature. The results suggest that patient variation, as opposed to the variation across dentists, is much more important in explaining total variation. This is a similar result to that found in both the labour and education literatures.

Abstract:
Oral Biology
M.S.
Introduction: Controversy exists over the value of a lateral cephalogram for treatment planning in different malocclusion types. Discord has been found in the literature, and no studies on the matter have focused solely on Class I dental malocclusions. The aim of this study was to elucidate whether treatment decisions of extraction or non-extraction and the perceived need for a lateral cephalogram would change depending on whether a lateral cephalogram was provided or eliminated in the patient record set. Methods: A survey was distributed to a random sample of AAO members. The survey consisted of four different patient record sets, each with a Class I dental malocclusion. Each record set had two versions—one with the lateral cephalogram and one without. The survey had randomization of each version of the record sets so that each record set would be viewed an equal number of times with and without its corresponding cephalogram. Subjects were then asked a series of four questions pertaining to each case: was a cephalogram necessary for treatment planning, treatment of extraction versus non-extraction, retention method, and if records were adequate to treatment plan. Results: In total, 79 orthodontists completed the survey. For all cases combined, record sets viewed with the lateral cephalogram had an increase in the: perceived need for a lateral cephalogram, decision of extraction, and opinion of the records being adequate to treatment plan. All were statistically significant at P ≤ 0.001. For each record set, the difference in perceived need for a lateral cephalogram was statically significant. Conclusions: The use of a lateral cephalogram for diagnosing and treatment planning seems to hold value in Class I dental malocclusions, justifying radiation exposure for this diagnostic record. Additionally, the perceived need for a lateral cephalogram is greater when it is presented with the record sets, demonstrating the diagnostic value this radiograph holds.
Temple University--Theses

Abstract:
Implementation of electronic health records by the Health Information Technology for Economic and Clinical Health has led to the implementation of electronic dental records (EDRs) and digital radiography in dental offices. The purpose of this study was to determine the state of the implementation and usage of EDRs and digital radiographs by the private general and pediatric dental practices in Mississippi as well as reasons why the dental practices are not moving forward with the advanced technology. A survey was emailed to 712 dental practices: 116 responded (16% response rate), and 104 consented to participate (89.66%). Results indicated dental practices in Mississippi using EDRs was 46.07%, EDRs with paper records was 42.70%, and only paper records was 11.24%. Results indicated dental practices using digital radiography was 76.40%, conventional radiography was 13.48%, and both was 10.11%. Common reasons for not advancing were cost, insufficient training, computer/software issues, and “too old.”

Abstract:
Abstract Today, digital imaging is widely used in dentistry. In medical radiography, the importance of displays and room illuminance has been shown in many studies, whereas the effect of these factors in the diagnosis of dental radiography is not clear and remains controversial. There is limited knowledge among dentists as to how observer performance is affected by the type of display, level of ambient light or grayscale calibration. The aim of this thesis was to compare observer performance in the detection of both anatomical structures and pathology in panoramic and bitewing radiographs using consumer grade display with γ 2.2- and DICOM-calibration, a tablet (3rd generation Apple iPad® and a 6 MegaPixel (MP) display under different lighting conditions. Furthermore, the thesis aimed at providing recommendations for type of display and acceptable illuminance levels in the room for interpretation of dental radiographs. Thirty panoramic and bitewing radiographs were randomly evaluated on four displays under bright (510 lx) and dim (16 lx) ambient lighting by two observers. Both anatomical structures and pathology were evaluated because they provided both low- and high-contrast structure. Consensus was considered as reference. Intra- and inter-observer agreement was determined. The proportion of equivalent ratings and weighted kappa were used to assess the reliability. The level of significance was set to P<0.05. DICOM calibration may improve observer performance in the detection of pathology in panoramic radiographs regardless of the room illuminance level. DICOM calibration improves the detection of enamel and dentinal caries in bitewing radiographs, particularly in bright lighting conditions. On the other hand, in dental practice the room illuminance level is often higher, and it is thus recommended that the overall lighting level should be decreased. Furthermore, a DICOM-calibrated consumer grade display can be used instead of a medical display in dental practice without compromising the diagnostic quality and it saves costs. Tablet displays are recommended to use with care in dental radiography

Abstract:
The age estimation method as described by Demirjian is the most frequently used tool to estimate the sub-adult dental age in forensic dentistry. This technique has been shown to over or under estimate the chronological age of sub-adults when applied to specific population groups. The aim of this study was to compare a black South African population sample with the original French-Canadian model to determine if Demirjian’s method accurately reflects the true chronological age of this population group. A sample of panoramic radiographs from 279 boys and 325 girls between the ages of 6 and 16 was obtained from the School of Dentistry University of Pretoria, and from orthodontists in private practice in the Pretoria region. The panoramic radiographs were used to score the seven left mandibular teeth. The calculated maturity score was used to determine the Demirjian dental age. All panoramic radiographs were scored by one examiner. A subset of 20 panoramic radiographs was scored by a second examiner and reliability tested using a Wilcoxon Matched Pairs Test. This research showed that black South African children have a more advanced dental age compared to French-Canadian children. Demirjian overestimated the age for boys by 0.8 years and for girls by 0.5 years. The dental age assessment provided by Demirjian is not suitable for black South African children. As a result, new standards of dental age assessment should be established for this population. Abstract:
Swedish Radiation Safety Authority has suggested guide-lines regarding information to individuals who undergo radiographic examinations. The guide-lines concerns medical care but patients in dental care can also be expected to have a wish to be informed about ionizing radiation. The main objective was to find out the patient’s perception of their knowledge about x-ray radiation and if they have a need to know more about x-rays. A questionnaire study was performed at two public dental clinics in Sweden during a twomonth period in the summer 2016. Patients ≥ 18 years old who gave their consent to participate were included in the study. Of all 429 participants 44 % declared an average knowledge about x-ray radiation and 37 % of all participants declared an inadequate knowledge. A total of 54 % had been informed about x-ray and its effects. School was the most common information source. Approximately 76 % had the perception that dental x-ray radiation is not harmful. 51 % declared the need to know more about x-ray. Approximately 52 % declared no knowledge about natural background radiation and 59 % declared it valuable to know more about dental x-ray in relation to natural background radiation. The participants showed mixed opinion and perception about x-ray radiation. A majority of patients want to know more about x-ray radiation. The study did not identify any particular group(s) needing more information about x-ray radiation than what is included in the justification of the radiographic examination.

Abstract:
Magister Scientiae Dentium - MSc(Dent)
Autologous bone grafting in conjunction with dental implant therapy is a well-accepted procedure in oral and maxillofacial rehabilitation. A variety of intraoral donor sites, such as the mandibular symphysis, the mandibular ramus and the maxillary tuberosity have been used in oral and maxillofacial reconstruction. However these sites are associated with complications. In order to reduce these complications, the anterior palate has been proposed as a potential donor site. However, the scientific literature in this regard is sparse, and larger studies are required to investigate the clinical potential of this proposed site. Aim: To determine the volume and density of available bone in the anterior palate that may be used for bone harvesting using cone-beam computed tomography (CBCT) in a select South African population. Materials and methods: One hundred previously acquired CBCT scans taken at the Diagnostic and Radiology Department of Tygerberg Oral Health Centre were analyzed for the required data. These were all acquired from a single CBCT machine (Newtom VGI®, Verona, Italy). The study sample included 52 females and 48 males ranging from ages 20 years to 80 years. The CBCT scans were divided into 3 different age groups. The first age group was between the ages of 20 and 39 years, the second age group was from 40 to 59 years and the third age group was ≥ 60 years. The volume and density of the anterior palate of the different age groups were analyzed using specific criterion. CBCT specific software (Simplant Pro Crystal®) Dentsply implants, Mannheim, Germany was used to standardize the data collection. All data was stored in a Microsoft Excel spreadsheet (Microsoft Corporation, Washington, USA). Results: The mean volume of the anterior palate in this study was 2.11 ± 0.55 cm3, with a minimum volume of 1.04 cm3 and a maximum volume of 3.82 cm3. There was no significant difference in the volume and density of the anterior palate between different age groups and no significant difference in the volume between males and females (p value = 0.227). Conclusions: The anterior palate affords a considerable amount of bone volume which is similar or even more than other intraoral donor sites. The anterior palate is a potential donor site for bone harvesting and CBCT may be regarded as an ideal tool to analyze the amount of bone available for harvesting.

Abstract:
Magister Chirurgiae Dentium (MChD)
There is a paucity of studies that make use of the alpha angle as a diagnostic tool to assist with the interceptive treatment, prognosis, treatment duration and surgical outcome of possible maxillary canine impaction in orthodontics. While the literature is replete with studies that utilise the sector method, the alpha angle is an alternative approach to assess the possible eruptive outcome of the unerupted canine. It has been reported that if the alpha angle is greater than 25°, there is the possibility of external root resorption. However, if the alpha angle is more than 31°, the prospect of canine eruption decreases even if the deciduous canine is extracted as an interceptive measure. The dental pantomograph has historically been used to predict canine eruption or possible impaction. The use of this method, however, is wrought with limitations. These limitations include magnification, distortion and blurred images. The use of cone-beam computed tomography has been advocated as a means to overcome these limitations.

Abstract:
Abstract Oral health among older people is shown to be worse compared to younger people. The aim of this study was to evaluate oral health and associating factors among ageing Finns. A cross-sectional, descriptive epidemiologic survey was performed in Kirkkonummi in southern Finland and in the Lakeus District in northern Finland. Altogether 1,191 subjects aged 60 to 78 years, 625 from Kirkkonummi and 566 from Lakeus, were examined and interviewed. Edentulousness and poor occlusal stability were recorded clinically. Gonial angle, ramus height, condylar height and the amount of residual ridge resorption in the mandible and the presence of fixed prostheses were evaluated from panoramic radiographs. Based on the radiographs the scoring of infection foci was performed with an infection focus index. The signs of infection recorded were deep caries, periapical lesions, furcal lesions, vertical bone pockets and horizontal bone loss. The prevalence of edentulousness was 37%, being significantly higher among women than among men and significantly higher in the northern compared to the southern region. Edentulousness was more prevalent among subjects with primary school education than among those with higher education. The mean of the gonial angle was smaller among dentate subjects than among edentulous ones. Dentate subjects had significantly greater ramus height and condylar height than edentulous subjects. Women had significantly more often than men severe residual ridge resorption in the mandible. Poor satisfaction with dentures associated significantly with poor occlusal stability in both genders. More than three quarters of the subjects had severe risk of infection, more in the southern than in the northern region. In the dentate sample women showed higher prevalence of fixed prostheses than men. In the Kirkkonummi region both men and women had more fixed prostheses than subjects in the Lakeus region. Edentulousness was very frequent among ageing Finns. Mandibular bone morphology changed as a consequence of tooth loss. The prevalence of fixed prostheses was low and the prevalence of signs of infection of dental origin was high. The geographical differences in oral health should be considered by health authorities in order to promote good oral health in rural areas, especially focusing on older people with low socio-economical status and poor health

The dental replanted has been considered the best treatment of the tooth avulsion, which is characterized by complete alveolar dislodgment of the tooth. Despite of the adequate clinical situations, the delayed replantation fails due to the root resorption that is detected during the radiographic evaluation. The use of the digital image represent a fast and easy diagnostic resource, furthest more precise. The quantitative radiographic analysis has been suggested to reduce the interpretation variability through the radiographic density. The purpose of this work was to analyze the process of repair of the dental replanted using the digital densitometry in the Digora system. The 30 male rats were divided in 3 groups of 10 animals, and their right superior incisor were extracted and treated agreement with their group: group I - immediate replanted, group II - delayed replanted without treatment and the group III - delayed replanted with treatment of the root surface using fluoride of sodium. The animals were sacrificed after 60 days operative act, your jawbones were separate and the radiographic images obtained so that on each optical plate were the two jawbones (treated and control), the aluminum step wedge and the identification. The radiographic density of radicular standardized area in Digora system was measured. The densitometric values were submitted to the variance analysis and test of Tuckey and analysis non parametric of the number of areas root resoption. We can end that digital densitometry analysis it an important complementary for replanted evaluation once it allowed the detection of root reabsorption in all of the groups, whose decreasing order of involvement was the group of the delayed replanted without treatment, following for the delayed replanted with treatment and last the immediate replanted.

AbstractThe mineralization and development of the third molars can be used as an indicator of age in the upper teens and is used in purpose of assessing age in asylum seekers without identification or birth certificates. The evaluation is determined with dental radiographs and different methods of interpretation. The aim of the study was to investigate the accuracy and precision of different age estimation models of radiographs of third molars, focusing on population groups and ethnicity. This is of great importance since the results of an age assessment constitute the basis in a juridical outcome, deciding whether an individual is an adult or a child (over or under 18 years of age). A systematic search of PubMed, Embase and Cochrane was made according to the PRISMA-statement, and 23 articles which met the inclusion criteria were assessed using a modified QUADAS 2 by two observers, ten of these were graded as having low risk of bias.Population groups were poorly described, leading to difficulties in evaluating results and precision, thereby also comparisons between different studies. The problem both lay at a general level where definitions are ambivalent, but also in the separate articles where definitions are missing. It was hard to compare the included papers since their aims and methods varied. The main finding was that most of the articles concluded that the use of the third molar as an age indicator in the upper teens is an insufficient system. The use and impact of the results should thereby be questioned.

Abstract: This study set out an assessment of the effects of two voxel sizes and scan modes of cone-beam computed tomography (CBCT) in periimplant fenestration and dehiscence detection. The images were acquired using the i-CAT Next Generation CBCT unit, with a FOV of 8 x 8 cm and the following protocols: A- 0.2 mm voxel size half-scan (180°); B- 0.2 mm voxel size full-scan (360°); and C- 0.12 mm voxel size full-scan (360°). Five filters of the XoranCat software were tested: Angio Sharpen High 5x5, Shadow, Sharpen 3×3, Sharpen Mild and Smooth. Three oral radiologists performed a dynamic evaluation of the presence/absence of the defects and ROC curves were created. The values of the area under the curve (Az) were compared with ANOVA and Bonferroni post-hoc test. The significance level was adopted at 5%. The McNemar test was applied to perform two comparisons in order to identify disagreements. All the enhanced and the original images were compared with the gold-standard and the original images were compared to the enhanced ones. The protocol A had significantly lower Az values than those obtained with the protocols B and C (p < 0.01) for dehiscence. The original images and the Shadow filter were statistically different from the gold-standard (p < 0.01). All the enhanced images also had statistically significant differences from the original images (p < 0.01). In conclusion, the voxel sizes did not influence in periimplant fenestration and dehiscence detection but the full-scan (360°) performed better than the half-scan for dehiscence. The use of enhancement CBCT filters for dehiscence detection is advised, with the exception of the Shadow filter
Doutorado
Radiologia Odontologica

The reactions of four different implant surfaces were evaluated in regard to the development and progression of peri-implantitis: titanium commercially pure (Ticp), titanium plasma spray (TPS) hidroxiapatite coated (HA) and acid attached surfaces (Ost). Six dogs had their premolars extracted. After 90 days, the implants were ramdonly placed and the dogs receive soft diet, chemical and mechanical plaque control; after the osseointegration period (90 days), the healing abutments were placed. After the healing period (45 days), the plaque control was suspended, and cotton ligatures were installed when the first clinical (probing depth, bleeding on probing, plaque index, bleeding index, and mobility) and radiographic (bone loss) exams were taken. These parameters were re-exanimate after 20, 40 and 60 days, when the ligatures were changed. The results did not show significant differences between the surfaces in any parameter observed except for the mobility (evaluated with PeriotestÒ), and bone loss. These two parameters had the TPS surface with the lower rates. It was conclude that the surfaces show similar behavior during the induction of peri-implantitis, except for the mobility and bone loss parameters witch the TPS implants had better behavior.

Abstract:
The aim of this thesis was to evaluate the interference of the presence of metallic materials on radiographic and tomographic image by assessing misfits tooth and restoration in metal-restored teeth. This investigation resulted in three articles: a systematic review of the literature to identify the state of the art on the use of radiographic methods; an in vitro study comparing the performance of conventional film and digital phosphor plate systems Vistascan Dürr Dental, Digora Classic Soredex and Instrumentarium Express – exported as original images and with the use of post processing filters; and an in vitro study evaluating the post-processing of CBCT images – using diverse reconstruction thicknesses. The systematic review retrived 14 studies, classified as low- / moderate quality based on QUADAS criteria. For radiographic evaluation (article II), higher sensitivity (0.67 0.83), specificity (0.81 0.92) and accuracy (0.73 0.86) values were obtained with conventional and digital original images. For tomographic evaluation (article III), mean aucROC ranged from 0.60 to 0.72, and the analysis of the 95% CI showed the lower bound of the curve bellow or very close to the reference line. Based on the results, it is concluded that original images (conventional or digital) should be preferred for the assessment of teeth with metal restorations. Considering CBCT images, even thought increased reconstruction thickness decreased perceived artifact, it did not improve diagnosis accuracy.

The severe root resorption diagnosed late can lead to tooth loss, and being the technique of digital subtraction radiography (SDR) as an important resource for early detection mineral changes, this study evaluated the efficiency in the early diagnosis simulated external root resorption apical and lingual by the technique of digital subtraction radiography (SRD) and digital radiography (DR). We used 14 human incisors jaw macerated with resorption different sizes, simulated in the apical and lingual, and radiographed with a range of projection angles. The radiographs were subtracted by the program Regeemy ®, and performance evaluation diagnostic methods, pairs intraoral radiographs and subtracted images were presented to two radiologists. According to the results, there were no statistically significant differences between methods in the detection lingual resorption, regardless of lesion size, as there was no difference between the methods in the detection of apical resorption, except the second examiner with respect to variation vertical and horizontal angle of 100 using the highest level of wear, which lodged the digital radiography as a method superior in the evaluation for subtraction radiography. And the increase in the level of wear and less variation vertical and horizontal angles allowed a better assessment resorption in the regions apical and lingual to detect and lingual apical resorption. With these results we conclude that the digital subtraction radiography is a test for the detection of small mineral loss in radiographs showing little or no variation in vertical and horizontal angles

Abstract: The aim of this in vitro study was to compare two remineralization protocols of artificial carious lesions in enamel, evaluated by Calcium (Ca) and Phosphorus (Pi) analysis, cross-section hardness test, polarized light microscopy and digital subtraction images (DSR); to assess the feasibility of using two different systems of digital radiography, photostimulable storage phosphor (PSP) plate and complementary metal oxide semiconductor (CMOS) sensor on the demineralization diagnosis, and the accuracy of digital conventional radiographs (DCR) and three methods of DSI (linear, advanced and logarithmic) on mineral changes diagnosis; and, to compare noise and reproducibility in linear and logarithmic DSI produced from two digital radiography systems. Artificial caries-like lesions on 100 approximal surfaces of sound teeth were produced. Twenty teeth were kept as control and eighty teeth were subjected to two different remineralization protocols for 4 and 8 weeks, with continuous immersion in artificial saliva, and additional fluoride treatment. Digital radiographs were taken before and after the remineralization protocols. Five examiners assessed demineralization and mineral changes on DCR, placed side by side, and three methods of DSI. Ca / Pi analysis and the placement of the teeth on the remineralization solution was the gold standard. The mean shades of gray and the standard deviation (SD) of the histogram were also assessed. The concentrations of Ca and Pi in the artificial saliva after the treatments were significantly lower than the original solution (p<0.05); and DSR showed differences between the images. However, the Hardness test and polarized light microscopy did not detect any changes. CMOS system was significantly more accurate than PSP system on demineralization and mineral changes diagnosis, and also linear DSR on mineral changes diagnosis. It was also statistically significant different the values of mean shades of gray and SD between both systems. It was concluded that (a) the additional Fluoride treatment provided higher values of mineral gained; (b) Ca / Pi analysis in the artificial saliva were the most sensitive method of mineral change evaluation; (c) linear DSI is a valuable method to disclose an intensity increase, as a sign of mineral gained; and (d) DSR images created from PSP plates had less noise than images produced from CMOS sensor
Doutorado
Radiologia Odontologica
Doutor em Radiologia Odontológica

Orientador: Luiz Cesar de Moraes
Banca: Pedro Luiz de Carvalho
Banca: Cristiane Yumi Koga Ito
Abstract:
Abstract: The reactions of four different implant surfaces were evaluated in regard to the development and progression of peri-implantitis: titanium commercially pure (Ticp), titanium plasma spray (TPS) hidroxiapatite coated (HA) and acid attached surfaces (Ost).
Orientador: Ricardo Samih Georges Abi Rached
Coorientador: Elcio Marcantonio Junior
Banca: Rosemary Adriana Chiérice Marcantonio
Banca: Luiz Antonio Pugliesi Alves de Lima
Mestre

Abstract:
Since the introduction of implants in Dentistry, controversies have arisen about the possibility of connecting implants to teeth in fixed prostheses. Osseointegrated implants do no present any periodontal ligature and are more firmly anchored to the bone. The difference presented by this type of anchorage has led to some concern about the limited flexibility of the osseointegrated implant to share the functional loads with the connected teeth. This study intended to follow-up prospectively the periodontal parameters bleeding index, probing depth between the pillar tooth of an implant supported prosthesis with one control tooth of the same patient and also the implants that support theses prostheses. That is why subtraction radiography of the region in which the prosthesis was placed was used, with radiographs taken at prosthesis placement, after six months and one year of use to verify if the periimplant osseous support had or had not undergone changes. Twelve patients with normal health conditions were selected who presented a posterior area edentulous jaw where a prosthetic rehabilitation by a fixed prosthesis joining tooth to implant was indicated. Patients were treated with an implant of one stage of 10mm standard Straumann (ITI) with a 2.8 mm polished neck, in which the remaining intraosseous portion of the implant has a treated surface of 4.1 mm diameter. The implant was placed in the space where according to the protocol, the more distal retainer of the future fixed partial prosthesis would be located. Digital radiographs were assessed with a subtraction radiography software Matrox Inspector version eight, to corroborate the change of contrast in the radiographs when superimposed, with values observed in the implant compared with the success criteria , widely mentioned in surveyed studies. All implants placed in patients became osseointegrated. There was no record of bone change around them and in the dental pillars of the prostheses at the one year follow-up. Prostheses continue to be in excellent conditions after the one year follow-up based upon clinical and radiographic analyses. Furthermore, in accordance with the Wilcoxon test of matched pairs and signed rank, it was concluded that there was no statistical difference between each group of each patient during the time period analyzed (baseline and one year) as well as when the pillars were compared to their respective controls.

Its excellent features have been described since the 1960 s, and some of these are the radiopacity and fluoride release. The aim of this work is to study the radiopacity measured by radiographic exposures of the following restorative glass ionomers: Vidrion R (SS White Manufacturing Ltd, Gloucester, England); Chemfil (DENTSPLY DeTrey, Konstanz, Germany); Vitro Molar (DFL); Maxxion R (FGM Dental products Ltd); Riva self cure powder and liquid and Riva self cure in capsules (Riva SC, SDI Ltd, Bayswater, Australia) and to make comparisons among them. Then, different radiographic densities from an aluminum scale were attained and they could be compared and used in the clinical work. The specimens were made using standard glass plates (25,4 x 76,2 x 1,0 mm) manufactured by Global Trade Technology and they were perforated by a 5mm bur (number 5, Black & Decker, Brazil) especially used for glass. For the radiopacity evaluation, the samples were radiographed using three different types of films, which are the following: Kodak Dental Intraoral ESpeed Film (EASTMAN KODAK Co., Rochester, N.Y.); Contrast DFL DV-58 (DFL Ltd); and Agfa Dentus M2 Confort (Heraeus Kulzer Inc.). A 99,5% pure aluminum step wedge and a Gnatus XR 6010 X-ray equipment were utilized in this work. The data were collected by 5 examiners, who were properly calibrated, and analyzed by Friedman and Kolmogorov-Smirnov tests. The results showed no statistical differences among the averages of each examiner comparing the cements and the films. And the results also showed that all the cements present radiographic density average values below 1.

It was evaluated, through digital and densitometric radiographic exams, the influence of the type of rotating instrument for cut, in the process of bony repair after osteotomy in swine jaw in controlled times. They were used for the study eight Large White swines, with initial corporal weight varying from 25 to 30 Kg. Two traverse cuts of the middle to the base of right jaw were accomplished, one through rotating instrument of high rotation and the other for the action of the low rotation, both with liquid irrigation. After established periods (7, 28, 60 and 90 days) the animals were sacrificed and your mandibles retired for direct and indirect digital radiografic studies, for the DIGORA and ODR systems and of optical densitometry for radiographic images for the CROMOX system. In the initial periods (7 and 28 days) the densitometry were larger in osteotomy areas accomplished with high rotation and in the final periods (60 and 90 days) the densitometry were larger in the osteotomy done with low rotation, indicating a better final bony repair when of the use of low rotation. The qualitative analysis of the repair process was made by the ODR system, that made possible obtaining three-dimensional and colored digital images, allowing comparison of the bony thickness with the one of a wedge of aluminum, evidencing that to the sixty days the bony repair was apparently complete. The histograms of DIGORA pointed values of the averages of ash tones in the osteotomy areas, evidencing a larger amount of ash tones in the osteotomy of high rotation in the first times, and in the osteotomy with low-rotation in the final times.

In this research digital subtraction radiography (DSR) was used that uses the subtraction of images longitudinally, in which the change in the alveolar bone is visualized against a uniform gray background. The objectives of this study were: (1) to evaluate the repair process of periapical lesions after endodontic treatment by using DSR; (2) to quantify by means of point/pixel (picture element), area (histogram) and linear measures (profile line), the gain or loss of mineral density in the area of the lesion, using the average of the pixel values; (3) to compare the diagnostic information, suggestive of the repair process, obtained through a subjective evaluation of DSR with a conventional radiographic evaluation and digitalized image and (4) to evaluate the contribution of DSR to an early identification of the repair of periapical lesions after endodontic treatment. The sample consisted of twelve patients with a total of seventeen periapical lesions. The x-rays were digitalized and submitted to DSR using DSR software. The pixel values of the subtracted images were determined by using Image Tool software. Both the conventional x-rays as well as the digitalized and subtracted images were qualitatively evaluated. The results showed a gain in mineral density with a meandp of 133.495.17, 130.275.77 and 129.414.46 for the points/pixel, histogram and profile line tools, respectively. In the evaluation of numerical gain Pearson s Coefficient of Correlation (r) presented these values: mean of points/histogram = 0.746; mean of points/profile line = 0.724 and histogram/profile line = 0.860. When the numerical values were transformed into percentile gain meandp of 0.674.01, 1.214.33 and 1.163.36 were obtained for the points/pixels, histogram and profile line tools, respectively. In the evaluation of the percentile gain Spearman s Coefficient of Correlation (rs) showed the following values: mean of points/histogram = 0.697; mean of the points/profile line = 0.646 and histogram/profile line = 0.844. In the qualitative analysis, the frequency of success in the ordering of the correct sequence of the repair process using conventional radiography, digitalized image and DSR was 37.3%, 31.4% and 31.4%, respectively. One concluded, therefore, that: (1) the process of repair of periapical lesions after endodontic treatment can be evaluated quantitatively by means of longitudinal analysis using DSR; (2) any one of the three tools can be used to quantify the repair, considering that correlation exists between the time of repair and the increase of the value of pixel; (3) the comparative evaluation between the subjective methods using conventional radiography, digitalized image and SDR, it showed that all had been capable to evidence the process of repair of periapical lesions from the first radiography (15 days), not having difference between them and (4) the quantitative evaluation by SDR obtained to after evidence the beginning of the repair with 15 days the beginning of the endodontic treatment, even so this repair was really effective from 105 days after the beginning of the endodontic treatment.

Abstract:
92
ABSTRACT Generally, radiation exposure from dental radiography is very trivial, especially followed by the newly developments in films and dental equipments. According to the US Food and Drug Administration (FDA) guidance, dental X-ray is a relatively low dose radiation, which is considered to be safe for clinical practice. However, all exposures due to ionizing radiation for medical and dental purposes could cause potential risks to both patients and staff. Besides these, leakage radiation increases unnecessary radiation doses as well. Radiation dose from dental radiography is highly variable depending on several factors: field of view (FOV), operating parameters (e.g., tube voltage, exposure time, and filter thickness, etc.), and types of the study (e.g., intraoral films, panoramic tomography, etc.). The aim of this study was to evaluate radiation doses from the following aspects: (1) verifying the operating parameters of dental x-ray units, (2) measurements of the leakage radiations, (3) assessments of the typical radiation dose of the staff and patients. Results were compared with respective standards recommended by the International Atomic Energy Agency (IAEA) and Atomic Energy Council (AEC) of Taiwan. In this study, the leakage radiation and output from dental x-ray radiographies were measured by utilizing a Barracuda multi-functional detector. For measuring the radiation dose from staff and patients, a Rando phantom and Thermoluminescent Dosimeter (TLD-100H) were used. The operating parameters of dental x-ray radiographies were evaluated and their results were provided. All of the results were compared with the current radiation regulation of AEC as shown in the parentheses. (1) Leakage radiation from x-ray units was 21.31 ± 15.24 mR/h (<100 mR/h). (2) The average filter thickness for x-ray machine was about 1.95 ± 0.3 mmAl (>1.5 mmAl). For panoramic x-ray machine, it was near 2.66 ± 0.11 mmAl (>2.5 mmAl). (3) Error of the actual tube voltage for the 60 kVp setting was from 0.2% to 6.5%, with an average of 2.5% (<7%). (4) The error of the exposure time for a 0.5~1.5 sec setting was within 0.7% to 8.5%, with an average of 7.3% (< 10%) error as well. (5) The average length of the x ray cone was 14.75 ± 0.5 cm, and the average target-to-skin distance was 18.63 ± 2.63 cm (>18 cm). (6) The diameter of the cone was 6 cm (<7.6 cm). The measured radiation dose from staff and patients during dental radiographies were shown as follows: (1) The annual equivalent dose of dentists and assistant staff were 1.44 ± 1.08 mSv /year and 2.04 ± 1.8 mSv/year, respectively (<50 mSv), (2). Patients dose of four different dental radiographic techniques were also assessed. Among these techniques, the highest effective dose was 55μSv and the lowest effective dose was only about 5μSv。 In conclusion, this study provides a systematic understanding of the current status of dental radiographies in dental clinics of Taiwan. This study also ensures that the doses to the medical staff and patients were considered reasonably low for clinical purposes. Results from this study might provide a valuable reference for future legislative considerations to associated regulation agencies of Taiwan.

99
In recent years, identity recognition technology is widely used in daily life, such as: iris, fingerprints, voice prints, facial recognition ... and so on. Varieties of biological characteristics are quickly applied in control, transaction security, law enforcement and other related fields. However, in the field of the legal profession, victims and biological characteristics of the behavior are often damaged cause of major accidents. Teeth are the hardest body tissue that can resist high temperatures and strong impact. It is very suitable as a basis for identification of the victims. Because of traditional method of dental identification required to identify on a large database manually, it is a time consuming and ineffective approach. An automated dental identification system is proposed to find the best fit people by comparing dental radiographs of victims (post-mortem) and the dental radiographs on database are recorded before his death (ante-mortem). In addition, all kinds of dental work, including filling, crowns, implants ... and so on have different characteristic, it can be used as a basis for supporting the identity recognition. Therefore, this paper proposed two methods, one is a hierarchical feature extraction method of classifying dental radiographs effectively. The other is a morphological feature extraction of classifying dental work effectively. An automated dental identification system can be achieved by these effective classifications.

Abstract:
99
We propose an effective dental work (DW) extraction method for uneven illumination dental radiographs and two DW matching methods: one for DW fully matching and one for DW partial matching. Through constraining edge-pixel’s intensity and edge length, we reduce most DW-contour irrelevant information first and then threshold the remaining pixels to retain coarse contours of DWs. Finally we obtain the complete shape of each DW by region growing. For DW fully matching, we adapt the method of Hofer et al. that uses number, position, size, and distance between two neighboring DWs as the matching features and add DW contour based on Fourier descriptors as an additional feature to resolve possible incorrect matching problem of Hofer et al.’s method when the database happens to contain imposter images with similar position, size and distance features but different shape of DWs. Because of later dental treatment, the number of DWs and the distance between two neighboring DWs could both be changed. Thus, we propose a partial matching method by discarding the matching cost of different number of DWs in the matching image pairs from the fully matching method and modifying the distance feature to be the distance between each DW and the most left DW. The method starts with finding the best shape-matching DW pairs between the query and each database image then computes the distance between two neighboring DWs by subtracting the values of the two neighboring DWs’ distance features. The experimental results show that our DW extraction method can effectively extract DW’s both in shapes and numbers. The experimental results also show that, for a database containing 29 pairs of matching and 51 non-matching dental radiographs, the accuracy of DW fully matching achieves 100% when using the top-1 retrieval. For a database containing nine pairs of matching and 31 non-matching dental radiographs, the accuracy of DW partial matching also achieves 100% when using the top-1 retrieval.
