Resumo: o presente trabalho teve a finalidade de estudar os estágios de desenvolvimento do primeiro molar permanente inferior e sua relação com a idade cronológica em crianças leucodermas da região de Piracicaba. O estudo contou com um total de 102 meninos e 104 meninas com idades entre 24 e 119 meses (2 anos e 9 anos 11 meses), que foram submetidos a uma tomada radiográfica obliqua em norma lateral. As radiografias foram utilizadas para detectar a faixa etária em que o primeiro molar permanente inferior apresentava a coroa totalmente formada, a raiz com 2/3 formados e, finalmente, o ápice radicular formado. Sobre esses achados radiográficos foram organizadas tabelas. Elas mostraram que o estágio de coroa totalmente formada se estendeu de 36 a 53 meses, para o sexo masculino, e de 30 a 53 meses, para o sexo feminino. O estágio de 2/3 de formação radicular do primeiro molar permanente inferior abrangeu a faixa etária de 54 a 119 meses, para o sexo masculino, e de 60 a 119 meses, para o sexo feminino. Por fim, o estágio de ápice radicular formado para ambos os sexos, compreendeu a faixa etária a partir de 102 a 119 meses. Os resultados permitiram observar que não houve diferença entre os estágios de desenvolvimento dentário quando comparados os sexos para uma determinada faixa etária, ou seja, a velocidade de desenvolvimento do primeiro molar permanente inferior, nos estágios, é a mesma para os dois sexos
Abstract: The present paper has had the purpose of studying the development stages of the right permanent mandibular first molar and its relationship with the cronological age of white children in the region of piracicaba. About 102 boys and 104 girls, between the ages of 24 months and 119 months (2 years and 9 years 11 months) were submited to a radiographic examination, the oblique jaw radiography. These radiographs were used to estimate the ages when the permanent mandibular first molar had the crown completed at alI, 2/3 of root completed and, finally, the apical end of root completed. Tables were made on these radiographic results which showed that the stage of crown completed extended from 36 to 53 months for the males and from 30 to 53 months for the females; the stage of 2/3 of root completed of the right permanent mandibular first molar showed the ages from 54 'to 119 months for the males and 60 to 119 months for the females and, at last, the stage of the apical end of root completed extended from 102 to 119 months for both sex. The results permited to observe that there wasn't any difference between the dental development stages when we compared the sex with a determined age: the rate of mineralization of the right permanent mandibular first molar in the stages was the same for both sex

Full textThis study aims to conduct a retrospective research regarding the interrelation between the clinical characteristics of natal and neonatal teeth diagnosed by the Oral and Maxillofacial Surgery Unit of the Hospital de Clínicas de Porto Alegre (HCPA), seeking an observational period of 20 years. Thus, it is expected to assist in the creation of a protocol to facilitate the early diagnosis of the condition, stimulating an improvement in the care of these patients, as well as to evaluate the clinical and systemic maternal and child conditions associated with natal and neonatal teeth, since the limitation of long term evidence concerning the subject in South America. Thus, the investigation was carried out through the analysis of specific clinical parameters, such as: demographic characteristics, systemic condition, hereditaryfactors and family history, clinical characteristics, of the tooth, complications resulting from the condition, treatment of choice and time interval between diagnosis and therapeutic management. For proper analysis, the medical records data was compiled through a collection form, which was developed specifically for this research. Through the analysis of consultancies of the Neonatology Unit and Pediatrics Unit referenced for Oral and Maxillofacial Surgery Unit of the HCPA, 53 medical records was verified, of which 13 met the inclusion criteria of this study. Thus, it was sought to obtain important information sought to facilitate the early diagnosis of the condition and, consequently, its treatment. It was suggested greater possibility of occurrence in females; positive relation between family history and hereditary factors; greater occurrence of eruption in pairs and in the region of lower central incisors (71 and 81); high association of the condition with dental mobility; and the presence of associated complications, especially related to painful symptomatology. Regarding the treatment of choice, it was chosen mainly by tooth extraction due to the presence of mobility and symptomatology, as well as risk of tooth aspiration. Finally, the time interval between diagnosis and treatment of HCPA cases of natal and neonatal teeth occurred between one and 16 days of the child's life, and was considered to be highly satisfactory. Therefore, strict control and agility are observed for the resolution of the cases diagnosed by the Maxillofacial Surgery Unit of the HCPA. Further prospective studies are also suggested to confirm these etiologic theories and to improve preventive actions for this condition, since the early diagnosis and its treatment are directly associated to the reduction of complications and risks for the newborn , as well as to the improvement of the maternal quality of life.

Resumo: O objetivo neste estudo foi pesquisar qual é a imagem que oferece melhor informação qualitativa e quantitativa, comparando a radiografia panorâmica com a tomografia computadorizada de feixe cônico, na avaliação da relação entre o seio maxilar e os ápices dentais. Para tanto, foi utilizada uma amostra de conveniência de 109 imagens de radiografia panorâmica e de TCFC, dos mesmos pacientes. Três observadores calibrados avaliaram um total de 1.875 ápices, que foram classificados de acordo com sua relação topográfica e mensurados quando o ápice se encontrava aquém ou além do assoalho do seio maxilar, em ambas as modalidades de imagens. Concordância entre as duas técnicas de imagens foram examinadas estatisticamente. Após um intervalo de 30 dias, a avaliação foi repetida em 25% da amostra para avaliar estatisticamente a concordância intraobservador; os valores encontrados para o Kappa ponderado na avaliação qualitativa e o coeficiente de correlação intraclasse para a avaliação quantitativa representaram correlação entre considerável-quase perfeito e excelente, respectivamente. Para observar a concordância entre as duas imagens, o Kappa ponderado na avaliação qualitativa mostrou uma leve concordância para as raízes palatinas dos primeiros molares do lado direito e dos segundo molares. Quando comparados os valores da avaliação quantitativa pelo coeficiente de correlação intraclasse, houve pobre concordância para as raízes palatinas do primeiro molar esquerdo e dos segundos molares, como também para as raízes mesiovestibulares dos segundos molares direitos e disto-vestibulares dos segundos molares esquerdos. Concluiu-se que há uma baixa concordância entre as duas modalidades de imagens quando as raízes se encontram em contato com o assoalho do seio maxilar ou quando foi observada uma projeção das raízes além do assoalho do mesmo na radiografia panorâmica; e uma alta concordância da radiografia panorâmica com a tomografia computadorizada de feixe cônico quando as raízes se encontram aquém do assoalho do seio maxilar
Abstract: The aim of this study was to assess what is the best image that offers better qualitative and quantitative information comparing the panoramic radiography with cone beam computed tomography to evaluate the relationship between the maxillary sinus and dental apices. For this, we used a convenience sample of 109 images of panoramic radiography and CBCT, from the same patients. Three calibrated observers evaluated a total of 1,875 apices, which were classified according to their topography relationship and measured when the apex was beyond/below the maxillary sinus floor in both imaging modalities. Agreement between the two imaging techniques was examined statistically. After an interval of 30 days, the assessment was repeated in 25% of the sample to statistically evaluate the intra-observer, the values found in the kappa test for qualitative assessment and the intraclass correlation coefficient for quantitative assessment represented a considerable - almost perfect correlation and excellent, respectively. To observe the correlation between the two images, the Kappa test for quality assessment showed a slight correlation to the palatal roots of the right first molars and second molars. When comparing the values of the quantitative assessment by intraclass correlation coefficient, there was poor agreement for the palatal roots of the left first molar and second molar, and also for the mesio-buccal roots of the right second molars and disto-buccal of the left second molars. It was concluded that there is a poor correlation between the two imaging modalities when the roots are in contact or when a projection of the roots was observed beyond the maxillary sinus floor in panoramic radiography, and a high concordance of panoramic radiography with cone beam computed tomography when the roots are below the maxillary sinus floor

Introdução: Os dentes decíduos são considerados pilares no desenvolvimento da oclusão, no entanto, a sua perda ainda é muito freqüente, e têm como principal fator etiológico a cárie dentária. O objetivo deste trabalho foi identificar os aspectos preditivos da perda precoce de dentes decíduos no agravamento das más oclusões em crianças de 6 a 8 anos de idade. Material e Métodos: Foi realizado um estudo transversal observacional quantitativo, cujo universo foi composto pelos 7.636 alunos das 32 escolas públicas municipais de Florianópolis. Foram examinadas 547 crianças de ambos os sexos, com idade entre 6 e 8 anos, estudantes do ensino fundamental. A anamnese e o exame clínico odontológico foram efetuados no pátio, sob luz natural com as crianças sentadas em cadeiras da própria escola e utilizando espátulas descartáveis. As anotações foram realizadas nas fichas clínicas de cada participante, no entanto para a documentação dos achados mais representativos do estudo foram realizadas fotografias extra e intra-orais. Foram incluídos no estudo 528 escolares, sendo 51,5% meninas e 48,5% meninos. Discussão: Em ambos os sexos, os dentes mais acometidos pela perda foram os primeiros molares decíduos aos seis anos de idade. Das variáveis investigadas, 63,4% das crianças apresentaram má-oclusão e 21,8% tiveram perda dental precoce. Observou-se forte associação entre má oclusão com perda dental precoce e o aumento da idade, presença de cárie e má higiene bucal. A prevalência de má oclusão foi maior nas meninas de 8 anos de idade que apresentaram perda dental precoce, interposição lingual e má higiene bucal. Conclusão: O presente estudo permite concluir, que, é necessário que políticas públicas incentivem boas práticas de higiene bucal, visando combater a doença cárie, que ainda é diretamente responsável pela perda dental precoce em crianças, prevenindo a manifestação e o agravamento de más oclusões possíveis de prevenir e corrigindo precocemente problemas iniciais.
Deciduous teeth are considered cornerstones in the development of occlusion, however, loss is still very frequent, and their main etiological factor in dental caries. The aim of this study was identify predictive aspects of early loss of deciduous teeth in malocclusions aggravation in 6-8 years old children. A crosssectional observational quantitative study was conduced, the universe was composed of 7636 students from 32 public schools in Florianopolis. 547 children of both sexes, aged between 6 and 8 years, elementary school students was examined. Clinical examination were performed in the courtyard, under natural light with children sitted in school's chairs and using disposable spatulas. The notes were taken at the medical records of each participant, however for the documentation of representative findings of the study were taken extra and intra-oral photographs. The study included 528 students, 51.5% girls and 48.5% boys. In both sexes, the teeth most affected by the loss were the first primary molars at six years old. Of the variables investigated, 63.4% of children had malocclusion and 21.8% had early tooth loss. A strong association between malocclusion and early tooth loss with increasing age, presence of caries and poor oral hygiene. The prevalence of malocclusion was higher in girls with 8 years old, who had an early loss of teeth, tongue interposition and poor oral hygiene. A strong association between malocclusion and early tooth loss with increasing age, presence of caries and poor oral hygiene. This study indicates the need of public politics, encourage good hygiene practices in orderto combat caries, which is still directly responsible for tooth loss in children, preventing the outbreak and avoid malocclusions possible to prevent and correcting early problems.

Resumo: A tentativa de se encontrar meios para a redução do desconforto pós-operatório nas cirurgias de terceiros molares, como a dor, o edema e o trismo tem sido objetivo de muitos estudos na literatura odontológica, onde a avaliação de novas drogas anestésicas, analgésicas ou antiinflamatórias, tem sido realizada com este propósito. O objetivo deste trabalho foi comparar, clinicamente, a eficácia de duas drogas de ação antiinflamatória (dexametasona e meloxicam), administradas em dose única pré-operatória, no controle do edema e trismo, após exodontia de terceiros molares inferiores inclusos. Foram selecionados 16 pacientes adultos, com idade variando entre 15 e 27 anos, sem nenhum comprometimento local ou sistêmico, com indicação de exodontia de terceiros molares inferiores inclusos, bilateralmente e em posições similares. As drogas foram administradas, na primeira ou segunda cirurgia, aleatoriamente de forma duplo-cega. Fez-se avaliação do edema através da variação de pontos de referências faciais, e do trismo através da variação da distância interincisal. As mensurações foram registradas nos períodos pré-operatório e pós-operatório de 24 e 48 horas, sendo que os valores obtidos em cada avaliação receberam 9 análise estatística. De acordo com os resultados, concluiu-se que não houve diferença significativa na limitação de abertura bucal, quando se compararam os tratamentos com o meloxicam ou dexametasona. Já em relação ao edema a dexametasona mostrou uma maior eficácia
Abstract: The attempt to find ways to reduce postsurgical discornfort pam, swelling and trismus on third molar removal, have been related in many studies. News drugs such: analgesics, anesthetics and anti-inflammatory have been evaluated for this purpose. The aim of this study was to compare clinically the efficiency of two antiinflammatory drugs ''f (Dexamethasone and Meloxicam), which were administered in a presurgical unique dose for control swelling and trismus, afier impacted bilateral mandibular third molar extraction. Sixteen adult patients were selected, with ages varying between 15 and 27 years old, without local and sistemic problems, with indicated extraction and presenting similar teeth positions. They were given drugs aleatorily at the fust or second surgery. The swelling were evaluated through facial references point variation, and trismus was analysed through measuring the incisal distance. AlI these measurements were registered within 24 and 48 hour - postsurgical periods. The results were submitted to statistics analysis. Taking theses results into account, we can conclude that the administration of meloxicam and dexamethasone did not present any difference as to trismus. But dexamethasoneusage showed betler results in the controI of swelling

Full textMinimally Invasive Dentistry is a systematic respect for the original tissue. The objective of this dissertation was to evaluate the clinical features and treatment of minimal intervention in oclusal caries radiographically limited to superficial dentin of primary molars. Two researches were conducted: a cross-sectional study from clinical records of the pediatric patient FO-UFRGS and another in a randomized clinical trial. Initially, a sample of 121 primary molars was previously selected by bitewing radiographs in 99 children between 3 and 9 years old, was examined for the presence or absence of cavitation and active or inactive caries lesion. There was a predominance of the cavitated and active lesion (64.8% - 70/121). Of these teeth, 49 received intervention restorations after total removal (n=24) or partial (n=25) caries. After one week and six month, clinical evaluation using criteria of the USPHS and radiographic analysis, there was no statistical difference between control and test groups (p=0.236), and the a presence of cavitation in the outer half of the dentin was a frequent feature, with a prevalence of active lesion. The minimally invasive treatment in these lesions, using the technique of partial removal of caries, showed clinical and radiographic performance satisfactory and similar to total removal of caries after six months of evaluation.

Este estudo avaliou e comparou a influência de diferentes espessuras de agentes clareadores na eficácia do clareamento dental, estabilidade de cor e translucidez. Setenta espécimes em forma de blocos padronizados de esmalte-dentina bovinos foram preparados, polidos sequencialmente e divididos aleatoriamente em três grupos: G1 (n=10) - Saliva artificial (Controle); G2 (n=30) - Peróxido de hidrogênio a 38% e G3 (n=30) - Peróxido de Carbamida a 10%. Os grupos G2 e G3 foram divididos em três subgrupos (n=10) de acordo com a espessura de agente clareador empregado: A - 0,5mm; B - 1,0mm; e C - 2,0mm. Os agentes clareadores foram utilizados durante 4 semanas. A cor foi mensurada antes e após (7 dias, 15 dias e 30 dias) o clareamento dental pelas coordenadas L*, a* e b* do sistema CIE-Lab, utilizando-se um espectrofotômetro. A diferença de cor (DE) e a translucidez (TP) foram calculadas. Os valores de DE foram analisados estatisticamente pelos testes ANOVA, post hoc de Tukey e Games-Howell (a=0,05). Os valores de TP foram analisados pelo teste T Student's (a=0,05). DE não apresentou diferença significativa no perÃodo de 1 mês entre os grupos clareados. Também não foram observadas diferenças estatÃsticas significativas entre a translucidez inicial e a translucidez final para todos os grupos. A espessura do gel e o agente clareador não influenciam na eficácia do clareamento dental e não alteram a translucidez do esmalte-dentina.
