Abstract:
From 1996 to 2006, the number of Department of Defense (DoD) contract transactions increased, leading to over expenditures and the need for agencies to determine benefit estimation to improve risk management of a project. The purpose of this qualitative case study was to apply a total quality management theory to explore if a standardized versus decentralized benefits framework within the cost benefit analysis process could improve the Department of the Army acquisition selection process. The two central research questions addressed (a) the differences between successful and failed acquisition systems cost benefit analysis, and (b) whether a standardized or decentralized cost benefits framework would best serve contract selection process. Data were collected via interviews with 20 DoD acquisition specialists and analysis of cost benefit analysis cases; NVIVO software was used to examine word frequency and comparative phrases. The data analysis resulted in themes that encompassed how standardization improves product quality, enhances innovation, and accelerates the acquisition procurement process. Other themes included the need to build metrics into the cost benefit assessment to measure risk management controls and cost-reduction initiatives. The DoD might benefit from the results of this study by reviewing and instituting a standardized benefit assessment within its cost benefit analysis framework to protect business stakeholders' from fraud, waste, or abuse. The implications of this doctoral study will promote social change in the form of government spending fiscal stewardship and could serve as a benchmark to improve the budget formulation and management of the American taxpayer's investment in national security.

Abstract:
Identity theft has become one of the fastest growing crimes in America and stems from the widespread and growing reliance of organizations across the nation to use Social Security Numbers (SSN) as a primary personal identifier. Originally intended for the very limited purpose of tracking social security benefits, the value of the SSN as a unique identifier was quickly recognized, and its use rapidly grew. This â functionality creepâ has led to the SSN becoming an almost de facto national ID number. Employers, universities, credit agencies and financial institutions began using the SSN as a unique personal identifier. The military started to use the SSN as a personal identifier in 1969 in place of the Military Serial Number. Today, the SSN is used pervasively throughout the military, from personnel rosters to medical records, from administrative records to operational orders. This thesis analyzes the elimination of the SSN as the primary personal identifier within the Department of Defense and the Veteransâ Administration, replacing it with a Military Identification Number (MIN). The elimination of the SSN at all but one critical location (pay related matters at the Defense Finance and Accounting System), would render all lost or stolen data useless to an identity thief. A Cost/Benefit Analysis of the transition from SSN to MIN using six methods of analysis
payback period method, discounted payback period, benefit cost ratio, net present value, internal rate of return, and a probabilistic NPV were examined. Each methodâ s benefits and drawbacks are discussed and the findings are summarized. The CBA shows that the transition to a MIN is a cost effective solution with a Net Present Value that falls between $701 million and $554 million over a 10 year period.

This thesis utilizes ideas taken from different Systems Engineering modeling tools to model the hiring process for the U.S. Department of Veterans Affairs (VA), Veterans Health Administration (VHA). This model is a guide for understanding the current state of the process and shows that inadequate Position Descriptions (PD) are not the primary reason why the VA cannot meet the 80 day window set forth by U.S. Office of Personnel Management (OPM). Additionally, the model can assist in identifying potential areas for reducing the overall process timeline and be used as a training tool to illustrate how the hiring process progresses. Existing models only show major steps in the process which can mask sources of delay, communication issues, and confusion. The developed model delves deeper into those major steps, showing individual sub-steps, accountability, timelines, and data flows. Data for the model was obtained by direct observations, interviews, analysis from data collected by the VHA, and documents released by the VA and OPM. When fully developed, the model allowed for the conduction of case studies on three different positions within VHA; these case studies illustrate that the inability to meet the hiring process timeline is only partially due to issues with the PD and that other factors (namely internal reviews and classification delays) have a significantly greater effect in the resulting timeline. The model itself and recommendations provided, such as establishing priorities, targeting specific areas of time delays, improving communication, and generating and providing access to knowledge can help the VHA to achieve a streamlined and compressed timeline.

Abstract:
Diabetes is the 7th leading cause of death in the United States. Uncontrolled diabetes and lack of self-care knowledge leads to increased emergency department (ED) visits, utilizing limited health care resources. The practice-focused question asked whether a patient-focused diabetic self-care toolkit could decrease the number of diabetes-related ED visits. The purpose of this project was to reduce the number of diabetes-related ED visits and was supported by the social cognitive theory. The sources of evidence included the facility's ED Information System (EDIS) data and a patient survey developed by the project lead. There were 149 participants (135 males and 14 females). Before program implementation there were 3240 total ED visits with 124 diabetes-related disposition diagnoses. Post-project there were 3362 total ED visits, with 126 diabetes-related disposition diagnoses. Data collected revealed no meaningful difference between the pre-project and post-project ED diabetes related visits. However, of the 124 pre-project diabetes-related visits, 59 had a triage complaint or disposition diagnosis of medication refill, and of the 126 post-project diabetes-related visits, 33 were medication refills. Although implementation of the diabetes resource toolkit did not help to decrease diabetes related emergency department visits, it did identify that 47.58% of pre-project visits and 26.19% of post-project visits were related to medication refills, showing a 21.39% post-project decrease in medication refill visits. This identifies one potential positive impact of the toolkit implementation. The implications for positive social change from this project is the recognition that medication refills are an impacting contributor to diabetes-related ED visits and from that, nursing practice can gain new knowledge in the fight against diabetes.

Abstract:
Evidence-based practices utilized by social workers working with veterans experiencing physical (e.g., spinal cord injuries and loss of limbs) and/or mental health (e.g., posttraumatic stress disorder, depression, substance use) issues are widely used with this vulnerable population. The number of social workers integrating animal-assisted interventions (AAI) or animal-assisted therapy (AAT) as a complementary intervention to these practices is extremely limited. A significant amount of research has been published on AAI and AAT, yet there is a gap in research regarding AAI and AAT as effective complementary approaches when working with veterans. The qualitative method utilized in this study was interviews in which MSWs and LCSWs were asked to share personal experiences while employed at veteran-supported agencies and/or non-profit organizations. The qualitative method of interviews and content analysis were used to help identify the themes and sub-themes of this study. The themes and sub-themes supported the phrases and statements drawn from the seven interviews conducted for this study. Findings supported in this study addressed the value of the human-animal bond, how integrating AAI and AAT with evidence-based practices can assist the veteran in improving quality of life (e.g., a decrease in social isolation, increase in social activity, decrease in substance use), and how important it is to recognize the need to provide the education in this field to social workers. Findings from this study can help to provide the foundation for future research, can underscore the value of offering and integrating AAI and AAT education into more MSW programs, and can contribute to establishing policy where those who can benefit by AAI and AAT are given access to it.

Department of Veterans Affairs (VA) audiologists were surveyed regarding their perceptions and evaluations of hearing aid (HA) brands on seven factors previously published in peer-reviewed research as important to the HA brand preference decision of audiologists. One of the seven factors formed a distinct dimension of brand differentiation based on Contract Pricing (dimension 1). Brand perceptions for the other six factors were highly correlated with one another; this dimension of correlated perceptions was labeled Propensity to Dispense Based on a Gestalt Percept (dimension 2). That is, a direct relation exists between the collective perception of HA brand and its likelihood of being dispensed. These two dimensions accounted for 93.1% of the variance in the perceived differences among the HA brands surveyed. Joint-space mapping was used to model the effect of altering perceptions on VA HA brand dispensing. Results indicate that few VA audiologists (7.7%) dispense HA brands in majority association with contract pricing. Instead, the vast majority (77%) form brand preferences in majority association with their individualized perceptions.

Abstract:
Patients with limited health literacy often fail to understand verbal and written discharge instructions, and they frequently return to the emergency department (ED) for care. Patients returning to the ED and 30-day readmission rate are core quality performance measures. The purpose of this project was to decrease repeat visits and readmissions to the ED by implementing components of health literacy programs within the ED on patient education and written discharge instructions. Change implementation consisted of (a) use of teach back method (b) modifying medical terminology to language that patient could understand (c) limiting use of words with more than three syllables and (d) discouraging nursing practice of copying and pasting other completed clinician notes in discharge notes. Following the tenets of the logic model and Watson's caring theory, ED nurses (n=45) at a veteran's healthcare facility participated in the modification of the ED discharge note design. A retrospective quantitative design was used to obtain data from 5,474 records related to each patient's language preference, educational level, and the readability index of the discharge note pre-and post-modification of the note. The comparative analysis of the descriptive statistics before and after modification of the discharge note indicated a decrease of 1.75% in the readability index of the discharge note, a 24% decrease in return visits within 30 days and a 40% decrease in readmission rate within 30 days. Healthcare costs and health disparities associated with health literacy decrease if patients comprehend discharge instructions. Understanding verbal and written discharge instructions correlates with healthy communities. Health literacy policies and technological innovation can promote health literacy and research on health literacy.

Veteran-owned small businesses that possess ratings as service-disabled companies (i.e., service-disabled, veteran-owned small businesses [SDVOSB] are 1 of 5 socioeconomic small business contracting goals that U.S. government agencies seek to comply with U.S. Small Business Administration (SBA) set-aside goals. Little is known about collaborative strategies for improving training regarding single point of entry into Department of Defense (DOD) subcontracting. The research questions in this study focus on how veteran-owned small businesses pursue acquisition opportunities from training opportunities. Sabatier's advocacy coalition theory was used as the theoretical foundation for this study. This qualitative study was employed using 6 semistructured interviews of small business owners who possessed SDVOSB status and a content analysis of training policies from corporate supplier diversity offices, procurement assistance centers, small business development centers, and DOD Office of Small Business programs from a midwestern city. The emergent themes were (a) availability of training from procurement support assistance agencies is of limited value to mature small businesses; (b) additional support and training are needed for subcontractors desiring to enter the DOD subcontracting market; (c) significant obstacles are present in gaining access to federal subcontracting opportunities, including complex acquisition strategies and selection of contract type; and (d) understanding and enforcement of SDVOSB regulations were perceived as insufficient. SDVOSB entities can use these findings to comprehend what questions to ask about subcontracting training.

Abstract:
The Department of Health (DH) is responsible for one of the largest estates in Europe. In this capacity, the DH produce and disseminate estates-related Standards & Guidance (S&G) to provide support to the briefing and design processes for new, and refurbishment projects in old healthcare buildings. The estate is made up of a variety of buildings, many ageing and in need of extensive refurbishment or replacement. It is therefore important to the stakeholders in the procurement and provision of healthcare environments that the DH S&G provide them with the information and data they need at the relevant time in the process to enable them to design and construct healthcare facilities that are safe and fit-for-purpose. Policy changes over the past 20 30 years have had a profound effect upon the estate. The estate was seen to be in need of modernisation, but Government lacked the extensive funding necessary to achieve anything like the extent of redevelopment required. The introduction of private sector funding to achieve this resulted in a major shift in the ownership of the estate, and latterly the regulation of the estate, both private sector and National Health Service (NHS). The NHS Constitution, introduced in 2009, was the first Government document explicitly to recognise the estate and the importance of it being fit-for-purpose. This research seeks to establish the importance of the DH S&G, and their benefits and dis-benefits to stakeholders using them, including organisations and individuals from the private and public sectors. The groups have differing roles and priorities and the research seeks to establish how these affect their requirements for S&G, how effectively the S&G meet those requirements and how they contribute to the overall provision of healthcare environments. Moreover, hospital accommodation has been proven to have an effect on the patients and staff, therefore, the provision of useful and helpful S&G could be seen to have an indirect influence on patient outcomes, and also on providing a pleasant and efficient environment for staff. The research has identified three major strands: Policy; the DH S&G themselves; and what is important to users about them and any benefits or dis-benefits incurred. Policy is viewed as the driver for the need for DH S&G. The changing political environment, amongst many other factors, affects how the S&G have been operationalized. This study of the application of DH S&G aims to establish how users view the benefits and dis-benefits and their effects on the healthcare environment. Research in the construction industry sector spans the scientific and social worlds, and the methodology is deductive research orientated, exploiting a range of data. Qualitative and quantitative data have been collected through open interviews with known experts and an on-line survey of the stakeholders using the S&G from private and public sector organisations involved with the provision of healthcare accommodation. Reference to the DH S&G and related unpublished DH documents traces their development and examines their content. The results have been mapped to the stakeholder categories (Designers, Service Users, Estates and Facilities Managers, Contractors and the DH/NHS), thus enabling comparisons to be made between each group, and between the public and private sectors. Analysis of the data identified the characteristics users found to be of importance and of benefit or dis-benefit. On balance, it was clear that the DH S&G are beneficial, but not universally. Of prime importance to its users is the DH endorsement of the S&G and its independence from commercial influences. However, the classification of the DH S&G, defined as best practice is often regarded and applied as mandatory. The content of the S&G varies in its scope, content and characteristics, being perceived as incomplete, inconsistent and out-of-date. Taking all these factors, therefore there is a danger that the DH S&G may contribute to healthcare buildings being unfit-for-purpose or out-of-date.

Some veterans transitioning from military duty to the classroom are not obtaining college degrees. The purpose of this study was to investigate student veterans’ perceptions of the transition services and support systems at their college that might impact the challenges they face while pursuing a college degree. The theoretical framework for this study was Schlossberg’s Theory of Transition. The guiding research question asked how military veterans perceived the transition services and support systems at their university in the context of their decision to obtain their degrees. A purposeful sampling approach was used for selecting student veteran participants who had returned from active duty and were enrolled at the university for 1 year. Moustakas’s transcendental approach was the model used for interviewing 12 veterans. Inductive analysis was used to analyze data, including coding the interview transcripts and identifying themes to capture the collective experience of the veterans. Participants indicated that existing emotional or social support programs, organizations, and personnel to assist them on campus were inadequate. Implications for social change include increasing faculty and staff understanding of veterans’ needs, which may lead to improved transition services, support systems, and communications within the university. Results may be used to improve retention and degree completion rates of student veterans.

The study also looks at diversity management studies conducted in Australia, Malaysia and Canada. The study involves an in-depth analysis of the literature on diversity management, which resulted in an untangling of the complexity of the subject .

Abstract:
Urinary incontinence, one of the most prevalent conditions in elderly living in nursing homes (NH) was shown to significantly impact patient's quality of life (QOL) and health outcomes. Bladder antimuscarinics (BAM), the main drug class to treat urinary incontinence, have limited effects in managing the condition; however, given their anticholinergic properties and the characteristics of those living in NH, BAM could potentially lead to serious health consequences in this population. We conducted a retrospective cohort study with a new-users design by linking existing Veterans Affairs (VA) data (inpatient, outpatient, pharmacy administrative files, and Minimum Data Set- MDS) between fiscal years 2003 and 2009. Potential risks (i.e. fractures and negative impact on cognitive performance) and benefits (i.e. improvement in urinary incontinence, social engagement and overall QOL) associated with initiation of a BAM were assessed in elderly (65+) admitted for long-term care in the VA Community Living Centers. Descriptive statistics were used to compare BAM new-users and non-users at baseline; in addition, logistic regression was used to identify important predictors of BAM initiation. Treatment selection bias was addressed by using the propensity score matching method. After balancing the groups on baseline characteristics, the risk of fractures (hip fracture, any fracture) in relationship with BAM initiation was evaluated using Cox proportional hazard analysis. BAM impact on the cognitive status measured by the MDS-Cognitive Performance Scale (CPS) was evaluated through generalized estimated equations (GEE) method. Similarly, possible benefits measured through MDS were assessed via GEE. The final cohort included 1195 BAM new-users (with the majority being prescribed Oxybutynin immediate-release) and 22,987 non-users. Predictors of BAM initiation included demographic characteristics, bladder and bowel continence status, comorbidities, medication use, cognitive performance and functional status. Our study showed that BAM improved urinary continence (OR=1.27, 95%CI: 1.07-1.50) in those treated; social engagement as measured by MDS-Index of Social Engagement also improved in users, although at a level that is not clinically significant (difference in mean MDS-ISE=0.2074, 95%CI: 0.0550-0.3598). However, BAM initiation increased the risk of fractures (hip: HR=3.69, 95% CI: 1.46 - 9.34, p=0.0059; any fracture: HR=2.64. 95% CI: 1.37 - 5.10, p=0.0039). Our results showed no difference between new-users and non-users with regard to mean CPS and overall QOL. The purpose of the study was to clarify the proper role of medication use in the management of urinary incontinence in elderly in the VA CLC. The results raise questions about the continued use of Oxybutynin IR, the main BAM prescribed in this population. Given the increased risk for fractures in the context of potential improvement in urinary continence with no clinically significant improvement in social engagement, a wiser step might be to investigate the safety profile for newer BAM for situations when an addition to non-pharmacologic management for urinary incontinence is desired for elderly in long-term care.

The Gauteng Department of Education (GDE) is powered by the Public Service Regulations Government Notice No R441 of 25 May 2001 (as amended) chapter 1, Regulation VIII, Section A, that outlines the principles of managing performance stating that Departments shall manage performance in a consultative, supportive and non-discriminatory manner in order to enhance organizational efficiency and effectiveness, accountability for the use of resources and the achievement of results. It is witnessed through personal observation and personalized interviews with some of the employees that supervisors and managers of the delegated units within the GDE does not take initiative to guide, inform and aware their subordinates regarding the availability of services benefits within the organization. It is furthermore related to the challenge regarding lack of training and informative sessions within the organization. In order to improve these challenges, the hypothesis for the study was formulated as 'the effective service benefits may improve employee performance at Gauteng Department of Education'. The semi-structured interviews were conducted with respondents from senior management, employees and officials. This included qualitative questionnaires to obtain their opinion on the impact of the service benefits for enhanced employee performance at GDE. The convenient sampling was utilized to approach target population in order to obtain responses. The findings explored that the GDE needs to facilitate information sessions guiding the employees at lower management levels regarding the job description and job specification requirements of the top and middle management levels. The information regarding the tasks and responsibilities associated with the position/rank are linked with remuneration packages and service benefits. The GDE furthermore needs to inform employees that the service benefits are equally considered at different levels of management horizontally. The service benefits differ at different levels of management on vertical basis based on job requirements in the organization. The study concludes that the GDE needs to understand that there are implications of salary levels to employees' performance at the workplace. Employees who are not financially secured and satisfied with service benefits are concerned regarding the cost of living. This concern may have negative impact on employees' motivation and morale to reflect better productivity in the organization. It is recommended to further explore the possibility of establishing a macroanalytical correlation between 'service benefits of employees' and 'organizational efficiency' for improved delivery of services.

The United States Department of Defense spends billions of dollars annually on outsourcing functions to private contracted companies without knowing if their actions are effective. Guided by Feigenbaum, Henig, and Hamnett's theory of privatization and President Eisenhower's warnings of the impending military-industrial complex, the intent of this grounded theory study was to develop relevant theory regarding how the Department of Defense might accomplish missions through outsourcing during current and future fiscal constraints. This study sought to understand the perceived effectiveness of outsourcing Department of Defense functions through the perspectives of 2 employment groups directly affected by such outsourcing: federal employees and privately contracted employees. In this study, 24 federal employees and 20 privately contracted employees completed qualitative surveys about their perceptions of effectiveness in regards to outsourcing Department of Defense functions. Data were inductively analyzed through open, axial, and selective coding via constant comparison. Findings from this study generated a grounded theory, one positing that 2 distinct elements are important in outsourcing during fiscal constraint: well defined legal requirements and private sector technical expertise. Evidence from this study suggests that when these elements are in place, outsourced Department of Defense functions can progress, regardless of fiscal restrictions. The implications for social change include assisting political leaders with better decision making in support of effective national security policies, while providing good stewardship of tax payer funds.

This study aims to measure the service quality with student loans that are provided by the Finance Department of a University of Technology (UoT). In the current academic environment, it is critical for a Finance Department to provide high quality of services to enable students fulfilling their academic performance. However, if the service quality of Finance Department is not up to standard, then students (customers) more likely to struggle in receiving all supporting financial aids for their studies. This will further impact on their whole academic life negatively. Based on the relevant literature, SERVQUAL model is one of the classical tools that were commonly used to measure the quality of services. Thus, this study applied SERVQUAL model to measure the service quality that provided by the Finance Department at the UoT. A combination of both qualitative and quantitative research approach was conducted in this study. Data were collected through interviews with staff members (n1=15) and a structured questionnaires for students (n2=43). All the key variables from the questionnaire and interview questions were derived from relevant literature. The reliability of dataset was tested through Cronbach’s Alpha value. Data were analysed through SPSS, MoonStats and Excel worksheet. The personal information of all the participants was protected. The findings revealed that there is a gap between students expectation and staff members perception towards the service quality that provided by the Finance Department. Students were generally satisfied with the service quality although staff members perceived that they have made efforts in providing good quality services to them. However, there are areas needs improvements in the service process such as management commitment, employee involvement, regular training programmes for skill development, and etc. This study further recommends that the financing office needs to use SERVQUAL model as an effective tool to track service delivery patterns and its nature of administration along tangibles, unwavering quality, responsiveness, confirmation and compassion measurements within the department. Moreover, SERVQUAL model can be used an useful approach to other service sector for quality improvements within the university.

The purpose of this mixed methods study was to examine the benefits of art as a therapeutic modality for identifying stress in Gulf War to active duty veterans suffering from Post-Traumatic Stress Disorders (PTSD). This is a mixed methodology study. The case and control sampling approach was a systematic random sample with a paired treatment control design. The questions where chosen to the specificity of the Gulf War veteran. The qualitative data was scrutinized using a mixed methods design utilizing the tool known as the 17 Point Questionnaire. The quantitative data was obtained using the tool known as the PSS 14 Stress Scale. Both qualitative data and quantitative data were tested for validity and reliability using various tools including Spearman’s Rho, or (Spearman's Rank Correlation Coefficient), Cluster Analysis, Pearson’s Coefficient, Dendorgrams, paired and independent T-tests, and summary statistics to determine data. Convergent validity was evaluated by comparing positive and negative PSS-14 factors and by examining relationships with the number of actual to perceived benefits of stress reduction using Likert Scale results over thirty days between two Gulf War veteran groups identified with PTSD. Findings have shown a significant reductions in stress in the treatment group. Findings have also shown significant correlations between the studies perceived benefits for specific modalities and the use of ceramic or clay work as a preferred method of choice were also identified within the research. Due to the ability of art to reduce stress in Gulf War victims suffering from PTSD, longitudinal studies have been recommended to provide art treatment therapies with conclusive evidence based research status. With this change in status it is anticipated that art therapy will have greater accessibility within the Veterans Administration (VA) system for those seeking alternative methods of treatment.

The U.S. Department of Defense spent $11 billion in enlistment and retention bonuses from 2006 to 2010, which had only a marginally positive effect on the enlistment rate for the Army. The case study addressed this business problem of recruiting by exploring marketing strategies successful recruiting professionals used to motivate individuals to join the military. The purpose of this study was to determine effective recruiting strategies. Therefore, it incorporated the conceptual framework of emergent strategy theory, which postulated the best strategies are neither completely planned nor completely random, but are rather an adaptation to changing dynamics and circumstances. The population consisted of 2 former recruiters, 1 from the Army and another from the Marine Corps, as well as 38 college students located in Wilkes-Barre, Pennsylvania. Data for the study morphed from face-to-face interviews and 3 focus groups comprised of 10 to 15 students each, for the purpose of addressing the research question. Data analysis occurred through a process of coding and theming. The 9 themes identified included tell the story, advertising strategies, and fit for duty. A lesson learned from these themes was that the key for successful recruiting strategies lies in aligning with the wants and needs of individuals in the target demographic. If senior leaders in the Department of Defense followed the recommendations provided, each of the branches of the military service could potentially achieve higher recruiting rates at a lower cost. The study could result in social change whereby eligible recruits could view the Army and Marine Corps as professions of arms in which individuals can live out their ideals of patriotism but also have a good quality of life due to the benefits of military service.

defined in their centralised repository. PMs need to use Financial and Planning Software (FPS)2 software to perform certain project management activities, as required by their 
job function. However, it was found that MPP3 software, another tool, was used for more detailed project schedules, as well as activities that were not strictly enforced by

through the results of the quantitative analysis. Findings: The main finding, based on the compilation of three models of user acceptance, proved that FPS was not being utilised 
as intended. There was also no evidence of an improvement in business operations. Therefore, benefits management was negatively impacted. Organisational processes were 
dentified as the most important organisational factor, influencing the usage of FPS software. Own technological capability was considered to be the least important factor, 
as respondents believed that they had sufficient IT skills in order to learn how to use FPS software. Change management was rated negatively
and as a result, it impacted the 
usage of FPS, as users were not involved in the decision to implement, and had limited interaction in the implementation process. In addition, there was no evidence found that 
benefits management was conducted in the IT department
and therefore, the impact of using alternative software could not be quantitatively assessed. Recommendations: In concluding this research, it is recommended that the &ldquo
best practice&rdquo
, derived from the pertinent literate should be followed more diligently if YZ organisation is to benefit from the

Health outcomes fulfill a number of roles in the health sector. Economists, clinicians, researchers and managers use health outcomes in a range of different contexts for distinct purposes. New management approaches that use contracts as the basis for health service accountability have attempted to take health outcomes from their clinical role into a management setting. In particular, the purchasers and managers of some health services expect that service providers should demonstrate that they improve the health outcomes of their patients to justify their on-going funding. However, a number of organisations have experienced barriers to the application of the outcomes approach to health service management and there has been no systematic evaluation of the approach. Nor has there been an investigation into why purchasing organisations have difficulty introducing health outcomes into purchasing contracts. The result is that managers and purchasers continue to assign resources to the pursuit of health outcomes as an accountability tool. This thesis addresses two research questions around the use of health outcomes in community and allied health service accountability. The first is the barriers to the application of health outcomes to health services accountability. The second question examines the conditions that must be met before health outcomes can be used as an accountability tool in purchasing contracts for allied health. The research questions are addressed through the analysis of case studies that explore systematically the approach taken by two organisations, the Department of Veterans� Affairs and ACT Community Care, in their attempts to identify health outcomes that could be used in purchasing contracts for community and allied health services. The case study analysis uses a health services research approach that draws on multidisciplinary techniques including epidemiology, health services management and anthropology. The thesis describes the accountability interactions within the purchaser-provider model. Accountability is not a uniform construct. It consists of many domains, levels and interactions. In health service delivery, there are a number of different actors and a wide range of interactions for which they are accountable. Two important interactions are identified: professional accountability, which describes the accountability of the health service professional to their patient; and contractual accountability, which is the obligation of the health service provider (or providing organisation) to the purchaser through their contractual agreement. I conclude that health outcomes are not an appropriate domain of contractual accountability but they are an important component of professional accountability and I discuss the implications of these finding for theory and practice.

The purpose of the present study was to examine the costs incurred and the benefits realized by institutions participating in the NCATE accreditation process and to formulate a cost-benefit model to guide teacher training institutions who are assessing the value of peerreview by NCATE. The study utilized quantitative methodology with a descriptive research design. The study featured researcher-designed questionnaires: Accreditation Cost-Benefit Analysis Scale for faculty (ACBAS) and the Costs Inventory Analysis (CIA) for administrators and was administered to a purposive sample of faculty and administrators at 54 colleges of education that had participated in the NCATE accreditation process and sitevisit during the period of January 2003-December 2004. The data indicated that faculty and administrators hold distinct perceptions regarding the benefits, costs, and other issues related to NCATE accreditation. Administrators specified the mean cost of NCATE accreditation was approximately $100,000, on average, as indicated by an analysis of the data provided on the CIA. Furthermore, a discriminant analysis of the data confirmed that administrators and those faculty considerably (7-10 hours per week) involved in the accreditation process had a greater appreciation for the benefits and costs of NCATE accreditation than did those faculty and significantly (3-6 hours per week) or only moderately (0-2 hours per week) involved. Finally, the data indicated that there was no difference in the perceptions between faculty and administrators regarding costs, benefits, and other issues related to accreditation when measured on the ACBAS.

Community engagement in universities of technology is evolving to become a game-changer in addressing the economic challenges of the country (Erasmus, 2005:4). Service learning (SL) as a form of community engagement is promoted in higher education, as a learning paradigm. Conway, Amel and Gerwien (2009:238) encourage enquiry into the benefits of this pedagogy and the partnerships involved, as faculties curriculate and adopt the service learning model into current programmes (Lazarus, Erasmus, Hendricks, Nduna & Slamat, 2008:60-61). The SL project of the Tourism Management Department at the Cape Peninsula University of Technology (CPUT) is distinctive in design from the generic social-change focussed projects within Universities of Technology (UoTs). Its outcomes are based on the development of business plans for small and medium enterprises for implementation. The assessment of this interactive learning process was essential to determine whether it yielded any positive change in the participants. Thus the aim of the study is to determine the success or otherwise of the service learning project, so as to ensure that the small, medium and micro enterprises (SMMEs) community and students benefit from the Tourism Management project. The study sample included nine small and medium tourism entrepreneurs (SMTEs) and 178 third year tourism management students. From the sample two SMTEs participated in one-on-one interviews and six students participated in a focus-group interview. Action research was undertaken for this study, and census sampling was adopted to collect quantitative and qualitative data from project participants. The ATLAS.ti 7 data analysis programme was employed to process the qualitative data, and SPSS 22.0 software was utilised to develop frequency tables from the quantitative data. All data was triangulated to obtain conclusions. Fifty-two percent (52%) of student participants learnt leadership skills, with 64% indicating essential benefits on business skills. Company visions were shared throughout the collaborative process, and 76% of SMTE responses, directed to the students’ solutions, being highly innovative for their businesses; and these were implemented to improve business processes. The findings of the study attested to the SL model improving reciprocal learning, and having positive benefits for all partners involved. In continuing the essential dialogue on implementation of SL in higher education, sustainable partnerships in SL projects are essential to ensure continued positive results. The majority of participants responded positively to a willingness to participate in a similar project, which is a positive indicator of the benefits of the project. Implications for future studies point towards a need for continued monitoring of growth performance in the participating small and medium enterprises.

The selection of an inappropriate regime by policy makers can thwart the effective implementation of public policy and lead to implementation failure. Competing values in the implementation process have a significant impact on the results of implementation, and the regime selected for implementation implies the choice of one value over another. Stoker has argued that on the one hand central leadership is valued for consistency, benefits of scale, co-ordination and cost sharing (instrumental model); and on the other, diffuse authority is valued as knowledge is particular and situational, and the task of analysis is to understand the problems, perspectives and interactions of implementation participants at the contact point between public programs and their clients (accommodation model). This study examines the implementation regime chosen by the Victorian Government for the introduction of a modern internal auditing function into the Victorian Public Service, using the Bureau of Internal Audit within its Department of Management and Budget. The selection of the Department of Management and Budget for the introduction of Internal Audit as a managerial accountability mechanism indicates that the instrumental model was held by the Victorian Government to be the most important in implementation. It is argued that use of a top-down central agency leadership approach resulted in implementation failure. Three propositions suggesting the mechanisms by which this result is brought about are examined: that lack of co-ordinated planning at the outset will result in inadequate definition of client needs as part of policy formulation; that intraorganisational conflict during the implementation success, as individual participants are likely to exercise their veto; and the increasing the number of participants contributes to complexity, so that they should only be involved in implementation when their presence is absolutely required. It is argued that the essential task of implementation is to create an environment where participants are likely to co-operate to achieve predetermined public policy goals; and that the introduction of a modern internal auditing approach into the Victorian Public Service required a more participatory implementation regime in order to facilitate policy outcomes and prevent implementation failure. It is also argued that the dominance of economic reform over accountability in Victoria restricted the ability of the Bureau of Internal Audit to implement change to internal audit practices within Victorian Government departments. The selection of an instrumental model of implementation by the Department of Management and Budget is examined in the context of the environment that existed in Victoria between 1982-1987; and while some of the values which Stoker associates with the top-down approach to policy making were observed, an alternative view to the development of internal auditing in the Victorian Public Service can be sustained.

Many elderly veterans were forced to leave home because of the war, and in the process of fleeing, suffered many obstacles. Due to ethnic culture, living habits, age gap and economic factors and other issues, many of the elderly veterans hid in a corner of society and became the so-called "social exclusion." But also because of the impact of a low material level and quality of life, they have gradually become the neglected vulnerable groups in society. There is relatively little research on social tourism for the elderly veterans. It is thus worthwhile to explore the demands and benefits of social tourism among this particular group. The selected data collection sites were Baihe Veterans Home, Gangshan Veterans Home, and Pingtung Veterans Home. The study objects were those living in the veterans homes and at least 65 years old. The interview was from December 5, 2016 to May 9, 2017, with a total of 10 interviewees. The average age of the interviewees was 79 years old with good health and had a wealth of tourism experience. The study found, survived after the war, the elderly veterans came thousands of miles to Taiwan and ultimately settled in the veterans homes, and this special experience created their strong physique and tough vitality. The study helps to understand the tourism demands and barriers of the elderly veterans in order to enhance their tourism benefits participating in social tourism. The results of the study can also be used as a reference for the social tourism industry and the government to formulate strategies, to maximize the benefits of social tourism for the veterans group, and to realize the right of everyone to enjoy travel.

This research aims to firstly explore the effects of mentor functions and mentoring structure on the benefits of mentors. We also examine whether the mentor benefits leads to enhance the department performance. This study used paired sampling to collect mentor-protégé dyads data. 310 mentors questionnaires were distributed, and 215 valid samples were returned. The returned rate was 69.3%. 1000 questionnaires were distributed to protégés. 676 valid questionnaires were returned, with a 67.6% return rate. This research found that mentoring functions facilitate to acquire mentor benefits. As well as various mentor structure cause different benefits that mentor gained. However, the benefits of mentor can’t lead to enhance the performance of department.

Previous studies have shown that natural environment and landscape has restorative qualities. People can perceive positive physiological and psychological benefits when they are in the natural environment or landscape. It was also shown that healing gardens surrounding the hospitals have positive effects on users’ psychophysiological benefits. However, how to import healing garden design guidelines to improve landscape quality of outdoor garden within medical facilities and enhance its restorative qualities and psychophysiological benefits still lacks supports from empirical studies. Therefore, based on Kaplan & Kaplan’s (1989) theory, this study was to explore the difference between the existing gardens and the simulated healing gardens in a medical institution regarding users’ restorative perception and perception of psychophysiological benefits. In order to compare the existing garden with simulated healing garden on the perceptions of restorativeness and psychophysiological benefits, the healing garden was presented by a photo which was simulated by using the photo of the existing garden as the background, and then adding into the characteristics of healing gardens. Data were collected using a self-reported questionnaire. The study subjects comprised patients, their families, and medical staff A convenient sampling survey was conducted at two gardens within Taichung Veterans General Hospita and 338 valid questionnaires were obtained. This study employed a one-group pretest-posttest design. In the first stage, the subjects were asked how they feel about the existing garden in terms of restorative perception and perception of psychophysiological benefits; in the second stage, the subjects were requested to watch a photo of the simulated healing garden, and then report how they feel about the simulated garden in terms of restorative perception and perception of psychophysiological benefits. It was found that both the restorative perception and the perception of psychophysiological benefits were higher for the simulated healing garden than for the existing garden. The phenomena are especially significant in the cognitive aspect of fascination and perception of pressure mitigation. Moreover, users reported higher emotional and pressure mitigation effects when they had higher perception on “being away” and “compatibility” for the simulated healing garden. Based on the findings, it was suggested that in order to improve the perception of psychophysiological benefits for patients, their families, and medical staff, the characteristics of healing gardens should be integrated into the design of gardens in the medical institution. These characteristics include integrity, water features, diversity of planting, pathways with mysterious features, public and private spaces, accessibility and security, and appropriate outdoor facilities.

Since Ulrich in 1984, the medical environment will influence the healing effect of patients, and the formation of hospital environmental atmosphere has gradually been paid attention to. In recent years, architects and landscape architects have begun to pay attention to the needs of hospital users and put forward patient centered design principles to plan hospital space allocation (Whitehouse, et, al., 2001). Therefore, the hospital planning and design, if you can consider the effect of outdoor garden space treatment, will be more conducive to the patient and other users of psychological benefits. This study mainly introduced the concept of restorative landscape, designed the healing garden, and then produced the effects of the current situation and simulation, and further explored the physiological and psychological benefits of the participants. The study tested of 30 students of Fengjia University were selected from this study. The Subjects are wearing a brainwave instrument to watch the current situation by the laptop garden and measure the fill in the questionnaire (pretest), and then wear the brain wave instrument by watching the computer simulation of the curvature of the garden photo measurement and fill the questionnaire (posttest), which compared the difference before and after the test. The results show that the garden and the healing garden for the physiological and psychological benefits of the view is different, the treatment of the garden regardless of the psychological benefits, the average number of physiological benefits are higher than the current situation, indicating that the subjects need more The garden of the healing garden. In the regression analysis of healing effect of perceived restoration in the garden in the simulation, can significantly affect the healing benefits - clear your head "compatibility" in the recovery more benefit directly attention can achieve a significant influence to the "compatibility", the restoration effects of cognition on peace can be achieved a significant influence to " extent " and "compatibility", more complex benefit endurance performance significantly affect " extent " and "compatibility", but in " cognitive quiet " and " duration " to " extent " negative influence. The South African National Department of Environment Affairs (DEA) in Pretoria has been experiencing a high vacancy rate of 20%, 18% and 26% in the financial years 2008/9, 2009/10, and 2010/11 respectively against a target of 16% . The vacancy rate for 2013/14 financial year has increased to 24.5% against a target of 10%. The Department is struggling to meet its own set target by end of financial year. The current recruitment method utilised by DEA is too slow and tedious, takes time to appoint and has human elements as some documents get lost and does not promote efficiency and value for money. The study was aimed at assessing the degree to which e-recruitment methods were used in the Department of Environmental Affairs. The study was conducted in line with the objective of the DEA to reduce the existing vacancy rate at the DEA to at least 10% by 31 March 2014. This study assessed and evaluated the impact of implementing the online recruitment process in the Department and the contributing factors to a high vacancy rate such as the long waiting process.

This report examines the place of non-citizen soldiers, veterans, and their families in U.S. political and civil life. Historically, military service has allowed marginalized groups to earn their social and political status as equal citizens. Part one of this report explores why, despite this history, recent legislative changes, and a 2002 Executive Order eliminating the legal and bureaucratic barriers to naturalization, less than forty percent of the non-citizen servicemen and women today actually acquire U.S. citizenship while on active duty. Part two examines the political and policy context surrounding a soldier's decisions to naturalize. It suggests that some soldiers may be “undocumented”; they forgo naturalization to protect themselves and their families. Part three discusses the legal, political, and normative implications of current policy. Some practices, such as the deportation of alien veterans, challenge the foundations of the American political order. The place of undocumented soldiers and veterans raises important issues about civic obligation, the cultural narratives that define membership in and service to the state, and the ruling political collations in which these narratives find support.

Total of 570 questionnaires sent formal questionnaires, 552 , 530 valid questionnaires were effective rate as high as 96%. Recovery data using descriptive statistics , chi-square test , t test, multiple regression , canonical correlation and Scheffé post hoc comparison test conducted statistical methods such as handling. Found that different personal background variables in leisure , recreation and leisure benefits involved significant difference factors such as: 1. Field studies have shown that police officers feel the leisure involvement and leisure benefits showed a highly degree of reaction . Leisure involvement was found in the " Years of Service "," religion "and "units do not " have a significant difference . Found in the benefits of leisure , " religion "and "units do not " have a significant difference. 2. Involvement in leisure and leisure benefits of correlation analysis found that leisure involvement and leisure benefits were positively related to leisure involvement and leisure benefits of experience can predict . Background: Increasing numbers of women veterans (WV) present an organizational challenge to a healthcare system that primarily serves men. WV use reproductive services traditionally not provided by the Veterans Health Administration (VHA). Objective: Examine the association of organizational factors and adoption of comprehensive women’s health (WH) care in the VHA. Study Design: Cross-sectional secondary analysis of the 2007 VHA Survey of Women’s Veterans Health Programs and Practices. Methods: Dependent measures were (a) model of women’s health care: separate women’s health clinic (WHC), designated women’s health provider within primary care (DWHP), both (WHC/DWHP), or neither and (b) availability of five basic WH services: cervical cancer screening and evaluation and management of: vaginitis, menstrual disorders, contraception and menopause. Exposure variables were organizational factors drawn from an adaptation of the Greenhalgh model of diffusion of innovations including structural factors, measures of absorptive capacity and system readiness for innovation. Results: Compared to sites with DWHP or neither, WHC and WHC/DWHP were more likely at facilities with: a gynecology clinic, an academic affiliation, a WH representative on high impact committee, and a greater number of WV. Academic affiliation and high impact committee remained significant in multivariable analysis. All five basic WH services were more likely to be offered at sites with WHC or WHC/DWHP, remaining significant after adjusting for organizational factors. Conclusion: Facilities that adopt WHC are associated with greater absorptive capacity (academic affiliation and WH representation on high-impact committees) and are more likely to deliver basic WH services. Separate WHCs may promote more comprehensive care for WV.

Since the first publication on the matter back in 2004, Service-dominant (S-D) logic has contributed intensely for the reformulation and contestation of Marketing principles and traditional organizational frameworks still adopted by companies. Thus, this paper focuses on a Mercedez-benz’s after-sales services case study, as an attempt to construct a visualization of Mercedes-benz’s actual after-sales services framework, and another visualization of that same framework through an S-D logic lens. After a literary analysis to the academic publications about the subject that were considered to be the most relevant, this research paper was based on stakeholders interviews and a consumer experience questionnaire. It was identified an existing gap between the firm’s value proposition and the value perceived by the consumer, whose adherence rate to the brand’s after-sales services is low regardless of recognizing them as important. This research paper then proposes a critical analysis that allows Mercedesbenz to create an organizational framework aligned with S-D logic’s premises that transforms consumers into active actors in the value co-creation process, as well as the managerial implications that would bring.
Desde a primeira publicação sobre o tema em 2004, Service-dominant (S-D) logic tem contribuido extensivamente para a reformulação e contestação dos princípios básicos do Marketing e dos modelos organizacionais tradicionais pelos quais se regem as organizações. No entanto, há uma necessidade latente de casos de estudo empíricos que apliquem os principais conceitos e premissas de S-D logic a casos reais. Como tal, o presente trabalho tem como foco um caso de estudo aos serviços pós-venda da Mercedesbenz, numa tentativa de construir uma visualização da actual estrutura organizacional dos serviços pós-venda da firma e outra visualização da mesma estrutura através de uma lente S-D logic. Após uma análise literária às publicações consideradas mais relevantes acerca do tema, o presente estudo teve por base em entrevistas feitas a stakeholders da empresa e de um questionário à satisfação do consumidor. Foi identificado um gap entre o valor proposto pela empresa e o valor perceptido pelos consumidores, que apesar de reconhecerem o valor dos serviços após-venda da marca não os utilizam. Este estudo propõe depois uma análisa crítica que permita à Mercedes-benz uma aproximação a um modelo organizacional alinhado com as premissas S-D logic e que coloca o consumidor actual como um actor activo no processo colaborativo de co-criação de valor, bem como as implicações que isso traria para a empresa.

This study employs the approach of quasi-experimental studies. It recruits 30 clinical nurses from Taipei Veterans General Hospital who are not exercising on a regular basis. Participants are assigned by their willing to the Junjian-yan trail group、stair climbing group and control group for 3 months of participation in Junjian-Yan Trail hiking ,stair climbing. Pre-participation, post-participation and 3 months later follow-up physicals activities are involved with the evaluation of health benefits. Evaluation indicators include body weight(BW), body mass index(BMI), blood sugar(BS), cholesterol, triglyceride(TG),high density cholesterol(HDL)and SF-36 questionnaires. The subjects in experiment group go hiking at Junjian-Yan Trail ,stair climbing at least 3 times or more a week. Every time the physical activity lasts for 30-50 minutes. Three months later ,and the following three months the same health evaluation indicators are obtained for comparing of the differences, and determine to evaluate the health benefit of physical activity on hiking to Junjian-Yan Trail and stair climbing. Research indicates that from Mar. 16th, 2005 to Sep 16, 2005, Junjian-yan trail group has 11 individuals, stair climbing group has 8 individuals ,and control group has 11 individuals had completed the experiment. Though the result is not significant, these figures show the tendency of improvement. This study has been conducted for the purpose of encouraging clinical nurses to engage in certain types of physical activities, including hiking ,stair climbing, maintaining healthful life-style; and for the objective of revealing the image of nurses as a healthy symbol as well as establishing a healthy indicator resulted from physical activities.

Research was conducted using hermeneutic-phenomenology and semi-structure interviews to explore and understand the lived experience of women veterans of the Canadian Forces. Women recently entered Canadian military combat positions, taking on a profession historically exclusively occupied by men. Due to the lack of research on women veterans of the Canadian Forces, knowledge was drawn from research about women in nontraditional work, American paramilitary and military occupations, as well as an historical review of women’s involvement in the Canadian Forces, to provide context to the research themes. Participants comprised of 6 women veterans who described 11 essential and 4 significant themes. Unique contributions to literature include essential themes such as, Slut or a lesbian, take your pick, Proving I’m good enough, Trying to be treated better, Got some support, Visible and singled out, Perpetual outsider, Given gender based tasks or opportunities, and Women demeaned. Suggestions for future studies and implications for counselling practice are discussed.

This paper aims to explore the operational efficiency of the Service Department of Veterans Affairs Commission. The paper applies the three-stage DEA developed by Fried et al. (2002) and included external environment variables such as "geographical location", "the number of veterans" and " the number of hospitals of regional size", analyzed by output-oriented model. We explore the impact of external environment variables on operational efficiency and analyzes the different between 2008 hereafter. Finally, we explore the factors that may affect the operational efficiency of The Veterans Service Department. The results are as follows: (1) There are significant differences in operational efficiency of the Service Department after the environment variables are included. (2) The operational efficiency reveals significant progress between 2008 hereafter. (3) The proportion of each learning course and the number of community service volunteers are positively related to the operational efficiency, the labor participation rate is negatively related to the operational efficiency values, and it’s non-related between educational background and the efficiency values.

Patient records are tremendous assets in hospital, not only processing diagnosis data, but more likely to manage the specialty of medical and health care information. The trend of hospital management will substitute electronic with paper-based records. Although many researches promote the use of electronic record, there are few studies that support the practices on any staff of medical work. This study was investigated for understanding the correlation of electronic record effectiveness and risk management of patient record after implementation. This study was designed with a structured questionnaire to measure seven sections of patient record in risk management and five sections of effectiveness of electronic record for the staffs in twenty-six public hospitals. The survey was 96.15% returned from hospital and individual, meanwhile questionnaires were accessed by SPSS 12.0 for statistic analysis. The results show that the time effectiveness of electronic record is just the same compare with published articles. Moreover, a significant difference between staffs and units is referred to the risk management of patient record. In addition, it has all been shown a positive influence of environment, legal, software and hardware devices risk except information risk. Therefore, hospital education on the safety and privacy will be mandatory for electronic health records, however, this study will be contributed to strategy of medical information in Taiwan.

In order to serve and take good care of military retired soldiers, Veterans Affairs Commission of Excutive Yuan has set Veterans Service Departments in every adminidtrative areas around the country to offer assistance. Changing with time and places, the official departments have adjusted their servicing, which therefore requires a more positive and refined way to serve and attend on the retired servicemen, to seek for prominent service meeting our veterans’ needs. This study studies on the veterans of Fenyuan Veterans Service Department, applying quota sampling, providing 1,063 questionaires, including 484 valid copies. The effective response rate is 45.5%. The collected data were analyzed by descriptive statistics, t-test, one-way ANOVA, Pearson correlation, and multiple regression analysis. The results of the study are summarized as follows: 1. Vetarans with different backgrounds have distinct expectation and satisfication toward veterans’ service departments, and their identification with the government also vary distinguishly. 2. Expectation of service qualitiy positively affect degrees of satisfication. 3. Degrees of service quality satisfication as well as degrees of satisfication positively affect on the degrees of the identification with Govenment 4. Degrees of satisfication of service quality mediates the influence, which expectation degrees of service quality affect the government identification. In conclusion, this study, as a referencial information, would provide practial suggestions to Veterans Affairs Commission of Excutive Yuan as well as the following researchers.

The New Public Management(NPM)movement was prevalent around the globe in the 1980s, which was dedicated to reforming public administration at national levels. NPM advocates hold that by applying strategic business management to the public sector, a capable government that is customer-oriented, innovative, authorized, flexible, and valuable will be formed. Hence, as the government promotes enhancing the quality of public service, the Taoyuan City Veterans Service Department should act accordingly to ensure veterans’ satisfaction and receive their recognition. The purpose of this study is to investigate the service quality, satisfaction, and the status quo, difference, and correlation of the identification of organization in the Taoyuan County Veterans Service Department. This study employed a questionnaire survey with veterans residing in Taoyuan as respondents. There were 477 questionnaires in total, with 428 collected, 405 of which were valid. The results were analyzed using descriptive statistics, one-way ANOVA, and Pearson Correlation. The findings were as follows: 1)Veterans in Taoyuan showed positive attitude towards the service quality, satisfaction, and identification of organization; 2)there is a stark difference in terms of tangible aspect of service quality due to differences in age, education background, and current work status; 3)veterans’ satisfaction with the service quality varies significantly due to different education backgrounds; 4)there is a significant difference in the identification of organization owing to different education backgrounds; 5)the service quality and satisfaction are moderately positive correlated; 6)satisfaction with the service quality and the identification of organization are moderately positive correlated; 7)contrary to what was expected, there is a significant difference between the service quality and satisfaction. Finally, the results of this study could offer suggestions to the Veterans Affairs Council(VAC), the Taoyuan County Veterans Service Department, service providers, and future researchers. This study provides feedback from the Extension Officers and community garden members involved in community garden projects of the KwaZulu-Natal Department of Agriculture and Environmental Affairs, by investigating the contribution of community gardens to household food security. The investigation included interviews with 61 Extension Officers from the Bergville, Hlanganani, Eshowe, Vryheid and Mbumbulu districts and 106 community garden members from 31 community gardens in the Bergville and Hlanganani districts. It was found that the people involved in community gardens were older persons, with little or no education. Those involved in community gardens were unemployed and relied on pensions, remittances and selling handicrafts to purchase garden inputs. The average household dependency ratios were 4, 5 and 5, 5 persons per active household member in Hlanganani and Bergville, respectively. Community garden members were mostly females, but most community garden members’ households were male-headed. The community garden members indicated that the most important reason for producing vegetables was for household use. Extension Officers were not satisfied with the state of community gardens and the relationship between the regional and district offices. Community gardens do not reflect the effort put in by the Extension Officers. The Extension Officers want to be involved in determining research priorities and be regularly informed about research findings and policy changes made at the regional level. The Extension Officers would like to see all government departments working together towards the development of communities. Extension Officers believe that if the government departments pool their resources, they would be able to serve the communities better than when each department works alone on community garden projects in the same community. Despite the initial funding of community gardens by the KwaZulu-Natal Department of Agriculture and Environmental Affairs, there were many factors limiting production of vegetables in community gardens. The limiting factors included poor soil fertility; small garden plot sizes; low water availability; high start-up costs and poor management of the community garden finances. The community garden members were positive about the contribution of community gardens to their lives. The benefits were in the form of information about vegetable production, cropping practices and the availability of fresh vegetables that provided nutritious food and allowed them to buy other household requirements instead of vegetables. Community garden projects have a potential role to play in the lives of many rural people if the following concerns raised by the Extension Officers are addressed: the relationship between the offices at district level and the regional level improves; the establishment and maintenance of sound channels of communication between the district field staff and the regions; research support is received from the regions; and government departments work together. The KwaZulu-Natal Department of Agriculture and Environmental Affairs should not only focus on providing infrastructure for projects but should also ascertain that the proposed project achieves the objectives of both the community garden members and the Department of Agriculture and Environmental Affairs and that the beneficiaries are trained before the project is handed over.
