Full textThe aim of this thesis is to give an account of the uneasiness of young adults who were seeking political asylum in France. Since their childhood, these young people have to deal with a certain numbers of traumatic, narcissistic experiences: a series of events which attest that they had to bear neglects, abandonments, break-ups and disappointments which are responsible for objectal and/or narcissistic losses which grief was problematic. They were confronted with the total collapse of infantile strength, in other words, an early loss of (imaginary or real) object: the problem of mother’s desertion. We can observe a few difficulties inherent to the process of individualization, characteristic of adolescence. They are likely to generate personalities similar to the affects considered as typical of pathological limit states and associated traumatic losses liable to provoke depressive symptoms

Full textNeuroimaging findings have been reported in emotional regions in both adults and adolescents with depression but it still remains unknown whether such brain alterations can be detected before depression onset or reflect disease progression. Although subthreshold-depression in adolescence is a condition at risk for Major Depressive Disorder, not all youths with subthreshold depression will develop full-syndrome depression. Thus, studying brain correlates of subthreshold-depression in adolescence may inform on the pathophysiology of depression. We used clinical and, T1 weighted and diffusion magnetic resonance imaging data from the IMAGEN study, an European and population-based cohort of 2131 adolescents recruited from secondary schools at age 14 and followed-up at age 16. Regional gray and white matter morphometry and white matter microstructure were compared between adolescents with subthreshold-depression and healthy control adolescents. Macro and micro structural brain changes were found in adolescents with subthreshold-depression in regions involved in Major Depressive Disorder. The relation between subthreshold-depression at baseline and clinical depression at follow-up was mediated by lower medial-prefrontal gray matter volume in girls and by lower fractional anisotropy in tracts projecting from the corpus callosum to the anterior cingulate cortex in both sexes. The findings suggest that subthreshold-depression in early adolescence is associated with structural volumetric and connectivity changes in emotion-regulation circuits, and that some of these changes might denote high risk for later clinical depression

Full textObjectives-The consequence of Psychological Distress (PD) on brain function is poorly understood, though PD caused by adverse life events (ALE) is known to contribute to anxio-depressive vulnerability. Given the rising incidence of anxio-depressive disorders beginning in early adolescence, we investigated the influence of PD on frontal-limbic networks implicated in anxio-depressive pathophysiology, using a social threat paradigm, in 14-year-old adolescents. Method-Six hundred and eighty five 14-year-old adolescents with no psychiatric diagnosis were included in the study from the Imagen database of 2,223 young adolescents recruited in community schools. Participants underwent functional Magnetic Resonance Imaging while viewing angry and neutral faces. Lifetime PD and ALE were assessed using a life-event dimensional questionnaire. The correlations between lifetime PD and ALE scores and neural response during angry faces processing were investigated using SPM8. In an additional exploratory analysis, the relation between lifetime PD and ALE and the occurrence of anxio-depressive disorders two years after the inclusion was investigated. Results-Lifetime PD correlated positively with lateral orbitofrontal and insula responses during angry faces processing. Lifetime PD scores correlated with the probabilities of meeting Generalized Anxiety Disorder or Major Depressive Disorder criteria at follow-up. Conclusion-This is the first report of a relation between PD, a vulnerability factor for anxio-depressive disorders, and neural response related to social threat processing in regions implicated in self-awareness and emotional regulation in healthy adolescents

Full textMental health issues are common in adolescents and can have a lasting negative impact ontheir overall health. Several risk factors for depressive symptoms in this population are wellknown, such as being a girl or being bullied. School connectedness appears to be a protective factor in adolescents’ depressive symptoms. However, its positive impact among vulnerable groups remains unknown. This master’s thesis has three distinct objectives. The first one isto assess the influence of school connectedness on depressive symptoms among adolescents.The second is to see if its influence varies among vulnerable groups, such as girls, victims of bullying and those who combine these two vulnerabilities. The third is to examine the hypothetical relative impact whether public health interventions aimed at increasing school connectedness, eliminating bullying or eliminating the gender gap could achieve on significant depressive symptoms based on the associations found above. To answer these questions, datafrom COMPASS-Quebec’s 2017-2018 cohort of 17,367 students from 37 high schools in Eastern Quebec were used ( Chapter 1). The results suggest that the students’ reported school connectedness has a protective influence on their depressive symptoms and its influence appears evengreater among young people cumulating vulnerabilities, such as bullied girls. However, evenwith the highest school connectedness scores, this influence may not be sufficient in the mostat-risk students for depressive symptoms.

Full textThe aim of this research is to study the psychic functioning of the suicidal adolescent through psychoanalytic thinking. The assessment of 17 adolescents enabled us to draw some conclusions: The suicidal gesture in adolescence appears to be linked to adolescents’ difficulties to deal with the instincts. This difficulty affects the adolescent’s entire psychic functioning and especially his/her thinking processes. The thinking processes are affected in two ways: either they become disorganized under the effect of the defusion of instincts, or they become rigid to avoid their impact. By attacking his own body the adolescent expresses a hate towards both himself and the other, as the two are not differentiated enough in moments when instincts’ activity is intense. A further analysis of the way the object is invested reveals that these adolescents tend either to merge with the other, or to avoid any interaction which could potentially lead to a state of fusion with the object. This state of confusion between the object and the subject, is linked to the difficulty of the adolescents to deal with the depressive feelings deriving from separation. For some adolescents the suicidal gesture is a direct consequence of depression. But for some others the suicidal gesture is part of a defensive strategy against it

Full textPeer victimization in school has important negative impact on the mental health and the academic achievement of many adolescents. The main objectives of this doctoral research project are to: (a) evaluate the prevalence, frequency and type of peer victimization among adolescents, (b) assess the prevalence and severity of depression among secondary school students and (c) determine the importance of the relationship between victimization and depression. Participants were 356 college students (age 12-15) from France and 360 secondary school students (age 13-15) from Quebec. They completed the Beck Depression Inventory and two victimization scales from the Questionnaire sur l'environnement socioéducatif. Results show that both peer victimization and depression are important realities in the life of French and Quebec students and that victims of peer violence are more depressed than non-victims. Gender, age and type of victimization affect this relationship. Verbal victimization appears to be more related to depression among adolescents. Results support the importance of timely and preventive interventions regarding violence in school and peer victimization in particular.

Full textWe spend almost the third of our lifetime sleeping. Sleep is part of the body’s vital functions. A nationwide survey was launched in Lebanon in order to examine and evaluate the Lebanese adolescents’ sleep patterns and lifestyles and to assess the most prevalent sleep disorders and their associated factors, with the purpose of filling in the gaps in this area. 41.2% of the teenagers slept less than 7 hours per night during the week, and 34.4% once in bed always surf the internet. Insomnia and delayed sleep phase disorder (DSPD) were the most common sleep disorders among Lebanese adolescents; they had a prevalence of 13.1% and 26.3% respectively. Depression was the common factor associated with these two disorders. In conclusion, conducting longitudinal studies, in order to identify the most common risk factors of insomnia and DSPD in Lebanese adolescents and suggesting solutions to facilitate the prevention of these disorders and their risk factors are currently a must

Full textLife events, Psychotraumas and Suicide Attempts in Child and Adolescents Psychotrauma corresponds to a life event which is characterised by its intensity, the incapacity to cope adequately and the overwhelming lasting pathogenic effects induced in the psychological organization. If the childhood psychotrama has been minimized for a long time, it is now well identified. However the issue of their exploration is rather recent. Studies concerning psychotrauma, and its consequences, have considerable difficulties in defining their boundaries. It is particularly difficult to detail mechanisms implicated in a trauma. These aspects are developed in the first part of this work. In a second part, the psychometric properties of the French version of the Child Post Traumatic Symptom Reaction Index (CPTS-RI), which is an auto administered tool elaborated to diagnose Post Traumatic Stress Disorders in children and adolescents have been studied. Thanks to its psychometrics properties, its speed and ease with which it is administrated and quoted, it is an interesting and usable instrument for clinicians as well as researchers to evaluate the PTSD in children and adolescents. In addition to its diagnostic aspect it allows to quantify symptomatology intensity. It had never been validated in its French version. In the third part, the associations between life events, the amount of worry and suicide attempts during adolescence. It is shown that the number and the nature of life events have a significant impact on the risk of suicide attempt. When these life events happen adolescents' worries are impacted by them. Finally the risk of suicide attempt is increased when the amounts of worry domains are accentuated

Full textDepression is an underdiagnosed complication of dementia, both disorders sharing some manifestations. The aim of this cross-sectional observational study was to examine the prevalence of depression among nursing home residents suffering from dementia, and to evaluate the sensitivity of detection of this disorder by the primary care team. Clinical profiles were assessed using the Cornell Scale for Depression in Dementia, the Structured Clinical Interview for DSM-IV, the Neuropsychiatric Inventory, the Quality of life in Alzheimer’s Disease Scale and the Mini Mental State Examination. Among 116 residents from 10 nursing homes, 17 cases of depression were diagnosed, from which only 7 had been detected by the clinical staff. Depression cases had a significantly poorer quality of life and a worse score on the NPI-NH distress scale. Overall, this study reveals that depressive syndromes are under-diagnosed in nursing homes residents and have a negative impact on quality of life of patients.

Full textDepression is among the most common psychopathology for which treatment is sought in psychological and psychiatric practices and its impact at all levels including its economical outlay in the coming years has been estimated on a worldwide scale to surpass that of current illnesses such as infectious diseases and cardiovascular illnesses. Research has demonstrated that stress and anxiety levels, as early as in adolescence, are among factors that contribute to the development of depression. The aim of this research study is therefore to investigate the psychometric properties of the French Depression Anxiety Stress Scales (DASS-21; Lovibond & Lovibond, 1995) for its eventual use in the Mauritian adolescent population as a means to screen for depression, anxiety, and stress. Factor analyses were conducted on 1002 response sets and the three-factor structure of the DASS-21 was supported. Internal consistency was satisfactory and the scales demonstrated concurrent validity with other measures of depression, anxiety and stress. The construct validity of the scales was further strengthened with gender invariance. Finally, appraisal of stress was the psychological dimension which predicted best depression in the Mauritian adolescent population

Full textThe present thesis investigates the links between burnout, academic stress and depression among adolescents. The study is based on two main approaches. In the first approach, quantitative and statistics results are obtained about the links between the three main variables within a sample of 1006 high school students. In the second approach, around twenty qualitative and longitudinal case studies are conducted using semi-directional interviews and projective Rorschach and TAT tests. Statistically, the three main variables show significant positives links. Burnout is explained by academic stress and depressive symptomatology is explained by burnout. Burnout is a mediator between academic stress and depressive symptomatology. A “school burnout” is indeed observed among high school students. It differs from depression by its exclusive exogenous pathogenic academic stress causality. It mainly roots on the work inter-subjective bonds. Burnout is independent from a specific psychic organization or structure. Burnout and depression links have thus been identified, allowing to consider prevention and/or therapeutic perspectives

Full textResearch on school drop-out has led to consider early school leaving as a medium- or long-term process down a path marked with numerous difficulties, as much on a personal and family level as educational (French Ministry of Education, 2012). These factors make a student's school éducation vulnerable, particularly for boys who generally experience more difficulty adapting to school than girls do (Royer, 2010). More recently, studies including psychological factors have pointed out the relevance of a more personalised préventive approach, based on the student's own educational experience. The goal of this thesis was to analyse the influence of self-esteem, academic motivation and dépression on the risk of dropping out, using quantitative methods on a sample of 265 middle- and high-school students in the general sector. First of all, we studied the above factors of self-esteem, academic motivation and dépression in relation to the sex and age of the students, taking into account the complexity of these three dimensions (Gurtner, Gorga, Monnard et Ntamakiliro, 2001 ; Harter, 1988 ). The results obtained correspond to the commonly held viewpoint that girls are more motivated and committed to their education and manifest more dépressive states than boys, while boys have a higher self-esteem. The results also show that the older students are, the less they are motivated and therefore more likely to drop out. Secondly, we studied the influence of these factors on the risk of drop-out. In particular, we pointed out the mediating role of depression in the established link between self-esteem in the area of morality and behavior and that of a state of anxiety towards maths and French with a risk of dropping out. More specifically, the study revealed that this same role played by depression is even more determinant for boys, making psychological distress an essential risk factor in the process of teenage dropout (Quiroga, Janosz et Marcotte, 2006). The research and analysis carried out for this thesis confirm the importance of focusing on the personal risk factors in order to understand the process that could lead a student to disconnect from school. It also shows that these personal factors become the central question for boys, since unlike girls, they seem to demonstrate more difficulties of a psychological order, creating a higher risk of dropout

Full textThe aim of this thesis relying on the PAQUID study was to better understand the natural history of neuropsychological impairments and their consequences on complex functional activities on preclinical stage of dementia, this in order to develop a better screening strategy of subjects at high risk of developing dementia. The first section of our work leaded to better identify the cognitive processes impaired in the phase preceding dementia. The results of the Articles 1 and 2 suggest that the controlled processes are deteriorated early in the preclinical phase of dementia. In the second part of this work, we studied the 4 Instrumental Activities of Daily Living and their relation with neuropsychological tests. The results of the articles 3 and 4 allowed to better understand the cognitive processes related to each of these IADL, and to better describe the evolution of each IADL in the 5 years preceding the clinical phase of dementia. In the last section, we determined for clinical practice a strategy of detection of subjects at high risk of developing dementia (article 5). Besides, we also defined a short-form of a depression scale which could be used in general medical practice ; depression is indeed a frequent differential diagnosis of dementia, but can also be a early sign of dementia (article 6)

Full textCurrently, there is essentially two diagnostic choices to explain cognitive impairment in the elderly: "depression"or " Alzheimer's Disease" (AD) the later being a clinical diagnostic that cannot be formally confirmed duringlife. Depression, whose spectral empowerment and biologisation allows the reductionist biomedical model tointegrate depression into pre-dementia chapters of AD, is presented as the modern and exclusive version ofpseudodementia that defines a potentially reversible non-organic dementia state. Our research shows that thisissue is complex and that several factors, unidentified so far, participated in a distortion of the psychiatricconcept of pseudodementia. The first factor is a lack of knowledge and a poor understanding of thepseudodementia primary paradigm: the Psychogene Pseudodemenz of German psychiatry (late 19th - early20th).The second factor is an incomplete and revised interpretation of the British secondary paradigm - theKiloh’s Pseudo-dementia (1961) - where the concept of depressive pseudodementia is introduced.The thirdfactor is an application of the " Alzheimer’s disease movement" (1975-1985), which has led, through the "dementia syndrome of depression " of American psychiatry, to the rise of the third paradigm : depression. The“Alzheimer’s disease movement” cancelled the possibility to use a conceptual framework within whichetiologies of dementia and psychopathological dimension of pseudodementia can be thought.This issue isbecoming increasingly important in the current context of the promotion of early diagnosis and preclinicaldiagnosis of AD.Our research, which supports an integrated approach to aging, proposes a new theoretical andpractical framework able to participate to the reformulation of aging issues and therefore on political, economicand social choices. Based on a qualitative analysis of case series, we show that Pseudodementia is closely tied toneurotic and psychotic inhibition, including depressive presentation but not limited to it. Today more than everthe notion of pseudodementia is required in the care of the elderly
