Full textThis report is part of the curricular unit "Internship" inserted in the Integrated Master in Pharmaceutical Sciences of the University of Beira Interior (Covilhã), being part of the study plan for the 2nd semester of the 5th year. This work is structured in three chapters and refers to the culmination of a whole period of learning and specialized activities in the internships of hospital and community pharmacy, as well as a research work in the hospital pharmacy that intends to study the problem of the medication waste disposal in several inpatient services with single daily dose and single dose, in view of the increasing number of medication in these services and possibly find solutions towered a necessary reduction of costs and waste. The first chapter refers to the research component, which was developed at the Pharmaceutical Services of Hospital Sousa Martins, at the Local Health Unit in Guarda, Portugal, aiming at the analysis of the medication returned to the pharmaceutical services in individual unit doses in various clinical and inpatient services. The management of hospital pharmaceutical services is undoubtedly one of the major challenges and competencies of hospital pharmacists, whom are responsible for the quality, efficacy and safety of drugs. Thus, while the implementation of individualized distribution systems for inpatient services is a pharmaceutical intervention that helps rationalize drug distribution, decrease medication errors, and decrease therapy costs. However, there are still problem with the returns of drugs distributed in this way. This has been constant problem in the reality of pharmaceutical services for some time and has become more and more relevant in recent years. Thus, at a time of constant budget constraints and in which health and the national health service becoming underfunded. The possibility of this type of analysis on drug control waste over a period of ten months (September 2018 to June 2019). Could identify the volume of returns, what type of return / waste are more significant, what are the representative disposed pharmaceutical forms and drug reuse or disposal as waste by recording and analyzing the daily information collected from all drugs returned through the unit dose delivery process from the various inpatient services. This study also aims to promote a multidisciplinary discussion of the problem, defining a set of practical measures, which aim to improve behaviors, procedures and attitudes between all health professionals responsible for handing use drugs, thus adopting a proactive stance in their daily practice in order for safety use of returned drugs and achieving cost savings while maintaining the hospital medication circuit. The second chapter of this report refers to the experience in hospital pharmacy, carried out at the Pharmaceutical Services of the Cova da Beira University Hospital Center, between September and November 2018, under the guidance of Dr. Olímpia Fonseca, which intended to focus on the main work areas or skills pharmacist have as well as the legal aspects intrinsic to the hospital pharmacy practice. The third and final chapter of this report deals with the internship component in community pharmacy, which was held at the Sé pharmacy in Guarda, Portugal under the guidance of Dr. Maria João Grilo and Dr. Cláudia Gomes, this internship was held between November the 2018 and January 2019, accounting for a total of 800 hours. This chapter describes the various responsibilities and activities assigned to the pharmacist in the community context, as well as some particularities related to the legal and financial framework of this aspect. This report also presents the various works presented at congresses and interviews and which aim to discover new perspectives and approaches to this theme.

Full textNos dias que correm, existe cada vez mais a necessidade de melhorar a qualidade dos serviços de saúde. Existem demasiadas lacunas associadas aos níveis de stocks, a ausência de conhecimento dos processos, falhas na dispensação e contabilização dos fármacos. Portanto, existe a necessidade de recorrer a uma filosofia que elimine o desperdício – Lean Thinking. Esta filosofia orientará o desenvolvimento deste projeto, o qual apresenta como principal objetivo a perceção do processo do circuito do medicamento, de modo a conseguir-se reduzir o erro no débito dos fármacos. A origem deste projeto, tem como base a tentativa de melhoria e a obtenção de eficácia nos processos. O estudo incide em dois serviços hospitalares – a Farmácia Hospitalar e o Bloco Operatório Central - do Hospital da CUF Descobertas do grupo José Mello Saúde. Através da filosofia Lean e recorrendo a uma das suas ferramentas – Value Stream Mapping – propõe-se a compreensão de todo o processo logístico associado ao circuito do medicamento. Numa primeira fase será realizado um estudo exaustivo e pormenorizado do circuito do medicamento, através de discussões diárias com os intervenientes do processo e de observações diretas de cirurgias. Após a identificação dos principais desperdícios, são criados e analisados os registos de débitos dos fármacos e posteriormente serão apresentadas propostas de melhoria que mitiguem os desperdícios identificados. A terceira fase consiste na análise e discussão dos resultados, onde será criada a situação futura ideal. Finalmente serão apresentadas diversas melhorias nestes serviços, de modo a que os processos se tornem mais eficientes e eficazes.
In the current days there is an increasing need to improve the quality of healthcare services. There are too many loopholes associated with stock levels, weak interaction between the elements of the process, lack of staff training, lack of knowledge of the processes, failures in dispensing and accounting of medicines. Thus emerges the need to apply a philosophy that eliminates waste, such as the Lean philosophy. The development of this project is based on this philosophy, which has as main objective the analysis of the medicine circuit, so that new methods are introduced in order to reduce the error in accounting for medicines. Focusing on the perspective of improving and attempt to obtain the highest effectiveness of the processes motivated this project to exist. The study focuses on two main hospital services - the Hospital Pharmacy and the Operating Room - Hospital CUF's Descobertas Health Group José de Mello. Using one of the Lean philosophy tools - Value Stream Mapping - will be clearer to understand the whole logistics process associated with the medicines circuit. The project was divided into three distinct phases. Initially there will be a comprehensive and detailed study of the circulation of medicines, through daily discussions with the participant elements of the process and by direct observations of several surgeries. After identifying the main waste causes the data collected will be analysed and based on the information improvement proposals will be suggested and applied to mitigate the identified waste.The third phase consists on a global overview and commentary of the changing procedures, and finally will be created the future ideal state. Lastly, various improvements will be made to these services, so that the process becomes even more efficient and effective.

Full textO cancro tem demonstrado não só um grande impacto na vida global bem como a nível orçamental. Cada vez mais se verifica um aumento da população portadora desta doença bem como dos custos suportados direta e indiretamente com esta patologia. Com o presente estudo pretende-se fazer uma avaliação fármaco económica da evolução dos consumos de citostáticos em regime de hospital de dia no centro oncológico do Centro Hospitalar de Trás-os-Montes e Alto Douro (CHTMAD), EPE, e verificar a evolução de custos antes e após a implementação de uma Unidade Centralizada de Preparação de Citostáticos (UCPC) sendo o principal objetivo analisar o impacto fármaco económico da implementação da UCPC no Centro Oncológico do CHTMAD, EPE. O estudo de caso caracteriza-se como descritivo e retrospetivo, reportando à análise da evolução fármaco económica num período de tempo entre 2006 a 2008 antes da implementação da UCPC e 2009 a 2015 após a implementação da UCPC, na UCPC do CHTMAD, EPE. Inicialmente foi efetuada uma revisão bibliográfica para enquadramento do tema proposto, elaboração da lista dos fármacos estudados, estudo do seu comportamento ao longo dos anos, sua evolução, doentes submetidos aos mesmos, nas diferentes patologias e na implementação de procedimentos para que a preparação seja efetuada com qualidade e de forma rigorosa e a avaliação do desperdício/reutilização realizada na UCPC. A partir desta análise constata-se que, com a implementação da UCPC, houve um aumento dos custos associados à quimioterapia, mais doentes tratados e introdução de novos fármacos inovadores para as diferentes patologias, nomeadamente linfomas, cancro mama e colon retal. O custo médio por doente diminuiu e a reutilização teve um saldo positivo face ao desperdício.
Cancer has shown not only a major impact on overall life as well as on the budget. Increasingly, there is an increase in the population carrying this disease as well as the costs incurred directly and indirectly with this pathology. The present study intends to make an economic evaluation of the evolution of the consumption of cytostatics in a day-care hospital at the Trás-os-Montes and Alto Douro Hospital Center (CHTMAD), EPE and verify the evolution of costs before and after the implementation of a Centralized Unit of Preparation of Cystostatics (UCPC) being the main objective to analyze the economic drug impact of the implementation of the UCPC in the Cancer Center of the CHTMAD, EPE. The case study is characterized as descriptive and retrospective, reporting to the analysis of the economic drug evolution in a period of time between 2006 and 2008 before the implementation of the UCPC and 2009 to 2015 after the implementation of the UCPC of CHTMAD, EPE. Initially a bibliographic review was done to frame the proposed theme, elaboration of the list of drugs studied, study of their behavior over the years, their evolution, patients submitted to them, in the different pathologies and in the implementation of procedures so that the preparation is carried out with quality and in a rigorous way and evaluation of the waste / reuse performed in the UCPC. From this analysis we can see that with the implementation of the UCPC there was an increase in the costs associated with chemotherapy, more patients treated and introduction of new innovative drugs for the different pathologies, namely lymphomas, breast cancer and rectal colon. The average cost per patient decreased and re-use had a positive balance on waste.
