Abstract:
The diagnostic criteria for posttraumatic stress disorder (PTSD) have evolved substantially since the disorder was introduced in DSM-III. The latest revision for DSM-5 involved several notable changes, including moving PTSD to a new chapter of trauma- and stressor-related disorders, reconceptualizing the stressor criterion, expanding from three to four symptom clusters, adding three new symptoms and revising others, adding a dissociative subtype, and creating separate criteria for children six years and younger. This chapter traces the evolution of the PTSD criteria and describes the DSM-5 cri

Central nervous system (CNS) lymphoma must be considered in the differential diagnosis of any immunocompromised patient with a solid brain lesion. In such patients, diagnosis can be made via a careful review of important signs, symptoms, and classic radiologic findings. While there is no single physical exam finding classic for lymphoma, the clinician must carefully evaluate patients for the presence or absence of findings that may suggest an alternative diagnosis. Such findings include the stigmata of endocarditis, symptoms suggestive of pneumonia, or additional non-CNS mass lesions. Addition

Guillain-Barré syndrome is the prototype of acute immune-mediated neuropathies. Guillain-Barré syndrome has several subtypes including acute inflammatory demyelinating polyneuropathy, acute motor axonal neuropathy, and acute motor sensory axonal neuropathy. Guillain-Barré syndrome has also several variants including Miller Fisher syndrome, ataxic form, and pharyngeal–cervical–brachial form. This case highlights the clinical findings in Guillain-Barré syndrome and discusses in details the diagnostic criteria that are essential in confirming the diagnosis and excluding mimickers of the disorder.

This chapter aims to highlight the diagnostic and prognostic value of cerebrospinal fluid (CSF) findings in multiple sclerosis with a special consideration of distinguishing it from neuromyelitis optica (NMO) and NMO spectrum disorder. Interpretation of CSF findings in daily clinical practice in patients with MS is thoroughly explained. New advances in CSF analysis and recently identified biomarkers may be helpful for diagnosis, help elucidate disease subtype and activity, or aid in prognosis and monitoring of the response to treatment. This chapter focuses on the role of echocardiography in dilated cardiomyopathy, showing diagnostic and associated findings along with the prognostic role of echocardiography. Primary myocardial disease is inadequate hypertrophy, independent of loading conditions and often other affected structures such as mitral valve apparatus, small coronary arteries, and cardiac interstitium. Arrhythmogenic RV cardiomyopathy is fatty or fibro-fatty infiltration of the RV with apoptosis and hypertrophied trabeculae of the RV. Myasthenia gravis is a relatively common neuromuscular junction disorder. The diagnosis of myasthenia gravis may be challenging particularly in seronegative patients. This case starts by discussing the diagnostic modalities available to confirm the diagnosis of myasthenia gravis including serum antibodies, the Tensilon test, and the ice pack test. After a detailed discussion of the physiology of neuromuscular transmission, the case emphasizes the role of electrodiagnostic studies in the diagnosis of myasthenic gravis. The patient’s history and exam findings can inform a preliminary differential diagnosis, which may be broadly divided into benign and malignant processes. Cervical facet dysfunction poses a diagnostic and therapeutic dilemma in patients with axial neck pain due to either degenerative changes or whiplash injuries as it presents with a paucity of diagnostic radiologic or examination findings. The specific orientation of the cervical facet joints renders them particularly vulnerable to whiplash injury. This chapter examines the clinically relevant anatomy with nuances unique to the cervical spine, etiology of the structural changes, diagnostic tools, and treatment of cervical facet dysfunction.
