Introduction: the problem of diagnostics of the quality of education at a higher education institution is becoming increasingly relevant in the conditions of the need to fulfill the criteria for the effectiveness of higher education institutions, the introduction of new generation standards in higher education institutions and the modernization of the educational program management system. Diagnostics of the quality of education is considered as an important process that ensures a steady position of the university in the educational services market, the growth of indicators for the admission campaign, the increase of reputational capital. Diagnostics of the quality of education makes it possible to identify significant factors in determining the prospects for the development of a university, having a significant impact on the performance indicators of a university and characterizing the results achieved and the resources used.Materials and Methods:the article discusses the system for the quality of education evaluation at Minin University, it describes its key components, reveals the elements of the diagnostic complex and their systemic interconnection. The characteristics of the elements of the diagnostic complex describes in detail the issue of improving the efficiency of the university in terms of the implementation of the management model of the main professional educational programs, the description of the diagnostic procedure for each category of participants in the educational process is presented.Results:the author describes the methodology and the main stages of diagnosing the quality of education at the university using the example of Minin University, gives integrated assessment criteria and their relationship with other elements of the monitoring system and evaluating the quality of education at the university, lists the effects of introducing a comprehensive system for diagnosing the quality of educational activities.Discussion and Conclusions:in the final part of the article, the possibilities of further improving the system of assessment of the quality of education in the educational system of higher education are considered. Arguments about the need to further expand the range of issues raised by the diagnostic process are made; the ratio of the existing positions of the university to the requirements of the legislation is made.

Abstract:
Purpose The frequency of diagnostic error in patients who have a lung mass and a pathology specimen is as high as 15%. This study examined the role of inter-pathologist agreement in identifying the cause of error in these patients. Methods Pathologists from six institutions reviewed the slides of 40 patients who had a pulmonary specimen false-negative diagnosis. The initial assessment of error cause arose from cytologic-histologic correlation slide review of discrepant diagnostic samples in patients who had both a bronchial brushing cytologic and surgical specimen. The cause of error was attributed either to clinical sampling (diagnostic material obtained in one but not the other sample) or interpretation (pathologist failed to identify the salient diagnostic features). The pairwise kappa (κ) statistic was used to calculate interobserver agreement between the review and original diagnoses and between the separate review diagnoses. Results The pairwise κ statistic ranged widely from −0.154 to 1.0, and the pairwise κ statistic of the slides from one institution was undetermined because that institutional pathologist never made the assessment that error was secondary to interpretation. Agreement for observers within the same institution was better than agreement between observers from different institutions. Conclusion Pathologists exhibit poor agreement in determining the cause of error for pulmonary specimens sent for cancer diagnosis. We developed a psychosocial hypothesis (the “Big Dog” Effect) that partially explains biases in error assessment. This lack of agreement precludes confident targeting of these errors for quality improvement interventions with prospects of success across a variety of institutions.

Majority of health institutions in Croatia do not have medical physicists in diagnostic radiology. Regarding this, in the west region of Croatia collaboration between public health institution and University Hospital Rijeka was initiated in 2015. Quality Assurance program was implemented in these public health institutions during 2015 and 2016 and the next step was to assess patient doses for the most frequent X-ray examinations. This included five public health institutions: 1 university hospital, 1 general hospital, 1 special hospital, 2 public health institutions with 13 facilities. The aim of this study was to carry out assessment of patient doses and to establish local diagnostic reference levels of entrance surface air kerma for every institution for six most frequent X-ray examinations. Also, local diagnostic reference levels for the whole west region of Croatia were established and compared with the national diagnostic reference levels and latest published data. Median entrance surface air kerma values for thorax PA, thorax LAT, cervical spine AP, thoracic spine AP, lumbar spine AP, pelvis AP and sinuses are 0.14 mGy, 0.50 mGy, 0.52 mGy, 1.50 mGy, 2.52 mGy, 2.03 mGy, and 1.03 mGy, respectively. Diagnostic reference levels proposed for our region were comparable with other studies.

Abstract Abstract 4272 Introduction: Demand for blood components in Canada is a significant and growing contributor to health care spending, but the drivers of this demand remain poorly characterized. While most institutions do not routinely monitor the transfusion requirements of individual diagnoses and procedures, an institution's overall demand for blood components and the types of clinical activities it provides are both well documented. Development of an institution-level inferential model may allow for more accurate demand forecasting for blood components. Methods: Hospital shipments of all blood components from Canadian Blood Services (red blood cells, platelets, plasma and cryoprecipitate) in the province of Ontario during the period 2006–2009 were merged with institution-level administrative data containing hospital characteristics and selected diagnostic and procedural codes as obtained from inpatient and ambulatory clinical databases maintained by the Canadian Institute for Health Information. Simple linear correlation and several nested multivariable linear regression models were fitted and compared. Results: From 2006–2009, our sample included 137 healthcare facilities, representing approximately 1.4 million units of RBCs, 400 000 units of plasma, 166 000 doses of platelets (defined as either one pool of whole blood-derived platelets or a single apheresis platelet), and 20 000 units of unpooled cryoprecipitate. Institutional demand for these blood components correlated with both hospital-level characteristics and diagnostic and procedural activity data. Institutional demand for red blood cells correlated most strongly with visits for cirrhosis and chemotherapy. Demand for plasma correlated most strongly with visits for cirrhosis and cardiac surgery procedures. Demand for platelets correlated most strongly with bone marrow and solid organ transplant procedures. Demand for cryoprecipitate correlated most strongly with visits for valvular heart disease and cardiac procedures. The strength of these correlations generally remained stable over the four years of analysis, during which time demand for red blood cells and platelets increased, demand for plasma decreased, and demand for cryoprecipitate remained stable. Conclusions: In the largest province of Canada, institutional demand for blood components correlate strongly with disease burden measured by diagnostic and procedural codes contained within administrative data. For nine weeks during the 1866 cholera epidemic, the registrar general for England and Wales published details of more than 13,000 deaths in London. Although the names of the deceased and the informant were withheld, all other information available from the death certificate was reproduced in the capital city's Weekly Returns, including registration district and subdistrict, precise address (house number and street, or institution), sex, age (sometimes down to hours for infants), occupation, cause(s) of death, and duration of final illness. Since historians’ access to original death certificates in England and Wales is restricted, this source presents an opportunity to analyze systematically the practice of cause of death certification in the middle of the nineteenth century, albeit during a period of mortality crisis. Variability of diagnostic “depth”—that is, the listing of multiple causes and duration of final illness—is considered for three major causes: cholera, diarrhea, and respiratory tuberculosis. Deaths in workhouses and general hospitals were chronically underdocumented compared to home deaths. This finding supports the notion that the institutionalization of sickness in the nineteenth century was accompanied by a loss of the “patient narrative” and also points to the entrenchment of institutional cultures of record keeping and administration.

Abstract:
Context.—Fine-needle aspiration (FNA) biopsies have been an important component in the preoperative evaluation of thyroid nodules. Until the introduction of the Bethesda System for Reporting Thyroid Cytopathology (BSRTC) in 2008, individual institutions had developed their own diagnostic categories. The BSRTC proposed 6 categories in an attempt to standardize reporting of thyroid FNA. Objective.—To present a 15-year experience of thyroid FNA at one institution, including data before and after introduction of the BSRTC. The risk of malignancy is compared with the meta-analysis of high-quality published data. Data Sources.—Data sources were PubMed, a manual search of references, and institutional data. Conclusions.—The diagnostic categories developed at our institution were similar to those proposed by the BSRTC, with best fit into the 6 categories easily accomplished and reported in the final 2 years of the study. Significant differences were noted in the frequencies of cases in diagnostic categories Benign (II; P = .003), Suspicious for follicular neoplasm/Follicular neoplasm (IV; P < .001), and Malignant (VI; P = .003) after the introduction of the BSRTC. Eighteen published articles met the criteria for inclusion in the meta-analysis. The risk of malignancy in each category in our institution was similar to that determined in the meta-analysis, except for Insufficient for diagnosis (I; 20% versus 9%–14%). Meta-analysis showed an overlapping 95% CI of risk of malignancy between Atypia of undetermined significance/Follicular lesion of undetermined significance (III; 11%–23%) and Suspicious for follicular neoplasm/Follicular neoplasm (IV; 20%–29%), suggesting similar risks of malignancy. The use of newer molecular tests for these indeterminate cases may further refine risk assessment.

Study rationale The incidence, location, morphology and size characteristics of cerebral aneurysms in patients presenting to Groote Schuur hospital with either subarachnoid haemorrhage or 3 rd nerve palsy have not been established by a formal audit. Objectives To determine the patient demographics, frequency of CT angiographic detection of cerebral aneurysms and aneurysmal characteristics in patients presenting to Groote Schuur Hospital with sub-arachnoid haemorrhage and /or 3 rd nerve palsy Materials and methods Computed tomographic angiographic reports of cerebral vessels of patients who presented either with subarachnoid haemorrhage or 3 rd nerve palsy to Groote Schuur hospital were reviewed over a 19-month period from January 2018 – July 2019. The data obtained were coded, entered and analysed using IBM SPSS version 25 software. Descriptive statistics was used to report the means, modes and frequencies. Demographic and aneurysmal data were compared with a similar period 5 years previously. Results One hundred and twenty-one aneurysms (121) were analysed in 2018 -2019 and 124 in 2013-2014. The large majority were solitary (92% in both groups), small (94% and 90%) and saccular (96% and 87%) respectively. Significantly more fusiform aneurysms (13% vs 6%) were seen in the earlier group. 8 % of patients had multiple aneurysms. Less than 1% were ‘giant' ( >20mm). The mean age of the patients was the same for both periods (47 years). The mean aneurysm body size was 5.7mm and 7.1mm and the mean body: neck ratio was 6 2.1 vs 1.8b). Themost frequent locations were the posterior communicating artery (31.4% [2018/2019], 35% [2013- 2014]), anterior communicating artery (29% [2018/2019], 18.5% [2013/2014]) and the middle Cerebral Arteries (13.2% [2018/2019], 13.7% [2013/2014]). The least common sites were the Superior Cerebellar artery (SCA) [2018/2019] and the Vertebral artery (0.8%) [2013/2014]. Conclusion This study has compared the demographics of patients presenting to Groote Schuur Hospital with CT angiographically confirmed symptomatic intracranial aneurysms over two periods (January to July) 5 years apart. Both the patient demographics and the aneurysmal architecture were consistent over these time periods. Further our findings conform to that described previously both in Southern Africa and abroad i.e aneurysms which have bled are most commonly related to the posterior communicating, anterior communicating and the middle cerebral arteries and most aneurysms are small and saccular in shape. Over the periods studied, there was no change in the number of patients presenting to Groote Schuur Hospital for CT cerebral angiography and Interventional treatment post aneurysm rupture. These data represent a baseline for future statistical comparison and the information extrapolated from this study will be useful for interventive planning and resource mobilization at our institution and within the Western Cape Department of Health.

Cette thèse étudie le phénomène d’institutionnalisation des pratiques organisationnelles avec pour terrain d’observation le diagnostic en psychiatrie. Fondamentalement attaché à l’organisation comme objet d’étude, nous chercherons à répondre à une question de recherche qui y est profondément ancrée : Quels sont les phénomènes organisationnels qui participent de l’institutionnalisation des pratiques au quotidien ? Notre travail aura alors pour ambition de se confronter à trois difficultés principales : théorique, épistémologique et empirique. Dans un premier temps, il s’agira de clarifier conceptuellement la notion de pratique organisationnelle. En effet, si le practice turn est maintenant déjà bien engagé en Sciences de gestion, nous sommes encore loin d’une théorie qui, sans être unifiée, aurait au moins posé une définition claire de son concept central. Nous proposerons une revue de littérature problématisée des travaux portant sur la pratique qui posera l’institutionnalisation comme leur acte de naissance. Cela débouchera sur une grille de lecture de ces manières de faire instituées que sont les pratiques. Dans un deuxième temps, il nous faudra nous confronter à une difficulté épistémologique. La pratique, objet d’étude classique en anthropologie comme en sociologie, est un élément de la vie sociale qu’il est complexe de saisir au travers des catégories théoriques classiques. Il faudra interroger plus encore la notion de pratique pour comprendre comment sortir de l’impasse épistémologique que constitue le projet de construction d’une théorie de la pratique. Nous verrons en quoi c’est une théorie de la logique pratique qui sera notre véritable projet, et plus particulièrement l’origine historique de cette pratique et sa transmission quotidienne. Dans un troisième temps, il s’agira d’aborder le terrain d’observation fort de toutes ces exigences. Au-delà des difficultés classiques de l’étude empirique, c’est la restitution d’une compréhension pratique fine, fruit de plusieurs mois d’immersion, qu’il faudra être capable de transmettre. Afin de laisser au terrain son foisonnement et de ne pas le réduire à un ensemble d’observations directement capturées dans un cadre théorique, nous ferons le choix d’une synthèse scénarisée qui condensera quatre semaines de la vie du service. C’est une solution stylistique qui nous permettra de rendre compte d’un terrain débordant toujours le prisme particulier qui est le nôtre : l’institutionnalisation des pratiques organisationnelles
This PhD thesis analyses the institutionalization of organizational practices with the psychiatric diagnostic as a field of study. Fundamentally attached to the organization as an empirical object, we will try to answer a research question deeply rooted in it: what are the organizational phenomenons involved in the daily institutionalization of practices? Our work will tackle three mains issues: theoretical, epistemological and empirical.In a first time, we will clarify the concept of organizational practice. If the practice turn is now well engaged in Organization Studies, scholars are still far from a theory that would have given a clear definition of its central concept. We will provide a review of the literature on practice that considers the process of institutionalization as its birth. This will lead to a grid analyzing the multiple institutionalized ways of doing that are practices.In a second time, we will address an epistemological issue. Practices, classical empirical objects in sociology and anthropology, are central elements of the social life that cannot be easily captured through classical theoretical categories. We will have to go further in the concept in order to get out of the epistemological dead-end that the elaboration of a theory of practice is. In fact, it is a theory of practical logic that will be our real project, and precisely, the historical origins of this practice and its daily transmission.In a third time, we will face our field study loaded with these requirements. Behind classical methodological difficulties, it is the restitution of a fine-grained practical understanding produced by several months of immersion that will be challenging. In order to respect the field’s abundance and to not reduce it to a series of observations seen through a theoretical lens, we choose to present a scenarized synthesis that will condense our six-months study into a four-weeks story of a psychiatric department in a teaching hospital. This stylistic solution will allow us to account for a field that always overwhelms our perspective: the institutionalization of organizational practices

Das vorliegende Buch hat die Analyse des funktionsgestörten Wassersektors auf den beiden Zykladen Inseln Tinos und Syros in Griechenland zum Ziel. Die Bevölkerung dieser Inseln leidet unter einer Kombination aus mangelnder Wasserqualität, -quantität und hohen Wasserpreisen. Die Arbeit setzt sich mit Gründen der Ineffektivität zentral geplanter Maßnahmen (technische Lösungen und administrative Reformen) bei der Lösung solcher Probleme auseinander. Die übergeordnete Forschungsfrage hierbei lautet: Warum ist der Wassersektor angesichts einer Vielzahl von Lösungsversuchen immer noch dysfunktional? Um diese Frage zu beantworten, liegt der Fokus auf der institutionellen statt auf der physikalischen Dimension des Problems. Hierbei verfolgte ich eine abduktive Strategie innerhalb des Forschungsprozesses. Auf institutionentheoretischen Annahmen basierend, komme ich zu zwei Schlussfolgerungen, die einen Teufelskreis vermuten lassen: i) Institutionen und Politik haben zu einer chronischen Dysfunktionalität, die sich in einer unsteten und unsicheren Wasserversorgung äußert, geführt. ii) Diese Dysfunktionalität verhindert eine Veränderung der existierenden Institutionen. Basierend auf meiner Analyse schlussfolgere ich, dass die Top-down-Strukturen und die zentral gesteuerten Regularien die Hauptursache sind, dass es zu keiner vernünftigen Veränderung oder gar einer Lösung der Probleme im Wassersektor kommt. Mehr noch, sie sind die eigentlichen Gründe für die Misere und deren Persistenz. Eine Dezentralisierung der Steuerung und eine Unabhängigkeit der lokalen politischen Ebene (institutionell und finanziell) stellen einen vielversprechenden Ansatz zur Verbesserung der Situation dar. Beamte vor Ort kennen die örtlichen Gegebenheiten und fühlen sich verantwortlich für die lokale Bevölkerung.
This study aims at diagnosing the dysfunctional water sector of Tinos and Syros, islands in the Cyclades island complex of Greece. At present, these islands suffer from insufficient water quantity, combined with poor quality and high cost. The research problem addressed in this thesis is the apparent inability of centrally planned technical and administrative-reform solutions that have been promoted to rectify the problems. This work explores the reasons for the inability to rectify the water sector and aims to answer the overarching research question: Why is the water sector in the Cyclades still dysfunctional, despite the attempted solutions? To explore these emergent questions related to policy making, I revisit existing legal rules and relations relevant to the context, viewed in parallel with aspects of human behaviour and transactions under the analytical lens classical institutional economics offers. The reasons for the water-supply dysfunction in the study islands are, thus, sought after in terms of transactions amongst actors, working rules regulating those transactions, and officials’ remedial power at all levels. Based on my analysis, I conclude that top-down structures and centrally planned policies have been hindering the meaningful change and rectification of the water sector and have, in fact, created and prolonged the dysfunction. Decentralising governance, granting local-level autonomy (institutional and financial) is a plausible way of rectifying the water sector, as local officials are knowledgeable about local conditions and feel liable to their fellow citizens, who would willingly cooperate with this level of government.

Paradoxalement, l’avènement du protectorat français au Maroc qui en soit, reste une période coloniale dans l'histoire du peuple marocain, a été marquée entre autre, par l’établissement de la première législation s'appliquant aux sources d’énergie ainsi que par l'introduction de l’électricité et la mise en valeur des sources d’énergie classiques (charbon et pétrole) avec les institutions spécifiques s'y rapportant. En outre, avec le protectorat français s'introduit au Maroc le droit administratif autonome, distinct du droit privé. Car le droit musulman, qui régissait le Maroc pré-colonial, ne connaît pas de droit administratif conçu comme un système d'autonomisation et d'auto-limitation du pouvoir étatique. Ce droit administratif s'est traduit naturellement par la reconnaissance de privilèges importants au profit de l'administration coloniale. En matière d’énergie et afin de mener à bien les activités "d’intérêt général" celle-ci a confié certaines taches qui sont normalement les siennes à des personnes privées (concernant la recherche et l'exploitation des minerais énergétiques). En 1956 date de l’indépendance du Maroc, cet héritage institutionnel et technique est revenu à la monarchie marocaine. Dans un premier temps afin de procéder à ces vastes réformes, adaptées à la situation nouvelle issue de l’indépendance, les autorités nouvelles ont manifesté -du moins en apparence- leur volonté de donner au droit administratif ses caractères les plus évolués, c'est à dire d’établir un droit conciliant l’autorité et la liberté : un droit consacrant la supériorité de l’intérêt général sur les intérêts particuliers et reposant sur l’idée de justice et sa traduction : le principe de l’égalité. Ces idées de justice et d’égalité qui dominent le droit français ont-elles reçu une consécration solennelle dans les textes constitutionnels qui régissent l'ordre juridique de la monarchie marocaine en général et dans le droit administratif, en particulier dans le domaine de l’énergie et de ses institutions ?
Paradoxically enough, the period of the french protectorate in Morocco, in itself a colonial era was marked, among other things, by the introduction of the first legislation on sources of energy, the use of electricity and the exploitation of more traditional sources of energy, such as coal and oil, together with their specific institution. Moreover, the french protectorate introduced an independent administrative law, distinct from private law, since muslim law, which prevailed before colonisation, includes no administrative law conceived as granting independence to and limiting the power of the state. This administrative law naturally confered great privileges to the colonial administration. As far as energy was concerned, and with a view to carrying out "general interest" activities, it entrusted private persons with certain tasks it would normally have carried out itself (concerning the search for and exploitation of mineral energy sources). In 1956, when Morocco became independent, this institutional and technical heritage was passed on to the moroccan monarchy. At first, as they wanted to undertake vast reforms adapted to the new postcolonial situation, the new authorities showed-at least apparently- willing to endow administrative law with its most advanced characteristics : a balance between authority and freedom, an affirmation of the superiority of public interest over private ones. In other words, a law to be grounded on the idea of justice expressed in the principle of equality. Have these ideas of justice and equality which prevail in french law been solemnly established in the constitutional texts which govern the laws of the moroccan monarchy at large and in the administrative law, concerning more particularly energy and its institutions ? It is therefore legitimate to ponder over the development given to the technical and institutional contributions which the protectorate left to the monarchy. Has this legacy been modified ? Do moroccan authorities content themselves with administering the space marked out by the colonial legislation or did they include other new spaces ? The analysis of the present situation reveals an inadequate energy policy -or rather an absence of policy- which was limited to administering and maintaining the colonial legacy. Any improvement of this policy requires that a new energy policy be worked out and implemented within a more general strategy of development fulfilling the basic needs of human beings in the fields of education, housing, health

Dans le domaine du retard mental profond de l'adulte, les perspectives nouvelles ouvertes par l'augmentation de la durée de la vie et l'amélioration des conditions de prise en charge autorisent aujourd'hui une approche évolutive de cette population, en rattachant sa situation présente à une histoire clinique passée. Parmi les indicateurs qui témoignent de ce déroulement historique, la formulation du diagnostic psychopathologique peut constituer un outil original de repérage des phénomènes évolutifs. L'étude catamnestique d'un échantillon représentatif de la diversité des étiologies du retard mental profond permet de mettre en évidence des typologies de fluctuations diagnostiques s'organisant selon trois profils : profil stable, de type "organogenèse", profil alternant pole déficitaire - pole psychotique, profil incertain à hypothèses variées. Dans le contexte institutionnel, qui apparait comme un facteur activateur du questionnement diagnostique, on observe deux tendances complémentaires : le regain d'intérêt pour les hypothèses étiologiques organiques chez des sujets que l'incertitude diagnostique orientait auparavant vers une causalité psychogénétique, et la tonalité pluri-axiale des évaluations récentes, conforme à l'évolution des classifications nosographiques. Une tentative de mise en relation entre modifications du diagnostic, variations du discours soignant et modalités de prise en charge suggère un lien entre incertitude diagnostique et dramatisation de l'intervention. Elle identifie, par ailleurs, les caractéristiques du rapport imaginaire qui lie le soignant de proximité au diagnostic et des représentations fortes qui guident son action. L'intérêt pratique de cette démarche valorise la systématisation des profils d'évolution diagnostique comme outil pertinent d'appréciation psychopathologique, au regard des limites des repérés psychométriques.

Tyrimo rezultatai taikytini organizuojant seminarus ikimokyklinio ugdymo įstaigų vadovams bei vykdant ikimokyklinio ugdymo įstaigų vadovų atrankos ir vertinimo procesus.
An objective, which is formulated in the master’s work is to diagnose the managerial competences of preschool education institutions of Šiauliai city and to reveal the attitude of managers and educators of preschool education institutions towards the managerial competences. The conception of competence was made with the help of theoretical resources. Lithuania’s and other countries’ scientists’ attitudes towards the personal, social, professional and communicative competences were analysed and systematized. The connection between a manager’s work and a competence was revealed. Respondents’ opinion about the managers’ competence, which they have attained and the competence that managers lack mostly were investigated. It was cleared which competences are most important in a manager’s work. Research results can be applied while organising seminars for the managers of preschool education institutions, also while realizing the processes of their selection and evaluation.

AbstractThe leading scientists debating climate change increasingly view the relationship between knowledge and governance as an “inconvenient democracy.” On the one hand, the discrepancy between the knowledge of climate change and citizens’ commitments to behavioral changes amounts to the diagnosis of an “inconvenient mind”; on the other hand, the inertia of policies to capture progress in knowledge leads to the diagnosis of “inconvenient institutions.” The sense of political ineffectiveness felt especially among climate scientists provokes a strong disenchantment with democratic governance. As a result, some scientists propose that political action based on principles of democratic governance be abandoned. In my article, I argue that such a view is mistaken.

The high number of exams that is done in healthcare institutions increases the medical doctors’ workload, leading to poor working conditions and the increase of wrong diagnoses. As consequence, an automatic system that can help medical doctors in diagnostic tasks is of major interest to any healthcare institution. The chapter proposes an Image Based Classification Platform suitable to help Medical Doctors diagnosing breast cancer, based on mammograms, i.e., to detect if a tumor is present in the image. The platform is twofold, i.e., in the first part the image descriptors are extracted from the image using image-processing algorithms. The obtained descriptors are used in the second part. The second part is related to classification, where computational intelligence methods are used to classify a given image, based on the descriptors obtained in the first phase. Texture analysis based on co-occurrence matrices are applied to obtain the descriptors from the MIAS database of mammograms. From these descriptors, fuzzy models, neural networks, and support vector machines are successfully used to classify the mammograms and obtain a diagnosis.

The writings of two classic thinkers in psychiatry in the 19th and 20th centuries, Emil Kraepelin and Aubrey Lewis, are provided and examined for insights they provided into continuing problems in the diagnostic and treatment of psychiatric conditions today. Kraepelin was the famed great late 19th-century psychiatric leader from Germany who identified the basic distinction between schizophrenia (dementia praecox) and manic-depressive illness. He laid the foundations of much of psychiatric diagnosis that remains relevant today, and he was a committed defender of the biological approach to psychiatry, although he was conservative with the use of drugs, which were ineffective in his day. Lewis (1900–1975) was the most prominent figure in British psychiatry through most of the 20th century. He was the leader of the Institute of Psychiatry at the Maudsley Hospital for much of the middle of the 20th century. That institution in London was the most influential educational center for psychiatry in the nation. Through his leadership there, Lewis was extremely influential. He tended to be skeptical about the use of psychotropic medications, and emphasized social aspects of psychiatric illness.

Aims and Objectives: This study is done to assess the accuracy of intraoperative frozen section in the diagnosis of various categories of ovarian neoplasm conducted in RGCI. Materials and Methods: Introperative frozen sections for suspected ovarian neoplasm that underwent surgery as primary line of therapy at this institution were analyzed retrospectively from Jan. 2014 - Dec. 2015. The results of frozen section were compared with the final histopathologic diagnosis on paraffin sections and the overall accuracy, sensitivity, specificity, positive and negative predictive values were determined. Results: The study included 159 cases and the mean age of patients was 44.72 ± 14.28 years (Range 19-75 years). The mean size of tumor was 12.5 ± 5.9 cm. Sensitivity of frozen section for benign, borderline and malignant tumors was 98.53%, 73.33% and 94.74% respectively. And the related specificities were 95.60%, 96.53% and 100% respectively. There were 150 concordant cases and 9 discordant cases. Overall diagnostic accuracy of frozen section was 94.33%. Conclusion: Intraoperative frozen section diagnosis appears to be an accurate and comparable technique for the histopathology diagnosis of ovarian tumours. It is a valuable tool to guide the surgical management of these patients.

The current tendency of the European educational strategy on the compulsory children’s involvement in the preschool education before elementary school raises the issue of preparing children for school and the preschool education functioning, modernization and updating the content and technology of pedagogical activities at this stage. The article highlights the innovation of the Polish theory and practice in preparing a child for school i.e. the transfer of the pedagogical diagnosis function in school readiness from psychologists to preschool teachers. The organization of systematic, daily observation and interpretation of children's activities has become an integral part of the teacher's work. The essence of systematic pedagogical diagnostics of school readiness in Polish preschool institutions is revealed. On the basis of retrospective analysis, it is highlighted the evolutionary development in Polish pedagogy of the diagnosing children's readiness problem in school. According to the chronological principle, diagnostic methods are systematized; their semantic and procedural aspects are characterized. It is substantiated that as a result of diagnostic techniques, mastering the tools and ability to perform diagnostic procedures, the teacher gets the opportunity to determine the degree of mental and social development of the child, which contributes to school tasks, as well as factors that determine this development. The teacher forms a complex child’s profileconcerningthe knowledge and competencies, and on its basis develops a strategy of pedagogical influence and interaction with the child at the beginning of school.

A Rede Neural Convolucional foi então utilizada para detectar padroes visuais das imagens da base de dados Warsaw BioBase Disease Iris v2.1, que posteriormente foram classificadas entre doentes e saudáveis. O classificador construído foi avaliado seguindo o protocolo de experimentação Holdout, obtendo uma taxa de acerto de 94%. Mediante tal resultado, é possível concluir que o modelo desenvolvido tem potencial para se tornar uma ferramenta para o auxílio ao diagnostico.

Abstract:
Today all over the world costs of medical services are growing and alternative ways of effective financing of health care are being researched. During the reforms the Kyrgyz Republic introduced a system of compulsory medical insurance, the institution of family medicine and a "single payer" system. Methods of payment for hospital services flush to an artificial increase in the number of hospitalizations and unnecessary assignment of diagnostic and therapeutic procedures. The main brake of health care reform is underfunding of sector. Improving health care is possible by limiting the free medical care. The replacement of free care by paid services occurs spontaneously, there are abuses and the shadow economy in health care. The Compulsory medical insurance doesn’t have such terms as an accident, insurance risk, and the current model in Kyrgyzstan is not a real model of insurance and serves as a kind of state-funding health care. The most part of the population in rural areas is not involved in the payment of health insurance due to unemployment. Patients pay a fee in addition to medication, and also carry out informal payments to doctors, that is, patient with co-payments have to repeatedly pay for the same medical service without a guarantee of a cure. Taking into account the experience of other countries, the imposition of patient payment for their own care is more just to bringing the patient for his treatment.

Currently, the diagnosis and monitoring of people affected by neurodegenerative diseases are made subjectively. Parkinson's disease (PD), for example, is a neurodegenerative disease that affects the motor part of the individual. The evaluation of PD usually is done through scales based on questionnaires. Thus, this research is dedicated to the study and development of a computational system capable of performing the collection, transmission and visual presentation of data related to the human movement, in order to assist the diagnosis and monitoring of PD.

A Inteligencia Artificial (IA) como ferramenta de apoio a disgnósticos especializados mostra-se bastante promissora. Dentro dessa perspectiva sabe-se que a esquizofrenia é um distúrbio psiquiátrico de difícil diagnostico. Nesse sentido este trabalho apresenta um sistema fuzzy cujo objetivo é auxiliar profissionais na avaliação comportamental de pacientes por meio da triagem de características que enseje no fechamento de diagnósticos. A metodologia foi realizada com base em simulações onde os sintomas como delírios, alucinações, discurso desorganizado entre outros foram utilizados como variáveis de entrada. Com base nos resultados pode-se concluir que a IA tem muito a contribuir na busca por soluções na área medica em especial no auxílio de diagnósticos.

Intro: Undifferentiated dyspnea can be a complicated presentation muddled by patient comorbidities and similar symptomology shared among etiologies. Some studies have shown increased mortality and length of stay in the hospital when incorrectly initially diagnosed in the ED. US has been shown more effective at differentiating these causes and improves diagnostic accuracy. This study will implement US exam upon initial exam of patient and chart time to diagnosis/treatment, length of stay in ED, length of stay in hospital admissions versus discharge rates, and 30 day mortality. ADHF and COPD/asthma patient differentiation will be the focus. Methods: Prospective cohort study of more than 18 years that present with the primary complaint of dyspnea with more than one complicating comorbid condition. Initial exam by physician will be accompanied by cardiothoracic US previously verified. Results: Study powered by previous year average of time to diagnosis of institution. Patient characteristics, distribution by diagnostic category, and characteristics found on US in correlation with diagnosis will be included for multivariate analysis. Conclusions: We expect to see a singificant difference in our time to diagnosis/treatment and mortality rate.

For most Caribbean countries, the COVID-19 pandemic will translate into the deepest single-year contraction of real GDP on record in 2020. With the exception of Guyana, countries have experienced deep recessions, severe increases in unemployment, and long-lasting damage to many corporate and household balance sheets. The social consequences of the crisis continue to mount, and despite governments best efforts to buffer the shock to families, enterprises, and domestic markets, there remains a dire need for continued and more broad-based stimulus to ensure that economic capital both human and other wise remains intact. This edition of the Caribbean Quarterly Bulletin briefly reflects on notable economic developments in 2020, then shifts to longer-term issues, including a summary of an upcoming IDB publication, Economic Institutions for a Resilient Caribbean, as well as summaries of the book's key diagnostics and recommendations for each country. In some cases, country sections focus on specific areas of institutional reforms. For example, the Suriname section focuses on fiscal institutions, given the public debt distress there.

The public and private sector, civil society, and academic institutions have developed many innovative solutions to manage public health aspects of the coronavirus disease (COVID-19) pandemic. Innovators have focused on tools for surveillance, supply chain management, clinical trials, diagnosis, communication, and developing vaccines. These have been supplemented by research collaboration platforms, isolation and hospital upgrading novelties, as well as risk stratification resources. This paper provides an overview of these solutions to enhance the evidence-based application of innovative public health approaches. The author’s also propose that a “living platform” for sharing public health innovations is developed.
