In an original assessment of all three branches, this book reveals a new way in which the American federal system is broken. Turning away from the partisan narratives of everyday politics, the book diagnoses the deeper and bipartisan nature of imbalance of power that undermines public deliberation and accountability, especially on war powers. By focusing on the lawsuits brought by Congressional members that challenge presidential unilateralism, the book provides a new diagnostic lens on the permanent institutional problems that have undermined the separation of powers system in the last five decades, across a diverse array of partisan and policy landscapes. As each chapter demonstrates, member lawsuits are an outlet for frustrated members of both parties who cannot get their House and Senate colleagues to confront overweening presidential action through normal legislative processes. But these lawsuits often backfire—leaving Congress as an institution even more disadvantaged. The book argues these suits are more symptoms of constitutional dysfunction than the cure. It shows federal judges will not and cannot restore the separation of powers system alone. Fifty years of congressional atrophy cannot be reversed in court.

The book considers the value of current classifications of disease, the meaning of diagnostic thresholds, and the potential for overdiagnosis. It examines the role of the patient-centred approach in this context. It develops a description of the diagnostic process, provides illustrative examples and metaphors, and refers to the dual-process model. It suggests that medical training does not consistently provide a coherent account of diagnostic thinking and the associated risks of error. It considers the role of probability in diagnostic reasoning, noting the contribution and the limitations of both informal and mathematical estimates. It refers to clear evidence that error in medical diagnosis is a prevalent and potent cause of harm and may result from systems factors or cognitive glitches such as bias and logical fallacy. It presents cases with commentaries, highlighting the cognitive processes in diagnostic successes, near misses, and disasters. It concludes with proposals for change, notably in institutional culture; in professional culture, education, and training; and in the structure of medical records. The book advocates the development and deployment of computerized diagnostic decision support. It argues that these changes could significantly enhance patient safety.

Tachyarrhythmias occur during approximately 12% of medical and surgical intensive care unit hospitalizations. The haemodynamic, metabolic, autonomic, inflammatory, and pharmacological factors associated with critical illness may act as arrhythmia triggers. The occurrence of new tachyarrhythmia in a critically-ill patient may be associated with an acute decrease in cardiac output, resulting in haemodynamic instability, an event that often results in increased morbidity and mortality. Accurate electrocardiographic arrhythmia diagnosis is necessary for instituting effective therapy. This chapter reviews the various strategies for identifying the causes, and obtain accurate diagnosis for supraventricular and ventricular tachyarrhythmias that commonly occur during critical illness.

President Harry Truman signed the National Heart Act in 1948, which resulted in the creation of the National Heart Institute and started federal funds flowing to academic centers to support cardiovascular research. Mayo cardiologist Arlie Barnes’s term as president of the American Heart Association coincided with its transformation from a low-budget professional society into a large voluntary health organization that raised funds from the public to support its programs. World War II research into shock contributed to the development of cardiac catheterization as a clinical diagnostic tool. Mayo’s wartime research program that focused on ways to protect fighter pilots from blackouts due to high gravitational forces led to the invention of technologies to measure blood pressure and blood oxygen content. Physiologist Earl Wood used these tools in Mayo’s cardiac catheterization laboratory, which was established at the institution in 1947. The clinic helped pioneer the emerging field of cardiac catheterization.

During the 1980s and 1990s, Mayo implemented strategies to confront increasing regional competition. Clinic leaders were concerned about the managed care movement and other factors causing uncertainty about the future of health care. The institution entered new markets in Florida and Arizona. Mayo re-emphasized its commitment to patient care at the same time it devoted additional resources to medical education and research. The Cardiovascular Division sought to enhance its national reputation by sponsoring more continuing medical education programs. To protect referral patterns, it established outreach programs that sent cardiologists and specialized diagnostic technologies (such as echocardiography) to sites in surrounding counties. Mayo has long been a leader in employing state-of-the-art heart care technologies. Increasingly, however, questions about the appropriate use of technologies have been raised. Mayo was an early adopter of the electronic medical record and the notion of empowering patients by providing information in several formats, including social media.

Communicating this information to both the referring physician and often directly to the patient has become increasingly important as the role of radiologists in patient care has evolved. As technology advances, and the field of radiology extends beyond the interpretation of diagnostic imaging into that of intervention and treatment, timely and clear communication of imaging results, limitations of radiology examinations, and the risks associated with image-guided interventional and therapeutic procedures is a priority. Instituting structured reporting, reporting lexicons, and formal communication skills training for radiologists are a few measures that radiologists can take to improve communication in the field. Such efforts to improve communication in radiology are integral components to enhancing and expanding the role of radiologists in patient care.

Echocardiography with its multiple modalities plays a central role in the evaluation of patients with known or suspected coronary artery disease, starting from the differential diagnosis of the patient presenting with acute chest pain. In the patient presenting with acute myocardial infarction (raised troponins) whether it is with ST-segment elevation or without, echocardiography is the first imaging modality used in order to ascertain the presence and extent of LV dysfunction and the presence of complications. In the absence of myocardial infarction (negative troponins), echocardiography will play an important diagnostic role in identifying the presence of reversible myocardial ischaemia. Stress echocardiography in many institutions is now the preferred stress modality associated with imaging as it is cost-effective and does not use ionizing radiation. Finally, echocardiography plays a pivotal role in the assessment of myocardial viability since the presence and extent of viable myocardium may guide therapeutic strategies. It has been stressed that laboratories and individuals need to have experience and be accredited by the authorities so that the results of echocardiographic investigations will be credible.

Abstract:
The chapter presents a sympathetic overview of labour geography in its various and evolving forms. This has mapped the shifting politics of production, together with old and new forms of labour organization; it has problematized the workplace, as a site of struggle and as an arena for the performance of social identities; it has tracked the restructuring of labour markets, as spaces of socio-institutional stress and regulatory transformation; and it has deployed labour as a diagnostic for understanding different (local) varieties of capitalism, economies of care and reproduction, and alternative modes of socio-economic organization. The field has consequently been shaped by an accumulating array of concerns with (industrial) restructuring, (labour) regulation, (union) reorganization, and (social) reproduction.

Abstract:
Although social abnormalities in young children raised in abnormal environments have been described for many years, it was not until the publication of the third edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM) that reactive attachment disorder (RAD) appeared in psychiatric nosologies. The disorder described aberrant social behaviors in young children that were believed to derive from being reared in caregiving environments lacking in species-typical nurturance and stimulation, such as in instances of maltreatment or institutional rearing. Subsequent to the original description of the disorder, criteria in the fifth edition of the DSM have been revised, bifurcating RAD into separate diagnoses. This chapter describes features, characteristics, and treatment considerations for RAD and disinhibited social engagement disorder.

Is it wrong for prospective parents not to select against having children with disabilities because these children impose substantial costs on society? Recent writers such as Stephen John have argued that the state should fund prenatal diagnosis based on expected cost savings and that parents who have had a reasonable opportunity to avoid having a child with a disability should bear some of the additional costs of raising the child. This chapter offers three responses to these social cost arguments: claims about increased costs and reduced productivity are greatly exaggerated, modern states subsidize many similar costs of childrearing and do not attempt to limit the number of children per family to reduce these costs, and luck-egalitarian theories of justice do not adequately respect the institution of the family.

The Mayo Clinic, recognized as a world center for comprehensive diagnosis and surgical therapy by World War I, became known for its research programs during the 1920s. Laboratories of experimental biochemistry and experimental surgery had already been established. In 1920 Will Mayo hired internist-pharmacologist Leonard Rowntree to build a hospital-based program of clinical research in Rochester, Minnesota. Rowntree assembled a group of internist-investigators that complemented internist Henry Plummer’s team of medical diagnosticians. Much of the research undertaken at Mayo focused on common clinical problems. The institution was among the first to study insulin therapy for diabetes. Steady growth of the multispecialty group practice led to the construction of a twenty-floor outpatient building that opened in 1928. In it, internist-diagnosticians were clustered in sections that reflected their interests in subspecialties, such as cardiology, gastroenterology, or hematology.

Chapter 5 discusses the different catalysts and inhibitors which firms encounter when they innovate their business model, including the various factors that prevented the Dutch telecoms company KPN from renewing its business model. There follows an analysis of how factors such as the organizational culture and style of leadership, the characteristics of the CEO, the degree of external orientation, organizational characteristics, and institutional stakeholders can delay or speed up business model innovation. The roles played, for example, by transformational leadership, the length of the tenure of the CEO, the corporate governance regime, absorptive capacity, and innovative culture are then considered. Case evidence from DSM, Randstad, Roche Diagnostics, and Shell supports the findings. Finally, the chapter considers various journeys that firms may undertake in the transformation of their business model.

This chapter looks at the epidemiology of lupus—this disease is more common than is generally recognized and occurs most frequently in women. There is a significantly higher risk of developing lupus in people of African, Hispanic, and Asian (especially Chinese) descent, with increased incidence and prevalence in some other ethnic minority races. They appear to develop a more severe phenotype, often with renal involvement and at an earlier age than white people of North European descent. Although deaths due to lupus are now less common in North American and European countries, deaths due to renal lupus remain a significant problem in the ethnic minorities. Management should target reducing factors that increase the risk of lupus disease activity, damage, and mortality. Raising awareness of lupus, leading to early diagnosis and institution of appropriate therapy to reduce disease activity, will reduce chronic damage and the associated risk of subsequent premature death.

Political abuse of psychiatry refers to the misuse of psychiatric diagnosis, treatment, and detention for the purposes of obstructing the fundamental human rights of certain individuals and groups in a given society. The practice is common to but not exclusive to countries governed by totalitarian regimes. In this chapter the question is raised as to why, in spite of the fact that in most countries medical practice takes place according to internationally accepted guidelines, there is at the same time continued evidence of cases of incarcerating dissidents in mental institutions and of social activists and/or practitioners of various religions who are subjected to psychiatric treatment. In addition, the authors analyze why in particular in mental health the ethical borders have been corrupted and which measures can be put in place to combat these excesses.

Railway spine, nerve prostration, combat neurosis, post-traumatic stress disorder: throughout the twentieth century, a complex array of terms has been codified by cultural, national, and medical institutions to describe a body and mind made dysfunctional by the inability to process intensely disturbing memories. In the wake of World War I, trauma-induced mental illness—diagnosed and treated as “shell-shock” in countless veterans—became an imperative focal point for sociopolitical and medical reform throughout Europe. This essay explores the connections between this historically contextualized psychiatric disorder and the music of Ivor Gurney, a soldier in the British Army whose life and work was significantly affected by his diagnosis in 1918. Through particular disturbances of form, structure, and texture, Gurney’s musical landscapes reenact the conditions of psychic trauma by creating a world in which memories are disruptive, invasive, and ultimately disabling.
