Abstract:
The statistical evaluation of diagnostic tests may be affected by several potential biases. These biases include those caused by a study design that results in a non-representative sub-sample who are further verified by the reference test (verification bias), and those caused by the absence of a definitive diagnostic test (gold standard test) for many diseases and conditions. In practice, an imperfect reference test is often assumed to be a perfect gold standard, potentially resulting in a large bias. Both Bayesian and frequentist methods have been proposed to adjust for each of these biases independently. To our knowledge, there is no Bayesian solution for that adjusts for both of these biases simultaneously. The objective of this thesis is to present a Bayesian method for the evaluation of diagnostic tests when both of these potential biases may be operating simultaneously. We develop a likelihood function that models both sources of bias, and suggest convenient prior distributions that simplify deriving posterior distributions. The models are based on dichotomous test results and the parameters of interest are estimated using a Gibbs sampler. Using both simulated and real data examples, we demonstrate that the method presented here can correct the verification bias even when a perfect gold standard test does not exist.

Abstract:
In the present study an attempt has been made to contribute for the analysis of the factors related with the technical acquisition, the quality of the diagnostic image and dose management in digital detector systems for projection radiography. The thesis methodology is organized in two components. The observational component is based on a retrospective and transversal design. The data collected from CR and DR systems allowed the evaluation of exposure parameters from digital images, absorbed dose and exposure index to the detector. Under this classification (retrospective and transversal design) it was also possible to develop diagnostic quality evaluations from digital radiographic images: observer performance studies from digital images stored in the PACS. Under the experimental component of this thesis several experiments using phantoms were performed in order to evaluate the relation between dose and image quality. The experiments allowed the characterization of physical properties of digital image systems and manipulation of variables such as those related to exposure parameters and evaluation of its influence in dose and image quality. Using a contrast-detail phantom, anthropomorphic phantoms and an animal bone phantom it was possible to provide objective measures concerning the quantification of diagnostic quality and measures of object detectability. According to the research undertaken several conclusions could be highlighted. Quantitative measures of DR and CR detectors performance provide a basis for optimization, allowing to measure and determinate the physical properties of digital radiology image systems. Exposure parameters being used in digital radiography shows that current routine radiographic practice does not comply with European guidelines concerning exposure techniques. There is a need to evaluate, improve and establish a baseline reference aiming exposure optimization and provide new guidelines for current digital systems. Exposure parameters influence patient dose in digital radiology but image quality perception seems not to be affected with exposure variation. Studies performed both with phantoms and patients showed that overexposure is a potential risk when working with digital systems. Evaluation of diagnostic quality of digital images when a variation of exposure parameters is provided showed no substantial decrease in image quality when dose reduction is achieved. New dose reference levels should be studied and implemented according to the digital system characteristics and performance. A conceptual framework (STDI) is proposed as an attempt to provide a practical method to optimize digital radiology systems.

Bai, Xueshi. "Laser-induced plasma as a function of the laser parameters and the ambient gas." Thesis, Lyon 1, 2014. http://www.theses.fr/2014LYO10333/document.

Full text
Abstract:
Laser-induced breakdown spectroscopy (LIBS) has been invented for more than 50 years, which analyzes the spectrum of the laser-induced plasma to determine the elemental composition of the ablated sample. Recently, LIBS technique has been well developed and applied in different domains, for example oceanic exploration, pollution monitoring in the environment. LIBS uses the ablation plasma as a light source that contains the elemental composition information of the sample. However, the laser-induced plasma exhibits a transient behavior. Although time-resolved and gated detection can greatly improve the performance of the LIBS technique especially that of calibration-free LIBS (CF-LIBS) with a better determination of plasma temperature, the temporal evolution of the plasma is correlated to its morphology and its spatial inhomogeneity. The determination of the morphology as well as the internal structure of the plasma together with their evolution during plasma expansion into the ambient gas is therefore crucial for the optimization of the use of ablation plasma as a spectroscopic emission source. Evolutions of the morphology and the internal structure of the ablation plasma are considered as the consequence of its hydrodynamic expansion into the ambient gas. Following the thesis of Qianli Ma which has studied the effect of laser wavelength on the behavior of the plasma induced in an ambient gas of argon, the present thesis has used the same diagnostic techniques (time- and space-resolved emission spectroscopy and fast spectroscopic images) together with 1064 nm ns laser pulse ablation of a target of aluminum to investigate the effects of other parameters, such as the fluence and the duration of laser pulse, the effect of different ambient gases (argon and air), on the morphology and internal structure of the plasma. Furthermore, in order to understand the effects of these parameters on the properties of the plasma, the microscopic mechanisms during post ablation and the propagation of the plasma are also studied. While the plasma cools down in air, molecules are formed, AlO for instance. So the thesis also studied the condition for the formation of the molecules in the plasma. Beyond the practical interest of this study for LIBS, it provides also insights to the kinetics of the AlO molecule formation in laserinduced plasma

Abstract:
This thesis focuses on fault diagnosis of wired electric networks using reflectometry. To develop diagnostic algorithms, we studied the direct problem (numerical simulations of electrical networks) and the inverse problem (determination of certain properties of a network from reflectometry measurements). For the direct problem, we developed a method for the computation of reflection coefficients. This method is based on the successive solving for a Riccati differential equation. We also generalized the BLT equation for the nonuniform electric networks and automated the resolution of this method. The thesis has made two new results concerning the inverse problem. The first result concerns the estimation of lengths and loss coefficients of the branches of a star network via an iterative method. The second focuses on the identification, at least partially, of the branches admittance matrices of a electric network modeled by the equation of BLT. The methodologies and formalisms proposed in this thesis are validated either by numerical simulations or by real measurements

Abstract:
The Fontan repair is a three-stage palliative surgical procedure for single ventricle congenital heart diseases, ultimately resulting in the right heart bypass. This is accomplished by routing the systemic venous return directly to the lungs. Although this procedure reduces the mortality rate, its long-term outcome is still considered far from optimal. Over the years several modifications have been suggested, ultimately leading to the total cavopulmonary connection (TCPC), which is the current procedure of choice. A better understanding of the hemodynamics in the TCPC is critical for further optimization of the TCPC design and surgical planning, which may lead to improved surgical outcome. Recent experimental and numerical studies have focused on characterizing the fluid dynamics of the TCPC but to date no study has attempted to relate the geometry of the TCPC anatomies with their hemodynamic parameters. The present study therefore proposes to quantify the complex geometrical characteristics of patient-specific TCPC anatomies and correlate these characteristics with their hemodynamic efficiency. A technique using skeletonization approach is thus developed to achieve this goal. The centerline approximation of the TCPC geometry is used to extract main geometric parameters such as vessel area, curvature and offset. The developed methodology is then applied to characterize the shape of various TCPC templates including extra-cardiac (EC) and intra-atrial (IA) TCPCs, TCPCs with bilateral Superior Vena Cavae and geometries before the third stage. The obtained geometric parameters are then related to the TCPC hemodynamics, particularly to the power loss.

Abstract:
ABSTRACT The colic, or acute abdominal pain, is a non specific sign which can be originated by several disorders of the gastrointestinal tract or others that do not regard this last one, in which case the colic is named “false colic”. The true colic is responsible for the majority of deaths in horses, except those cases in which horses die due to advanced aging. The aetiology of the disorders of the gastrointestinal tract that lead to equine colic is complex and diverse. Still, with no explanation remain some gut displacements or twists and impactions. In the practical daily clinic the majority of colic cases have an unknown cause, but in general, they are a result of gut distention by ingesta, gas, fluids or due to an interruption of the normal motility (Íleus). The most severe cases can also result from damages of the intestinal wall by processes of isquemia, inflammation, edema or infarction. In the diagnosis of this pathology there are several parameters that must be evaluated, such as: pain degree; abdominal distention; the respiratory, heart and pulse rate; mucous membranes; membrane refill time; rectal temperature; gastrointestinal motility; gastric reflux; findings in rectal examination; packed cell volume; total plasma protein value; plasma fibrinogen value; white blood cell count; electrolytic quantification; analysis of blood gases; quantification of serum enzymes; plasma lactate value; characteristics of the peritoneal fluid collected by abdominal paracentesis; ultrasonographic findings; radiographic findings; endoscopic findings; laparoscopic findings; and fecal analysis. The goal of this work consisted on the establishment of guiding points in the difficult decision of the appropriate treatment, analyzing some parameters of diagnosis that, jointly, supply tracks to reach a more specific diagnosis of the disorder which is causing the abdominal pain, and as such to be able to make the right decision regarding the necessary treatment, being either surgical or medical. The study, which was carried on 13 cases of abdominal colic, has shown that surgical cases had a more violent pain, and had presented a higher heart rate and membrane refill time than the patients who only needed medical treatment, whereas the total plasma protein value was higher in medical cases. From these results and other previously existing studies, one can conclude that when exists a severe degree of pain, abnormal findings in the rectal examination and absence of response to the analgesic therapy, the colic case must be referred for surgery, even thought there are no significant changes in the heart rate, mucous membrane, membrane refill time and in the analysis of the peritoneal fluid.

Abstract:
One of the most promising biophysical markers of preterm labor is the electrical activity of the uterus, picked up on woman’s abdomen, the electrohysterogram (EHG). Several processing tools of the EHG signal (linear, nonlinear), allow the analysis of both excitability and propagation of the uterine electrical activity in order to differentiate between pregnancy contractions, which are ineffective, from labor effective contractions that might cause preterm birth. Therefore, on these multiple studies, the parameters being computed from different signal databases, obtained with different recording protocols, it is sometimes difficult to compare their results in order to choose the “best” parameter for preterm labor detection. Additionally, this large number of parameters increases the computational complexity for diagnostic purpose. Therefore, the main objective of this thesis is to select, among all the features of interest extracted from multiple studies, the most pertinent feature subsets in order to discriminate, on a given population, pregnancy and labor contractions. For this purpose, several methods for feature selection are tested. The first one, developed in this work, is based on the measurement of the Jeffrey divergence (JD) distance between the histograms of the parameters of the 2 classes, pregnancy and labor. The other are “Filter” and “Wrapper” Data Mining methods, extracted from the literature. In our work monovariate (in one given EHG channel) and bivariate analysis (propagation of EHG by measuring the coupling between channels) are used. The EHG signals are recorded using a multichannel system positioned on the woman’s abdomen for the simultaneous recording of 16 channels of EHG. Using all channels, for the monovariate, or all combinations of channels for the bivariate analysis, leads to a large dimension of parameters for each contraction. Therefore, another objective of our thesis is the selection of the best channels, for the monovariate, or best channel combinations, for the bivariate analysis, that provide the most useful information to discriminate between pregnancy and labor classes. This channel selection step is then followed by the feature selection for the channels or channel combinations selected. Additionally, we tested all our work using monopolar and bipolar signals. The results of this thesis permits us to evidence, when processing the EHG, which channels and features can be used with the best chance of success as inputs of a diagnosis system for discrimination between pregnancy and labor contractions. This could be further used for preterm labor diagnosis

Abstract:
Electrical parameter profiles of human breast images can be used to simulate and analyze the anticipated effects on tissue from its interaction with electromagnetic fields involved in the cancer treatment exposure. In part, the success of this approach depends on the accuracy and precision in identifying the different tissue types. In this work, we propose two methods of segmenting human breast images with malignant tumors. The first method of algorithmic partitioning of the image involves manual color-edge contouring of the tissues using a cursor and subsequent identification of the tissue types. For the second method, MRI T1 values and thresholds are used to perform segmentation and we investigate the potential of incorporating edge detection. The first method is effective, while the second lacks precision, but eliminates the need of manual contouring. The images are imported as BMP files into SEMCAD, an electromagnetic simulation tool based on finite-difference time-domain method, which recognizes the grouped tissues and creates a model of the image. The model allows the user to easily assign electrical parameter values to the grouped tissues, according to the measured values reported in the literature.

Abstract:
This thesis presents an approach for qualitative model based fault diagnosis via parameter estimation. Estimating parameters qualitatively in this research is based on the relationships among involved qualitative quantities. To maintain the consistency of ordinal relationships an inequality reasoner is developed. Existing inequality reasoners (or commonly in the AI community referred to as quantity lattices) infer relationships between quantities involved in a system (qualitative or quantitative) based on the real values associated with them. The novel feature of our inequality reasoner is that it infers and maintains relationships among pure qualitative quantities. Inferring ordinal relationships between sign based quantities removes the traditional drawback of qualitative domain, the lack of discriminating power.

BPL technology is quite widespread nowadays and the public is also somewhat aware of it. This technology can be used as a means of communication without the need to install additional media and thus use the existing infrastructure. In an effort to use the full potential of the technology, BPL communication began to be used to monitor the current state of cables in distribution networks online. Where the technology is already fully implemented, the distributor can use the data to evaluate the current situation. In this way, BPL technology can be used as a diagnostic tool to test the quality of a cable connection on which not only time but also humidity and other external influences could have a significant effect.

Abstract:

The performance of different types of nebulisers: Vulkan direct injection nebuliser (Vulkan DIN), direct injection high efficiency nebuliser (DIHEN), microconcentric nebuliser coupled to cyclonic or double pass spray chamber (MCN-C or MCN-DP, respectively) was investigated and compared when used for sample introduction to ICP-MS or ICP-OES. With ICP-OES, in axial viewing mode, intensity distributions across the radius of the plasma (radial intensity profiles) were determined for different spectral lines with Esum 1.85-15.41 eV to determine fundamental plasma properties for various matrices using Vulkan DIN and MCN-C. The results showed that with the MCN-C the ionisation temperature (Tion) was about the same across the measured region of the plasma (±3.0 mm) whereas with the Vulkan DIN the Tion was significantly lower in the centre of the plasma. A large deviation from local thermodynamic equilibrium, as well as deteriorated stability, was observed for the plasma when using the Vulkan DIN.

With ICP-MS noise power spectra (NPS) were generated to identify sources of noise. NPS showed that the magnitude of white noise for the tested sample introduction systems decreased in the following order: Vulkan DIN > DIHEN > MCN-C > MCN-DP. This order follows the decrease of mean droplet size and span of the size distribution, indicating that the white noise is caused by spatial and temporal non-uniform desolvation and ionisation. Another source of noise arose from the peristaltic pump and the magnitude of pump interference noise decreased in the following order: DIHEN > MCN-C/DP > Vulkan DIN. Mains power interference noise and 1/f noise were lower for the direct injection nebulisers compared to the spray chamber systems. The contribution or effects of these noise components on relative standard deviations of steady-state ion-count rate and isotope ratio measurements is discussed in this thesis.

Aerosols generated by the Vulkan DIN and the DIHEN were also directly characterised using Particle Dynamic Analysis. The Vulkan DIN produced particles with a mean diameter of ~30 µm and a size distribution between 2-80 µm. With the DIHEN the corresponding values were ~11 µm and 1-40 µm, respectively, with a few particles at 55-78 µm. The mean velocity of particles from the Vulkan DIN was ~10 m s-1 and from the DIHEN ~18 m s-1. The lower velocity allows longer residence time counteracting the effects of the larger droplet size.

Abstract: Purpose: To test the ability of structural parameters (as measured by the Scanning Laser Polarimetry and Confocal Scanning Laser Ophthalmoscopy) to discriminate between normal and glaucomatous eyes. Methods: After approval of the Ethics Committee of the University of Campinas, primary open-angle glaucoma patients were selected from the glaucoma service. Normal individuals were recruited from volunteers among the medical staff, university members, family and friends of patients. All individuals underwent a thorough ophthalmic evaluation, a 24-2 full threshold Humphrey visual field, Scanning Laser Polarimetry with the GDx and Confocal Scanning Laser Ophthalmoscopy with the TOPSS. Cut-off points were selected and ROC curves were created for each individual Confocal Scanning Laser Ophthalmoscopy and Scanning Laser Polarimetry parameters. Finally, multivariate discriminant formulas were developed in order to achieve a better sensitivity (Se)/specificity (Sp) ratio for the diagnosis of glaucoma, initially separately for each device, and then combining parameters from Confocal Scanning Laser Ophthalmoscopy and Scanning Laser Polarimetry. Results: One-hundred and twelve patients with primary open-angle glaucoma and 88 normal individuals were enrolled in the study. The best Scanning Laser Polarimetry parameters in differentiating normal individuals from glaucoma patients were: The Number (Se: 79,5%, Es: 81,8%, area under the ROC curve- aROC: 0,870), Maximum Modulation (Se: 83,0%, Es: 76,1%, aROC: 0,842) and Ellipse Modulation (Se: 65,2%, Es: 88,6%, aROC: 0,831), while the best Confocal Scanning Laser Ophthalmoscopy individual parameters were: Average Disc Diameter (Se: 64%, Sp: 89%, aROC: 0,824), Total Disc Area (Se: 85%, Sp: 66%, aROC: 0,802) and Cup Area (Se: 69%, Sp: 85%, aROC: 0,797). However, the multivariate analysis resulted in better results for the Scanning Laser Polarimetry (Se: 87,5%, Es: 86,4%, aROC: 0,900), Confocal Scanning Laser Ophthalmoscopy (Se: 90%, Sp: 81%, aROC: 0,910), and for the combination of parameters from both devices (Se: 93%, Es: 91%, aROC: 0,970). Conclusion: Structural parameters, as measured by the Confocal Scanning Laser Ophthalmoscopy and Scanning Laser Polarimetry have good ability to differentiate normal individuals from glaucoma patients. However, better results are obtained combining parameters from both devices in a multivariate discriminant formula

The objective of this work was to propose a systematic and reasoned approach of the quantification tools available in clinical routine and in research in the management of patients with endocrine tumors. Our results report the limitations of semi-quantitative parameters to assess the malignancy of fTI detected by 18FDG-PET / CT. Our data on the interest of textural parameters in the diagnosis of malignancy of adrenal tumors show excellent performances of these indices. However these indices do not appear superior in comparison with conventional semi-quantitative parameters. Our approach raises new hypotheses regarding the interest of texture analysis in the characterization of some rare adrenal tumors (pheochromocytomas and adrenocortical carcinoma) that will be the subject of future work. Finally, our work on the multi-parametric 68Ga-DOTATOC-PET / CT imaging highlights the feasibility of a whole-body dynamic 68Ga-DOTATOC-PET / CT acquisition and the possibilities to perform quantification of NETs uptake treated with PRRT on post-therapeutic SPECT / CT acquisitions. Our results open up multiple perspectives for the management of NETs at different stages of the disease (diagnosis, prognosis and therapeutic evaluation)

Abstract:
Microarrays containing long oligonucleotides provide sensitive and specific detection of gene expression and are becoming a popular experimental platform. In the process of designing an oligonucleotide microarray for Brucella, we optimized the overall design of the array and created probes to distinguish among the known Brucella species. A 3-way genome comparison identified a set of genes which occur uniquely in only one or two of the sequenced Brucella genomes. Reverse transcriptase PCR assays of over one hundred unique and pairwise-differential regions identified in Brucella revealed several groups of genes that are transcribed in vivo with potential significance for virulence. The structural and thermodynamic properties of a set of 70mer oligonucleotide probes for a combined B. abortus, B. melitensis and B. suis microarray were modeled to help perform quantitative interpretation of the microarray data. Prediction and thermodynamic analysis of secondary structure formation in a genome-wide set of transcripts from Brucella suis 1330 demonstrated that properties of the target molecule have the potential to strongly influence the rate and extent of hybridization between transcript and an oligonucleotide probe in a microarray experiment. Despite relatively high hybridization temperatures used in the modeling process, parts of the target molecules are predicted to be inaccessible to intermolecular hybridization due to the formation of stable intramolecular secondary structure. Features in the Brucella genomes with potential diagnostic use were identified, and the extent to which target secondary structure, a molecular property which is not considered in the array design process, may influence the quality of results was characterized.

Abstract:
The behaviour of rotating machinery during deceleration when the power of the drive to the rotating machinery is stopped is known as Coast Down Phenomenon (CDP). The total time elapsed for the entire momentum due to sustained operation of the rotating machinery to dissipate is known as Coast Down Time (CDT). The characteristic profile of CDT (CDT-P) depends on inertia forces of the machinery components, tribological behaviour and environmental effects such as fluid drag. The aim of the research is to investigate the potential and the feasibiJity of using CDT as an effective tool for condition monitoring of machinery. A review of current literature has given a clear understanding to what extent the CDP has been explored with respect to utilising it as a diagnostic parameter. However no evidence for effective use of CDT in condition based monitoring programmes can be found. The main focus of this research is concentrated on ascertaining the validity of Coast Down Time as one of the condition monitoring parameters for a horizontal rotor system. Extensive experimental analysis is conducted on a rotor system consisting of a journal bearing using different lubricants under various mechanical and operating conditions An empirical formula is developed to determine the Coast Down Factor (CDF). Furthermore, the effect of rub on the motor has been studied with the aid of an electromagnetic clutch and isolating the rotor from the drive system. This research has shown a novel way to interpret the CDT-P to be used as a diagnostic parameter in condition monitoring by the formulation of Coast Down Factor. CDF gives a simple, but highly flexible tool for CDT analysis. COT monitoring through the trend of CDF can be used to analyse the performance of a journal bearing under given tribological conditions; to utilise as a tool for optimisation of selection of lubrication oil; to evaluate and understand. the tribological behaviour of lubrication and the performance of a journal bearing under different oil pressures; and to diagnose the mechanical degradation with simulated unbalance to the rotor system.

Abstract:
This thesis work aims at modeling cortico-cortical evoked potentials (CCEPs) induced by intracortical direct electrical stimulation in epileptic patients being recorded with stereo-electroencephalography during epilepsy surgery. Neural mass models implemented within the dynamic causal modeling (DCM) framework are used for this purpose. We first demonstrate the importance of using an accurate integration scheme to solve the system of differential equations governing the global dynamics of the model, in particular to obtain precise estimates of the neuronal parameters of the model (Lemaréchal et al., 2018).In a second study, this methodology is applied to a large dataset from the F-TRACT project. The axonal conduction delays and speeds between brain regions, as well as the local synaptic time constants are estimated and their spatial mapping is obtained based on validated cortical parcellation schemes. Interestingly, the large amount of data included in this study allow to highlight brain dynamics differences between the young and the older populations (Lemaréchal et al., submitted). Finally, in the Bayesian context of DCM, we show that an atlas of connectivity can improve the specification and the estimation of a neural mass model, for electroencephalographic and magnetoencephalographic studies, by providing a priori distributions on the connectivity parameters of the model. To sum up, this work provides novel insights on dynamical properties of cortico-cortical interactions. The publication of our results in the form of an atlas of neuronal properties already provides an effective tool for a better specification of whole brain neuronal models

Abstract:
This research introduces a new method of diagnosing the internal condition of a reciprocating air compressor. Using only measured load torques and shaft dynamics, pressures, temperatures, flow rates, leakages, and heat transfer conditions are quantified to within 5%. The load torque acting on the rotor of the machine is shown to be a function of the dynamics (instantaneous position, velocity, and acceleration) of the driving shaft, the kinematic construction, and the internal condition of the machine. If the load torque, the kinematic construction of the machine, and the dynamics of the rotor are known, then the condition of the machine can be assessed. A theoretical model is developed to describe the physical behavior of the slider-crank mechanism and the shaft system. Solution techniques, which are based on the machine construction, crankshaft dynamics, and load torque measurements, are presented to determine the machine parameters. A personal computer based system used to measure the quantities necessary to solve for the machine parameters and the quantities used to compare with calculations is also documented. The solution algorithm for multi-stage compressors is verified by decoupling the load torque contributed by each cylinder. Pressure data for a four-stage two-cylinder high pressure air compressor (HPAC) is used. Also, the mathematical model is proven feasible by using measured angular velocity of the crankshaft and direct measurements of the load torque of a single stage, single cylinder air compressor to solve for the machine parameters. With this unintrusive and nondestructive method of quantifying the operating parameters, the cylinder pressures, operating temperatures, heat transfer conditions, leakage, and power consumption of a reciprocating air compressor can be evaluated.
