Abstract:
The concepts of reliability and validity are fundamental for evaluating psychiatric diagnosis, including the "operationalist" approach pioneered in DSM-III. This chapter explores the complexity of these psychometric concepts and their interrelations. Although reliability constrains validity it does not guarantee it, and pursuing reliability in diagnosis can reduce validity. It is widely believed that the operationalist emphasis on diagnostic reliability has compromised the validity of recent psychiatric classifications. In particular, writers have argued that the drive for atheoretical diagnostic criteria has come at the cost of phenomenological richness and psychodynamic complexity. This chapter argues that although the operationalist turn may have impaired the validity of psychiatric diagnosis in some respects, these criticisms must be balanced by an appreciation of its benefits. In addition, it is suggested that some criticisms rest on a misunderstanding of the goals of operational descriptions. They should be evaluated primarily on pragmatic grounds as identification procedures and judged on their success in serving epistemic and communicative functions. Operational descriptions should not be viewed as comprehensive definitions of clinical phenomena or judged on their failure to encompass the richness and complexity of mental disorders. A diagnostic system is best understood as an intentionally delimited instrument for enabling clinical inference and communication. In essence, it is a simplified pidgin with which clinicians who speak different first languages (theoretical orientations) can conduct their shared business.

The two main psychiatric taxonomies set out codifications of psychiatric diagnoses via lists of symptoms with the aim of maximizing the reliability of diagnostic judgements. This approach has been criticized, however, for failing to capture the precise connection between diagnostic judgements and symptoms as detected by skilled clinicians. Assuming that this criticism is correct, this chapter offers two related accounts of why this might be so. First, skilled diagnostic judgement may be an exercise of tacit knowledge: a practical skill the exercise of which requires the presence of the patient. Second, the conception of criteria implicit in the DSM and ICD is based on a mistaken view of how what people say and do connects to their mental states. On an alternative account, in an overall gestalt diagnostic judgement the various criteria are abstractions from a whole that directly expresses the underlying psychopathological state of patients or clients.

This chapter describes the key investigations used in the diagnosis of sleep disorders. The recording and value of polysomnography (PSG), with and without electroencephalography is discussed, including the specific parameters essential for the various sleep related breathing disorders. The technical aspects of the multiple sleep latency test and maintenance of wakefulness test are followed by interpretation of findings, recognizing their limitations, especially in adults with co-morbid disorders, and the effect of medication and also shift work. Finally, the value of wrist actigraphy as a diagnostic tool in circadian rhythm sleep–wake disorders is discussed. The chapter emphasizes the importance of selecting appropriate tests based on clinical information and the importance of units providing diagnostic studies for sleep disorders, constantly reviewing their study protocols and auditing inter-rater reliability of PSG sleep staging in children and adults of various ages and sleep disorders.

Journals are becoming increasingly more stringent in their requirements for what must be reported in articles about the psychometric properties of scales. This chapter reviews three of the most commonly used guidelines; the Standards for Educational and Psychological Testing, the STARD initiative (Standards for Reporting of Diagnostic Accuracy), and the Guidelines for Reporting Reliability and Agreement Studies (GRRAS). It abstracts portions of these guidelines that are most relevant for scales used in research settings. These cover the reporting of test development, reliability, and validity. The chapter also has a flow chart, adapted from STARD, that should be included when submitting a manuscript about scale development to a journal.

Chapter 23 covers how tThe notion of "operational” diagnoses/criteria is ubiquitously revered in psychiatry, evoking a reassuring sense of reliability and objectivity. However, most psychiatrists are unaware of what the term “operational” actually signifies. It also addresses the origins of this term, its historical permutations, and its current status in the diagnostic manuals, and that the descriptive psychiatric terms are not (and cannot be) “operational” in any remotely significant scientific or theoretical sense. This is partly so because the individuation of symptoms and signs involve an articulation of Gestaltic, context-dependent wholes, comprising the “outer” (expressivity, behavior) and the “inner” (subjectivity, experience). The so-called operational revolution entailed an oversimplification of psychopathological discourse to a lay level, rendering irrelevant the previously achieved clinical and conceptual insights. The chapter discusses the widely recognized lack of progress in clinical psychiatry as one, inevitable, epistemological consequence of the “operational” remake of psychiatry.
