Abstract:
Diagnostic reasoning is the mental process by which physicians turn information about the patient into the name of a disease. To do this, physicians must gather and evaluate evidence relevant to the clinical problem, and then choose a diagnosis or make a decision about management. As in detective work, with which diagnostic reasoning has many similarities, physicians have to reason from observed effects to their possible causes. This chapter explores the thinking behind diagnostic reasoning, drawing on insights from cognitive psychology, philosophy, and the design of computer programs. If physicians have a deeper understanding of the reasoning that underpins making a diagnosis, they may be more astute diagnosticians, and better able to teach the skill to novices. And, however physicians arrive at a diagnosis, they need to be able to articulate their reasoning to the patient and their colleagues.

Abstract:
The two main psychiatric taxonomies set out codifications of psychiatric diagnoses via lists of symptoms with the aim of maximizing the reliability of diagnostic judgements. This approach has been criticized, however, for failing to capture the precise connection between diagnostic judgements and symptoms as detected by skilled clinicians. Assuming that this criticism is correct, this chapter offers two related accounts of why this might be so. First, skilled diagnostic judgement may be an exercise of tacit knowledge: a practical skill the exercise of which requires the presence of the patient. Second, the conception of criteria implicit in the DSM and ICD is based on a mistaken view of how what people say and do connects to their mental states. On an alternative account, in an overall gestalt diagnostic judgement the various criteria are abstractions from a whole that directly expresses the underlying psychopathological state of patients or clients.

Abstract:
The diagnostic and treatment approach for patients with skull lesions begins with a thorough history and physical and careful attention to anatomic localization. The patient’s history and exam findings can inform a preliminary differential diagnosis, which may be broadly divided into benign and malignant processes. Based on a preliminary assessment, appropriate neuro-imaging involving magnetic resonance, computed tomograph, and/or vascular modalities may be pursued. Characteristic image findings may further refine a differential. While conservative management may be indicated for the most assuredly benign lesions, surgery is appropriate for cases involving compression of neural structures, deformity, pain or when a tissue diagnosis is required.
