Abstract:
Objectives: To review the literature evaluating outcomes resulting from expansion of the bipolar disorder (BD) diagnostic category. We were particularly interested in identifying high-level evidence for improved clinical outcomes as documented by randomized controlled trials (RCTs) or cohort studies. Methods: The English-language literature was searched using Ovid MEDLINE for studies of BD referenced against the key word spectrum. We used bibliographies and other databases to extend this search when no relevant RCTs or relevant cohort studies were identified. Results: In the MEDLINE searches, abstracts and titles of 86 studies were examined and 48 were found to be related to the topic of bipolar spectrum disorders (BSD). No RCTs or prospective cohort studies evaluating modified diagnostic or therapeutic practices were identified. The literature about the BSD consists mostly of expert opinion emphasizing: various links between bipolar and unipolar mood disorders; a proposal that a greater proportion of the population without a mood disorder as defined by the Diagnostic and Statistical Manual of Mental Disorders should be diagnosed under the BD category; and, proposals that syndromes currently classified elsewhere should be subsumed under the BD category. Conclusions: Our search failed to uncover high-level evidence demonstrating the clinical utility of proposed diagnostic realignments. The widespread acceptance of the expanded spectrum concept appears to be based on interpretation of descriptive epidemiologic data by high-profile experts.

Abstract:
On one hand, the membrane must be sufficiently hydrated to allow the transport of protons from the anode side to the cathode side. But on the other hand, supersaturation of water vapour in the gas phase results in liquid water formation, which can be detrimental to the fuel cell operation. Liquid water can clog up the porous structure of the gas diffusion layer (GDL) and hinder the transport of gases in the catalyst layer. Liquid accumulation can also lead to the formation of water columns inside the gas flow channels, thus preventing the fuel gas from flowing into the reaction area.Micro-sensors have been designed to detect the presence of liquid water. Three of them were inserted inside the gas distribution channels of a 25 cm2 cell. The tests run with this equipped cell highlighted the link between the presence of liquid water and the variations of the cell voltage. Besides, existing correlations were used to calculate the pressure drop between the inlet and the outlet of the cathode channel due to two-phase flow during the cell operation. The results of this calculation were compared with experimental results. This part of the study highlighted the influence of the GDL and of the operating conditions on the flows inside the channels

Abstract:
In Automotive Industries, to be confident regarding the success of a planned operation, performing accurate methods in order to detect abnormal operating conditions, known as faults, is crucial. An effective method for diagnosis and fault recognition ensures the safety of the operation, reduces manufacturing cost and any other potential impacts. In addition, mobile solutions have been widely adopted among automotive manufactures during recent years and they have taken full advantage of mobile strategies. Accordingly, it is necessary for there to be a future-proof plan to control the diagnostic operations in advance. In this thesis, the immediate objective has been to offer a future-proof and user-friendly solution to assist engineers and service technicians in the monitoring, detecting, and diagnosing of faults on Toyota/BT/CESAB branded trucks. A mobile cross-platform framework is used to develop the diagnostic mobile solution which is not only able to be deployed on Android and iOS mobile platforms, but also provides wireless communication between truck machines and mobile devices through Bluetooth and Wi-Fi ad hoc technologies. The diagnostic mobile tool is capable of processing real-time controller area network messages and visualizing the condition of different sensors in a more user-friendly way through rich hybrid and client-side web user interfaces. The experience of evaluating a cross-platform diagnostic tool on different mobile operating systems proved that cross-platform mobile development methodology can be a reliable technique for developing projects that essentially require real-time data processing. In addition, it indicates that Apple iOS offers a better runtime performance than Google Android for the current tool.

Abstract:
The geometry of pulsar beams is one of the intrinsic properties of neutron stars, governing the pulse-profile phenomenon and other aspects of pulsar astron- omy. With a number of pulsars in our dataset, their beam geometry is derived from the polarisation position angle (PPA) using the simple polar cap emission and dipole field model. This includes the rotating vector model (RVM), for which the solutions can hardly be constrained or fail to be consistent because of the lim- itations of the model itself. The inconsistencies in the results suggest that the initial PPAs can be strongly perturbed by additional parameters above the emis- sion altitude, such as the plasma medium or rotational aberration effects, after which their characteristic shape is no longer related to the geometry via the RVM. We investigate further into the effects of wave propagation in the pulsar magne- tosphere, and find an indication that, in most cases, the RVM-calculated PPAs are likely to be altered by plasma effects. In recent years, there have been an increasing number of intermittent and mode-switching pulsars observed to have their radio pulse profiles correlated with the change in pulsar spin frequency (ν ̇) (e.g. Lorimer et al. 2012, Lyne et al. 2010). These two phenomena are understood to be related via the states of plasma in the magnetosphere. As one such pulsar, and also one with known geometry and other astonishing behaviour, PSR B1822–09 is studied in terms of the mode- switching properties, the hollow-cone model and the wave propagation in the magnetosphere. We also study the model for explaining the intermittent pulsars PSRs B1931+24, J1841+0500 and J1832+0029, and find it can be consistently applied for PSRs B1822–09 and B0943+10, and other profile-switching pulsars. However, aspects of the conclusions are limited because of the lack of understand- ing of the connection between the radio flux and the states of plasma. We are also able to use the difference in the PPAs between two states of PSR B0943+10 to predict the change in plasma states and ν ̇, which cannot be measured directly from timing analysis as its switching timescale is too short.

Brandt, Rynier. "Rynmar value adding process design diagnostic tool." Thesis, Stellenbosch : Stellenbosch University, 2001. http://hdl.handle.net/10019.1/52442.

Full text
Abstract:
Thesis (MBA)--Stellenbosch University, 2001.<br>ENGLISH ABSTRACT: "Value adding process design" entails two underlying concepts, namely "business process" and "value adding": Business process: According to Dr Michael Hammer (Hammer, 1999), the recognised father of process thinking, a business process is an "organised group of related activities that together create customer value". The focus in the process is not on individual units of work, which by themselves accomplish nothing for a customer, but rather on an entire group of activities that, when effectively brought together, create a result that customers value. Value adding: The concept of "value adding" can be defined in different ways depending on the receiver of the value (shareholders, customers or employees). From a shareholder perspective, value adding can be measured by using EVA (Economic Value Added). EVA is a measure of economic profit generally meaning that a positive EVA indicates that value has been created, whereas a negative EVA means value has been destroyed. The perspective from which process improvement is addressed is the value that is added for the customer, but always with the constraint of not negatively impacting the EVA of the organisation. Value adding process design: "Value adding process" design entails the design of a business process or interrelated business processes to ensure that employee and customer needs are satisfied, whilst creating value for shareholders. The objective of this study is to develop a methodology and supporting tools to enable a organisation to make the transition from being task focused to becoming a truly process organisation. The approach that is proposed is the Rynmar VAP Diagnostic Tool. The approach consists of 5 phases, best explained by the metaphor of building a house: • Setting the stage (phase 0) is identifying the need for a house, i.e. being unhappy with the current situation to an extent that one has the burning desire to change surrounding, even if it will cost a lot of time, effort and financial resources. • Visioning (phase 1) is drawing an artist impression of the house. It involves thinking what the new house should look like, for example do I want a Cape-Dutch house with thatched roof and white walls, or an Italian design with tiled roof and off-white walls. Visioning is the magnetic force that one holds on to whenever the question is asked: "Is it worth the effort?" • Design Process (phase 2) entails applying different techniques to draw an architectural design of the firstly the current processes, followed by the future process that will meet the different aspects of the vision. • Prototype & Build (phase 3) involves firstly building a small scale model of the house to test and improve the design made in phase 3, followed by building the actual house. • Train & Implement (phase 4) firstly entails training the different people in the skills required by the new process design and then implementing the process under careful guidance of the project team, i.e. moving into the house. • Continuous Improvement (phase 5) involves continuously improving the process to ensure that incremental performance improvement is achieved, which will lead to a dramatic cumulative improvement over time.<br>AFRIKAANSE OPSOMMING: "Waarde toegevoeging proses ontwerp" behels twee onderliggende konsepte, naamlik "besigheidsproses" en "waarde toevoeging": Besigheidsproses: Volgens Dr Michael Hammer (Hammer, 1999), die erkende vader van prosesdenke, kan 'n besigheidsproses definieer word as 'n georganiseerde groep van aktiwiteite wat gesamentlik waarde skep vir 'n kliënt.

Abstract:
To improve the frequently performed lymphedema measuring session – important for an earlydetection of the disease – a novel device was evaluated and further developed in this study.Lymphedema appears as limbs swelling, often arms and legs, partly because of cancertreatment. The current assessment technique includes several circumference measurements witha regular tape measure, and the method had room for improvement regarding consistency andeffectiveness. Different clinicians may pull the tape measure differently while measuring thecircumferences of a patient’s limb. An additional skin compliance evaluation, performed by theclinicians, could be quantified and analyzed if the assessments were put into numbers. The goals of this project were to evaluate and improve the accuracy and precision of this noveldevice, as well as develop and integrate a feature for measuring skin compliance. Furthermore,a user interface adapted to the clinicians’ demands and a design adapted to testing and futuremanufacturing was essential. Due to patient safety reasons, a part of the device was madereplaceable between patients.Developing the device, video recordings of hands-on testing were performed, as well as asurvey distributed to clinicians who provided lymphedema therapy. The circumferencemeasurement developed in this study was, according to the performed tests, accurate enoughcompared to the accuracy required the current measuring routine. An automatic calculation of askin bulk modulus K, was developed as a feature of the device to be able to obtain a value ofthe compliance clinicians felt for with their hands. The K-value was proven as a concept bymeasuring different foams. Measuring K-values of the tissue, the device was capable ofdetecting an increase in muscle tension, but results were only predictable to a certain limit inthe test presented in the report. The method could not yet be validated by comparing it to a skinhardness measurement with a durometer. A conclusion was however, that the K-value stillcould be a useful indicator in addition to the current assessment and that the device could savetime in the measuring session. This saved time could be used for more measurement sessions, beneficial in order to detect lymphedema early (Lawenda, Mondry, and Johnstone, 2009),which in turn was essential for a successful treatment.

Abstract:
The diagnosis of mycobacterial infections remained practically unchanged during numerous decades and would not probably have progressed of the whole without the unexpected re-emergence of the tuberculosis (TB) during the last twenty years of the 20th century. The diagnosis of laboratory of pulmonary tuberculosis is mainly based on the detection of microorganisms of the Mycobacterium tuberculosis complex (MTC) in the respiratory specimens. For the patients who do not expectorate, samples of the respiratory tract can be only obtained according to invasive procedures. Based on the survival known for MTC organisms in the gastric liquid, we considered that MTC organisms would be detectable also in samples of stools. We compared the presence of bodies MTC in samples of respiratory tract and the specimens of stools collected from the same patients. MTC was detected in cultures on egg-based media after appropriate decontamination using the chlorhexidine and by the microscopic examination after Ziehl-Neelsen staining. After the set on of an original protocol of the total DNA extraction from stools, we were able to use by the detection with the real-time PCR of IS6110 using internal controls as PCR inhibitor control. This protocol illustrate the utility of stool analysis for the diagnosis of pulmonary tuberculosis, this technique is actually used in routine in our laboratory, it is also under investigation by other research teams.Phenotypic identification of mycobacteria based on the analysis of biochemical pattern (urase test, nitrate reductase test, resistance to pyrazinamide (pza) test, susceptibility to thiophen-2-carboxylic acid hydrazide (TCH) test, etc …) is more often replaced by molecular methods using DNA, including the development by PCR of a specific gene. These methods are generally laborious and can require a considerable degree of expertise. To simplify the identification specifies isolates of mycobacteria routinely in the laboratory, we estimated the use of the mass spectrometry "matrix-assisted laser desorption and ionization-time-of-flight" (MALDI-TOF MS) in association with a specific bioIT software, was capable of identifying and of distinguishing between the members of Mycobacterium tuberculosis complex, Mycobacterium avium complex and the non- tuberculosis mycobacteria. We verified the MTC inactivation by heating or by ethanol allowing the analysis of the inactivated MTC out of the P3 laboratory; we studied the cost-effectiveness of this method from the blood solid media by extrapolating this approach in the emergent countries. We then applied the same method to identify mycobacteria from Middlebrook 7H10 liquid media used in an automated oven. Identification of cultured Mycobacteria is now done in routine in our laboratory by MALDI-TOF-MS. These data contribute in improving the routine diagnosis of pulmonary tuberculosis and this protocol is then used routinely in our laboratory, particularly with the “kit de tuberculose” we set on

Full text
Abstract:
Endocarditis is defined as an inflammation of the endocardial surface of the heart. This may include heart valves, mural endocardium or the endocardium that covers implanted material, such as prosthetic valves, pacemaker/defibrillator leads and catheters. Infective and non-infective-related causes must be distinguished. In most cases, the inflammation is related to a bacterial or fungal infection with oral streptococci, group D streptococci, staphylococci and enterococci accounting for 85% of episodes. Infective endocarditis (IE) is a serious disease with an incidence ranging from 30 to 100 episodes/million patient-years. From various portals of entry (e.g. oral, digestive, cutaneous) and a subsequent bacteraemia, pathogens can adhere and colonize intracardiac foreign material or onto previously damaged endocardium due to numerous complex processes based on a unique host–pathogen interaction. Rarely, endocarditis can be related to non-infective causes, such as immunological or neoplastic. Mortality is high, with more than one-third dying within a year of diagnosis from complications such as acute heart failure or emboli. This disease still remains a diagnostic challenge with many cases being identified and subsequently treated too late. Diagnosis of IE usually relies on the association between an infectious syndrome and recent endocardial involvement. Blood cultures and echocardiography are the main diagnostic procedures, but are negative in almost 30% of cases, requiring the use of more sophisticated techniques. Computed tomography, magnetic resonance imaging and positron emission tomography are promising imaging modalities. Improved understanding of its pathophysiology and the development of relevant diagnostic strategies enables accelerated identification and treatment, and thus an improved prognosis.

Full text
Abstract:
Diagnostic reasoning is the mental process by which physicians turn information about the patient into the name of a disease. To do this, physicians must gather and evaluate evidence relevant to the clinical problem, and then choose a diagnosis or make a decision about management. As in detective work, with which diagnostic reasoning has many similarities, physicians have to reason from observed effects to their possible causes. This chapter explores the thinking behind diagnostic reasoning, drawing on insights from cognitive psychology, philosophy, and the design of computer programs. If physicians have a deeper understanding of the reasoning that underpins making a diagnosis, they may be more astute diagnosticians, and better able to teach the skill to novices. And, however physicians arrive at a diagnosis, they need to be able to articulate their reasoning to the patient and their colleagues.

In ASME Turbo Expo 2012: Turbine Technical Conference and Exposition. American Society of Mechanical Engineers, 2012. http://dx.doi.org/10.1115/gt2012-69368.

Full text
Abstract:
Gas turbine diagnostic algorithms widely use fault simulation schemes, in which measurement errors are usually given by theoretical random number distributions, like the Gaussian probability density function. The scatter of simulated noise is determined on the basis of known information on maximum errors for every sensor type. Such simulation differs from real diagnosis because instead of measurements themselves the diagnostic algorithms work with their deviations from an engine baseline. In addition to simulated measurement inaccuracy, the deviations computed for real data have other error components. In this way, simulated and real deviation errors differ by amplitude and distribution. As a result, simulation-based investigations might result in too optimistic conclusions on gas turbine diagnosis reliability. To understand error features, deviations of real measurements are analyzed in the present paper. To make error presentation more realistic, it is proposed to extract an error component from real deviations and to integrate it in fault description. Finally, the effect of the new noise representation mode on diagnostic reliability is estimated. It is shown that the reliability change due to inexact error simulation can be significant.

Abstract:
Unified gas transmission system which includes about 140 thousand kilometres of pipelines has been developed in Russia. The mains of pipelines cross the regions with difficult natural and climatic conditions, that originates specific distinctions during the fulfilling of the diagnostic works. Conception of the main gas transmission pipelines that is based on the determination of potential danger parts and individual inspection of each large-scale gas transmission pipeline has been presented in this work. Estimation procedure of flaw detection results with the purpose of operational capability prognostication of a gas transmission pipeline is also shown in this paper. The fields of application of the early and standard diagnostics are considered, and the analysis of possibility of inpipe and outpipe (from the ground) flaw detection is given in the conditions of Russia too. Practical samples of the diagnostics of gas transmission pipelines and technological equipment in several regions (Orenburg, West Siberia, Central Russia) are presented also. Integrated inspection program of RAO “Gazprom” gas transmission pipelines is shown and the main procedures of its realization beginning since 1995 are given in this document.

Abstract:
This paper presents a novel architecture of an impedance spectrometer dedicated to the characterization and diagnostic of large Fuel Cell (FC) stacks operated in galvanostatic mode. The validation tests are first performed on a single Proton Exchange Membrane Fuel Cell (PEMFC). Then, experiments are carried out on a twenty-cell PEMFC stack delivering more significant power levels. The proposed impedancemeter allows spectrum measurements on cells located in the middle of the stack, where common mode potentials are usually too high for commercial devices. Moreover, the impedances of different individual cells in the stack are acquired with a synchronous measurement reference (global stack impedance). This capability allows distinguishing any singular cell behavior or drift effect of operational parameters (e.g. stack temperature and polarization current).

In ASME 2006 International Mechanical Engineering Congress and Exposition. ASMEDC, 2006. http://dx.doi.org/10.1115/imece2006-13146.

Full text
Abstract:
In-house developed centrifugal compressor performance software was customized for PETROKEMYA (an affiliate of SABIC) Olefins III cracked gas compressor (CGC), the heart of ethylene plant. The software provides live performance analysis of the machine by calculating, per section, polytropic efficiency, polytropic head, and gas power using Lee-Kesler equation of state. Gas analysis is read online to reflect the minute changes in the composition on the calculated parameters. Actual operating parameters are plotted continuously and compared to the performance curves in order to predict possible performance deterioration before it becomes too late. Such information is useful in determining future maintenance requirements based on the actual performance of the machine rather than the traditional time-based maintenance. Moreover, it ensures proper timely action through optimization of wash oil or other chemical injection to help minimize fouling accumulation rate and maximize production and machine availability. Features are introduced to enable performance analysis of the compressor since commissioning to be carried out by click of a button, providing useful information for upgrading and troubleshooting. Unlike its commercially available counterpart with write-protected restrictions, the software is easily modified for additional features that are of interest to Engineers. Features planned for the future include adding intelligent system capability for predicting performance deterioration severity, auto-notifying agents etc. Performance evaluation of a 45 MW, five section process gas compressor before and after turnaround (T/A), clearly revealed the fouling prediction capability of the software.

Full text
Abstract:
This paper presents an investigation of a conventional gas turbine diagnostic process and its generalization. A usual sequence of diagnostic actions consists of two stages: monitoring (fault detection) and subsequent proper diagnosis (fault identification). Such an approach neither implies fault identification nor uses the information about incipient faults unless the engine is recognized as faulty. In previous investigations for engine steady state operation conditions we addressed diagnostics problems without their relation with the monitoring process. Fault classes were given by samples of patterns generated by a static gas turbine performance model. This fault simulation took into account faults of varying severity including incipient ones. A diagnostic algorithm employed artificial neural networks to identify an actual fault. In the present paper we consider the monitoring and diagnosis as joint processes extending our previous approach over both of them. It is proposed to form two classes for the monitoring using the above-mentioned classes constructed for the diagnosis. A two-shaft industrial gas turbine has been chosen to test the proposed integrated approach to monitoring and diagnosis. A general recommendation following from the presented investigation is to identify faults simultaneously with fault detection. This permits accumulating preliminary diagnoses before the engine faulty condition is detected and a rapid final diagnosis after the fault detection.

Abstract:
Maintenance is essential in all kinds of machines. In past, the machine operators would recognize the machine condition by touching the machine or hearing the machine operating sound. However, this is too subjective and not effective for inexperienced operators. In fact, most modern machineries are so complex that many components may run together, making the operator impossible to distinguish the difference between a normal and anomalous machine. Although more scientific fault diagnostic systems are available, they are expensive and difficult to use without comprehensive learning. Therefore, there is a need from industry to have an economy and efficient machine fault diagnostic system. The occurrence of fault must be identified as early as possible to avoid fatal breakdown of machines. The aim of developing the Smart Asset Maintenance System (SAMS) is to provide a portable and comprehensive but low-cost and simple-to-use solution for the industry to perform equipment maintenance effectively.

Abstract:
Pressure-sensitive paint (PSP) is an optical measurement technique based on the photo-chemical reaction between oxygen and luminescent molecules, and has potential as a diagnostic tool for pressure measurement in the high Knudsen number regime. However, the application of PSP to micro flow measurement is not straightforward, because conventional PSPs are too thick owing to their polymer binder. In our previous work, we fabricated pressure-sensitive molecular film (PSMF) by using the Langmuir-Blodgett (LB) technique. In this study, we investigated the temperature dependency of Pt(II) Mesoporphyrin IX (PtMP) based PSMF, and found that the temperature dependency of the pressure sensitivity is very small. Moreover, we have applied PSMF to the pressure measurement of micro gas flows through the 170μm width micro channel and the 100μm width micro nozzle, and the pressure distributions were successfully obtained.

Abstract:
A method for solving the gas path analysis problem of jet engine diagnostics based on a probabilistic approach is presented. The method is materialized through the use of a Bayesian Belief Network (BBN). Building a BBN for gas turbine performance fault diagnosis requires information of a stochastic nature expressing the probability of whether a series of events occurred or not. This information can be extracted by a deterministic model and does not depend on hard to find flight data of different faulty operations of the engine. The diagnostic problem and the overall diagnostic procedure are first described. A detailed description of the way the diagnostic procedure is set-up, with focus on building the BBN from an engine performance model, follows. The case of a turbofan engine is used to evaluate the effectiveness of the method. Several simulated and benchmark fault case scenarios have been considered for this reason. The examined cases demonstrate that the proposed BBN-based diagnostic method composes a powerful tool. This work also shows that building a diagnostic tool, based on information provided by an engine performance model, is feasible and can be efficient as well.

Abstract:
Intro: Undifferentiated dyspnea can be a complicated presentation muddled by patient comorbidities and similar symptomology shared among etiologies. Some studies have shown increased mortality and length of stay in the hospital when incorrectly initially diagnosed in the ED. US has been shown more effective at differentiating these causes and improves diagnostic accuracy. This study will implement US exam upon initial exam of patient and chart time to diagnosis/treatment, length of stay in ED, length of stay in hospital admissions versus discharge rates, and 30 day mortality. ADHF and COPD/asthma patient differentiation will be the focus. Methods: Prospective cohort study of more than 18 years that present with the primary complaint of dyspnea with more than one complicating comorbid condition. Initial exam by physician will be accompanied by cardiothoracic US previously verified. Results: Study powered by previous year average of time to diagnosis of institution. Patient characteristics, distribution by diagnostic category, and characteristics found on US in correlation with diagnosis will be included for multivariate analysis. Conclusions: We expect to see a singificant difference in our time to diagnosis/treatment and mortality rate.
