Abstract:
Konversionsstörungen umfassen nichtepileptische Anfälle sowie sensorische oder motorische Funktionsausfälle (Gangstörungen, Kraftverlust und Lähmung, Sprechstörungen) ohne organmedizinische Ursache. Sie nehmen in klassifikatorischer, ätiologischer und therapeutischer Hinsicht eine Zwischenstellung zwischen den somatoformen und dissoziativen Störungen ein. Bei hohen Raten psychopathologischer Vulnerabilität und Komorbidität können traumatische Erfahrungen, akute Belastungsfaktoren sowie aktuell überfordernde Entwicklungsaufgaben als Auslöser wirken. Besonders typisch ist ein emotional invalidierendes Familienklima, das den authentischen und direkten Ausdruck von Bedürfnissen und Befindlichkeiten des Kindes hemmt. Die Manifestation der Konversionssymptome erfüllt einerseits eine entlastende und verstärkende Funktion, erzeugt andererseits aber neue Belastungsfaktoren wie funktionelle Behinderung, Einschränkungen der sozialen und schulischen Teilhabe, hypochondrische Sorge vor einer unerkannten Grunderkrankung sowie Angst vor Stigmatisierung als Simulant oder psychisch Kranker. Ein Störungsmodell wird vorgestellt, das auch als Grundlage für einen Leitfaden zur klinisch-psychologischen Exploration, Anamneseerhebung, Fallkonzeption sowie das das Procedere der Diagnosemitteilung dient. Patientenorientierte Metaphern werden im Rahmen der Psychoedukation eingesetzt, um die Entstehung der Symptomatik trotz intakter neuronaler Reizleitung für Patient und Eltern nachvollziehbar zu gestalten. Die psychotherapeutische Intervention startet mit einer schrittweisen Wiederaufnahme altersgerechter Aktivitäten und Rollen. Physiotherapie zum Training der geschwächten Muskulatur wird initiiert, um dem Patienten ein Ritual anzubieten, das eine Wiederherstellung der Funktionsausfälle ohne Gesichtsverlust ermöglicht. Psychotherapeutische Strategien im engeren Sinne greifen die vorgegebene Dissoziation des Bewusstseins von der körperlichen Symptomatik auf. Altersgerecht wird dem Kind ein Persönlichkeitsanteil beschrieben, der die Symptomatik überwinden will, ein anderer Persönlichkeitsanteil wird gegenübergestellt, der die Konversionssymptomatik und dessen Anliegen repräsentiert. Der Therapeut entwickelt und moderiert mit dem Kind Lösungen, die geeignet sind, beide Persönlichkeitsanteile wieder neu zu integrieren und damit die Dissoziation zwischen Bewusstsein und körperlicher Symptomatik aufzulösen.

Zusammenfassung. Angelehnt an das transaktionale Streßmodell gibt der vorliegende Beitrag einen Überblick über den Forschungsstand zu Streßerleben und Streßbewältigung im Kindesalter. Nach einer Übersicht über mögliche Belastungsquellen und Belastungssymptomatiken im Kindesalter wird auf das Bewältigungspotential von Kindern eingegangen, wobei deutlich wird, daß es nicht nur darauf ankommt, über ein breites Spektrum an Bewältigungsressourcen zu verfügen, sondern auch darauf, es situationsgerecht einzusetzen. Es folgt eine Übersicht über diagnostische Ansätze zur Erhebung von Streßerleben und Streßbewältigung im Kindesalter, wobei eine Schwerpunktsetzung auf deutschsprachige Verfahren erfolgt. Den Abschluß des Beitrags bildet eine Zusammenfassung von Interventionsverfahren zur Verbesserung der Streßbewältigungskompetenzen von Kindern. Der Schwerpunkt liegt hier auf Interventionsverfahren, die in primär-präventiven Kontexten zum Einsatz gelangen können.

Der Beitrag diskutiert Probleme der raschen Weiterentwicklung von Klinischer Psychologie und der Verhaltenstherapie im besonderen. Dabei werden drei Perspektiven angesprochen: (a) Binnenbeziehungen innerhalb des Fachs Klinische Psychologie sowie zu Nachbardisziplinen, (b) Transferprobleme wissenschaftlicher Erkenntnisse von der Forschung zur Praxis und (c) Probleme der Fort-und Weiterbildung sowie der Qualitätssicherung in der Verhaltenstherapie. Als Beispiele von Fortschritt und Erosion werden diskutiert: (a) die Verhaltensmedizin, als Muster für gut abgestimmte und in die Klinische Psychologie als Fach integrierte Entwicklung, (b) die Gesundheitspsychologie für eine schlechte Interaktionskultur mit mangelhaftem gegenseitigem Informationstransfer und (c) die Psychotherapieszene als Beispiel für Erosionsprozesse in Forschung, Praxis sowie vor allem rort-und Weiterbildung. Der Beitrag fordert eine wesentliche Stärkung des Fachs Klinische Psychologie als fachliche und organisatorische Klammer zwischen den auseinanderdriftenden Entwicklungen. Eine erfolgreiche Übernahme dieser universitär verankerten Koordinations-und Integrations aufgabe erfordert allerdings gleichzeitig auch eine erhebliche Ausweitung personeller Ressourcen und fachlicher Kompetenzen. Eine zentrale neue Herausforderung für klinisch-psychologische Universitätsinstitute besteht auch in der Entwicklung von Qualitätssicherungsmaßnahmen. Der Beitrag empfiehlt in diesem Zusammenhang, vor allem in der Fort-und Weiterbildung den verstärkten Einsatz von Therapiemanualen sowie die lnstitutionalisierung von regelmäßigen Konsensuskonferenzen mit Empfehlungen zur Therapiedurchführung
This paper discusses progress and erosion aspects of c1inical psychology and behavior therapy in Germany from three interrelated perspectives: (a) the relationship of behavior therapy and c1inical psychology to other basic and applied psychological disciplines as weIl as neighboring disciplines, (b) the transfer problems from the scientific fields to practice, and (c) the problem of quality assurance in practice and postgraduate education. Specific emphasis is laid on a discussion of the field of behavioral medicine, as an example for well-integrated and coordinated research and practice activities; health psychology as an example for deficient communication patterns with clinical psychology and behavior therapy, and psychotherapy as an example for erosion in research, education and practice. The paper strongly recommends a more dominant steering role of clinical psychology as the most comprehensive scientific discipline. This steering role, however, would also require a considerably expanded infrastructure of clinical psychology departments in universities together with several mechanisms (competence enhancement, consensus conferences, development of postgraduate education guidelines, quality assurance activities, coordination) to be able to fulfill this mission. The paper also suggests the more frequent use of standardized treatment manuals in postgraduate courses

Abstract:
La présente étude vise à vérifier l'efficacité d'un traitement cognitif-comportemental multimodal pour réduire la détresse psychologique (ie., anxiété et dépression) et certains symptômes psychophysiologiques (i.e., difficultés érectiles, insomnie et fatigue) auprès d'hommes ayant subi une prostatectomie radicale pour un cancer de la prostate localisé. Un protocole expérimental A-B à niveaux de base multiples avec réplications directes et suivis est utilisé. Six participants complètent le traitement. Les analyses de séries chronologiques effectuées sur des indices de détresse psychologique suggèrent des améliorations significatives au post-traitement pour deux des participants sur au moins deux variables et qui se maintiennent au suivi de trois mois. Des tests non-paramétriques effectués sur les questionnaires d'auto-évaluation appuient généralement ces résultats. D'autre part, des bénéfices au niveau de la fatigue et du fonctionnement érectile apparaissent au suivi trois mois. En somme, l'intervention telle qu'administrée dans cette étude semble être efficace pour certains patients, mais pourrait l'être davantage si l'emphase était mise sur le fonctionnement sexuel qui constitue le problème primordial chez cette population.

Questa lavoro di tesi nasce durante il periodo di tirocinio svolto presso la società di servizi Energika srl di Rimini in affiancamento all’Ufficio Tecnico, concentrandomi sull’attività di diagnosi energetica. Dopo una descrizione del panorama energetico europeo e italiano, si sono voluti affrontare sia aspetti più tecnici che altri meno pratici legati all’attività di diagnosi energetiche: gli obblighi normativi, i suggerimenti ministeriali sulla stesura, la figura dell’energy auditor e l’importanza del cambiamento di mentalità per il successo dell’efficienza energetica. La tesi procede poi seguendo lo schema di un audit vero e proprio. Si parte dallo studio del processo produttivo dell’azienda fino alla raccolta delle bollette di fornitura di elettricità e gas naturale. In seguito a tutte le informazioni raccolte, si sono costruiti i bilanci energetici per allocare i consumi ai diversi reparti e identificare quelli più energivori. Una parte importante del lavoro poi è stata la creazione e lo studio di indici di prestazione energetica. Attraverso la creazione degli indici di consumo specifico è stato possibile commentare le performance energetiche di questa azienda con lo scorrere degli anni e in confronto ad altre realtà industriali. Infine, si sono studiate diverse possibilità di efficientamento, sia elettrico che termico, per il sito in esame. Le varie tecnologia innovative proposte sono analizzate e confrontante attraverso un’analisi costi/benefici.

Abstract:
La strategia “20-20-20” ha stabilito per l’Unione europea tre ambiziosi obiettivi da raggiungere entro il 2020: soddisfare il 20% del fabbisogno energetico europeo con le energie rinnovabili, ridurre i gas ad effetto serra del 20% e ridurre i consumi energetici del 20% aumentando l'efficienza energetica. In quest’ottica la diagnosi energetica può svolgere un ruolo importante oltre che nei processi produttivi e nei trasporti, anche negli edifici. Nella mia tesi, sono andato ad approfondire quindi la tematica dell'efficientamento energetico nell'edilizia. A maggior ragione perché il patrimonio immobiliare italiano risulta per buona parte realizzato prima degli anni ottanta, e quindi con un notevole margine di miglioramento. Il potenziale di risparmio energetico è di conseguenza ampio e spesso ottenibile tramite interventi dai ridotti tempi di ritorno. Per studiare le caratteristiche energetiche del sistema edificio impianto vi è la necessità di utilizzare un software di simulazione energetica. Per il mio lavoro mi sono avvalso del SEAS 3.0 (Software Energetico per Audit Semplificati) sviluppato dall'ENEA in collaborazione con L’Università di Pisa (DESTEC), in quanto è il primo software gratuito nazionale, con interfaccia grafica, per le diagnosi energetiche. Ho quindi studiato tale software svolgendo una diagnosi energetica di un Istituto Scolastico Superiore analizzando nel dettaglio il sistema edificio-impianto ottenendo un modello energetico attendibile. Ho così potuto ipotizzare vari interventi migliorativi studiandone il rapporto costi-benefici e producendo un riepilogo delle priorità. Questo per fornire all'amministrazione pubblica un documento che la aiutasse a scegliere in modo consapevole come impiegare al meglio le proprie limitate risorse nell'efficientamento del proprio patrimonio edilizio.

Abstract:
Introduction : La tumeur à cellules géantes (TCG) est une lésion osseuse qui se développe préférentiellement au niveau de l’épiphyse des os longs chez des sujets de 20 à 40 ans, mais exceptionnellement au niveau des maxillaires. D’étiologie inconnue, elle fait partie du groupe des tumeurs osseuses bénignes. Ce groupe nosologique comprend le granulome central à cellules géantes (GCCG), le chérubisme, le kyste anévrismal ainsi que les TCG et les tumeurs brunes liées à l’hyperthyroïdie. L’histologie ne permet pas de poser un diagnostic de certitude entre la TCG et le GCCG. Cependant, les TCG présentent un tableau clinique plus agressif et récidivant. Il existe un risque de transformation maligne en sarcome dans 10 à 20% des cas (Barthélemy 2009) et un fort potentiel métastatique (Martin-Duverneuil 2004). Observation : Le cas rapporté est celui d’une patiente de 28 ans qui présentait une tuméfaction intrabuccale douloureuse de 35mm de grand axe, en distal de 47. Le Cone Beam (CBCT) montrait une lésion osseuse radioclaire sous-jacente de 22mm de grand axe, à proximité d’un apex résiduel de 48. Le diagnostic initial était celui d’un kyste résiduel compliqué d’une cellulite. Le traitement a consisté en une énucléation simple. L’examen anatomopathologique suspectait un granulome périphérique à cellules géantes (GPCG) avec atteinte osseuse. La patiente a été perdue de vue 4 mois jusqu’à la récidive de la lésion. Le nouveau CBCT montrait une lésion ostéolytique de 40mm de grand axe, au niveau de l’angle mandibulaire, envahissant la branche montante avec perforation des corticales et atteinte des tissus mous. Une chirurgie interruptrice mandibulaire en marges saines avec reconstruction par attelle en titane préformée a été réalisée. L’examen anatomopathologique de la pièce d’exérèse n’a pas pu conclure entre GCCG et TCG. La patiente a été suivie 2 ans sans récidive. Discussion : Contrairement au GPCG, le GCCG et la TCG se développent d’abord dans l’os spongieux puis de manière excentrique jusqu’aux corticales osseuses qui peuvent être détruites et aux tissus mous qui sont refoulés ou envahis. Le contenu est mou, de couleur brun-rouge, parfois vacuolaire ou hémorragique. L’examen histologique montre un stroma assez homogène, très vasculaire, contenant, à côté de cellules mononuclées, de grandes cellules multinucléées : les cellules géantes. Le nombre de noyaux serait corrélé avec l’agressivité de la tumeur (Ficarra 1987). Moins fréquente que le GCCG, la TCG est plus agressive. Elle récidive dans environ 50% des cas. La recommandation actuelle est de la traiter par exérèse chirurgicale réglée avec des limites histologiques saines. Le curetage est insuffisant pour prévenir le risque de récidive et de transformation maligne (Barthélémy 2009). Dans le cas rapporté, la forme particulièrement agressive de la tumeur chez cette jeune patiente (récidive en 4 mois avec perforation des corticales et envahissement des parties molles) nous a orienté vers le diagnostic de TCG et un traitement radical de sa récidive. Conclusion : La TCG nécessite un diagnostic précoce et une exérèse en marges saines dès la première intervention afin de diminuer le risque de récidive et d’éviter des traitements plus mutilants.
