Abstract:
Operative dictation training is a poorly studied area of surgical resident education and there is currently no literature on the national experience with operative dictation training in Canada. We therefore undertook a web-based survey of residents and program directors in general surgery training programs in Canada to determine if there is an ongoing desire for formal operative dictation training. Every resident and program director in English speaking General Surgery programs in Canada was contacted by email and asked to participate in the study. A secure, web based survey program was developed to conduct the survey and collect the data. The survey contained questions regarding comfort level with dictation, feedback on dictations and methods of dictation training employed by the individual residents and the resident training programs. Two hundred and seventy-four residents and 11 program directors responded to the survey (70.4% and 78.5% response rate, respectively). Among residents, 201 residents (73.3%) reported that their dictations were in need of improvement while 191 residents (69.7%) requested further training in dictation. Two-hundred and six residents (75.2%) reported that their training program did not employ any formal methods to help improve their dictations. Furthermore, 153 residents (55.8%) had never received feedback on their dictations. Ten program directors (90.9%) felt that residency programs should include formal training in operative dictation. Unfortunately, half of these program directors could not identify any formal methods currently being employed in their training programs, this despite the fact that 45.5% of program directors could identify specific instances where direct patient care was negatively affected by a poor operative report. Both program directors and residents identified operative dictation templates and formal feedback on dictations as the two interventions they would like to see instituted to improve operative dictations in their residency training programs. Abstract:
The purpose of this study was to determine whether the introduction of a Standardized Operative Dictation Template improved the quality of resident dictation. General surgery residents (N = 20) from the University of Manitoba participated in the study. All residents were shown a standardized surgical procedure video and then asked to dictate an operative report. Residents were randomized with the intervention group receiving the Operative Dictation Template. Three months later, all residents were retested. Resident comfort level with dictation and satisfaction with the template was evaluated with a questionnaire. Dictations were evaluated by two blinded independent reviewers using the previously validated SAFE-OR assessment tool. This tool consists of a Structured Assessment Form (SAF) scored out of 45 and a Global Quality Ratings Scale (GQRS) out of 36. Pre and post scores were analyzed using a two-tailed Wilcoxon signed rank test. Subjective comfort level with dictation, as measured by responses on a five point Likert scale, improved significantly in the intervention group (p=0.02). In addition, 90% of residents in the intervention group subjectively reported their dictations improved because of the template. There was no overall difference in the quality of dictation in the intervention group pre and post intervention as measured by the SAF (28.6 vs. 30.0; p=0.36) and GQRS (21.7 vs. 21.8, p=0.96). However, junior resident subgroup analysis revealed statistically significant improvement in the intervention on both the SAF (23.2 vs. 28.3; p=0.02) and GQRS (17.1 vs. 20.4; p=0.02). Senior resident subgroup analysis showed no difference in the intervention group on either the SAF (36.8 vs. 32.4; p=0.07) and GQRS (28.5 vs. 23.9; p=0.07). The Operative Dictation Template can result in a significant improvement in resident comfort level with dictation and has the potential to improve the quality of junior resident dictations. The operative note records surgical indication and pertinent events.In addition, it is a central facet in billing, malpractice lawsuits, research, and future medical planning. However, few residencies have structured education concerning dictation and there is little research on effective techniques for dictation training. The purpose of this study was to evaluate the efficacy of an educational intervention on the dictations of operative cases in which the residents were participants. Two hundred and eighty operative reports were reviewed for the presence or absence of criteria listed in Table 1 and given a score equal to the number of items included. One hundred and forty reports were evaluated before and 140 after an educational intervention. The intervention consisted of a lecture provided by a faculty expert while residents received an instructional card similar to Table 1 as a template. Primary endpoint was dictation score before and after the intervention. The pre- and postintervention scores for all residents were 16.28 and 17.37, respectively (P = 0.001). Junior and senior residents’ preintervention average differed by 1.18 (P < 0.001), however there was no significant postintervention difference. The four most commonly missed data points were the amount of intravenous fluids given, preoperative indications, intraoperative findings, and whether or not a drain was placed. This study used real operative reports to show the benefit of dictation templates for improving resident dictations. These data support previous studies on dictation templates and depict the benefits within one surgical program after implementing a formal plan for dictation education.

Abstract In pronunciation learning, there is a need for resources and tools that help students monitor their speech or provide feedback on errors. While researchers have seen ASR-based technologies as potential tools, little attention has been paid to dictation programs, which have been criticized for low levels of recognition, but offer advantages such as accessibility and flexibility. This study examines two groups of learners in a pronunciation workshop: CONV, which had fully face-to-face instruction, and HYBRID, which had half of the instruction face-to-face and half using the computer, practicing production using a dictation program, Windows Speech Recognition. Results show that both groups improved from pre- to post-test and that there were no statistically significant differences between the two groups. Results indicate that dictation programs may be useful as a complement to face-to-face pronunciation teaching, especially if in-class time for pronunciation teaching is limited.

Ear training and musical literacy (ETML) education is one of the main dimensions of the bachelor degree program of music teacher education departments, which provides professional music education. In ETML education, hearing, sight-reading and dictation studies for Turkish music makams have an important place. In this study, it was aimed to investigate the effects of different instrument sources and tuning systems; namely, equal tempered or traditionally tempered, on the modal dictation performances of students in dictation exercises in the scope of ETML education. This research was conducted with 56 bachelor degree music education students who were taking ETML course. While equal tempered and traditional instruments (piano and kanun) were used as the sound source in the research, examples of equal difficulty level in Huseyni makam scale, which is one of the main makams of Turkish music, were used as dictation material. Both instruments were tuned in accordance with both the equal temperament and the traditional makam system so that four different dictation types were created. As a result of the research, it has been determined that the students are more successful when piano is used in dictation than kanun-dictated trials and in terms of tuning system, the students are found to be more successful with equal tempered system when compared with traditional tuning system. The instrument and tuning interaction was not statistically significant.

The objectives of this study are to find out whether split dictation technique is an effective technique to improve students’ listening skill and whether there is a significant difference between students who were taught by using split dictation and those who were taught by using a conventional method. This study is a quantitative research using quasy experimental as the method of collecting data. The subject of this research was 72 students of the tenth-grade students of SMA N 1 Sukorejo. In obtaining the data, the researcher conducted the pre-test, treatments, and the post-test. The results of this research showed that the mean score of the experimental group is 82,7 and the control group is 77,638. By using IBM SPSS 23th version program, the computation of the t-test which score of the sig (2 tailed) is (0.000) lower than (0.05), and the result of t-value (4.189) is higher than the t-table (1.994). In conclusion, there is a significant difference between the students’ listening test achievement. The students who were taught by using split dictation has higher achievement, this indicated that split dictation is effective enough to improve students’ listening achievement. Abstract:
This paper describes the design and implementation of the deaf and blind communicator with the aim that they can communicate by using a Braille Codes per character and speech recognition using a computer. Communicator device is designed using AT89C51 microcontroller to change the characters that is sent by the computer into Braille code. The signal transmitted from the computer by hearing impairment through RS-232 serial interface with the help of driver IC ULN2803 to drive the solenoid. The end of the solenoid form a pattern of Braille code. Speech pattern recognition used are dictation mode with discrete speech method, whole word, large vocabulary and speaker dependent is designed with SAPI 5.1, Microsoft Speech Engine SDK 5.1 and Delphi software 6.0 to create an application program. On testing who performed indicate that the all the characters that is sent from keyboard can be converted become Braille Character who represented through the solenoid. While the recognition of sound patterns of the microphone most of the well can be translated into the characters displayed on a computer monitor. The success rate in speech recognition can be influenced by several factors such as differences in the sound at the time of training and at the time of dictation, noise from the environment and the quality of the microphone being used.

Abstract:

This project investigates the importance of availability in the scope of dictation programs. Using speech recognition technology for dictating has not reached the public, and that may very well be a result of poor availability in today’s technical solutions.

I have constructed a persona character, Johanna, who personalizes the target user. I have also developed a solution that streams audio into a speech recognition server and sends back interpreted text. Johanna affirmed that the solution was successful in theory.

I then incorporated test users that tried out the solution in practice. Half of them do indeed claim that their usage has been and will continue to be increased thanks to the new level of availability.

Abstract:
Att kunna uttrycka sig skriftligt är en förutsättning för delaktighet i samhället och att kunna utbilda sig inför yrkeslivet. Forskning visar att kompensatoriska hjälpmedel i form av assisterande teknik för individer med läs- och skrivsvårigheter är särskilt viktigt i inlärningssammanhang. Denna studie undersökte likheter och skillnader i elevers textkvalitet och textlängd vid skrivande med tangentbord jämfört med användning av assisterande teknik i form av tal-till-text-program. I studien deltog 41 svenska mellanstadieelever. Resultaten visade att användning av taligenkänningsprogram, varigenom elever får producera text genom att tala istället för att skriva med tangentbord, genererar både längre texter och texter av högre kvalitet. Tal-till-text-program sparade också tid jämfört med skrivande med tangentbord. Utifrån dessa resultat så kan taligenkänningsteknik medföra pedagogiska fördelar. Resultaten diskuteras utifrån tidigare forskning och metodologiska begränsningar. Mer forskning behövs bland annat i syfte att förstå hur långsiktig användning av assisterande teknik kan påverka elevers skrivförmåga.
Being able to express yourself in writing is a prerequisite for academic success and participation in society. Research shows that compensatory aids in the form of assistive technologies for individuals with reading and writing difficulties are particularly important in learning contexts. This study examined similarities and differences in students’ text quality and text length when typing with keyboard compared to when using a particular type of assistive technology in the form of a speech-to-text program. The study comprised of 41 Swedish middle school pupils. The results showed that using speech recognition software, whereby students are allowed to produce text by speaking instead of typing, generates both longer texts and higher-quality texts. Speech-to-text programs were also significantly more time efficient. Based on these results, speech recognition technology can bring educational benefits. The results are discussed based on previous research and methodological limitations. More research is needed, among other things, in order to understand how long-term use of assistant technology can affect students’ writing ability.

The topic of this diploma thesis is Using dictation to develop listening and writing skills in English language classes at the lower secondary level. The aim of the theoretical part is to describe dictation and its contribution to the development of listening and writing skills in English language classes. This part of the thesis also deals with how dictation, listening and writing skills are reflected in the Framework Educational Programme and School Educational Programme. The practical part is based on a project whose goal is to teach a few English lessons based on dictation. A detailed description of each particular lesson is presented in this part. Questionnaires are used to find out the popularity of dictation among pupils and teachers and the frequency of their use in English language classes.

In the activity outlined in this chapter, students will explore some basic functions of the Scratch programming language to learn coding through music. As an introductory exercise, using a pre-programmed musical puzzle, students will sequence existing melodic phrases as well as program two missing phrases to complete the musical sequence. They will explore the internal MIDI functions, play note blocks, and control blocks of Scratch. By engaging in this activity, students can learn critical listening skills through structural/macro analysis, such as chunking, structural dictation, form, pattern recognition, and the ability to hear the slight differences that occur in similar musical phrases.

This chapter describes a small reading intervention program, in the form of participatory action research, which was conceived for African-born, elementary level immigrant children upon a third grader's indication that she did not know the meaning of the word of. The twofold purpose of the intervention was to: (1) uncover the challenges of being from a non-reading culture and being taught by a teacher from a reading culture, and (2) propose ways these challenges could be minimized. The chapter describes in detail the noticing strategies—punctuation discovery, sentence recall, copying, word dictation, etc.—that were used to develop basic reading and writing skills of p-6 French-speaking, African immigrant children. The chapter concludes with a call on educational policy makers to sponsor reading immersion programs for newcomer students, with a recommendation that these courses be taught by qualified immigrant educators, to ensure that these students' school integration process assures success.

Chapter 4 examines the proliferation dispute between the United States and Pakistan. As with the Middle East, averting containment failure in South Asia was the overriding aim of the Ford, Carter, and Reagan administrations. Slowing or halting the clandestine Pakistani nuclear weapons program was always a subordinate goal. The Soviet Union’s invasion of Afghanistan in 1979 was the turning point. Chapter 4 examines the oscillations in US nonproliferation policies toward Pakistan, from the Ford administration’s offer of advanced fighters for nuclear restraint in 1975–1976, to the Carter administration’s imposition of sanctions in early1979, to the Reagan administration’s provision of a $1.4 billion foreign military assistance package and efforts to circumvent nonproliferation legislation in exchange for Pakistani dictator General Muhammad Zia-ul-Haq’s pledge not to cross four nuclear “red lines” from 1981 to 1988, to the George H. W. Bush administration’s resumption of sanctions after the Soviet pullout from Afghanistan in 1990.

This chapter explores how the CIA's Counterterrorist Center (CTC) was established to function as a war room against terrorists, under the leadership of its first director, Duane Clarridge. Though the consequences of the Iran-Contra affair initially tempered the CTC's aggression, the foundations of the agency's post-9/11 role in the War on Terror were laid during this time, and its aggressive pursuit of al-Qaeda today owes much to the approach Clarridge sought to instill in his new department. The origins of the agency's involvement with drones as tools of counterterrorism are also traced back to this period—as early as 1986. Through its classified Eagle program, the CIA first explored the concept of using unmanned aircraft for both intelligence gathering and to conduct lethal precision strikes against targets such as the Libyan dictator and state sponsor of terror, Mu'ammar Gaddafi.

This chapter considers the impact of the Spanish television programme Cine de barrio on popular discourses surrounding national film and performance styles. First airing in 1995, Cine de barrio pairs the viewing of a classic national film (generally made sometime between 1950 and the late 70s, after the death of Spanish dictator Francisco Franco) with a talk show segment between a host and an invited guest. By linking discourses on cultural and historical memory with the subsequent revival of classic national cinema brought about by Cine de barrio, the chapter explores the relationship between actors, their films and their audiences; the affective response produced in this encounter, it argues, generates a nostalgia for classic national cinema that also influences contemporary Spanish film. The chapter also addresses the links between the seemingly disparate Spanish films of the 70s and the comedic box office blowouts of the 2010s, as well as arguing for a sustained reflection on nostalgia, memory, and their connections to acting and performance.

For the 1937 Paris World’s Fair, Pablo Picasso unveiled a vivid portrayal of human suffering entitled Guernica. Inspired by the brutal civilian attack on a small Basque town in northern Spain, the painting became an artistic indictment of fascism, exposing the horrors of the Spanish Civil War to the world. Gernika suffered this major urban trauma on April 26, 1937, when Hitler’s Condor Legion demolished the town at the request of General Francisco Franco, leader of the National Forces and subsequent dictator of Spain. While the town of Gernika lacked the global prominence and political freedom to convey its own account of the bombing, the Picasso painting, in conjunction with the international press, exposed the brutality of the attack. In the ensuing decades, the painting traveled the globe promoting a message of martyrdom and suffering. This international attention occurred in sharp contrast to Gernika’s concealed recovery, which received little publicity under the censorship of Franco’s fascist regime. Franco deliberately targeted Gernika because of its cultural significance to the Basque people, for whom the town symbolized democracy and autonomy. In attacking this town, which held no military or strategic significance, Franco aimed to destroy the symbolic center of Basque self-rule and crush his enemy’s morale. This deadly assault affected the lives of all of Gernika’s citizens, killing many, scattering survivors, and almost completely destroying the town’s physical structure. Following the bombing, the town experienced a precarious recovery. The man who ordered Gernika to be destroyed guided its reconstruction and dictated the rules of the Spanish public realm from 1939 to 1975. Franco simultaneously directed a rapid physical restoration and a strict program of Basque cultural subjugation. The politics of recovery meant that decades passed before town members could publicly mourn or openly place blame. Under these circumstances, recovery in Gernika exhibited distinct physical, emotional, and cultural dimensions. Physical resilience denotes the ability of a city to rebuild its physical structure. It considers how the city was rebuilt, who was in charge of the rebuilding process, and the politics involved in physical reconstruction. Emotional resilience refers to the ability of individuals, families, and communities to cope and heal from trauma.

Minimizing the maintenance costs has become an ever growing topic in the power generation industry. Consequently, OEM’s constantly increase their efforts to exploit the full lifetime potential of capital intensive components, as for instance gas turbine rotors. More than ever, it is essential to have a closed loop from the design knowledge to solid facts about the actual condition of a gas turbine rotor in the field, in order to optimize the utilization and therefore to reduce total lifecycle costs. During initial design an iterative component design process is applied, comprising of analytical calculations and component and engine tests, to design a reliable, robust and maintenance friendly component. Individual component risk profiles (i.e. failure probability and consequences) have to be taken into account during the design phase to cover uncertainties regarding the operating regime, operating conditions and environment, resulting in the minimum expected lifetime. The lifetime of gas turbine components is limited and primarily depends on factors like the operating regime, dictating the mechanical and thermal loads, actual material properties, environmental conditions and the level of maintenance. Those parameters have a scatter and also interact with each other. As a consequence the progress of component degradation varies and the individual component lifetime isn’t known in detail at the time of the design. The purpose of a Rotor Lifecycle Management Program is to detect individual component degradation early in the lifecycle, in order to exploit the maximum rotor lifetime while ensuring safe operation. Periodic monitoring and condition assessment of the individual rotor is the first step, performed during standard type C inspections and comprising of visual inspection, dimensional and advanced non-destructive measurements. Typically the monitoring schedule is based on the operating regime and the operating history of the specific rotor. The combination of the monitoring results with validated analytical design models, extensive material data and the overall gas turbine rotor fleet experience of the OEM, allows proper planning of lifetime extension measures, such as repair and reconditioning activities. Specific repair solutions for both on-site and shop applications have been developed. A rotor lifecycle management program is a „living” program. The monitoring scope and schedule as well as the repair solutions portfolio are continuously optimized and adapted as a result of detailed analysis of operating data and inspection results of the whole global fleet.

Gas turbine engine development continues to accelerate, creating more demanding requirements for abradable seal coatings. These coatings are necessary to provide very small clearances between the rotating and stationary parts in order to minimize gap losses and so Increase efficiency. The relatively few abradable coating materials developed over the last 20 years still perform well in many blade tip seal and labyrinth seal applications. However, rising operating temperatures, corrosion and other environmental changes, longer overhaul times and even better tip clearances are dictating the design of new coating materials which requires a strong scientific approach. For example, ways are being Investigated to replace Nickel-Graphite and other flame sprayed coatings being used between 450 and 700°C respectively because of steady state/corrosion/oxidation/erosion and wear problems respectively. New plasma and HVOF sprayed coatings have been developed using a systematic approach based on material response to operating conditions, minimizing trial and error. The major steps in the programme were: 1. Selection of constituent materials able to withstand service temperatures up to 325 (AISI-Polyester or Polyimide), 450 (AISI base), 700 (MCrAlY base) and 1100°C (ceramic base) respectively. 2. Powder particle manufacture and coating deposition to guarantee highly reproducible coatings. 3. Coating optimization based on wear tests carried out using a fully instrumented abradability test rig and wear mechanism analysis. 4. An investigation of blade tipping systems for high temperature applications. This paper discusses the results of plasma sprayed coatings developed for use at 450 and 700°C.

The accurate prediction of the forced performance of tilting pad journal bearings (TPJB) relies on coupling a fluid film model that includes thermal energy transport, and on occasion fluid inertia, to the structural stiffness of the pads’ pivots and the thermomechanical deformation of the pads’ surfaces. Often enough, the flexibility of both pads and pivots is ignored prior to the bearing actual operation; practice dictating that force coefficients, damping in particular, decrease dramatically due to pivot flexibility. Even in carefully conducted experiments, components’ flexibilities are invoked to explain dramatic differences between measurements and predictions. A multiple-year test program at TAMU has demonstrated the dynamic forced response of TPJBs can be modeled accurately with matrices of constant stiffness K, damping C, and added mass M coefficients. The K-C-M model, representing frequency independent force coefficients, is satisfactory for excitation frequencies less or equal to the shaft synchronous speed. However, as shown by the authors in Ref. [1], pivot flexibility reduces the applicability of the simple constant parameter model to much lower excitation frequencies. Presently, a fluid film flow model predicts the journal eccentricity and force coefficients of a five-pad rocker-back TPJB tested at TAMU under a load-between-pad (LBP) configuration. The predictions agree well with the test results provided the model uses actual hot bearing clearances and an empirical characterization of the pivot stiffness. A study follows to determine the effects of pad preload, r¯p = 0.0, 0.27 (test article) and 0.50, as well as the load orientation, LBP and load-on-pad (LOP), on bearing performance with an emphasis on ascertaining the configuration with most damping and stiffness, largest film thickness, and the least drag friction. In the study, a rigid pivot and two flexible pivots are considered throughout. Further examples present the effective contribution of the pads’ mass and mass moment of inertia and film fluid inertia on the bearing force coefficients. To advance results of general character, predictions are shown versus Sommerfeld number (S), a design parameter proportional to shaft speed and decreasing with applied load. Both LBP and LOP configurations show similar performance characteristics; the journal eccentricity increasing with pivot flexibility. For LBP and LOP bearings with 0.27 preload, pivot flexibility decreases dramatically the bearing damping coefficients, in particular at the low end of S, i.e., large loads. The model and predictions aid to better design TPJBs supporting large specific loads.
