Abstract:
Gordon Cox, who died in 1996 at the age of 90, had two distinct careers. The first, as a crystallographer and structural chemist, lasted from 1927, when he joined Sir William Bragg's group at the Royal Institution, to 1960, when he left the University of Leeds. He was a pioneer of three-dimensional methods in X-ray structure analysis. From 1960 to 1971 he was the highly regarded secretary of the Agricultural Research Council. Cox was born on 24 April 1906 at Pretoria Cottage, Southdown, Twerton, in Somerset, where his father was a market gardener. Three years later his sister Christine was born. A third child, Kenneth, died in infancy. His father, Ernest Henry Cox, was born in 1884. He outlived two wives, married a third in 1941 and died in 1987 at the age of 103. Gordon's mother, Rosina Ring, was twelve years older than his father. They had married at Claverton, near Bath, on 29 April 1905. Rosina, one of many children of a chef, had been sent with two of her siblings to Canada to work on a farm in Ontario in the 1880s. She returned in 1895, and her son Herbert Moffat was born on her return to England. Gordon's family consisted of this elder half-brother, whom he worshipped, and the younger sister, with whom he often quarrelled. His mother often told him of the harsh life in Canada but she had also had some enjoyment there. She died in 1931.

Abstract:
Humans order their world by assigning names, that is, by means of “designation”. We name children, fellow humans, animals and plants according to our respective language. And we can just as easily extinguish a name if it strikes us as the appropriate thing to do. This is the also the task of philosophy: to grasp the world via the right “concepts”, a reduction of complexity through naming. To “grasp” the world also means to master it. In this respect, the assignment of names and titles as well as the strict connexion of a name with a bodily person (identity) is a characteristic means or instrument of domination tied to the modern state.]

Abstract:

Full text
Abstract:
AbstractMargaret Ann Harvey was born on 18 May 1792 in St Peter Port, which is the capital city of Guernsey, the second-largest of the Channel Islands; and died there on 4 April 1903 at the reported age of 110. In this contribution, her exceptional age is thoroughly validated. Considering the data collected on her parents and siblings, there is no possibility of an erroneous linkage, as the name of Margaret and Ann appears only once in the birth records, her family’s birth intervals were narrow, and the dates of death of her siblings have been checked. As she did not have children, her name was not found in civil registration records after her marriage in 1823 until her death in 1903. This lack of records might have made it difficult to prove that the person who died at age 110 in 1903 was the same person who married in 1823 at age 30. Fortunately, she was enumerated in six successive censuses from 1851 to 1901, and a comparison of the ages reported in these censuses and her exact ages shows only minor deviations. Moreover, numerous letters and her numerous diaries help us to follow her life during that long period. Upon reaching age 100, she became famous in Guernsey. Thus, there are many photos of her and press articles about her life. These data support the reliability of the reported chronology of her life events, and thus allow us to validate this exceptional case. Accordingly, we can state that Margaret Ann Harvey Neve is the first documented female supercentenarian. As in the case of recently deceased supercentenarian Emma Morano, her life spanned three successive centuries – albeit one century earlier.

Abstract:
AbstractEmma Morano was born on 29 November 1899 in a small mountain village in Piemonte, and died on 15 April 2017 in Verbania on Lake Maggiore (100 km north of Milano). She was the daughter of Giovanni Morano, a miner; and Mathilde Bresciani, aged 24, a weaver. She was the first child in the family, and her arrival was followed by the births of seven siblings, four sisters and three brothers, all of whom she all survived. On 16 October 1926, Emma Morano married Giovanni Martinuzzi, but they separated a few years later after the death of their child. For more than 30 years, she worked in a jute factory. She then worked for about 20 years in the kitchen of a Marianist boarding school until she retired at the age of 75. After retirement, she lived in a small two-room apartment. In her final years, her hearing and sight were greatly reduced, but she could recognise faces and could communicate when spoken to loudly. She seemed to remember both past events and more recent ones. She had never been hospitalised, but had been treated for gastrointestinal bleeding and for urinary infections. She took no drugs regularly except laxatives. In the archives of four municipalities in the region, we obtained copies of the death certificates of her parents, the birth certificates of all of her siblings, her marriage certificate, and the birth and death certificates of her child. We found no inconsistences in the documents.

Abstract:
Objectives: The objective of our present study was to evaluate the efficacy of radical vaginectomy with or without radical hysterectomy in patients with FIGO stage I and II vaginal cancers. Materials and Methods: A retrospective study was carried out on 13 patients aged 35 – 78 years. All the patients underwent radical surgery for vaginal cancer from April 2010 till June 2015. Kaplan- meier analyses was used to calculate the disease free survival and overall survival at 12 months. Results: The mean age of patients was 54.9 years. Twelve patients were with FIGO stage I while one had stage II vaginal cancer. The histopathology was squamous cell cancer in 9 patients, small cell neuroendocrine cancer in two patients and malignant melanoma in 2 patients. The lesion was confined to upper 2/3 of vagina in 8 cases and lower 1/3 was involved in 5 cases. All the patients underwent radical surgery. Lymph node dissection was done in eleven patients out of whom lymph nodes were positive in 4 patients. Three patients had positive margins. Adjuvant treatment was given to patients with positive margins or positive nodes. Six patients did not require any adjuvant treatment and two patients defaulted adjuvant treatment. One patient developed Vesico-vaginal fistula. Over a follow up period ranging from 6 to 67 months, recurrence developed in two patients and one of them died of disease. The 12 months Disease free survival was 82.1% and 12 months Overall Survival was 90.9%. Conclusion: Stage I and selected stage II vaginal cancer patients have good outcomes in terms of survival and local tumor control if managed judiciously by initial surgery followed by selective adjuvant therapy.

Abstract:
COVID-19 (coronavirus), which emerged in Wuhan, China, spread all over the world in a short time. World Health Organization (WHO) declared a pandemic on the 11th of March 2020, when the first case was seen in Turkey. According to the data of the World Health Organization, the COVID-19 virus has been seen in 114 countries and 4,291 people have died so far. Strict measures began to be taken in our country after the case was seen. First of all, schools were provided for online education transition. Later, entertainment venues were closed, and sports matches were suspended. As a result, during the pandemic period, various events, especially concerts, were broadcasted online on different platforms such as YouTube, Instagram, and Facebook. In this study, online Instagram concerts in the period of April-May 2020, by the time the cases first increased in Turkey, will be examined. The transformation of the concept of space has accelerated with the pandemic period. This transformation acting based on the assumption that music concerts, where concrete space is significant, are separated from time and space with new media technologies. Henri Lefebvre's concept of space is emphasized, and the decentralization of concert venues is explained over the concept of time, and space of flows proposed by Manuel Castells. In the study, a focus group study was conducted with 8 people who attended Instagram concerts in the specified time interval, and the data obtained were compared with the theoretical level and tried to be verified.

Recent data show that those with children at home were more likely to drink heavily during the pandemic (Rodriguez et al., in press). Gaps remain, however, in understanding whether these effects are due to the stresses of homeschooling and whether they extend to cannabis use. Seven-hundred-and-sixty Canadian romantic couples (total N=1520 participants; mean age = 57 years; 50% women) who were self-isolating together during the month of April 2020 were recruited through Qualtrics Panel Surveys. Measures were completed retrospectively in early July 2020; participants were asked to report on their feelings and behavior in April 2020 during lockdown. They completed the GAD-7 (Spitzer et al., 2006) and the PHQ-9 (Kronke et al., 2001) to assess anxiety and depression, brief versions of four subscales of the COVID-19 Stress Scales (Taylor et al., 2020) to assess stress around the pandemic, and the Life Orientation Test – Revised (Chiesi et al., 2013) to assess optimism. They completed a measure of role strain (Statistics Canada, 2015) and a measure of conflict with their partner (Murray et al., 2003). They also completed a validated measure of cannabis use frequency and quantity (Cuttler et al., 2017), as well as two validated items from the Brief Cannabis Motives Measures (Bartel et al., 2020) to assess cannabis use to cope with depression and anxiety, respectively. All measures were completed for a 30-day timeframe during the month of April. Participants also reported on whether they were homeschooling one or more children in Grade 1-12 during the month of April. Data was analyzed with a one-way (homeschooling group) Analysis of Covariance (ANCOVA) controlling for group differences in age; a Bonferroni-correction was applied to account for multiple tests. Compared to those who did not homeschool (n=1116), those who did homeschool (n=404) experienced significantly more depression (p=.001), more COVID-19-related stress around socioeconomic consequences (p<.001) and traumatic stress (p<.001), and less optimism (p=.002). And those who homeschooled experienced more role strain between their home and work responsibilities (p<.001) and more conflict both toward and from their partner (p’s<.001) than those who did not homeschool. Those who homeschooled also used cannabis significantly more frequently in the month of April than those who did not homeschool (p=.003). Compared to cannabis users who did not homeschool (n=122), cannabis users who did homeschool (n=61) reported more frequent cannabis use to cope with both depression and anxiety (p’s = .003). These findings suggest that unintended consequences of our societal viral containment strategies include more depression, pessimism, role strain, inter-parental conflict, and certain COVID-related stresses, and extend to more frequent cannabis use to cope with negative affect, among parents required to homeschool during the pandemic. These unintended mental health and substance misuse consequences for parents need to be considered when planning for an educational strategy in the fall and for any future waves of the pandemic.

Abstract:

Abstract:
This study was conducted in selected provinces of Bicol Region from April 2015 to May 2015 to describe the knowledge, attitudes and practices (KAP) of dog owners regarding rabies and dog bites. A purposive sampling was used in selecting the respondents of the study. Frequencies were tabulated for all variables. Of the 1,200 respondents, 2,193 dogs were recorded making a 2:1 ratio of dogs to householders in this particular study. Among these dogs 58% were vaccinated against rabies. The majority of the ones taking care of the dogs were female (57.3%) but the primary owner (62.9%) was the head of the family. Only 34.7% of the respondents knew that it is their duty to get their pets vaccinated against rabies. Around one-fourth (20.7%) admitted that someone in their household had been bitten by a dog but most respondents (62.5%) did nothing to the dog. The majority (57.7%) of the bite victims were youths (1-14 years old) and almost all (82.7%) of the wound bites were washed with soap and water. Television (44.9%) was the primary source of knowledge about rabies. The majority of participants (67.3%) said that humans are the main end-hosts that can be infected with rabies. Salivation or drooling (42.7%) and craziness (34.2%) were the main signs cited as behaviour of rabid dogs while craziness (40.2%) and hydrophobia or fear of water (25.4%) were cited for rabid humans. Most (33.9%) do not know the source of rabies but the majority (61.8%) believe that vaccination is the main preventive measure against rabies. The majority of participants (63%) reported that the local ordinances regarding rabies in their locality is about the Local Anti-Rabies Act and almost all (93.2%) admitted that vaccination is the most common anti-rabies program of the government.

McKenna, Patrick, and Mark Evans. Emergency Relief and complex service delivery: Towards better outcomes. Queensland University of Technology, 2021. http://dx.doi.org/10.5204/rep.eprints.211133.

Full text
Abstract:
Emergency Relief (ER) is a Department of Social Services (DSS) funded program, delivered by 197 community organisations (ER Providers) across Australia, to assist people facing a financial crisis with financial/material aid and referrals to other support programs. ER has been playing this important role in Australian communities since 1979. Without ER, more people living in Australia who experience a financial crisis might face further harm such as crippling debt or homelessness. The Emergency Relief National Coordination Group (NCG) was established in April 2020 at the start of the COVID-19 pandemic to advise the Minister for Families and Social Services on the implementation of ER. To inform its advice to the Minister, the NCG partnered with the Institute for Governance at the University of Canberra to conduct research to understand the issues and challenges faced by ER Providers and Service Users in local contexts across Australia. The research involved a desktop review of the existing literature on ER service provision, a large survey which all Commonwealth ER Providers were invited to participate in (and 122 responses were received), interviews with a purposive sample of 18 ER Providers, and the development of a program logic and theory of change for the Commonwealth ER program to assess progress. The surveys and interviews focussed on ER Provider perceptions of the strengths, weaknesses, future challenges, and areas of improvement for current ER provision. The trend of increasing case complexity, the effectiveness of ER service delivery models in achieving outcomes for Service Users, and the significance of volunteering in the sector were investigated. Separately, an evaluation of the performance of the NCG was conducted and a summary of the evaluation is provided as an appendix to this report. Several themes emerged from the review of the existing literature such as service delivery shortcomings in dealing with case complexity, the effectiveness of case management, and repeat requests for service. Interviews with ER workers and Service Users found that an uplift in workforce capability was required to deal with increasing case complexity, leading to recommendations for more training and service standards. Several service evaluations found that ER delivered with case management led to high Service User satisfaction, played an integral role in transforming the lives of people with complex needs, and lowered repeat requests for service. A large longitudinal quantitative study revealed that more time spent with participants substantially decreased the number of repeat requests for service; and, given that repeat requests for service can be an indicator of entrenched poverty, not accessing further services is likely to suggest improvement. The interviews identified the main strengths of ER to be the rapid response and flexible use of funds to stabilise crisis situations and connect people to other supports through strong local networks. Service Users trusted the system because of these strengths, and ER was often an access point to holistic support. There were three main weaknesses identified. First, funding contracts were too short and did not cover the full costs of the program—in particular, case management for complex cases. Second, many Service Users were dependent on ER which was inconsistent with the definition and intent of the program. Third, there was inconsistency in the level of service received by Service Users in different geographic locations. These weaknesses can be improved upon with a joined-up approach featuring co-design and collaborative governance, leading to the successful commissioning of social services. The survey confirmed that volunteers were significant for ER, making up 92% of all workers and 51% of all hours worked in respondent ER programs. Of the 122 respondents, volunteers amounted to 554 full-time equivalents, a contribution valued at $39.4 million. In total there were 8,316 volunteers working in the 122 respondent ER programs. The sector can support and upskill these volunteers (and employees in addition) by developing scalable training solutions such as online training modules, updating ER service standards, and engaging in collaborative learning arrangements where large and small ER Providers share resources. More engagement with peak bodies such as Volunteering Australia might also assist the sector to improve the focus on volunteer engagement. Integrated services achieve better outcomes for complex ER cases—97% of survey respondents either agreed or strongly agreed this was the case. The research identified the dimensions of service integration most relevant to ER Providers to be case management, referrals, the breadth of services offered internally, co-location with interrelated service providers, an established network of support, workforce capability, and Service User engagement. Providers can individually focus on increasing the level of service integration for their ER program to improve their ability to deal with complex cases, which are clearly on the rise. At the system level, a more joined-up approach can also improve service integration across Australia. The key dimensions of this finding are discussed next in more detail. Case management is key for achieving Service User outcomes for complex cases—89% of survey respondents either agreed or strongly agreed this was the case. Interviewees most frequently said they would provide more case management if they could change their service model. Case management allows for more time spent with the Service User, follow up with referral partners, and a higher level of expertise in service delivery to support complex cases. Of course, it is a costly model and not currently funded for all Service Users through ER. Where case management is not available as part of ER, it might be available through a related service that is part of a network of support. Where possible, ER Providers should facilitate access to case management for Service Users who would benefit. At a system level, ER models with a greater component of case management could be implemented as test cases. Referral systems are also key for achieving Service User outcomes, which is reflected in the ER Program Logic presented on page 31. The survey and interview data show that referrals within an integrated service (internal) or in a service hub (co-located) are most effective. Where this is not possible, warm referrals within a trusted network of support are more effective than cold referrals leading to higher take-up and beneficial Service User outcomes. However, cold referrals are most common, pointing to a weakness in ER referral systems. This is because ER Providers do not operate or co-locate with interrelated services in many cases, nor do they have the case management capacity to provide warm referrals in many other cases. For mental illness support, which interviewees identified as one of the most difficult issues to deal with, ER Providers offer an integrated service only 23% of the time, warm referrals 34% of the time, and cold referrals 43% of the time. A focus on referral systems at the individual ER Provider level, and system level through a joined-up approach, might lead to better outcomes for Service Users. The program logic and theory of change for ER have been documented with input from the research findings and included in Section 4.3 on page 31. These show that ER helps people facing a financial crisis to meet their immediate needs, avoid further harm, and access a path to recovery. The research demonstrates that ER is fundamental to supporting vulnerable people in Australia and should therefore continue to be funded by government.
