Abstract This personal perspective focuses on small-molecule inhibitors of proteostasis networks in cancer—specifically the discovery and development of chemical probes and drugs acting on the molecular chaperones HSP90 and HSP70, and on the HSF1 stress pathway. Emphasis is on progress made and lessons learned and a future outlook is provided. Highly potent, selective HSP90 inhibitors have proved invaluable in exploring the role of this molecular chaperone family in biology and disease pathology. Clinical activity was observed, especially in non small cell lung cancer and HER2 positive breast cancer. Optimal use of HSP90 inhibitors in oncology will likely require development of creative combination strategies. HSP70 family members have proved technically harder to drug. However, recent progress has been made towards useful chemical tool compounds and these may signpost future clinical drug candidates. The HSF1 stress pathway is strongly validated as a target for cancer therapy. HSF1 itself is a ligandless transcription factor that is extremely challenging to drug directly. HSF1 pathway inhibitors have been identified mostly by phenotypic screening, including a series of bisamides from which a clinical candidate has been identified for treatment of ovarian cancer, multiple myeloma and potentially other cancers.

Gastric acid, hydrochloric acid (HCl), is essential to our digestion. It helps break down proteins, fats, and starches in our food into nutrients such as amino acids and carbohydrates that our body can absorb. Alas, there can be too much of a good thing. Too much secretion of gastric acid for too long can cause heartburn or acid reflux, also known as gastroesophageal reflux disease (GERD). In addition, having too much gastric acid for too long will damage the mucosa of the stomach and may cause ulcer and even stomach cancers! While the exact pathogenesis of ulcers is not known, theories abound. It is still a popular belief that ulcer is caused by stress. Thirty years ago, the discovery of Helicobacter pylori demonstrated that the bacterium contributes to gastritis and peptic ulcers. Regardless of the root of stomach ulcers, they are closely associated with excessive secretion of gastric acid, so that was what early ulcer drugs intended to treat. Dozens of antacids are on the market to neutralize excess HCl: For instance, Mylanta and Maalox contain Al(OH)3 and Mg(OH)2. Tums contains CaCO3. Alka-Seltzer contains NaHCO3 (baking soda). However, antacids only treat the symptoms of excessive acid secretion, and are therefore only effective as a short-term treatment. A better therapy would treat the causation of excessive acid secretion. Histamine-2 receptor antagonists work by blocking the receptors that are responsible for excessive acid secretion: the histamine- 2 receptors. As a consequence, they are more efficacious and have fewer adverse effects in comparison to conventional antacids. The fact that histamine stimulates gastric acid secretion in the stomach was first observed by Popielski in the 1920s. Popielski studied under Pavlov, who won the Nobel Prize (for Physiology or Medicine) in 1904 for his studies in digestive system, but who is now better known for his classical dog-conditioning experiments. As an independent researcher at the University of Kraków, Popielski discovered that histamine was a stimulant of gastric glands, and acted directly without the involvement of vagal nerves. After injecting histamine subcutaneously into gastric fistulas in dogs, Popielski observed copious gastric acid secretion and extremely high acidity in the dogs’ stomachs.

Solar cells convert sunlight into electricity directly. It is a reliable, non-toxic and pollution free source of electricity. Since 19th century researchers have been trying to investigate different materials for solar cell devices. Commercially, Si based solar are predominate in this field, however, with passage of time different materials have been reported. Solar cell techniques are based on three different generations. 1st generation is based on Si and 2nd generation includes thin-films of CuInGaSe, GaAs, CdTe and GaInP etc. whereas 3rd generation is based on organic, hybrid perovskites, quantum dot (QD)-sensitizers &amp; dye-sensitizers solar cells. Among all these, the 3rd generation solar cells are the most efficient and more cost effective than 1nd and 2nd generation solar cells. The 2nd generation is less costly but also less efficient compared to 1st generation. 3rd generation faces degradation of the photovoltaic materials which is a major problem. In this chapter different reported materials since 19th century for solar cells are mentioned. The past and present scenarios of solar cells are discussed comprehensively. It is observed that Si-based and multijunction solar cells dominate the market. Although, theoretically it is reported that hybrid perovskites and quantum dot materials for solar cell are the most efficient materials for photovoltaic PV devices. In spite of the high efficiency the stability of organic, hybrid perovskites, QD-sensitizers &amp;dye-sensitizer materials is a big challenge.

Abstract:
This chapter describes a presurgical protocol for patients with moderate to severe obstructive sleep apnea (OSA) who plan to undergo treatment with upper airway stimulation (UAStim). Patients must receive an initial evaluation including a medical and sleep history and physical examination focused on characteristics suggestive of upper airway narrowing. Criteria related to UAStim therapy and possible exclusion from implantation may be considered at this point. Some patients may be referred to a sleep specialist, but all must undergo in-laboratory or at-home polysomnography to diagnose OSA. Following an OSA diagnosis, treatment with continuous or auto-titrating positive airway pressure should be initiated. Unfortunately, CPAP adherence is low, and while there are several nonsurgical alternatives, many patients who are unable or unwilling to use CPAP will seek surgical treatment. Patients who are referred to otolaryngology for evaluation for UAStim therapy should undergo a medical and sleep history and physical examination including flexible fiberoptic laryngoscopy to evaluate upper airway anatomy. Patients must next undergo drug-induced sleep endoscopy (DISE), during which the upper airway is directly visualized in the operating room with fiberoptic endoscopy under sedation. The most common classification system to describe the location and pattern of upper airway collapse observed during DISE is the Velum, Oropharyngeal walls, Tonsils, Epiglottis (VOTE) system. Patients older than 22 years of age, with an apnea–hypopnea index between 15 and 64 (with central/mixed apneas &lt;25% of the total), a body mass index &lt;32 m/kg<sup>2</sup>, and without palatal complete concentric collapse may be offered UAStim treatment.
