Abstract:
Background. A discoid meniscus is a thickened variant of the normal C-shaped meniscus prone to injury. Discoid medial meniscal tears have rarely been reported within families and may suggest familial or developmental origins.Methods. We report the cases of two Caucasian brothers with symptomatic discoid medial meniscus tears. A literature review was conducted addressing discoid medial meniscus and cases of familial meniscus tears.Case Presentation. Physically active brothers presented with progressively worsening knee pain. MRI revealed medial meniscus tears in both brothers. The family history of medial meniscus tears in their mother and the discoid medial meniscus injuries found on arthroscopy suggested evidence for familial discoid medial meniscus tears.Conclusions. Discoid medial meniscus tears within a family have not been previously reported. Two cases of families with discoid lateral meniscus tears have been reported. Discoid medial meniscus is rare relative to the discoid lateral meniscus and predisposes children to symptomatic tears.

Wahie, Shyamal. "Response to hydroxychloroquine in discoid lupus erythematosus." Thesis, University of Newcastle Upon Tyne, 2011. http://ethos.bl.uk/OrderDetails.do?uin=uk.bl.ethos.580679.

Abstract:
Discoid Lupus Erythematosus (DLE) is the most common form of cutaneous lupus and causes irreversible facial scarring and disfigurement. Hydroxychloroquine is the oral therapy of choice. The factors influencing clinical response to hydroxychloroquine are unclear. Patients with Systemic Lupus Erythematosus (SLE) treated with hydroxychloroquine demonstrate a positive correlation between whole-blood hydroxychloroquine levels and clinical response. Variation in response to hydroxychloroquine might, in part, be due to genetic differences in the CYP450 genes responsible for hepatic metabolism of hydroxychloroquine between individuals, which therein could influence whole-blood hydroxychloroquine levels. The influence of genetic polymorphisms on the therapeutic response to hydroxychloroquine, in comparison with host or disease attributes such as smoking status or disease extent, is unknown. A multicentre retrospective cohort study was conducted in 200 patients with DLE treated with hydroxychloroquine. The outcomes of interest were: (i) the clinical response to hydroxychloroquine by 6 months and (ii) the effects of disease attributes, smoking, and CYP450 polymorphisms on clinical response. Although the majority of patients responded to hydroxychloroquine, a significant proportion (39%) either failed to respond or was intolerant of the drug. Cigarette smoking and CYP 206 and 2CB genotypes did not have any Significant influence on response to hydroxychloroquine. Multivariate analysis indicated that disseminated disease and concomitant SLE at baseline were both Significantly associated with lack of response to hydroxychloroquine. These findings suggest that baseline lupus severity and SLE are important predictors of response to hydroxychloroquine. A prospective study is required to further explore the response of DLE to hydroxychloroquine and to investigate the relationships between disease activity and eventual response. A validated scoring instrument was designed to specifically measure activity and damage attributable to DLE and will be essential for quantifying disease and appraising responsiveness to hydroxychloroquine in future prospective studies. Following peer review, pilot work and two reliability studies, the Score of Activity and Damage in DLE (SADDLE) was developed as a measure of disease severity in DLE. Activity is measured using erythema, induration and scaling and damage is represented by scarring and dyspigmentation. SADDLE demonstrates evidence of inter and intra-rater reliability and construct validity. In summary, these inter-related studies not only indicate that 1 in 3 patients with DLE do not respond to hydroxychloroquine by 6 months, but also show that lack of response may be associated with disease extent and concomitant SLE at baseline. Cigarette smoking did not influence response, casting doubt on the results of previous small single-site retrospective studies, which did not assess confounding variables such as disease extent and SLE and suggested smoking was associated with poor response to therapy. A disease activity score has been developed for future use in prospective trials to help investigate these findings further.

Abstract:
BACKGROUND: Scarring alopecias are classified into primary and secondary according to the initial site of inflammation. In primary scarring alopecias, the hair follicle is the main target of destruction. The term secondary cicatricial alopecia implies that follicular destruction is not the primary pathologic event. AIMS: To review the histopathologic diagnoses of cases of cicatricial alopecia in order to classify them according to the North American Hair Research Society. PATIENTS AND METHODS: Patients with biopsy specimens diagnosed as cicatricial alopecia seen from 2000 to 2005 at the Dermatologic Department of Hospital das Clínicas, São Paulo University Medical School had hematoxylin and eosin, Periodic Acid-Schiff (PAS) and Weigert stained slides reevaluated and sub-typed into different primary cicatricial alopecias. RESULTS: Thirty-seven cases of primary cicatricial alopecias were reclassified as: discoid lupus erythematosus (16), lichen planus pilaris (4), pseudopelade of Brocq (12), folliculitis decalvans (3), dissecting folliculitis (1), and non-specific scarring alopecia (1). CONCLUSIONS: Even in late, pauci or noninflammatory phases, an approach with systematic evaluation of a constellation of criteria in routine hematoxylin and eosin stain, PAS and Weigert stain permitted an accurate diagnosis of cicatricial alopecias

Abstract:
Abstract: Osteoarthritis (OA) affects 250 million people worldwide. Currently, no targets for disease-modifying osteoarthritis drugs exist. Matrix metalloproteinase-13 (MMP-13) would make it an ideal target; however, its broad biological effects restrict its application as a target enzyme of inhibitory drugs in the treatment of OA. The expression and activation of discoidin domain receptor 2 (DDR2) is increased in human OA tissues and mouse models of OA and was co-localized with elevated expression of MMP-13 in degenerative articular cartilages. In healthy articular cartilage, DDR2 is kept inactivated by the pericellular matrix, which separates the receptor from its ligand, type II collagen. Once enzymes capable of degrading the pericellular molecules expose chondrocytes to type II collagen, DDR2 is activated and induces expression of MMP-13 leading to degradation of type II collagen and proteoglycans resulting in joint destruction and OA. We tested the hypothesis that complete removal of Ddr2 from the knee joint of mouse adult articular cartilage can delay progression of osteoarthritis prior to or after initiation of articular cartilage degeneration. To accomplish this goal, conditional knock out techniques were used with Aggrecan-CreERT2 mice and floxed Ddr2 mice, Ddr2 was removed from articular cartilage of knee joints in mice at 8 weeks of age via intraperitoneal Tamoxifen injection (2mg/10g body weight) for 5 consecutive days (Group A). Mice were subjected to destabilization of the medial meniscus (DMM) or sham surgery at 10 weeks of age. An additional experimental group was subjected to DMM or sham surgery at 10 weeks of age and then DDR2 was removed by intraperitoneal Tamoxifen injection 8 weeks later (Group B). Knee joints from mice in Group A and their corresponding controls were harvested at 8 weeks or 16 weeks post-surgery and mice from Group B and their controls were harvested at 16 weeks post surgery. Histology was performed and the OARSI Modified Mankin Score was used to evaluate articular cartilage degeneration. Statistically significant differences were determined via T-test. We found the average modified score for Group A 8 week control was 1.64 (n=7) whereas with Ddr2 removed was 0.64 (n=7) [P<0.05]. 2) The average modified score for Group A 16 week control was 4.67 (n=7) and with Ddr2 removed was 1.27 (n=9) [P<0.05]. 3) The average modified score for Group B was 1.1 (n=5). In conclusion, conditional removal of Ddr2 in articular cartilage attenuated articular cartilage degeneration in mature knee joints of mouse models of OA.

Introduction: Age at diagnosis of childhood-onset systemic lupus erythematosus (cSLE) may influence disease expression in terms of initial clinical presentation, pattern of organ involvement and serological findings. Objective: To evaluate demographic data, clinical and laboratory features at disease diagnosis in three different age groups of childhood systemic lupus erythematosus (cSLE): group A early-onset ( < 6 years), group B school age ( >= 6 and < 12 years) and group C adolescent ( >= 12 and < 18 years). Methods: Brazilian multicenter cohort retrospective study in 10 Pediatric Rheumatology centers, including 847 cSLE patients. Results: Patients were divided in three groups: A with 39 (4%), B 395 (47%) and C 413 (49%). Of 39 cSLE patients of group A, 3 (8%) were < 2 years, 4 (10%) >= 2 to < 3 years and 32 (82%) >= 3 and < 6 years. Seventy-four cSLE patients were analyzed for C1q levels and complete C1q deficiency was observed in 3/74 (4%), all of them of group A. Groups were similar regarding high frequencies of female gender, nephritis, neuropsychiatric involvement, SLEDAI-2K ( >= 8), autoantibody profile, elevated acute phase proteins and low complement levels (p > 0.05). However, the frequency of fever (78% vs. 61% vs. 47%, p < 0.0001), hepatomegaly (42% vs. 29% vs. 14%, p < 0.0001), splenomegaly (28% vs. 12% vs. 4%, p < 0.0001) and discoid lupus (13% vs. 4% vs. 4%, p=0.020) was significantly higher in the group A compared to groups B and C. The frequency of weight loss > 2kg (19% vs. 28% vs. 36%, p=0.017), photosensitivity (34% vs. 41% vs. 51%, p=0.006), leukopenia < 4,000/mm3 (14% vs. 25% vs. 30%, p=0.048) and lymphopenia < 1,500/mm3 (22% vs. 41% vs. 47%, p=0.011) was significantly lower in the group A. Conclusions: Our large multicenter study identified that the initial presentation of cSLE is characterized by comparable high frequency of internal organ involvement and some distinct clinical and laboratory features in early-onset and adolescent groups

The use of topical medications, such as clobetasol propionate (CLO) and tacrolimus (TAC) is the main treatment for inflammatory skin diseases including discoid lupus erythematosus (DLE). The DLE is a chronic disease that affects the skin and its treatment can be improved with the simultaneous use of these drugs. However, this condition is difficult to treat topically because of the thickening of the stratum corneum (SC). New formulations such as lipid nanoparticles and the use of permeation enhancers can improve drug penetration in the skin and improve the topical treatment of different pathologies. Therefore, the aim of this study was to develop and characterize nanostructured lipid carriers (NLC) containing TAC and CLO and NLC coated or not with chitosan oligosaccharide (CO), in order to increase the skin permeation and retention of drugs. NLCs were prepared by the dilution of the microemulsion and were subsequently coated with OQ. The carriers obtained contained both drugs or TAC or CLO separately. The formulations obtained were evaluated for average diameter and polydispersity index (PDI), zeta potential, drug recovery (DR), encapsulation efficiency (EE) and drug loading (DL). The drug penetration from the NLC was assessed in pig ear skin in Franz cells. Co-encapsulated NLCs showed an average diameter of 143.3 nm and PdI of 0.264. The particles showed negative zeta potential of about -40 mV. After coating with OQ, NLCs showed a significant increase in diameter (311.9 nm) (p <0.05) and positive zeta potential (24,4mV) due to the adsorption of CO on the surface of the NLC. Co-encapsulation of the drug was effective, EE was greater than 90% for both TAC and CLO. In studies of electron paramagnetic resonance (EPR) it was observed that the drugs slightly modified the dynamics of lipids in NLC on the surface of the matrix (5-DSA). There was a significant increase in the 2A// parameter, the TAC+CLO-NLC (47,1G) compared to NLCs with no drug (45,6G). When the samples were coated with CO, the lipid dynamic modified considerably. The 2A// values increased from 45.5 to 50.1 in the NLC and 46.5 to 50.7 in the TAC+CLO-NLC. The CO probably forms a frame around the nanocarriers which significantly reduces the lipids mobility. The encapsulation of the drugs increased penetration of both TAC and CLO to the skin layers when compared to non-encapsulated drugs. The NLC coated increased the amount of TAC retained in the deeper layers of the skin (approximately 1.8 times more drug). For the CLO, the coating favored retention in the EC (1.7 fold) and on the remaining skin (3 times more drug) as compared to uncoated particles. When co-encapsulated with TAC and CLO, TAC’s penetration was superior compared with the particle containing only TAC. Thus, this strategy has shown to be promising to increase TAC’s skin penetration , which is a drug that hardly passes through the SC and promote greater retention of CLO in the upper layers of the skin. The results suggest that the co-encapsulated system is a potential formulation for skin drug delivery of the drugs.

Planetary Sciences are constantly progressing. Current facilities have allowed the discovery of a considerable amount of exoplanets and a more detailed study of protoplanetary disks, which is where planets are thought to form. Among the important tracers of disk evolution is the measurement of physical properties of these protoplanetary disks. This thesis focuses in two main parts; the first one in the diagnostic of gas line emission; while the second explores disks during even earlier stages of planet formation going through outbursts of accretion. In Chapter 2, we describe a line emission diagnostic that allows the measurement of temperature, turbulent broadening and the column density (particle number per unit surface) in protoplanetary disks. We test our line emission diagnostic through detailed analysis on post processed hydrodynamical simulations of a protoplanetary disk with an embedded giant planet. Our diagnostic could potentially allow the detection of kinematical signatures in line emission of planet-disk interactions, such as the Rossby wave (RWI) vortices associated with giant planet formation. We could conclude that our line emission diagnostic measures a kinematical signature in the disk, although very small. When the line emission diagnostic is applied to the hydrodynamical simulations, the measured turbulent velocity shows a small enhancement for optically thick molecular transitions at the location of the vortex. In Chapter 3, we explored an intriguing class of circumstellar objects, FU Orionis objects. FU Ori objects have outbursts with high accretion rates. They could solve the low luminosity problem in the formation of low mass stars. It is believed that low mass stars go through this phenomenon more than one time during their formation. Chapter 3 addresses how accretion outbursts shape the radial structure of the circumstellar disk in V883 Ori, a FU Ori object. With our approach, we measure its temperature and how fast it accretes. We use a self-iterative algorithm that takes the radial profile of a circumstellar source and returns its shape and how much it is accreting, separating optically thin with optically thick regimes. From our analysis of V883 Ori, we find that passive disk heating mechanisms do not reproduce the steep thermal profile of V883 Ori closer to the star than r=30 AU. Thus, we conclude that viscous dissipation provides the supplemental amount of energy necessary to generate the emission profile observed in V883 Ori.

Abstract:
Flow fields around an oscillating airfoil at the low Reynolds number region are extremely unsteady because the change direction of leading edge produces unsteady vortex motions. Studies of unsteady propulsion system of birds, insects, and fish are few and inconclusive. It has been noted that the unsteady fluid force plays an important role in biological flight. To evaluate the force correctly, it is necessary to know the unsteady properties determined from the vortex dynamics. The actual motion of a hand in swimming is obviously unsteady, and time-dependent fluid forces must be considered, because a quasi-steady-state approach towards predicting the fluid forces acting on a hand under unsteady conditions yielded errors. The three-dimensionality and the unsteady effect of a hand must be important to the estimation of the fluid forces acting on a swimmer’s hand. The purpose of this study is to investigate the relationship between unsteady fluid forces and vortex structures for a three-dimensional airfoil during the pitch-oscillating motion. Particle image velocimetry (PIV) was used to aid in understanding the flow field in the near field of the airfoil edge for the wind tunnel test. It is confirmed that the unsteady fluid forces were affected by the vortices shed from the airfoil edge during up-stroke in pitching oscillation. There are two peaks in the fluid force during one pitch-oscillating due to the vortex behavior. The vortex behavior was strongly affected by the reduced frequency, and the fluid force acting on the airfoil model increases with increasing the reduced frequency.

Full text
Abstract:
Platelet aggregation kinetics, according to the particle collision theory, generally assumed to apply, ought to conform to a second order type of rate law. But published data on the time-course of ADP-induced single platelet recruitment into aggregates were found not to do so and to lead to abnormal second order rate constants much larger than even their theoretical upper bounds. The data were, instead, found to fit a first order type of rate law rather well with rate constants in the range of 0.04 - 0.27 s-1. These results were confirmed in our laboratory employing gelfiltered calf platelets. Thus a mechanism much more complex than hithertofore recognized, is operative. The following kinetic scheme was formulated on the basis of information gleaned from the literature.where P is the nonaggregable, discoid platelet, A the agonist, P* an aggregable platelet form with membranous protrusions, and P** another aggregable platelet form with pseudopods. Taking into account the relative magnitudes of the k*s and assuming aggregation to be driven by hydrophobic interaction between complementary surfaces of P* and P** species, a rate equation was derived for aggregation. The kinetic scheme and the rate equation could account for the apparent first order rate law and other empirical observations in the literature.
