Full textOropharyngeal dysphagia (OD) is the difficulty in forming and / or moving the alimentary bolus safely from the mouth to the oesophagus. Despite its high prevalence among the older population, and its association with severe respiratory and nutritional complications, both OD and malnutrition (MN) are underdiagnosed and undertreated disorders among this group of patients. The first hypothesis of this thesis is that the prevalence of OD and MN can be established using clinical assessment and is high among older patients. We assessed a cohort of older patients admitted to a geriatric acute-‐care unit, using the volume-‐viscosity swallow test (V-‐VST) for dysphagia and the Mini Nutritional Assessment (MNA) for malnutrition. The second hypothesis is that the study of OD and MN of these older patients using videofluoroscopy and the evaluation of nutritional status would allow us to advance in the knowledge of their pathophysiology and to develop new nutritional treatments addressed to their needs. The first objective was to evaluate the prevalence and prognosis of OD and MN in the older hospitalized population. We evaluated the prevalence of OD and MN in 1662 patients (≥70 years) admitted to an acute-‐care geriatric unit of a general hospital. We found a higher prevalence of OD than of MN (47.4% vs 30.6%, p <0.05) and a higher prevalence of MN in patients with OD (45.3% vs 18%, p <0.001), OD being an independent risk factor for MN in the multivariate study (OR 2.31 (1.70-‐3.14)) Both OD and MN patients had an increase in in-‐hospital mortality and one year after discharge (p <0.05), the worst prognosis being for patients with both conditions, with a mortality rate of 65.8% per year. The second objective was to describe the nutritional characteristics of these patients both in a chronic situation and during an acute process. We studied 95 patients (≥70 years old) with OD secondary to neurological processes or to ageing, 23 with pneumonia, and 15 without OD. Our results show that patients with chronic obstructive pulmonary disease have a reduction of the muscular and fatty compartment, together with reduced body weight. Chronic patients at risk of malnutrition or malnourished had poorer swallowing function with greater need for thickeners to adapt the liquids to nectar viscosity for safe swallowing, and more residue at pudding viscosity. The group of patients with pneumonia presented a similar nutritional status as patients in a chronic situation with a greater decrease in serum protein influenced by the inflammatory response associated with the infection. The prevalence of nutritional risk (MNA<17: 33.7% in patients with chronic conditions and 56.5% in patients with pneumonia), dehydration with decrease of intracellular water (100%) and sarcopenia (16% and 29%, respectively) was very high in both groups of patients. Finally, we designed nutritional treatment with two normocaloric (1700-‐ 1800 kcal/day) and normoproteic (70gr/day) diets with two rheological adaptations, easy mastication and mashed, following the traditional diet and adaptation of the fluids according to the V-‐VST, all easily reproducible at patients' homes. The studies in this doctoral thesis confirm that OD and MN are highly prevalent conditions in older patients, that OD is a risk factor for MN, and that both conditions severely affect prognosis. The application of diets based on the traditional diet, according to nutritional status, and the adaptation of fluid viscosity according to swallowing capacity are essential in the management of older patients with OD.
