Abstract:
Chinese medicine is a time-honored and deeply valued medical skill in the Chinese community. Although currently the scientific Chinese medicine powder is prevalent, its varieties are numerous and have similar appearance, color, and odor, resulting in difficulties in being distinguished. Moreover, dosages are still weighed and mixed manually, rather than being automated; therefore, the workload for each Chinese medicine pharmacist is heavy, leading to it being easy for the wrong medicine to be administered due to human negligence. An innovative shelf-based automatic powder dispensing machine is therefore presented in this paper in order to promote the quality and efficiency of powder dispensing. This machine consists of linear guide mechanisms, weighing mechanisms, retractable mechanisms, gallipot base discharge mechanisms, and a lifting platform mechanism, and offers several innovative or important features that are beneficial to Chinese medicine. It can automatically and accurately dispense different powders in the correct weights, reducing the possibility of human error. Each gallipot is equipped with one low-cost gallipot base, which can be controlled by the weighing mechanism to discharge powder with the desired weight. Powder cross-contamination among different gallipots can also be avoided. Stepping motors are used in the presented machine for easy control, low cost, and durability. A parabolic type of S mode speed control for stepping motor is implemented, providing faster and more stable moving speed and accuracy than the traditional trapezoidal mode. Hardware and software are integrated and tested. The error of the weighed powder is reduced to +/−0.2 g. The dispensing process is very efficient, the moving speed of the weighing mechanism reaches 23.89 cm/s, and the discharge speed is up to 0.22 g/s. The system is indeed able to improve the accuracy and efficiency of powder dispensing for Chinese medicine.

Abstract:
Abstract Purpose Hospital emergency medication kits (HEMKs) are used to provide certain critical medications in emergent situations, despite many technological advancements for patient safety and medication distribution. We sought to evaluate HEMK usage and analyze associated costs to identify and recommend process improvements. Methods Mayo Clinic in Rochester, MN, is a large multisite academic medical center with 2 hospital campuses and many ambulatory clinics. All documentation of the approximately 250 HEMKs in circulation was analyzed from January to November 2017. The primary outcome was HEMK use. Secondary outcomes included individual medication usage and associated costs. These data were then used to recommend process improvements. Results Of 880 HEMKs evaluated, 675 (76.7%) were used, resulting in expiration 23.3% of the time. A total of 1,024 emergency medications were used, most commonly for hypoglycemia. Many of these medications are also available in automated dispensing machines for patient care use. Cost analysis revealed an average annual cost of nearly $200,000 associated with HEMKs. The results of our analysis indicated little added benefit of HEMKs in the setting of automated dispensing machine optimization. Steps for HEMK retirement are described. Conclusion HEMKs offered little added benefit considering technological advancements that have been made in patient safety and medication distribution since their inception. Retirement of HEMKs is anticipated to increase pharmacy operational efficiency by using automated dispensing machine technology and appropriate emergency response protocols to ensure optimal patient care.

Abstract:
The number of deaths due to medical errors in hospitals ranges from 44,000 to 98,000 yearly. More than 7,000 of these deaths have taken place due to medication errors. This project evaluated the implementation of an automated medication dispensing cabinet or PYXIS machine in a 25-bed upper Midwestern critical access hospital. Lewin's stage theory of organizational change and Roger's diffusion of innovations theory supported the project. Nursing staff members were asked to complete an anonymous, qualitative survey approximately 1 month after the implementation of the PYXIS and again 1 year later. Questions were focused on the device and its use in preventing medication errors in the hospital. In addition to the surveys that were completed, interviews were conducted with the pharmacist, the pharmacy techs, and the director of nursing 1 year after implementation to ascertain perceptions of the change from paper-based medication administration to use of the automated medication dispensing cabinet. Medication errors before, during, and after the PYXIS implementation were analyzed. The small sample and the small number of medication errors allowed simple counts and qualitative analysis of the data. The staff members were generally satisfied with the change, although they acknowledged workflow disruption and increased medication errors. The increase in medication errors may be due in part to better documentation of errors during the transition and after implementation. Social change in practice was supported through the patient safety mechanisms and ongoing process changes that were put in place to support the new technology. This project provides direction to other critical access hospitals regarding planning considerations and best practices in implementing a PYXIS machine.

碩士
國立聯合大學
電機工程學系碩士班
107
Chinese medicine is a time-honored medical skill domestically and is deeply emphasized in Chinese community. Nevertheless, the number of Chinese medicine pharmacists is obviously fewer than the number of modern medicine pharmacists. Although there is Chinese patent medicine (powder), which is more convenient for dispensing, the dosage is still weighted and mixed manually, rather than being automated. This study aims to improve the Chinese medicine dispensing environment by designing automation equipment and developing a warehouse automated powder dispensing machine to complete the process from physicians’ descriptions to patients’ medicine receiving with automation. It would ease the dispensing workload of Chinese medicine pharmacists and dispense each pack with accurate amount of powder to promote the dispensing quality. The double-layer warehouse structure designed in this study allows the weighing mechanism horizontally moving and positioning to the front of an assigned gallipot through a microcontroller controlling the linear guide motor on each layer. The discharge mechanism is then activated for the powder dropping down to the spoon container of the weighing mechanism. The weighing element then measures the weight of powder to the assigned weight to stop discharge. The spoon of the weighing mechanism then returns to the mixing tank for pouring the powder. Aiming at the warehouse automated powder dispensing machine, the hardware and software systems are designed and integrated in this study. The actual measurement proves that the system could automatically position to the assigned gallipot for automatic discharge and dispensing, according to the medicine and weight on the input prescription, and pour the powder into the mixing tank. The powder weight measured by the system and the actually measured weight appear the error within ±0.5g, the horizontal moving speed of the weighing mechanism reaches 21.79 cm/s, and the discharge speed is up to 0.22 g/s. This study makes contributions to the automation of Chinese medicine powder dispensing.

Abstract:
碩士
國立臺北科技大學
工業工程與管理系EMBA班
105
With the coming of industry 4.0, intelligentization and automatization have become future trend in manufacturing. The industry of dispensers, which plays a very significant role in manufacturing, has gradually evolved from manual dispensers to semi-automatic ones, and to the current full-automatic ones. The price of a qualified intelligent machine is usually high. The objective of the study is to help companies to choose a better solution from using semi-automatic dispensers or using full-automatic ones through the analysis of operating cost, productivity, and labour cost. The study compares the productivity of semi-automatic dispensers with full-automatic ones from the same period and then finds out their differences. Through adopting the line of semi-variable cost to form a simple cost model, the cost difference of semi-automatic dispensers and full-automatic ones can be calculated based on depreciation of machines and labour cost. In the end, by combining the differences of cost and productivity, the selection of using semi-automatic dispensers or using full-automatic ones will be analyzed accordingly.

Abstract:
This master thesis results from the work developed in an internship at the company ITEC - Iberian Technical, located in Frossos, Braga. This thesis describes the development of a semi-automatic equipment to dispense grease in a rib of a polymeric product. First a bibliography research was conducted to identify mechatronic project methodologies and also topics related to practical issues about equipment project. The study of concepts related with design for manufacturing and assembly, equipment safety by machine directive 2006/42/CE and ergonomic concerns reveals essential to this mechatronic project. This work was based on the design project methodology in which first specifications, objectives and functions are defined and then solutions are idealized and evaluated. After that, the conceptual solution is detailed to obtain a result possible to be executed and is based in the concept developed. This also involves the selection of off-the-shelf components and the dimension of the components related to which assembly group, such as: product housing, dispensing tool and structure. All of these components were projected considering the norms defined by design for manufacturing and assembly, equipment safety and ergonomics. In order to verify if the dispensing tool and the needle are in the correct position, if the needle squeeze is complete and if it is the right size, a vision system prototype was developed. The results of the measurements of the needle size and the needle squeeze show the high precision that this system can have. After manufacturing and assembling, the equipment was tested. These tests revealed project errors that should have been avoided, however, all of them were immediately corrected and the specifications defined by the client were achieved. After the equipment was sent to the costumer, it was noted that the amount of grease dispensed was decreasing with time. This problem was solved by replacing the valve by a recent model. Thus, we can conclude that the specification of the off-the-shelf components sometimes does not match what is specified by the provider.

Abstract:
This chapter investigates the history of the ubiquitous yet banal Automated Teller Machine, or ATM. There is no single inventor of the ATM. Rather, it emerged through innovation around the globe and across the industry. In order to build a successful ATM system, engineers and bankers had to overcome challenges that ranged from security and authorization to weather-proofing electronics. This chapter surveys some of those developments. Increasingly, ATMs are being designed to offer a variety of services beyond dispensing cash. In the future, the ATM may prove to an important site of automated retail banking and consumer financial services.

Abstract:
Since late 2019, a novel Coronavirus disease 2019 (COVID-19) has spread globally. As a result, businesses were forced to send their workforce into remote working, wherever possible. While research in this area has seen an increase in studying and developing technologies that allow and support such remote working style, not every sector is currently prepared for such a transition. Especially the manufacturing sector has faced challenges in this regard. In this paper, the mental workload of two groups of participants is studied during a human-robot interaction task. Participants were asked to bring a robotised cell used in a dispensing task to full production by tuning system parameters. After the experiment, a self-assessment of the participants’ perceived mental workload using the NASA Task Load Index (NASA-TLX) was used. The results show that remote participants tend to have lower perceived workload compared to the local participants.

Abstract:
This chapter covers how, from the very beginning, the Law and Justice (Prawo i Sprawiedliwość (PiS)) party targeted five democratic institutions as obstacles to its capture of the state. First, it looks at the parliamentary opposition being marginalized and effectively silenced. Second, it details how the civil service has been subjected to political control, and the principles of professionalism and neutrality in the civil service have been abandoned. Third, the chapter looks at the public media being transformed into a propaganda machine for the ruling party, and the commercial media being threatened with laws that would constrain their operation. Fourth, it considers how electoral commissions have been effectively subjected to the executive, which renders massive electoral frauds possible. Finally, the chapter looks at how institutions dispensing grants and subsidies to civil society organizations have been centralized and subjected to the executive. All these institutions provide a buffer for the population from the arbitrary will of politicians, which is why PiS could not tolerate them.
