Gertrude Stein's 1909 extended textual portrait of Picasso at once tries to capture the artist anew, as he has never been rendered before, and resists closure and flatness of meaning. It is multi-faceted, cubist, repetitive but never static, trying to arrive at the essence of the artist, but acknowledging the slipperiness of its objective. In writing a portrait, biography or monograph, the author's challenge is to apprehend a subject and to distil the themes and techniques of his/her life and work, but without reducing that person to a one-dimensional outline. In 2008, a new treatment modality for esophageal achalasia was introduced—peroral endoscopic myotomy (POEM). POEM is a procedure performed endoscopically, which allows transection of the muscular fibers of the distal esophagus and of the lower esophageal sphincter. The procedure is therefore similar to a laparoscopic Heller myotomy without a fundoplication. Short-term studies have shown that POEM is very effective in relieving dysphagia and regurgitation, but concerns have been raised about the incidence of post-POEM gastroesophageal reflux. Achalasia is a rare primary esophageal motility disorder characterized by failure of the lower esophageal sphincter (LES) to relax and absence of normal peristalsis in the esophagus. Patients typically present with progressive dysphagia to both solids and liquids, regurgitation, chest pain, and weight loss. Diagnosis is confirmed by high-resolution manometry demonstrating incomplete LES relaxation and aperistalsis, with supportive findings on barium esophagram (e.g. “bird’s beak” narrowing) and endoscopy to exclude pseudo achalasia. Achalasia is subdivided into three manometric subtypes (I–III

Abstract Background Idiopathic achalasia is a disease of the esophagus causing impaired peristalsis and absent lower esophageal sphincter relaxation. The etiopathogenesis of the disease is unclear but most histopathologic studies from surgical resections show an absence of nitric oxide-producing neurons in the myenteric plexus. The peroral endoscopic myotomy (POEM) procedure has evolved in the last decade to treat achalasia and has provided a unique way to sample diseased tissue from an otherwise inaccessible tissue compartment. Intussusception is defined by Treves in 1899 as invagination of segment of bowel into adjacent segment usually proximal into distal. It’s a common cause of abdominal emergencies especially in children two years of age and younger. It accounts for 1 in 2000 infants and children and results in intestinal obstruction. Post-operative intussusception is one of the rare etiologies of intestinal obstruction that represents 0.01 to 0.25% after laparotomies and 5 -10% of all early postoperative intestinal obstructions. Miscarriage is a common event. Statistics show that more than a quarter of all pregnancies result in miscarriage, with the majority occurring in the first twelve weeks of pregnancy. However, there is very little research into its long-term impact on women.
