Full textSyftet med studien var att beskriva distriktssköterskors upplevelse av sin yrkesroll och vad som ingår i uppdraget. Studien hade en beskrivande design och tio distriktssköterskor från tre olika hälsocentraler i Mellansverige deltog i studien. Data samlades in via strukturerade intervjuer och analyserades utifrån kvalitativ innehållsanalys. Resultatet som framkom under intervjuerna redovisas utifrån tre kategorier och ett tema: Att utvecklas i yrkesrollen och i teamarbetet trots avsaknaden av tydliga styrdokument om uppdraget. Kategorierna som framkom var: Yrkesroll, teamarbete och distriktssköterskans uppdrag. Distriktssköterskorna upplevde sin yrkesroll som ett brett, fritt och ett varierande yrke med många möjligheter att utvecklas i. Att arbeta som distriktssköterska idag byggde på ett teamarbete med flera olika yrkeskategorier inom vården och distriktssköterskan är den som är spindeln i nätet kring patienterna. Distriktssköterskorna vet vad som ingår i deras arbetsuppgifter men de har svårt att uttrycka vad som egentligen är deras uppdrag. Distriktssköterskorna upplevde att de inte vet om uppdraget finns nedskrivet någonstans och om det diskuterats eller om det arbetats med vad som ska ingå i uppdraget på arbetsplatsen. Distriktssköterskorna kände viss oro i sin yrkesroll inför framtiden relaterat till att flera omorganisationer inom landstinget var på gång.
The purpose of the study was to describe the district handles cross experience of their professional and what is included in the mission. The study had a descriptive design and ten district nurses from three health centers in central Sweden participated in the study. Data were collected through structured interviews and analyzed according to qualitative content analysis. The result that emerged during the interviews reported on the basis of three categories, creating a theme: To develop in the professional role and team work, despite the lack of a clear governing documents of the mission. The categories that emerged were: professional, team-work and district nurse assignments. District nurses felt their professional role as a broad, free, and a diverse profession with many opportunities to develop in. Working as a district nurse today was based on a team work with various professionals in health care and district nurse is the spider in the net around patients. District nurses know what is included in their duties but they have difficulty expressing what exactly their mission is. District nurses felt that they did not know if the order is written down somewhere and if it discussed or if it worked with what should be included in the mission at work. District nurses felt some anxiety in their professional capacity for the future related to several reorganizations within the County Council was at once

Full textBackground: The district nurse in child health care is responsible for offering all children vaccines from the child vaccination program. When a new vaccine is added to the child vaccination program, the district nurse leads the implementation and communicate evidence to the guardian, so they can make decisions about vaccination for their child. In previous implementations of vaccines in the childhood vaccination program, the district nurse has experienced difficulties with insufficient support and information from the organization. Purpose: Describe the district nurse experience of introducing rotavirus vaccination into the child vaccination program in child health care. Method: The study was conducted with a qualitative method and an inductive approach, with semi-structured interviews with 13 district nurses in child health care in Central Sweden. The analysis was performed with a qualitative content analysis according to Elo and Kyngäs. Results: The study resulted in two categories Conditions for change and Approach of guardians and six subcategories. The district nurse experienced that preparation and support from the organization is required for good implementation. The guardian has different approaches and levels off knowledge regarding vaccines and the district nurse need to adapt the information. Conclusion: Opportunity for preparation promotes good implementation. The district nurse needs to be able to adapt the information given to the guardians because they have different approaches to vaccines and knowledge.

Full textBackground: In recent years, more people are fleeing from war, violence and persecution, which has led to an increasing number of asylum seekers in Sweden. This affects opportunities in primary care to meet the needs of asylum seekers. By law, asylum seekers have the right to immediate health care based on medical needs. In the meeting with the asylum seeker, the district nurse has an important role and a major responsibility for the patient to get good care. A lack of communication between district nurse and asylum seeker is something that may complicate the health care. The opportunities for communication increases when using interpreters. A good treatment is important to an equal health and health care. Big differences in treatment have disfavored foreign-born patients. Purpose: The purpose of this study was to illustrate the district nurse's experiences of meeting with asylum seekers in primary care. Method: The study was conducted with a qualitative inductive approach. The study was conducted with semi-structured interviews with nine district nurses in primary care. The data collection was analyzed according to qualitative content analysis. Results: The data analysis resulted in four main categories: The district nurse´s attitude affects the treatment, Language barriers and strategies to prevent them, Environmental conditions and Cultural differences, and four subcategories: Person centred response, Creating a caring relationship, Interpretation and Creative strategies. Conclusion: This study contributes with knowledge about district nurses experience of meeting asylum seekers in primary care. More education appeared to be desirable, with hope to improve future meetings.

Full textBackground: Chronic leg ulcers are today considered as a growing public health problem. This affects mainly people over 65 years and often results in pain and impaired quality of life because they often take a long time to heal. Prevention and treatment of these wounds require large health-care resources. The district nurse/nurse in primary health care addresses a significant portion of chronic leg ulcers. Aim: The aim of the study was to describe the district nurse/nurse's experiences to assess chronic leg ulcers. Method: A qualitative inductive approach was used. Fourteen district nurses/nurses in Southeast Skåne were interviewed individually. During the interview a semi-structured interview-guide was used. The interviews was analyzed using qualitative content analysis. Results: District nurses/nurses felt that they had sufficient knowledge to assess chronic leg ulcers, and that they had the ability to translate knowledge into practice. They used no guidelines in the assessment but felt that their experience ruled the assessment and measures to be taken. They said it was important to see the patient from a holistic perspective and that it was an advantage to have a commitment for leg ulcers because treatments are often demaning and extended over a long time. The district nurse used in varying degrees other people's skills. For them it was obvious necessity for them to consult others when they were in need of advice or help. Conclusion: Chronic leg ulcers represent a significant volume of primary care, and the district nurse needs the right conditions to be able to assess these. Implementation of the guidelines and scoring protocols would be a way to go to meet this need. There is also a need to develop cooperation between the concerned professions who treat patients with chronic leg ulcers. To give the district nurse the right conditions to dispose of chronic leg ulcers mean that she can work on guidelines and opportunities for development and training on evidence-based level.

Full textBackground One of the main demands on Swedish and global health care in general is to meet the patient’s need for safety. By general health care law this also comprises district nurses’ field of responsibility. Although there are numerous studies describing the concept of safety and its different shapes, no research could be found exploring how safety is, or supposed to be, mediated by district nurses’ in a Swedish context. Research about this is therefore needed. Aim The aim of this qualitative study was to explore how district nurses’ mediate safety to their patients. Method Qualitative data were collected from seven district nurses’ by means of semi structured interviews. Thereafter, a concept analysis was carried out. Results Responses revealed that district nurses’ consider themselves mediate safety through a variety of ways. Five major categories emerged: (1) complaisance’s; (2) competence; (3) patient participation; (4) same caregiver; (5) personal characteristics. Conclusion District nurses’ mediate safety through a combination of general attitudes and concrete acts. Preconditions are bound to each district nurse’s individual ability to give a good complaisance, his/her competence and ability to involve patients in treatment and care. Other, less pronounced, are bound to the district nurse’s ability to create continuity in contact with patients’ and his/her personal characteristics.

Full textBackground: District nurses come in contact with people who have dementia diagnosis to different extent. Aim: The aim of this study was to examine nurses' experiences in meeting people with dementia diagnosis and how they adapt giving care that are based on the patient's needs. Method: The study was qualitative and based on interviews with four district nurses. For processing the collected data content analysis was used. Results: The analysis revealed the three domains, the nurse's situation, the person with dementia diagnosis´ situation and adapting care measures. Further analysis led to four categories, facing a complex care situation, to be challenged, to take care of resources and to adapt their health care measures to the individual or to the dementia. The district nurses identified that people with dementia is a fragile and vulnerable group and to care for them was a challenge on many levels. It requires in-depth knowledge and an ability to see all the factors that surround this population, it´s challenging, complex and exciting. Working person-centered is something that both sought and done in practice, they also take good care of resources that will benefit the patients. Conclusion: The district nurses showed joy and commitment that were prominent and seemed to be linked to their education and knowledge. Research including people with dementia and others with limited decision-making capacaty are needed and desirable. Their participation would provide valuable knowledge that could lead to the development in the care concerning this vulnerable population.

Full textMental illness is a growing problem in society, both global and in Sweden and the risk of suffering from depressive symptoms (DS) increases with ageing. It can be difficult to identify DS in an older person. Elderly people are sometimes at a higher risk of mental pressure, when at the same time dealing with sickness, loss of close relationships and loneliness, which often lead to DS. These symptoms cause the elderly person a great suffering, increases the risk of dying prematurely and is a big cost for the society. The aim was to study district nurses experiences when it comes to identify and improve DS by elderly people in municipal home nursing. The method was qualitative and had an inductive approach. Eight district nurses from five different municipalities in southern Sweden were interviewed for 30-60 minutes. The analysis was qualitative which means that parts with meaning contents were identified and processed into categories and subcategories. The result showed that district nurses used their intuitive ability and leaned on their experience, when in an encounter and in a conversation with an elderly person identified DS. The encounter and the conversation with an elderly person turned out to be an important instrument even when it came to improve these symptoms. A valuation scale was not used in any of the municipalities. The district nurse in home nursing has and unique possibility to, in an early stage, identify DS and by different caring actions improve these symptoms. By standing up for the elderly person's right to get the care and treatment he/she is in need of, the district nurse has a key function in the role as the elderly person’s representative.

Full textSummary Background: Overweight among children is increasing around the world, which entails an increasing risk of lifestyle diseases later in life. The district nurse meets virtually all children and families during the child´s first five years. There will be several opportunities to identify and follow up the children who are in the risk area for overweight, as well as spread knowledge and help to a healthy and sustainable lifestyle. Aim: To describe district nurses' experiences of the encounter with parents of children with overweight within child healthcare. Method: Qualitative content analysis with inductive approach. Semi-structured interviews with 15 district nurses’ and pediatric nurses that is analysed with qualitative content analysis (Elo & Kyngäs, 2008). Result: Is presented in two main categories “A challenge” with the subcategories “An emotionally and difficult area” and “Parents’ attitudes” and “Prerequisites for the meeting” with the subcategories “Knowledge and competence” and “Trust and interaction”. The experience was that the subject overweight was sensitive and difficult to raise with the parents and that the parents own attitudes influenced the meeting. The prerequisites for a good meeting were the district nurse knowledge and competence in the subject and that the relationship between the district nurses’ and the parents was based on trust and interaction. Conclusion: Overweight is a sensitive topic to talk about and the meeting is influenced by the parent’s attitudes and knowledge. Knowledge and competence of the district nurse led to experience and a reliable relationship with the parents, promoted the meeting and facilitated the conversation. Full textBackground: There is a different in management and organization in primary health care and in municipal home health nursing. This different may effect activities and tasks at work, which in turn can affect the psychological and social work environment of district nurses / nurses. The purpose of this study was to compare any differences in the psychological and social work environment of district nurses/nurses in primary health care and home health nursing. Method: A quantitative, comparative cross-sectional study was conducted and the data was collected with the survey QPSNordic 34+. The survey was administred to 159 participants in primary care and to 148 participants in home health nursing. The survey was answered by (n = 39) district nurses /nurses in primary health care and (n = 39) district nurses/nurses in home health nursing. The result showed significant differences in four out of eleven categories in the psychological and social work environment between the district nurses/nurses in primary care and the district nurses/nurses in home care. Differences were found in "work requirements", "control at work", "work skills" and "stress". In all of these four categories, district nurses/nurses in primary health care estimated their psychological and social work environment lower than the estimation from district nurses/nurses in home health nursing. The conclusion of this study was that district nurses/nurses in primary health care estimate their psychological and social work environment lower in comparison to the district nurses/nurses in home care nursing.

Full textBackground: District nurses´ have been allowed, with some limitations, to prescribe since 1994. The right to prescribe were intended as a way to improve the healthcare for the entire population as well as strengthen the profession. Aim: The aim of this literature review was to illustrate the district nurses´ experience of prescribing. Method: This literature review was conducted based on 11 original articles. Searches were made in the databases PubMed and CINAHL. Data were analyzed using the process for qualitative content analyses according to Elo och Kyngäs (2008). The analysis resulted in three categories and seven subcategories. Result: The prescription rights of the district nurse had multiple positive effects on the healthcare for both the patients and the profession, however there are some factors that limits the use of the prescription right. These factors could be structural problems, lack of support, knowing your limitations, knowledge and possibility for further training.Discussion: The right to prescribe was believed to have potential to improve the healthcare. The district nurses could get a better view of the patient’s medical- and health needs by giving them a chance to increase their knowledge. Structural factors have had a major impact on the usage of prescription rights. Conclusion: Lack of support and development was shown to have a negative impact on the district nurse´s use of prescription rights which could have led to worse healthcare for the patients. The management within healthcare can use this study as support when developing strategies for increasing the district nurses use of the prescription rights. Continuing research on the subject should have focus on the patients’ perspective in Sweden as well as the time consumption effect.

Full textIn 2015, the increased number of refugees arriving in Sweden was covered in media. Public health nurses may in their encounter with immigrants be influenced by discourses arising from medial constructions of refugees. The aim of this study was to examine mass medial constructions of and discourses about refugees, since preconceptions can influence district nurses in encounters with these persons. It is a matter of importance to discuss how this can manifest itself, in order to make public health nurses encounters with refugees as well-functioning as possible. Person-centred care has been uses as a theoretical frame of reference in this study. A qualitative approach was chosen. The methods used were corpus analysis and discourse analysis. Data was collected from three daily newspapers: Dagens Nyheter, Sydsvenskan and Svenska Dagbladet, published in September, October and November 2015. During this time the concept refugee occurred in 4022 articles. The result shows that refugees in media are contoured as a homogenous group. Four discourses based on thirteen constructions were identified. The four discourses were a crisis discourse, a helplessness discourse, a political discourse, and an economical discourse. From the discourses, it is assumed that public health nurses reading of medial texts about refugees influences their view, and therefore their care, of refugees. Full textBackground: First line managers' support and understanding for district nurses is important to promote good work environment and satisfaction at the workplace. Factors that may affect: resources, feeling involved, knowing what's happening, development opportunities. Aim: The aim of this study was to describe experiences and reflections first-line managers’ had concerning their work on structural empowerment for district nurses in primary care. Method: The study had a qualitative approach with descriptive design. Semi-structured interviews, with eight first-line primary care managers, were performed and analyzed through qualitative content analysis based on an inductive approach. Result: Information was highlighted as difficult. Most common way to convey information were by mail. It was important to give district nurses opportunity of seeking information themselves. The managers held different meetings, which took a lot of time. The managers did their utmost to ensure that there were enough staff. The managers trusted the district nurses' assessments regarding materials, equipment and planning their time. The managers expressed that the time of district nurses was scarce. The managers felt they’d high attendance and availability. Feedback was given in different ways, managers would like to improve on this. The managers were positive about education and felt that opportunities for development were available. Conclusion: First-line managers in primary care experienced themselves as good support to district nurses and their intentions were to provide opportunities for structural empowerment. Sometimes they experienced being controlled from top. They found that some district nurses didn’t share the same interest as the organization, which may make it difficult to adjust at individual level.

For district nurses, palliative care at the end of life is a task that involves the care and commitment to achieve the best possible preventive and palliative treatment for the individual patient. Objective: To highlight the district nurses ' experiences of palliative care of patient’s end-oflife at home nursing. Method: Qualitative study with content analysis. Semi-structured interviews were conducted by the ten district nurses working in the municipal home care in three different municipalities. Results: The study resulted in four major categories and nine subcategories as follows: A well-functioning team work in the municipality is valuable with its subcategories Collaboration with other professionals within the community is vital and Skilled staff time missing. The other main category was: Collaboration with the County Council is important in subcategories Palliative team provides security for patient and nursing care is dependent on cooperation with the local doctor. The third category was Family members are important part of treatment with subcategories Important to establish relationship to support relatives and Relatives can falter. The fourth main category became Important mission to care at life's end with three subcategories as follows The mission requires special expertise, Can give the feeling of inadequacy and putting the patient's needs in focus. Conclusion The study showed that district nurses in home care is to establish a wellfunctioning team work with municipal and County Council professionals to establish a good care at the end-of-life for patients dying at home

Full textIntroduction: Pressure ulcer is a common and expensive problem for society which causes suffering and pain for the patient. Registered nurses should promote health, prevent illness and relieve suffering and is responsible for identifying risk and prescribing measures for pressure ulcer prevention. The purpose of this study was to describe and compare district nurses and registered nurses knowledge and attitude to pressure ulcer prevention in municipal health care. The method: Descriptive and comparative design with quantitative approach. A questionnaire was distributed to district nurses and registered nurses who worked in nursing homes and home healthcare in five municipalities. It was 67 out of 150 respondents who participated in the study. The result: The result showed that district nurse and registered nurse’s lack in knowledge about pressure ulcer prevention. Specific in the question about position change that reduces pressure ulcer risk the most, relief of heels on pressure-reducing mattress, the right way to reduce the pressure force when a patient slides down in a chair and that oxygen deficiency causes pressure ulcer. No significant difference between the groups knowledge on pressure ulcer prevention was found. The attitude of both the district nurse and the registered nurse was positive and a significant difference was found between the groups. The district nurse had a more positive attitude compared to the registered nurse. Nurses felt less confident of their ability to prevent pressure ulcer and district nurses responded to a greater extent that pressure ulcer prevention is an important task. Conclusion: In this study, participants have insufficient knowledge about pressure ulcers prevention and the attitude of both the district nurse and the registered nurse was positive to pressure ulcer prevention. A difference between the groups was where the district nurse had a more positive attitude compared to the registered nurse.

Full textWhen the patient wishes to die at their own home requires that the advanced care also moved to their home. District nurses have primary care responsibilities in palliative home care and may experience ethical, moral and communicative difficulties. For a good palliative care requires that district nurses is involved, have the experience and have advanced skills in palliative care. The purpose of this study is to illuminate district nurses' experiences of caring for patients in the final stage in palliative home care. Nine district nurses were interviewed. To analyze the data, a qualitative content analysis with an inductive approach was applied. The analysis resulted in four main themes with its eight sub- themes. Main themes titled; Being able to collaborate and cooperate in care, Being able to care for relatives, Being able to manage negative emotions and Able to create a good care environment. The conclusion was that, a good teamwork is important for district nurses but having different care providers can complicate caring. It is important for district nurses to meet relatives and receive grief when grief could come as dissatisfaction. This could affect the nursing staff negatively. It is important that district nurses are supported in the sense of inadequacy and emotional reactions. Further research may be aimed at relatives ' point of view in order to understand family members' reactions.
