Abstract:

Objective. To compare the incidence of venous thromboembolism (VTE) after total hip arthroplasty (THA) using the direct anterior approach (DAA) with that using the direct lateral approach (DLA). In addition, patient background characteristics and the incidence of VTE were compared between the first half and the latter half of the period after introducing DAA and against DLA. Method. This was a retrospective, multicenter study involving 109 patients (116 hips) who had undergone primary unilateral THA. Thirty-six hips underwent THA using DAA and 80 hips underwent THA using DLA. Patient information including sex, age, and preoperative diagnosis was collected. The incidence of VTE was compared between DAA and DLA. Moreover, the patients who underwent THA using DAA were divided into 2 groups (first half and latter half groups), and sex, age, body mass index (BMI), and surgical time were compared between the 2 groups. Moreover, the incidence of VTE was compared among the 3 groups (first half of DAA, latter half of DAA, and DLA). Results. The incidence of VTE in the DAA group was significantly higher than that in the DLA group p=0.014. The incidence of VTE in the first half group was significantly higher than in the latter half group and the DLA group (p=0.035 and p=0.001, respectively), and there was no difference in the incidence of VTE between the latter half group and the DLA group p=0.923. Surgical time was significantly longer in the first half group than in the latter half group p=0.046. Conclusions. In the first half of the period after introducing the DAA, more VTEs occurred than in the DLA. It may be important to shorten the surgical time in the early stage of introducing the DAA, and aggressive anticoagulation therapy may be required until the surgeon becomes familiar with the procedure.

Abstract:

When thinking about nature, it is impossible to overlook fauna, which contributes to its constant transformation, mainly through the development of new species. The author of the article sheds light on the current situation of animals living e.g. in national parks, focusing on pro-environmental activities on social media and non-virtual media, and on destructive human behaviour. The growing tourism- and economy-related infrastructurisation of the mountains as well as their increasingly frequent exploration by humans force animals to change their familiar habitats. Owing to the animals’ natural territorialism as well as instinctive fight for species survival, in the case of stronger specimens we often see altruistic actions to protect the group under threat. The interference of humans with the natural environment forces animals to acquire social behaviours which are a consequence of a long adaptation process. The author of the article also examines selected aspects of ethology, that is study of inherited and acquired animal behaviour. However, the appropriation of the mountains by humans and their adaptation to human needs are characterised by a relatively high awareness of the impact on animal life, a fact reflected in numerous ecological and faunistic campaigns seeking to protect natural habitats and their indigenous residents. The article also features an analysis of campaigns and projects that are to make people sensitive to the fact that they are guests at a home of species other than the human species.

Abstract:

The article is consecrated to Constantine’s conversion and to its consequences for the Church, for the Roman Empire and for Europe. There is a general opinion that, even if his attachment to Christianity was not very mature, he worked for the Christian religion during all his life. He has taken many decisions on behalf of the Church; he protected her against the Donatists in Africa. His position towards the Arian heresy was not very clear. He did not pay attention to the dogmatic formulas, but especially to those solutions which guaranteed peace among people. Surely, the emperor once introduced into the Church, remained there as her protector and head. The society was accustomed the emperor’s position as pontifex maximus. Bishops did not protested against his involving into ecclesiastic matters because he worked on their behalf. The effect of Constantine’s attitude was: the Christianization of the Roman Empire and the connection of the Church to the State. In later centuries such an alliance of the altar with the throne was boring for the Church. It is said that every privilege has to be paid. The Roman Empire was collapsed in the end of the fifth century, but its heritage remained in Europe. Charlemagne, cooperating with Pope Leon III, tried to restore the Roman Empire as a Christian State, but he failed to do it. Surely, by his support for schools and studies, he contributed to the European culture. The idea of the Sacrum Imperium Romanum appeared again in the times of Otto I, and especially of Otto III. Such an idea was not possible to be put into practice. The Roman Empire has never been restored, but many of its elements were assimilated by the Church and by medieval Europe. There are to be noticed in all European countries in our time.

Abstract:

2530 Background: PI3K/mTOR and MAPK signaling pathways are often deregulated in tumors. Simultaneous inhibition of these pathways with the MEK1/2 inhibitor, pimasertib, plus the dual PI3K/mTOR inhibitor, SAR245409, (ClinicalTrials.gov NCT01390818) was investigated. Methods: This was a phase Ib, modified 3+3, dose-escalation trial in patients (pts) with advanced solid tumors. Pts received pimasertib and SAR245409 at the following dose levels (DLs): DL1, 15/30; DL2a, 30/30; DL2b, 15/50; DL3, 30/50; DL4a, 60/50; DL4b, 30/70; DL5, 60/70; DL6a, 90/70; DL6b 60/90 and DL7, 90/90 mg (once-daily, qd). After the qd maximum tolerated dose (MTD) was established, twice-daily (bid) dosing was tested: DL1a, 60/30; DL1b, 45/50 and DL2 60/50 mg bid. A recommended phase II dose (RP2D) was determined. Enrollment continued at the RP2D in four expansion cohorts (18 pts each): dual KRAS/PIK3CA mutated (mt) colorectal cancer (CRC), triple-negative breast cancer, KRAS mt non-small cell lung cancer (NSCLC) and BRAFmt melanoma. Results: 53 pts were treated qd and 7 pts bid. The most common tumors were CRC (n=16), NSCLC (n=8), ovarian and pancreatic (n=7, each). At DL6b 2/3 pts had dose-limiting toxicities (DLTs; both grade [Gr] 3 nausea/vomiting). DL6a was confirmed as the MTD for the qd schedule. At bid DL1a 2/4 pts (both Gr 3 skin rash) and at DL1b 2/3 pts (Gr 3 skin rash and Gr 3 asthenia) had DLTs. DL5 was the RP2D based on tolerability after prolonged exposure. The most common adverse events in qd schedule were: rash (62%, 13% Gr 3), diarrhea (56%, 4% Gr 3), fatigue (51%, 2% Gr 3), nausea (49%, 2% Gr 3), vomiting (45%, 2% Gr 3), peripheral edema and pyrexia (34%, each) and visual impairment with underlying serous retinal detachment (21%). Preliminary pharmacokinetic results suggest no drug-drug interaction. There were 4 partial responses: KRAS mt CRC (n=1) and low-grade ovarian cancer (n=3, 1 KRAS mt/PIK3CA mt and 2 wild-type). Enrollment in expansion cohorts at DL5 is ongoing. Conclusions: Continuousqd dosing of pimasertib and SAR245409 is tolerated and has shown signs of activity. Phase II trials are being planned. Abstract:

The rite included in Leviticus 16 belongs to penitential rites of purification. It may be a reminiscence of antic religious medical-magic rites, practiced in multiple regions of ancient world. The rites contained the idea of substitution, “magic elimination.” Sending the goat represents rejection of anything that should not and can not exist among the people of Israel. The desert as an “infertile” place, the theological idea of separating the good from the evil as well as the ancient idea of substitution point at the theology of penance expressed in the rite of separation. The text of Leviticus 16 applies the term sa‘ir for the goat. The same term is also applied in priestly text to describe demons relating to the misshapen form of Israel religion. There is an undoubted connection with the idea of Convenant that constitutes the basic background and point of reference for the rite of Leviticus 16.

Abstract:

W ostatnich latach obserwujemy stopniową zmianę w sposobie definiowania społecznej roli i odpowiedzialności instytucji muzealnych (a także szerzej - instytucji kultury). Sygnałem tej zmiany są chociażby dyskusje wokół nowej definicji muzeum prowadzone na forum ICOM, czy publikacje proklamujące zwrot ku modelowi instytucji zaangażowanej. Pomimo iż przywykło się postrzegać instytucje muzealne jako specjalizujące się w zagadnieniach „długiego trwania”, nie zaś w aktywistycznym zaangażowaniu w bieżące wyzwania i problemy współczesności, kryzys klimatyczny jawi się jako jedno ze zjawisk, które wymuszają rewizję takiego rozumienia publicznej odpowiedzialności muzeów. Nieformalna grupa aktywistyczna Kultura dla klimatu podjęła próbę przełożenia tych postulatów na praktykę instytucjonalną. Artykuł analizuje praktyczne konsekwencje, jakie niesie ze sobą taka zmiana paradygmatu dla instytucji muzealnych oraz próbując uchwycić znaczenie tej zmiany dla sektora muzealnego, a także szerzej - całego sektora dziedzictwa.

Abstract:

<p>A guardianship court may appoint in non-litigious proceedings a guardian for a disabled person (Article 183 of the Polish Family and Guardianship Code, hereinafter: FGC) and for a partially incapacitated person (Article 181 FGC). It is not always possible to precisely delimit the areas of application of these provisions which entails problems in practice. Particularly problematic is the nature of the guardianship established for a disabled person and for a partially incapacitated person and the scope of the powers of both guardians. Especially debatable is the status of the guardian of a partially incapacitated person, who has not been authorized by the court to manage the assets of the ward and represent him/her. The purpose of this article is to indicate the scope of action of the guardian appointed for a disabled person and for a partially incapacitated person together with the determination of their status. The guardian is established for a partially incapacitated person, who, under the authority to represent and manage the property of the ward, is the ward’s statutory representative. The guardian appointed for a disabled person, who is not a statutory representative of this person, has a different status. The guardian under Article 183 FGC provides, above all, factual assistance to a person who has not been incapacitated, even if the person is affected by mental dysfunctions, while the guardian under Article 181 FGC provides assistance in legal acts, procedural acts and factual acts not relating to legal acts for a person who has been partially incapacitated.</p>

Abstract:

Podczas gdy w przeszłości place zabaw były zazwyczaj przeznaczone tylko dla dzieci, dziś możemy obserwować więcej miejsc publicznych, terenów sportu i wypoczynku przeznaczonych dla wszystkich grup wiekowych, w tym osób dorosłych i w wieku podeszłym. Tradycyjne place zabaw wyposażone są w sprzęt rekreacyjny adresowany do użytkowników najmłodszych: huśtawki, zjeżdżalnie i karuzele, natomiast place zabaw o charakterze uniwersalnym mają również wyposażenie atrakcyjne dla osób dorosłych, takie jak boisko do koszykówki czy szachy. Te udogodnienia pozwalają rodzinom spędzać czas wspólnie, rozwijać koordynację fizyczną, siłę, elastyczność poczucia własnej wartości i umiejętności społeczne, a także zapewnić rekreację i przyjemność. Na podstawie publicznych placów zabaw mieszczących się w Warszawie autorzy próbowali odpowiedzieć na następujące pytania: które urządzenia pomagają pobudzać i rozwijać umiejętności społeczne, umysłowe i fizyczne oraz jakie są najczęstsze sugestie użytkowników w sprawie zmian i ulepszeń? Z wykorzystaniem badania terenowego (obserwacje uczestników, wywiady strukturalne i semistrukturalne) określono wiele czynników subiektywnych i obiektywnych, które mogłyby określić potencjał „inkluzyjnego terenu rekreacji”. Wyróżniono silne i słabe punkty placów zabaw (dostępność, lokalizację, atrakcyjność urządzeń, ich poziom bezpieczeństwa), a także oceniono relacje między zestawem urządzeń a płcią, grupą wiekową, stymulacją fizyczną i psychiczną – za pomocą przyjętych kryteriów. Uniwersalne, zintegrowane place zabaw (dla zróżnicowanych grup wiekowych) powinny być dostępne dla wszystkich członków społeczności, w tym seniorów i osób niepełnosprawnych. Aby to osiągnąć, należy przestrzegać podstawowych zasad: dostępności, infrastruktury przeznaczonej dla użytkowników o różnych możliwościach fizycznych i wieku, dostosowanych do oczekiwań i zainteresowań społeczności lokalnej. Jak się okazuje, inspirujące i ekscytujące urządzenia, atrakcyjnie urządzony i dostępny teren sportu i rekreacji jest ważny, aby zachęcić do codziennej aktywności fizycznej, ma znaczenie integrujące i pomaga w zacieśnianiu więzi społecznych. Artykuł powstał na podstawie materiału badań w ramach projektu statutowego ds. -300 AWF zrealizowanego w Akademii Wychowania Fizycznego Józefa Piłsudskiego w Warszawie, dzięki dotacji grantu Ministerstwa Nauki i Szkolnictwa Wyższego.

Abstract:

Classes conducted in the field, thanks to a variety of educational functions, such as demonstrative learning of reality, shaping various skills and attitudes of students, are an opp ortunity for geographic education and showing geography as a useful subject in life. For many geography teachers, conducting fieldwork is a big challenge, mainly organizational. The aim of the research conducted by the authors was: a) learning the opinions of teachers about the difficulties in conducting geographical field classes and b) the legitimacy of their mandatory introduction to the new core curriculum. The article presents the results of a nationwide survey of geography teachers about fieldwork, which, according to the assumptions of the new curriculum, are to constitute an important element of geographic education. Research results indicate that teachers are full of concerns about readiness to conduct such classes, indicating organizational difficulties and deficiencies in methodical preparation. The effect of research is a conclusion about the need and possibilities to supp ort teachers in meeting this new challenge. The existence of the entries themselves in the core curriculum does not guarantee their implementation in the work with students.

Abstract:

W niniejszym artykule Autor koncentruje się na analizie implikacji Transatlantyckiego Partnerstwa Handlowego i Inwestycjnego (TTIP) dla Chin. W ostatnim czasie w związku z negocjacjami w sprawie owego TTIP między USA a Unią Europejską, podkreślano, że rozmowy te mają doprowadzić do redukcji barier regulacyjnych, a to z kolei miało doprowadzić do istotnych korzyści ekonomicznych. W niniejszym artykule analizuje się problem barier regulacyjnych i dokonuje się oceny tego, co może zostać w związku z tym osiągnięte. Idealnie, najlepszym sposobem, aby rozwiązać problemy wynikające z rozbieżności regulacyjnych byłoby, na zasadzie wielostronnej współpracy, również uwzględniać stosunki UE i USA z Chinami. Głównym celem artykułu jest prezentacja implikacji TTIP dla Chin. W szczególności celem badawczym pozostają m.in. regulacyjne bariery handlowe polityki handlu zagranicznego w stosunkach transatlantyckich USA–UE, oraz specyfiki TTIP.

Abstract:

It is known that parametric measures such as the Journal Index Factor or Hirsh Index provoke negative consequences in academic communities, significantly lowering job satisfaction and quality. A “game with algorithms” replaces researchers and academic teacher ethos, frequently leading to frustration and scientific misconduct. This situation is analyzed and compared with similar phenomena in business caused by so-called “motivations systems” colloquially called “stick and carrot.” Since about 1980, several experts in management like E.Demming, A.Kohn, P.Senge, or M.Kossewski, point out a highly destructive role of these methods. Also, more and more organizations implement new management styles known as well-being, holacracy, participatory management, or teal self-organization. The main message of the paper is that it is worth considering similar approaches in academic communities. The author hopes that his voice will at least provoke a discussion on that matter.

Abstract:

L’auteur aborde une question de l’honneur, dont le prêtre doit avoir à la très sainte Eucharistie dans laquelle le Christ Seigneur lui-même est contenu, offert et reçu, de qui parle le can. 898 du CIC. Enumérant des négligences et des abus en matière de la vénération de prêtre envers la très sainte Eucharistie et soulignant qu’une seule dignité de ce sacrement de l’amour dénote déjà une nécessité de découvrir constamment, de faire la connaissance et de tirer positivement profit de la splendeur du sacrement de l’autel, il présente tour à tour des formes de la vénération eucharistique du prêtre, c’est-à-dire: la célébration eucharistique, l’adoration eucharistique, des autres formes de la piété eucharistique. En détaillant beaucoup de comportements et d’activités concernant ces trois formes de la vénération eucharistique et confirmatoires la foi du prêtre à la présence réelle du Christ sous les espèces du pain et du vin, l’auteur constate que le rapport de chaque prêtre à la très sainte Eucharistie est d’importance sociale. Il détermine en quelque sorte le développement spirituel du ministre saint qui influe sur l’engagement pastoral du prêtre et décide du point de la mise à profit par lui de l’Eucharistie comme le moyen de la sanctification, tant au propre développement spirituel qu’à leur service pastoral. Du reste, l’honneur du prêtre à la très sainte Eucharistie exerce une influence sur des rapports des autres fidèles devant cet admirable sacrement. Le prêtre dont la formation eucharistique est insuffisante et sa relation avec le Christ présent dans le sacrement de l’Eucharistie est lointaine de celle attendue par l’Eglise, non seulement ne tire pas en tout point de profit de la richesse de ce sacrement qui doit être en plein centre de sa vie, mais il réduit encore les droits des autres fidèles à jouir des biens spirituels de l’Eglise unis avec la très sainte Eucharistie. Tel ordre des choses montre un besoin de garantir par l’évêque diocésain et par la législation particulière la formation eucharistique des ministres saints, arrangée et profonde, préparatoire et permanente. Ce besoin est plus urgent, plus souvent on observe dans le milieu sacerdotal les négligences et les abus comme: un irrespect de la norme „il n’est pas permis à un prêtre de célébrer l’Eucharistie plus d’une fois par jour”, un manque de respect pour les dispositions sur les intentions de messes „collectives”, la célébration eucharistique à la hâte et négligée, un comportement blâmable en église ou en chapelle où la très sainte Eucharistie est conservée.
