Abstract:
OBJETIVO: Estudar a heterogeneidade e a coexistência das neuropatias no diabetes melito tipos 1 (DMT1) e 2 (DMT2). MÉTODOS: Foram avaliados 74 DMT2 e 20 DMT1 em relação à idade (anos), tempo de diagnóstico do DM (TDDM, em anos), índice de massa corpórea (IMC, kg/m²), HbA1c e tipo de neuropatia (critérios da American Diabetes Association). RESULTADOS: DMT1 era mais jovem (32,7 ± 11 versus 56,9 ± 10,3; p = 0,0001), com maior TDDM (17,1 ± 9,7 versus 10,4 ± 6,8; p = 0,003) e menor IMC (23,6 ± 3,8 versus 28,4 ± 5,3; p = 0,0005). A neuropatia autonômica cardiovascular (NAC) (60% versus 32,4%; p = 0,02) e a coexistência desta com polineuropatia (PND) (62,5% versus 33,3%; p = 0,03) foram mais prevalentes no DMT1; a PND dolorosa crônica (PNDDC) (60,8% versus 30,0%; p = 0,009) o foi no DMT2. A HbA1c (p = 0,04) foi preditiva de PND em ambos os grupos. O TDDM (p = 0,03) e a PNDDC (p = 0,003) foram preditivos de NAC no DMT1. A idade (p = 0,0004) teve valor preditivo para PNDDC no DMT2. CONCLUSÕES: As neuropatias apresentam distribuição heterogênea no DMT1 e no DMT2. Com exceção do controle glicêmico, os fatores relacionados a essa complicação diferem de acordo com o tipo de diabetes.

Abstract The aim of the study was to investigate the lymphocyte (lymph) subpopulations in peripheral blood as a part of the immune response among patients with diabetes mellitus type 1 (DMT1) and diabetes mellitus type 2 (DMT2). Patients and methods: A prospective, cross-sectional, comparative, “case-control” study was conducted among 22 patients with DMT1 and 70 patients with DMT2. The levels of lymph subtypes [general nonspecific T-lymph (CD3+); T-helper lymph (CD4+); T-cytotoxic lymph (CD8+), natural killers [NK cells (CD3\ CD16+/CD56)] and B-lymph (CD19+)] in blood was measured and compared by flow-cytometric analisys (FAC Sort, BD). Results were compared to those of 21 healthy persons. The data was processed using the statistics software. Results: Patients with DMT1 had longer duration of the disease, compared to patients with DMT2. No significant differences between arterial blood pressure, НвА1с levels and lipid profile among the patients with DMT1 and DMT2 were present. There were no differences in the total leukocyte count between the groups (DMT1-6,91 ± 1,32.109/l; DMT2-7,28 ± 1,85.109/l; controls-6,89 ± 1,07.109/l). The results from the flowcytometric investigation showed significantly higher absolute number of T-all lymph (CD3+), Th lymph (CD4+) and all NK (CD3\ CD16+/CD56), as well as a lower absolute number of Ts (CD8+) and B (CD19+) lymph among the diabetic patients compared to healthy subjects. The Th/Ts ratio in patients with DMT1 (2,02 ± 0,44) and DMT2 (2,36 ± 0,37) was also significantly higher compared to ratio of controls (1,02 ± 0,06). No significant differences were noted in the lymph subpopulations between the two groups with DM. Conclusions: Changes of lymph types in peripheral blood in diabetic patients demonstrate immune activation and dysregulation among the two types of diabetes.

The Significance of Urinary Markers in the Evaluation of Diabetic Nephropathy Oxidative stress is considered to be a unifying link between diabetes mellitus (DM) and its complications, including nephropathy (DN). The aim of this study was to determine the parameters of oxidative injury of lipids and proteins as well as the activity of ectoenzymes in the urine of DN patients. The study included 40 individuals: 10 patients with type 2 diabetes mellitus and microalbuminuria (DMT2-MIA), 10 type 2 diabetic patients with macroalbuminuria (DMT2-MAA), 10 patients with type 1 diabetes and microalbuminuria (DMT1-MIA) and 10 age- and sex-matched healthy subjects (control). In the urine we determined TBA reactive substances (TBARS), reactive carbonyl groups (RCG), and the activity of ectoenzymes N-acetyl-β-d-glucosaminidase (NAG), plasma cell differentiation antigen (PC-1), aminopeptidase N (APN) and dipeptidyl peptidase IV (DPP IV). A higher concentration of TBARS in the urine was found in DMT2-MIA and DMT1-MIA, compared to the control group (p<0.001 and P<0.05). The urine concentration of RCD shows similar results with a significant elevation in the groups with DMT2-MAA and DMT1-MIA, compared to the DMT2-MIA (p<0.001) and control group (p<0.001). Activities of NAG, APN and DPPIV were significantly higher in the urine of DMT2-MAA, compared to the control (p<0.01). The activity of PC-1 was slightly increased in that group, but not significantly. In conclusion, the level of oxidative stress markers and activities of brush border ectoenzymes in the urine may be a useful non-invasive and easily repeatable test in DN.

Abstract:
Objective: To examine randomized controlled trials (RCTs) for treatment evidence for Black male adolescents suffering from comorbid mental illness and diabetes mellitus. Method: A review of the studies published in English-language journals was conducted. Results: We found no RCT focused on Black males with diabetes mellitus Type 2 (DMT2). However, we found RCT inclusive of Black male adolescents with diabetes mellitus Type 1 (DMT1). Multisystemic therapy appears to be the best supported overall treatment for DMT1 management and psychosocial functioning followed by an enhanced form of behavioral family systems therapy for diabetics. Metformin was the only treatment in this review noted for use within DMT2. Metformin and a nursing-based telephone case management intervention realized utility as secondary services. Conclusions: There are gaps present for what effectively treats comorbid mental illness and DMT2 in Black male adolescents. For comorbid mental illness and DMT1, there are gaps in additional efficacious treatments, effectiveness across conditions, and effect duration beyond 24 months.

Abstract:
Diabetes mellitus is a metabolic disorder characterized by hyperglycemia. In general, diabetes is classified into type 1 diabetes mellitus (T1DM), type 2 diabetes mellitus (T2DM), gestational, and other specific diabetes. The causes of diabetes are genetic disorders and environmental. Common symptoms of diabetes include: polydipsia, polyphagia, glycosuria, polyuria, dehydration, fatigue, weight loss, reduced vision, cramps, constipation, and candida infection. Test for diagnosis of diabetes include: fasting plasma glucose test, plasma glucose test after 2 hours of 75 g oral glucose administration, the glycated hemoglobin test (HbA1C), and random blood glucose test. Prevention of T1DM is still difficult because of the limited knowledge of metabolic, genetic, and immunological processes in the development of T1DM. T2DM is prevented by lifestyle and medical intervention. Insulin is the only drug for T1DM, whereas T2DM is treated with metformin as drug’s primary choice for reducing blood glucose levels. Diabetes melitus merupakan penyakit kelainan metabolisme yang ditandai dengan hiperglikemia. Secara umum, diabetes diklasifikasikan menjadi: diabetes melitus tipe 1 (DMT1), diabetes melitus tipe 2 (DMT2), gestasional, dan diabetes spesifik lain. Penyebab diabetes adalah kelainan genetik dan lingkungan. Gejala umum diabetes antara lain: polidipsia, polifagia, glikosuria, poliuria, dehidrasi, kelelahan, penurunan berat badan, daya penglihatan berkurang, kram, konstipasi, dan infeksi candida. Pemeriksaaan untuk diagnosis diabetes meliputi: pemeriksaan glukosa plasma saat puasa, pemeriksaan glukosa plasma setelah 2 jam pemberian glukosa oral 75 g, pemeriksaan hemoglobin terglikasi (HbA1C), dan pemeriksaan glukosa darah acak. Pencegahan DMT1 masih sulit karena terbatasnya pengetahuan proses metabolisme, genetik, dan imunologi pada perkembangan DMT1. DMT2 dicegah dengan intervensi gaya hidup dan intervensi medis. Insulin merupakan satu-satunya obat untuk DMT1, sedangkan DMT2 diobati dengan metformin sebagai pilihan utama dan non obat untuk menurunkan kadar glukosa dalam darah.

Berbagai penelitian menunjukkan bahwa ekstrak media penumbuh sel punca mesenkimal (EMPSPM), tanpa sel punca itu sendiri, telah ditemukan terdapat berbagai faktor tropik hasil sekresi sel punca mesenkimal di dalam media kultur yang dapat meregenerasi jaringan yang rusak. Penelitian ini bertujuan untuk mengetahui gambaran kadar malondialdehid (MDA) dalam plasma dan ginjal tikus diabetes melitus tipe 2 (DMT2) dengan terapi EMPSPM. Dua puluh lima ekor tikus wistar digunakan dalam penelitian ini dibagi menjadi 5 kelompok yaitu kelompok DMT2 + 0,05 ml/kg BB EMPSPM (0,05); kelompok DMT2 + 0,1 ml/kg BB EMPSPM (0,1); kelompok DMT2 + 0,2 ml/kg BB EMPSPM (0,2), kelompok kontrol DMT2 (DMT2), dan kelompok sehat (KS). Induksi DMT2 dengan menggunakan streptozotosin nikotinamid (STZ-NA). Terapi mulai dilakukan pada hari ke 7 setelah kondisi DM tercapai, diberikan 4 kali dengan selang waktu 7 hari secara intraperitoneal. Data kadar glukosa darah dan MDA dianalisi secara statistik. Hasil penelitian menunjukkan bahwa induksi DMT2 dengan STZ-NA dapat menaikkan kadar glukosa dan MDA dalam darah (P<0,05). Terapi 0,05; 0,1; dan 0,2 ml/kg BB EMPSPM menunjukkan dapat menurunkan kadar glukosa darah, kadar MDA plasma dan ginjal (P<0,05). Kadar 0,2 ml/kg BB EMPSPM menunjukkan aktifitas lebih baik dalam menurunkan kadar glukosa darah dan MDA. Berdasarkan penelitian ini, EMPSPM dapat menurunkan kadar glukosa darah serta kadar MDA dalam darah dan ginjal tikus DMT2.

Abstract Introduction. Oxidative stress is one of the key mechanisms responsible for disease progression in non-alcoholic fatty liver disease. The aim of this study was to evaluate the serum levels of oxidative stress markers in patients with type 2 diabetes mellitus (DMT2) and non-alcoholic steatohepatitis (NASH) and test their relationships with clinical and biochemical patient characteristics, compared to patients with DMT2 without non-alcoholic fatty liver disease (NAFLD), and controls. Materials and methods. In all, 60 consecutive patients with DMT2 and NASH, 55 with DMT2 without NAFLD, and 50 age-and-gender-matched healthy subjects participated in the study. The serum levels of protein carbonyls and 8-isoprostane were determined by ELISA methods, while the serum levels of malondialdehyde (MDA) were detected by means of the spectrophotometric method. Clinical, demographic, and laboratory parameters were examined for all the subjects included in the study. Multivariate logistic regression was used to test the independent predictive factors in the relationships investigated here. Results. Patients with DMT2 and NASH displayed significantly higher serum levels of protein carbonyls (1.112 ± 0.42 nmol/dL), MDA (6.181 ± 1.81 ng/mL), and 8-isoprostane (338.6 ± 98.5 pg/mL) compared to patients with DMT2 without NAFLD, and controls. Results of multivariate logistic regression analyses indicate that in patients with DMT2 and NASH, the serum levels of oxidative stress markers were independently and positively associated with: HbA1c, duration of diabetes, the UKPDS cardiovascular risk score (for protein carbonyls); age, LDL-cholesterol (for 8-isoprostane); and triglycerides serum levels (for MDA). Conclusions. Our findings indicate that the process of oxidative stress tends to increase in patients with DMT2 and NASH, compared to patients with DMT2 without NAFLD, and controls. This evidence suggests that an antioxidant therapy might prove useful in the treatment of patients with DMT2 and NASH.

Abstract:
Background: Status of minerals such as zinc, magnesium, and chromium among diabetic patients are considered lower than non-diabetic peoples, both in the serum and intake. Some reviews show that those minerals contribute to carbohydrate metabolism among diabetic patients.Objective: This study aims to explore intake of zinc, magnesium, and chromium among DMT2 in Denpasar.Method: This is a cross-sectional analytic study. The population was all (70) DMT2 patients registered at Chronic Disease Service (Prolanis) at two Primary Health Care (PHC) Denpasar Utara III PHC and Denpasar Timur I PHC. The sample for the study was DMT2 patients and non-diabetic people age 50-70 years reside in Denpasar city. DMT2 patients were randomly selected with simple random technique, while non-diabetic peoples were non-randomly selected. The level of serum chromium was measured by AAS method, whereas microminerals intake were collected by a recall and SQ-FFQ method. Data was analyzed descriptively and differences were tested with two independent t-test at 95% confidence level.Results: The mean of serum chromium DMT2 and non-diabetic were 0.044 mg/L and 0.094 mg/L respectively. The mean±SEM magnesium (DMT2 316.1±22.4 mg/day; non-diabetic 211.0±33.5 mg/day), zinc intake was (DMT2 8.4±0.7 mg/day; non-diabetic 6.2±0.8 mg/day), and chromium (DMT2 10.8±1.3 µg/day; non-diabetic 8.7±0.9µg/day). A significant difference was found between magnesium intake (p=0.01).Conclusion: The study showed that zinc, magnesium and chromium intake is appeared higher among DMT2 patients compare to non-diabetic in Denpasar region.

Abstract:
Kendali glikemik merupakan hal yang penting bagi penyandang Diabetes Melitus (DM) karena berimplikasi terhadap upaya pengelolaan DM. Tuberkulosis (TB) paru merupakan salah satu komplikasi yang banyak pada pasien DM yang berhubungan dengan proses dan hasil pengobatan penyakit tersebut. Penelitian bertujuan mengetahui kendali glikemik berdasarkan parameter kadar Glukosa Darah Puasa (GDP), glukosa darah 2 jam post prandial (GD2JPP) dan HbA1c pada pasien diabetes melitus tipe 2 (DMT2) yang mengalami TB paru. Penelitian potong-lintang dilakukan pada 45 subyek DMT2 dengan TB paru dan 45 subyek DMT2 tanpa TB paru yang berobat jalan di dua rumah sakit di Surabaya. Hasil pemeriksaan GDP, GD2JPP, dan HbA1c yang dianalisis secara deskriptif, ditampilkan berupa nilai rerata dan Standar Deviasi (SD). Diperoleh rerata dan SD adalah GDP (202,11 ± 78,68 mg/dl), GD2JPP (283,20 ± 107,20 mg/dl), dan HbA1c (11,20 ± 2,61%) pada subyek DMT2 dengan TB paru sedangkan pada subyek DMT2 tanpa TB paru diperoleh GDP (175,29 ± 61,38 mg/dl), GD2JPP (208,22 ± 75,60 mg/dl), dan HbA1c (9,34 ± 2,22%). Nilai rerata GDP, GD2JPP dan HbA1c pasien DMT2 dengan TB paru lebih tinggi daripada DMT2 tanpa TB paru. Hal ini menunjukkan kriteria pengendalian DM yang belum mencapai sasaran sehingga perlu dilakukan upaya pemantauan kendali glikemik yang lebih baik

Latar Belakang: Lemak viseral berkaitan dengan peningkatan risiko kardiovaskular khususnya pada pasien obesitas dengan Diabetes Mellitus tipe 2 (DMT2). Obesitas khususnya obesitas sentral merupakan kondisi penumpukan lemak pada regio perut atau abdominal yang dibuktikan dengan pengukuran indikator lingkar abdomen (atau rasio lingkar abdomen dengan lingkar pinggang). Diabetes sering terkait dengan obesitas dan khususnya pada lansia merupakan populasi yang sangat rentan. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan kadar lemak visceral dengan kejadian obesitas pada lansia DMT2 di Kabupaten Sumbawa Besar. Metode: Metode yang digunakan kasus kontrol, dengan jumlah sampel penelitian berdasar rumus besar sampel sejumlah 41 sampel pasien lansia DMT2 yang berkunjung ke Poliklinik Geriatri RSUD Sumbawa Besar dan RSU Provinsi Manambai, yang memenuhi kriteria inklusi. Analisis data dengan uji Chi Square lalu dengan menghitung rasio odd. Pengolahan data menggunakan program SPSS 16.0 for Windows. Hasil: Didapatkan hubungan yang bermakna antara tingginya kadar lemak viseral dengan kejadian obesitas pada lansia DMT2 dengan p=0.008 (<0.05). Pasien DMT2 dengan kadar lemak viseral tinggi memiliki kemungkinan 6.3 kali untuk mengalami obesitas {IK 95% (1.522 - 26.081)}. Simpulan: Terdapat hubungan tingginya kadar lemak viseral dengan obesitas pada lansia dengan DMT2. Perlu diadakan penelitian lanjutan sehingga memberikan efek preventif pada lansia DMT2 dengan obesitas.

Bakgrund: Diabetes typ 2 (DMT2) står för majoriteten av all diabetes i Sverige som internationellt, vilket har satts i samband med vår moderna livsstil, minskad fysisk aktivitet och en tilltagande ökning av kroppsvikt. Orsaken kan t.ex. relateras till patientens ärftliga betingad eller individens levnadsvanor. Egenvård är viktig för att förebygga eller minska senkomplikationer. Syftet: Syftet med det föreliggandeexamensarbetetvar attbeskriva vuxna patienters erfarenheter av egenvård vid DMT2samt granska datainsamlingsmetoder och urvalsgrupp använts i artiklarna.  Metod: Litteraturstudieutfördes med en beskrivande design där sammanfattas 12 kvalitativa artiklar. Huvudresultat:Patienter hade positiv inställning livsstilsförändringar och började ta ansvar för sin egenvård. En del patienter hade svårigheter att ta till sig kunskaper och tillämpa dessa i sin egenvård, arbetssituation påverkad, låg inkomst och en känsla av utanförskap både från hemmet och omgivningen. En del av patienter upplevde socialt stöd från familj och närstående samt hade god kontakt med vården.  Slutsats:Föreliggande examenarbetevisade att patientens levnadssituation, relation till sin omgivning måste sättas i centrum. Patienten möjlighet att få en vårdkontakt förbättras, med syfte att ge individanpassad kunskap, motivera och ge mer tid. Vidare forskning bör undersöka familjens förhållningssätt till personen som har DMT2 för att få en djupare förståelse om vad familjen kan göra för att skapa en holistisk syn.  Nyckelord: DMT2, Egenvård, Erfarenhet, Vuxna patienter<br>Background:Diabetes type 2 (DMT2) accounts for the majority of all diabetes in Sweden as well as global, which has been associated with our modern lifestyles, reduced physical activities and an increase in body weight. Causes can be, for example, related to the patient's hereditary condition or the individual's living habits. Self-care is important to prevent or reduces the late complications. Aim: The aim of the present thesis was to describe adult patients’ experiences of self-care with DMT2 and examine the data collection methods and sample group used of the articles. Methods: A literature study was conducted with a descriptive design summarizing 12 qualitative articles. Main result: Patients had positive attitude to lifestyle changes and began taking responsibility for their self-care. Some patients had difficulty in acquiring knowledge and applying them for their own care, affected in work situation, weak income and a sense of alienation both at home and surrounding. Some of patients experienced social support from family and close relatives and had good contact with healthcare. Conclusion: The present study shows that the patient's living situation, relationship with his surrounding, must be placed at the center. The patient possibilities for care contact should be improved in order to give him individualized knowledge, motivate and give more time.  Further research should investigate the family's approach to the person with DMT2 to gain a deeper understanding of what the family can do to create holistic view. Abstract The aim of this thesis was to measure patient reported outcomes, such as health related quality of life and general diabetes knowledge of elderly diabetes mellitus type 2 (DMT2) patients in Slovenia. Patient reported outcomes demonstrate patient perspectives when evaluating the delivery of care. In Slovenia, a new, multidisciplinary model of chronic care was introduced in 2011, which yet needs to demonstrate the efficiency, care improvement and cost reduction. Thus, another aim of the study was to research if multidisciplinary teams in diabetes care are economically viable. To achieve the above, the study was divided into 4 subprojects. Firstly, the general level of diabetes knowledge of elderly DMT2 patients (n=179) was measured. Secondly, the reliability and validity of generic, European Quality of Life- 5 Dimensions (EQ-5D), and disease specific, Audit on Diabetes Dependent Quality of Life (ADDQoL ) instruments were examined. Thirdly, health related quality of life of elderly diabetic patients (n=285) was assessed. Fourthly, the systematic literature review on the cost-effectiveness of multidisciplinary teams was conducted. The data were collected during the period 2011–2012. The main contributions of the current thesis can be summarised as follows: This was the first study to measure general diabetes knowledge of elderly DMT2 patients in Slovenia. The results showed that the place of living does not have an impact neither on diabetes knowledge nor the health related quality of life of these patients. Secondly, a pioneering example of measuring health related quality of life (HRQoL) in elderly diabetic patients in Slovenia, using a validated and reliable instrument (ADDQoL) was provided. A study to evaluate the relationships between diabetic and other co-existing chronic medical conditions on health related quality of life was performed. As part of that study, the reliability and validity of the instruments (EQ-5D and ADDQoL) were measured, and the analysis showed that both instruments are reliable. Thirdly, a systematic way of finding evidence for understanding the cost-effectiveness of multidisciplinary teams was applied. The results of the literature review show weak improvements in the economic outcomes. In general, the thesis contributes to the improved understanding of patient reported outcomes in elderly diabetic patients, which can be a measure in assessing diabetes care program in Slovenia, and offers a basis for a national evaluation of the Model Practices. Furthermore, patient reported outcomes of elderly diabetic patients is important to Slovenian decision makers to identify and implement appropriate interventions for achieving better management of diabetes and ultimately improving the quality of life of diabetes patients<br>Tiivistelmä Tutkimuksen tarkoitus oli mitata iäkkäiden tyypin 2 diabetespotilaiden itse ilmoittamia tuloksia, kuten terveyteen liittyvä elämänlaatu ja yleinen diabetekseen liittyvä tietämys Sloveniassa. Tulokset valottavat potilaiden näkökulmaa hoidon arvioinnissa. Sloveniassa otettiin 2011 käyttöön monitieteellinen hoitomalli, jonka tehoa sekä kykyä parantaa hoitoa ja vähentää hoitokustannuksia ei ole osoitettu. Toisena tavoitteena oli selvittää, onko moniammatillisten tiimien käyttö diabeteshoidossa taloudellisesti järkevää. Tutkimus jaettiin neljään osaprojektiin. Ensin mitattiin iäkkäiden tyypin 2 diabetespotilaiden (n=179) yleistä diabetestietämystä. Toiseksi selvitettiin geneerisen European Quality of Life- 5 Dimensions- (EQ-5D) ja diabeteskohtaisen ADDQoL -mittarin luotettavuus ja validiteetti. Tulosten mukaan asuinpaikka ei vaikuta potilaiden diabetestietämykseen tai terveyteen liittyvään elämänlaatuun. Kolmanneksi, tutkimuksessa etsittiin systemaattisesti näyttöä moniammatillisten tiimien kustannustehokkuuden arvioimiseksi. Kirjallisuuskatsauksen mukaan taloudellinen tulos paranee vain vähän. Tutkimus lisää tietoa potilaiden ilmoittamista tuloksista iäkkäiden diabetespotilaiden kohdalla, mitä voidaan käyttää diabeteshoito-ohjelman arvioinnissa Sloveniassa sekä mallikäytäntöjen kansallisen arvioinnin perustana. Abstract:
The ESR g-value and susceptibility measurements for DMTM(TCNQ)₂ have been studied as a function of angle made by crystal with magnetic field and temperature. The angular dependence of g-value is fitted to g² = α+βcos2θ - γsin2θ for three orthogonal directions of crystal rotation. The principal g-values are close to those expected for TCNQ compounds: g₁ = 2.0034, g₂ = 2.0030, g₃ = 2.0024. The susceptibility as a function of temperature agrees with bulk susceptibility measurements except that the maximum position occurs at about 30 K. The results of Oostra et al. for bulk susceptibility showed a maximum at around 50 K. The phase transition reported by Visser et al. at 272 K is observed in the ESR data as a 15% decrease in susceptibility. The linewidth is remarkably anisotropic typical of TCNQ salts. The phase transition study is done for two orientations of the crystal with the magnet field. In one of the orientations the linewidth narrows from 0.15 to 0.11 gauss and in the other it narrows from 0.24 to 0.18 gauss. In the former case there is a growth of a second line due to the twinned stack; transformation twinning takes place at the phase transition. Dissertation presented to obtain the PhD degree in Biology<br>It may come as a surprise that vertebrates are not entirely bilaterally symmetric. In fact, at the internal level they are asymmetric with the organs acquiring a biased disposition along the left-right (LR) axis. Problems in the establishment of both symmetric and asymmetric features may give rise to several human developmental disorders. Thus, it is very important to study in more detail how LR embryonic development is initiated and maintained. The built of the axial skeleton and its associated skeletal muscles is dependent on somite formation and differentiation. The somites are formed as pairs and are bilaterally symmetric between both sides of the LR axis. Interestingly, it has been suggested that asymmetric somite formation may account for problems at the level of the axial skeleton. At the same time that the somites are being formed, asymmetric signals are being transferred only to the left side of the axis being responsible to set up the correct asymmetric internal organs distribution. In fact, an incorrect transfer of information leads to serious problems in organ situs. It has become clear that several mechanisms are necessary to initiate the LR asymmetry pathway however, something that only recently has been acknowledge is that symmetry is not a default state and there are in fact mechanisms responsible for promoting it. It is not completely understood how, but there are indications that there is a crosstalk between the mechanisms necessary to regulate symmetry vs. asymmetry. Surprisingly, a zebrafish transcription factor named Dmrt2a/ Terra, a gene that belongs to a family mainly associated with sex determination, as been shown to play a role in regulating these two contradictory features. Not only is it necessary for bilateral symmetric somite formation but also for the correct establishment of asymmetry in the left side of the axis. This thesis examines the dynamic mechanical properties of natural silk fibres, mainly from silkworm species Bombyx mori (B. mori) and spider species Nephila edulis, using dynamic mechanical thermal analysis, DMTA. The aim is not only to provide novel data on mechanical properties of silk, but also to relate these properties to the structure and morphology of silk. A systematic approach is adopted to evaluate the effect of the three principal factors of stress, temperature and hydration on the properties and structure of silk. The methods developed in this work are then used to examine commercially important aspects of the ‘quality’ of silk. I show that the dynamic storage modulus of silks increases with loading stress in the deformation through yield to failure, whereas the conventional engineering tensile modulus decreases significantly post-yield. Analyses of the effects of temperature and thermal history show a number of important effects: (1) the loss peak at -60 °C is found to be associated the protein-water glass transition; (2) the increase in the dynamic storage modulus of native silks between temperature +25 and 100 °C is due simply to water loss; (3) a number of discrete loss peaks from +150 to +220°C are observed and attributed to the glass transition of different states of disordered structure with different intermolecular hydrogen bonding. Excess environmental humidity results in a lower effective glass transition temperature (Tg) for disordered silk fractions. Also, humidity-dynamic mechanical analysis on Nephila edulis spider dragline silks has shown that the glass transition induces a partial supercontraction, called Tg contraction. This new finding leads to the conclusion of two independent mechanisms for supercontraction in spider dragline silks. Study of three commercial B. mori cocoon silk grades and a variety of processed silks or artificial silks shows that lower grade and poorly processed silks display lower Tg values, and often have a greater loss tangent at Tg due to increased disorder. This suggests that processing contributes significantly to the differences in the structural order among natural or unnatural silks. More importantly, dynamic mechanical thermal analysis is proposed to be a potential tool for quality evaluation and control in silk production and processing. In summary, I demonstrate that DMTA is a valuable analytical tool for understanding the structure and properties of silk, and use a systematic approach to understand quantitatively the important mechanical properties of silk in terms of a generic structural framework in silk proteins.

A&barbelow;symmetric d&barbelow;igital s&barbelow;ubscriber l&barbelow;ine (ADSL) technology provides transport of high-bit-rate digital information over digital subscriber lines (i.e., common telephone lines), which were initially engineered to carry a single voice signal with a 3.4 kHz bandwidth. Sophisticated digital transmission techniques have to be utilized to overcome digital subscriber line transmission impairments due to the high signal attenuation, i&barbelow;nter-s&barbelow;ymbol i&barbelow;nterference (ISI), c&barbelow;rosst&barbelow;alk noise (XT) from the signals present on adjacent wires, signal reflections, radio-frequency noise, and impulse noise. In current ADSL standard, a multicazrier modulation technique named d&barbelow;iscrete m&barbelow;ultit&barbelow;one (DMT) is used to deal with the impairments caused by heavy ISI and signal reflections. A concatenated channel coding structure is employed to efficiently increase the transmission capability in the presence of strong additive crosstalk noise. This channel coding scheme consists of an inner 4&barbelow;-D&barbelow;imensional T&barbelow;rellis C&barbelow;oded M&barbelow;odulation (4D-TCM) and an outer R&barbelow;eed-S&barbelow;olomon (RS) code separated by an interleaver. In this thesis, we investigated the potential transmission performance improvement by using Turbo code in ADSL DMT systems. To achieve this, we first proposed a turbo coding structure that fits well in the current ADSL DMT system architecture. This structure is very similar to the existing concatenated channel coding structure, except a bandwidth efficient T&barbelow;urbo T&barbelow;rellis C&barbelow;oded M&barbelow;odulation (TTCM) is used instead of the 4D-TCM. To obtain high bandwidth efficiency, only part of the bits are turbo coded and the uncoded bits are protected by a constellation mapping similar to Ungerboeck's set partitioning in TCM. The proposed TTCM was shown to provide extremely good performance when a S&barbelow;ymbol M&barbelow;aximum a&barbelow; p&barbelow;osteriori (SMAP) decoding algorithm is employed. In spite of the good performance of this SMAP algorithm, we proposed to use a simplified TTCM decoding structure to avoid the excessive decoding complexity introduced by the possibly large number of uncoded bits. The proposed turbo coded ADSL DMT system was simulated to evaluate the achievable transmission performance in a stationary noise environment and its performance was compared to that obtained by using the 4D-TCM. Simulation results showed significant performance improvements by using turbo codes.

The treatment of multiple sclerosis has become much more complicated with the development of new oral and other disease-modifying therapies (DMT) that have been added to the formulary of older injectables and natalizumab. To date, twelve medications have been approved in the United States for use in cases of relapsing multiple sclerosis. Neurologists managing patients with multiple sclerosis must integrate numerous variables when selecting the appropriate agent for each individual. This chapter presents an overview of the DMTs currently approved by the U.S. Food and Drug Administration for multiple sclerosis and suggests a therapeutic algorithm to aid in choosing among these medications.

Introdução: Diabetes Mellitus tipo 2 (DMT2) é uma doença por decorrência de resistência a insulina nas células musculares e adiposas devido à redução de sua ação, resultando em hiperinsulinismo e hiperglicemia. O DMT2 é o mais prevalente, corresponde de 90% a 95% dos casos e acomete em sua maioria, indivíduos adultos. Dados da Sociedade Brasileira de Diabetes mostram que o número de casos do DMT2 eleva a cada ano, em consequências de numerosas condições que influenciam sua etiologia, como fator genético, obesidade, sedentarismo, crescimento e envelhecimento populacional, tabaco, dentre outros fatores. Objetivo: O Diagnóstico depende de várias avaliações para sua definição. Sendo assim, o objetivo desse estudo é reunir os principais métodos laboratoriais utilizados nos laboratórios de análises clínicas, no Brasil. Materiais e métodos: Foram utilizados conteúdos dos últimos 8 anos para revisão bibliográfica, compostos por livros, artigos científicos, dissertações e órgãos. Através da busca nas seguintes bases de dados “Google Acadêmico” e “Sociedade Brasileira de Diabetes”. Resultados e Discussão: Sua identificação é realizada através dos seguintes exames: Para a glicemia de jejum, é de suma importância que o exame seja realizado com no mínimo 8 horas de jejum. Seu valor de referência é &lt;100mg/dl, pré-diabetes vão de ≥100 a &lt;126mg/dl e para DMT2 é ≥126 mg/dl. O teste oral de tolerância a glicose é realizado após 2 horas da ingestão de 75g de glicose em 250 a 300ml de água, fazer a ingestão durante 5 minutos. Esse exame deve seguir alguns critérios para sua execução, como jejum de 10 a 16 horas e fazer consumo de pelo menos 150g de carboidratos por dia durante os 3 dias anteriores. Seu valor de referência é &lt;140mg/dl, pré-diabetes vão de ≥140 a &lt;200mg/dl e para DMT2 é ≥ 200mg/dl. A Hemoglobina glicada não é específica para DMT2, podendo ter interferência de outras doenças e etnia para seu diagnóstico. O valor de referência para o DMT2 é ≥ 6,5%. Conclusão: É de suma importância um diagnóstico precoce e fidedigno do Diabetes Mellitus, para que medidas sejam tomadas antecipadamente impedindo seu desencadeamento ou postergar o surgimento de complicações crônicas nos indivíduos.

El avance en la compresión de este tipo de conectividad y los mecanismos de adaptación del sistema nervioso, desde las moléculas hasta las conexiones sinápticas, las redes neuronales y el comportamiento humano, ha ido generando una nueva perspectiva en la investigación del funcionamiento cerebral y sus mecanismos de recuperación de regeneración. En los últimos años, los investigadores han buscado mecanismos subyacentes a esta capacidad, identificando posibles factores epigenéticos, biomarcadores y algunos procesos cognitivos relacionados, con el fin de determinar diagnósticos eficaces, prevención y pronóstico de la enfermedad y desarrollar terapias más potentes para mejorar el funcionamiento cognitivo y social, y la calidad de vida de las personas afectadas por trastornos mentales graves (TMG) como el trastorno depresivo mayor (TDM), el trastorno bipolar (TB) y la esquizofrenia (EZ), y el de otras con enfermedades crónicas que cursan con deterioro cognitivo como la diabetes mellitus tipo 2 (DMT2). Se han identificado terapias prometedoras, intervenciones neurofarmacéuticas y estimulación cerebral, que permiten optimizar el funcionamiento de estas personas, sin embargo, su eficacia para estimular la capacidad neuroplástica como tal o impedir su deterioro, sigue siendo muy limitada. La memoria, el aprendizaje, y funciones ejecutivas como la memoria de trabajo, la velocidad de procesamiento de la información y la flexibilidad cognitiva son dominios cognitivos que posibilitan que el ser humano pueda hacer cambios en su organismo para adaptarse a un entorno variable, por tanto se les podría relacionar directamente con la neuroplasticidad humana. Por esto, los nuevos métodos para evaluar y estimular la neuroplasticidad humana, además de la identificación e intervención sobre marcadores y mecanismos biológicos, deberían incluir en sus protocolos aquellos procesos cognitivos que dependen de la experiencia. El objetivo general de este trabajo fue describir la neuroplasticidad de personas con DMT2, TDM, TB, y EZ, en comparación con controles sanos (CS), desde un enfoque neuropsicológico, por medio de la evaluación de funciones cognitivas asociadas directamente a la neuroplasticidad humana como la memoria, el aprendizaje y la flexibilidad cognitiva. Se incluyeron 135 participantes: 30 con EZ, 41 con TB y 34 con TDM de acuerdo con los criterios del DSM-5 y un grupo de 30 CS; los cuales fueron evaluados en dos momentos distintos a lo largo de un año de seguimiento. Además de evaluar el estado clínico y el funcionamiento social, se utilizó una amplia batería para evaluar el funcionamiento cognitivo, seleccionando para los análisis las variables de interés: 1) Memoria (inmediata, a corto plazo y a largo plazo) evaluada con el TAVEC, 2) Aprendizaje evaluado con el subtest Dígitos-Directo del Wais-III y la variable V3-RI-AT del TAVEC; 3) Funciones ejectivas: a) Memoria de trabajo evaluada con el subtest Dígitos-Inverso del Wais-III y el TMT-B, b) Velocidad de procesamiento evaluada con el subtes Clave de Números del Wais-III y el TMT-A, c) Flexibilidad cognitiva evaluada con el WCST. Los resultados mostraron un funcionamiento cognitivo superior en el grupo de CS, permaneciendo estable en ambos momentos. Los grupos con diagnóstico de TMG tuvieron puntuaciones más bajas que los CS, con pocas diferencias significativas entre ellos. En la mayoría de las variables, las personas con TB y con EZ tuvieron puntuaciones medias similares, en comparación con las de las personas con TDM; estos hallazgos se mantienen a lo largo del tiempo. Se puede concluir que existe evidencia para sugerir que las funciones cognitivas asociadas a la neuroplasticidad se preservan a lo largo del tiempo en una población normal, mientras que en las personas adultas diagnosticadas con TMG se presenta un deterioro cognitivo estable, relacionado con una menor neuroplasticidad, y se comporta de manera similar e inferior en los grupos diagnosticados con EZ y BD que en personas con TDM.

In this investigated work, we have analysed the miscellaneous figure of merit for Double metal Triple gate TFET. Various techniques have been utilized to improve the ON-state driven current in the drain by doing a comprehensive analysis. Different techniques are examined and correlated by using the TCAD Silvaco tool to get excellent ON current. Further work function engineering has been done in the optimized DMTG-TFET to increase its performance and finally, we introduce pocket doping that increases the ON current (2.34×10-3) and also ION/IOFF ratio (4.36×1014) with subthreshold (SS) of 25.8mV/decade. The pocket doped DMTG-TFET adequately suppress the ambipolarity and endeavour about 20 times higher ION as compared to conventional DMTG-TFET.

Abstract:
Dynamic mold surface temperature control (DMTC) has the advantage of improving molded part qualities without significant increases in cycle time. A gas-assisted heating system combined with water cooling was developed to achieve DMTC for injection molding. With gas-assisted heating, it takes 2s for the mold surface temperature to vary from 60 °C to 120 °C whereas it requires 186s using water heating. Further, it takes 21s and 84s for the mold surface to cool to 60 °C under gas heating and water heating, respectively. The gas-assisted heating system also shows excellent efficiency for micro injection molding of biochips to achieve high replication accuracy of the micro channels.

The requirements for field research for Polish conditions demand proper dependences. Dependences based on more than 30 localizations for different genetic types of soils were derived for the area of Poland. Direct results from CPT, DMT and profiles from boreholes have been collected at the test sites for individual localization. For interpretation of results, dependences and diagrams of CPTU versus DMT were created. Correlations on the background of results from literature for different types of soils for numerous research areas were collected, with established dependences for Polish grounds conditions. Additionally some recommendations for the interpretation of the results from CPTU and DMT tests for analysed soils have been proposed.

Introdução: Neoplasias mamárias são as principais responsáveis pela mortalidade em animais de companhia. Um dos tratamentos é feito com a administração de fármacos citotóxicos que induzem danos químicos as células neoplásicas. A terapia mais utilizada segue o conceito da dose máxima tolerada (DMT) pelo paciente, tratamento este, cujos intervalos entre as doses favorece a regeneração das células endoteliais, permitindo a persistência da angiogênese tumoral. A Quimioterapia Metronômica (QM) consiste na observação dos agentes citotóxicos convencionais, administrados em baixas dosagens, exercendo efeitos antiangiogênicos, tornando-se uma alternativa pioneira viável para o tratamento de neoplasias mamárias, sendo um método de baixo custo que promove efeitos colaterais menores quando comparada a DMT. Objetivo: Analisar as implicações da QM em relação a terapia (DMT) no tratamento das neoplasias mamárias. Materiais e métodos: O presente estudo trata-se de uma revisão de literatura com base em dados adquiridos do Instituto Nacional do Câncer e Google Acadêmico. A pesquisa foi realizada a partir dos termos Quimioterapia animal, Quimioterapia Metronômica, Tratamento de câncer em animais e Quimioterapia convencional. Critérios para inclusão foram ano de publicação (2008 a 2016), ser do idioma português e os critérios de exclusão foram fuga parcial ou total aos objetivos da pesquisa e acesso pago. Resultados: Para amenizar os danos, efeitos colaterais e intervalos de tempo promovidos pela DMT, desenvolveram-se alternativas como a QM. O uso dessa terapia elimina longos períodos entre doses, diminuindo as chances de surgir alterações oportunistas nas células tumorais. O tratamento dos carcinomas mamários é feito pela administração oral de Ciclofosfamida em regime de QM através da ingestão do fármaco em baixa quantidade. Diferente da DMT que é realizada de maneira intravenosa. Evidências sugerem que a ciclofosfamida em regime de QM em cães e gatos, mostrou-se mais eficiente no tratamento de carcinomas mamários, em relação aos outros cânceres, devido ao seu efeito antigiogênico sobre as células tumorais. Conclusão: A comparação entre o tratamento com a terapia convencional e a terapia metronômica, mostrou que a QM é tão eficaz quanto a quimioterapia tradicional (DMT). Porém, a DMT ainda possui uma maior aplicabilidade, devido seus estudos e utilização mais prevalentes no campo veterinário.

&lt;p&gt;DMT’s Rope Testing Centre – a laboratory for non-destructive and destructive testing and part of TÜV NORD GROUP - has long-time experience in the inspection of steel wire ropes and cables in varying applications. DMT provides different non-destructive testing (NDT) methods, which allow a more detailed evaluation of the inner condition of the cables compared to only visual inspection. Ultrasonic testing (UT) at cable end connections is a very effective NDT method which is executed regularly by DMT experts. Another NDT method suitable to inspect the free length of stay or hanger cables is magnetic rope testing (MRT). Starting in 1931 with MRT on steel wire ropes, DMT has since then continuously developed and optimized this testing method and gained extensive operating skills.&lt;/p&gt;

INTRODUÇÃO: Pode-se identificar na literatura uma associação entre Diabetes Mellitus Tipo 2 (DM2), doença cardiovascular e transtorno depressivo. Ademais, a Síndrome Metabólica (SM) tem sido descrita como significativamente associada à mortalidade precoce em pacientes com DM2. A prevalência de SM é de mais de 80% entre os pacientes com DM2. SM se configura como uma condição clínica complexa a partir de fatores de risco cardiovasculares (dislipidemia, obesidade centrípeta, alteração na homeostase glicêmica e hipertensão arterial sistêmica) que se relacionam com a deposição central de gordura e com a resistência insulínica marcada pela hipersecreção de cortisol. No entanto, SM também é apresentada em associação com mortalidade precoce em indivíduos não-diabéticos. OBJETIVO: Investigar se ocorre uma associação entre SM e transtorno depressivo, independentemente de DM2. MÉTODO: Durante o mês de março de 2021, revisou-se publicações, em inglês, tendo como referência a base de dados MedLine via PubMed. Foi utilizado MeSH, a fim de obter as variações dos descritores e filtrados artigos publicados nos últimos treze anos. RESULTADOS: Foram incluídos na revisão sete estudos relacionados ao tema. Por um lado, resultados indicam que indivíduos com SM (sem e com DM2) podem desenvolver transtorno depressivo a partir de anormalidades ocorridas no eixo Hipotálamo-Hipófise-Adrenal a partir de dano no hipocampo, com ciclo vicioso de elevações crônicas do cortisol. Por outro, foi encontrado também que o transtorno depressivo se associa às citocinas pró-inflamatórias que podem induzir a resistência insulínica, contribuindo para o risco de SM. Os níveis dos marcadores inflamatórios (proteína C reativa e PAI-1) foram maiores entre pacientes com SM. Indivíduos com SM tiveram escores mais elevados para depressão (n=409, mean score 3.41, 95% CI 3.12-3.70) que aqueles sem SM (n=936, mean 2.95, 95% CI 2.76-3.13). Neste contexto, foi identificada uma prevalência de SM de 53,2% em pacientes deprimidos, enquanto no grupo controle foi de 32,3%. Finalmente, a maior parte dos resultados indicam que tanto SM configura-se como fator de risco para depressão como o inverso pode ocorrer. CONCLUSÃO: A grande maioria dos resultados encontrados indicam associação entre SM e transtorno depressivo, independentemente de DM2.

The conventional modal testing, hereafter referred as Displacement Modal Testing (DMT) is based on measurement of displacement, velocity or acceleration as well as excitation force. An enormous literature regards the DMT, on the contrary, a few papers address modal testing based on strain gauges or strain sensor, hereafter referred as Strain Modal Testing (SMT). The main reason of this scenario is due to practical problems in the use of strain gauges as calibration procedure, ground loop, sensitivity not adequate at high frequency, bonding quality. In this work, a novel piezoelectric strain sensor is used for SMT. It is demonstrated in the present work that this sensor overcomes the practical drawbacks related to the use of strain gauges. Thus, SMT based on piezoelectric strain sensors can be a valid alternative to DMT, usually based on accelerometers. Comparisons between the modal testing results concerning brackets with different constraint conditions using both accelerometers and strain sensors are given in terms of modal parameters, highlighting their pros and cons.

Abstract The new industrial revolution called Industry 4.0 embraces diverse Digital Manufacturing Tools (DMT). Trying to improve Product Life-cycle Management (PLM), some companies are trying to implement DMT to create Digital Twins (DT). New Product Introduction (NPI) demands large effort in digitalization and virtual simulation through the PLM process. Sometimes a new product could be only the development of a single component and/or an entire manufacturing process including complex instruments and controls. In this direction, it is important to accelerate Manufacturing Integrated Systems (MIS) by improving the automation not only in the NPI, but also in the PLM. This paper integrates Siemens PLM software as DMT called Tecnomatix Process Simulate (TPS), Totally Integrated Automation (TIA) and PLCSIM advanced. The scope in the NPI is showing how a Digital Twin could help the MIS. The aim of this paper is to evaluate the interconnectivity of a small physical prototype with its virtually simulated clone to support the virtual commissioning for a NPI.

Background Diabetes mellitus (DM) and depression are important comorbid conditions that can lead to more serious health outcomes. The American Diabetes Association (ADA) supports routine screening for depression as part of standard diabetes management. The PHQ2 and PHQ9 questionnaires are good diagnostic screening tools used for major depressive disorders in Type 2 diabetes mellitus (DM2). This quality improvement study aims to compare the rate of depression screening, treatment, and referral to behavioral health in adult patients with DM2 pre and post-integration of depression screening tools into the electronic health record (EHR). Methods We conducted a retrospective chart review on patients aged 18 years and above with a diagnosis of DM2 and no initial diagnosis of depression or other mental illnesses. Chart reviews included those from 2018 or prior for before integration data and 2020 to present for after integration. Sixty subjects were randomly selected from a pool of 33,695 patients in the clinic with DM2 from the year 2013-2021. Thirty of the patients were prior to the integration of depression screening tools PHQ2 and PHQ9 into the EHR, while the other half were post-integration. The study population ranged from 18-83 years old. Results All subjects (100%) were screened using PHQ2 before integration and after integration. Twenty percent of patients screened had a positive PHQ2 among subjects before integration, while 10% had a positive PHQ2 after integration. Twenty percent of patients were screened with a PHQ9 pre-integration which accounted for 100% of those subjects with a positive PHQ2. However, of the 10% of patients with a positive PHQ2 post-integration, only 6.7 % of subjects were screened, which means not all patients with a positive PHQ2 were adequately screened post-integration. Interestingly, 10% of patients were treated with antidepressants before integration, while none were treated with medications in the post-integration group. There were no referrals made to the behavior team in either group. Conclusion There is no difference between the prevalence of depression screening before or after integration of depression screening tools in the EHR. The study noted that there is a decrease in the treatment using antidepressants after integration. However, other undetermined conditions could have influenced this. Furthermore, not all patients with positive PHQ2 in the after-integration group were screened with PHQ9. The authors are unsure if the integration of the depression screens influenced this change. In both groups, there is no difference between referrals to the behavior team. Implications to Nursing Practice This quality improvement study shows that providers are good at screening their DM2 patients for depression whether the screening tools were incorporated in the EHR or not. However, future studies regarding providers, support staff, and patient convenience relating to accessibility and availability of the tool should be made. Additional issues to consider are documentation reliability, hours of work to scan documents in the chart, risk of documentation getting lost, and the use of paper that requires shredding to comply with privacy.
