Abstract:
This article highlights how can companies archive their 3D CAD files as their software races toward obsolescence. Digital designs, though, are created on software and computers that are outdated when they are delivered. Computer files can be hard to retrieve in as little as five years down the road. This is a big problem for the engineering community and, of course, for corporations, government agencies, and organizations that store information digitally—in short, for everyone. Most information today—not just engineering data—is created and stored digitally on computer systems that become outdated sooner than bread gets stale. Companies may also store blueprints or CAD documents as portable document files (PDFs) or as tagged image files (TIFs). These are 3D digital files that can be accessed fairly universally from any computer. Again, much is lost, including geometry, when swooshing a 3D file as flat as a pancake.

Abstract:
The data warehouse community has paid particular attention to the document warehouse (DocW) paradigm during the last two decades. However, some important issues related to the semantics are still pending and therefore need a deep research investigation. Indeed, the semantic exploitation of the DocW is not yet mature despite it representing a main concern for decision-makers. This paper aims to enhancing the multidimensional model called Diamond Document Warehouse Model with semantics aspects; in particular, it suggests semantic OLAP (on-line analytical processing) operators for querying the DocW.

Abstract:
Background The growing availability of clinical and administrative data collected in electronic health records (EHRs) have led researchers and policy makers to implement data warehouses to improve the reuse of EHR data for secondary purposes. This approach can take advantages from a unique source of information that collects data from providers across multiple organizations. Moreover, the development of a data warehouse benefits from the standards adopted to exchange data provided by heterogeneous systems. Objective This article aims to design and implement a conceptual framework that semiautomatically extracts information collected in Health Level 7 Clinical Document Architecture (CDA) documents stored in an EHR and transforms them to be loaded in a target data warehouse. Results The solution adopted in this article supports the integration of the EHR as an operational data store in a data warehouse infrastructure. Moreover, data structure of EHR clinical documents and the data warehouse modeling schemas are analyzed to define a semiautomatic framework that maps the primitives of the CDA with the concepts of the dimensional model. The case study successfully tests this approach. Conclusion The proposed solution guarantees data quality using structured documents already integrated in a large-scale infrastructure, with a timely updated information flow. It ensures data integrity and consistency and has the advantage to be based on a sample size that covers a broad target population. Moreover, the use of CDAs simplifies the definition of extract, transform, and load tools through the adoption of a conceptual framework that load the information stored in the CDA in the data warehouse.

Abstract:
The traditional data warehousing approaches should adapt to take into consideration novel needs and data structures. In this context, NoSQL technology is progressively gaining a place in the research and industry domains. This paper proposes an approach for building a NoSQL document-oriented warehouse (DocW). This approach has two methods, namely 1) document warehouse builder and 2) NoSQL-Converter. The first method generates the DocW schema as a galaxy model whereas the second one translates the generated galaxy into a document-oriented NoSQL model. This relies on two types of rules: structure and hierarchical rules. Furthermore, in order to help understanding the textual results of analytical queries on the NoSQL-DocW, the authors define two semantic operators S-Drill-Up and S-Drill-Down to aggregate/expand the terms of query. The implementation of our proposals uses MangoDB and Talend. The experiment uses the medical collection Clef-2007 and two metrics called write request latency and read request latency to evaluate respectively the loading time and the response time to queries.

Abstract:
This article describes how, in the era of big data, a data warehouse is an integrated multidimensional database that provides the basis for the decision making required to establish crucial business strategies. Efficient, effective analysis requires a data organization system that integrates and manages data of various dimensions. However, conventional data warehousing techniques do not consider the various data manipulation operations required for data-mining activities. With the current explosion of text data, much research has examined text (or document) repositories to support text mining and document retrieval. Therefore, this article presents a method of developing a text warehouse that provides a machine-learning-based text classification service. The document is represented as a term-by-concept matrix using a 3rd-order tensor-based textual representation model, which emphasizes the meaning of words occurring in the document. As a result, the proposed text warehouse makes it possible to develop a semantic Naïve Bayes text classifier only by executing appropriate SQL statements.

Abstract:
Purpose The purpose of this paper is to document and appraise two innovations by which nontraditional forms of collateral are being used to make smallholder crop and livestock farmers bankable in Ghana and Zimbabwe. Design/methodology/approach The setup and operations of the warehouse receipt system (WRS) in Ghana were evaluated for the extent to which the WRS was meeting crop farmers’ expectations and the WRS’s own objectives. Owners of the WRS, a certified warehouse operator in a big city, and two operators of certified community warehouses in farming communities were interviewed. Two focus group discussions with crop farmers were also held. Information about the setup and operations of the Tawanda Nyambirai Livestock Trust (TNLT) Private Limited in Zimbabwe (TNLT) and extent of serving the credit needs of livestock farmers was obtained by telephone from the managing director. Data were gathered in April 2014 and were analyzed later. Findings Due to low output no smallholder farmer targeted by the WRS had been issued with a tradable certified warehouse receipts to serve as collateral to potential lenders. Grain aggregators (non-farmers) have aggregated enough grains from farmers to be issued warehouse receipts. Grain farmers report substantial reduction in post-harvest losses when they lodge farm proceeds with certified community warehouses. For the TNLT, more than 140 farmers had deposited 700 cattle and had been issued with tradable certificates of deposit within one year of TNLT to obtain revolving credit from one bank. Other benefits and challenges are highlighted. Originality/value Both approaches have potential of helping to solve liquidity constraints of farmers.

Abstract:
The article addresses the problem of redistribution of warehouse resources when working in a virtual organization or when changing the logistics structure of the enterprise. A two-stage model of providing warehouse resources to a common access to participants of a virtual en-terprise is considered. An automated system for managing redistribution of resources for the stage of disconnecting from a virtual enterprise or when closing your own warehouses was developed. A description of the control object - a distributed warehouse system of an industrial enterprise specializing in the production of sheet metal. A formalized description of the technological process for the production of cold-rolled steel, including the stages of stockpiling, has been developed. The task of managing the distribution of resources is posed, the functional structure of the software complex is designed. Information, mathematical and algorithmic support for solving the problem of redistributing resources is presented. The architecture of information support includes a database containing regulatory and reference information and document management of the enterprise. The database information is used by the algorithmic support of four functional modules: management of normative and reference information, management of receipts and sales, management of warehouse movements, management of multi-turn packaging. The result of the work is presented by the system of dynamic redistribution of warehouse resources and implemented on the 1C: Enterprise platform, which allows solving the problems of the operational movement of material resources between the enterprise’s warehouses, also under the conditions of the virtual enterprise functioning. Absolute and relative distribution criteria are formed. The first group includes absolute criteria: volume, quantity, amount, weight. The second group consists of relative distribution criteria: quantity/volume, amount/volume, weight/volume. Testing the system using the example of criteria from each group showed the operability of all types of support using the example of a warehouse complex of an enterprise manufacturing metal rolling. Flexible customizable system structure allows you to expand the application of the proposed algorithmic support to other types of resources.

Declining of agricultural commodity prices during the harvest season has become detrimental to farmers. It often makes farmers not harvest their crops because the cost of harvesting is more expensive than the selling price of the product. The government makes a Warehouse Receipt System (WRS) program to help farmer to solve the cost of farm business and anticipation of low price of farmer income in harvest time. The goal of the research is to analyze and to describe regarding the potency and challenge of warehouse receipts system implementation in Niaga Mukti Cooperation, Cianjur. The method used was descriptive analysis with the subject of Niaga Mukti Cooperation as the sample of WRS for rice commodity. The result of the research shows that warehouse receipts system has potency to become collateral credit document to obtain farming budgeting for the member of Niaga Mukti cooperation with 6% per year or 1,5% per month after receiving government grant. In conducting WRS program, the cooperation confronts various challenges such as economic society empowerment through WRS, warehouse empowerment receipts as alternative finance, finance instruments and agricultural marketing. The process of extracting comparative heterogeneous web content data which are derived and historical from related web pages is still at its infancy and not developed. Discovering potentially useful and previously unknown information or knowledge from web contents such as “list all articles on ’Sequential Pattern Mining’ written between 2007 and 2011 including title, authors, volume, abstract, paper, citation, year of publication,” would require finding the schema of web documents from different web pages, performing web content data integration, building their virtual or physical data warehouse before web content extraction and mining from the database. This paper proposes a technique for automatic web content data extraction, the WebOMiner system, which models web sites of a specific domain like Business to Customer (B2C) web sites, as object oriented database schemas. Then, non-deterministic finite state automata (NFA) based wrappers for recognizing content types from this domain are built and used for extraction of related contents from data blocks into an integrated database for future second level mining for deep knowledge discovery.

Abstract:
Background Clinical data warehouses are now widely used to foster clinical and translational research and the Informatics for Integrating Biology and the Bedside (i2b2) platform has become a de facto standard for storing clinical data in many projects. However, to design predictive models and assist in personalized treatment planning in cancer or radiation oncology, all available patient data need to be integrated into i2b2, including radiation therapy data that are currently not addressed in many existing i2b2 sites. Objective To use radiation therapy data in projects related to rectal cancer patients, we assessed the feasibility of integrating radiation oncology data into the i2b2 platform. Methods The Georges Pompidou European Hospital, a hospital from the Assistance Publique – Hôpitaux de Paris group, has developed an i2b2-based clinical data warehouse of various structured and unstructured clinical data for research since 2008. To store and reuse various radiation therapy data—dose details, activities scheduling, and dose-volume histogram (DVH) curves—in this repository, we first extracted raw data by using some reverse engineering techniques and a vendor's application programming interface. Then, we implemented a hybrid storage approach by combining the standard i2b2 “Entity-Attribute-Value” storage mechanism with a “JavaScript Object Notation (JSON) document-based” storage mechanism without modifying the i2b2 core tables. Validation was performed using (1) the Business Objects framework for replicating vendor's application screens showing dose details and activities scheduling data and (2) the R software for displaying the DVH curves. Results We developed a pipeline to integrate the radiation therapy data into the Georges Pompidou European Hospital i2b2 instance and evaluated it on a cohort of 262 patients. We were able to use the radiation therapy data on a preliminary use case by fetching the DVH curve data from the clinical data warehouse and displaying them in a R chart. Conclusion By adding radiation therapy data into the clinical data warehouse, we were able to analyze radiation therapy response in cancer patients and we have leveraged the i2b2 platform to store radiation therapy data, including detailed information such as the DVH to create new ontology-based modules that provides research investigators with a wider spectrum of clinical data.

Abstract:
Brazilian public hospitals face several operational problems not only related to poor public management practices and their complex nature, but also the economic, and social contexts. Considering this scenario and the fact that efforts in supply management might affect a hospital organization’s excellence, this research aims to identify improvements in the logistic operations at the surgical center satellite warehouse of a Public Hospital located in Brazil. A case study based on an exploratory and qualitative approach was conducted by employing document analysis, semi-structured interviews, and on-site observations. Seven major problems concerning lack of surgical material, the non-definition of crucial logistic parameters, low information flow, surgical supply control, and management problems were pointed and addressed by seven independent but complementary actions that considered the Brazilian healthcare system’s particularities. Given the nature of exploratory research, the results are not exhaustive and cannot be generalized to different contexts. However, they help understand that reducing the waste of the logistics processes makes it possible to improve the attention to the local population that uses public health services.

Abstract:
AbstractObjectiveNatural language processing (NLP) engines such as the clinical Text Analysis and Knowledge Extraction System are a solution for processing notes for research, but optimizing their performance for a clinical data warehouse remains a challenge. We aim to develop a high throughput NLP architecture using the clinical Text Analysis and Knowledge Extraction System and present a predictive model use case.Materials and MethodsThe CDW was comprised of 1 103 038 patients across 10 years. The architecture was constructed using the Hadoop data repository for source data and 3 large-scale symmetric processing servers for NLP. Each named entity mention in a clinical document was mapped to the Unified Medical Language System concept unique identifier (CUI).ResultsThe NLP architecture processed 83 867 802 clinical documents in 13.33 days and produced 37 721 886 606 CUIs across 8 standardized medical vocabularies. Performance of the architecture exceeded 500 000 documents per hour across 30 parallel instances of the clinical Text Analysis and Knowledge Extraction System including 10 instances dedicated to documents greater than 20 000 bytes. In a use–case example for predicting 30-day hospital readmission, a CUI-based model had similar discrimination to n-grams with an area under the curve receiver operating characteristic of 0.75 (95% CI, 0.74–0.76).Discussion and ConclusionOur health system’s high throughput NLP architecture may serve as a benchmark for large-scale clinical research using a CUI-based approach.

Abstract:
Summary Objectives: Our objectives were to develop, based on the analysis of archived clinical trial documents and data and on the requirements of GCP-compliant electronic archiving, a concept for legally secure and technically feasible archiving of the entire clinical trial, including the essential documents of the trial master file and the study database. Methods: Based on own experiences with CDISC, existing implementations and future developments, CDISC standards were evaluated concerning requirements for archiving clinical studies. Trial master files of a small, medium and large clinical study were analyzed to collect specifications for electronic archiving of records. Results: Two different ways of long-term storage exist for the clinical trial archive: document-oriented archival and data archiving of the study database. The trial master file has a highly complex structure; its different parts can vary greatly in size, depending of the working style of investigators, number of patients recruited, the number of adverse event reports and the number of queries. The CDISC standard ODM is especially suited for archiving clinical trials, because among other features it contains the entire clinical trial data and full audit trail information. On the other hand SDTM is a content standard suited for data warehouses. Two recent developments in CDISC will affect the archival of studies: the further development of ODM in the area of “eCRF submission” and the use of “Electronic Source Data”. Conclusions: The complexity and size of the trial master file requires new solutions. Though ODM provides effective means to archive the study database, it shows still deficiencies, especially for the joint archiving of data and the complex documentation of the trial master file. A concept was developed in which the ODM standard is part of an integrated archiving of the trial data and documents. ODM archiving of the study database enables long-term storage which is GCP-compliant. Archiving of documents of the trial master file in PDF/A, including links and electronic signatures, as well as the storage of selected study data in a data warehouse at the sponsor site in SDTM are the other components of the concept.

Abstract:
The article investigates the methodological aspects of the management process in the field of the logistics customer services in terms of improving its digitalization potential. The phenomenon of digitalization has been studied insufficiently and, according to some foreign authors, it has significant potential for development from the theoretical perspective. However, the digitalization of logistics in Ukraine is only gaining popularity in the scientific community. Consequently, a large number of domestic enterprises have certain difficulties while they are implementing innovations and digital solutions into the optimization of their activity. This actualizes the problems of this study as well as determines the aim and the practical tasks of the given article. In particular, it has been determined that the potential for digitalization of logistics processes is manifested primarily in increasing the management quality and efficiency, the possibility of timely correction of threats and risks that arise, and in facilitating analytical processing and obtaining more accurate assessment of large amounts of information. To identify the digitalization potential of the enterprise logistics, the article identifies the key changes that the logistics operator delivering goods within the territory of Ukraine based on the implementation of a range of digitalization tools may deal with. Thus, the primary optimization measures taken in the process of implementing digitalization at the researched enterprise are the following: digitalization of document circulation at the enterprise, the process of loading and unloading delivery vehicles during the working day, the system of cargo placement in the warehouse. These measures will optimize various processes and reduce the influence of the human factor. Taken together, this will speed up the processes and increase the quality of their implementation by reducing the number of errors and delays. To optimize intra-warehouse movements and the delivery process by digitalization of operating processes carried out by the logistics operator, it is advisable to prioritize the implementation of not only electronic document management system but also the following digital tools: RFID systems – for the efficient placement of goods in the warehouse and quick determination of their location, based on the use of RFID tags; GPS tracking – to increase the efficiency of vehicle use and reduce the cost of vehicle tracking, based on the use of GPS trackers; scale scanners as advanced weighing and measuring technologies that scan the load, determine its weight and volume, display the results and send them to the system. The authors applied the AHP method to justify the application sequence of each technology and form an optimal digitalization plan as exemplified by the logistics operator under investigation. Using a multicriteria approach to assess the feasibility of implementing each digital solution based on the survey data, the respondents of which include company executives and experts of the considered domestic logistics operator, the optimal sequence of introducing digital technologies in this company has been identified. Therefore, it is recommended to introduce the RFID system first. Then, the electronic document management should be launched. After that, it is recommended to introduce a scale scanner. Finally, the GPS system should be applied. Using the recommended algorithm for modeling the digitalization process and key changes that may occur at the investigated logistics operator resulting from digitalization tools set implementation, will allow domestic logistics operators to make optimal decisions.

Abstract:
Academic work on crime and punishment has focused mostly on urban centers, leaving rural communities understudied, except for acknowledgement that rural communities warehouse a large number of prisoners and that rural prisons provide jobs and economic development for some struggling communities. This study uses a novel dataset that includes information on the home addresses of all prisoners in Arkansas from 1993 to 2003 to document imprisonment rates and racial disparities in imprisonment rates across metropolitan and nonmetropolitan counties. We show how rural communities both receive and produce prisoners and that imprisonment and racial disparities in imprisonment vary more within different types of communities than across different types of communities. Further, we find that nonmetropolitan rates of imprisonment are higher than would be expected, based on observed local risk factors such as poverty rate. We close with a discussion of what these findings illustrate about concentrated disadvantage across the rural-urban interface.

Abstract:
On the basis of theory of stationary thermal explosion of A.D. Frank-Kamenetsky, depending on the temperature of the critical size of the ammonium nitrate embankment, the calculations were performed, which show that it can be stored in the large volumes at the temperatures up to 30 °C. On the contrary, at the temperatures above 100 °C (for example, at 200 °C), the decomposition of nitrate occurs with acceleration and can lead to an explosion. Based on the studies performed, it is shown that the changes and additions to the fire safety requirements for ammonium nitrate storage in the buildings and structures should be determined by the requirements for fire resistance of buildings (at least II degree of fire resistance), for the purity of the product and its packaging, for the exclusion of contacts with organic substances and materials, storage conditions. Additional fire safety requirements were developed for inclusion in the normative document regarding ammonium nitrate storage. Ammonium nitrate is allowed to be stored in one-story warehouse buildings of at least II degree of fire resistance, structural fire hazard class C0. The floor area within the fire compartment should not exceed 10 500 m2. Between the fire-fighting walls of the 1st type, it is allowed to store no more than 25 000 tons of nitrite in bulk or in the special bags, as well as in the soft specialized containers for bulk products in accordance with GOST 2—2013.The conditions for placing ammonium nitrate in the stacks should be accepted in accordance with the requirements of SP 92.13330.2012. Temperature in the storage room of ammonium nitrate should not exceed 30 °C with a relative humidity of not more than 60 %. Warehouses for storing ammonium nitrate should be equipped with general exchange supply and exhaust and (or) emergency ventilation, in order to exclude the formation of a fire and explosion hazardous environment in the room during the decomposition of ammonium nitrate. Warehouses for storing ammonium nitrate in an amount of not more than 5 thousand tons may be separated from other premises, including from the warehouses for fertilizers and pesticides, by solid (without openings) type 2 fire walls.

Abstract:
This research investigates adverse events and patient safety in healthcare due to poor supply chain management practices, and inadequate and disorganized product validation procedures. Focusing on commodity medical and surgical products, this research investigates correct product validation points for maximum patient safety. This study also explores benefits of standard product identifying technologies such as HIBC or GS1 data standards as well as automated validation systems such as barcode or Auto ID to minimize workflow interruptions. Site visits and phone interviews are conducted with six healthcare providers to document common product validation practices and procedures. Based on observations and collected data, a simulation model is developed. Different scenarios are compared for patient safety, care delay, and system efficiency. The results show that validation points during PAR picking or bedside product administration, and warehouse picking operations provide optimal overall system performance. The results also indicate that standard product identifying technologies and automated validation systems significantly impact the efficiency of supply chain.

Abstract:
Four large-scale centrifuge tests were performed at the NEES@UCDavis equipment site to study the cyclic behavior of levee structures resting atop soft organic peat. The model configurations using a non-liquefiable levee focused on the seismic deformation potential of peat during primary consolidation and secondary compression. The tests performed with a sandy levee studied the liquefaction potential of saturated loose sand fill overlying soft peat as well as the levee-peat-interaction under different loading conditions. The models were subjected to scaled ground motions representative of the Sacramento/San Joaquin Delta. System instrumentation consisted of linear potentiometers, pore pressure sensors and accelerometers. Slow data recorded at 1 Hz document the settlements during spin up, application of ground motions, and spin down. Fast data sampled at 4,167 Hz measured the dynamic response of the system, the excess pore pressure increase and immediate settlements. Abstract:
Data Mining is extraction of relevant information about data set. A data-warehouse is a location where information is stored. There are various services of data mining, clustering is one of them. Clustering is an effort to group similar data onto single cluster. In this paper we propose and implement k-mean and neural network for clustering same components in single cluster. Clustering reduces the search space by grouping similar test cases together according to the requirements and, hence minimizing the search time, for the retrieval of the test cases, resulting in reduced time complexity. In this research paper we proposed approach for re-usability of test cases by unsupervised approach and supervised approach. In unsupervised learning we proposed k-mean and in supervised learning neural network. We have designed the algorithm for requirement and test case document clustering according to its tf-idf vector space and the output is set of highly cohesive pattern groups.

AbstractObjectivesTo create test collections for evaluating clinical information retrieval (IR) systems and advancing clinical IR research.Materials and MethodsElectronic health record (EHR) data, including structured and free-text data, from 45 000 patients who are a part of the Mayo Clinic Biobank cohort was retrieved from the clinical data warehouse. The clinical IR system indexed a total of 42 million free-text EHR documents. The search queries consisted of 56 topics developed through a collaboration between Mayo Clinic and Oregon Health & Science University. We described the creation of test collections, including a to-be-evaluated document pool using five retrieval models, and human assessment guidelines. We analyzed the relevance judgment results in terms of human agreement and time spent, and results of three levels of relevance, and reported performance of five retrieval models.ResultsThe two judges had a moderate overall agreement with a Kappa value of 0.49, spent a consistent amount of time judging the relevance, and were able to identify easy and difficult topics. The conventional retrieval model performed best on most topics while a concept-based retrieval model had better performance on the topics requiring conceptual level retrieval.DiscussionIR can provide an alternate approach to leveraging clinical narratives for patient information discovery as it is less dependent on semantics. Our study showed the feasibility of test collections along with a few challenges.ConclusionThe conventional test collections for evaluating the IR system show potential for successfully evaluating clinical IR systems with a few challenges to be investigated.

Abstract:
The task repurposing of heterogeneous, distributed data for originally unintended research objectives is a non-trivial problem because the mappings required may not be precise. A particular case is clinical data collected for patient care being used for medical research. The fact that research repositories will record data differently means that assumptions must be made as how to transform of this data. Records of provenance that document how this process has taken place will enable users of the data warehouse to utilise the data appropriately and ensure that future data added from another source is transformed using comparable assumptions. For a provenance-based approach to be reusable and supportable with software tools, the provenance records must use a well-defined model of the transformation process. In this paper, we propose such a model, including a classification of the individual ‘sub-functions’ that make up the overall transformation. This model enables meaningful provenance data to be generated automatically. A case study is used to illustrate this approach and an initial classification of transformations that alter the information is created.

Abstract:
Abstract. Bonded warehouses are the most important infrastructure link in the distribution systems of the leading countries in international trade. The largest transnational companies of such States, having a piece number of bond structures, provide huge supplies of their products to the domestic and international markets. The purpose of the study was to justify the need to develop bonded warehouses and bonded zones in Russia in order to create infrastructure conditions for the development of production and trade in agricultural products. Research methods included observations, groupings, comparisons, and statistical evaluations. According to the results of the study, it was determined that bond structures in Russia are regulated by legal documents applicable to temporary storage warehouses, adopted within the framework of the common economic space of the member States of the Eurasian economic Union. The procedure for temporary storage in bonded warehouses allows avoiding tax and other duties, obtaining licenses, permits, etc., entering products into the customs regime and filing a Declaration during the storage period. In addition, bonded warehouses provide users of their services with economic benefits due to lower prices for services, comprehensive maintenance, and faster product movement. The analysis showed that in Russia, bonded warehouses and zones have a low level of development: their number and capacity are insufficient, and the services offered are limited to transport, warehouse, and customs. At the same time, warehouse activity in Russia is highly profitable: the profitability of assets is 2.3 times higher than the indicator for the economy as a whole. For further development of bonded warehouses in Russia, it is recommended to increase their capacity in the regions that are key in the production and trade of agricultural products, expand the services provided, and integrate with agricultural producers. The scientific novelty of the work consists in substantiating the theoretical foundations of the role and necessary characteristics of bonded warehouses as an infrastructure basis for the development of trade in agricultural goods in Russia.

Abstract:
e23120 Background: Cancer patients experience symptoms which may precede diagnosis, occur during therapy, and persist into survivorship. Symptom assessments and patient reported outcomes have been shown to improve quality of life and extend survival in cancer patients. The Veterans Administration Symptom Assessment Scale (VSAS) is a clinical tool to document cancer-related symptoms. Here, we report VSAS adoption in five Veterans Health Administration (VHA) hematology-oncology sites during a five-year period. Methods: Data was collected from the Corporate Data Warehouse (CDW): VSAS data, date of birth, gender, race, ethnicity, vital status, year of cancer diagnosis, ICD-O-3, VHA facility, and clinic stop codes. Descriptive analyses of patient demographics, disease characteristics, and VSAS utilization was performed. VSAS is comprised of 13 symptoms, rated on a 0 to 10 scale. A VSAS visit was defined as the date the VSAS was administered to a patient. Results: From January 2013 through June 2018, there were 41,917 instances of VSAS administered to 9,409 unique patients at five VHA facilities: 8,512 (90.5%) were male; 5,241 (56%) were non-Hispanic White, 3,743 (40%) were Black, and 297 (3%) were other racial categories or not available. The median age at the first VSAS visit was 67 years. In patients whose oncologic diagnosis was recorded by a cancer registrar (4,536), the most common diagnoses were prostate cancer, lung cancer (all histologies), colon cancer, and multiple myeloma. The number of VSAS administrations increased annually, from 866 in 2013 to 12,775 in 2017, which may be attributed to increased adoption of VSAS across and within VHA facilities. Individual patients completed the VSAS 1-57 times (median 2). At the five VHA facilities utilizing VSAS in 2017, VSAS was administered at 56% of hematology-oncology clinic visits. Conclusions: VSAS is a tool to measure and document patient symptom burden, and has been successfully adopted in the last five years at select VHA facilities. Expanded use of VSAS throughout the VHA is an important step in improving the quality of life and extending survival of veterans with cancer and blood disorders.

Abstract:
The Law of Ukraine "On Grain and the Grain Market in Ukraine" of 4 July 2002 defines grain as the fruits of cereals, legumes and oilseeds used for food, seed, fodder and technical purposes. Peculiarities of grain as a valuable agricultural raw material determine the peculiarities of its storage, which affects the specifics of legal regulation in this area. The article clarifies the characteristics of a grain storage contract, substantiates the list of its essential conditions, examines the legal nature and features of grain storage documents, identifies shortcomings in the legal regulation and execution of grain storage contracts, and substantiates proposals to eliminate them. It is established that the legal nature of the contract of warehouse storage of grain is manifested in the following: it is a contract for the provision of services; it is a public contract; it is a written contract; it is a bilateral contract; as a rule, it is a retaliatory contract; it is a fixed-term contract; it is a real contract; concluded in a special order; it is a contract of accession. The article proves that the essential terms of the grain storage contract are: the subject matter of the contract (grain storage services with preservation of its quality and prevention of losses above natural); the object of the contract (grain of a certain type, class, quantity, which is defined in the contract of grain storage); the term of the contract (with an indication of a certain period of time or without specifying the one that determines the validity of the contract to the first claim of the pledgor). In order to simplify the circulation of grain storage documents and reduce the level of abuse in the grain market, it is proposed to amend Article 961 of the Civil Code of Ukraine, the Law of Ukraine "On Grain and Grain Market in Ukraine" and the Law of Ukraine "On Certified Warehouses and Simple and Double Warehouse Certificates" in order to provide the legal possibility of issuing warehouse certificates in electronic form. Keywords: a double warehouse certificate; grain; grain storage; a grain storage contract; grain storage documents; a simple warehouse certificate; a warehouse receipt.

Abstract:
Leading institutions throughout the country have established Precision Medicine programs to support personalized treatment of patients. A cornerstone for these programs is the establishment of enterprise-wide Clinical Data Warehouses. Working shoulder-to-shoulder, a team of physicians, systems biologists, engineers, and scientists at Rutgers Cancer Institute of New Jersey have designed, developed, and implemented the Warehouse with information originating from data sources, including Electronic Medical Records, Clinical Trial Management Systems, Tumor Registries, Biospecimen Repositories, Radiology and Pathology archives, and Next Generation Sequencing services. Innovative solutions were implemented to detect and extract unstructured clinical information that was embedded in paper/text documents, including synoptic pathology reports. Supporting important precision medicine use cases, the growing Warehouse enables physicians to systematically mine and review the molecular, genomic, image-based, and correlated clinical information of patient tumors individually or as part of large cohorts to identify changes and patterns that may influence treatment decisions and potential outcomes.

Abstract:
Background Managing prescription renewal requests is a labor-intensive challenge in ambulatory care. In 2009, Vanderbilt University Medical Center developed clinic-specific standing prescription renewal orders that allowed nurses, under specific conditions, to authorize renewal requests. Formulary and authorization changes made maintaining these documents very challenging. Objective This article aims to review, standardize, and restructure legacy standing prescription renewal orders into a modular, scalable, and easier to manage format for conversion and use in a new electronic health record (EHR). Methods We created an enterprise-wide renewal domain model using modular subgroups within the main institutional standing renewal order policy by extracting metadata, medication group names, medication ingredient names, and renewal criteria from approved legacy standing renewal orders. Instance-based matching compared medication groups in a pairwise manner to calculate a similarity score between medication groups. We grouped and standardized medication groups with high similarity by mapping them to medication classes from a medication terminology vendor and filtering them by intended route (e.g., oral, subcutaneous, inhalation). After standardizing the renewal criteria to a short list of reusable criteria, the Pharmacy and Therapeutics (P&T) committee reviewed and approved candidate medication groups and corresponding renewal criteria. Results Seventy-eight legacy standing prescription renewal orders covered 135 clinics (some applied to multiple clinics). Several standing orders were perfectly congruent, listing identical medications for renewal. We consolidated 870 distinct medication classes to 164 subgroups and assigned renewal criteria. We consolidated 379 distinct legacy renewal criteria to 21 criteria. After approval by the P&T committee, we built subgroups in a structured and consistent format in the new EHR, where they facilitated chart review and standing order adherence by nurses. Additionally, clinicians could search an autogenerated document of the standing order content from the EHR data warehouse. Conclusion We describe a methodology for standardizing and scaling standing prescription renewal orders at an enterprise level while transitioning to a new EHR.

Abstract:
Data warehousing systems enable enterprise managers to acquire and integrate information from heterogeneous sources and to query very large databases efficiently. Building a data warehouse requires adopting design and implementation techniques completely different from those underlying operational information systems. Though most scientific literature on the design of data warehouses concerns their logical and physical models, an accurate conceptual design is the necessary foundations for building a DW which is well-documented and fully satisfies requirements. In this paper we formalize a graphical conceptual model for data warehouses, called Dimensional Fact model, and propose a semi-automated methodology to build it from the pre-existing (conceptual or logical) schemes describing the enterprise relational database. The representation of reality built using our conceptual model consists of a set of fact schemes whose basic elements are facts, measures, attributes, dimensions and hierarchies; other features which may be represented on fact schemes are the additivity of fact attributes along dimensions, the optionality of dimension attributes and the existence of non-dimension attributes. Compatible fact schemes may be overlapped in order to relate and compare data for drill-across queries. Fact schemes should be integrated with information of the conjectured workload, to be used as the input of logical and physical design phases; to this end, we propose a simple language to denote data warehouse queries in terms of sets of fact instances.

Abstract:
SummaryObjective: Document information needs, gaps within the current electronic applications and reports, and workflow interruptions requiring manual information searches that decreased the ability of our antimicrobial stewardship program (ASP) at Intermountain Healthcare (IH) to prospectively audit and provide feedback to clinicians to improve antimicrobial use. Methods: A framework was used to provide access to patient information contained in the electronic medical record, the enterprise-wide data warehouse, the data-driven alert file and the enterprise-wide encounter file to generate alerts and reports via pagers, emails and through the Centers for Diseases and Control’s National Healthcare Surveillance Network. Results: Four new applications were developed and used by ASPs at Intermountain Medical Center (IMC) and Primary Children’s Hospital (PCH) based on the design and input from the pharmacists and infectious diseases physicians and the new Center for Diseases Control and Prevention/ National Healthcare Safety Network (NHSN) antibiotic utilization specifications. Data from IMC and PCH now show a general decrease in the use of drugs initially targeted by the ASP at both facilities. Conclusions: To be effective, ASPs need an enormous amount of “timely” information. Members of the ASP at IH report these new applications help them improve antibiotic use by allowing efficient, timely review and effective prioritization of patients receiving antimicrobials in order to optimize patient care.Citation: Evans RS, Olson JA, Stenehjem E, Buckel WR, Thorell EA, Howe S, Wu X, Jones PS, Lloyd JF.
