Land use policy is at the center of suburban political economies because everything has to happen somewhere but nothing happens by itself. This book explores how well a century of strategic land-use decisions served the public interest in Montgomery County, Maryland, a suburb of Washington, D.C. Transformed from a rural hinterland into the home of a million people and a half-million jobs, Montgomery County built a national reputation for innovation in land use policy—including inclusive zoning, linking zoning to master plans, preservation of farmland and open space, growth management, and transit-oriented development. A pervasive theme of the book involves the struggle for influence over land use policy between two virtual suburban republics. Developers, their business allies, and sympathetic officials sought a virtuous cycle of market-guided growth in which land was a commodity and residents were customers who voted with their feet. Homeowners, environmentalists, and their allies saw themselves as citizens and stakeholders with moral claims on the way development occurred and made their wishes known at the ballot box. This book evaluates how well the development pattern produced by decades of planning decisions served the public interest.

Under the New Jersey State Constitution as interpreted by the State Supreme Court in 1975 and 1983, municipalities are required to use their zoning authority to create realistic opportunities for a fair share of affordable housing for low- and moderate-income households. Mount Laurel was the town at the center of the court decisions. As a result, Mount Laurel has become synonymous with the debate over affordable housing policy designed to create economically integrated communities. What was the impact of the Mount Laurel decision on those most affected by it? What does the case tell us about economic inequality? This book undertakes a systematic evaluation of the Ethel Lawrence Homes—a housing development produced as a result of the Mount Laurel decision. The book assesses the consequences for the surrounding neighborhoods and their inhabitants, the township of Mount Laurel, and the residents of the Ethel Lawrence Homes. Their analysis reveals what social scientists call neighborhood effects—the notion that neighborhoods can shape the life trajectories of their inhabitants. The book proves that the building of affordable housing projects is an efficacious, cost-effective approach to integration and improving the lives of the poor, with reasonable cost and no drawbacks for the community at large.

The proportion of children who are deaf or hard of hearing and also have other medical, neurodevelopmental, behavioral, or psychosocial conditions is increasing. Prevalence estimates run as high as 50% to 70%. The shifting complexity challenges not only the learners but also the teachers, administrators, and policymakers responsible for the education of deaf students. Documentation of diagnostic profiles contributes to understanding the learning profile of deaf students with concomitant conditions. This may also inform policy decisions, programmatic design, calibration of parental expectations, and implementation of effective teaching strategies. Through illustrative case examples, this chapter explores the principles of differential diagnosis and the implications of various conditions. Topics covered include genetic syndromes, vestibular dysfunction, intrauterine viral infection with associated congenital hearing loss, autism spectrum disorder, reactive attachment disorder, complex medical histories resulting in severe neurologic compromise, intellectual disabilities, cerebral palsy, and nonsyndromic genetic conditions.

Abstract:
Psychopharmacology in general is a challenging field that includes much art as well as science. Clinicians usually depend upon self-report in making decisions regarding medication selection and dosing. When a patient becomes incarcerated, there are multiple potentially conflicting, or synergistic, situations. There are issues of different formularies, different environmental stressors, changed support groups, and practice patterns that all may contribute differentially to medication management decisions. Current community medications may have been determined while ongoing illicit drug use confounded the diagnostic picture. Collaboration between clinician and patient may have been poor, and subsequently treatment adherence may in turn have been marginal. Many similar issues apply when a patient transfers from a jail to a prison or from one prison to another. Preparation and review of transfer summary sheets and more detailed records are just as important in these situations and should be seen as the minimum standard in policy and in practice. Ideally, continuity of care, and any concerns about diagnosis or treatment are best shared through direct communication. A telephone exchange between treating psychiatrists is always better than simple written documentation. This chapter discusses both the issues and pragmatic management opportunities that can lead to improved patient care and enhanced functioning.

Echocardiography can provide rapid and accurate assessment of cardiac morphology and haemodynamics under stressful conditions in the emergency room (ER). Using this information, critical decisions regarding management of cardiovascular emergencies and the critically ill are made. To avoid potentially catastrophic errors with medicolegal consequences, adequate education and experience in echocardiography and cardiology are required and teamwork is encouraged. In addition, emergency cases must be well documented and this documentation stored and retrievable. Transthoracic echocardiography is the main source of the information in the emergency setting, while transoesophageal, contrast, and stress echocardiography are used when needed and in special circumstances.In this chapter, the principles, practice, and specific considerations related to echocardiography in the ER are discussed and a brief overview of echocardiographic assessment in cardiac emergencies is provided. Detailed information regarding echocardiographic features of particular cardiovascular diseases and conditions that may be presented to the emergency physician in the ER can be found elsewhere in this book in the related chapters.

This chapter discusses why dialysis withdrawal is so common in the United States, when dialysis withdrawal and withholding is appropriate, and the role of palliative care in the renal patient. It discusses guidelines that were created to aid in dialysis withdrawal, dialysis initiation, and prognostic calculation. Factors contributing to the phenomenon of dialysis discontinuation include variability in the criteria for initiation of renal replacement therapy, increasing numbers of incident and prevalent geriatric patients, new emphasis on autonomy and quality of life, the development of improved prognostic instruments for prediction of survival, growing acceptance of medical decisions that accelerate dying, and a clearer appreciation of the quality of dying that ensues following the cessation of dialysis. There continues to be a need for further integration of palliative medicine in the clinical management of patients with chronic kidney disease, especially since hospice services are often unavailable unless a decision is made to stop dialysis treatment. The importance of communication between staff and patients and the documentation of the plan of care is critical.

This chapter looks at the purpose and history of the development of good clinical practice (GCP). The international conference on harmonisation (ICH) GCP is the international quality standard for conducting clinical research to ensure the rights and well-being of patients are protected and the resulting data are valid. The cornerstone of ethics in research stems from the Declaration of Helsinki and the chapter looks at the changes in the Declaration and the impact on clinical trials. The development of the ICH process is described and the E, S, Q and M guidelines are discussed, The efficacy guidelines affect the practical aspects of trials and the efficacy guideline number 6 (E6) is on GCP. The content of the E6 guidelines is reviewed including the responsibilities of ethics committees, investigator and sponsor. Documentation requirements including the Protocol and Investigator Brochure as well as all the other documents are outlined. The guidelines are written to be interpreted and companies and institutions have to document their interpretation using standard operating procedures (SOPs). Although ICH GCP is regarded as the world-wide standard it sits alongside countries' legislation. In Europe CTIMPs have to follow the EU Directives and Regulation. Non pharmaceutical/non interventional healthcare research has no legal requirements to adhere to ICH GCP and is carried out under different research governance frameworks (RGF), however they all have their principles based on ICH GCP. The chapter also discusses the definition of an IMP and the decisions and processes that have to be followed when conducting non CTIMP studies.
