The use of trained dogs for the detection of volatile biomarkers in biological samples has great potential to be used for non-invasive diagnosis and monitoring of several diseases such as cancer. It offers early, highly accurate detection with fast response times, non-invasive to patients and allows for repeated sampling. The aforementioned methods are useful as a portable technology to increase detection, screening, and monitoring coverage in populations at risk. In this sense, Cervical Cancer (CC) has become a public health concern of alarming proportions in many developing countries, particularly in low-income sectors and marginalized regions due to different factors that limit the coverage of screening methods and the acceptance rates of women attending their routine gynecological examination. As such, early detection is a crucial medical factor in improving not only their population’s quality of life but also its life expectancy. For the above, the great odor detection threshold exhibited by dogs is not unheard of and represents a potential opportunity to develop an affordable, accessible, and non-invasive method for detection of CC with high sensibility and specificity values.

In almost no other surgical specialty does use of a diagnostic screen have such prominence than when considering the diagnosis of a lump in the head or neck. Is the lump neoplastic and if so, is it likely to be of benign, malignant, or of mixed malignant potential? Does the mass have features of a developmental condition or could it represent sequelae of acute or chronic infection? Are there associated signs suggesting endocrine or metabolic dysfunction in association with the mass? Is there a traumatic origin or are there features of degenerative disease? Enlarged lymph nodes may be reactive due to inflammation, acute or chronic infection, or may represent primary or secondary neoplastic dis­ease. Anatomical knowledge of lymph drainage patterns provides clues as to the region of primary pathology. Developmental abnormalities such as thyroglossal cyst, dermoid inclu­sion cyst, and brachial fistula exhibit specific clinical signs and are found in characteristic anatomical locations. Systemic clinical signs may occur in endocrine abnormalities asso­ciated with hyper- or hypothyroidism and, for instance, in primary hyperparathyroidism. Bimanual examination will assist localization of sialolithiasis and the alert examiner will search for clinical involvement of the facial nerve which may help to suggest the nature of the neoplastic pathology involv­ing a pre-auricular mass. CT scan and fibre-endoscopy are powerful diagnostic tools in inves­tigation of the oro- and nasopharynx and of the pathologies that may lie therein. Head and neck pathologies may be treated by ENT, head &amp; neck, or general surgical specialists. Irrespective of which specialty eventually manages the patient, it behoves the medical student and surgical trainee to ensure that he or she is conversant with the common clinical mani­festations and anatomical features of conditions that occur in this region. This chapter will help you to revise distinct features of some of the most common clinical pathologies affecting the head and neck, and should remind you of rationales for specific diagnostic and therapeutic avenues.

Pharyngitis is common with incidence peaking from autumn to spring. Respiratory viruses are most commonly implicated, and are generally self-limiting conditions not requiring diagnostic workup or treatment. Bacterial pharyngitis is less common, is spread by droplets or direct transmission, and Streptococcus pyogenes (Group A strep, or GAS) is the most frequent cause. Haemophilus influenzae, Mycoplasma pneumoniae, and Neisseria gonorrhoeae are less frequent causes. Rapid antigen detection tests make the point-of-care assessment of GAS pharyngitis possible, although a negative test does not exclude infection. No method can distinguish oropharyngeal colonization from actual infection, but culture can obtain antibiotic susceptibility testing. Suspicion of infection with Neisseria gonorrhoeae, Bordetella pertussis, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydophila pneumoniae, or Corynebacterium diptheriae should be communicated to the laboratory so that the appropriate culture media is utilized. The Centor criteria provide a clinical predictive score that can give the likelihood a sore throat is due to a bacterial infection with the following: the presence of tonsillar exudate, tender anterior cervical adenopathy, fever over 38°C, and absence of cough. If three or four of these criteria are met, the positive predictive value is 40% to 60%. The absence of three or four of the Centor criteria has a relatively high negative predictive value of 80%, and may be use to evaluate whether antibiotics can be withheld or deferred. Oral penicillin or macrolide are used to treat streptococcal pharyngitis. Treatment may reduce severity, duration, transmission, and risk of post-infectious sequelae like rheumatic heart disease and post-streptococcal glomerulonephritis. Other complications include scarlet fever, streptococcal toxic shock syndrome, and quinsy. Otitis media, is frequent in the young children, possibly due to a short and horizontal Eustachian tube. Purulent material buils up leading to a bulging, red tympanic membrane which may rupture and discharge. Intense local pain and fevers may occur. Streptococcus pneumoniae, Moraxella catarrhalis, and Haemophilus influenzae are frequently implicated. Frequently there are no sequelae, although complications include hearing impairment, and less common are mastoiditis, bacteraemia, and meningitis. Diagnosis is clinical based on presentation and otoscopic examination. Microbiological diagnosis is possible through culture of exuate on swab or following tympanocentesis.

This chapter introduces the basic principles of Clinical Decision Support (CDS) systems. CDS systems aim to codify and strategically manage biomedical knowledge to handle challenges in clinical practice using mathematical modelling tools, medical data processing techniques and Artificial Intelligence (AI) methods. CDS systems cover a wide range of applications, from diagnosis support to modelling the possibility of occurrence of various diseases or the efficiency of alternative therapeutic schemes, using not only individual patient data but also data on risk factors and efficiency of available therapeutic schemes stored in databases. Computer-Aided Diagnosis (CAD) systems can enhance the diagnostic capabilities of physicians and reduce the time required for accurate diagnosis. Modern Therapeutic Decision Support (TDS) systemsmake use of advanced modelling techniques and available patient data to optimise and individualise patient treatment. CDS systems aim to improve the overall health of the population by improving the quality of healthcare services, as well as by controlling the cost-effectiveness of medical examinations and treatment.

Abstract:
Medical diagnosis with the help of computational techniques is a very useful tool for doctors for the purpose of better diagnosis. In this chapter, an attempt has been made to reduce the number of tests required for diagnosis using generalized fuzzy sets for initial decision making as per the characteristics of ingested water in normal routine. The quality of water in terms of type and concentration of contaminants varies from region to region. The analogy between concentration of different contaminants in drinking water and risk analysis based on intuitionistic fuzzy sets (IFSs) have been investigated. Hypothetical data was processed in view of the finite set of heavy metals, diseases, and places to investigate the effect of selected heavy metal on the human health. The findings will not only help in diagnosis but also offer cost-cutting and ecofriendly strategy by avoiding unnecessary clinical laboratory examinations.

Abstract:
Pain has been defined as ‘‘an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage’’ (Lindblom et al., 1986). Table 5–1 contains definitions of terms commonly used to describe pain sensations (Merskey et al., 1986). Pain is the most common reason a patient presents to a physician for evaluation. The U.S. Center for Health Statistics found that 32.8% of the general population suffers from chronic pain symptoms (Magni et al., 1993). Many factors can influence patients’ reports of pain, including medical and psychiatric disorders, social circumstances, disease states, personality traits, memory of past pain experiences, and personal interpretations of the meaning of pain (Clark and Treisman, 2004). There is no simple algorithm for determining whether the cause of pain is psychologic or neurologic (Clark and Chodynicki, 2005). The clinical evaluation of patients complaining of pain should be comprehensive and incorporate the patient’s descriptions of pain (ie, location, intensity, duration, precipitants, ameliorators); observations of pain-related behaviors (eg, limping, guarding, moaning); descriptions of problems performing activities; and neurologic and psychiatric examinations (Clark and Cox, 2002). Post-herpetic neuralgia (PHN) is defined as pain persisting or recurring at the site of shingles at least 3 months after the onset of the acute varicella zoster viral rash. PHN occurs in about 10% of patients with acute herpes zoster. More than 50% of patients older than 65 years of age with shingles develop PHN, and it is more likely to occur in patients with cancer, diabetes mellitus, and immunosuppression. During the acute episode of shingles, characteristics such as more severe pain and rash, presence of sensory impairment, and higher levels of emotional distress are associated with developing PHN (Schmader, 2002). Most cases gradually improve, with only about 25% of patients with PHN experiencing pain 1 year after diagnosis. Approximately 15% of patients referred to pain clinics suffer from PHN. Early treatment of varicella zoster with low-dose amitriptyline (25–100mg QD) can reduce the prevalence of pain at 6 months by 50% (Bowsher, 1997).

Abstract:
Dentistry science is a new specialty in veterinary medicine that has been growing in recent years, accompanied by the development of professionals who seek to improve the quality of life of pets. Cases related to problems in the oral cavity have gained significant importance in the medical clinic of professionals who treat small animals. Due to lack of professional knowledge or due to animal behavioral problems, such as aggressiveness, the anamnesis of the oral cavity is not performed most of the time, which ends up delaying the diagnosis of the pathology. In addition, an animal with a problem in the oral cavity may take years to show signs of the disease. In general, animals have an oral microbiota composed of various species of fungi, which, under specific conditions, can change from saprophytes to pathogens, compromising their health. Thus, the pre-knowledge of potentially pathogenic yeasts belonging to oral microbiota of dogs and their susceptibility profile compared to the main drugs used in antifungal therapy, is of fundamental importance as it ensures a clinical auxiliary support for the diagnosis and treatment of most diseases of the oral cavity.
