Abstract:
Adverse effects during clozapine treatment are common, and can be divided into very common (>10%: constipation, weight gain, metabolic side effects, sedation, and sialorrhea), common (1–10%: seizures and enuresis), and cardiac (sinus tachycardia, electrocardiogram abnormalities, and orthostatic hypotension) adverse effects. Most adverse effects are benign, but often reduce the quality of life for the patient, leading to reduced adherence and thereby psychotic relapse. As a consequence, treatment of these adverse effects is important and should not be neglected. In this chapter, we present s

The antidepressant and neuroprotective effects of oestradiol are described. Psychiatric consequences of oophorectomy, and treatment with tamoxifen and aromatase inhibitors, are then discussed. Androgen-deprivation therapy has temporary effects on cognitive function and mood that reflect the distribution of androgen receptors in the brain. The rapid-onset adverse psychiatric effects of high-dose glucocorticoids are presented (including ‘steroid psychosis’) and a novel, non-genomic molecular mechanism highlighted. In contrast, the depressive effect of chronic glucocorticoid use is then considere

In a postmodern age where the media's depictions of reality serve as stand-ins for the real thing for so many Americans, how much government policy is being made on the basis of those mediated realities and on the public reaction to them? When those mediated depictions deviate from the truth of the actual situation, how serious a situation is that? Time and again, both anecdotal evidence and scientific research seem to confirm that the news media often influence government action. At the least, they speed up policy making that would otherwise take a slower, more reasoned course. Sometimes the

The case illustrates how to approach imaging in pregnancy. Some basic principles of radiation exposure are reviewed. The effects of radiation are divided into either deterministic, dose-related, or stochastic, where exposure determines the probability of an outcome. Deterministic effects have a threshold below which the effect does not occur. The threshold may be small and may vary from person to person; whereas stochastic effects occur by chance and may occur without a threshold level. Cancer caused by radiation is an example of a stochastic effect. The overall safety of magnetic resonance im

This chapter picks up the story where the previous chapter left off, showing how the Supreme Court of Canada initially made a show of rejecting the application of the Charter of Rights and Freedoms to private law relationships in Dolphin Delivery. But at the same time, the Court left itself room to interpret the common law to comport with Charter values. In the years since, litigants have pressed the Court to use this power to meet their normative demands, and the Court has done so on many occasions. At the same time, the Court has showed solicitude for legislative initiatives, often using the

Biological effects of radiation have been interpreted based on the assumption that DNA is the primary target, but recent research has shown that non-targeted mechanisms may affect cells that are not directly exposed. The most important effect in humans from low doses of radiation is the induction of cancer, but risks of other effects such as cataract and cardiac or circulatory disease are becoming apparent. Epidemiological studies of Japanese survivors of atomic bombs demonstrate a clear linear relationship between solid cancer incidence and organ dose. This is supported by other epidemiologic

This chapter reports on an open, uncontrolled study to assess the effects of subcutaneous morphine on dyspnea in patients with terminal cancer. Twenty patients with dyspnea from restrictive respiratory failure received a subcutaneous dose of morphine relative to their opioid tolerance: 5mg for opioid naïve (5 patients) and 2.5 times regular dose for opioid tolerant (15 patients). Dyspnea and pain scores were measured every 15 minutes for 150 minutes. Dyspnea scores, but not respiratory rate, respiratory effort, nor arterial saturation of oxygen were affected.
