This chapter argues that the distinctive qualities of John Donne’s religious thought and temperament are revealed as much through the manner or expressive mode of his religious writing as they are through its matter or doctrinal content. To illustrate, the chapter analyses the rhetoric and prosody of Holy Sonnet 19 (‘Oh, to vex me’) in the light of two key contexts: Donne’s letters, poems, and prose works from his middle years (1606–14), and the religious and theological controversies of the same period, including fiercely argued doctrinal debates about the means of salvation and bitter religio-political disputes over the Oath of Allegiance. The chapter concludes by showing the degree to which Donne’s compelling union of dialectical reason and associative poesis in his religious writing both shares in and departs from literary traits and mentalities found in other religious writers of the period.

Cowboy’s story reminds us that the care of the dying is best done in community. His increasing vulnerability is an uncomfortable reminder that we, too, may someday be dependent upon others to meet our needs. Despite our attempts to distance ourselves from the dying, we each may all too soon find ourselves alienated and marginalized in the very institutional “system” that at present so poorly cares for others. Cowboy is a reminder that we, too, may one day have to rely upon others for compassionate and competent care that is reliably delivered. Cowboy’s story calls us to action, for in designing a system that will reliably meet his needs, we are also taking steps to better ensure that our own needs will be met when our time comes.

Manuscript Matters illuminates responses to some of John Donne’s most elusive texts by his contemporary audiences. Since examples of seventeenth-century literary criticism prove somewhat rare and frequently ambiguous, this book emphasizes a critical framework rarely used for exhibiting early readers’ exegeses of literary texts: the complete manuscripts containing them. Many literary manuscripts that include poems by Donne and his contemporaries were compiled during their lifetimes, often by members of their circles. For this reason, and because various early modern poems and prose works satirize topical events and prominent figures in highly coded language, attempting to understand early literary interpretations proves challenging but highly valuable. Compilers, scribes, owners, and other readers—men and women who shared in Donne’s political, religious, and social contexts—offer clues to their literary responses within a range of features related to the construction and subsequent use of the manuscripts. This study’s findings call us to investigate more extensively and systematically how certain early manuscripts were constructed through analysis of such features as scripts, titles, sequence of contents, ascriptions, and variant diction. While such studies can throw light on many early modern texts, exploring artifacts containing Donne’s works proves particularly useful because more of his poetry circulated in manuscript than did that of any other early modern poet. Manuscript Matters engages Donne’s satiric, lyric, and religious poetry, as well as his prose paradoxes and problems—refocusing modern interpretation through an early modern lens.

Plantaciones de Eucalyptus cladocalyx y E. camaldulensis, establecidas en la región de Coquimbo, fueron caracterizadas de acuerdo a los distritos edafoclimáticos de las localidades donde fueron establecidas. Las plantaciones de Eucalyptus camaldulensis se han desarrollado en áreas con precipitaciones medias anuales entre 113,0 y 243,7 mm, con una temperatura mínima media del mes más frío (Julio) entre 3,7 y 8,4ºC, y una temperatura máxima media del mes más cálido (Enero) entre 19,9 y 30,4ºC. Eucalyptus cladocalyx se ubica en sectores con precipitaciones sobre los 200 mm (entre 201,1 y 243,7), con una temperatura mínima media del mes más frío entre 4,2 y 7,0ºC, y una temperatura máxima media del mes más cálido entre 24,1 y 29,0ºC. Ambas especies extienden su rango de tolerancia al frío y al estrés hídrico en relación con su distribución natural. La superficie potencial estimada para la forestación con estas especies fue de 1.023.347 ha y 695.892 ha para Eucalyptus camaldulensis y E. cladocalyx, respectivamente.

Physicians have examined urine for over 6000 years. Urine microscopy was first employed to diagnose kidney disease in the seventeenth century and remains an indispensable tool. The value of urinalysis for diagnosis and management of renal and genitourinary disease is well accepted. Urinalysis aids in the diagnosis of renal disease especially in cases when a renal biopsy is not immediately available or is contraindicated. It is most informative when done by the treating physician with knowledge of the clinical context. Inspection is done by eye. Routine chemical analysis is done by dipstick but urine microscopy is essential for it may reveal abnormalities even when chemical evaluation is normal. Dysmorphic red cells, red cell casts, white blood cells, renal cells, and specific crystals may be diagnostically important. Urinalysis and microscopy can narrow the differential diagnosis faster than many more complex tests are able to.

This chapter introduces the reader to some of the common investigations used in the diagnosis of endocrine disorders. Tests used to diagnose disorders of the thyroid, pituitary, and adrenal glands are discussed. Measurement of plasma levels of thyroid-stimulating hormone, free thyroxine, and free triiodothyronine define the thyroid status. Imaging of the thyroid is usually done in the first instance by ultrasonography. The investigation of pituitary disorders involves measurement of the levels of the multiple pituitary hormones, with basal and dynamic tests as appropriate to the clinical problem. Imaging of the pituitary is typically done using magnetic resonance with gadolinium enhancement. Adrenal disorders (which include hyperaldosteronism, Cushing’s syndrome, and adrenal insufficiency) can be investigated with a range of basal and dynamic tests.

Psychological testing is of substantial value in a range of correctional situations and can be a helpful adjunct to standard clinical assessments. Tests provide additional sources of data for use in comprehensive assessments, but they do not substitute for clinical evaluations. This chapter discusses the rationales and purposes for using psychological testing, special issues in administering and interpreting these tests in correctional settings, and caveats about their proper use. Well-validated psychological tests can be an important adjunct to a comprehensive mental health assessment, and help to identify psychiatric symptoms and cognitive deficits that may impair functioning in correctional institutions. In addition, testing can assist in determining the validity of self-reported symptoms by inmates. Given the range of tests, with varying suitability to a correctional population, a qualified doctoral psychologist should make the decision about which tests to administer. The referral should identify the issues and questions to address, instead of requesting specific tests, to allow the psychologist to choose the best instruments. The psychologist should produce a report that explains the results, their applicability to the referral issues, and any caveats about their validity. The results can then be integrated into the diagnostic assessment and treatment plan for the inmate. This chapter will present some of the history and contexts for when it is done, when it can be done and when it should not be done on the basis of best practice and evidence based practice.

Joseph Toynbee, an English physician, believed that the only way to develop a rational strategy for the treatment of ear diseases was to dissect the temporal bones of patients with known inner ear diseases. In his book Diseases of the Ear (1860), Toynbee noted that not one dissection of a diseased ear had been done before 1800, although thousands of dissections had been done on most other organs of the body. The techniques for examination of the temporal bones developed by Joseph Toynbee had multiple technical problems. German otolaryngologist Karl Wittmaack solved these problems and revolutionized the study of temporal bone specimens. An otologist in London, Charles Skinner Hallpike, became interested in studying temporal bone pathology while visiting Wittmaack’s clinic in Germany. Hallpike went on to develop his own temporal bone laboratory in which he worked with neurosurgeon Hugh Cairns to describe the pathology of Ménière’s disease.

The five articles that are a part of this meta-ethnography took very different views on the development of Native American youth identity. Over the past decade, professionals working to reform justice systems and reestablish the rule of law in countries that previously had repressive regimes developed a framework for thinking about the demands that arise for dealing with the brutal heritage of government repression. The framework has four parts: holding perpetrators accountable, ensuring that there is no recurrence by reforming institutions and vetting personnel, seeking the truth of what happened to individuals and to the society as a whole, and obtaining reparation for the harm done, personally or collectively. Each of these demands can be met through various mechanisms, but all of them depend on archives. Rebuilding the justice sector and the nation through access to archives is a vital element in coming to terms with the past. This chapter reviews the topic of problematic sexual behaviors committed by elderly men and women. Given the dearth of research done on problematic sexual behaviors in older people, references in this chapter include studies involving people as young as 55. Age-relevant findings regarding criminal sex offenses committed by elderly sex offenders and recidivism are considered along with aspects of this population that are similar to and different from younger men and women. Important ethical and sexual boundaries and their implications are discussed. Patterns of sexual offenses committed by elderly offenders against children are reviewed. Specific risk factors, particularly the role of cognitive impairment and psychiatric illness, are identified and described. Approaches to assessment and treatment are also presented. The chapter concludes with suggestions for future research.

I’m not sure where to begin my story. My name is Amanda Keller, and in the late 1990s, when I was 13, I was placed in out-of-home placement by a psychiatrist in Cincinnati, Ohio. I’d been abused most of my childhood by my stepfather, who was a sadist. I have a number of poorly healed fractures to testify to what he did to me physically. Psychologically, the effect of being tortured for years really took its toll, and I think the psychological impact was much worse. In addition to being in a bad mental state because of what was done to me, I was also not well because of what wasn’t done for me. I’d reported the abuse formally four times to professionals before anything happened. Relatives and neighbors were aware of what was happening, and I guess those that knew the most didn’t know what to do. One of the times I reported my stepdad we moved to a different state, so I can’t blame the police on that occasion, but then again, no one saved me. This effect of not being helped when I reached out, which is a scary thing to do when your relative plans his abuses, made me hate everyone, in particular adults....

Guillain-Barré syndrome may present in several ways, although predominant proximal weakness is a common feature of the disease to recognize. The differential diagnosis may be extensive and can include infection, vasculitis, toxin exposure, and malignancy. A lumbar puncture must be done with minimal delay to evaluate for cerebrospinal fluid (CSF) albuminocytological dissociation, however results may be normal early in the course of the disease. EMG/NCS are helpful to support the diagnosis, and early treatment with intravenous immunoglobulin (IVIG) is essential. This chapter discusses the clinical features and diagnostic considerations of this important condition.

The case is an example of a ruptured arteriovenous malformation (AVM) causing an intracranial hemorrhage during pregnancy. Volume changes that occur during pregnancy predispose to AVM rupture. These changes peak at 25–30 weeks of gestation. An unruptured AVM if identified should be treated in the nonpregnant state. Once there has been a hemorrhage related to an AVM the risk of re-rupture is high. Therefore, treatment is dictated by best neurosurgical considerations rather than obstetric concerns. If an AVM ruptures near term an emergent caesarian-section and hemicraniectomy could be done in tandem.
