Abstract:
AbstractTranscranial Doppler (TCD) is a bedside, non-invasive, reproducible, non-expensive neuromonitoring device which can be used in many clinical scenarios. Based on the principle of the Doppler shift, blood flow velocity (FV) in the cerebral vessels can be measured. It should be noted that TCD measures blood FV and not the cerebral blood flow (CBF). However, in a given condition, FV can be used as a surrogate marker for vessel diameter or CBF. Indirectly, it can also measure the CBF and the intracranial pressure. This review describes briefly the method of using the equipment and the vario

The detection of micro-embolic signals (MES) is a mature application of transcranial Doppler (TCD) ultrasound. It involves the identification of abnormally highpitched signals within the arterial waveform as a method of diagnosis and prediction of embolic complications in stroke patients. More recently, algorithms have been developed to help characterise and classify MES using advanced signal processing techniques. These advances aim to improve our understanding of the causes of cereberovascular disease, helping to target the most appropriate interventions and quantifying the risk to patients

This thesis uses functional transcranial Doppler sonography (fTCD) to investigate hemispheric asymmetries in brain activity during language processing. FTCD is a simple method that provides a measure of relative lateralisation. Given its portability and tolerance for movement, it allows physiological activity and behaviour to be measured simultaneously in understudied paediatric populations. The fi rst half of the thesis describes three methodologically motivated studies with adults. The results indicated that the strength of lateralisation is affected by experimental manipulations of task and

Intracranial pressure (ICP) is an important monitoring modality in the clinical management of several neurological diseases carrying the risk of fatal intracranial hypertension. However, this parameter is not always considered due to its invasive assessment. In this scenario, a non-invasive estimation of ICP (nICP) may be essential, and indeed it has become a Holy Grail in Clinical Neurosciences: extensively searched, albeit never found. This thesis is devoted to the assessment, applications and development of transcranial Doppler (TCD)-based non-invasive methods for ICP and cerebral perfusion

Transcranial Doppler ultrasound (TCD) is a non-invasive, safe and readily repeatable technique of measuring cerebral blood flow velocity (CBFV) which may be ideally suited for the bedside assessment of cerebrohaemo-dynamic changes in children. A number of clinical conditions in which this technique could yield useful information was investigated i.e in infants and children with hydrocephalus, pyogenic meningitis and during cardiopulmonary bypass (CPB) surgery. A range of normal CBFV values was first established from 51 normal children. Data on the peak-systolic (PSV), end-diastolic (EDV), mean
