Abstract:
4133 Background: Due to the small anatomic size, multiplicity of epitheliums, and suboptimal diagnostics determining the site of origin of PAA is a challenge. We investigated the ability of a 92-gene RT-PCR assay (CancerTYPE ID) to categorize PAA and to prognostically stratify ampullary adenocarcinomas. Methods: 171 PAA patients who underwent pancreaticoduodenectomies were included; samples were histopathologically verified for tumor subtype determination: pancreatic (PAN), extrahepatic biliary (EB), ampullary (AMP), or duodenal (DOUD). Blinded FFPE tumor sections underwent molecular testing. Analytical sets were an initial 45 PAA set evaluating concordance to histopathological tumor types and prognostic performance, and a second set of 126 AMP and DOUD adenocarcinoma for validation of prognostic performance. Results: Of the initial 45 patient cohort, molecular classification of 43 (96%) evaluable samples (13 AMP, 10 PAN, 10 EB, 10 DOUD) showed 91% concordance: AMP [5 intestinal (int), 7 pancreaticobiliary (pb)], PAN [10 pb], DOUD [3 int, 7 gastroesophageal (ge)], EB [7 pb]. The 92-gene assay was prognostic with a median OS of 70 m for ge/int cases vs. 32 m for pb cases, P=0.05. Discordant classifications were ge for 1 AMP and 2 EB samples, and ovary for 1 EB case. Previous unsupervised RNA hierarchical clustering (Overman GI ASCO 2011 a161) of all 13 AMP cases had identified two prognostic groups (a good-prognosis int-like and a poor-prognosis pb-like), which were identical to the 92-gene classification for 12 of the 13 cases. Conclusions: In the initial cohort of 45 patients, the 92-gene assay demonstrated diagnostic utility for molecular site-of-origin classification of PAA; evaluation of the remaining 126 ampullary and duodenal cases will be presented. Results support exploration of this approach for the management of metastatic PAA (in which pathologic review of a primary resection specimen is not an option). Molecular classification of ampullary adenocarcinomas into intestinal and pancreaticobiliary subgroups is prognostically relevant; these and the gastric-like molecular profile of duodenal adenocarcinomas may have therapeutic implications.

ObjectivesThis study aimed to compare and evaluate any differences in clinical and radiological outcomes between three surgical techniques of cranio-cervical decompression (CCD) in adults with symptomatic Chiari malformation type I (CM1).DesignRetrospective review using the theatre management system (ORSOS) and records of the patients who underwent CCD for CM1 from January 2011 to January 2018.SubjectsPatients over 16 years of age who underwent CCD secondary to CM1 diagnosed by clinical and radiological criteria.MethodsPatients were divided in three cohorts according the operative technique used: an extradural osteo-ligamentous decompression (BD), BD plus dural opening either without duroplasty (DOWD) or with watertight augmentative duroplasty (DOPD). The primary clinical outcome was measured by utilizing the Chicago Chiari Outcome Scale (CCOS). Syrinx outcome was measured on post-op MRI. Statistical analysis was performed using IBM SPSS 24 with α=0.05.ResultsFifty-two adults underwent fifty-three CCD: 7 BD, 29 DOWD, 17 DOPD. Median follow-up was 12 months (IQR 6–23). Patients who underwent BD or DOPD had a shorter median hospital stay (6 days) than DOWD (11 days). Median CCOS was lower following DOWD (13) compared to BD (14) and DOPD (15). Post-operative complications were higher in DOWD (51.7%) compared to DOPD (17.5%).ConclusionsBetter clinical outcomes, lower complication risk and shorter duration of hospital stay were associated with DOPD. Prospective randomized studies could confirm these findings.

Această sărbătoare a fost o bună ocazie pentru organizarea a două conferințe, una derulată la Universitatea de Științe Agricole și Medicină Veterinară din Timișoara (15 noiembrie) și cealaltă, o săptămână mai târziu (22 noiembrie) susținută la Academia de Științe Agricole și Silvice „Gheorghe Ionescu- Șișești”. Prima conferință a concentrat mulți oameni de știință din România care au susținut zece lucrări diferite, axate pe diferite aspecte ale ecologiei forestiere aplicate, în timp ce a doua a fost mai restrictivă în ceea ce privește standardele științifice: doar patru lucrări au fost permise și subiectele lor au fost strict legate de populație, biocenoze și ecosisteme.

Ea continuă cu o a doua secțiune, în care cititorul va observa un soi de dialog între APIA și First Investment Bank AD; cele două entități au discutat pe următoarea temă: ar trebui să fie întrebată ÎCCJ dacă scrisoarea de garanție este titlu executoriu? A treia secțiune evidențiază că o instanță română a hotărât să se adreseze ÎCCJ, astfel încât cea din urmă să stabilească răspunsul la interogația expusă. În secțiunea a patra, încercăm să stabilim dacă art. 120 din OUG nr. 99/2006 i se aplică unei scrisori de garanție. În cea de-a cincea, concluzionăm că art. 120 îi poate fi aplicat unei scrisori de garanție, dar numai dacă contractul de bază este unul de credit. A șasea secțiune subliniază decizia ÎCCJ și se încheie cu niște hrană pentru minte.

În cea de a doua jumătate a secolului al XX-lea, societatea românească a fost marcată de două evenimente care i-au marcat destinul: instaurarea regimului comunist, după abdicarea Regelui Mihai I în 1948, și Revoluția Română din 1989, care a marcat sfârșitul acestui regim. Monodia bizantină are o tradiție milenară în această zonă a Europei, iar contribuția psalților autohtoni la perpetuarea muzicii de strană a Bisericii Ortodoxe – inclusiv prin creații proprii – este atestată de numeroasele manuscrise redactate de români și de tipăriturile ultimelor două secole. Creația muzicală redată cu notația neumatică specifică Bisericii Ortodoxe s-a manifestat în România, în secolul al XX-lea, cu discontinuități datorate evenimentelor istorice amintite. Cântarea de strană, în stilul psaltic tradițional, a supraviețuit, deși a fost afectată de hotărârile luate de Partidul Comunist, în privința Bisericii, respectiv de încercarea de uniformizare a muzicii de strană. Lucrarea surprinde felul în care s-a reușit păstrarea tradiției și perpetuarea muzicii bisericești în dificila perioadă comunistă, insistând asupra creației muzicale religioase redată cu notația neumatică și asupra unor particularități specifice perioadei, datorate regimului politic. Muzicienii formați înainte de instaurarea comunismului – de dascăli păstrători ai bunei tradiții de cântare psaltică, în școli mănăstirești și seminarii teologice prestigioase din perioada interbelică – au fost liantul care a asigurat revigorarea rapidă a muzicii de tradiție bizantină în ultimul deceniu al secolului al XX-lea și au îmbogățit repertoriul stranelor românești cu valoroase creații originale.

În plină campanie de eliberare a Transilvaniei, Consiliul Dirigent din Sibiu a hotărât să sprijine efortul armatei române și a dispus înființarea unui Comandament General Teritorial, cu misiunea de a începe recrutarea în Ardeal, în Banat și în ținuturile românești din Ungaria, pentru constituirea unor unități de voluntari. Mai târziu, din rândul acestora, au fost înființate Corpurile 6 și 7 armată, recrutate exclusiv dintre ardeleni, indiferent de naționalitate. În baza Decretului I al Consiliului Dirigent, legile, ordonanțele, regulamentele și statutele legale emise înainte de 18 octombrie 1918, data proclamării independenţei Transilvaniei față de Budapesta, au rămas provizoriu în vigoare. În acest context, militarii diviziilor ardelene erau supuși jurisdicției militare, în baza Codului penal militar austro-ungar din 1855, în timp ce militarii armatei române care acționau sub comanda Comandamentului Trupelor din Transilvania se aflau sub jurisdicția legilor românești, implicit a Codului de justiție militară.

Rezumat În cadrul articolului cu titlul „Beton monolit versus beton prefabricat la execuția în consolă a podurilor” vor fi abordate o serie de probleme care apar atât la proiectarea cât şi la execuţia structurilor de poduri din beton precomprimat utilizând metoda execuţiei în consolă. Această metodă a revoluţionat podurile din beton, din punct de vedere al deschiderilor şi al uşurinţei traversării unor obstacole dificile. În afara avantajelor realizării unor structuri din beton monolit prin execuţie în consolă, utilizarea elementelor prefabricate conduce la micşorarea semnificativă a timpului de execuţie şi la diminuarea problemelor care pot apărea din cauza curgerii lente şi a contracţiei betonului. Materialul prezentat în cadrul articolului cu titlul „Beton monolit versus beton prefabricat la execuția în consolă a podurilor” va prezenta principiul metodei de execuţie în consolă, avantajele utilizării acestei metode, cazurile particulare pentru care se pretează o astfel de structură, scheme statice şi secţiunile transversale adoptate pentru acest tip de structuri, cât şi modul de îmbinare al tronsoanelor prefabricate în funcţie de generaţia din care fac parte. De asemenea, se va prezenta un studiu de caz în care vor fi analizate cele doua variante de execuție în consolă (turnare monolită a tronsoanelor respectiv montare de tronsoane prefabricate) și vor fi prezentate concluzii privind oportunitatea utilizării uneia dintre cele două variante.

Background:Vasculopathy is a key feature of systemic sclerosis (SSc), manifesting clinically as Raynaud’s phenomenon (RP) with or without digital ischemia. Laser doppler flowmetry (LDF) with thermal challenge is a safe, noninvasive and reproducible technique to detect digital occlusive arterial disease (DOAD) with a high sensitivity and specificity of >90% (1).Objectives:To study the prevalence and clinical correlates of DOAD assessed by LDF in patients with SSc referred for evaluation of RP at a tertiary referral center.Methods:Medical records of all patients with SSc meeting ACR/EULAR 2013 classification criteria that underwent LDF between Jan 2001-Dec 2018 at our institution were retrospectively reviewed to abstract the presence or absence of DOAD. The presence of DOAD on LDF was confirmed if pre- and post-warming skin blood flow was ≤206 arbitrary units. Severity of DOAD was assessed based on number of digits involved. Risk factors associated with presence of DOAD in SSc, and correlation between presence and severity of DOAD with digital ischemic complications were studied.Results:304 patients with SSc (mean age 57.1 ± 3.3 y, 81% females, 93% Caucasians) underwent LDF during the study period. Median time between SSc diagnosis and performing LDF was 12.9 months. Majority of patients with SSc had limited cutaneous SSc (lcSSc) (79.6%) and 64.1% had a positive SSc specific antibody.On LDF with thermal challenge, presence of DOAD was noted in 243 (79.9%) patients, of whom 78.6% had lcSSc, 42.4% had a centromere antibody (Ab), 17.3% had a Scl-70 Ab, 53.5% had interstitial lung disease, 36.6% had pulmonary arterial hypertension, and 73.3% had gastrointestinal dysmotility (GID). Of 159 patients with DOAD who also had a nailfold capillaroscopy, 70.4% had abnormalities. Large vessel occlusive disease was significantly higher in patients with DOAD in comparison to those without DOAD (29.2% vs 16.4%; p: 0.04). After adjusting for age and sex, GID (OR: 2.73 [95%CI 1.52-4.92]) and telangiectasia (OR: 2.83 [95%CI 1.23-6.40]) were significantly associated with DOAD.Digital ischemic complications among patients with SSc with DOAD were significantly higher than among those without DOAD (79.8% vs 41.0% had digital ulcers, 53.9% vs 26.2% had pitting/scars, 31.3% vs 8.2% had gangrene/amputation; p <0.001). The presence and severity of DOAD strongly correlates with digital ischemic complications and can be used as a guide to counsel patients and determine the aggressiveness of therapeutic interventions. Abstract:
Abstract Bursaphelenchus doui was isolated from a dead Japanese red pine, Pinus densiflora, in Shizuoka, and from the tracheal system of a species of longhorn beetle, Monochamus subfasciatus, collected at Tama Forest Science Garden of Forestry and Forest Products Research Institute, Tokyo, Japan. The Japanese populations of B. doui were compared with the original description of material obtained from coniferous packaging materials imported from Taiwan and Korea to continental China. Additional characters from the Japanese population include a constricted female mucron with a step-like appearance and several morphometric values. The molecular profiles of the Japanese B. doui populations were determined by DNA sequencing and ITS-RFLP profiles and were compared with those of the Taiwanese and Chinese populations of B. doui and other species in the genus. The phylogenetic analysis of the small subunit and large subunit ribosomal DNA indicated that B. doui is clearly included in the xylophilus-group of the genus Bursaphelenchus and may be close to B. conicaudatus and B. luxuriosae. The potential risk of B. doui for pine species is considered to be relatively low because B. doui did not display any pathogenicity to Japanese black pine in an inoculation test.

Rostropria, gen. nov., comprising six new species (casta, garbo, gondola, inopicida, simplex and spiniventris) from eastern Australia, is most closely related to Neurogalesus Kieffer. The palpal formula is variable within Rostropria. R. inopicida is a parasite of the sugarcane soldier fly, Inopus rubriceps (Macquart) (Stratiomyidae), and a potential biological control agent for this pest of pastures and sugar cane. Euhoplopria Dodd is synonymised with Neurogalesus and the three described species (E. carinatifrons Dodd, E. lativentris Dodd, E. emargipennis Dodd) are transferred to Neurogalesus.

We argue that implementation of the Dodd-Frank Wall Street Reform and Consumer Protection Act has contributed significantly to the reduction of systemic risk in the United States. However, Dodd-Frank also introduced burdensome rules that have little to do with systemic risk. This article evaluates the trade-off between capital regulation and regulation of scope in the context of Dodd-Frank, with a particular emphasis on the Volcker Rule.
