Abstract:

Abstract Several studies have demonstrated that homeobox (Hox) genes are involved in the regulation of hematopoietic stem cells (HSC), and overexpression with retroviral vectors containing HOXB4 generate increased numbers of repopulating stem cells in vitro, but may also perturb differentiation of hematopoietic cells when the concentration of HOXB4 is very high. HOXA10 is expressed in primitive hematopoietic cells and myeloid progenitors. To study the effect of this gene we generated an inducible system based on a tetracycline transactivator, controlling the expression of HOXA10, aiming to study how different concentrations of HOXA10 affect the fate of hematopoietic progenitors and stem cells. We mated our tetO-HOXA10 mouse with the Rosa26rtTA strain, allowing activation of HOXA10 in all hematopoietic tissues after administration of doxycycline. Mice were born at normal ratios with no hematopoietic pathology. Inducible bone marrow was harvest and cultured for 12 days in 6 different concentrations of doxycyclin, revealing an increased proliferation at low concentrations, but a decline in proliferation capacity with higher concentrations. To verify that hematopoietic progenitors were affected, a CFU-GM colony assay was performed on cells cultured for 12 days, showing a two fold increase in the number of CFU-GM formed from the highly proliferating cells compared to wt and uninduced HOXA10 cells (p = 0.01). To study the effect of HOXA10 in more primitive cells, sorted inducible HOXA10 lin−, Sca1+, c-kit+ (LSK) cells were cultured for 13 days in different concentrations of doxycyclin. Lower concentrations of doxycyclin resulted in increased proliferation, while increasing concentrations resulted in decreased proliferation. Furthermore, using Q-RT-PCR, we found that the expression of HOXA10 was directly proportional to the concentration of doxycycline and no leakiness was detected in the uninduced LSK cells. The cultured cells were transplanted in a competitive setting into lethally irradiated mice to evaluate the repopulating ability of the expanded cells. Three weeks post BMT (short-term repopulation), intermediate levels of HOXA10 (0.08–0.2 mg/ml doxycyclin) resulted in a three-fold increase in repopulating capacity of the HOXA10 LSK cells whereas uninduced and higher levels of HOXA10 resulted in decreased reconstitution compared with fresh LSK cells (fresh LSK = 100%, intermediate: 313±182%, high: 45±35%, uninduced 35±33%, n=7 p< 0.01). However, sixteen weeks after transplantation we found that cells cultured for 13 days at intermediate levels of HOXA10 (0.08–0.2 mg/ml doxycyclin) preserved the stem cell reconstitution capacity compared to fresh LSK cells (fresh LSK = 100%, 0.2 mg/ml 153±82% n=7). Furthermore, uninduced LSK cells and higher levels of HOXA10 resulted in a 3 fold lower long-term reconstitution compared to Fresh LSK cells (0 mg/ml 34±32 %, high HOXA10 9±8% significant to both fresh cells and cells cultured in 0.2 mg/ml, p<0.003, n=7). These findings show that intermediate expression of HOXA10 can increase the short-term HSCs repopulating potential and can maintain the long-term repopulating stem cells for up to 13 days of in vitro culture. These results suggest that HOXA10 plays an important role in the regulation of HSCs and indicate that the effect of HOXA10 on stem cell fate decisions is dependent on the level of HOXA10 expression.

Abstract:

Hintergrund: Eine der wichtigsten dermatologischen Nebenwirkungen von Doxycyclin ist erhöhte Lichtempfindlichkeit. Da Doxycyclin eine wichtige Rolle für die Malariaprophylaxe spielt und Malaria vor allem in sonnenreichen Ländern verbreitet ist, gebührt dieser Nebenwirkung besondere Aufmerksamkeit. Zwar liegen bereits zahlreiche Publikationen zur Fototoxizität von Tetrazyklinen im Allgemeinen vor, doch nur wenige befassen sich gezielt mit Doxycyclin. Das Ziel der vorliegenden systematischen Übersichtsarbeit war es, alle verfügbaren Berichte zu klinischen Manifestation, Einflussfaktoren wie UV- und Arzneimitteldosis sowie die Möglichkeiten der Prävention durch Sonnenschutz zusammenzufassen. Methoden: Die vorliegende Übersicht beruht auf einer systematischen Literatursuche in PubMed nach Artikeln in englischer und deutscher Sprache sowie einer manuellen Suche für den Zeitraum von 1990 bis 2015. Ergebnisse: Die Zahl der Publikationen ist gering. Die klinischen Symptome reichen von leichten sonnenbrandartigen Erscheinungen (brennendes Gefühl, Hautrötung) bis zu großflächiger Fotodermatitis. Auch Onycholyse kann vorkommen. Das auslösende UV-Spektrum scheint vorwiegend im UVA1-Bereich (340-400 nm) zu liegen, daher sollten Lichtschutzmittel angewandt werden, die diesen Bereich abdecken. Personen, die in tropische Regionen reisen und Doxycyclin als Malariaprophylaxe einnehmen, bedürfen eingehender ärztlicher Beratung, um potenziell schwerwiegende fototoxische Reaktionen zu vermeiden. Schlussfolgerung: Es bedarf einer verbesserten Evidenzbasis, um Reisende, die Doxycyclin einnehmen und dem Risiko hoher UV-Exposition unterliegen, zu angemessenen Präventionsmaßnahmen beraten zu können.

ZusammenfassungSeit etwa zwei Jahrzehnten ist eine Zunahme der Syphilisinzidenz zu beobachten, insbesondere unter Männern, die Sex mit Männern haben (MSM). Diese Entwicklung hat zu einem wachsenden Interesse an Studien zur prophylaktischen Anwendung des Antibiotikums Doxycyclin zur Vermeidung von Syphilisinfektionen und anderen sexuell übertragbaren Infektionen (STI), international meist als Doxycyclin‐Prä‐ beziehungsweise Post‐Expositionsprophylaxe (Doxy‐PrEP, Doxy‐PEP) bezeichnet, geführt. Zugleich steigt die Nachfrage seitens potenzieller Nutzer nach dieser präventiven Maßnahme.Mehrere randomisierte kontrollierte Studien haben gezeigt, dass die prophylaktische Anwendung von Doxycyclin bei MSM und Trans‐Frauen mit HIV‐PrEP‐Gebrauch oder bekannter HIV‐Infektion das Risiko einer Syphilis‐ und Chlamydien‐Infektion wirksam reduziert. Zum gegenwärtigen Zeitpunkt bestehen jedoch noch offene Fragen, insbesondere hinsichtlich der möglichen Auswirkungen einer breiten Anwendung der antibiotischen STI‐Prophylaxe mit Doxycyclin auf Tetrazyklin‐ und andere antimikrobielle Resistenzen bei bakteriellen STI, anderen bakteriellen Pathogenen und Bakterien des Mikrobioms.Angesichts der steigenden Nachfrage und der Herausforderung, eine Abwägung zwischen Wirksamkeit, Sicherheit und dem Risiko der Förderung von Antibiotikaresistenzen vorzunehmen, hat die Deutsche STI‐Gesellschaft (DSTIG) die vorliegende Stellungnahme erarbeitet. Diese enthält spezifische Empfehlungen zu potenziellen Indikationen, Kriterien und Anlässen für den Einsatz von Doxycyclin zur Prävention von STI. Die Empfehlungen basieren auf aktuellen Studienergebnissen und der Meinung von Fachpersonen.

Abstract:

The objective of the study is to formulate the emulgel of Doxycyclin hyclate and Eugenol for sustained or controlled release gel for oral care. Doxycyclin hyclate is an antibiotic which belongs to chemical class tetracyclines and widely used in treatment of oral care bacterial infections. Eugenol oil is extracted from the clove and act as good analgesic and anti-inflammatory. The combination of Doxycyclin hyclate and eugenol can be formulated for better oral care treatment. The compatibility study performed of drug and drug-polymer by IR spectra indicates good compatibility. The simple manufacturing method is employed as hot emulsification. The formulations were evaluated for description, pH, viscosity, spreadability, extrudability, % drug content and content uniformity. Optimized formulations were evaluated for In-vitro release, antibacterial activity, and stability study. Formulated formulations were complying with all the evaluation parameters. The highest drug release found with the formulation OFEG 01 and OFEG 06. The formulations OFEG 06 is the most stable formulation in stability at accelerated, intermediate and room temperature storage condition for 3 months. Abstract:

Zusammenfassung Gegenstand und Ziel: Untersuchung der klinischen Wirksamkeit von über das Trinkwasser verabreichtem Doxycyclin zur Behandlung der Chlamydiose bei Briefund Rassetauben (Columba livia f. domestica) in infizierten, klinisch erkrankten Beständen unter Praxisbedingungen. Material und Methoden: Im Rahmen einer kontrollierten Feldstudie zur klinischen Wirksamkeit wurden 13 Taubenbestände mit klinisch manifester Ornithose über 25 Tage mit einer neuartigen Formulierung von Doxycyclin über das Trinkwasser behandelt. In jedem der Bestände war die Infektion mit Chlamydia psittaci zuvor anhand molekularbiologischer Untersuchung von Organmaterial zur pathologischen Untersuchung eingesandter Tiere festgestellt worden. Zur Überprüfung des Therapieerfolgs wurden am 25. Tag der Behandlung und 7 Tage nach Abschluss der Therapie Dreifachtupfer von 10 Tieren jedes Bestandes genommen und mittels PCR untersucht. Ferner fanden wiederholt Einzeltier- und täglich Herdenuntersuchungen in den Beständen statt. Ergebnisse: Der Erreger Chlamydia psittaci sowie andere, in sechs der 13 Bestände zusätzlich dokumentierte atypische Chlamydienspezies ließen sich bei den genannten Untersuchungen nicht mehr nachweisen. Zudem wurde in jedem Bestand ein Verschwinden der klinischen Krankheitssymptomatik erreicht. Schlussfolgerung: Die Anwendung von Doxycyclin im beschriebenen Therapieregime erwies sich als wirksam zur Behandlung der Ornithose. Darüber hinaus konnte eine hervorragende Akzeptanz und Verträglichkeit der eingesetzten Formulierung in praxi bestätigt werden.

ZusammenfassungDie Urethritis ist ein häufiges, überwiegend durch sexuell übertragene Erreger wie Chlamydia trachomatis, Neisseria gonorrhoeae oder Mycoplasma genitalium verursachtes Krankheitsbild. Alleine aufgrund klinischer Merkmale ist die Unterscheidung der Erreger und der Ausschluss von Koinfektionen nicht sicher möglich. In der praktischen Versorgungssituation wird jedoch oftmals, noch vor dem Erregernachweis, eine empirische antibiotische Therapie durchgeführt. Ziel dieser evidenzbasierten S3‐Leitline ist die Förderung einer rationalen Syndrom‐orientierten Herangehensweise an das klinische Management männlicher Jugendlicher und Erwachsener mit Symptomen einer Urethritis.Neben Empfehlungen für Diagnostik, Klassifikation und Therapie beinhaltet die Leitlinie Empfehlungen zur Indikationsstellung für die empirische antibiotische Behandlung der penilen Urethritis. Neu gegenüber vorbestehenden, erregerspezifischen Leitlinien ist unter anderem das Flowchart für das Syndrom‐orientierte praktische Management. Bei Indikation zur empirischen Behandlung und Verdacht auf eine gonorrhoische Urethritis soll Ceftriaxon angewandt werden. Zusätzlich sollte, aufgrund des Risikos für eine Koinfektion mit Chlamydia trachomatis, Doxycyclin verordnet werden, es sei denn, eine Wiedervorstellung zur Behandlung möglicher Koinfektionen ist gesichert. Bei Verdachtsdiagnose einer nichtgonorrhoischen Urethritis soll die empirische Therapie mit Doxycyclin erfolgen. Azithromycin ist zur empirischen Behandlung der gonorrhoischen und nichtgonorrhoischen penilen Urethritis nur einzusetzen, wenn Kontraindikationen gegenüber Doxycyclin bestehen. Detaillierte Empfehlungen zur Differenzialdiagnostik, erregerspezifischen Behandlungen, spezifischen Situationen und zur Beratung und Nachsorge finden sich in der Leitlinie.

Abstract:

ABSTRACT: An in vitro test on the pharmacological interaction of the antibiotic combination between kitasamycin tartrate and Doxycycline hydrate was carried out. An in vitro challenge method was used for this purpose. The results were used to calculate their interaction using the graphical representation known as ISOBOLOGRAM. The pure salts of Kitasamycin tartrap and Doxycycline Hydrate were used and were obtained from the manufacturer of the product, Lapi - Doxi. The agar bioassay method was used. The following criteria were used to quantify the results: A synergistic effect was defined when the actual/theoretical effect (SMEF) was greater than 1. The additive effect was defined when the SMEF was equal to 1. The area between the antagonism and additive effect was defined as the zone of indifference. When studying the interaction between florfenicol and tylosin tartrate it was observed that the best SMF of this antibiotic combination was 1.45 when the concentration ratio between the antibiotics was 1 : 4 (Kitasamycin tartrate and doxycycline hydrate respectively). Consequently it can be postulated that the combination of kitasamycin tartrate with doxycycline hydrate showed a synergistic effect and can be recommended in poultry and swine for the treatment of mycoplasma infections, most gram + and gram- germs and other infectious agents sensitive to the formulation.

Abstract:

The article presents the results of studies on the absorption, distribution and excretion of the antibiotic Poledoxin (the active substance is doxycycline hyclact) from the body of healthy broiler chickens of the Koob-500 cross when taken orally. Studies have established the rapid absorption of doxycycline hyclact from the digestive tract of the chicken into the blood and its entry into the internal organs. The maximum concentration of doxycycline hyclact was detected 2-4 hours after the start of Poledoxin administration, the content of which in the liver was 9,07±0,07 μg/g, in the lungs – 8,39±0,20 μg/g, in the heart – 4,24±0,09 μg/g, in the pectoral muscles – 4,28±0,23 μg/g. In the kidneys, the maximum concentration was found after 12 hours – 7,63±0,38 μg/g. When clinically healthy broiler chickens were fed with Poledoxin solution, the ability to materially cumulate doxycycline hyclact (active ingredient) was not established, since its content in the internal organs and muscles of the bird did not depend on the duration of the drug use. Studies have established the absence of organ affiliation of doxycycline hyclact, because its maximum content, depending on the study period, was recorded in various organs that provide biotransformation and excretion – liver, kidneys, lungs. The cancellation of drinking Poledoxin ensured the rapid elimination of doxycyclin hyclact from the body of broiler chickens and, after 5 days (for 216 hours of the experiment), its content was minimal for the entire study period and amounted to: in the kidneys – 0,15±0,01 μg/g; liver – 0,12±0,01 μg/g; pectoral muscles – 0,05±0,01 μg/g; lungs – 0,04±0,01 μg/g. Only in one sample of the heart, doxycycline hyclact was shown in an amount of 0,01 μg/g. 14102 Background: The phenotypical characteristics and spread of ovarian cancer cells, suggest intraperitoneal gene therapy of this disease. Phase I trials have been conducted to investigate the potential clinical benefits of adenoviral E1A-based gene therapy. Further gene therapeutic approaches which aim to enhance the efficacy of E1A-induced apoptosis in a multimodal approach including conventional chemotherapy are planned. However, besides tumor-suppressive effects, E1A is also known to transform rodent cells in conjunction with other factors, e.g. an activated ras oncogene. In an effort to eliminate elements favouring malignant conversion, the potential therapeutic effect of the E1AdelCR2 deletion mutant on ovarian cancer cells was studied. Methods: To avoid any selection bias, a doxycyclin-regulated system was used to express E1A wildtype protein and the mutant E1AdelCR2 lacking the p105RB-binding motif. The effects of the E1A proteins on proliferation and induction of apoptosis in ovarian carcinoma cell lines was studied with a WST-1 assay and fluorcytometric analysis of FITC labelled AnnexinV. Results: As confirmed by Western blot analyses, expression of the mutant proteins was almost completely suppressed in the presence of doxycyclin. Substantial reduction in proliferation was achieved by expression of both wildtype E1A and E1AdelCR2. Expression of E1AdelCR2 was sufficient by itself to induce apoptosis in 8,7% of ovarian carcinoma cells as shown by an increase of the fraction of Annexin binding OVMZ-8 cells. A strong synergistic effect with an increase of the fraction of apoptotoc cells by 16.7% was found when the cells were treated with paclitaxel. Conclusion: Deletion of the CR2 sequence should increase the safety of therapeutic applications of E1A without affecting tumor suppression. A doxycyclin-regulated expression system was established allowing the elucidation of the mechanisms underlying the therapeutic effects of E1A in ovarian cancer cells in the future by means of expression profiling and quantification of activated components of signal transduction pathways. Genetically modified pigs are important models for biomedical research. For certain applications, tightly controlled transgene expression is mandatory. For instance, expression of soluble receptor activator of the NFκB ligand (RANKL) is an established principle for inducing clinically relevant osteoporosis (Mizuno et al. 2002 J. Bone Miner. Metab. 20, 337–344). However, constitutive overexpression of RANKL is lethal; thus, an inducible system is required. A second example is the expression of porcine CTLA-4Ig to prevent co-stimulation of T-cells in pig-to-primate xenotransplantation. Because constitutive expression of porcine CTLA-4Ig causes immunosuppression in pigs (Phelps et al. 2009 Xenotransplantation 16, 477–485), an inducible expression system is desirable. The Tet-On system is used most widely. It is based on a tetracycline transactivator (TA) and a transactivator response element (TARE), which drives expression of the coding sequence for the transgene product of choice. Association of doxycyclin with TA allows binding to TARE, resulting in activation of transgene expression. Although widely used in mice, this system has not yet been established in pigs. Because consistently high expression of TA is required for a functional system, we first cloned an expression vector for TA under the control of the CMV enhancer/chicken β-actin promoter (CAG). A floxed neomycin resistance cassette was linked to this vector to facilitate selection of stable clones after transfection of primary porcine kidney cells. The latter were used for nuclear transfer, and cloned embryos were transferred to synchronized recipient gilts, resulting in the birth of 5 CAG-TA transgenic piglets. A spectrum of organs and tissues was investigated by RT-PCR and Western blot for TA mRNA and protein expression. Primary kidney cells from all 5 TA transgenic piglets were transfected with TARE-RANKL and TARE-CTLA-4Ig constructs linked with a blasticidin resistance cassette to facilitate selection of double transgenic/transfected cell clones. These cell clones were individually stimulated with doxycyclin to identify cell clones with low background and high inducibility of expression of RANKL or CTLA-4Ig. For both combinations, several clones meeting these criteria were identified. Such cell clones were used for nuclear transfer to produce double-transgenic pigs. Application of doxycyclin (25 to 50 mg kg–1 of body weight/day) to a TA/TARE-RANKL transgenic pig resulted in a marked increase in plasma RANKL concentrations. In addition, 4 TA/TARE-CTLA-4Ig transgenic piglets have been born, which will undergo doxycyclin stimulation experiments in the near future. In summary, CAG-TA transgenic pigs serve as a reliable basis for inducible transgene expression, as shown in the present study for RANKL and CTLA-4Ig. Im Jahr 2004 war er bei angeborener Aortenklappenstenose mittels Ross-Operation versorgt worden. Es zeigte sich ein Anstieg der Titer für Coxiella burnetii, den Erreger des Q‑Fiebers. Unter antibiotischer Therapie mit Levofloxacin und Doxycyclin kam es zu einer Besserung der Symptomatik und einem Rückgang der Entzündungsparameter. Die Therapie der Endokarditis war erfolgreich.

Die anderen Manifestationen bedürfen der serologischen Bestätigung. Die beste Prophylaxe ist die zügige Entfernung anheftender Zecken.

AimThere is growing evidence that bacteria encountered in periprosthetic joint infections (PJI) form surface-attached biofilms on prostheses, as well as biofilm aggregates embedded in synovial fluid and tissues. However, models allowing the investigation of these biofilms and the assessment of their antimicrobial susceptibility in physiologically relevant conditions are currently lacking. To address this, we developed a synthetic synovial fluid (SSF) model and we validated this model in terms of growth, aggregate formation and antimicrobial susceptibility testing, using multiple PJI isolates.Methods17 PJI isolates were included, belonging to Staphylococcus aureus, coagulase negative staphylococci, Cutibacterium acnes, Pseudomonas aeruginosa, enterococci, streptococci, Candida species and Enterobacterales. Growth and aggregate formation in SSF, under microaerophilic or anaerobic conditions, were evaluated using light microscopy. The biofilm preventing concentration (BPC) and minimum biofilm inhibitory concentration (MBIC) of relevant antibiotics (doxycyclin, rifampicin and oxacillin) were determined for the staphylococcal strains (n=8). To this end, a high throughput approach was developed, using a fluorescent viability resazurin staining. BPC and MBIC values were compared to the minimum inhibitory concentration (MIC) obtained with conventional methods.ResultsThe SSF model allowed all isolates to grow well under microaerophilic or anaerobic conditions. When cultured in SSF, all isolates formed biofilm aggregates, varying in size and shape along different species. A susceptibility testing method based on measuring resazurin-derived fluorescence was successfully developed, allowing high throughput determination of the BPC and the MBIC in SSF. For all staphylococci cultured in SSF a reduced susceptibility to the tested antibiotics was observed when compared to susceptibility data obtained in general medium. For rifampicin and doxycyclin the BPC was consistently higher than the MIC (two- to fourfold dilution difference for rifampicin and four- to sixfold dilution difference for doxycyclin). For oxacillin the MIC equaled the BPC for two isolates, while for the other isolates the BPC was higher than the MIC (two- to fourfold dilution difference). Expectedly, the MBIC was higher than the BPC and differences with the MIC were even more pronounced for all antibiotics tested (differences of six- to fourteenfold dilutions were observed).ConclusionOur data indicate that the in vitro SSF model could provide more insight in how PJI-related pathogens form biofilms in physiologically relevant conditions. The BPC and MBIC were consistently and substantially higher than MIC. The authors describe the case of a 46-year-old man, who developed atypical pneumonia caused by Coxiella burnetii. Chest X-ray revealed interstitial pneumonia. Western blot and ELISA test were positive for Coxiella burnetii antibody. After treatment with doxycyclin and amoxicillin supplemented with vitamin B6 for 10 days, the patient displayed a clinical improvement. ZusammenfassungBei einem 2,5-jährigen, männlich-intakten Foxhound-Beagle-Mischling mit akuter, multifokaler, langsam fortschreitender, erosiv-ulzerierender Dermatitis ohne Juckreiz wurde basierend auf klinischen Symptomen und histologischer Untersuchung von Hautbioptaten ein mukokutanes Pemphigoid diagnostiziert. Betroffen waren Nasenspiegel, Augenlider und Schnauze. Bläschen fanden sich auch an Ohreninnenseiten, Maulschleimhaut und Zunge. Der Patient sprach nicht auf eine immunsuppressive Therapie mit Prednisolon an. Mit oraler Gabe von Doxycyclin und Niacinamid ließ sich eine vollständige Remission erreichen und die Erkrankung blieb unter langfristiger ausschließlicher Therapie mit Niacinamid in Remission.

ABSTRACTThough melioidosis is rare in India, it has gained importance as one of the most potent emerging infections. In India, the cases have been under-reported because of the lack of awareness. The majority of cases present with multifocal pyogenic infections with septicemia. We present an unusual case of melioidosis presenting as acute intestinal perforation. The organism was ceftazidime resistant, and we successfully treated the case with imipenem and doxycyclin. This case highlights ruling out the possibility of melioidosis in acute abdomen and existence of ceftazidime resistant cases in India.

ZusammenfassungBei 2 Hunden wurden im Rahmen der dermatologischen Untersuchung noduläre Hautläsionen im Kopf-/Ohrmuschelbereich nachgewiesen, die mittels histopathologischer, zytologischer und molekularbiologischer Untersuchungen als kanine leproide Granulome charakterisiert werden konnten. Die Erkrankung wird durch nichttuberkulöse, bisher nicht vollständig klassifizierte Mykobakterien verursacht und stellt eine im pazifischen Raum sowie in Nord- und Südamerika dokumentierte Hautläsion dar, die in Deutschland bisher nicht beschrieben wurde. Durch die in beiden Fällen erfolgte Kombination aus chirurgischer und medikamentöser Langzeittherapie (Rifampicin, Clarithromycin, Doxycyclin und lokal Clofazimin) konnten die Granulome entfernt bzw. zur Regression gebracht werden.

Wir stellen einen Patienten mit einem seit 2 Jahren bestehenden ausgeprägten multilokulären B-Zell-Pseudolymphom am Rücken vor. In der weiteren Diagnostik zeigten sich Antikörper gegenüber Borrelien vom IgG- und IgM-Typ sowie ein CD20-positives Infiltrat, nukleär eine bcl-6-Expression sowie ein Fehlen einer bcl-2-Expression. Es wurde ein IgM-positives Borrelien-assoziiertes multilokuläres Pseudolymphom diagnostiziert und eine Therapie mit Doxycyclin 200 mg/Tag über 3 Wochen eingeleitet.

Jährlich werden 20000 bis 30000 importierte Fälle von Malaria in industrialisierten Ländern diagnostiziert, in der Schweiz sind es rund 700 Erkrankungen. Neben der Expositionsprophylaxe wird in Hochrisikoländern eine Chemoprophylaxe empfohlen. Darunter versteht man die regelmäßige Einnahme eines Malariamedikamentes in sub-therapeutischer Dosierung zur Unterdrückung einer symptomatischen Infektion. Die Einnahme der aktuell gebräuchlichen Medikamente beginnt in der Regel eine Woche vor der Abreise und wird bis einschließlich vier Wochen nach der Ausreise aus einem Malariagebiet fortgesetzt. In der Schweiz stehen Mefloquin, Doxycyclin, Chloroquin plus Proguanil sowie vermutlich bald Atovaquone plus Proguanil für die Chemoprophylaxe zur Verfügung.

Abstract:

Introduction: Cutaneous anthrax (CA), a zoonotic infectious disease is an important endemic public health disease in rural areas around the world, accounting for 95% of anthrax cases. Methodology: Fifty patients with CA were diagnosed by the presence of characteristic skin lesions and positive response to treatment. Twenty-nine patients had been treated with oral ciprofloxacin or doxycyclin for 14 days and 21 patients had been treated with intramuscular procaine penicillin for 7 days. The demographic risk factors, characteristics and treatment of CA in rural areas were evaluated. The responses to two different systemic medications were compared using χ2 test. Results: Twenty-two males and 28 females were included in this study. The predominant skin lesions were black eschar, ulcer and swelling of the skin. The predilection sites were the hand and fingers. The most common route of contamination for both male and female patients was handling raw meat. The most common occupation was housewife for female patients and animal industry for male patients. The patients under ciprofloxacin or doxycyclin administration responded better to treatment; pain at lesion site and new lesions at the time of treatment were significantly lower. Secondary infection appeared to be higher in patients under procaine penicillin administration, although this difference was not statistically significant. Conclusions: In rural areas that lack medical facilities with diagnostic tools, in the presence of black eschar, rapid diagnosis and treatment of CA is essential. Abstract:

Zusammenfassung Gegenstand: Retrospektive Untersuchung der mittels bronchoalveolärer Lavage (BAL) gewonnenen Keimflora der tiefen Atemwege erkrankter Hunde auf ihre Antibiotikaempfindlichkeit über 5 Jahre. Material und Methoden: Auswertung der Ergebnisse der Agardiffusionstests von Bakterienisolaten, die in den Jahren 2004–2009 von 84 Hunden isoliert wurden, und Vergleich mit den Resultaten einer gleichartigen Analyse der Jahre 1999/2000. Ergebnisse: Bei den 99 geprüften Bakterienisolaten handelte es sich um Pasteurella spp. (27,3%), Bordetella bronchiseptica (20,2%), Staphylococcus spp. (18,2%), Escherichia coli (15,2%), Klebsiella spp. (8,1%), Pseudomonas spp. (7,0%) und Streptococcus spp. (4,0%). Bei acht Hunden lagen Mischkulturen vor. Die Mehrzahl der Bordetella-(B.-)bronchiseptica-Isolate erwies sich als sensibel gegenüber den Fluorchinolonen sowie Tetracyclin, Doxycyclin und Polymyxin B. Im Vergleich zu 1999/2000 zeigte sich ein höherer Anteil der gegenüber Amoxicillin/Clavulansäure und Chloramphenicol empfindlichen B.-bronchiseptica-Isolate, wobei die Zunahme bezüglich Chloramphenicol statistisch signifikant war. Die überwiegende Anzahl der Staphylococcus-Isolate erwies sich gegenüber Enrofloxacin, Marbofloxacin, Amoxicillin/Clavulansäure, Chloramphenicol, Cephalexin, Doxycyclin und Polymyxin B sensibel. Im Vergleich zu 1999/2000 ergab sich ein erhöhter Anteil an Chloramphenicolund Tetracyclin-sensiblen Staphylococcus-Isolaten. Der Anteil Enrofloxacin-sensibler KlebsiellenIsolate betrug 62,5%, während er 1999/2000 bei 100% lag. Eine Sensibilität aller getesteten Klebsiellen-Isolate ließ sich noch gegenüber Polymyxin B nachweisen. Kein Antibiotikum zeigte eine Wirksamkeit gegenüber allen E.-coli-Isolaten. Statistisch signifikant war die Abnahme der gegenüber Amoxicillin/Clavulansäure-sensiblen Isolate. Schlussfolgerung und klinische Relevanz: Die Studie unterstreicht den Nutzen der BAL für eine ätiologische Diagnostik und den anschließenden fundierten Einsatz von Antibiotika. Die Mehrzahl der Bakterienisolate erwies sich als empfindlich gegenüber den Fluorchinolonen.

Abstract:

Abklärung eines 38-jährigen Patienten mit rezidivierendem Status febrilis und mit computertomographisch nachgewiesener Splenomegalie mit grosser scholliger Verkalkung lienal. Differentialdiagnostisch ist das Spektrum sehr gross und reicht von viralen über bakterielle Infekte bis zu den Kollagenosen und malignen Tumoren. Als eher seltene Ursache einer febrilen Erkrankung sollte auch die Brucellose in die Differentialdiagnose miteinbezogen werden. Bei unserem Patienen wurden Brucellen nachgewiesen, möglicherweise akquiriert durch Verzehr von Fetakäse in Zypern. Trotz resistenzgerechter antibiotischer Therapie mit Rifampicin und Doxycyclin wurde der Patient nicht beschwerdefrei. Erst nach Splenektomie, resp. Entfernung des Bakterienherdes in der Milzverkalkung konnte der Patient definitiv geheilt werden.

Abstract:

Eine 39-jährige Patientin präsentierte sich mit seit einer Woche bestehenden krampfartigen Unterbauchschmerzen, zuletzt mit Schmerzverlagerung in den rechten Oberbauch. Klinisch bestand eine Druckdolenz im rechten Oberbauch, laborchemisch fand sich einzig ein erhöhtes CRP, die Abdomen-Sonographie fiel unauffällig aus, computertomographisch wurden eine diffuse Wandverdickung im duodeno-jejunalen Übergang und freie Flüssigkeit im Douglasraum beschrieben. Mittels PCR aus Urin und Zervixabstrich konnte die Diagnose einer Infektion mit Chlamydia trachomatis gestellt werden, die Oberbauchschmerzen wurden als Perihepatitis interpretiert (Fitz-Hugh-Curtis-Syndrom). Unter einer Therapie mit 2×100 mg Doxycyclin über 14 Tage war eine rasche Normalisierung aller Parameter feststellbar.
