Abstract:
Research about lactation and breastfeeding has exploded since the Journal of Human Lactation ( JHL) began publishing in 1985. To discuss the 3-decade-long role of the Journal in promoting, supporting, and disseminating lactation research, editors convened a multidisciplinary group of lactation researchers and providers which comprised three senior researchers and one clinical practitioner, all of whom have a long history of lactation advocacy. Their discussion took place on July 1, 2019. Dr Aimee Eden is a medical anthropologist who leads the qualitative research efforts in a small research department. Her dissertation research focused on the maternal and child healthcare workforce, and the professionalization of breastfeeding support. She served on the Board of Directors of the International Board of Lactation Examiners (2010–2016) and currently serves on the board of the Monetary Investment for Lactation Consultant Certification. Dr Karleen Gribble has been conducting research for 15 years about infant and young child feeding in emergencies, long-term breastfeeding, milk sharing, early childhood trauma, adoption, and fostering. She is an Australian Breastfeeding Association community educator and breastfeeding counselor and a member of the Infant and Young Child Feeding in Emergencies Core Group. Elien Rouw is a practicing physician in Germany specialized in healthy infant care, with a long-standing specialization in breastfeeding medicine. She serves on the Board of Directors of the Academy of Breastfeeding Medicine, is a member of the German National Breastfeeding Committee and their delegate to the World Alliance for Breastfeeding Action. Dr Jackie Wolf is an historian of medicine, whose research focuses on the history of childbirth and breastfeeding practices in the United States and how those practices have shaped women’s and children’s health, as well as public health, over time. Her latest book, published by Johns Hopkins University Press and funded by a 3 year grant from the National Institutes of Health, is Cesarean Section: An American History of Risk, Technology, and Consequence. Dr Sara Gill moderated the discussion. She was a member of the Board of Directors of the International Lactation Consultant Association for 5 years, and has been an Associate Editor of the Journal of Human Lactation for the past 4 years. Her research has focused on breastfeeding among vulnerable populations. Abstract:
Diabetic retinopathy (DR) is one of the most prevalent microvascular complications of diabetes, and the most leading cause of visual loss around the world. The lack of effective and approved treatment in DR is a major challenge for diabetic patients. Nowadays, natural compounds have got attention of the researchers for management of DR. Many evidences suggest that puerarin as a natural polyphenol exerts advantageous effects against DR. In the present review, we summarized the protective effects of puerarin against DR, and discussed the underlying mechanisms of these effects. Puerarin attenuates retinal neovascularization and neurodegeneration in diabetes mellitus, and the underlying mechanisms are related to antioxidant, anti-inflammatory, and antiapoptotic properties of the compound. In conclusion, puerarin might be a potential adjuvant agent for the prevention and treatment of DR. However, comprehensive studies are necessary to show its effectiveness and safety, particularly in human.

Abstract:
As set out in the Australian Constitution at Federation, the Commonwealth’s only health responsibility was in the area of quarantine. States retained jurisdiction for public health. The Commonwealth became involved in more extensive matters of health during World War One, however, by intervening in and managing the medical services of the defence forces. Following the war there were calls to establish a Royal Commission to consider national health policy and practice. This thesis analyses the Royal Commission and contextualises it within the post-war period of reconstruction and change. The study shows the importance of Director-General of Quarantine, Dr John Howard Lidgett Cumpston, and suggests that he was the driving force behind the Royal Commission. His new Department of Health’s powers were limited, especially concerning relationships between the States and Commonwealth, and Cumpston wanted a Royal Commission to examine all matters of health, including relations between the States and the Commonwealth. He used his influence to persuade Prime Minister Stanley Bruce to agree to a Royal Commission. An exploration of Cumpston’s private papers, letters, notes and publications, shows that although the Commission was meant to be independent of the Department of Health, Cumpston was influential throughout the whole process. The central focus of this thesis is an analysis of the proceedings of the Royal Commission, and from several themes to emerge two are taken as separate objects of examination. Firstly, Infant and Maternal Welfare, in particular the 1912 Maternity Allowance; and secondly, the Medical Services of the Defence Forces and of the Repatriation Department.

Az egyre élesedő piaci verseny minden gazdálkodó számára megnehezíti a hatékony működést, különös tekintettel a Covid19 világjárvány okozta gazdasági nehézségekre és recesszióra. A világjárvány terjedése miatt az egész világon és hazánkban is számos gazdaságvédelmi intézkedés lépett hatályba, amely közül tanulmányunkban a számviteli, adózási változásokra fókuszálunk. Ilyen gazdasági periódusban felerősödik a számviteli értékelések, számviteli politikai döntések hatása, hiszen a gazdálkodóknak olyan fontos döntéseket kell meghozniuk, amelyek alapvetően befolyásolják a vállalkozások további működését és eredményességét. A vezetőknek fokozottabban figyelembe kell venniük a jövőre vonatkozó információkat, a körülmények várható alakulását és lehetséges alternatívakat kell megfogalmazniuk. A világjárvány következményei, a homeoffice, a távolságtartás jelentős hatást gyakorolt a dolgozók juttatásaira, a cafeteria szabályokra is. A munkáltatói költségek csökkentése és a munkavállalók többletjuttatásainak növelése egyaránt érvényesítésre kerül a jól átgondolt béren kívüli juttatások rendszerében. Kutatásunkban azt a kérdést vizsgáljuk, milyen értékelési lehetőségek támogathatják a vállalkozások folyamatos működését számviteli és adózási területen, hogyan tudnak a gazdálkodók ezekkel az eszközökkel hatékonyan válságot kezelni. Tanulmányunkban kitérünk a koronavírus járvány miatt cafeteria rendszerben bekövetkezett komoly változásokra, azok hazai tendenciáira.

Abstract:
ABSTRACT Background: Work fatigue experienced by nurses can hurt the service provided by the hospital. The workload was difference in the capacity or ability of worker with job demand that must be faced. Interpersonal interaction is a relationship, communication, and interaction with other people. The purpose of this study was to determine relationship between workload and interpersonal interactions with subjective work fatigue in inpatient nurses at dr. M Yunus hospital, Bengkulu, Indonesia. Subjects and Method: This was a cross sectional conducted at dr. M Yunus Bengkulu from August to September, 2020. A sample of 128 nurses in the inpatient room of Dr. Hospital. M Yunus Bengkulu selected by total sampling. The dependent variable was subjective work fatigue. The independent variables were workload and interpersonal interactions. The instrument used was a questionnaire. The data was analyzed by chi square. Results: There was a statistically significant relationship between workload (OR= 9.31; 95% CI= 3.86 to 22.85; p< o.001) and interpersonal interactions (OR= 7.11; 95% CI= 3.05 up to 16.76; p< 0.001) with subjective work fatigue. Conclusion: Workload and interpersonal interactions are factors that can cause subjective work fatigue in the inpatient nurses at RSUD Dr. M Yunus Bengkulu. Abstract:
ABSTRACT Background: The management of job-related stress among health-care workers is critical for the improvement of healthcare services. This study was aimed to investigate determinants of job performance in medical rehabilitation health workers at Dr. Moewardi hospital, Central Java, using social cognitive theory. Subjects and Method: A cross sectional was carried out at Dr. Moewardi hospital, Surakarta, Central Java, May to June 2020. A sample of 200 medical rehabilitation health workers was selected by exhaustive sampling. The dependent variable was job performance. The independent variables were motivation, training, leadership style, incentive, and reinforcement. The data were collected by questionnaire and analyzed by a multiple logistic regression run on Stata 13. Results: Good job performance in medical rehabilitation health workers increased with high work motivation (b= 3.38; 95% CI= 1.67 to 6.91; p<0.001), had training (b= 1.86; 95% CI= 0.96 to 3.59; p=0.064), participative leadership (b= 2.24; 95% CI= 1.09 to 4.22; p= 0.025), high incentive (b= 3.19; 95% CI= 1.52 to 5.86; p<0.001), and strong reinforcement (b= 1.72; 95% CI= 0.91 to 4.16; p=0.086). Conclusion: Good job performance in medical rehabilitation health workers increases with high work motivation, had training, participative leadership, high incentive, and strong reinforcement. ABSTRACT Background: The patient’s statement of the nurse’s poor and needy performance reflects dissatisfaction. Communities need patient-focused services, and the success of nursing care is determined mainly by choice of nursing care delivery models. This study aimed to determine implementation of integrated nursing models toward patient satisfaction in Mitra Medika Amplas Hospital, Medan, North Sumatra. Subject dan Method: This was a quasi-experiment with two group comparison conducted at Mitra Medika Amplas Hospital, Medan, Indonesia. A sample of 77 patients was selected by consecutive sampling. The data were collected by questionnare and analyzed by t-test. Result: The results showed that the level of satisfaction in the intervention group stated that 60 people were satisfied (78%) while the control group stated that 4 people were satisfied (5%). It was statistically significant (p<0,05). Conclusion: The level of patient satisfaction based on the dimensions of tangible quality, empathy, reliability, responsiveness and assurance was higher in the intervention group than in the control group. ABSTRACT Background: Chronic obstructive pulmonary disease (COPD), a persistent airway obstruction, is caused by continuous exposure to harmful particles or irritants. Airway inflammation contributes the pathogenesis of COPD. Thymoquinone, has an anti-inflammatory effect. This study aimed to examine the effect of thymoquinone on interleukin-8 (IL-8) level, percentage of FEV1, and COPD assessment test score (CAT) in stable chronic obstructive lung disease. Subjects and Method: A quasi-experiment with pretest and posttest control group was conducted at the outpatient pulmonology unit, Dr. Moewardi, Surakarta, Central Java, from June to August 2019. A sample of 40 patients diagnosed with stable COPD was selected and allocated into two groups: (1) 20 patients treated with a combination of standard therapy and thymoquinone oil capsules 500 mg/ day; and (2) 20 patients treated with standard therapy. The dependent variables were IL-8 level, FEV1, and COPD assessment test score (CAT). The independent variables were treatment status. The data were analyzed by the paired-t-test. Results: The level of IL-8 was lower after treatment with combination of standard therapy and thymoquinone oil capsules 500 mg/ day (Mean= 14.35; SD= 3.95) than before treatment (Mean= 19.62; SD= 12.14), and it was not statistically significant (p= 0.052). The percentage of FEV1 was lower after treatment with combination of standard therapy and thymoquinone oil capsules 500 mg/ day (Mean= 42.36; SD= 14.43) than before treatment (Mean= 42.56; SD= 16.09), and it was not statistically significant (p= 0.943). The CAT score was lower after treatment with combination of standard therapy and thymoquinone oil capsules 500 mg/ day (Mean= 13.70; SD= 4.24) than before treatment (Mean= 13.70; SD= 4.24) than before treatment (Mean= 18.65; SD= 5.92), and it was statistically significant. Conclusion: Chronic obstructive pulmonary disease assessment test (CAT) score significantly decreases with the treatment combination of standard COPD therapy and thymoquinone oil capsules 500 mg/ day. ABSTRACT Background: In Indonesia, nutrition is still the 5 biggest problem for mothers and children. Nutrition in the mother during pregnancy will affect the nutrition the infant gets while in the womb. Nutrition in children is very important in the first 1000 days of life because it will affect the growth and development of the infant. One way to assess the nutritional adequacy of new born is by measuring the infant’s weight at birth. This study aimed to determine the relationship between maternal weight and birth weight. Subjects and Method: This was a cross sectional study conducted at Pajang Community Health Center, Surakarta, Central Java. The study subjects were all mothers and infants whose birth weight were measured during February to March 2020. The dependent variable in this study was maternal weight. The independent variable was birth weight. The data were taken from the medical records of the Pajang Community Health Center. The data were analyzed using logistic regression test Results: Maternal weight below the normal weight had tendency to produce babies with less weight than pregnant women with normal maternal weight (OR= 55.00; p<0.001), and it was statistically significant. ABSTRACT Background: The positive and negative symptoms of schizophrenia can cause anxiety symptoms. This anxiety makes people with schizophrenia tend to remain silent, avoid other, and ignore daily activities. Performing daily activities requires good motor and coordination skill. Structured performance exercise (gymnastics) can affect anxiety symptoms. This study aimed to determine low impact aerobic exercise activity on anxiety levels in schizophrenia patients at dr.rm soedjarwadi hospital, Klaten, Central Java. Subjects and Method: This was a pre-experimental one-group pretest-posttest design was conducted at Psychosocial Rehabilitation Unit of Dr.RM. Soedjarwadi Psychiatric Hospital, Klaten, Central java from January to February 2019. A sample of 21 people with an age range of 19-50-year schizophrenia patients who received a psychosocial rehabilitation selected by purposive sampling. The dependent variable was the level of anxiety. The independent variable was a low impact aerobic exercise. The data were collected by Hamilton Rating Scale for Anxiety (HARS). The data were analyzed by t-test. Results: The level of anxiety before the intervention (Mean = 27.52) was higher than after the intervention (Mean = 20.43), and it was statistically significant (p< 0.001). Conclusion: Low impact aerobic exercise activities lower the anxiety level of schizophrenia patients at RSJD dr. RM Soedjarwadi, Central Java Province. Schizophrenic patients are encouraged to participate in low impact aerobic exercise held in psychosocial rehabilitation installation. ABSTRACT Background: Maternal mortality in Indonesia is caused by multifactors that are both direct and indirect factors. Complications during pregnancy and after delivery, including preeclampsia is the direct cause of 90% of maternal deaths. This case report aimed to describe the anaesthesia management on the incidence of severe preeclampsia to prevent the complications. Subjects and Method: We reported a 33-year-old G3P2A0 woman with 33 weeks of gestational age, diagnosed with severe pre-eclampsia partial HELLP syndrome, fetal dis-tress, type II diabetes mellitus pro SCTP emergency with physical status ASA II. Regional anaesthesia with sub-arachnoid block was performed by using Lidodex 75 mg and fentanyl 25 mcg intrathecally. Results: From the operation process, a baby boy with birth weight 2.900 gram and APGAR Score 7-8-9 was born. Two-hour post operation examination on patient showed compos mentis (consciousness), blood pressure 121/ 80 mmHg, heart rate 64 bpm, respiration rate 20 breath per minute, blood oxygen saturation levels (SpO2) 99% with 3 L/min nasal cannula. Patient was administered to HCU post operation to be monitored vital sign and signs of impending eclampsia. Post-operative refeeding was performed after bowel sound was positive. Conclusion: Selection of appropriate anaesthetic management in severe preeclampsia cases can prevent complications. Viruses may be important in atherosclerosis as inciters of arterial injury or as modulators of the metabolism of the infected cells. Cytomegalovirus (CMV) is one of the members of the herpesvirus family ;that infect human beings. Virus was detected in arterial walls of patients with atherosclerosis (1). The effect of CMV on endothelial cells and on the metabolism of these cells was subject of this study.Human endothelial cells monolayers were established from cells obtained by collagenase treatment of human umbilical cord veins and arteries. The cells were grown in medium 199 containing growth factor and 20% fetal calf serum. All monolayers were near confluency at the time of viral infection. The cells were purified by FACS cell sorting using dilacLDL as marker (2). Infection of endothelial cells was performed using CMV laboratory strains ADI69 and Kerr and wild strains isolated from immunosuppressed patients (obtained by Dr. H.T. Wei land, Dept. Virology, University of Leiden, The Netherlands). Intracytoplasmic and intra-nuclear viral antigens were detected in 10-20% of the endothelial cells by fluorescence microscppy using TRITC-labeled monoclonal antibodies against CMV. Centrifugation of the cell monolayer during virus incubation resulted in an increase in the percentage of virus positive cells. The effect of virus infection on presence ot Tacxor VIII in the cells was studied using fluorescence microscopic examination of cell monolayers using a double staining technique. The presence of factor VIII in the cytoplasm of the cells was demonstrated using FITC-labeled antibodies and viral antigens were demonstrated by TRITC-labeled antibodies The results of these experiments showed that HCMV-infection of endothelial cells resulted in the disappearance of factor VIII from the cytoplasm of the endothelial cells at 48-72 hrs post infection without affecting the viability of the infected cells. ABSTRACT Background: Previous studies suggest that tobacco use affects systems of the human body involved in the reproductive process. Tobacco smoke exposure affects uterine receptivity, which may lead the risk of infertility. This study aimed to investigate the associations between cigarette smoke exposure, family history of infertility, and the risk of infertility among women in reproductive age. Subjects and Method: A case control study was conducted at obstetrics and gynecology polyclinic and Sekar fertility clinic, in Dr. Moewardi hospital, Surakarta, Central Java, from October to November 2019. A sample of 200 reproductive age women was selected by fixed disease sampling. The dependent variable was infertility. The independent variables were age, body mass index (BMI), physical activity, endometriosis, family history of infertility, smoking exposure, and stress. The data were obtained from medical record and questionnaire. The data were analyzed by a multiple linear regression. Results: The risk of infertility increased with age ≥35 years (b= 1.24; 95% CI= 0.18 to 2.30; p= 0.012), BMI <18 or ≥25 (b= 2.76; 95% CI= 1.74 to 3.76; p<0.001), high physical activity (b= 1.44; 95% CI= 0.38 to 2.51; p= 0.009), endometriosis (b= 1.06; 95% CI= 0.06 to 2.05; p= 0.038), tobacco smoke exposure (b= 1.37; 95% CI= 0.30 to 2.43; p= 0.012), and severe stress (b= 1.07; 95% CI= 0.16 to 2.00; p= 0.022). Conclusion: The risk of infertility increases with age ≥35 years, BMI <18 or ≥25, high physical activity, endometriosis, smoke exposure, and severe stress. ABSTRACT Background: Previous studies have shown that better glycemic control in type 2 diabetes mellitus (DM) is associated with fewer physical symptoms and better well being. The purpose of this study was to determine factors affecting the quality of life in patients with type 2 diabetes mellitus in Surakarta, Central Java. Subjects and Method: A cross-sectional study was conducted at Dr. Moewardi hospital, Surakarta, Central Java, from February to March 2020. A sample of 200 patients with type 2 DM aged ≥18 years was selected by simple random sampling. The dependent variable was quality of life. The independent variables were self-care, family support, peer support, length of disease, body mass index (BMI), and education. The data were collected by questionnaire and analyzed by a multiple logistic regression run on Stata 13. Results: Quality of life in type 2 DM patients increased with good self care (OR= 14.34; 95% CI= 1.47 to 140.06; p= 0.022), strong family support (OR= 21.52; 95% CI= 2.98 to 155.44; p=0.002), strong peer support (OR= 31.03; 95% CI= 3.14 to 306.78; p=0.003), length of disease <6 years (OR= 0.05; 95% CI= 0.01 to 0.45; p= 0.007), normal BMI (OR= 0.08; 95%= 0.01 to 0.60; p= 0.014), and education ≥Senior high school (OR= 11.76; 95% CI= 2.05 to 67.41; p= 0.006). Conclusion: Quality of life in type 2 DM patients increases with good self care, strong family support, strong peer support, length of disease <6 years, normal BMI, and education ≥Senior high school.
