ObjectivesThis study examines the use of expedited approval pathways by Health Canada over the period 1995 to 2016 inclusive and the relationship between the use of these pathways and the therapeutic gain offered by new products.DesignCross-sectional study.Data sourcesTherapeutic Products Directorate, Biologics and Genetic Therapies Directorate, Notice of Compliance database, Notice of Compliance with conditions web site, Patented Medicine Prices Review Board, La revue Prescrire, WHO Anatomical Therapeutic Chemical classification system.Primary and secondary outcomesPercent of new drugs evaluated by Health Canada that went through an expedited pathway between 1995 and 2016 inclusive. Kappa values comparing the review status with assessments of therapeutic value for individual drugs.ResultsOf 623 drugs approved by Health Canada between 1995 and 2016, 438 (70.3%) drugs went through the standard pathway and 185 (29.7%) an expedited pathway. Therapeutic evaluations were available for 509 drugs. Health Canada used an expedited approval pathway for 159 of the 509 drugs, whereas only 55 were judged to be therapeutically innovative. Forty-two of the 55 therapeutically innovative drugs received an expedited review and 13 received a standard review. The Kappa value for the entire period for all 509 drugs was 0.276 (95% CI 0.194 to 0.359) indicating ‘fair’ agreement between Health Canada’s use of expedited pathways and independent evaluations of therapeutic innovation.ConclusionHealth Canada’s use of expedited approvals was stable over the entire time period. It was unable to reliably predict which drugs will offer major therapeutic gains. The findings in this study should provoke a discussion about whether Health Canada should continue to use these pathways and if so how their use can be improved.

Abstract:
Herpes simplex virus (HSV) orolabial and anogenital infection causes substantial and recurring disease in healthy individuals due directly to infection of these sites and, indirectly, due to its complications. These complications include eczema herpeticum plus erythema multiforme and neonatal HSV infection, respectively. Four drugs: acyclovir, famciclovir, valacyclovir and penciclovir, are currently licensed by the Therapeutics Products Directorate of Health Canada for the management of HSV infections. Although these drugs are only approved for four orolabial and anogenital infections in healthy persons, their efficacy and safety for 13 other related uses in this population have been demonstrated in controlled clinical trials, so called off-label uses. In this review, the evidence supporting these 17 uses, the drugs and regimens evaluated, and their current costs, are described.

Abstract:
Research on reconstruction on the role of Tuan Guru (teacher) in the prevention and eradication of drugs in Lombok is field research using a qualitative research approach. This study aims to explore three critical issues. First, it analyzes the role of Tuan Guru in Lombok society. Second, it explains the reconstruction format of Tuan Guru as the efforts of preventing and eradicating drugs. Third, it describes the impact of the reconstruction role of Tuan Guru on drug prevention and eradication efforts. The findings elucidate that first: the roles of Tuan Guru in the Lombok Community were: 1) as an Ulama, 2) as a social problem controller or supervisor, 3) as an innovator of change towards betterness, 4) as a supporter of community independence. Second, the formats of reconstruction roles which were carried out in the efforts of preventing and eradicating drugs were as follows: 1) carry out a personal approach mechanism, 2) enforce a real action, 3) serve a counseling activity, 4) empower the community economy. Third, the roles played by Tuan Guru in preaching to deal with the drug problem were found to be very effective. Tuan Guru had succeeded in eliminating drug effects on the young generation. This could be proven as the number of drug cases decreases as reported by the drug investigation directorate of Lombok Barat.

Abstract:
To assess the completeness and quality of pharmaceutical care when dispensing OTC-drugs in pharmacies in the city of Olexandria, an anonymous survey of 55 pharmacists was conducted. The questionnaire developed on the example of the European Directorate for the Quality of Medicines and Healthcare of the Council of Europe included questions about the elements of its implementation (identifying threatening symptoms, informing about the storage of the preparation, possible side effects, etc.), as well as factors affecting the results and quality of care. Also, pharmacists were asked to indicate areas that need to be improved first. It was found that the level of pharmaceutical care in the dispensing of OTC-drugs was insufficient. This opinion was completely shared by the majority of pharmacists. Pharmaceutical care positions studied were provided in pharmacies in the range of 3685%. The lowest positions were occupied by information about the interaction of the preparation with other medicines (36%) and the medicine's compatibility with food and alcohol (39%). The most important aspect of improvement was indicated by individual patient counseling, noted by 64.4% of the surveyed pharmacists. Based on the results of the work, recommendations for improving pharmaceutical care were proposed.

ObjectiveWe assessed compliance with new guidelines for prophylactic antibiotics in hip fracture surgery in Norway introduced in 2013.DesignThe data from the Norwegian Hip Fracture Register was used to assess the proportion of antibiotics given according to the national guidelines.SettingAll hospitals in Norway performing hip fracture surgery in the period from 2011 to 2016.ParticipantsWe studied 13 329 hemiarthroplasties (HAs) for acute hip fracture.Main outcome measureType and timing between first and last dose of prophylactic antibiotics compared with the national guidelines.ResultsBefore the guidelines were introduced, the recommended drugs cephalotin or clindamycin was used in only 86.2% of all HAs. In 2016, one of the two recommended drugs was administered in 99.2% of HAs. However, hospitals’ adaption of the recommended administration of the two drugs improved slowly, and by the end of the study period, only three out of five HAs were performed with the correct drug administered in the correct manner. We found major differences in compliance between hospitals.ConclusionsThe change towards correct administration of antibiotic prophylaxis was varied both when investigating university and non-university hospitals. We suggest that both hospital leaders and the national Directorate of Health need to investigate routines for better dissemination of information and education to involved parties. Strong leadership concerning evidence-based guidelines on antibiotic prophylaxis in surgery may take away some autonomy from executing healthcare professionals, but will result in better patient care and antibiotic stewardship.

Antimicrobial resistance is a daunting public health threat affecting both human and animal health and it is a cause for concern wherever antimicrobial agents are in use. The usage of antimicrobial drugs in food producing animals could result in a significant food safety issue - antimicrobial resistance among zoonotic bacteria in these animals. The resistant bacteria may then be transmitted to humans through the food supply and increase risk of treatment failures. Campylobacter spp. and Salmonella spp. are two examples of food borne pathogens in which increasing resistance gives rise to concern. No monitoring program has been applied in Serbia. Therefore, the Veterinary Directorate of the Ministry of Agriculture financed three projects of the Scientific Veterinary Institute 'Novi Sad' to screen the spread of resistant food borne pathogens. The examinations have shown a similar situation as in the EU. Resistance to one or more antimicrobial drugs was found in 25-40.58% Salmonella spp. and 40-75.32% Campylobacter spp. isolates from poultry and pig carcasses. Zoonotic pathogens in food have to be controlled through a complete, continuous farm-to-fork system. Application of prophylactic and sanitary measures in farm breeding, slaughtering, food distribution and preparation of food in the kitchen may considerably reduce risk from the incidence of resistant zoonotic pathogens.

Aim. The aim of this study is to detect the prevalence of substance use among tenth-grade students; their thoughts, attitudes, behaviors, and tendencies towards substance use; and risk factors of substance use in tenth-grade students in general. Methods. This study is descriptive and cross-sectional conducted between April and May 2016. Research population consists of tenth-grade students in 2015-2016 school year in the city of Ordu. Since the study involved all tenth-grade students, no sampling was done. Questions on substance use were prepared by Ordu Public Health Directorate and the authors by making use of European School Survey Project on Alcohol and Other Drugs (ESPAD) study questions, AMATEM’s “Drugs and Addiction Youth Survey” study conducted on May 1996, and scientific studies conducted previously on similar subjects. Results. 9825 tenth-grade students in 88 schools from 19 counties in the city of Ordu were included in the study. 8714 of the students participated in the survey. Being male, being over the age of 15, mother and father being separated, living with relatives, being in low income, negative feelings about school, perception of being unsuccessful in school, failing a year, absenteeism, and not being content with life are the risk factors for substance use. Conclusions. The tendency of illegal substance use becoming more and more prevalent especially among youth requires the development of new treatment strategies.

Abstract This paper uses data collected for an MA Thesis on the challenges facing the Directorate of Criminal Investigation Department on management of criminal gang activities, in Nairobi County, Kenya. This study has been necessitated by continued concern among policymakers, security pundits and citizens about the rising criminal gang activities in the country, more pertinently in Nairobi County. Although there are few studies that tried to scratch this area, there has never been a detailed and systematic inquiry and analysis of this problem. The study was guided by several specific objectives; the first objective examined the tactical challenges that the police face in policing gang related activities in Nairobi County, Kenya; assessed the effect of corruption in undermining the war against gang related activities in Nairobi County, Kenya; and finally explored other technological challenges that the police face in policing gang related activities in Nairobi County, Kenya. The study adopted descriptive research design and purposive sampling technique to collect data. Interview schedules and key informants (K.I) guide were the main tools used to collect data using the interview method of data collection. 60 officers of the Special Crime Prevention Unit were the main respondents. Major challenges faced by the DCI officers when dealing with criminal gang activities especially within Nairobi County include; poor relationship between police and the general public as reported by 89.1% of the respondents, increased involvement of women in gang activities which is exacerbated by low ratio of female-male officers for undercover operations and 90.9%. of respondents indicated due to woman innocence will create a greater challenge. Others include; gang related activities with networks and markets especially for stolen vehicles across the borders as a result of corruption along the borders by both police, custom and military officers. Further, 85.5% indicated that cross border gang activities were worsened by lack of harmonized law for the regulation and prevention of these activities that led to enhanced proliferation of weapons as well as drugs to and from the neighboring countries. Major recommendations for policy considerations included; increased recruitment and deployment of female officers to beef up their ratio in undercover operations. Harmonized laws for the neighboring East African community countries and increased cooperation between the security organs of these countries in order to have a coordinated approach of tracking and apprehending criminals who escape to neighboring countries. The study also recommends mandatory training for DCI officers in computer/ technological skills to be able to intercept, interpret and or decode gang related communication and cybercrime related activities.

ABSTRACT Pharmacovigilance is the pharmacological science relating to the detection, assessment, understanding, and prevention of adverse effects, particularly the long-term and short-term adverse effects of drugs or treatment. The World Health Organization (WHO) established pharmacovigilance Programme for International Drug Monitoring in response to the thalidomide disaster detected in 1961. India joined the WHO adverse drug reaction (ADR) monitoring program based in Uppsala, Sweden, in 1998. The Central Drugs Standard Control Organization (CDSCO), Directorate General of Health Services, under the aegis of the Ministry of Health and Family Welfare, Government of India, in collaboration with Indian Pharmacopoeia Commission, Ghaziabad, is a National Coordinating Centre (NCC). Adverse drug reaction monitoring centers (AMCs) under the Pharmacovigilance Programme of India (PvPI) play a vital role in the collection and follow-up of ADR reports from the patients, as drug trials in animals and humans (Phase I–III) do not predict accurately the adverse drug events. Patients with chronic kidney disease (CKD)/liver disease are at risk of enhanced drug-related events. These patients should be closely monitored for any adverse events and it should be reported. Thus, inculcating the habit of ADR reporting will help in generating data specific to Indian population and will contribute toward patient safety. This will also help in modifying the treatment given to the patients, as early identification of ADRs will help in reducing morbidity and mortality in patients. This report is a summary of the activities developed during 9 months of curricular training at the Directorate of Risk Management for Medicines as part of the master degree in Pharmaceutical Medicine. The internship has the purpose of complement the theoretical knowledge previously acquired and was focused on the two areas of the Directorate: Safety Monitoring and Risk Minimization. The activities of the Directorate include the collection and validation of Individual Case Safety Report received from healthcare professionals, patients and pharmaceutical companies. The activities of Risk Minimization include the implementation of safety variations, management of periodic safety update report and risk management plan, validation of the direct healthcare professional communication and the evaluation of educational materials. The Directorate is also responsible to identify and manage safety signals.
Este relatório é o sumário das atividades realizadas durante 9 meses de estágio na Direção de Gestão do Risco de Medicamentos no âmbito do mestrado em Biomedicina Farmacêutica. O estágio teve o objetivo de complementar os conhecimentos teóricos adquiridos e contemplou as duas principais áreas de ação da Direção: a Monitorização de Segurança de Medicamentos de Uso Humano e a Minimização do Risco. De entre as atividades da Direção inclui-se a recolha e avaliação de notificações de reações adversas enviadas por profissionais de saúde, utentes e empresas farmacêuticas. As atividades de minimização do risco associado a medicamentos incluem atividades como as alterações de segurança, a gestão dos relatórios periódicos de segurança e dos planos de gestão do risco, a validação das comunicações dirigidas aos profissionais de saúde e a avaliação dos materiais educacionais. The City of Cape Town Municipality is the implementing agent of service delivery and developmental programmes. The importance and impact of the services and programmes are imperative to the citizens it serves; thus, the standard of work and efficiency provided by public officials is instrumental in achieving set priorities. Workplace substance abuse hampers service delivery and can cause damage to the employee, the public as well as the Municipality. Accidents, injuries or inability to perform functions by employees may have tremendous legal, financial and social repercussions for the City of Cape Town. This study seeks to evaluate efficacy of processes that seek to curb the existence of workplace substance abuse in the City of Cape Town, specifically within the Safety and Security Directorate. A qualitative and quantitative research methodology was applied. A combination of quantitative questionnaires, qualitative semi-structured interviews and focus group discussions were employed with employees in the Safety and Security Directorate. A purposeful sample was selected. The study results indicate that there is a prevalence of employees reporting for duty with a ‘hangover’. This is accepted as the norm, and staff are protected by their colleagues. The participating departments, Fire and Rescue Service and Metro Police, have highly stressful and traumatic working environments for staff to work in. There is a lack of debriefing and regular counselling after call-outs. Staff feel that management does not care and are tardy in providing support to them. A lack of trust between management and staff and among staff exists. The paper concludes with recommendations for each of the research findings.

The legislative deliberations of Plan Colombia and the Mérida Initiative also supported elite theories of the state and showed that corporate campaign contributions and the reciprocal relationships between corporate elites and the federal government also influence American counternarcotic aid flows. This chapter uses the Heckman selection estimator to ascertain whether these outcomes are also generalizable. First, it uses principal component factor analysis to create an index to operationalize C. Wright Mills’ concept of an interlocking directorate, which measures the interconnections among corporate board of directors for the corporations in the data set and their interconnections with policy think tanks and the U.S. government. The statistical findings provide evidence that corporate campaign contributions, corporate inter-locks with think tanks and the federal government, and an interlocking directorate systematically increased U.S. counternarcotic aid to eighty recipient countries. And since drug enforcement policy making toward Colombia and Mexico also demonstrated that congressional funding for the drug war is a source of corporate revenues, the chapter concludes by utilizes a time-series cross section statistical analysis that shows that increasing levels of counternarcotic aid flows increases corporate capital accumulation again confirming that the case study findings are generalizable.

Persisting worldwide, the COVID-19 pandemic results not only in major public health concerns, but also in several social and industrial issues in economy, labor, education and many other fields. In this context, the epidemic continues to take a negative hold. Due to, amongst others, the increasing and aging population, improper urbanization, irregularities in income distribution worldwide, increased resistance to antimicrobial agents, global warming and climate changes, intrusive approaches to wildlife, the epidemiology of infections is undergoing a worldwide change and new infectious diseases are emerging, some of which pose a threat of outbreak, even pandemic. Therefore, different policies should be adopted against outbreaks both on a national and global scale.An “Institute of Infectious Diseases and Epidemics” should be established very urgently within TUSEB (Directorate of Health Institutes of Turkey). Actions that attach importance to the understanding of “Singular Health” should be taken throughout the country. The efforts to take structural measures that will work in case of epidemics, to raise qualified manpower in medicine and to achieve the required capacity of vaccine and drug production along with rapid diagnosis and advanced analysis in epidemic situations should be maintained consistently. During the outbreak, further actions should be taken for potential issues in social life, economy, labor, education, agriculture and other fields. To achieve these goals, specific policies should be set, and an advanced cooperation should be established between the World Health Organization and other concerned international organizations by ensuring the integrated operation of institutions within the country.

Abstract:
The government of Iraq, through the Ministry of Science and Technology (MoST) is decommissioning Iraq’s former nuclear facilities. The 18 former facilities at the Al-Tuwaitha Nuclear Research Center near Baghdad include partially destroyed research reactors, a fuel fabrication facility and radioisotope production facilities. These 18 former facilities contain large numbers of silos and drums of uncharacterized radioactive waste and approximately 30 tanks that contain or did contain uncharacterized liquid radioactive wastes. Other key sites outside of Al Tuwaitha include facilities at Jesira (uranium processing and waste storage facility), Rashdiya (centrifuge facility) and Tarmiya (enrichment plant). The newly created Radioactive Waste Treatment Management Directorate (RWTMD) within MoST is responsible for Iraq’s centralized management of radioactive waste, including safe and secure disposal. In addition to being responsible for the uncharacterized wastes at Al Tuwaitha, the RWTMD will be responsible for future decommissioning wastes, approximately 900 disused sealed radioactive sources, and unknown quantities of NORM wastes from oil production in Iraq. This paper presents the challenges and progress that the RWTMD has made in setting-up a radioactive waste management program. The progress includes the establishment of a staffing structure, staff, participation in international training, rehabilitation of portions of the former Radioactive Waste Treatment Station at Al-Tuwaitha and the acquisition of equipment.
