Abstract:
Abstract Background To standardize outcome reporting in esophageal surgery, the Esophageal Complications Consensus Group (ECCG) developed a standardized platform. Recently, this group published outcomes of 2704 patients that underwent an esophagectomyin 24 high-volume hospitals in the period 2015–2016. The aim of this study was to report postoperative morbidity and mortality in the Netherlands using the definitions of the ECCG. Methods All patients who underwent esophagectomy for cancer of the esophagus or esophagogastric junction in the Netherlands in 2016 were selected from the Dutch Upper gastrointestinal Cancer Audit (DUCA). Patient outcomes including postoperative complications, 30-day/in-hospital mortality readmission rate were reported according to the definitions of the ECCG platform (Low et al. Ann Surg 2017). Outcomes of the DUCA were compared with the recently published outcomes of the ECCG with Chi-square analysis. Results Some 797 patients were included from 22 hospitals. In 1 patient, the postoperative outcome was unkown. In 17 patients readmission status was unknown. Some 168 (21%) patients had an ASA score of ≥ III and 250 (31%) patients a Charlson comorbidity score of ≥ 2. In total, 498 patients (63%) had at least one postoperative complication (versus ECCG: 57%, P = 0.07). The most common complications were pneumonia (21% DUCA versus 15% ECCG, P < 0.01), anastomotic/staple-line failure or localized conduit necrosis (18% DUCA versus 11% ECCG, P < 0.01) and atrial dysrhythmia requiring treatment (13% DUCA versus 15% ECCG, P = 0.28). Readmissions occurred in 105 of 780 patients (13% DUCA versus 11% ECCG, P = 0.13). The 30-day/in-hospital mortality was 2.5% for the DUCA group and 2.4% for the ECCG group (P = 0.88). Conclusion The registration of complications according to the ECCG platform in the national audit promotes the use of uniform definitions and allows international comparison of outcomes. The overall complication rate, readmission rate and mortality in the Netherlands were comparable with the outcomes of the ECGG. However, anastomotic leakage and pneumonia were more frequently reported in the Netherlands. Disclosure All authors have declared no conflicts of interest.

Abstract:
309 Background: In 2011, the Dutch Upper GI Cancer Audit (DUCA) group started with a nationwide registration of all patients who underwent surgery for esophageal or gastric cancer. The aim of this study was to describe the initiation and implementation of the DUCA and to provide an overview of the results. Methods: The DUCA is part of the Dutch Institute for Clinical Auditing. It provides (surgical) teams with reliable, weekly updated, benchmarked information on process and (casemix-adjusted) outcome measures. A web-based registration was designed, based on a set of predefined quality measures. Results: Between 2011 and 2014, a total of 4672 patients with esophageal or gastric cancer was registered in the DUCA. Case ascertainment has approached 100% for patients registered in 2014. The percentage of patients with esophageal cancer starting treatment within 5 weeks after diagnosis significantly increased over time (33 to 41%) and the percentage of patients with a minimum of 15 lymph nodes in the resected specimen significantly increased for both esophageal cancer (50 to 73%) and gastric cancer (48 to 74%). Postoperative mortality decreased for patients with gastric cancer (8.0% in 2011 to 4.0% in 2014; p = 0.020) and remained stable (around 4%) for patients with esophageal cancer. Conclusions: Nationwide implementation of the DUCA has been successful. Results give a valuable insight in the quality of the surgical care for patients with esophageal or gastric cancer and show a positive trend for various process and outcome measures.

Abstract:
PURPOSE The aim of this study was to examine the external validity of the randomized TIME trial, when minimally invasive esophagectomy (MIE) was implemented nationally in the Netherlands, using data from the Dutch Upper GI Cancer Audit (DUCA) for transthoracic esophagectomy. METHODS Original patient data from the TIME trial were extracted along with data from the DUCA dataset (2011-2017). Multivariate analysis, with adjustment for patient factors, tumor factors, and year of surgery, was performed for the effect of MIE versus open esophagectomy on clinical outcomes. RESULTS One hundred fifteen patients from the TIME trial (59 MIE v 56 open) and 4,605 patients from the DUCA dataset (2,652 MIE v 1,953 open) were included. In the TIME trial, univariate analysis showed that MIE reduced pulmonary complications and length of hospital stay. On the contrary, in the DUCA dataset, MIE was associated with increased total and pulmonary complications and reoperations; however, benefits included increased proportion of R0 margin and lymph nodes harvested, and reduced 30-day mortality. Multivariate analysis from the TIME trial showed that MIE reduced pulmonary complications (odds ratio [OR], 0.19; 95% CI, 0.06 to 0.61). In the DUCA dataset, MIE was associated with increased total complications (OR, 1.36; 95% CI, 1.19 to 1.57), pulmonary complications (OR, 1.50; 95% CI, 1.29 to 1.74), reoperations (OR, 1.74; 95% CI, 1.42 to 2.14), and length of hospital stay. Multivariate analysis of the combined and MIE datasets showed that inclusion in the TIME trial was associated with a reduction in reoperations, Clavien-Dindo grade > 1 complications, and length of hospital stay. CONCLUSION When adopted nationally outside the TIME trial, MIE was associated with an increase in total and pulmonary complications and reoperation rate. This may reflect nonexpert surgeons outside of high-volume centers performing this minimally invasive technique in a nonstandardized fashion outside of a controlled environment.

Tra il XIX ed il XX secolo, sono stati effettuati diversi studi riguardo al patronato letterario di Isabella d’Este e al mecenatismo praticato verso gli artisti da suo figlio, Federico II Gonzaga, primo duca di Mantova. Una nuova prospettiva di ricerca induce oggi a indagare il ruolo di Federico II come committente letterario, distinto dalla Marchesana Isabella, e i suoi interessi letterari. Vari autori, più o meno celebri, risultano avere avuto relazioni con Federico II Gonzaga, in quanto o gli hanno dedicato le loro opere o lo hanno citato in esse oppure perché è stato loro richiesto di comporre testi dal duca stesso. Molti di questi lavori sono registrati nell’inventario di Federico II, che ci dà alcune idee circa le sue preferenze, rivolte soprattutto alle opere astrologiche e ai testi cavallereschi, e circa le sue relazioni letterarie. Gli scrittori legati a Federico II hanno spesso cercato di procurare vantaggi tramite l’attività letteraria a se stessi e al loro mecenate. Si può ritenere che Federico II fosse inserito in una più ampia rete di relazioni letterarie sviluppatesi intorno alla corte di Mantova e che abbia impiegato la propria cultura letteraria e le opere letterarie come uno strumento di potere.
Between the 19th and the 20th century several studies were carried out with regard to the literary patronage of Isabella d’Este and in connection with the maecenatism practiced towards artists by her son Federico II Gonzaga, first Duke of Mantua. Nowadays, a new perspective of research induces to investigate the role of Federico II as literary client, separated from the Marchesana Isabella, as well as his literary interests. Various more or less famous authors have been found to have had some relationships with Federico II Gonzaga, either because they mentioned him in their works and dedicated them to him, or because they were requested to compose texts by the duke himself. Many of these works are registered in Federico II’s inventory, which gives us some ideas about his literary preferences, particularly directed to astrological works and chivalric texts, and his literary relationships. The writers linked to Federico II often tried to obtain some advantages for themselves and their patron through their literary activity. It can also be maintained that there was a wider network of literary relationship around the court of Mantua, and that Federico II used his literary culture and the literary works as a tool of power.

La ricerca che si presenta in questa tesi, svolta in cotutela tra l’Università degli Studi di Napoli “Federico II” e l’Universitat de Barcelona, ha avuto come obiettivo l’analisi delle vicende biografiche e collezionistiche di Ramiro Felípez Núñez de Guzmán, II duca di Medina de las Torres e viceré a Napoli dal 1637 al 1644. Per dare organicità e consequenzialità allo studio, nella stesura della tesi si è deciso di seguire il percorso biografico di Ramiro Núñez de Guzmán nelle sue tappe principali. Nella prima parte sono stati affrontati gli inizi della sua carriera politica, per la quale giocò un ruolo determinante il matrimonio con la figlia del conte duca di Olivares, celebrato nel 1626. Ciò ha necessariamente implicato una riflessione sulle dinamiche della corte spagnola, specie in riferimento all’assegnazione degli incarichi diplomatici e di palazzo. Nei capitoli successivi si è scelto di focalizzare l’attenzione sugli anni in cui il Guzmán fu inviato a Napoli come viceré (1637-1644), e sulle implicazioni politiche del matrimonio con la principessa Anna Carafa. Considerata l’influenza che questa unione ebbe anche per l’affacciarsi di interessi artistici nel duca, si è dedicato ampio spazio alle novità più interessanti emerse in tal senso dalle indagini archivistiche. Lo studio degli inventari di famiglia ha consentito un affondo sulla ricchissima quadreria e sulla rilevante raccolta di arazzi e panni ricamati che, stando a quanto riportato dalle fonti, godeva di particolare ammirazione presso i contemporanei per l’ampiezza e il pregio. Gli ultimi due capitoli affrontano il rientro a Madrid al termine del mandato vicereale e il ritorno agli incarichi di palazzo e di corte, dove nel frattempo si erano affermate nuove personalità, come quella di Luis de Haro, che ne avrebbero ostacolato l’ascesa a primo ministro. Oltre all’ambizioso compito di sumiller de corps del monarca, il duca di Medina riprese a svolgere l’impegnativo ruolo di decano al Consiglio di Stato, in un momento in cui la monarchia spagnola, dopo l’uscita di scena del conte duca di Olivares, cominciava il suo lento processo di declino politico in ambito europeo. Questi anni coincisero infatti con una intensa attività diplomatica per il Guzmán, chiamato a partecipare alle negoziazioni con le delegazioni olandesi, francesi e portoghesi per la firma, rispettivamente, del trattato di Münster, della Pace dei Pirenei e di quella di Lisbona. L’attitudine collezionistica del Guzmán sembra in questa fase subire una forte battuta d’arresto, in quanto i nuclei di dipinti, arazzi e oggetti preziosi cominciarono a disperdersi tra la nuova residenza madrilena, quella del figlio Nicola (che in un secondo momento lo seguì a corte) e i reales sitios di Filippo IV, dove giunsero in forma di dono. Alla morte del duca di Medina de las Torres, avvenuta alla fine del 1668, ciò che restava delle raccolte di dipinti e oggetti d’arte venne inventariato e messo in vendita, andando incontro all’oblio; tuttavia, nel frattempo una cospicua parte era passata al figlio Nicola, che già precedentemente era stato nominato erede universale del patrimonio materno. L’indagine si è a questo punto dirottata sulle vicende collezionistiche del primogenito, svelando alcune inaspettate sorprese. Di particolare interesse si è rivelata ad esempio la messa a punto della sua attività di donatore e patrocinatore nei confronti del convento madrileno di Santa Teresa de Jesús, oltre che la ricostruzione dei rapporti che continuò a mantenere con l’ambiente artistico napoletano dopo il trasferimento in Spagna, com’è testimoniato da alcuni documenti inediti che si presentano in questa occasione.
This thesis, carried out both at the University of Naples “Federico II” and the University of Barcelona, aims to analyse some biographical events and the artistic collection of Ramiro Felípez Núñez de Guzmán (1600-1668), Duke of Medina de las Torres. In the drafting of this thesis I decided to follow the sequence of biographical events of Ramiro de Guzmán. In the first chapter I analyzed his early political career, in which his marriage to the daughter of the Count Duke of Olivares (1626) played a decisive role. This necessarily had to reflect upon the dynamics of the Spanish court, especially regarding the allocation of diplomatic posts and the palace. In the subsequent chapters I focused on the years when Guzmán was sent as viceroy to Naples (1637-1644), and on the political implications of his marriage to Princess Anna Carafa. Thanks to this union, an incredible collection of paintings, hangings and tapestries began, which enjoyed particular admiration among his contemporaries. The last two chapters deal with the return to Madrid in 1644, when, in addition to the ambitious task of Sumiller de corps of the monarch, the Duke of Medina began to act as minister of the Council of State. In fact, these years were ones of intense diplomatic activity for Guzmán, who participated in the negotiations with the Dutch, French and Portuguese delegations for the signature of the Treaty of Münster, the Peace of the Pyrenees and the Treaty of Lisbon. At the death of the Duke of Medina de las Torres, at the end of 1668, what remained of the collection of paintings and art objects was inventoried and put on sale, but in the meantime a large part had passed on to his son Nicholas. Of particular interest were, for instance, also his activities as donor and patron of the Madrid convent of Santa Teresa de Jesús, as well as the recreation of the relations he kept with the Neapolitan artistic milieu after moving back to Spain, as witnessed by some unpublished documents that I am herein introducing.
La investigación que se presenta en esta tesis, hecha en cotutela entre la Universidad de Nápoles “Federico II” y la Universitat de Barcelona, tuvo como objetivo el análisis de los acontecimientos biográficos y también de la colección de arte particular de Ramiro Felípez Núñez de Guzmán, duque de Medina de las Torres y virrey de Nápoles entre el 1637 y el 1644. Para dar organicidad al estudio, en la redacción de la tesis se decidió seguir el orden cronológico de Ramiro Núñez de Guzmán, analizando las principales etapas de su vida. La primera parte trata del comienzo de su carrera política, en la que jugó un papel decisivo el casamiento con la hija del Conde Duque de Olivares, celebrado en el 1626. Esto ha necesariamente implicado una reflexión sobre la dinámica de la corte de Felipe IV, sobre todo en relación con la asignación de títulos y encargos de palacio. En los capítulos dos y tres se ha tratado de los años napolitanos del Guzmán (1637-1644), de las implicaciones políticas de matrimonio con la princesa Anna Carafa. Dada la influencia que esta unión tuvo también en el desarrollo de los intereses artísticos en el duque, se ha dedicado amplio espacio a las noticias más interesantes surgidas de la investigación de archivo. El estudio de los inventarios de la familia ha permitido profundizar sobre la colección, que constaba de pinturas y tapices de telas bordadas, que, según los informes de las fuentes, generaron particular admiración entre sus contemporáneos por amplitud y calidad. Los dos últimos capítulos tratan de la vuelta a Madrid, después del mandato virreinal, cuando el duque volvió a trabajar en la corte, a donde, mientras tanto, se habían afirmado nuevos personajes, como Luis de Haro, que contrastaron su subida como primer ministro. Además de la ambiciosa tarea de sumiller de corps del monarca, el duque de Medina comenzó a desempeñar el difícil papel de decano del Consejo de Estado, en un momento en que la monarquía española, después de la salida del Conde Duque de Olivares, había empezado el lento proceso de decadencia política en Europa. De hecho, estos años coincidieron con una intensa actividad diplomática para el Guzmán, llamado a participar en las negociaciones con las delegaciones de los holandeses, franceses y portugueses para la firma, respectivamente, del Tratado de Münster, de la Paz de los Pirineos y del Tratado de Lisboa. A la muerte del duque de Medina de las Torres a finales de 1668, lo que quedaba de la colección de pinturas y objetos de arte fue puesto a la venta. La investigación se centra pues al desarrollo de esta figura en relación con las donaciones al convento de Santa Teresa de Jesús, así como la reconstrucción de las relaciones que continuó manteniendo con Nápoles, tal como refieren unos inéditos documentan que se presentan por primera vez en esta ocasión.

Abstract:
Junto al poema en prosa, el género poético que mejor representa la modernidad literaria y el pensamiento contemporáneo (fragmentario, caótico y sumido en su propia crisis de identidad) es el poema extenso. El presente proyecto de investigación pretende definir los rasgos ontológicos y aspectos semánticos de esta modalidad genérica, demostrando –frente al poema largo tradicional– su componente lírico, criterios diferenciadores y capacidades expresivas propias. Además de una propuesta de clasificación y un canon del poema extenso occidental, con este trabajo se intenta demostrar cómo su extensión responde a necesidades literarias tales como la densidad temática y la complejidad estructural de la obra. Tras la formulación genérica, en la segunda parte de proyecto se estudia la articulación de este modo expresivo en tres textos poéticos paradigmáticos del ámbito hispánico contemporáneo: Espacio de Juan Ramón Jiménez; Metropolitano de Carlos Barral; y El libro, tras la duna de Andrés Sánchez Robayna.
The poetic genre that, together with the prose poem, best represents the literary modernity and the contemporary thought (fragmentary, chaotic and lost in its own identity crisis) is the long poem. The first section of this doctoral dissertation aims to define the ontological features and semantic aspects of this genre and –opposite to the traditional long poem– to show its lyricism, distinguishing criteria and its own expressive abilities. In addition to a proposal of classification and canon of the western long poem, this research aims to prove that the length of this expressive mode meets literary needs such as the thematic density, the structural complexity or its planning. After the generic formulation, the second section analyses three paradigmatic long poems of the literary Spanish context: Espacio of Juan Ramón Jiménez; Metropolitano of Carlos Barral; and El libro, tras la duna of Andrés Sánchez Robayna.

Abstract:
The in-duct attenuation performance, or insertion loss, of a range of absorbers was measured, and guidelines for the design and application of absorbers in air ducts were developed from the measured data. A test facility that met the requirements of ISO 7235 was designed and constructed. Insertion loss measurements were made using the test facility to investigate the effects of absorbent material and absorbent thickness, facing type, airflow conditions, and fixing method It was found that absorbent flow resistance was the most influential factor in determining the insertion loss of an absorber. The insertion loss of wall absorbers was significantly modified by changes in absorber thickness, which was attributed to the consequential change in flow resistance. Similarly, the insertion loss performance of bar absorbers with significantly varying thickness, and therefore varying flow resistance, was greater than that of bar absorbers with approximately constant thickness. The insertion loss of wall absorbers was very sensitive to impedance conditions at the boundary between the rear surface of the absorber and the duct wall. Insertion loss performance was improved by applying thin facings to the exposed surface of wall absorbers. Airflow velocity did not affect insertion loss over airflow velocities typical of building services ducts.

Abstract:
Contexto. Os baixos índices de qualidade de ensino no Brasil demonstram os prejuízos advindos de um setor formado por complexas camadas administrativas e baixa transparência. Objetivo. Por meio de um aplicativo cujo nome carinhoso também é o seu mascote, a Duca, facilitar a cooperação e o controle social da população, bem como direcionar as ações por parte dos órgãos de controle, justiça e poder executivo. Método. Além da construção ágil do aplicativo, um estudo de caso avaliativo foi realizado com uma escola pública estadual, no qual professores, alunos e pais sugeriram correções e melhorias. Resultados. O aplicativo, como ferramenta de colaboração participativa, promove o engajamento da comunidade escolar, mudando a vida desta comunidade com aspectos inovadores, entre os quais se destacaram as possibilidades de visualização do cardápio real da merenda e de aplicações de surveys dinâmicos. Conclusão. Aproximando a sociedade civil com órgãos diretores, a aplicação eleva a governança pública em níveis econômico, social e acadêmico, sob a perspectiva de um planejamento mais eficiente e eficaz.
