Abstract:
Abstract Background To standardize outcome reporting in esophageal surgery, the Esophageal Complications Consensus Group (ECCG) developed a standardized platform. Recently, this group published outcomes of 2704 patients that underwent an esophagectomyin 24 high-volume hospitals in the period 2015–2016. The aim of this study was to report postoperative morbidity and mortality in the Netherlands using the definitions of the ECCG. Methods All patients who underwent esophagectomy for cancer of the esophagus or esophagogastric junction in the Netherlands in 2016 were selected from the Dutch Upper gastrointestinal Cancer Audit (DUCA). Patient outcomes including postoperative complications, 30-day/in-hospital mortality readmission rate were reported according to the definitions of the ECCG platform (Low et al. Ann Surg 2017). Outcomes of the DUCA were compared with the recently published outcomes of the ECCG with Chi-square analysis. Results Some 797 patients were included from 22 hospitals. In 1 patient, the postoperative outcome was unkown. In 17 patients readmission status was unknown. Some 168 (21%) patients had an ASA score of ≥ III and 250 (31%) patients a Charlson comorbidity score of ≥ 2. In total, 498 patients (63%) had at least one postoperative complication (versus ECCG: 57%, P = 0.07). The most common complications were pneumonia (21% DUCA versus 15% ECCG, P < 0.01), anastomotic/staple-line failure or localized conduit necrosis (18% DUCA versus 11% ECCG, P < 0.01) and atrial dysrhythmia requiring treatment (13% DUCA versus 15% ECCG, P = 0.28). Readmissions occurred in 105 of 780 patients (13% DUCA versus 11% ECCG, P = 0.13). The 30-day/in-hospital mortality was 2.5% for the DUCA group and 2.4% for the ECCG group (P = 0.88). Conclusion The registration of complications according to the ECCG platform in the national audit promotes the use of uniform definitions and allows international comparison of outcomes. The overall complication rate, readmission rate and mortality in the Netherlands were comparable with the outcomes of the ECGG. However, anastomotic leakage and pneumonia were more frequently reported in the Netherlands. Disclosure All authors have declared no conflicts of interest.

309 Background: In 2011, the Dutch Upper GI Cancer Audit (DUCA) group started with a nationwide registration of all patients who underwent surgery for esophageal or gastric cancer. The aim of this study was to describe the initiation and implementation of the DUCA and to provide an overview of the results. Methods: The DUCA is part of the Dutch Institute for Clinical Auditing. It provides (surgical) teams with reliable, weekly updated, benchmarked information on process and (casemix-adjusted) outcome measures. A web-based registration was designed, based on a set of predefined quality measures. Results: Between 2011 and 2014, a total of 4672 patients with esophageal or gastric cancer was registered in the DUCA. Case ascertainment has approached 100% for patients registered in 2014. The percentage of patients with esophageal cancer starting treatment within 5 weeks after diagnosis significantly increased over time (33 to 41%) and the percentage of patients with a minimum of 15 lymph nodes in the resected specimen significantly increased for both esophageal cancer (50 to 73%) and gastric cancer (48 to 74%). Postoperative mortality decreased for patients with gastric cancer (8.0% in 2011 to 4.0% in 2014; p = 0.020) and remained stable (around 4%) for patients with esophageal cancer. Conclusions: Nationwide implementation of the DUCA has been successful. Results give a valuable insight in the quality of the surgical care for patients with esophageal or gastric cancer and show a positive trend for various process and outcome measures.

PURPOSE The aim of this study was to examine the external validity of the randomized TIME trial, when minimally invasive esophagectomy (MIE) was implemented nationally in the Netherlands, using data from the Dutch Upper GI Cancer Audit (DUCA) for transthoracic esophagectomy. METHODS Original patient data from the TIME trial were extracted along with data from the DUCA dataset (2011-2017). Multivariate analysis, with adjustment for patient factors, tumor factors, and year of surgery, was performed for the effect of MIE versus open esophagectomy on clinical outcomes. RESULTS One hundred fifteen patients from the TIME trial (59 MIE v 56 open) and 4,605 patients from the DUCA dataset (2,652 MIE v 1,953 open) were included. In the TIME trial, univariate analysis showed that MIE reduced pulmonary complications and length of hospital stay. On the contrary, in the DUCA dataset, MIE was associated with increased total and pulmonary complications and reoperations; however, benefits included increased proportion of R0 margin and lymph nodes harvested, and reduced 30-day mortality. Multivariate analysis from the TIME trial showed that MIE reduced pulmonary complications (odds ratio [OR], 0.19; 95% CI, 0.06 to 0.61). In the DUCA dataset, MIE was associated with increased total complications (OR, 1.36; 95% CI, 1.19 to 1.57), pulmonary complications (OR, 1.50; 95% CI, 1.29 to 1.74), reoperations (OR, 1.74; 95% CI, 1.42 to 2.14), and length of hospital stay. Multivariate analysis of the combined and MIE datasets showed that inclusion in the TIME trial was associated with a reduction in reoperations, Clavien-Dindo grade > 1 complications, and length of hospital stay. CONCLUSION When adopted nationally outside the TIME trial, MIE was associated with an increase in total and pulmonary complications and reoperation rate. This may reflect nonexpert surgeons outside of high-volume centers performing this minimally invasive technique in a nonstandardized fashion outside of a controlled environment.

Un architetto portoghese, Damiano di Roan, fu inviato a Parma da Don Duarte alla sorella Maria che era sposata al principe Alessandro Farnese affinché potesse progredire nell'arte delle fortificazioni. Fu inviato ad Ancona presso Francesco Paciotto e successivamente a Torino presso Ferrante Vitelli che lavorava alla Cittadella. Nel 1575 in seguito ad un litigio, in cui intervenne anche il duca Emanuele Filiberto di Savoia, venne rinviato a Parma. I principi Farnese non lo vollero trattenere e decisero di mandarlo a Roma. Non si conosce la sorte dell'architetto, che è del tutto ignorato dai documenti degli archivi portoghesi.

ABSTRACTComposer Pascal Dusapin has crafted an intriguing and idiosyncratic musical style that is both expressive and rigorously formal. Born in 1955 in Nancy, France, he studied piano and organ in his youth and later Plastic Arts and Sciences, Arts, and Aesthetic at the University of Paris 1 Panthéon-Sorbonne, notably under composer Iannis Xenakis. From 1981 to 1983, he held a resident scholarship from the Villa Medici in Rome. Among the numerous accolades Dusapin has received are the Cino del Duca Prize in 2005 and the Dan David Prize in 2007. The Collège de France conferred upon him a professorship to hold the Chair of Artistic Creation from 2006 to 2007. He is a Commander of the French Order of Arts and Letters. His body of work includes extensive explorations of solo, chamber, orchestral, and operatic forces. What follows is an edited translation of an interview I made with Dusapin in his studio in Paris on 9 July 2016.

Este trabalho tem como intuito refletir sobre uma modalidade narrativa amplamente difundida na contemporaneidade, a autobiografia. Interessa-nos, neste sentido, sua vertente literária, as autobiografias que galgam o status de arte. Claro está que não se trata de um gênero estanque ou limitado às formas convencionais, sendo que um de nossos propósitos é verificar as soluções estéticas de renovação deste objeto. Como corpus de análise, elegemos o romance A Favor do Vento, de Duca Leindecker, músico com larga e bem sucedida carreira, principalmente como líder da banda Cidadão Quem, e escritor de três livros, com boa aceitação do público leitor. Julgamos que A Favor do Vento configura-se como uma autobiografia não convencional, pelo uso de artifícios que manejam constantemente a ação de cifrar e decifrar, dando ao leitor relevante tarefa na efetivação do texto literário. A avaliação das possibilidades estéticas utilizadas nas narrativas em que autor e personagem compartilham o mesmo contexto axiológico é, sem dúvida, uma reflexão pungente na contemporaneidade, e nosso propósito aqui é modestamente contribuir na construção deste panorama tão vário.

Nelle opere di Giorgio Scerbanenco il giallo ha quasi sempre avuto un ruolo prominente che tuttavia ha lasciato spazio a una vasta possibilità di intrecci con forme stilistiche, tematiche e narrative di varia natura. Uno dei principali riconoscimenti che fanno di Scerbanenco uno dei padri fondatori del giallo italiano è l’aver saputo introdurre in modo originale e credibile un elemento contaminante come l’hard-boiled americano nel contesto del poliziesco italiano. Questo ha permesso di porre l’attenzione sull’ambiente sociale e sulla psicologia dei personaggi, che si riflettono, da un lato, nella rilevanza dei luoghi e dell’inquadramento storico-sociale in cui sono ambientate le trame, dall’altro, nella meticolosa opera di caratterizzazione dei personaggi dei quattro romanzi della serie Duca Lamberti. Lo scopo di questo articolo è quello di mostrare come l’esperienza dello scrittore italo-ucraino in qualità di direttore e redattore di diverse riviste femminili del dopoguerra abbia ricoperto un ruolo fondamentale nella caratterizzazione di alcuni personaggi, in particolare quelli femminili, e nella descrizione della loro condizione sociale in quel determinato periodo storico.

Resumo Os estudos de ergonomia visam adequar os ambientes às atividades desenvolvidas e aos anseios dos usuários. Historicamente, os estudos da ergonomia aplicada ao ambiente construído registram o foco direcionado às análises de situações existentes. Entretanto, recentes pesquisas vêm incorporando princípios de ergonomia no fazer projetual, como estratégia de antecipação de problemas, e permitindo a participação dos usuários nos processos decisórios. Este artigo aborda o processo de sistematização de etapas projetuais do ambiente construído, baseado em princípios ergonômicos. O objetivo é demonstrar como métodos de projeto podem ser enriquecidos, a fim de inserir os usuários finais em etapas projetuais, atendendo-os em suas necessidades e anseios. A metodologia utilizada é baseada em design science research (DRESCH; LACERDA; ANTUNES JUNIOR, 2015) e em duas abordagens ergonômicas: a metodologia ergonômica para o ambiente construído (VILLAROUCO, 2009, 2011) e as etapas de projetação ergonômicas de Attaianese e Duca (2012). São apresentadas abordagens teóricas e projetuais, parâmetros projetuais de ergonomia aplicados ao ambiente construído. Os resultados apontaram para uma forma de pensar o projeto baseado na compreensão de problemas pré-existentes, na participação ativa dos usuários e na adequação dos ambientes às atividades que nele serão desenvolvidas.

Abstract:
In the Cultural Heritage (CH) context, art galleries and museums employ technology devices to enhance and personalise the museum visit experience. However, the most challenging aspect is to determine what the visitor is interested in. In this work, a novel Visual Attentive Model (VAM) has been proposed that is learned from eye tracking data. In particular, eye-tracking data of adults and children observing five paintings with similar characteristics have been collected. The images are selected by CH experts and are—the three “Ideal Cities” (Urbino, Baltimore and Berlin), the Inlaid chest in the National Gallery of Marche and Wooden panel in the “Studiolo del Duca” with Marche view. These pictures have been recognized by experts as having analogous features thus providing coherent visual stimuli. Our proposed method combines a new coordinates representation from eye sequences by using Geometric Algebra with a deep learning model for automated recognition (to identify, differentiate, or authenticate individuals) of people by the attention focus of distinctive eye movement patterns. I suoi progetti editoriali sia filologici che storici spesso necessitavano, nel contesto delle procedure di censura interne all’ordine, dell’approvazione da parte della curia generalizia. Da cio derivava un regolare scambio epistolare con la Citta eterna che apriva a Rader l’accesso a manoscritti rari, conservati a Roma, e lo metteva in contatto con i bibliotecari locali. Tali rapporti facevano di lui un importante interlocutore per viaggiatori diretti da Roma verso la Baviera, ad esempio Leone Allacci, il quale nel 1623 avrebbe dovuto trasferire i fondi della Palatina in Italia. La posizione di Rader come storiografo ufficiale della corte bavarese comportava che egli propagasse a Roma la visione storica del suo committente, il duca Massimiliano I. Riassunto Dopo la Riforma, Baviera divenne il primo interlocutore tedesco della Curia romana a livello religioso, politico e culturale. Le relazioni con la Santa Sede erano ispirate dalla cattolicità programmatica dei Wittelsbach. La special relationship si manifestava, inoltre, in una lunga tradizione diplomatica inaugurata all’inizio del Seicento su spinta del papato. Durante la Guerra dei Trent’anni gli intensi contatti si estesero anche al campo militare. La Curia romana vide nel duca Massimiliano l’incontestata guida dell’armata cattolica, definendola la „colonna della religione cattolica“ nel Sacro Impero Romano. Promuovere gli interessi del principe tedesco (sussidi per il finanziamento dell’esercito e l’acquisizione dell’elettorato in perpetuo per la Baviera), era tra i primi obiettivi della concezione strategica del papato. Il carteggio della Nunziatura di Vienna, di cui la quarta serie per gli anni tra 1628 e 1635 è consultabile tuttora, mette in luce queste tendenze in favore della Baviera. Nelle corrispondenze curiali si delinea, essenzialmente, il tentativo di attribuire a quel territorio la qualità di Stato, termine in cui si esprime l’idea centrale del discorso politico, ma anche giuridico dell’epoca. Dalla parte della Curia romana, la strategia di state-building si ricollegò all’ambizione di assegnare un posto primario alla Baviera nel sistema geopolitico europeo („unione delle corone cattoliche“ sotto il patrocinio del papa come „padre comune“).

Izraelici pojmowali Go nie tyle jako samoistny byt, lecz raczej jako wyraz działania Boga Jahwe. Przypuszcza się, że nieosobowa forma działania wynikała z potrzeby zachowania monoteizmu. Na ka­rtach Nowego Testamentu dostrzegamy działanie Ducha Świętego w stosunku do osób, które żyjących na pograniczu Starego i Nowego Przymierza. Duch Święty natchniął Symeona co do Dzieciatka Jezus; On zstąpił na Maryję, aby mogła począć Syna Bożego, o Nim mówił Jan Chrzciciel. O Nim nauczał Jezus Chrystus; zapowiadał, że będzie prosić Ojca, aby zesłał Ducha Świętego, zapowiedział, że On, gdy przyjdzie, doprowadzi uczniów do całej prawdy i pomoże im zrozumieć to, czego Jezus nauczał oraz dzieło zbawcze. Istotnie, po Zmartwychwstaniu Duch Święty został zesłany. Mamy dwa opisy: jeden w Ewangelii Jana, a drugi w Dziejach Apostolskich. Według relacji Jana sami tylko apostołowie otrzymali dar Ducha Świętego połączony z prawem do odpuszczeniem grzechów, co jest – jak wiadomo – atrybutem samego Boga. Zesłanie Ducha Świętego, o którym mówi Łukasz miało charakter bardziej okazały, było związane z darem języków (glosolalia), a także i ukazaniem się nadzwyczajnego zjawiska podobnego do ję­zyków z ognia. Była to realizacja zapowiedzi Jana Chrzciciela, że Apostołowie będą ochrzczeni Duchem Świetnym i ogniem. Ogień, jak wiadomo, jest symbo­lem obecności i działania Boga. Duch Święty raz udzielony przez Boga Ojca na prośbę Chrystusa, pozostaje w Kościele i kontynuuje dzieło oświecenia, uświeca­nia i umocnienia wiernych w wyznawaniu wiary. Duch Święty zawsze działa w łączności z Ojcem i Synem dla zbawienia wierzących.

Seeking the solution to the problems of contemporary man and approaching the social events through mythology is the other way to use the healing power of literature education. Having served as a guide for people in the past, mythology is the mirror of the past, which indicates the reasons and possible results of the events that have experienced today. The communities, which internalize the information well through myths, can direct their future, as well. In this study, we aim to examine the child education and the social problems that arise as a result of this education through the protagonist of Duca Kocaoğlu Deli Dumrul story, his relation with his parents and his life told in one of the twelve texts in the Book of Dede Korkut, which is an epic of Oghuz Turks. To this end, Suphi Altındöken, which had sparked a debate by killing Özgecen Aslan, a university student in Mersin in 2015, and Deli Dumrul, who shows relatively different characteristics from the other protagonists in the Book of Dede Korkut will be compared by their education that they receive from their parents and the environment that they grew up. In this comparison, we have used two tragedia; of Alcestis and of Antigone by Euripides and Sophocles, two playwrights from 5th and 4th centuries B.C. respectively, and used the story of Dumrul and Azrael, published by Murathan Mungan in 2000. In the context of a mythological story from the 12th century and an event from the 21st century, the main elements of child education, especially the parents' attitude, will be included.

ABSTRACT Primary cultures of intrahepatic bile duct epithelial (IBDE) cells isolated from duckling livers were successfully grown for studies of duck hepatitis B virus (DHBV). The primary IBDE cells were characterized by immunohistochemistry using CAM 5.2, a cytokeratin marker which was shown to react specifically to IBDE cells in duck liver tissue sections and in primary cultures of total duck liver cells. Immunofluorescence assay using anti-duck albumin, a marker for hepatocytes, revealed that these IBDE cultures did not appear to contain hepatocytes. A striking feature of these cultures was the duct-like structures present within each cell colony of multilayered IBDE cells. Normal duck serum in the growth medium was found to be essential for the development of these cells into duct-like structures. When the primary cultures of duck IBDE cells were acutely infected with DHBV, dual-labeled confocal microscopy using a combination of anti-DHBV core proteins and CAM 5.2 or a combination of anti-pre-S1 proteins and CAM 5.2 revealed that the IBDE cell colonies contained DHBV proteins. Immunoblot analysis of these cells showed that the DHBV pre-S1 and core proteins were similar to their counterparts in infected primary duck hepatocyte cultures. Southern blot analysis of infected IBDE preparations using a digoxigenin-labeled positive-sense DHBV riboprobe revealed the presence of hepadnavirus covalently closed circular (CCC) DNA, minus-sense single-stranded (SS) DNA , double-stranded linear DNA, and relaxed circular DNA. The presence of minus-sense SS DNA in the acutely infected IBDE cultures is indicative of DHBV reverse transcriptase activity, while the establishment of a pool of viral CCC DNA reveals the ability of these cells to maintain persistent infection. Taken collectively, the results from this study demonstrated that primary duck IBDE cells supported hepadnavirus replication as shown by the de novo synthesis of DHBV proteins and DNA replicative intermediates.

Duch Święty jest w tych tekstach odrębną Osobą, mającą więź z Ojcem i działającą w Jego imieniu. Dlatego poczęte Dziecko będzie Święte i nazwane od samego poczęcia Synem Bożym. W ten sposób tekst Mateuszowy otrzymuje pełniejsze zrozumienie, gdy jest odczytany w perspektywie Łukaszowej.

Artykuł porusza kwestię obietnic Ducha dla „martwych” w świetle Ap 14,13. Punktem wyjścia jest stan badań nad tekstem Ap 14,13. Zostało wykazane, że – jak dotąd – uczeni głownie skupiali się na adresatach tego makaryzmu, poszukując odpowiedzi na pytanie o to, czy idzie tutaj o wszystkich chrześcijan, czy tylko o męczenników. Autor artykułu proponuje rozumieć „martwych” w kategoriach duchowych (jako martwych duchowo). Wskazuje na to kontekst makaryzmu, którym jest wizja 14,6-13, jak również fakt, że duchowe znaczenie terminu „martwi” nie stoi w sprzeczności z innymi tekstami Apokalipsy. W związku z tym mamy tu błogosławieństwo dla tych, którzy się nawracają. Duch Święty jawi się tutaj jako Ten, który potwierdza obietnice dane w makaryzmie, a co za tym idzie, jest Tym, który nadaje dynamizm głoszonej „odwiecznej dobrej nowinie”. Duch, przemawiając w Ap 14,13, jawi się jako Osoba, która wyraża siebie samego, przekazując orędzie ludziom. Paralelne do wypowiedzi Ducha polecenie spisania treści błogosławieństwa ukazuje rolę tegoż Ducha w procesie, który nazywamy natchnieniem skrypturystycznym. Zatem Duch odgrywa tutaj bardzo ważną rolę w przekazie dobrej nowiny, która z jednej strony wzywa do nawrócenia, a z drugiej, w przypadku, kiedy to nawrócenie nastąpi, wskazuje na rychłą realizację zbawczych obietnic.

Wystąpienie tego samego wyrażenia rûHî („mój duch”) w 36,27; 37,14 i 39,29 potwierdza nie tylko charakter stwórczy Bożego ducha, ale również pozwala uznać go za hipostazę Boga. Wewnętrze odrodzenie dokonane przez ducha Jahwe czyni nie tylko z Ezechiela, ale również z Izraelitów manifestację obecności Boga w świecie.

Artykuł jest teologiczno-katechetyczną analizą owoców Ducha Świętego (Ga 5,22-23) na drodze do kształtowania dojrzałej wiary w procesie katechetycznym. Studium zostało podzielone na trzy punkty: owoce Ducha Świętego, dojrzała wiara i dojrzały człowiek. Analiza tekstów biblijnych oraz dokumentów katechetycznych pozwoliła na wyprowadzenie trzech wniosków. Po pierwsze: to Duch Święty jest dawcą łaski i źródłem dającym wzrost wierze. Po drugie: cnoty, które wymienia św. Paweł w Liście do Galatów jako owoc działającego w człowieku Ducha, należy uznać za cechy dojrzałej wiary. Po trzecie: zadaniem katechezy jest pomoc człowiekowi w odkrywaniu bogactwa darów Ducha Świętego oraz w ich urzeczywistnianiu.
