Abstract:
Venous thromboembolism (VTE) encompasses both deep vein thrombosis (DVT) and pulmonary embolism (PE).1,2 Factors such as venous stasis, vascular injury, hypercoagulability and major orthopaedic surgery such as hip/ knee replacement or hip fracture surgery increases the risk for thrombosis. Currently there is limited information on the incidence and risk of VTE following ankle fracture surgery and the economic impact on the use of prophylaxis for the prevention of VTE in these patients. Chapter one of this thesis provides a comprehensive literature review highlighting the epidemiology, risk factors, prophylactic modalities used and complications in both medical and surgical patients for the prevention of VTE. Chapter two is a retrospective study that examines this area, in particular, practice patterns in VTE prevention both in-hospital and beyond hospitalisation following ankle fracture surgery. These results showed that most of these patients received low molecular weight heparin (LMWH) and that the overall in-hospital VTE incidence was 2.9% (95% CI: 1.3 - 4.4). There was a 1.5-fold increase risk of VTE at three months post hospitalisation (4.3% (95% CI: 2.3 - 6.2), bleeding rates were low and there were no major complications. This led to a systematic literature review for chapter three evaluating the economic impact with use of pharmacological prophylaxis in major orthopaedic surgery for VTE prevention as well as complications and treatment costs. These results highlighted economic benefits with the use of LMWH compared to difference in healthcare systems. The first two chapters emphasise the importance of immobility for the risk of thrombosis. This is discussed in chapter four together with a summary of the main findings, gaps and future directions. The need to explore the importance of immobility on DVT risk led to a protocol development and ethics approval for the evaluation of the rates of DVT in diabetic foot ulcer patients with total contact casts.

Abstract:
Objective Formulation of a clinical practice guideline (CPG) for the use of thromboprophylaxis (TP) in pediatric patients with a central venous catheter (CVC). Participants The development team consisted of five experts and a doctoral candidate acting as the primary author. Evidence The guideline was developed utilizing the Appraisal of Guidelines for Research and Evaluation (AGREE) II framework. A systematic review of the evidence was performed and evidence was graded using the American Academy of Pediatrics (2004) evidence classifications for CPG recommendations. An appraisal team evaluated the guideline quality utilizing the AGREE Plus platform rating the guideline as “highest quality.” Consensus Employing a modified Delphi methodology, members of the development team reviewed available evidence and voted on proposed Key Action Statements (KAS). Consensus is defined as 80% rating the KAS “usually appropriate.” Conclusion Five KAS are included in the final CPG. Each KAS indicates level of evidence, benefit-harm relationship, and level of recommendation. KAS 1. Providers of hospitalized children (0-18 years of age) may assess for VTE risk factors using the Skrocki VTE risk stratification tool if the patient has a CVC. (Evidence Quality:C, Rec. Strength: Option). KAS 2. Providers of hospitalized children should initiate targeted pharmacologic thromboprophylaxis (tpTP) at the time of CVC insertion or hospital admission (if CVC present on admission). (Evidence Quality: B, Rec. Strength: Strong Recommendation) KAS 3. Providers of hospitalized children with a CVC should implement mechanical thromboprophylaxis (mTP) if the child is immobile (Braden Q score <2) or moderate/ high risk for VTE using the Skrocki VTE risk stratification tool and have no contraindications to mTP. (Evidence Quality: B, Rec. Strength: Recommendation). KAS 4. Providers of hospitalized children with a CVC may prescribe systemic pharmacologic thromboprophylaxis (spTP) if the patient is found to be at high risk for VTE using Skrocki VTE risk stratification tool and the patient has no contraindications to spTP. Abstract:
I assessed the accuracy of two clinical prediction rules, the d-dimer blood test and point of care ultrasound for diagnosing lower limb deep vein thrombosis. Emergency physicians were trained in ultrasound and prospectively scanned emergency department patients with suspected deep vein thrombosis. Accuracy of the Wells and AMUSE rules and the ultrasound result was compared to radiology-performed ultrasound and a 90-day clinical outcome. Univariate and multivariate analyses were performed assessing which factors were associated with the outcome. The sensitivity and specificity of the Wells score for the clinical outcome was 85.7% and 68.5%; the AMUSE score 85.7% and 54.4%. Ultrasound had a sensitivity of 91.7% and specificity of 91.7% for radiology-diagnosed thrombus and 78.6% and 95.0% for clinical outcome. The odds ratio of a positive outcome with a positive ultrasound was 65.1. After receiving the ultrasound training program, emergency physicians were unable to demonstrate sufficient accuracy to replace current diagnostic strategies.

Abstract:

Företaget Flextronics AB i Linköping testar moderna mobiltelefoner för tillverkares räkning. Som examensarbete har jag fått företagets uppdrag att utvärdera och förbättra deras testsystem DVT-UMTS (3G).

DVT-UMTS är ett komplext system av komponenter, och man misstänker att dessa stör varandra inbördes, vilket i sin tur kan ge felaktiga mätresultat.

Beträffande mottagarsidan hos en mobiltelefon tror man att DVT-UMTS p g a störningar, s k intermodulationsprodukter, uppvisar bitfelshalter i testet Rx Out of band blocking. Komponenter som kan tänkas ge störningar är continuous wave-generator och basstationssimulatorn. Antennanslutningen bortses från i undersökningen. Kontroll av CW-generatorn sker med hjälp av lågpassfilter, riktkopplare och dämpare. Kontrollen visar att den är orsak till bitfel men inte ensam orsak. Basstationssimulatorn kontrolleras med hjälp av högpassfilter, riktkopplare och varierade kabellängder. Även denna visar sig ge upphov till bitfel.

Bitfel orsakade av intermodulationsprodukter från CW-generatorn kan reduceras med lågpassfilter och dämpare. Motsvarande problem hos basstationssimulatorn kan åtgärdas med högpassfilter och varierade kabellängder.

Kvarstående bitfel är acceptabla enligt specificerade krav från det internationella samarbetsorganet 3GPP.

Mobiltelefonens sändarefunktion testas med programmet Tx Spurious emission, där otillåtet höga dämpningsnivåer kan ge problem. Dessa nivåer beror av signalvägarna mellan testad enhet och spektrumanalysatorn. Med hjälp av gordon, darlingtontransistor och switch kan signalvägarna optimeras. En alternativ testuppkoppling på DVT-UMTS, innehållande switch med styrprogram, ger dämpningsnivåer innanför specificerade gränser.

Abstract:
Background: Lung transplant is the fastest growing solid organ transplant procedure and venous thromboembolism has been described to occur in 8-29% of cases. This is much higher than any other solid organ and the exact incidence has yet to be determined. Risk factors for the development of venous thromboembolic disease (VTE) specific to lung transplant are not fully understood. Purpose: The purpose of this research was to describe the incidence and risk factors for VTE disease during the first year following lung transplant at a busy transplant center in the Southwest United States. Methods: A descriptive retrospective study design was used. Virchow's triad was used as a theoretical model to identify selected variables common to lung transplant in an effort to understand possible risk factors for the development of VTE in this patient population. Consecutive lung transplant cases between June 1, 2013 and May 31, 2014 at St. Joseph's Medical Center in Phoenix, Arizona were retrospectively reviewed and followed for exactly one year following the lung transplant date. Demographic variables, Virchow's triad variables, and variables previously identified in the literature as being risk factors for VTE were collected and analyzed using descriptive, frequency, t-test, chi-square, and logistic regression. Results: The incidence of VTE in this patient population was 25.8% and is consistent with findings from previous studies. Using the constructs of Virchow's triad did not yield any statistically significant predictors for VTE in this patient population. However, lung allocation score (LAS) (OR 1.109, CI 1.038-1.185), body mass index (BMI) (OR 1.362, CI 1.034-1.794), and time on the waitlist (OR 1.094, CI 1.023-1.171) did reach statistical significance as possible predictors for VTE following lung transplant in this patient sample. Conclusions: VTE is a common complication of lung transplant that has a high incidence during the first year following lung transplant. This study did identify LAS, BMI, and time on the waitlist as being possible risk factors for the development of VTE following lung transplant. LAS may be a useful surrogate to determine the risk for VTE in this population.

Abstract:
Introduction: Inferior vena cava (IVC) malformations are extremely rare. Their impact with additional risk factors has not been well studied. Here, we present a case of a young female with absent IVC. She presented with an extensive left lower extremity (LLE) deep venous thrombosis (DVT) after starting oral contraceptives. Case: A 21-year-old female with no past medical history presented with progressively worsening back pain and LLE edema. Her only medication was combined oral contraceptive (OCP) which was started 4 months ago. Physical exam was significant for blue discoloration of her LLE extending from foot to thigh. The ultrasound showed multiple thrombi within the left common and deep femoral veins, left popliteal vein and left peroneal vein. The abdominal computerized tomography revealed the absence of an infrarenal IVC with collateral azygous, hemiazygous and prominent left gonadal veins. Occlusion by the thrombi extended from the left femoral vein to the left iliac vein. Some extension was noted into the right common iliac vein. She was treated with a catheter directed tissue plasminogen activator (tPA) infusion into the left iliac vein followed by continuous heparin infusion. Hypercoagulable testing was negative. While on heparin, she developed asymptomatic gross hematuria. CT urogram showed a significant dilation of the left gonadal vein, a large vein along the left psoas muscle draining into the left renal vein, and a mild enlargement of right gonadal vein. The urine analysis was negative for bacterial infection; but it contained a numerous amount of red blood cells and protein. Once the hematuria had resolved, she was discharged on apixaban. Her OCP was discontinued. Discussion: There have been several case reports describing IVC abnormalities associated with DVT. In our case, DVT was likely provoked by usage of OCP. Current guidelines would require provoked DVTs to be treat for at least 3-12 months with anticoagulation therapy. Case reports of IVC malformations illustrated that these patients are at high risk for DVT at baseline. Hence the underlying abnormality still poses a lifetime risk. Also, with abnormal venous vasculature, gross hematuria, proteinuria and the lack of other causes, we suspect underlying Nutcracker syndrome (NS). Her pelvic abdominal pain can also be attributed to pelvic congestion syndrome, which is commonly associated with NS. Conclusion: There are several questions that arise from this case including: selecting a type of contraceptive method; safety of direct thrombin inhibitors in patients with NS; indefinite direct thrombin inhibitors in young patients. Unfortunately, due to the rarity of this condition, no set guidelines are available on how to manage these individuals.

Abstract:
A mathematical model of blood coagulation under defined flow conditions, initiated and modulated by spatially discrete regions of surface bound tissue factor (TF) and thrombomodulin (TM), respectively, is presented. The model incorporates fluid phase and surface-associated reactions of the extrinsic, intrinsic, and common pathways, as well as three inhibitory pathways. The spatially heterogeneous model is formulated by finite element method, and an effective prothrombotic zone, which quantifies the spatial propagation of thrombin generation is defined. Characteristic features of coagulation are simulated under physiologic conditions, and the behavior of the system in response to perturbations in TF and TM surface densities, TF site dimensions, and wall shear rate is explored. The major findings of these studies include: (i) The model system responds in an 'all-or-none', threshold-like manner to changes in model parameters. (ii) It was found that prothrombotic effects may extend significantly beyond the dimensions of the spatially discrete site of TF expression in both axial and radial directions. (iii) The relationship between the length of the effective prothrombotic zone and the interval distance between tandem sites of TF expression dictate the net response of the system. Additive prothrombotic effects of sub-clinical lesions as well as suppressive antithrombotic effects of intervening TM-containing regions were observed. Secondly, the computational model is applied to calculate an individualized, systems-based metric of clotting potential for 210 pre-menopausal women in the Leiden Thrombophilia Study (LETS). The simulated variable was found to be a highly predictive parameter for deep venous thrombosis risk.

Abstract:
This research uses economic evaluation methods to model the current practice of VTE prevention at the Gold Coast Hospital and Health Services compared to historical controls. Additionally the research tested the hypothesis that improved prescribing compliance would achieve better clinical outcomes and would be cost effective. This research revealed that current VTE prophylaxis practice were cost effective as compared to either no or suboptimal prevention. Economic modeling revealed that increasing prescribing compliance to 95% to achieve a lower incidence of DVT and PE was cost effective for General Surgical and Orthopaedic patients. For Medical Patients a blanket prescribing approach was not cost effective.

Background. Ultrasound has until recently been regarded as a sophisticated examination reserved for tertiary health care. In reality it is well suited to the district or primary health-care situation. A DVT (deep vein thrombosis) is an important complication of the treatment of TB and this can lead to more devastating sequelae such as a pulmonary embolus. Many DVTs are clinically silent, making the diagnosis difficult. Method. This study was a prospective, longitudinal observational study. The study documented the incidence of DVTs and their onset, assessed certain aspects in an attempt to identify some risk factors, and noted the most common position of the DVT in a TB population. The feasibility of a sonographer-led ultrasound clinic for the diagnosis of DVTs was also assessed. This was achieved by screening the in-patient population at a district TB hospital. The participants received up to four routine duplex Doppler compression ultrasound examinations of the venous system of the lower extremities on week 0, week 4, week 8 and week 14. In addition a single abdominal ultrasound was performed at week 0. Results The incidence of DVTs in this TB population was 15.3%. A median of day 10 from commencing TB treatment was identified as the most common day to develop a DVT. The popliteal vein was the most frequent position for a DVT. Several statistically significant factors were identified, including a decreased ambulatory status, TB regimen and the use of anticoagulants. Only 52% of the DVTs were clinically symptomatic. The clinical evaluation for a DVT diagnosis in this study population had a sensitivity of 52.4% and a specificity of 65.3%. The positive predictive value (PPV) was 21.7%. Of the abdominal ultrasound reports there were 75.5% (281) abnormal reports, 22.5% (n = 90) normal reports and 4.5% (n = 18) with no report. Conclusion This body of work has shown how an effective ultrasound service can be provided at a district level TB hospital successfully administered by a trained ultrasonographer. This also facilitated a screening service to diagnose both symptomatic and asymptomatic deep vein thromboses in newly diagnosed tuberculosis patients. This study confirmed a higher rate of DVT in newly diagnosed TB patients than has been previously seen. It also provided detail on additional risk factors. The study illustrates the poor performance of clinical signs and symptoms as a trigger for further investigation for the confirmation of a DVT. Given the frequency and impact of the embolic complications of DVT, this study provides a strong justification for further research into routine serial ultrasonic screening and/or prophylactic antithrombolytics in newly diagnosed TB patients. As well as the DVT ultrasound scans there was the ancillary service offered by the research sonographers. This included an abdominal scan that detected abdominal pathology in 75% of the scans performed. An ultrasound scan is not pathognomonic but it does provide significant insight into the extent of some of the abdominal pathologies seen in TB patients. The information provided from this study gives a good indication of the problem that DVTs present in this population and the complexities of the disease TB. It is hoped that the results from this study will better equip the medical team in the non-tertiary situation to be vigilant for the presence of a DVT and educate them on the usefulness of the ultrasound scan.

Neste trabalho apresenta-se um elemento finito de casca plano que é obtido pela composição de um elemento de placa com um de membrana. O elemento de placa usado, o DKT (Discrete Kirchhoff Theory), pertence à classe dos elementos triangulares com nove graus de liberdade (uma translação e duas rotações por nó), e é obtido pela imposição da hipótese de Kirchhoff nos seus pontos nodais. Para o elemento de membrana, usou-se o elemento CST (Constant Strain Triangle), com seis graus de liberdade (duas translações por nó). O elemento finito resultante DCT, possui dezoito graus de liberdade (três translações e três rotações, sendo uma das rotações fictícia- no plano do elemento). Simples exemplos de placa e chapa no espaço foram analisados para se testar o elemento, e finalmente uma casca cilíndrica uniformemente carregada foi analisada e o resultado obtido comparado com o fornecido por outros autores. Neste estudo, o elemento DKT mostrou-se eficiente na análise de cascas.
This work presents a flat shell finite element composed of plates in bending and in tension finite elements. The bending element used, the DKT (Discrete Kirchhoff Theory) belongs to the class of triangular elements with nine degrees of freedom (the transverse displacement and its derivatives at each node) and it is obtained from the theory of plates with shear force influence with Kirchhoff hypothesis imposed in the nodes of the element. The tension finite element is the well known CST (Constant Strain Triangle) with six degrees of freedom (the displacement u and v at each node). The resulting finite element, called DCT, has 18 degrees of freedom, including an extra and fictitious rotation in the normal direction to the surface of elements. Single plates in bending and in tension described in a tridimensional system of coordinates were analyzed to test the adopted finite elements and finally cylindrical shell uniformly loaded were analyzed and the results compared with those of authors. In the work the DKT element shown to be a reliable finite element to be used in shell analysis.

Die Radiologie fungiert als wesentliches Instrument in der Diagnostik und Nachsorge fibro-ossärer Läsionen (FOL). Hierbei gewinnen überlagerungsfreie, dreidimensionale Aufnahmen aufgrund der im Kopf-Halsbereich vorhandenen hohen Dichte und Vielfalt anatomischer Strukturen und der damit einhergehenden Fülle von Differentialdiagnosen an Bedeutung. Anhand der Studie wurden die röntgenologischen Charakteristika von ossären Dysplasien (OD) und ossifizierenden Fibromen (OF) im dentalen Volumentomogramm herausgestellt, sowie diagnostische und therapeutische Vorteile der dentalen Volumentomographie (DVT) im Vergleich zur Orthopantomographie (OPG) und Computertomographie (CT) ermittelt und gegenübergestellt. Zu diesem Zwecke wurden anhand eines Fragebogens 18 Röntgenbildpaare (OPG-DVT) von FOL durch zehn Betrachter auf (A) deren röntgenologische Eigenschaften sowie Metallartefakte befundet und (B) deren Abbildungsqualität von sehr gut (1) bis schlecht (5) bzw. nicht beurteilbar bewertet. Insgesamt wurden 360 Analysebögen ausgewertet. Entitäts- und röntgentechnikspezifische Unterschiede wurden statistisch ermittelt. Die Abbildungsqualitäten der DVT und CT wurden auf Grundlage einer intensiven Literaturrecherche verglichen. Die Ergebnisse dieser Studie stellten signifikante Unterschiede in den röntgenologischen Eigenschaften von OD und OF heraus. Acht von zehn Strukturen zeigten in den DVT-Aufnahmen eine signifikant bessere Abbildungsqualität im Vergleich zu den OPG-Aufnahmen. Die teilweise gravierenderen Befunde in den DVT-Aufnahmen deuteten auf eine Unterinterpretation dieser Befunde im OPG hin. Die Literaturrecherche zu Gegenüberstellungen der Abbildungsqualitäten in CT und DVT wies nahezu ausnahmslos auf eine Überlegenheit der DVT hin.
