Abstract:
The high number of patients who come for ear therapy with massage skills, the P3T Center intends to carry out an observational screening of ear therapy. This screening aims to find out how the diagnostic method and application of ear therapy with massage skills and the benefits of ear therapy to overcome the problem of hearing loss (hearing impairment). This screening design uses an observational method in which the researcher makes observations and interviews with respondents. Data collection techniques were carried out by looking at the results of the examination of respondents' hearing tests and the results of interviews with respondents. Patient therapy results in 18 people (60%) no progress or the same as the original condition. this shows that therapy can be said to be ineffective in curing ear disorders. The results of the audiometry examination of the right and left ear stated that none of the patients was normal. Even though there are patients who claim to be able to hear loud sounds in the questionnaire, it is not supported by audiometry results. So it can be concluded that ear therapy with massage skills has no impact on patients suffering from hearing loss.

Abstract:
Abstract Introduction With a severe facial burn, injury often involves the ear(s) resulting in damage or loss which may require reconstruction. In consideration of the functional and psychological effect that loss of a defined ear lobe may entail, reconstruction is an important consideration for the burn survivor. Two cases described in detail with illustration will highlight an auricular contracture release; one with V-Y Advancement and the other with a transposition flap. Preservation of the auricular reconstruction is accomplished by implementing necessary splinting. The auricular splint fabricated was made from a combination of silicone and low-temperature thermoplastic material. A step-by-step process will illustrate the needed materials and techniques to fabricate a worthy splint in order to preserve what was surgically achieved. The splint maintains the space between the ear and head to properly position and prevent contracture through the healing and scar maturation process. It secured with a one-inch head band that is sublime enough to foster compliance with wear. The auricular splint material also serves as scar management with the many proven benefits of silicone. Methods Two case studies are reviewed with auricular contractures that required release and reconstruction. The auricular splints were fabricated to stabilize and preserve the reconstructed ear lobe. The wear schedule was intermittent during the day and overnight in combination with scar massage. The patients reported good compliance with wear and stability of splint position with overnight wear. Skin integrity was not compromised with overnight or intermittent wear. The splint fabrication process is reviewed and illustrated with a simplistic innovative end result that is user friendly. Results Surgical reconstruction was required following development of auricular contractures and partial loss. With post-operative bandage removal and wound closure, positioning was a necessary consideration to preserve post-surgical reconstruction. Conclusions In conclusion, the peri-auricular splint is fabricated with low temperature splinting material lined in silicone proved worthy for effective positioning and reported comfort with wear. Effective splinting is necessary for the preservation of surgical reconstruction and to prevent contracture from sabotaging surgical efforts. Applicability of Research to Practice Consideration of fabrication of silicone auricular splints to preserve the accomplishments of auricular surgical reconstruction.

Abstract:
Neonatal piglets are often used in biomedical research applications that require artificial rearing. Social housing can be problematic because the piglets develop belly nosing, navel and ear sucking that can result in injury. Our objective was to determine the effectiveness of using feeding devices that provide various opportunities for sucking and nosing behaviour on reducing piglet-directed behaviour of group-housed laboratory piglets. Fifteen piglets were used in each of four trials. The piglets nursed their dam for approximately 72 h to obtain passive immunity before transfer to a laboratory facility where they were allotted, five per group, to one of three stainless steel isolator units. Each unit featured a different style of feeding system for the delivery of milk replacer: a plastic trough (T), a nipple (N) mounted on a smooth plexiglass wall, or a nipple mounted on a pliant bag of sterile water (artificial udder [AU]). Each system had five feeding spaces so that all piglets fed simultaneously. Milk was provided at 6-h intervals, and behaviour was recorded on alternate days for 12 days post-weaning. Although trough-fed piglets began to eat much sooner than those piglets fed from nipples, time spent nosing, chewing or sucking on pen-mates and belly nosing were markedly higher in T piglets than in either N or AU, overall (mean: P <0.05) and over time (quadratic: P <0.05). Over time, N piglets developed a stereotypic snout rubbing on the wall behind the nipples, while AU piglets massaged and often fell asleep in contact with the udder from day 2 of the trial. Resting patterns were also affected. N and AU piglets settled down to rest more quickly (P <0.01) and spent significantly more time resting in the hour following feeding than T piglets (P <0.05). A feeding device that accommodates both sucking and massage can significantly reduce piglet-directed behaviour and may facilitate social housing of artificially reared piglets.

Abstract:
The objective of our study was to investigate if auriculotherapy (AT) can modulate pain threshold. In our experiments, AT consisted of placingVaccariaseeds over the “fingers point” of one ear. Two groups of healthy volunteers were enrolled for the study. Each subject was asked to perform an autoalgometric test developed by our group on three occasions: before, 1 hour after, AT and 24 hours after AT. Participants of the first group received a 2-minute long session of AT, while participants of the second group received a 2-minute long session of sham treatment, consisting of a puncture/massage above the skin of the neck. The autoalgometric test consisted of applying an increasing pressure with the finger-tips and finger-backs of four fingers by the subjects themselves (i.e., eight sites were evaluated) against a round-shaped needle for two times: until a minimum pain sensation (first time, minimal test) or a maximally tolerable pain sensation (second time, maximal test). Our results showed a significant higher pain threshold in the maximal test at 24 hours after AT compared to sham treatment. This result indicates for the first time that AT can increase pain tolerability, rather than affecting the minimal pain threshold.

Abstract:
Background The Facial Grading Scale (FGS) is a quantitative instrument used to evaluate facial function after facial nerve injury. However, quantitative improvements in function after facial rehabilitation in people with chronic facial paralysis have not been shown. Objective The objectives of this study were to use the FGS in a large series of consecutive subjects with facial paralysis to quantitatively evaluate improvements in facial function after facial nerve rehabilitation and to describe the management of chronic facial paralysis. Design The study was a retrospective review. Methods A total of 303 individuals with facial paralysis were evaluated by 1 physical therapist at a tertiary care facial nerve center during a 5-year period. Facial rehabilitation included education, neuromuscular training, massage, meditation-relaxation, and an individualized home program. After 2 months of home exercises, the participants were re-evaluated, and the home program was tailored as necessary. All participants were evaluated with the FGS before the initiation of facial rehabilitation, and 160 participants were re-evaluated after receiving treatment. All participants underwent the initial evaluation at least 4 months after the onset of facial paralysis; for 49 participants, the evaluation took place more than 3 years after onset. Results Statistically significant increases in FGS scores were seen after treatment (P<.001, t test). The average initial score was 56 (SD=21, range=13–98), and the average score after treatment was 70 (SD=18, range=25–100). Limitations A limitation of this study was that evaluations were performed by only 1 therapist. Conclusions For 160 patients with facial paralysis, statistically significant improvements after facial rehabilitation were shown; the improvements appeared to be long lasting with continued treatment. The improvements in the FGS scores indicated that patients can successfully manage symptoms with rehabilitation and underscored the importance of specialized therapy in the management of facial paralysis.

Abstract:

Abstract:

Abstract:

Full text
Abstract:
La validation du modèle est effectuée grâce de nombreuses comparaisons calcul-expérience<br>This thesis deals with the modelling and the computation of steam-water two-phase flows. Liquid water flows are involved in many industrial facilities and a second phase may appear in some transients situations. Thus, pressure wave propagation and mass transfer are physical phenomena that need to be properly included in the modelling of such two-phase flows. A statistical two-fluid approach is used, leading to models similar to the Baer-Nunziato one. They include both convective and source terms without any assumption on the equilibrium between phases. Different closure laws for such models are compared on steam-water transient experiments including water-hammers and fast depressurization of liquid water. The computation of the different models is based on a fractional step method. A new convective scheme, able to deal with any Equation Of State, is used in the first step of the method. When compared with other schemes, it appears to be accurate, efficient and very robust. The second step of the method is dedicated to the treatment of source terms and requires several implicit schemes. Particular attention is paid to the verification of every scheme involved in the method. Convergence studies are carried out on test-cases with analytical solutions to that purpose. Based on existing work on fast depressurization of liquid water in an homogeneous approach, a new formulation of the mass transfer is proposed. Many comparisons between computational and experimental results are detailled in order to validate the models

Abstract:

Abstract:
This introductory chapter provides an overview of the book's main themes. This book argues that the post-Stalin leadership, having inherited a massive, inefficient, violent, and corrupt penal system, engaged in a serious and substantive reforming effort bent on transforming the Soviet Gulag both quantitatively and qualitatively. In order to make proper sense of Khrushchev-era reforms in the penal sector, this book places them within three broader narratives. First, they were just one part, albeit a very important and lasting part, of an extensive reformist program instituted by Stalin's heirs and Khrushchev in particular. Second, they were part of a global postwar penal transition toward rehabilitation and greater leniency and respect for inmates. Finally, Khrushchev's Gulag should be understood within the broader context of Soviet crime and punishment, particularly in relation to the Stalin-era Gulag.

Abstract:
This chapter focuses on the years 1321–1324, a period that forms merely an epilogue to Aymer de Valence's life. In August 1321, when the full force of the Marchers was at his back, the Earl of Pembroke was no longer in a position to persuade or cajole Edward II. Once the Despensers were in exile, it would take more ability and force than Pembroke could command to stop the King from recalling them. The chapter examines Pembroke's role in the campaign that finally destroyed the Marchers and Thomas, Earl of Lancaster in 1321 and 1322 and that resulted in total victory for the King, the rise of the Despensers, a massacre of the magnates, and the creation in England of a regime supported only on fear and military force. It also considers Pembroke's activities in the area of royal diplomacy and concludes with a reflection on his death.

Abstract:
AbstractThis review will be concerned with our knowledge of extended matter under the governance of strong interaction, in short: QCD matter. Strictly speaking, the hadrons are representing the first layer of extended QCD architecture. In fact we encounter the characteristic phenomena of confinement as distances grow to the scale of 1 fm (i.e. hadron size): loss of the chiral symmetry property of the elementary QCD Lagrangian via non-perturbative generation of “massive” quark and gluon condensates, that replace the bare QCD vacuum. However, given such first experiences of transition from short range perturbative QCD phenomena (jet physics etc.), toward extended, non perturbative QCD hadron structure, we shall proceed here to systems with dimensions far exceeding the force range: matter in the interior of heavy nuclei, or in neutron stars, and primordial matter in the cosmological era from electro-weak decoupling (10−12 s) to hadron formation (0.5 ⋅ 10−5 s). This primordial matter, prior to hadronization, should be deconfined in its QCD sector, forming a plasma (i.e. color conducting) state of quarks and gluons: the Quark Gluon Plasma (QGP).

Abstract:
Advocates of practical though controversial lifestyle approaches have always found a sympathetic ear in the United States since the time folk practitioner Sylvester Graham’s principles of health, nutrition, and fitness (in addition to inventing the Graham cracker) achieved cult status in the 1840s. Heroic, misguided therapies were administered by allopathic (mainstream) physicians throughout the nineteenth century. This created fertile ground for promoters of patent medicines and nostrums to those escaping organized medicine’s use of leeches, cupping, phlebotomy (blood drawing) knives, and brutal laxative regimens. During the Progressive Era, medicine started to improve with the establishment of postgraduate training programs at Johns Hopkins University just before the turn of the century and the regulation of medicines as part of the Pure Food and Drug Act of 1906. The final revolution occurred when two-thirds of the medical schools in the United States closed following revelations of their inadequacies by the investigative Flexner Report funded by the Carnegie Foundation in 1910. Despite these changes, however, the appeal of alternative therapies to the American public continues unabated. The previous two chapters have described how mainstream, organized, conventional medicine approaches fibromyalgia. Even though their therapies usually provide significant relief of symptoms and signs, traditional physicians to some extent must regard themselves as failures. In the United States, one person in three has consulted a complementary medicine practitioner. These individuals spend $23 billion a year on this approach, $13 billion of which is out-of-pocket and not reimbursed by insurance. This exceeds all expenditures on hospital care in the United States. A 1996 Canadian study found that of several hundred fibromyalgia patients, 70 percent purchased unproven over-the-counter rubs, creams, vitamins, or herbs; 40 percent sought help from alternative medicine practitioners such as chiropractors, massage therapists, homeopaths, reflexologists, or acupuncturists; and 26 percent went on special diets. Since it is logical to believe that people who are tired and hurt want to get better, it follows that some fibromyalgia patients will try anything that is not harmful to improve their medical condition. This chapter is dedicated to patients who wish to “look before they leap” into nontraditional therapies.

Abstract:
Opioid addiction is a complex issue. New Mexico has historically experienced some of the highest rates of deaths from opioid overdose, and opioid addictions have affected generations of New Mexicans -- starting many years before the more recent national crisis. Treatment approaches to opioid and other addictions are fraught with paternalism, stigma, surveillance, criminalization, shaming, racism, discrimination, and issues with access to care. Current treatment paradigms fail to take into account the social and economic factors of people, community, and context. New paradigms embracing a broader, more-just contextualization of addictions, along with evidence-based treatment approaches are needed to transform medicine’s historic role in the “war on drugs”. The Strong Roots/Raices Fuertes program was developed by two community clinics, Casa de Salud and Centro Sávila, in Albuquerque, New Mexico. The program evolved from a desire to to acknowledge and right historical harms that the medical-industrial complex has caused; to provide rapid access to dignified, life-saving, evidence-based holistic treatment for opioid addictions in a community setting; and to build a model of care that transforms the biomedical model into one of solidarity with community and collective care. Five key concepts underpin the program design: 1) Harm Reduction, Autonomy, and Agency; 2) Healing-Centered Engagement; 3) Language and Cultural Humility; 4) Transforming Health Systems Design; 5) Workforce Diversity and Pipeline Training. The program’s core components include conventional approaches such as low-barrier access buprenorphine (suboxone®) to medication treatment, primary care, case management, syringe exchange, and counseling/therapy in addition to more community-rooted and integrative healing modalities such as healing circles, acupuncture, massage, reiki, ear acudetox, and civic engagement. In sharing the values, lessons learned, and tools from our work in the Strong Roots/Raices Fuertes program, we hope to inspire and encourage others wishing to develop new systems of care for people dealing with addiction issues.

Abstract:
In times of stringent emission standards for automotive and truck applications, exhaust gas recirculation (EGR) is used in IC engines to reduce NOx emissions by recirculating a portion of an engine’s exhaust gas. The amount of exhaust gas determined for EGR is withdrawn from the exhaust gas route and routed back into the combustion chamber. The recirculated exhaust gas acts as an inert gas and, when mixed with the pre-combustion mixture, helps to decrease the combustion temperature and thus NOx emissions. Designed for a diesel engine within a truck application, the turbine in this particular research project is fed by two cylinder groups, however, only the exhaust gas of one group is recirculated. The reduced mass flow in the small turbine scroll (EGR-scroll) through EGR withdrawal, along with the increased pressure required for EGR transport, leads to a massive reduction in the mass flow parameter of the EGR-scroll. The common turbocharger design process has been based on steady admission rather than unsteady admission given through the pulsating nature of multi-cylinder admission. This has lead to diverging results of turbochargers performing well on steady hot gas test rigs compared to performing badly in the final tests on the engine itself. In this paper however, unequal admission resulting from pulsating admission is taken into account. Based on unsteady admission, a methodology is proposed for steady computations with unequal admissions, and a thorough 3D CFD loss analysis is to be presented to understand the turbine behaviour, reveal the regions for improvements, and provide a framework for further development.

Abstract:
Abstract To mitigate its impact on global climate, the power generation sector must strive towards a transition to net-zero emissions of greenhouse gases. This can be achieved by a massive penetration of renewable power generation. However, a high share of renewable power generation requires dispatchable and flexible power generation technologies such as gas turbines to maintain the stability of power grids. To achieve net-zero green house gas emissions, gas turbines have to be operated exclusively with carbon-neutral fuels. Hydrogen is a promising carbon-neutral fuel, although it comes along with several challenges regarding stable combustion. A possible measure to stabilize hydrogen combustion is the partial external recirculation of exhaust gases (EGR). In a previous study, the authors presented a model-based thermodynamic analysis of an industrial gas turbine featuring EGR. The next step was to answer the question of whether the thermodynamically negative impact of EGR (i.e. lower thermal efficiency) is justified by positive effects, such as reduced NOx emissions or a more controllable combustion of hydrogen. By means of a simple 1-D flame approach, the present study provides further insight into the flame behaviour and stability limits during a fuel switch from natural gas to hydrogen. In a following step, the same approach is used to investigate the flame behaviour in an EGR environment at two recirculation temperatures. The results show that if a hydrogen-fired, diffusion-type combustor is combined with sufficiently high EGR ratios, NOx emissions are potentially in the order of a state-of-the-art diffusion-type combustor fired with natural gas. In addition, based on the calculated laminar flame speeds and extinction strain rates, the higher reactivity of hydrogen could potentially be controlled by employing EGR. However, relevant literature suggests that stronger dilution might be required to compensate for the additional impact of turbulence-chemistry interaction in real application which could lead to flame stabilization issues and higher NOx emissions. Moreover, considering the industry efforts to develop hydrogen-capable premixed-type combustors, the results show that EGR has no significantly positive influence on the reactivity of a premixed pure hydrogen flame. The question regarding the preferred EGR temperature is addressed but cannot be answered conclusively.

Abstract:
Since their appearance in the early 2000s, Massive Open Online Courses (MOOCs) and Open Educational Resources (OERs) arose among the most important educational priorities. Many top universities worldwide have been involved in the research and direct implementation of this innovative pedagogical approach. Simultaneously with the development and massive deployment of the new learning and teaching method, European regulations responsible for data privacy and protection, and information security have significantly evolved. This paper assesses the compliance of the ten most popular MOOCs and OERs with the General Data Protection Regulation (GDPR) and the Directive on security of network and information systems (NIS Directive). In order to systematically examine their online platforms, a few privacy indicators were outlined and thoroughly observed. Alongside this, the involvement of the open education providers in the NIS Directive was examined. Research findings are presented and elaborated in a way that it makes easy to generate recommendations on how to anticipate the future of open education as a reasonable reaction to global change in the era of rapid technological growth, and at the same time to obey the crucial ethical principles defined by this development.
