Abstract:
A Qualidade de Vida no Trabalho (QVT) tem sido objeto de preocupação entre profissionais de diferentes áreas e muitas vezes é avaliada como sendo o bem-estar do trabalhador durante a execução de sua tarefa. A satisfação no trabalho tem se tornado um conceito chave dentro das pesquisas de QVT. Alguns estudos têm apontado a importância da mensuração da satisfação profissional e sugerido a associação da satisfação no trabalho com melhor Qualidade de Vida (QV) e menor índice de estresse relacionado ao trabalho e com a produtividade. No setor público fala-se pouco em Gestão da QVT, porém percebe-se que o tema vem ganhando espaço nesse setor. As pesquisas relativas ao bem-estar dos trabalhadores encaminham-se no sentido de identificar se as categorias organizacionais interferem positiva ou negativamente sobre esses. Neste estudo, considerou-se a QVT sob a perspectiva de avaliação do funcionário de dois aspectos do ambiente de trabalho: a sua ocupação específica e as ações que a Universidade desenvolve que podem influenciar o bem estar no trabalho. O objetivo geral foi estudar os determinantes da QVT dos funcionários não docentes da Universidade de São Paulo (USP). A amostra estudada era relativamente jovem, com o predomínio de mulheres, elevada escolaridade, e alta incidência de sobrepeso e obesidade, bem acima da média nacional que é de 40% de excesso de peso. Em relação a qualidade da alimentação dos funcionários a maioria (83%) necessita ser melhorada, há um baixo consumo de alimentos fonte de fibras como as frutas e as verduras e um alto consumo de carnes em geral e ovos, o que ultrapassou os limites de recomendação de proteínas e gorduras saturadas. De maneira geral, esses funcionários apresentaram-se satisfeitos com a sua QVT, fosse ela medida em relação a sua ocupação, fosse ela medida em relação às ações da USP para o seu bem-estar, fosse ainda, em relação à agregação dessas duas perspectivas. As hipóteses iniciais do estudo referiam-se ao impacto do estado nutricional sobre a QVT, mais especificamente, o efeito negativo do sobrepeso sobre a QVT; e ao impacto da QV em geral sobre a QVT. A primeira hipótese se confirmou, não importando a forma de mensurar a QVT, ao passo que a QV mostrou-se ter um papel importante apenas em relação à QVT medida em relação à ocupação específica, ao passo que a escolaridade mostrou-se importante para uma avaliação mais crítica sobre as ações da organização. Poderíamos então concluir que cuidar do estado nutricional dos seus funcionários é uma ação fundamental para melhorar não apenas o bem-estar do funcionário, mas também o nível de produtividade dentro da empresa, ao passo que funcionários com maior nível de escolaridade terão sempre um olhar mais crítico acerca das ações de QVT da organização, mesmo sendo ela do setor público.
The Quality of Life at Work (QLW) has been the object of concern among professionals in different areas and is often assessed as the welfare of the worker during the performance of their task. The satisfaction at work has become a key concept in the research of QLW. Some studies have shown the importance of measurement of job satisfaction and suggested the association of satisfaction in working with better Quality of Life (QL) and lower rate of stress related to work and productivity. In the public sector is low speech in Management of QLW, but realizes that the issue has been gaining space in that sector. The research on the welfare of workers heading is to identify whether the organizational categories interfere positively or negatively on these. In this study, it was regard the QLW from the perspective of the official assessment of two aspects of the work environment: its occupation and the specific actions that the University develops that can influence the well-being at work. The general objective was to study the determinants of QLW officials not teachers of the University of São Paulo (USP). The study sample was relatively young, with the predominance of women, high school, and high incidence of overweight and obesity, well above the national average which is 40% of excess weight. For quality of food the majority of staff (83%) needs to be improved, a low consumption of food sources of fiber such as fruits and vegetables and high consumption of meat and eggs in general, which exceeded the limits of recommendation for protein and saturated fat. In general, these employees had to be satisfied with your QLW, it was measured in relation to its occupation, it was measured in relation to shares of USP for their welfare, even in relation to aggregation of these two perspectives. Initial hypotheses of the study concerned the impact of nutritional status on the QLW, more specifically, the negative effect of overweight on the QLW and the impact of QL in general on the QLW. The first hypothesis is confirmed, no matter how to measure the QLW, while QL was shown to play an important role only in relation to QLW measure on specific occupation, while the school was found to be important for a more critical assessment of the actions of the organization. We could then conclude that care of the nutritional status of their employees is a key action to improve not only the welfare of the employee, but also the level of productivity within the company, while employees with higher level of education will always have a look more critical of the actions of QLW the organization, even if it\'s public sector.

Abstract:
A matched case-control study was carried out to identify the factors associated with the nutritional status of children in Livingstone district, Zambia. A trained field researcher collected data on demographic, feeding practices and socio-economic factors in matched groups of underweight children and normal weight children from the mothers/caregivers of the children. Interviews, using a structured pre-tested questionnaire, were used to obtain the data. All children aged from six to fifty-nine months admitted at Livingstone General Hospital during the months of October to December, 2003 with a weight for age below &ndash
2SD from the median of the reference population formed the underweight children (cases). The total was 47 children. The cases were then matched according to sex and age to 47 normal weight children attending the under five clinic at Livingstone General Hospital during the same months, with a weight for age above &ndash
2SD from the median of the reference population (controls). The final sample was 94 children. Weights were recorded using the same scale in the outpatient&rsquo
s department prior to admission and during the under five clinic session. Interviews then took place in a separate room after the clinic nurse had attended them. A separate visit was then made to the homes of the children on a different day. Odds ratios and 95% confidence intervals were used to measure strengths of associations. Results revealed that the basic causes of underweight were the poor economic state of the country, unfavorable policies and insufficient government support in the areas of health, education, agriculture, housing and employment. Underlying causes were inadequate access to food, inadequate care of children, poor access to health services and unhealthy living environments, while immediate causes were poor food intake and disease. Low educational and literacy levels of the mothers/caregivers, unemployment and lack of sufficient finances to access basic necessities such as food, housing and health contributed to underweight.

Abstract:
The broad objective of the study was to determine the economic value of local pigs in marketand subsistence-oriented production systems in communal areas of Southern Africa. Data were collected from 288 households to investigate farmer perceptions, effects on pig production and handling of disease outbreaks such as classical swine fever (CSF) in market- and subsistenceoriented production systems. The utilisation of local pigs in these market- and subsistenceoriented production systems in improving people‟s welfare was evaluated. Climate change was identified by farmers in these production systems as a major constraint to pig production hence an experiment was carried out in the hottest season to determine diurnal heat-related physiological and behavioural responses in Large White (LW) and South African local pigs. The same genotypes were used to determine effects of diurnal heat-related stress on their growth performance. Choice experiment was done to determine farmer preferences for local pig traits and implicit prices for these traits in CSF-affected and unaffected areas that were under subsistence- and market-oriented production systems. In this experiment, the importance of heat tolerance was assessed relative to other productive and climate change adaptation traits. Significantly more pigs were culled in the CSF-affected areas that were market-oriented (8.0 ± 1.76) than subsistence-oriented (4.1 ± 1.00) production system. The risk of parasites and disease challenges was high in subsistence-oriented production system and coastal areas. In both production systems, CSF was perceived as destructive since the culling of pigs affected pork availability and income generation. The high risk of disease outbreaks and threat of climate change caused farmers in subsistence-oriented production system to select local pigs for their adaptive traits while those in the market-oriented production system focused on productive imported pigs. Farmers (83 %) indicated that they wanted pig genotypes that were adapted to climate change effects such as hot conditions. Local pigs were found to have superior heat tolerance over LW pigs (P < 0.05) in terms of lower heart rate and skin surface temperature. Frequency per day and duration for behavioural heat loss activities such as wallowing, sleeping in a prostrate posture and sprawling in slurry were also lower (P < 0.05) for local than LW pigs. The superiority of heat tolerance of local over LW pigs was further confirmed by their uncompromised growth performance under high diurnal temperatures. The Pearson‟s product moment correlation coefficient between temperature and feed conversion ratio for LW pigs was strongly positive (r = 0.50; P < 0.001) unlike the weak and positive correlation for local pigs (r = 0.20; P < 0.05). There was a quadratic relationship between temperature and average daily gain (ADG) for both pig genotypes. The regression coefficients for ADG were higher (P < 0.001) for LW than local pigs. It was concluded that at high ambient temperatures, performance of local pigs was less compromised than for LW pigs. Although local pigs were found to be heat tolerant, results of choice experiment showed that this trait was not selected for relative to other traits. Keeping pigs that required bought-in feeds, fell sick often and produced low pork quality (eating quality based on farmer perceptions) negatively affected farmers‟ livelihoods more in subsistence- than market-oriented production system. Farmers in market-oriented production system derived more benefit from productive traits such as heavier slaughter weights and large litter size than subsistence-oriented farmers. Under the subsistence-oriented production system, farmers in CSF-affected areas placed high prices on adaptive traits than the unaffected areas. Subsistence-oriented farmers who were affected by CSF wanted a total compensation price of R10 944.00 (USD1563.43) for keeping a pig genotype with unfavourable traits when compared to R4235.00 (USD605.00) for their CSF-unaffected counterparts. Implicit prices for traits could not be determined for market-oriented production system. It was concluded that farmers in CSFaffected areas placed high economic values on pig traits than farmers from the CSF-unaffected areas. The findings suggest that adapted local pigs can be promoted in subsistence-oriented production systems while productive imported pigs and their crosses with local pigs can be kept in market-oriented production systems.

This thesis focuses on the idea that food insecurity and access are real issues in the lives of many Americans. Simply stated, food insecurity is when a person does not have enough food to eat or does now know where his/her next meal is coming from. More importantly when looking at food insecurity is the realization that healthy, local food access is even more prevalent an issue – with increasingly more under-resourced individuals and families being food insecure and unhealthy at the same time. This thesis includes a literature review on diet and nutrition in the United States, a chapter on methodology, history of Bowling Green, Kentucky, where this case study is focused, the benefits of shopping at farmers’ markets, perceived barriers to shopping at those farmers’ markets, and suggestions for overcoming these barriers. Local, sustainable food is the hope for a future of planet earth. It is what nourishes and sustains lives. And, it should not be a privilege. Through researching the benefits and barriers to farmers’ markets, examining these barriers, developing suggestions for overcoming these barriers, and implementing as many as these initiatives as possible in Bowling Green, Kentucky, I have not only compiled a detailed thesis, but I have also been a small part of creating change in the food community in Bowling Green. This thesis can serve as a nationwide model and describes the way to overcome food accessbarriers in urban/rural communities.

Leading theme in this master’s thesis is adaptation and preparation to ever changing requirements of skills and knowledge in the working life. These changes of requirements in the working life are implied by the changes in the occupational structure in Finland that is presented in the thesis. The data used in thesis to show the changes in the occupational structure in Finland Finnish Longitudinal Employer-Employee Data, FLEED, which is gathered by Statistics Finland and it shows changes in occupational structure of Finland between years 1995 and 2007. Education is considered in this thesis so that its object is to prepare children and future generations for working life with suitable skills and knowledge. The variety of other, equally important, objects that education has and possibly contradictions as well as problems that might arise in the course of achieving all the objects is beyond of this study. Changing occupational structure arises a question that how education prepares itself to the unknown changes? To answer the question asked, I will introduce Economics of education and Educational planning and how these two relate to each other, especially how economics of education has affected education and educational planning. Therefore I will first take historical review of the economics of education and introduce two main theories; human capital theory and screening theory. The main difference of these two will be explained and also what kind of difference it would make to education if either one could be proved right. It would also have an effect on how education should be arranged in the society. Educational planning has had, in its course, different approaches to analyze and predict future. I will review those approaches; also these approaches have been influenced by economics of education. In the last part of the thesis I bring up studies in Finland has tried to prepare and project changing work life. Also I discuss about can education bring something new to economics of education
Tämän Pro gradu-tutkielman johtava teema on mukautuminen ja valmistautuminen muuttuviin vaatimuksiin tiedoista ja taidoista työelämässä. Ammattirakenteen muutos Suomessa viittaa siihen että vaatimukset erilaisille tiedoille ja taidoille työelämässä on tapahtumassa. Aineisto ammattirakenteen muutoksesta on Tilastokeskuksen yhdistetty työntekijä-työnantaja-aineisto (Finnish Longitudinal Employer-Employee Data FLEED). Ammattirakenteen muutos ajoittuu vuosien 1995 ja 2007 välille. Koulutus mielletään tässä Pro gradu-tutkielmassa sellaiseksi toiminnaksi jonka tavoite on valmistaa lapsia sekä tulevia sukupolvia työelämään oikeanlaisilla taidoilla ja tiedoilla. Yhtälailla tärkeät muut tavoitteet, mitä koulutukselle ja kasvatukselle on annettu, sekä mahdollisesti syntyviä ristiriitoja tavoitteita saavuttamisessa ovat tämän tutkielman ulkopuolella. Muutokset ammattirakenteessa luo kysymyksen, että miten koulutus on valmistautunut tällaisiin tuntemattomiin muutoksiin? Vastatakseni kysymykseen esittelen koulutusekonomian ja koulutussuunnittelun, sekä erityisesti miten koulutusekonomia on vaikuttanut koulutussuunnitteluun. Aluksi käyn läpi koulutusekonomian ja esittelen sen kaksi pääteoriaa: inhimillisen pääoman teorian ja siiviläteorian. Selitän millä tavoin nämä teoriat eroavat toisistaan ja mikäli jompikumpi voidaan todistaa oikeaksi millainen vaikutus sillä olisi koulutukseen. Koulutussuunnittelussa on käytetty monenlaisia analysointivälineitä tulevaisuuden ennakoimiseksi. Esittelen nämä eri tavat ja työkalut, sekä miten koulutussuunnittelu on historiansa aikana muuttunut ja miten taloudellinen ajattelu on muuttanut koulutussuunnittelua. Viimeisessä osassa tuon esille tutkimuksia siitä miten Suomessa pyritään ennakoimaan ja valmistautumaan muutoksiin työelämässä. Käyn myös keskustelua siitä voiko kasvatustiede tuoda jotain uutta koulutusekonomiaan

Abstract:
L’inflammation apparait comme un mécanisme ubiquitaire sous-tendant de nombreuses pathologies chroniques. Les nutriments participent à de multiples niveaux, de façon directe ou indirecte a la régulation de la réaction inflammatoire. Les acides gras polyinsaturés (PUFAs) sont les précurseurs de nombreux médiateurs de l’inflammation. Les nutriments antioxydants (vitamine C, E et caroténoïdes) interviennent dans la prévention et la régulation des réactions oxydantes participant à la réaction inflammatoire et aux dommages cellulaires l’accompagnant. L’approche épidémiologique, notamment au travers des études de cohortes, contribue a une meilleure compréhension des relations entre nutrition et sante dans la population. L’étude des relations entre statut en antioxydants (concentrations sanguines en vitamine C, E et β-carotène) et CRP augmentée (C-reactive protein) à long terme nous a permis de montrer que le statut en β-carotène était négativement associe a une CRP augmentée. L’étude des relations entre apports en PUFAs n-3 et n-6 et CRP augmentée nous ont permis de montrer que les apports en PUFAs n-3 et n-6 étaient négativement associés à une CRP augmentée. Les apports en vitamine E étaient modulateurs de cette relation, qui n’apparaissait significative que pour les sujets avec des apports faibles en vitamine E. Enfin, la construction de profils alimentaires spécifiquement associés aux nutriments connus pour avoir des propriétés anti ou pro-inflammatoires (PUFAs, nutriments antioxydants) nous a permis de montrer qu’un profil alimentaire riche en acides gras essentiels et en nutriments antioxydants était négativement associe a une CRP augmentée a long terme. Ces résultat concordants montrent bien l’intérêt d’apports adéquats en PUFAs et en nutriments antioxydants dans les mécanismes inflammatoires. Néanmoins, la balance entre ces différents nutriments doit être prise en compte, en particulier du fait d’interactions multiples entre leurs effets
Inflammation appears as an ubiquitous mechanism involved in numerous chronic diseases, including metabolic diseases, cardiovascular diseases and cancer. Nutrients are involved at multiple levels and in multiple pathways of inflammation regulation. Polyunsaturated fatty acids are precursors for a range of inflammatory mediators: eicosanoids, resolvins and protectins. Antioxidant nutrients (vitamin C, E and β-carotene) prevent and regulate oxidant reactions,preventing in particular lipid peroxidation in cell membranes, which participate in inflammatory reactions and cellular damages. Epidemiological approaches, particularly through cohort studies allow for a better understanding of the relationships between nutrition and health in the population. The study of the long-term relationships between antioxidant status (circulating concentrations of vitamin C, E and β-carotene) and C reactive protein (CRP) allowed us to show that β-carotene circulating concentrations were negatively associated to elevated CRP. The study of relationships between dietary intakes of n-3 and n-6 PUFA and elevated CRP allowed us to show a negative association between n-3 PUFA intakes (particularly docosapentaenoic acid DPA) and elevated CRP and between n-6 PUFA intakes (particularly linoleic acid LA) and elevated CRP The construction of dietary patterns specifically associated to nutrients having pro- or antiinflammatory properties (PUFA and antioxidant nutrients’ intakes) allowed us to show that a dietary pattern rich in essential fatty acids and antioxidant nutrients was negatively associated to elevated CRP in the long-term. These consistent results corroborate the importance of adequate intakes of PUFA and antioxidant nutrients in inflammatory mechanisms. However, dietary balance between these nutrients needs to be carefully considered, given the multiple interactions existing in the mechanisms they are involved in

Abstract:
To ensure that the resources of the health care sector are used effectively, new technologies need to be evaluated before implementation to examine if they generate health outcomes at an acceptable cost. This information can be collected by performing health economic evaluations in which the costs and health outcomes of different technologies are compared. To estimate the effect on health care budgets, there is also a need for information about the prevalence of the specific disease. Health outcomes in health economic evaluations are often measured in quality-adjusted life years (QALYs), which are calculated by multiplying the remaining life years after an intervention by a weight representing the health-related quality of life (HRQoL) during those years. This thesis aims to provide deeper knowledge of the health economic aspects of diabetic retinopathy (DR), an eye complication that affects patients with diabetes and may in the worst case lead to blindness. The focus is on three empirical and two methodological health economic research questions. The empirical research areas cover prevalence, costs, and HRQoL related to patients with DR. The methodological research questions explore the performance of different methods for estimation of QALY weights. This is of interest since it has been argued that the most common methods for estimating QALY weights may not capture all relevant vision-related aspects of quality of life. The analyses comprehend the validity of different methods for estimating QALY weights among patients with DR and if the results of one of the specific methods for estimating QALY weights, the time trade-off (TTO) exercise, are affected by patients’ subjective life expectancy (SLE). The empirical results demonstrate that DR is seen in approximately 40% and 30% of patients with type I and type II diabetes respectively, indicating that the prevalence of DR has decreased in both of these patient groups. Healthcare costs vary considerably between different severity levels of the disease, being estimated at €26, €257, €216, and €433 per patient per year for background retinopathy, proliferative diabetic retinopathy (PDR), diabetic macular oedema (DMO), and PDR combined with DMO respectively. Blindness due to DR is associated with an increased use of transportation services, caregiving services, and assistive technologies as well as productivity losses. This suggests that preventing the progression of DR may lower healthcare costs. Patients with vision impairment due to DR have lowered HRQoL in various dimensions, but the diagnosis of DR in itself has only a limited effect on HRQoL. The results on the methodological research questions show that different methods for estimating QALY weights seem to give different results. In comparison to EQ-5D, the Health Utilities Index Mark 3 (HUI-3) is the most sensitive method for detecting differences in QALY weights due to DR, and if decisions are to be made based on values from the general public, it can be recommended for use in cost-utility analyses of interventions directed at DR. Neither of the direct methods, TTO and the visual analogue scale, seems to be sensitive to differences in visual function, and more research is needed concerning the role of vision in people’s responses to the TTO exercises. In TTO exercises with time frames based on actuarial life expectancy, the patients’ SLE has an effect on their willingness to trade off years for full health. Thus, applying time frames deviating from patients’ SLE may result in biased QALY weights. Such bias may appear stronger within patient populations than within the general public. In conclusion, this thesis offers estimates for prevalence, costs, and QALY weights that can be used in economic evaluations of interventions directed at DR and as benchmarks for future DR research in order to follow up consequences of changes in diabetes care. In addition, it demonstrates that the choice of method for estimating QALY weights may have an impact on whether an intervention is considered cost-effective.

The results show that the presence of bicarbonate in the nutrient solution shifted the activity of PEPC and TCA cycle enzymes and the accumulation/translocation of organic acids in roots of Fe-deprived plants. The rootstock 140 Ruggeri displayed a typical behavior of calcicole plants under bicarbonate stress. The Fe chlorosis susceptible rootstock 101-14 reacted to a prolonged Fe-deficiency reducing the root activity of PEPC and MDH. Noteworthy, it accumulates high levels of citric acid in roots, indicating a low capacity to utilizing, transporting and/or exudating organic acids into the rhizosfere. In contrast, 110 Richter rootstock is capable to maintain an active metabolism of organic acids in roots, accumulating them to a lesser extent than 101-14. Similarly to 101-14, SO4 genotype displays a strong decrease of mechanisms associated to Fe chlorosis tolerance (PEPC and MDH enzymes). Nevertheless it is able to avoid excessive accumulation of citric acid in roots, similar as 110 Richter rootstock. Intercropping with Festuca rubra increased leaf chlorophyll content and net photosynthesis. In addition, intercropping reduces the activity of PEPC in roots, similary to Fe-chelate supply. Applications of NH4+ with nitrification inhibitor prevents efficiently Fe-deficiency, increases chlorophyll content, and induces similar root biochemical responses as Fe-EDDHA. The aspects intertwined in this investigation highlight the complexity of Fe physiology and the fine metabolic tuning of grapevine genotypes to Fe availability and soil-related environmental factors. Motivation and study design: Iron deficiency is a common condition in infancy, particularly in lower socio-economic groups. In Cape Town it remains a problem in spite of public health measures taken against it: a recent survey found a prevalence of iron deficiency anaemia of 34% in healthy 1-year old term infants who had ready access to a municipal health clinic where iron fortified milk formula is sold at subsidized prices. The consequences of iron deficiency extend beyond anaemia- to involve all organ systems including the immune system. Since Helen Mackay's report in 1928 of a striking decrease in incidents of infection in infants treated with iron, clinicians have assumed that iron deficiency predisposes to infection. Despite a sound theoretical basis for this belief, the clinical evidence for the assumption is poor as studies to date have displayed methodological deficiencies. On the other hand, iron is also essential for the growth of micro-organisms. As such, supplemental iron may predispose to infection. Indeed, there is much laboratory and clinical evidence to show that excess iron can result in the recrudescence of quiescent infections and increase the virulence of newly acquired infections. Thus, the competition between host and parasite may sometimes hinge on the relative availability of iron and it has been speculated that excess iron in infant milk formula may increase susceptibility to infectious diarrhoeal disease. The problem addressed by this thesis was to determine the utility of increasing the level of iron fortification of infant milk formula. Three questions were posed: Does increasing the level of iron fortification of conventional infant milk formula improve the iron nutrition of normal infants fed on the formula? Does increased iron fortification of infant milk formula alter immunity as reflected by incidence of infection and laboratory tests of immune function? Are there any handful effects of increasing the quantity of iron in conventional infant milk formula? A double blind randomized trial was carried out in 1983 and 1984 to answer these questions. A group of 149 healthy, well-nourished infants from a lower socio-economic community of so called Cape Coloureds were followed from the age of 3 months to 1 year. Half of the infants, the Control group, were given a commercially available infant milk formula (Lactogen Full Protein) which has 8.3 mg Fe/ 100 g formula and 37 mg ascorbic acid/ 100 g. The other half of subjects, the Test group, were given the same milk formula but fortified with iron to a concentration of 40 mg Fe/ 100 g. The children were examined every 3 or 4 weeks and any infection or history of infection was noted. Laboratory tests were done at the start of the trial and again on completion. During the trial, laboratory tests were performed only if clinically indicated. The tests included full blood count and differential analysis, red cell zinc protoporphyrin, plasma ferritin, plasma and hair zinc and lymphocyte subtyping with monoclonal antibodies. Within each group, half of the infants were randomly selected for assay of neutrophil bactericidal activity. The other half were assayed for lymphocyte blastogenic response to stimulation with phytohaemagglutinin. Tests of delayed cutaneous hypersensitivity to Candida antigen and PPD were done and all children and their mothers had antibodies to tetanus and polio determined. Results: 74 infants in the Control group started the trial and 62 completed it. In the Test group, 75 infants began and 70 completed the study. Intake of milk and solid foods was not quantified, but the ages of weaning and of introduction of new foods were determined. The Control and Test groups did not differ significantly on any test item. The mean age of completion of weaning was 3.60 months for the Control group and 4.04 months for the Test group. The Control group was first given meat or fish at a mean age of 5.19 months; the Test. group had meat or fish introduced to their diets at a mean age of 4.36 months. These differences were not statistically significant. The children in the Control group were lighter and shorter than the Test group at the end of the year. Mean standard deviation scores for weight were 0.23 and 0.48 respectively (P = 20%), while for length the SD scores were -0.13 and 0.06 (P = 20%).

Abstract The aim was to determine the association between baseline factors, such as oral health-related behavior, attitudes, knowledge and beliefs in relation to caries increment during a randomized clinical trial (RCT). A further aim was to evaluate treatment costs and health outcomes during and after the RCT. In Pori Finland, 11- to 12-year-old children with active initial caries lesion(s) participated in the RCT in 2001–05. The experimental group (n=250) received multiple measures for controlling caries. The control group (n=247) received standard dental care. In 2005–08, all received standard dental care. Regression analyses were used to study the associations between behavioral factors and caries increment. Cost-effectiveness analysis was conducted (trial period), and costs and health outcomes as well as dental service utilization were evaluated (post-trial period). In the experimental group, brushing teeth twice a day was indicative of developing no new caries lesions, whereas eating candy at least once a day, predicted new lesions. In the experimental and control groups, lack of concern about cavities and lack of knowledge about mother’s dental health predicted new caries lesions. The average incremental cost for averting one DMF surface was €34. The experimental regimen was more effective and more costly than the standard dental care. The total costs decreased year after year. The mean total cost per adolescent was lower and the clinical outcome was better among the former participants in the experimental group. The utilization of dental services was significantly more regular among the former participants in this group. It may be feasible to control caries more effectively by affecting toothbrushing, candy eating and oral health-related attitudes, as preventive procedures may be ineffective if those factors are not in order. It is important to discuss oral health-related topics in families, because this may improve the oral health-related behavior of children. Cost-effectiveness of regimen used for the experimental group may be improved by division of work or by selective reduction of preventive procedures. Well-timed caries control can decrease treatment cost and yield long-term improvement of dental health
Tiivistelmä Tutkimuksen tarkoituksena oli selvittää satunnaistetun kliinisen kokeen alussa rekisteröityjen suunterveyteen liittyvien tapojen, tietojen, asenteiden, uskomusten ja karieskertymän välisiä yhteyksiä. Lisäksi arvioitiin hoitokustannuksia ja hoidollisia tuloksia sekä kokeen ajalta että kokeen jälkeiseltä ajalta. Ne 11–12-vuotiaat lapset, joilla oli ainakin yksi alkava aktiivinen kariesvaurio, osallistuivat kokeeseen Porissa vuosina 2001–05. Koeryhmän lapset (n=250) saivat tehostettua ehkäisevää hoitoa ja kontrolliryhmän lapset (n=247) tavanomaista hammashoitoa. Kaikki saivat tavanomaista hammashoitoa vuosina 2005–08. Käyttäytymisellisten tekijöiden ja karieskertymän välisiä yhteyksiä tutkittiin regressioanalyysien avulla. Kustannusvaikuttavuusanalyysi tehtiin kokeen ajalta. Hoitokustannuksia ja hoidollisia tuloksia sekä palveluiden käyttöä arvioitiin kokeen jälkeiseltä ajalta. Lapsilla, jotka harjasivat vähintään kaksi kertaa päivässä, oli yleensä ehjät hampaat, kun taas lapsilla, jotka söivät päivittäin makeisia, oli useasti reikiä. Huolettomuus reikiintymistä kohtaan ja tietämättömyys äidin hampaiden kunnosta näkyi lasten hampaiden reikiintymisenä. Yhden hammaspinnan säästyminen paikkaukselta maksoi keskimäärin 34 €. Koeryhmän saama hoito oli vaikuttavampaa, mutta kalliimpaa kuin kontrolliryhmän saama hoito. Kokonaiskustannukset laskivat vuosi vuodelta. Keskimääräiset hoitokustannukset olivat pienemmät ja hammasterveys parempi entisen koeryhmän jäsenillä kuin kontrolliryhmäläisillä. Myös palveluiden käyttö oli säännöllisempää koeryhmässä. Karieksen hallintaa voidaan todennäköisesti tehostaa vaikuttamalla hampaiden harjaukseen, makeisten syöntiin ja suunterveyteen liittyviin asenteisiin. On tärkeää varmistaa, että nämä asiat ovat kunnossa, koska ehkäisevät toimenpiteet saattavat muuten jäädä tehottomiksi. Suunterveyteen liittyvistä asioista olisi hyvä keskustella perheissä, koska asioiden esillä pitäminen voi vaikuttaa suotuisasti lasten terveystapoihin. Koeryhmän saaman hoito-ohjelman kustannusvaikuttavuutta voisi todennäköisesti parantaa muuttamalla suunterveydenhuollon henkilöstön työnjakoa tai karsimalla valikoiden ehkäisevien toimenpiteiden määrää. Oikea-aikainen karieksen hallinta voi vähentää hoitokustannuksia ja lisätä hammasterveyttä pitkällä aikavälillä

In recent years the spatial dimension of economic development has attracted increasing interest in the development field. However there is still little analysis and evidence of the ways many spatial dimensions interact with other economic dimensions in the development process. This thesis aims to help filling this gap by bringing a geographical perspective into development economics frameworks. It is empirical in nature and uses data on different sub-national units from India and Uganda. The work is structured around four main papers (divided into six chapters). The first paper analyses two important aspects of the Indian urbanisation process. First it finds a U-shaped relation between rural-urban disparities in living standards and income per capita across Indian states in the Post-Independence period. Second, it shows that the urbanisation process in India has been characterised by convergence in the 20th century: smaller towns grow faster than large ones. The second paper examines the role of the agricultural sector in influencing the shape of the urban system. The analysis suggests that the elasticity of rural-urban labour supply increases both urban primacy and the urbanisation rate in Indian states during the Post-Independence period. The third paper tests for the impact of urban growth on rural poverty using a sample of Indian districts in the period 1981-1999. It finds that urbanisation reduces poverty surrounding rural areas. This effect is largely attributable to positive spillovers from urbanisation rather than to the movement of the rural poor to urban areas. The final paper examines the deteminants of rising returns to schooling in Ugandan districts during the 1990s. The findings suggest that both educational supply and demand factors influenced the wedge between skilled and unskilled labour. Moreover while trade opening reduced this wedge, pro-market reforms increasing inter-district trade raised returns to education in districts relatively abundant in skilled labour.

L’objectif de cette thèse est d’explorer plusieurs phénomènes liés à la migration en prenant en considération différents aspects de la réalité économique contemporaine : l’importance du secteur non-échangeable, l’asymétrie entre les flux migratoires et les flux des investissements, ainsi que les problèmes persistants entre différentes communautés ethniques. Dans le premier chapitre introductif, nous explorons la littérature sur la politique économique de l’immigration et nous étudions les différentes voies par lesquelles les immigrés peuvent affecter le bien-être des résidents domestiques. Dans la deuxième partie, nous développons un cadre théorique afin d’analyser les effets de l’immigration sur le bien-être individuel dans une petite économie ouverte avec le secteur non-échangeable. Nos résultats expliquent pourquoi les résidents domestiques sont généralement opposés à l’immigration peu qualifiée et favorisent l’influx des immigrés hautement qualifiés. Dans le chapitre trois, nous faisons une extension du modèle élaboré dans le chapitre deux, en prenant en compte les flux internationaux du capital. D’abord nous cherchons à décrire le lien entre la migration peu et hautement qualifiée et les investissements directs à l’étranger. Puis, nous analysons le changement dans les attitudes envers l’immigration suite à l’introduction de la mobilité internationale du capital. Dans le quatrième chapitre, nous démontrons comment la diversité ethnique peut affecter les intentions d’émigrer. Nous traitons le cas de la Lettonie où les minorités ethniques constituent 40% de la population. Nous pouvons constater que les individus appartenant aux minorités ethniques sont plus probables d’émigrer et que cette probabilité augmente avec le revenu. Les individus appartenant à la majorité ethnique, au contraire, sont plus probables d’émigrer si leurs revenus sont plus bas
The objective of this thesis is to contribute to a better understanding of migration-related economic issues in the world today. We concentrate both on immigration and emigration and at various stages of our work address all three parties involved in migration process : people hosting immigrants, people left behind and the migrants themselves. We account for several important features of today’s rapidly globalising life : the importance of the non-traded sector, asymmetry between capital and labour flows, and persisting problems between ethnic communities. The first chapter in an overview of the political economy of immigration literature and addresses the multiple ways in which immigrants may affect natives’ welfare. In particular, we discuss the role of economic and non-economic arguments in shaping immigration attitudes and summarise main labour market and welfare-state effects of immigration. Chapter two develops open economy with a non-traded sector. Our finding provide additional understanding of why native population is generally opposed to low-skilled immigrants and favouring high-skilled foreign workers. The third chapter extends the model developed in chapter two to accommodate internationally mobile capital. First, we investigate whether immigration of high-skilled and low-skilled labour leads to positive or negative FDI. Then, we find out how would immigration attitudes change if a country allows international capital movements. Chapter four investigates how ethnic diversity at home may influence emigration intentions of an individual. We explore the case of Latvia where ethnic minorities constitute 40% of the population. We find that ethnic minorities are more likely to emigrate and are positively self-selected on the basis of income, while the opposite is true for ethnic majority population

The description presented reaches both ends of the distribution chain: from how the video signals are acquired and processed, thru all the way to how they are delivered to the client, passing by the challenges and consequences that such services have on the network. Abstract:
This thesis investigates the impact of School Nutrition and Health programs implemented by the Government in Public primary schools in India. Section A focuses on evaluating the National Program of Nutritional support to Primary Education launched in 1995. Under this scheme, children enrolled in government primary schools received 3 kilograms of food grains per month, free of cost, conditional on enrolment and a minimum attendance requirement. In chapter 1, we provide a detailed survey of the related literature, highlighting the multi-dimensional impacts of these programs on educational and health outcomes. In Chapter 2, we evaluate the impact of the School feeding program (SFP) in India on primary school starting age and enrolment using the National sample survey. We adopt two methodological frameworks to estimate the program impact, namely, a difference-in-differences (DID) technique and duration analysis. The findings indicate that the program was effective in increasing enrolment and encouraging children to start school at the stipulated entry age. In chapter 3, we study the impact of the SFP in India on primary school completion using the District Level Household survey. Using the DID methodology, we find that the program had a positive effect on primary school completion, with differential effects by gender and years of program exposure. Additionally, we identify whether the program generated positive educational externalities between siblings in the family. Section B of this thesis evaluates a complementary policy, The School Health Program implemented in Government primary schools in Karnataka, India. The program provided free health services to students in public schools, consisting of- micronutrient supplements, deworming treatment and regular health screenings by Doctors at the school premises. We investigate whether this program was effective in improving pupils’ educational and health status. Using administrative data on student’s academic and health records collected from public schools, we find that the program led to an increase in school participation measures and academic performance, with heterogeneous effects across subjects and performance distribution. The program impacts on anthropometric indicators are positive, but statistically insignificant for both boys and girls. We conclude that School Nutrition and Health programs are extremely beneficial in a developing country context to improve children’s educational and health status, by lowering schooling costs and by providing parents with incentives to send their children to school. These programs have the potential to improve future welfare and quality of life, through increased educational attainment and improved health and nutrition.

Abstract:
This thesis provides one of the first (to the author's knowledge, the first) micro-economic analyses of audit regulation and auditor liability. The analysis draws on insights from the economics and law literature that liability and regulation affect behaviour of individuals and organisations. The major research questions addressed in the thesis are the following: 1) How is demand for external audit services affected by joint and several liability of directors and external auditors? 2) How do auditor liability and professional audit standards affect audit quality? 3) Is it in the public interest to use auditor liability and professional audit standards jointly to monitor audit quality? The analysis is general, in the sense that a number of alternative regulatory scenarios are considered, and therefore hopes to be of relevance to various legal environments. Propositions about audit demand and production behaviour are drawn, as well as corollaries about the welfare implications of audit regulation and liability. Some major conclusions from the economic analysis are the following. 1) Consistency in judicial reasoning should be promoted. Certainty about what constitutes 'due care' leads to compliance by directors and auditors. 2) Uncertainty about due care crucially affects behaviour, both of auditors and directors. 3) Liability insurance arguments are irrelevant for audit demand when the due care level for directors is fairly certain. 4) Statutory audit requirements should only be imposed under limited circumstances. 5) More prescriptive professional audit standards have a positive effect on audit quality, but one standard for all client situations can never lead to social efficiency. 6) Liability restriction has a negative effect on audit quality. It may however promote socially efficient behaviour when there is overproduction of audit quality. 7) The joint use of liability restriction and more prescriptive professional audit standards may lead to a status quo in terms of audit quality produced, and therefore not welfare improving.
