Abstract:

The edentulous jaw is a common feature in elderly patients that had lost their teeth during life due to local reasons such as poor oral hygiene and dental caries as well as periodontal disease. Hence these patients are the victims of biological phenomenona that lead to difficulties in different aspects of patient comfort with dentures. Clinicians are always concerned to minimize these difficulties and increase patient comfort through manufacturing a proper prosthesis to substitute for the loss of the natural teeth as well as the surrounding structures for optimum satisfaction and improved quality of life of the patient. The aim of this study was to evaluate patient satisfaction regarding function and aesthetics with conventional mandibular dentures and implant retained mandibular overdentures in denture wearers.

Self-perception of potential outcomes of prosthodontic treatment, including benefits, risks and consequences of not replacement of missing teeth, were studied in edentulous patients. Initially, using a qualitative approach, it was developed a instrument containing 41 affirmatives that comprise a scale and sub-scales that measure subject’s perception score in a 5-point Likert-type scale (1=strongly disagree; 2=disagree; 3=neutral; 4=agree; 5=strongly agree). Subsequently 126 partially or totally edentulous subjects were interviewed. Mean age was 51.8-years (SD=12.3), 74% female. Cronbach’s alfa coefficient of scale and sub-scale ranged from 0.70 and 0.90. Influence of age, gender and clinical variables were tested using analysis of variance and independent t-test. Sum of scores for each subject ranged from 123 to 198, mean score was 173.8 (SD=14.8). Subjects tended to agree with the proposed affirmatives (scores 4 and 5). Considering the total scale, lower scores were obtained by younger (p=0.033), male (p=0.019) and subjects with a single missing tooth (p=0.004). Using sub-scale of benefits, with single missing teeth subjects ranked lower scores (p=0.021). No variables showed significant relationship with perception scores using risks sub-scale. When considering sub-scale that measures perception of consequences of not replacement, lower scores were obtained by males (p=0.033), single missing tooth (p=0.018) and non wearers of prosthodontics (p=0.000). It was concluded that perception of potential outcomes of prosthodontic treatment was influenced by clinical variables. Older subjects, female, subjects with grater number of missing teeth and that have been previously treated have greater perception of benefits and risks of prosthodontic treatment.
Foi avaliada em indivíduos desdentados a percepção de desfechos potenciais relacionados a benefícios e riscos do tratamento e conseqüências do não tratamento protético. Inicialmente foi desenvolvido um instrumento com 41 afirmativas que compuseram as escalas e sub-escalas com categorias ordinais tipo-Likert de 5 pontos (1=discordo totalmente; 2=discordo; 3=neutro; 4=concordo; 5=concordo totalmente). A seguir foram entrevistados 126 indivíduos desdentados parciais ou totais, usuários ou não de prótese, idade média de 51,8 anos (DP=12,3), sendo 74% do sexo feminino. O coeficiente alfa de Cronbach da escala e sub-escalas variou entre 0,70 e 0,90. A influência da idade, sexo e dados clínicos foram avaliados através de análise de variância e teste t não pareado. A soma dos escores dos 41 itens para cada indivíduo variou entre 123 e 198, média de 173,8 (DP=14,8), com valor mínimo de 41 e valor máximo de 205. A proximidade da soma dos escores com valores máximos indicou tendência dos indivíduos em concordar com as afirmativas da escala (escores 4 e 5). Na escala total os menores escores foram para indivíduos jovens (p=0,033), do sexo masculino (p=0,019) e com espaços protéticos unitários (p=0,004). Na sub-escala de benefícios os menores escores foram para os desdentados unitários (p=0,021). Nenhumas das variáveis foram significativas para a sub-escala de riscos. Na sub-escala de conseqüências da não reposição, os menores escores foram para os homens (p=0,033), desdentados unitários (p=0,018) e não usuários de prótese (p=0,00). A percepção individual dos desfechos do tratamento protético foi influenciada por variáveis clínicas, sendo mais alta em indivíduos com maior idade, mulheres, maior número de dentes perdidos e usuários de prótese.

Abstract:
Objectives: The aim of this study is to investigate the impact of multiple factors in the Oral Health Related Quality of Life of patients being treated with Conventional Complete Dentures, including: a) Level of expertise of the dental provider (third year dental student vs. fourth year dental student); b) Length of time edentulous; c) Number of previous prostheses; d) Race; e) Sex; f) Age; g) Systemic conditions; h) Condition of the bone ridge; i) Economic factor and insurance coverage, in patients at the University of Iowa College of Dentistry (D3 and D4 Clinics). Materials and methods: Eighty-one subjects who had been completely edentulous (49 men and 32 women; age range from 23 to 93 years old, mean of 58.9+13) for a minimum of 3 months were recruited. Out of the eighty-one, eleven subjects were not included in the final analysis because their prostheses were not delivered due to various reasons (time, death, discontinued treatment). Subjects were asked to complete the OHIP-EDENT (Oral Health Impact Profile for Edentulous Patients) and the patient satisfaction questionnaire to record data at baseline, at 1-week post-delivery and at 1-month post-delivery of the new prostheses. Additional to these two instruments, the PDI Classification instrument (to assess bone/tissue condition) and the Kapur Index (to assess retention/stability of the new prostheses) were also used. Baseline comparisons of subjects seen at the D3 vs. the D4 clinic were carried out using chi-square tests of association for nominal ordinal variables, and the Wilcoxon-Mann-Whitney test for ordinal and quantitative outcomes. Bivariate analyses were conducted to assess potential relationships between covariates and the primary outcomes (total OHIP-EDENT and total Patient Satisfaction Questionnaire scores at one-week and one-month follow-ups). The Spearman rank correlation was used to identify increasing or decreasing relationships between primary outcomes and quantitative and ordinal variables; the Wilcoxon-Mann-Whitney test was used to assess relationships between primary outcomes and categorical covariates. Results: Overall, there was not enough statistical evidence to conclude that patient satisfaction and quality of life were significantly affected by most of the factors being studied. However, there was suggestive evidence of the presence of higher level of quality of life for patients seen in the D4 clinic (students with higher level of expertise). At the one-month follow-up, the comparison of the total OHIP-EDENT score between the D3 and D4 clinics was significant at the 0.05 level (p=0.018, exact Wilcoxon-Mann-Whitney test). The total OHIP-EDENT scores tended to be lower (indicating greater satisfaction) in the subjects seen in the D4 clinic. Two of the nineteen items from the patient satisfaction questionnaires also suggested a greater level of patient satisfaction for patients seen in the D4 clinic (students with higher level of expertise). Conclusion: 1) Oral health related quality of life at 1-week follow-up was not significantly different between patients treated by D3 and D4 students. 2) Oral health related quality of life at 1-week follow up was significantly better for patients who had been edentulous for a longer period of time than those who were recently edentulous. However, this result can only be considered suggestive due to the need for adjustment for multiple comparisons. 3) Oral health related quality of life at 1-month follow up was significantly better for patients treated by D4 students in comparison to D3 students. However, this result can only be considered suggestive due to the need for adjustment for multiple comparisons. 4) Overall, patient satisfaction at 1-week follow-up was not significantly different between patients treated by D3 and D4 students. However, patients treated in the D4 clinic reported significantly better outcomes in regard to speaking with their new prostheses. 5) Overall, patient satisfaction at 1-month follow-up was not significantly different between patients treated by D3 and D4 students. However, patients treated in the D4 clinic reported that they could chew better with their new prostheses compared to those treated by D3 students.

Les facteurs déterminants des caractéristiques biomécaniques chez la mandibule humaine sont les propriétés des tissues et sa géométrie externe et interne. Des procédures non-invasives pour prédire de façon précise, les contraintes des mandibules soumis aux stress mécaniques sont importantes dans des diverses situations en Odontologie. Les modèles éléments finis (EF), montrent une bonne capacité d’évaluer les relations géométriques, mécaniques et fonctionnelles sur la mandibule. Malgré les progrès récents, la conception de modèles numériques présente encore des limites, liées au manque de connaissances des propriétés mécaniques de tous les tissus, à la conception intégrale du modèles et la possibilité de personnalisation de ceux-ci en tenant compte de la variabilité anatomique, morphologique, physiologique et biologique des individus. Le présent travail vise à définir un modèle d'éléments finis de la mâchoire pouvant être modélisé en fonction des conditions anatomiques et morphologiques particulières d'un individu, de manière à prédire avec précision les réponses mécaniques et biologiques de la mandibule. La définition des paramètres pertinents à l’obtention des modèles de simulation, permettra une évaluation in silico des réponses spécifiques chez les patients aux traitements pratiqués dans le domaine dentaire, en tenant compte de la variabilité morphologique de la mandibule associée aux pertes dentaires. L’objectif de cette recherche est la parameterisation géométrique et mécanique d’un modèle EF d’une mandibule, intégrant les particularités anatomiques d'un individu et permettant de prédire, avec précision, les réponses mécaniques vis-à-vis des traitements dentaires
The determining factors of biomechanical characteristics in the human mandible are the properties of the tissues and its external and internal geometry. Non-invasive procedures to predict accurately the mandible stress and strains under mechanical loads are important in a variety of dental situations. The finite element models shows a good ability to evaluate the geometric, mechanical and functional relations on the mandible. Despite recent advances, the design of numerical models still has limitations: the lack of knowledge of the mechanical properties of all tissues, the integral design of the models and the possibility of personalization of these, taking into account the anatomical, morphological, physiological and biological variability of individuals. The present work aims to define a model of finite elements of the jaw that can be modeled according to the particular anatomical and morphological conditions of an individual, in order to accurately predict the mechanical and biological responses of the mandible against loads or mechanical stimuli that affect them. The definition of the parameters relevant to obtaining the appropriate simulation models will enable an in silico assessment of any specific patient responses to dental treatments, in particular in dental implantology, taking into account the morphological variability of the mandible with dental losses. . The objective of this research is the geometrical and mechanical parametrization of an EF model of a mandible, integrating the anatomical particularities of an individual and allowing predicting with precision, the mechanical responses to dental treatments

Despite implant-supported overdentures' advantages over conventional dentures, edentulous patients often refuse dental implants for reasons that may be related to fear. Objective. The purpose of this study was to uncover and explain the various forms and sources of dental implant-related fear among edentulous patients. Methods. A convenient sample of 8 edentulous/semi-edentulous participants was used. Participants were recruited from a current study at McGill University's Faculty of Dentistry, posting of recruitment flyers, and local homes and community centers for seniors. One-on-one interviews were conducted in Montreal from March 2005 to February 2006, transcribed verbatim, and coded into multiple theme-based sections. Results. Three categories of implant-related fear were identified: (1) fear of dental implant surgery; (2) fear of the recovery period; and (3) miscellaneous fears. As well, four main sources of dental implant-related fear were uncovered: (1) past negative experiences at the dentist; (2) "horror stories" of dental implant surgery; (3) having relatively little knowledge of dental implants; and (4) poor dentist-patient relationship. Discussion. The various forms of dental implant-related fear seem to play a substantial role in influencing a patient's decision to undergo implant surgery.

Introduction: Forensic odontology is a valuable tool in human identification processes. Antemortem radiographic examinations make it possible to use postmortem examinations for comparison.  Aims: The overall aim was to determine the possibility to identify edentulous persons using panoramic examinations by I) investigating the possibility to match two panoramic radiographs of one person performed at two different occasions, II) determining what anatomical features are used as the base for matching III) investigating if there is a difference between oral and maxillofacial radiologists (OMR) and non- oral and maxillofacial radiologists (NOMR) in the ability to match the images, IV) determining if the time elapsed between the images to be compared affect the results or the confidence in the match. Materials and Methods: Panoramic image pairs from 19 patients examined twice at different occasions, plus 10 images from other edentulous patients comprised the material. The time elapsed between the image pairs varied between four months andsix years. Four OMR and four NOMR matched image pairs depicting the same patient. The participants marked each match as “certain”, “likely” or “possible” and what anatomical structure that was used for matching.  Results:The OMR group had 100% correct matches and the NOMR group had 96%. The anatomy of the mandible was most used for matching. The OMR group was more certain in their decisions than the NOMR group. The time elapsed between the examinations did not affect the result or the confidence in the matches. Conclusion: Panoramic images can be helpful when identifying edentulous patients.

This study explored the oral pathological conditions and related normal aberrations found within an edentulous sample group of the Western Cape of South Africa. The initial literature review concentrated on similar epidemiological surveys from a national and international perspective, from which a research framework was formulated and then applied to the chosen sample size. Statistical analysis revealed the mean age of the sample group as 57,3 +1- 1 years and that the male, female ratio was 1:2.16. Results indicated that the percentage of healthy individuals were 78%. Normal aberrations such as mobile alveolar ridge (flabby ridge) (2,6%), oral mucosal pigmentation (1,9%), chronic cheek chewing (1 %), torus mandibularis and palatinus (0,5%) had the highest incidence. Twenty three percent exhibited some sort of oral pathological condition such as denture stomatitis (7%), ulcerations (5,2%), angular cheilitis (1,8%), white lesions (4,4%), and denture-related hyperplasia (1,9%) were the most prevalent. Systemic diseases were recorded in 38,2% of patients: hypertension (22,7%), heart disorders (6,6%), diabetes mellitus (6,5%) and asthma (4,4%) were the most common. Individuals that exhibited oral pathoses were found to be wearing older dentures than individuals who replace their dentures more regularly. No significant difference was found between the age of the denture and the age of the patient. The empirical investigation revealed that the number of prominent oral conditions was low for the Western Cape of South Africa and could perhaps be incorporated into an educational module for dental technologists. A better understanding of such oral pathological conditions may aid communication and patient service between the parties involved. The concept of a more collaborative approach between the dentist and the dental technologists was discussed. Reference is made to a community service model that focuses more specifically on the needs of the poorer edentulous individuals of the Western Cape.

La colocación de implantes dentales para rehabilitar pacientes con edentulismos parciales o totales se considera un procedimiento rutinario con excelentes porcentajes de éxito a largo plazo. La alta predictibilidad de estos resultados se basa en una secuencia de tratamiento protocolizada, que comprende una fase de diagnóstico y planificación, una fase quirúrgica y una fase restauradora. La fase de diagnóstico preoperatorio implica un detallado análisis protésico, clínico y radiográfico. En este momento, se realiza un set-up protésico que permite predecir el contorno de la futura reconstrucción protésica con el fin de determinar la posición ideal del implante. Este set-up se transforma en una férula radiológica que el paciente porta durante el examen radiográfico. La imagen resultante incorpora la referencia protésica a la imagen radiográfica y permite, conjuntamente con la información obtenida de la exploración clínica, el estudio de la disponibilidad ósea, la identificación de estructuras anatómicas relevantes, y la determinación de la posición y dimensiones del implante a colocar. Posteriormente, el clínico intenta transferir la posición planeada al escenario clínico con la ayuda de una férula quirúrgica convencional, que transfiere la información protésica al campo operatorio.
The present investigation has tried to analyze and compare two implant planning and placement treatment concepts from a holistic approach. Conventional and, more frequently, computer-assisted implant planning and placement protocols have been repeatedly investigated in terms of implant placement accuracy. However, the implant placement precision is only one of the relevant aspects when evaluating an implant planning and placement procedure. Other aspects such as treatment’s predictability, complications or unexpected events derived, treatment outcomes, patients’ perception, influence on the patient’s quality of life, or time and economic costs derived, are identically relevant when assessing the treatment’s value. Therefore, the aim of this study was to investigate all of these parameters in order to quantify the treatments’ efficiency.

Resumo: Estudos revelam melhora na mastigação após o uso de próteses sobre implantes osseointegrados. Entretanto, poucos são aqueles que avaliam a mastigação de pacientes parcialmente edêntulos, comparando a função mastigatória após a reabilitação por meio de diferentes próteses parciais, o qual foi o objetivo deste ensaio clínico pareado. Para tanto, foram selecionados 12 voluntários (8 gênero feminino, idade média 62.6 ± 7.8 anos), apresentando edentulismo total superior e parcial inferior (classe I de Kennedy), os quais receberam, de forma sequencial, próteses parciais removíveis (PPR), PPR com encaixe implanto-retido (PPRI) e prótese parcial fixa sobre implantes (PPFI). Todos os tratamentos foram realizados na mandíbula e utilizados por 2 meses antes da avaliação mastigatória, enquanto a maxila recebeu uma nova prótese total que foi utilizada durante todo o estudo. A mastigação foi avaliada por meio da mensuração da performance mastigatória (PM), índice de trituração dos alimentos (ITA), força máxima de mordida (FMM), espessura dos músculos masseter e temporal, movimento mastigatório, limiar de deglutição (LD), habilidade mastigatória, estado nutricional, qualidade de vida (QV) e satisfação do paciente. Foi realizada a análise exploratória dos dados e aplicada análise de variância para medidas repetidas seguido de teste de Tukey-Kramer para as comparações múltiplas entre os tratamentos. Todas as análises foram realizadas utilizando SAS software (release 9.1, 2003; SAS Institute Inc., Cary, USA) (p ? 0.05). A PM melhorou consideravelmente após o uso de PPRI e PPFI com aumento de 85% e 87%, respectivamente. Resultados similares foram observados em relação ao ITA e à FMM com aumento (p ? 0.0001) de 91% e 62% no ITA de 79% e 62% na FMM após o uso de PPRI e PPFI, respectivamente. Independente do tipo de prótese, o uso de PPRI e PPFI aumentou a espessura do masseter durante a contração voluntária máxima (p ? 0.0001) e alterou o movimento mastigatório, reduzindo o tempo total do ciclo, bem como a duração das fases de abertura e fechamento (p ? 0.05). A habilidade mastigatória melhorou após o uso da PPRI e PPFI, independente do alimento avaliado. O LD foi alterado pelo tratamento reabilitador, com redução no número de ciclos e tamanho da partícula triturada, sendo os menores valores observados com o uso da PPFI. Houve aumento no consumo de fibras (p = 0.007), cálcio (p = 0.001) e ferro (p = 0.02) após o uso de PPFI, além da redução no consumo de alimentos com altos níveis de colesterol (p = 0.02). A satisfação aumentou (p ? 0.05) e o impacto da saúde oral na QV reduziu, tanto no score geral (p = 0.04) quanto no domínio de dor física (p = 0.02) após o uso da PPFI. A reabilitação de pacientes parcialmente edêntulos com PPRI e PPFI melhorou significativamente a função mastigatória e a magnitude do efeito relacionou-se ao tipo de prótese
Abstract: Studies revealed the masticatory improvement after the use of prosthesis over dental implants. However, few are those who evaluated the chewing of partially edentulous patients, comparing the masticatory function after treatment with different partial dentures, which was the aim of this paired clinical trial. Therefore, 12 volunteers (8 females, mean age 62.6 ± 7.8 years) presenting total edentulism in maxilla and partial edentulism in the mandible (Kennedy class I) were selected and received, in a sequential way, a conventional removable partial dentures (RPD), implant-retained partial dentures (IRPD) and implant fixed partial denture (IFPD). All treatment were assembled in the mandible and used for 2 months, while the edentulous maxilla received a new complete denture which was used throughout the study. Mastication was assessed by measuring masticatory performance (MP), food comminution index (FCI), maximum bite force (MBF), masseter and temporal muscle thickness, chewing movements, swallowing threshold (ST), masticatory ability, nutritional status, quality of life (QOL) and patient satisfaction. Data were analyzed and repeated measures analysis of variance was applied followed by Tukey-Kramer multiple for comparisons between treatments. All analyzes were performed using SAS software (release 9.1, 2003, SAS Institute Inc., Cary, USA) (p ? 0.05). MP greatly improved after IRPD and IFPD use with an increase of 85% and 87% respectively. Similar results were observed in respect to FCI and MBF with an increase (p ? 0.0001) of 91% and 62% in FCI and of 79% and 62% in MBF after the IRPD and IFPD use, respectively. Regardless the prosthesis type, the use of IRPD and IFPD increased the masseter thickness during maximum voluntary clenching (p ? 0.0001) and altered the chewing movements, reducing the total cycle time, as well as the duration of opening and closing phases (p ? 0.05). MA improved after IRPD and IFPD use, irrespectively of the food rated. ST was affected by prosthetic treatment, showing a reduction in the number of chewing cycles and in the size of the comminuted particle, with the lowest values observed after IFPD use. There was a raise in fiber (p = 0.007), calcium (p = 0.001) and iron (p = 0.02) intake after the IFPD use and a reduction in the intake of food with high cholesterol levels (p = 0.02). Patients satisfaction also increased (p ? 0.05) and the impact of oral health on QOL decreased in overall score (p = 0.04) and in the physical pain domain (p = 0.02) after the IFPD use. The rehabilitation of partially edentulous patients with IRPD and IFPD significantly improved masticatory function and the magnitude of the effect was related to the prosthesis type

Resumo: A necessidade de reabilitação oral gera o desenvolvimento de inúmeras pesquisas com o intuito de inovações nesta área. O aumento da expectativa de vida, a busca pelo conforto e saúde são fatores primordiais para os aprimoramentos dos novos dispositivos de reabilitação oral. A atual pesquisa trata do desenvolvimento de um dispositivo de titânio intra-oral visando a reabilitação oral em pacientes com regiões ósseas atróficas. Para tanto foram realizados: a obtenção e tratamento da tomografia de um caso adequado; a criação do modelo virtual da mandíbula e do dispositivo; a confecção destes pelo processo de manufatura aditiva; ensaios de flexão e estudos in vitro do material Os valores de resistência à flexão do material foram superiores ao da região mandibular de um humano saudável. A análise do resultado do ensaio de citotoxicidade do titânio Ti-6Al-4V evidencia as características deste de ser um biomaterial. A análise dos resultados dos ensaios, mecânicos e do estudo in vitro, viabiliza a aplicação da liga de titânio Ti-6Al-4V no desenvolvimento deste dispositivo para reabilitação oral. O dispositivo se adapta perfeitamente ao protótipo da mandíbula, permitindo que posteriormente se torne justaposto ao osso e seja fixada a parte protética. Desse modo, o dispositivo desenvolvido possui características adequadas para se tornar uma opção factível para pacientes com regiões orais atróficas posteriores
Abstract: Research and new techniques are a real needed for a better treatment in oral rehabilitation. The increase of life expectancy leads to a search for comfort and health, Are fundamental for the development of devices that may provide an increase in life quality. The scope of this research is the development of a titanium device for intra-oral use that may allow oral rehabilitation in patients with atrophic and extended areas of bone lost. This research aims the device development which will give oral rehabilitation solution to cases where absences of vertical bone quantity does not allow the placement of traditional implants. In this study, were made the gathering and processing of the tomography of an appropriate case, the manufacture of the device by the virtual model and by additive manufacturing, the mechanical flexural test and cytotoxicity study where performed. The results presented show viability for the application that was studied. The flexural strength of the biomaterial was higher than the mandible region of a healthy human and the choice of the materials (titanium Ti - 6Al -4V) did not show any characteristic of cytotoxicity. The developed device is suitable as a prototype of the mandible, allowing subsequent juxtaposition to the bone and be is fixed in the prosthetic part. Thus, the developed device has suitable characteristics to become a feasible option for patients with atrophic oral posterior regions

Resumo: O conceito das desordens temporomandibulares (DTM), as define como um conjunto de alterações articulares e musculares na região orofacial, caracterizados principalmente por dor, ruídos nas articulações temporomandibulares (ATM) e alteração da função mandibular, podendo estar associada as alterações cervicais oriundas das Desordens Crânio Cervicais. Estudos eletromiográficos demonstraram que mudanças na posição de cabeça podem alterar o padrão de atividade dos músculos da mastigação e a posição da mandíbula. Em função destes aspectos, o objetivo deste trabalho foi avaliar a atividade eletromiográfica dos músculos supra-­? hioideos, esternocleidomastoide, trapézio, temporal anterior e masseter em 15 voluntários desdentados totais, com dimensão vertical de oclusão (DVO) baixa, portadores de DTM submetidos a terapia com aparelhos oclusais planos, que foram tratados de acordo com o protocolo clínico do CETASE (Centro de Estudos e Tratamento das Alterações Funcionais do Sistema Estomatognático) da FOP -­? Unicamp. As avaliações eletromiográficas foram realizadas simultânea e bilateralmente, no período inicial e aos 30, 60 e 90 dias de tratamento, com a mandíbula nas posições de repouso e fechamento isométrico com resistência do aparelho. Os resultados demonstraram na posição de repouso um aumento significante na atividade eletromiográfica do músculo esternocleidomastoideo direito, quando comparados os períodos inicial e após 60 e 90 dias (p=0,03), e nos músculos trapézio médio direito após 60 dias (p=0,03), e esquerdo, após 90 dias de tratamento (p=0,04).Na situação de fechamento isométrico com resistência resultados significantes foram encontrados para as atividades eletromiográficas dos músculos supra-­?hioideos direito (p=0,04) e trapézio médio direito (p=0,04) e esquerdo (p=0,02), após 90 dias de tratamento. Uma correlação significante pôde ser observada quando comparamos bilateralmente todos os músculos avaliados, nas duas situações (p<0,05). Após a terapia com aparelhos oclusais planos ocorreu uma equalização funcional significativa na atividade eletromiográfica dos músculos cervicais e supra-­?hioideos, podendo sugerir que a alteração na posição mandibular influencia na atividade elétrica dos músculos cervicais
Abstract: The concept of temporomandibular disorders (TMD), define them as a set of joint and muscle disorders in the orofacial region, mainly characterized by pain, noise in the temporomandibular joints (TMJ) and altered mandibular function, and may be associated with cervical changes arising from Cranio Cervical Disorders. Electromyographic studies have shown that changes in head position can change the pattern of the mastication muscles activities and the jaw position. Based on these aspects, the aim of this study was to evaluate the electromyographic activity of the supra-­?hyoid muscles, sternocleidomastoid, trapezius, anterior temporal and masseter in 15 edentulous volunteers with low vertical dimension of occlusion (VDO), TMD treated with plan occlusal splints according to the clinical protocol of CETASE (Center for Studies and Treatment of Functional Changes of the Stomatognathic System) FOP -­? Unicamp. The electromyographic evaluations were performed simultaneously and bilaterally, before the beginning of the treatment and at 30, 60 and 90 days of treatment, with the jaw at rest position and isometric closure with resistance of the splint. The results showed in the jaw rest position a significant increase in electromyographic activity of the right sternocleidomastoid muscle, when comparing the initial period and after 60 and 90 days (p = 0.03), and right medium trapezius muscles after 60 days (p = 0.03), and left medium trapezius muscles, after 90 days of treatment (p=0.04). Related to isometric closure with resistance significant results were found for the electromyographic activity of the right suprahyoid muscles (p=0.04) and right (p=0.04) and left middle trapezius (p=0.02) at 90 days after treatment. A significant correlation was observed when comparing bilaterally all muscles in both conditions (p<0.05). After the plan occlusal splint therapy, there was a significant functional equalization of the electromyographic activity of neck and suprahyoid muscles, suggesting that changes in mandible position can influence the electrical activity of cervical muscles

Background: Following the enhancement of living standard in Taiwan, the population of its denture usage has been increasing dramatically. Since July of 1999, in order to promote better living quality and health for its local ageing population, Kaohsiung city government has established their first “Free Denture Plan” for the local residents whose age above 65. Within the first seven years, there are 25,000 people benefited from it. As the old person denture has become hugely popular universally, how to make a set of highly practical and usable denture is essential. Research: 1.To study the relationships between the denture and tongue position. 2. To study the relationships between partially Edentulous and tongue retraction position. 3. To investigate the tongue position followed by gender, age and teeth availability for the dental patient. Research method: This research has been examined and approved by the human body experiment committee of Chung Ho Memorial Hospital at Kaohsiung Medical University. Oral examination and survey has been carried out to those who are denture patients and natural teeth who are above 45 years old by qualified dentist from local dental hospitals, clinics and other organizations. All the surveys were analyzed and classified by JMP5.12 (p<0.05 was statistically significant). Result: Partial denture patient, Wright’s tongue position is classified as 29.29% for normal, 69.23% for tongue retraction position, p<0.001 (was statistically significant). Nature teeth, Wright’s tongue position is classified as 70.71% for normal, 30.77% for tongue retraction position, p<0.001(was statistically significant). Conclusion: 1.) The dental practitioner should increase the awareness of the oral hygiene education and the oral reconstruction. 2.) The partially Edentulous patient (Kennedy I and Kennedy II cases) will result with a higher possibility of tongue retraction by increasing age.3. )This can be the reference of stability and dentition for the denture for dental practitioner.

The tongue is an important attribute in denture success or failure. The purpose of this project was to study the relationship between completely edentulous patient and tongue position. It also studied the relationship between sex, age, period of edentulous state and tongue position.172 samples was included in the study. 86 patients with natural dentition was the control group and 86 patients with full edentulous ridges was the experimental group. The baseline data and oral function evaluation of both groups was collected. Furthermore, the tongue position was determined by Wright classification and Kotsiomiti & Kapari method related to edentulous state. And the retracted distance was measured by a modified periodontal probe. The relationship between tongue position and variables was evaluated by statistic method. The completely edentulous patient expressed higher rate of retracted tongue or abnormal tongue position than the natural dentition (P<.005). There were no significant difference between tongue position evaluation by Wright or Kotsiomiti & Kapari method and the completely edentulous period of upper and lower arches (P>0.05). However, the length of full mouth edentulous period significantly related to the tendency of the retracted tongue or abnormal tongue position. These results suggest the tongue position of completely edentulous patient need to be evaluated carefully when making complete dentures.

Introduction: Dissatisfaction with complete denture treatment among edentulous patients has long been a frustrating problem, especially for mandibular edentulous patients. Implant overdentures have been proposed to be the first choice of treatment for these patients. To date, the outcomes of implant therapy have been reported mainly in terms of implant survival rates and durability of prosthetic components. Mini dental implants have the advantage of low cost and are suitable for edentulous patients with narrow alveolar ridges. However, the patient-based outcome was not well understood in patients receiving mini dental implant retained overdenture therapy. Objectives: The aim of this study was to investigate patient-based outcomes of mini dental implant retained overdenture using Denture Satisfaction Scale(DS) and Oral Health Impact Profile for edentulous(OHIP-edent) in terms of phonetics, chewing function, denture retention, stability and oral health related quality of life. Materials and methods: This study included patients who came to dental department of National Taiwan University Hospital(NTUH) searching for new lower complete denture. Two treatment options: complete mandibular denture, and implant supported overdenture were informed. Patients were assigned according to their preference. The control group (CD group) includes patients receiving therapies including a conventional mandibular complete denture. For the experimental group (MDI group), a conventional mandibular complete denture was fabricated first and then 4 mini dental implants(MDIs, sendax 3M) were placed in the mandible to retain the denture. Two questionnaires were used in this study including patient satisfaction questionnaire (visual analogue scale) and oral health impact profile for edentulous (OHIP-edent,Likert scale) to assess effect of therapies on patient’s quality of life and health status. All patients filled the questionnaires before the treatment about their old prosthesis (baseline) and then one month and three month after receiving a new mandibular denture. After the mandibular complete denture was delivered and followed up for 3 months, 4 mini dental implants were inserted between mental foramens. The patients were requested to fill questionnaires one month and three months after implant placement. Results: The denture satisfaction in CD group three months after new denture delivery was not statistically significant different from the baseline, except for ease of removal and esthetic(p&lt;0.05)。 The OHIP summary score and functional and physical and psychological subscale scores were statistically significantly different at three months after implant insertion versus baseline (p&lt;0.05). The OHIP summary scores and functional and physical subscale scores were statistically significantly different at 3 month after implant insertion versus 3 months after new denture delivery (p&lt;0.05). The data indicated an improvement both globally and for the functional subscales. Patients with high scores on the Denture satisfaction scale for their mandibular prosthesis reported better OHIP outcomes after treatment with implants. Postoperative functional satisfaction with the implant-retained prosthesis resulted improved quality of life. Conclusions: Implant retained overdenture using mini dental implants improves denture satisfaction and oral health related quality of life when compared to a conventional denture.
