Abstract:
Abstract: The relationship between Aragon and England during the reign of Alfonso III of Aragon (1285-1291), also known by historians as «the Franco» or «the Liberal», was very close and not only with Eduardo I, but also with his wife Eleanor of Castile. As a result of this alliance, the treaties of Oloron-Sainte-Marie (1287) and Canfranc (1288) were signed within a lot of turbulent relations between the European kingdoms. Although Thomas Rymer transcribed the most of the documents about this relationship between the 17th and 18th centuries, he didn’t include all the existing documents and he was wrong in the dating of some of them. This work completes Rymer's study and analyzes which topics were discussed between the kings of Aragon and England during their very regular contacts. Key words: Alfonso III of Aragon, Edward I of England, Eleanor of Castile, international relations, 13th century.

There are figures that the passage of time is blurring from the memory archive of peoples. Others, on the contrary, take root with such force that they remain registered forever in the poetic memory of humanity». That is the case of Dr. Eduardo Bernabé Ordaz Ducungé (1921-2006), described by professor and senior researcher Raúl Fuillerat Alfonso (1950-2015) as the «intellectual and spiritual father of island psychologists» This chronicle evokes the sacred memory of the founding director of the Havana Psychiatric Hospital, on the occasion of Parents’ Day, in the year of the 101st anniversary of his birth.

Abstract:
The creation and development of the Servicio Sismológico Español (1906-1930) clearly illustrates the complexity of the organisation of seismological networks in Spain during the Restauración and the reign of Alfonso XIII. A unique combination of technical advances, seismological knowledge and the need for safety and seismic prediction gave rise to the emergence of both state and private observatories, an important aspect of the process of scientific modernisation after the Spanish-American war of 1898. This paper aims to show how the engineer Eduardo Mier y Miura (1858-1917) played a key role in the foundation of the Servicio, despite the fact that some of his decisions and contributions were highly criticised by the next generation of seismologists. Figures such as Vicente Inglada, José Galbis, and Alfonso Rey Pastor carried out a complete reorganisation of the Servicio. These events did not stem from the well-known regeneracionismo movement which took place at the turn of the century, but were rather the result of internal dynamics.

Abstract:
On June 6, 1900, a Royal Order authorized the creation of a College of Physicians and another of Pharmacists for the towns of Cartagena and La Unión in the province of Murcia. The College of Pharmacists was established on September 1, 1900 with a Governing Board chaired by Eduardo Romero Germes, Gonzalo Robles Fernández as secretary, first member Antonio Rodríguez Comellas, second member Alfonso Carrión Inglés and treasurer José Díaz Benzal. Starting from the pharmacists of both towns in the last third of the 19th century, the history of the application for the College, the constitution process, the news about its operation and the cause of its dissolution on May 30, 1904 are reviewed. In addition to the list of registered pharmacists, the biography of the five members of its Governing Board is recalled and the constitution of the College is analyzed in relation to the cantonal uprising of 1873.

Abstract:

Abstract:

Abstract:

Abstract:

Abstract:
Background:Left ventricular diastolic dysfunction (LVDD) is a very common finding in heart involvement in Systemic Sclerosis (SSc).Objectives:To determine the prevalence, risk factors and mortality associated with LVDD in a cohort of patients with SSc.Methods:A retrospective study was conducted with data from the multicentre Spanish Scleroderma Registry (RESCLE). A case-control study was performed to identify factors associated with LVDD.Results:Out of 1517 cases of SSc, 319 (21%) developed LVDD. Basal characteristics are shown in Table 1. In multivariate analysis, LVDD was associated to older age at diagnosis of SSc [54 vs 44 years, OR 1.05 (1,04-1.06)], presence of telangiectasia [67 vs 59%, OR 1.42 (1,88-1.08)], and treatment with calcium channel blockers [50 vs. 45%, OR 1.55 (1.16-1.96)], and inversely correlated to treatment with ACE inhibitors [74 vs. 83%, OR 0.59 (0.44-0.8)]. Mortality was increased in patients with LVDD (24 vs 17%, OR 1.4, p = 0.01). Kaplan–Meier cumulative survival for the SSc cohort, according to the presence or absence of LVDD showed significant differences in 30 years from the first SSc symptom (59 vs. 70%, p = 0.04).Table 1.Number of patients1517319 (21%)1198 (79%)PLimited SSc923 (61%)201 (63%)722 (60%)0.438Diffuse SSc304 (20%)51 (16%)253 (21%)0.041Sine Sclerodema SSc174 (11%)47 (15%)127 (11%)0.048Age at disease onset(years) med±SD46.4±16.554.1±15.944.5±16.1<0.001Arterial hypertensión491 (33%)148 (48%)343 (30%)<0.001Digital ulcers615 (41%)108 (34%)507 (42%)0.006Telangiectasia918 (61%)212 (67%)706 (59%)0.011Interstitial lung disease645 (43%)153 (48%)492 (41%)0.021Pulmonary hypertension139 (9.2%)34 (11%)105 (8.8%)0.325Capillaroscopy Slow pattern673 (51%)130 (46%)543 (53%)0.038Centromere Antibodies679 (49%)154 (55%)525 (48%)0.032Conclusion:In our cohort, LVDD is relative common in SSc patients, and it is associated with older age at diagnosis, treatment with calcium channel blockers and telangiectasia. Abstract:

Abstract:
Abstract The stroma of solid tumors is populated by myeloid cells, which mostly represent macrophages. Tumor-associated macrophages (TAMs), strongly influenced by cancer cell-derived factors, are key drivers of immunosuppression and support tumor growth and spread to distant sites. Increasing evidence demonstrates their ability to hamper cancer patients' response to most treatments currently applied in the clinic, including immunotherapy. Therefore, strategies to counteract negative effects of TAMs are nowadays gaining momentum at preclinical, translational, and clinical levels. Here, we present the development and evaluation of nanoparticles (NPs) loaded with TLR agonists and/or inhibitors of immunosuppressive pathways to reprogram TAMs and the tumor microenvironment (TME) to unleash an effective immune response to fight against the tumor. Firstly, we have evaluated in vitro the ability of poly(I:C) and/or R848, agonists of TLR3 and TLR7/8 respectively, to reprogram TAMs into antitumor effector cells, and also the efficacy of Stattic and/or Galunisertib, inhibitors of STAT3 and TGF-β pathways respectively, to inhibit immunosuppression by cancer cells and/or TAMs. The best combinations of these drugs were encapsulated in nanoemulsions or polymeric nanocapsules for improved TAM-targeting and pharmacokinetics in vivo. These NPs were characterized for their physicochemical properties and also tested in vitro using primary human macrophages. For in vivo evaluation, subcutaneous and orthotopic murine models of lung cancer (CMT167) were used, showing antitumoral efficacy and TME reprogramming as evaluated by FACS, RNA analysis and multiplex immunofluorescence. Protamine-NCs-loaded with poly(I:C)+R848 and coated with an additional layer of hyaluronic acid functionalized with mannose were used to target the CD206 receptors, showing antitumoral efficacy mediated by TAM reprogramming, evaluated as higher CD86 while lower CD206 and Arg1 expression. Nanoemulsions with a PEGylated surfactant, encapsulating Stattic+Galunisertib+R848 showed faster antitumoral activity versus the free drugs. Experiments performed in IFN-γ KO mice and immune deficient mice (NSG and Balb/c nude) revealed that a fully functional immune system is crucial for the response to the treatment. In conclusion, our work demonstrates the antitumoral efficacy and reprogramming of the tumor microenvironment by combinations of TLR agonists and/or inhibitors of immunosuppression, which can be improved by nanotechnological approaches. Further investigations are ongoing to assess their antitumoral efficacy in other tumor models (i.e. breast and pancreatic cancer), with the final aim of clinical translation. Abstract:
Abstract Introduction: The Spanish familial pancreatic cancer registry (PANGENFAM) was established in 2009, with the main objective to study the genetic and phenotypic characteristics of familial pancreatic cancer families and offer screening of high-risk individuals for early detection. Methods: Targeted panel and exome sequencing of 52 FPC and 33 sporadic PDAC cases was performed to identify genes and novel variants related to PDAC risk. Healthy high-risk relatives of FPC cases enter into a screening that consists of an annual MRI, echo-endoscopy and routine blood tests, which is followed by a biopsy if a suspicious lesion is identified. Results: The registry and associated biobank is maintained thanks to a stable collaboration between the departments of Medical Oncology, Gastroenterology, Radiology General Surgery and Pathology. The registry currently includes over 150 families and 350 individuals, including PDAC cases and high risk individuals. Targeted sequencing analysis of DNA repair genes involved in familial cancer identified pathogenic variants in 7.5% of FPC cases in the MLH1 and CDKN2A genes, likely pathogenic variants were found in 5.7% of cases in the genes MUTYH, TERT and ATM. Likely pathogenic variants were found in 9.4% of sporadic cases in the genes POLQ, CHEK2 and FANCM and no pathogenic variants were identified. By exome sequencing, missense variants with a damaging effect were found in 11 FPC cases, including stop gain, stop loss or frameshift variants in 5 FPC cases (45.45%), affecting WWOX, C2orf83, CYP3A5 and TANGO2. KRAS somatic mutations in codon 12 and 13 were more frequently detected in sporadic PDAC cases (70%) compared to familial cases (16%) via the liquid biopsy. Interestingly, the median overall survival for sporadic and familial PDAC cases in our cohort was 10.2 vs. 21.7 months (p⇒0.01), respectively, even when adjusted for stage at diagnosis and first line adjuvant therapy. From all the families included in the registry, 185 were offered the high-risk screening program for early detection. Currently 129 individuals are actively participating in the screening program, of which, 72% of these individuals have a non-normal looking pancreas via MRI or EUS. The most commonly detected lesions were cysts and IPMN found in 32% and 12% individuals, respectively. Screening has successfully detected and treated solid lesions in 4 high-risk individuals, including 3 early PDAC and 1 neuroendocrine tumor. An additional 11 individuals with lesions with worrisome features are under close follow-up. Conclusions: Panel sequencing has identified pathogenic variants in known familial cancer genes that are also associated with FPC risk. Exome sequencing in panel negative FPC cases has identified novel risk associated genes. The molecular and clinical profile of FPC and sporadic cases are distinct which should be taken into account in the clinic. Abstract:
Background:Prior literature shows a global increase of cancer risk among patients with systemic sclerosis (SSc). Although breast cancer (BC) is one of the most frequent malignancies in SSc patients, the characteristics of this neoplasm among SSc patients is not well established and it is uncertain whether SSc significantly increases the risk of this cancer. Objectives:Describe the characteristics and risk factors for BC among Spanish patients with SSc.Methods:Ambispective and multicenter study of patients with a diagnosis of SSc according to ACR/EULAR criteria and/or LeRoy classification included in the Spanish Scleroderma Registry (RESCLE) from 2006 to 2018. Characteristics of patients with BC were described and incidence was compared with that of the general population. Results:Among 1930 patients with SSc, 206 (10.7%) had cancer. BC was the most frequent tumor location (47 patients of 206 with cancer [22.8%]), followed by lung cancer (29, 14.1%). The risk of BC was increased in patients with SSc compared to the general population (standardized incidence ratio [SIR] 1.31; 95% CI 1.10-1.54;P= 0.003).The comparison of patients with BC and those without cancer showed that patients with BC had older age at diagnosis of SSc (50.9 vs 45.9 years, respectively;P=0.004), were more frequently diagnosed of interstitial lung disease (ILD) (30/47 [63.8%] vs 694/1714 [40.5];P= 0.002) and pulmonary hypertension (12/24 [50.0%] vs 262/917 [28.6%];P= 0.037), had higher frequency of puffy hands as the first manifestation of SSc (4/45 [8.9%] vs 38/1664 [2.3 %];P= 0.023) and had more frequently primary biliary colangitis (PBC) (7 /46 [15.2%] vs 72 /1708 [4.2%];P= 0.004). Regarding autoimmunity profile, patients with SSc and BC had a significantly higher presence of anti-Ro (11/44 [25.0%] vs 214/1528 [14.0%];P= 0.049) and anti-mitochondrial antibodies (7/28 [25.0%] vs 96/837 [11.5%];P= 0.039). Multivariable regression analysis showed an independent association between the puffy hands (OR = 6.40; 95% CI 1.73-23.60;P= 0.005), diagnosis of PBC (OR = 5.70; 95% CI 2.16-15.07;P= 0.001), presence of ILD (OR = 3.29, 95% CI 1.69-6.39;P<0.001) and the presence of the anti-Ro antibody (OR 2.14; 95% CI 1.01-4.56;P= 0.048) with the presence of BC.Conclusion:BC risk was increased in patients with SSc. The development of ILD, PBC, the presence of anti Ro and puffy hands as the first clinical manifestation of SSc were identified as independent factors associated with the development of BC in our cohort.References:Colaci M et al. Abstract:
Abstract An anonymous response on behalf of Charles V to statements made by Edward Lee, English ambassador to the imperial court (published in Exhibita quaedam per Eduardum Leum …, Antwerp, 1528), is ascribed to the imperial secretary Alfonso Valdés. There are verbal and contextual parallels between the anonymous response and Valdés's Diálogo de Mercurio y Carón. Both works, moreover, contain references to Erasmus, with whom Lee had engaged in a polemic over the text and interpretation of the new Testament. The exchange, therefore, transcends the political sphere, and pits Valdés, an admirer of Erasmian humanism, against Lee, a defender of the scholastic method.

Abstract:
Background:Nailfold Capillaroscopy is a simple, inexpensive and non-invasive technique that allows microvascular damage to be observed, gaining recent importance in the diagnosis, monitoring and prognosis of many diseases with microangiopathy. However, the variability in the results interpretation has led to the development of new computerized systems that allow the automatic analysis of capillaroscopic images. Objectives:to compare the degree of agreement between the automatic system Capillary.io and a gold standard obtained from the agreement of 9 expert capillaroscopists and to know the degree of the interobserver reliability To demonstrate the validity of the system to detect normal and enlarged capillaries, hemorrhages, megacapillaries, ramifications and tortuosities. Methods:a cross-sectional study was performed in which 300 random and anonymous nailfold capillaroscopic images (1165 capillaries) were analyzed by 9 experienced observers. The degree of interobserver agreement was calculated from the 5 users. Likewise, the system performed an automatic assessment of the images and their agreement with the gold standard was calculated (interobserver agreement greater than 5, 6, 7, 8 and 9 successively). The validity of the program for each variable was also analyzed using sensitivity and specificity, positive and negative predictive values, and likelihood ratios, as well as their degree of agreement using the weighted kappa statistic (95% CI, p <0.05). The programs used for statistical calculations were SPSS 22.0 and EPIDAT 3.0.Results:the degree of interobserver agreement was 76.5% for the agreement of 5 or more observers, progressively decreasing to 15.4% for the 9 observers. Capillary.io obtained higher levels of agreement, reaching 97.7% for the 9 observers. Statistically significant results were obtained in the automated detection of all the morphological alterations analyzed Capillary.io presented a sensitivity (S) of 79.82% and a specificity (E) of 82% in the recognition of normal capillaries. The automatized system was able to recognize enlarged capillaries with a sensitivity of 86.97% and a specificity of 81.38%. Megacapillaries were detected with 89.41% sensitivity and 78.75% specificity. Similarly, the system was able to detect tortuosities (S 66.94%; E 67.71%), ramifications (S 54.34%; E 58.61%) and hemorrhages (S 71.36; E 73.97%).Conclusion:Capillary.io demonstrated a high degree of agreement with the gold standard, stronger with greater consensus among observers. It was able to detect with great sensitivity and specificity hemorrhages and megacapillaries, very relevant alterations in microangiopathies.References:[1]Roldán LMC, Franco CJV, Navas MAM. Abstract:
Abstract Background: There is a lack of predictive biomarkers for immunotherapy in melanoma. Immunohistochemistry and gene sequencing are frequently assessed as part of clinical research. Radiomic signatures may also add valuable information, based on parameters which can be related to immune infiltration, therefore defining an imaging biomarkers panel for this clinical scenario. BO-112 is a double stranded synthetic RNA formulated with polyethyleneimine (PEI) that, by mimicking the effect of a viral infection, mobilizes the immune system. The role of imaging biomarkers is being explored in the present phase II clinical trial. Study design: Single arm study with BO-112 plus pembrolizumab (NCT04570332) in patients with advanced melanoma in progression to anti-PD1 therapy. As part of exploratory objectives, a radiomics analysis was performed to detect changes in lesion texture features. Quantitative features were obtained by using Quibim Precision 2.9 platform (Quibim, Valencia, Spain) after the manual delineation of lesions on the CT study of each subject at each timepoint to obtain information about injected/non-injected lesions. Images were normalized by taking into account Hounsfield Units (HU) bias across scanners in a cross-calibration phantom and the Z-score. The difference (Delta) in the features between baseline and week 8 was calculated. Results: Studies were assessed based on event (progression) and based on whether lesions had been injected. Patients with only cutaneous disease were not included in this analysis. Out of 23 patients who had at least two imaging assessments, 6 developed progressive disease by week 8, and 17 subjects had no event by that time. Due to the small sample size, the radiomic analysis was based on hypothesis testing. With regards to volume, 50% of the non-progressing lesions reduced their value from that of the baseline. Regarding injected versus non-injected changes, up to 50% of injected lesions decreased their volume after 8 weeks of injected treatment whereas in non-injected lesions volume decreased in less than 25% of lesions. From the independent sample t-test of delta radiomics features against the injected/non-injected lesions variable, there were 4 features with a statistically significant difference between groups; all of them related to the Low Grey-Level Zone Emphasis. Specifically, Delta original GLRLM Low Grey-Level Run Emphasis showed the highest significant difference between injected and non-injected lesions (p=0.004), with higher and positive delta values in the injected group (75% injected lesions were above 0). Conclusions: Imaging biomarkers provide a large number of quantitative image features with a wide span of information, from size to heterogeneity of the tissue which may be indicator of tumor progression and immune infiltrate. In the analysis of radiomics features, delta GLRLM low grey-level zone emphasis was sensitive to the tumoral changes happening in injected lesions at week 8. This might add insights into the imaging-based evaluation of immune infiltration in intratumoral immunotherapy and the creation of associated imaging biomarkers panels. Abstract:
Abstract Background: BO-112 is a double stranded synthetic RNA formulated with polyethyleneimine that mimics a viral infection. Through dendritic cells activation, increase in CD8 T-cell infiltration and interferons induction, it produces an immunogenic cell death. Phase 1 study (NCT02828098) showed ORR 20% in patients (pts) with anti PD-1 resistant melanoma (mel). Hence phase 2 trial was designed. Study design: Single arm study (NCT04570332) with intratumoral BO-112 plus intravenous pembrolizumab in pts with mel (cutaneous, acral or mucosal) and confirmed progressive disease (PD) while on anti-PD1 based therapy. Pts were treated with BO-112, 1-2 mg on a weekly basis for 7 weeks and thereafter Q3W in 1-8 different lesions. Pembrolizumab 200 mg was administered Q3W. Primary endpoint was overall response rate (ORR) by RECIST 1.1 by independent reviewer. Secondary endpoints included disease control rate (DCR), progression free survival (PFS) and safety. Exploratory objectives included radiomic signatures, itRECIST and evaluation of tumor microenvironment. At least 20% of pts had to achieve response in order to consider primary endpoint met. Results: Recruitment was completed 24th August 2021 with 42 pts; female 43%; median age 66 (rank 27-88). Table summarizes basal characteristics. With 40 evaluable for response pts, 10 achieved response (25%): 3 complete response (CR) and 7 partial response (PR). 17 pts (44%) achieved a stable disease (SD), meaning a DCR of 68% with 18 pts still on treatment. The 4 pts with a baseline LDH>3xULN developed PD no later than week 8. Responses per histology were: 66% mucosal, 28% cutaneous, 0% acral. Responses per BRAF/NRAS status were: BRAF mutant (Mut) 43%, NRAS Mut 31%, and BRAF/NRAS wild type (WT) 17%. 33 pts (79%) had at least one BO-112 related adverse event (AE) being only in 2 cases grade>3 (G4 infusion reaction and G3 myalgia). Most common related AEs were asthenia, pyrexia, diarrhea, vomiting and chills. Study treatment was not discontinued in any pts due to related AE. Conclusions: The primary efficacy endpoint has been met. Additionally, disease control (PR+CR+SD) is meaningful and durable in a population with no current standard treatment options. Very high LDH levels (LDH >3xULN) and acral mel could predict poor outcome. Safety profile was manageable without treatment discontinuation due to AEs. Abstract:
Abstract Background: There is a lack of predictive biomarkers for immunotherapy in melanoma. In terms of IHC, there is no strong rational to support the use of PD-L1 expression. BRAF mutations occur in 40-50% of melanomas and the MAPK pathway may also be activated by NRAS mutations. Patients harboring these mutations face usually a worse prognosis. BO-112 is a double stranded synthetic RNA formulated with polyethyleneimine (PEI) that, by mimicking the effect of a viral infection, mobilizes the immune system. Prior data from a phase I trial (NCT02828098) suggest that, when administered intratumorally, it causes an increase in CD8 infiltration and PD-L1 expression. The role of these and other biomarkers is being explored in the present phase II clinical trial. Study Design: Single arm study with BO-112 plus pembrolizumab (NCT04570332) in patients with advanced melanoma in progression to anti-PD1 therapy. Tumors were genotyped by next generation sequencing, whole exome sequencing and tumor mutation burden. Antitumor and immunological effects of the treatment in the tumor microenvironment were assessed by PDL1 and CD8 immunohistochemistry with a paired biopsy performed after 21 days of treatment. Results: A preliminary analysis has been performed, based on patients evaluable for clinical benefit (defined as response or stable disease>16 weeks). Samples from 35 patients have been analyzed, with 24 patients paired biopsies available. Patients with “cold” tumors (PD-L1 negative and CD8 low) at baseline had a trend to lack of clinical benefit. Only basal PD-L1 in the inflammatory component showed a statistically significant correlation with clinical outcome (4/20 (25%) tumors PDL1 IC negative had benefit versus 10/15 (67%) positive), p=0.0053. Fifteen patients had an increase in PD-L1 and 14 patients had increase in CD8 infiltrate after BO-112 treatment; the lack of increase in PDL1 and CD8 after treatment was also predictive of lack of response (p=0.04). BRAF/NRAS driver mutations correlated with positive outcome. Clinical benefit was observed in 4 of 17 (24%) patients not carrying activating mutations whereas 11 out of 18 (61%) patients with BRAF/NRAS activating mutations had clinical benefit (p=0.02), mainly in cutaneous histology (14% versus 65%, p=0.02). Mucosal melanoma patients (n=3) achieved an ORR 66.7% and DCR 100%. The two mucosal melanoma patients with partial response harbored SETD2 mutations and one of them showed extensive cyclic nucleotide-gated (CNGs), indicative of defects in DNA repair pathways. Regarding acral melanoma patients (n=9), no responses were observed, even in the single case with a NRAS mutation. The only patient achieving clinical benefit, with stable disease>16 weeks; had a unique mutation profile, with TP53 (inactivating) and KIT (activating) mutations. Conclusions: Patients basal mutant BRAF/NRAS could have more probability of benefit from BO-112 and pembrolizumab combination. PD-L1 and/or CD8 increase is an early marker of response. These findings could help to select patients in future clinical trials. Further investigation into predictive biomarkers is warranted. Abstract:
Background: A nailfold capillaroscopy procedure is a non-invasive, low-cost, and well-established examination that can be used to diagnose several rheumatic autoimmune diseases and support the necessary follow-up of patients. There are two main elements to nailfold capillaroscopy: image acquisition and image interpretation. Both of these can present challenges: we need to ensure that the best possible images are captured, and we need to define the enlarged capillaries, capillary loss and pericapillary hemorrhages objectively. We introduce Capillary.io, an automatic image reading system able to recognize capillaries in images obtained with any microscope, generate automatic measurements of each capillary and take advantage of this information to report capillary morphology. Together it allows a comprehensive analysis that is capable of producing detailed reports for each patient. Objectives: The primary outcome was general sensitivity and specificity, using images assessed by expert capillaroscopists as the gold standard. Methods: 6500 images previously analyzed by capillaroscopists from GREC were compared with Capillary.io. Capillary morphology (enlarged capillaries, tortuosities, ramifications, megacapillaries, hemorrhages) of each of the capillaries contained in each of the images was analyzed manually by at least one expert capillaroscopist. Subsequently, the automatic image interpretation system was used to fully automatically analyze each of the capillaries contained in each image and the results obtained were compared. Results: Overall, a total of 78.347 capillaries were compared, of which 47.734 were normal capillaries, 21.991 enlarged capillaries, 2672 megacapillaries, 8512 tortuosities, 1322 ramifications and 5149 hemorrhages. Capillary.io was able to detect 38.101 normal capillaries, 19.126 enlarged capillaries, 2389 megacapillaries, 5698 tortuosities, 718 ramifications and 3706 hemorraghes. Capillary.io presented a sensitivity (S) of 79.82% and a specificity (E) of 82% in the recognition of normal capillaries. The automatized system was able to recognize enlarged capillaries with a sensitivity of 86.97% and a specificity of 81.38%. Megacapillaries were detected with 89.41% sensitivity and 78.75% specificity. Similarly, the system was able to detect tortuosities (S 66.94%; E 67.71%), ramifications (S 54.34%; E 58.61%) and hemorrhages (S 71.36; E 73.97%). Conclusion: Capillary.io is a simple, easy-learning web-based system to get interpretation of nailfold capillaroscopic images. It may be a very useful tool to standardize the interpretation of capillaroscopic pictures and could provide great research in that field. Abstract:
Abstract Background: We previously demonstrated that a kinase-based taxonomy of TNBC was most parsimonious than next-generation sequencing in defining TNBC subtypes associated with prognostic categories in early disease. The most aggressive TNBC variants were driven by a heterogeneous set of genetic aberrations that converged in the increased activity of 6 kinases: KIT, PNKP, PRKCE, P70S6K, ERK and CDK6 (Nat Commun; 9:3501-18). The combined inhibition of these kinases in pairs led to potent tumor regression in preclinical models, being the most powerful combination that one directed against CDK6 and ERK. This prompted us to design a phase II trial testing the combination of palbociclib (against CDK6) and binimetinib (against the ERK upstream kinase MEK, since no ERK inhibitor was available at that moment outside phase I trials) in advanced TNBC. Hyperactivation of CDK6 and/or ERK was selected as entry criterion. Trial design: This was a single-arm, prospective, multicentric, open-label, phase II investigator-initiated trial. CDK6 and phosphor-ERK levels were measured in tumor samples by immunohistochemistry and normalized with a reference sample collection to a Z-score. Patients with scores for either kinase above the median were candidates for the trial. Key inclusion criteria included metastatic >18year-old TNBC, adequate organ function, measurable disease, and progression to 1-2 prior treatment lines (including immunotherapy, and a PARP inhibitor in case of germline BRCA1/2 mutation). Patients started continuous oral binimetinib at 45 mg/BID and palbociclib 100 mg/day from days 1 to 21, in 28-day cycles. Patients experiencing ≤ grade 1 tolerable side effects as the greatest toxicity were escalated to palbociclib to 125 in cycle 2. RECIST 1.1 and NCI CTC AE V 5.0 were used for assessing disease control (q8 weeks) and toxicity. The primary aims were to assess the efficacy and toxicity of this combination, and the secondary one to detect biomarkers of activity. At the time of trial design, in absence of available Sacituzumab for prescription, the reference PFS to beat in advanced lines for physician’s choice in TNBC was 1.7 month (NEJM; 384:1529-41, 2021). With alpha and beta errors of 0.05 and 0.2, the minimum number of patients to demonstrate a 30% improvement in PFS to 2.5 months was 25. Results: From November 2020 to April 2023, 69 patients were screened and 24 entered the trial (5 positive for phosphor-ERK; 2 for CDK6; 17 for both). Toxicity was generally mild and included grade 1-2 diarrhea (33% of the patients), grade 1-2 asthenia (50%), grade 1-3 neutropenia (75%), grade 2 retinal toxicity (8.3%) and grade 3 rash (4.2%); no grade 4/5 toxicities were observed. Median PFS was 1.83 months (range 0.3 to 11.3+). Phospho-ERK and CDK6 levels were not correlated (Pearson’s R= -0.089; P=0.68); CDK6 levels did not show association with PFS time (R = -0.120; P=0.58). Interestingly, however, phosphor-ERK levels in the baseline tumor sample showed correlation with PFS time (R = 0.428; P=0.037). Conclusion: The combination of palbociclib and binimetinib was generally safe, and PFS time showed correlation with baseline phosphorylation levels of ERK. However, the trial did not meet its primary endpoint. Abstract:
Abstract Background: Novel effective and safe therapies are required for advanced TNBC after progression to standard-of-care first line treatment with anti-PD-1/L1 + chemotherapy. We have found that the most aggressive TNBC variants are driven by a heterogeneous set of genetic aberrations that converge in the increased activity of 6 kinases: KIT, PNKP, PRKCE, P70S6K, ERK and CDK6 (Nat Commun; 9:3501-18). Although the inhibition of each kinase in monotherapy yielded little efficacy in preclinical models, the combinations targeting pairs of the former kinases led to therapeutic synergy in most cases. The combined inhibition of CDK6 and ERK led to 5-fold increase in overall survival in preclinical TNBC models (PDXs and spontaneous murine cancer models). The greatest activity was observed in models with increased activity of either CDK6 or ERK. Thus, we aimed to test the safety and preliminary efficacy of combined ERK and CDK6 inhibition with binimetinib and palbociclib in women with advanced TNBC with hyperactivation of ERK and/or CDK6. The combination has been preliminary tested in lung cancer, where a phase I dose-escalation trial established the RP2D in binimetinib 45 mg/BID plus Palbociclib 125 mg daily 21/7. Trial design: Single-arm, prospective, multicentric, open-label, phase IB/II trial with intra-patient dose-escalation. Patients candidate for pre-screening will have determined the activity of ERK and CDK6 with an in-house developed assay and acquisition algorithm at the central lab. Those testing positive for either marker will undergo full screening. Patients will start treatment with continuous oral binimetinib at 45 mg/BID and palbociclib 100 mg/day from days 1 to 21, in 28-day cycles. Patients experiencing ≤ grade 1 tolerable side effects as the greatest toxicity will be allowed to escalate to palbociclib to 125 in cycle 2. Fresh biopsies will be harvested at baseline and disease progression, in order to establish patient-derived organoids (PDOs) and perform WES. PBMCs will be harvested at baseline, +24 hours, and at the beginning of cycle 2, to study changes in the immunophenotype. RECIST 1.1 and NCI CTC AE V 5.0 criteria will be used for assessing disease control (q8 weeks) and toxicity. Eligibility criteria: Pre-screening: Women >18 year old diagnosed of advanced, non-curable TNBC; 2) who have received a minimum of 1 treatment line including immunotherapy; 3) no more than 2 treatment lines for advanced disease; 4) Adequate organ function and recovery from previous toxicity to < tolerable G2. Full screening: 5) Demonstration of hyper-activation of CDK6 and/or ERK in the tumor sample; 6) PD to the previous treatment regimen within the last 28 days. Specific aims: Primary: 1) To determine the progression-free survival time of the combination of binimetinib and palbociclib in TNBC patients with hyperactivation of ERK and/or CDK6 in second/third line 2) To assess the safety and tolerability of the former combination Secondary: 1) 6-month PFS rate, response rate, and overall survival of the combination 2) To study biomarkers of sensitivity and primary and acquired resistance to the combination taking advantage of PDOs 3) To determine the immunodynamics of the combination. Statistical methods: The null hypothesis is that the median PFS time for second/third line TNBC with best physician’s choice is 1.7 months (NEJM; 384:1529-41, 2021). With alpha and beta errors of 0.05 and 0.2, the minimum number of patients to demonstrate a 30% improvement in PFS to 2.5 months is 25. Preliminary data suggest that approximately 40% of the patients show hyper-activation of CDK6 and/or ERK; thus, and assuming a methodological failure of 10%, the minimum number of patients to screen is 69. Present accrual/target accrual: 27 screened of 69 planed; 11 accrued of 25 planned Citation Format: Luis Manso, Alfonso Cortes, Juan M Cejalvo, Serafin Morales, Jose A García Saenz, Ramon Colomer, Rodrigo Sanchez-Bayona, Jorge Silva, Juan A Guerra, Diego Malon, Silvana Mouron, Eduardo Caleiras, Miguel Quintela-Fandino. Abstract:
Abstract Background: Male breast cancer (MBC) accounts for approximately 0.25% of all male cancers and < 1% of all BCs. Its incidence is increasing by 1.1% per year. Most population studies indicate that 10-15% of MBC patients (pts) have a germline BRCA2 mutation (BRCA2 mut). Around 5-10% of BRCA2mut male carriers will develop BC at their lives. Preliminary evidence suggests that BRCA2mut MBC pts tend to have a more aggressive disease. Here we present comparative series of MBC with and without BRCA to gain insight on differences between both groups. Methods: GEICAM/2016-04 (NCT03800355) is a retrospective, observational study which includes data from MBC pts diagnosed between 2000 and 2019 throughout Spain. This analysis includes the 1st unselected 186 pts included in the study, who had BRCA1/2 genes mutational status assessed within clinical practice. BC clinical subtypes are defined: luminal (HR+, HER2-), Triple Negative (TN) (HR-, HER2-), and HER2+ (HR+ or HR-); subtype is unknown in 6% pts. Results: 186 validated pts with BRCA1/2 testing were identified, representing the 24% of pts available in the database at the cut-off date (6-Mar-2023). 36 (19%) pts had germline BRCA1/2 mut (4 pts in BRCA1, 31 pts in BRCA 2 and 1 pt in both BRCA1 and BRCA2), and 150 (81%) pts had BRCA1/2 wild type (wt) or a variant of uncertain significance (VUS)/unknown (UK). Median age at BC diagnosis was 62 (34-87) years, 98% pts were Caucasian, and the median body mass index was 27 (18-46) Kg/m2 (overweight). Prior cancer history was reported in 6% BRCA1/2mut BC pts and 11% BRCA1/2wt or vus/uk, and family history included: 56% BC, 8% ovarian cancer, and 14% prostate cancer (PC). PC as part of prior medical history, cancer family history, and 2nd non-breast primary malignancy, was most frequent in BRCA1/2wt or vus/uk pts, with no statistical differences. At first diagnosis, stages III-IV were higher in BRCA1/2mut pts (28% vs. 17%, p=0.0093). Morphologically, invasive carcinoma of no special type (NST) was the most common pattern (92% in BRCA1/2mut vs. 86% in BRCA1/2wt or vus/uk), and lobular invasive carcinoma was not reported up-to-date. Histological grade 2 was the most frequent (54%), but grade 3 was present in 17/36 BRCA1/2mut pts. 88% pts received adjuvant treatment, 7% (neo-)adjuvant, 2% neoadjuvant and 3% did not receive any systemic therapy. Breast conserving surgery was performed in 4% (5/140) BRCA1/2wt or vus/uk EBC pts, and mastectomy was performed in 55% (n=6/11) de novo metastatic pts. In the advanced setting (n=38), visceral lesions were more frequent in BRCA1/2mut pts. Additionally, 92% BRCA1/2mut pts had ≤ 3 organs involved while one third of BRCA1/2wt or vus/uk pts had ≥ 4 metastatic locations. With a median follow-up of 64 months (mo.), median iDFS and DDFS were higher, but not statistically significant, in BRCA1/2wt or vus/uk pts vs. BRCA1/2mut pts. In advanced setting, median PFS to 1L-3L do not show statistically significant differences between both groups. Regarding OS, no differences were also observed, but the median values were not reached. Further detailed information according to BRCA1/2 mutational status and BC subtypes is included in the table below. Conclusions: In this subset analysis of GEICAM/2016-04, BRCA1/2mut pts had some features of worse prognosis, with a more prevalent de novo metastatic disease. No statistically significant differences were observed in outcomes in both early-stage and advanced setting vs. BRCA1/2wt or vus/uk pts. <graphic> Citation Format: Noelia Martínez-Jáñez, Purificación Martínez, Cristina Hernando, Sabela Recalde, Alfonso Sánchez, David Morales, Marta Santisteban, Isaura Fernández, Sonia Del Barco, Esther Zamora, Vega Iranzo, Tamara Martos, Eduardo Martínez-de Dueñas, Silvia Antolin Novoa, Severina Domínguez, Paula Domínguez, Mª José Echarri, Ana Santaballa Bertrán, Xavier Mira, Andrea Blasco, Susana Bezares, Ander Urriticoechea. Abstract:

Abstract:
Romanian composition in the nineteenth century went through rapid changes, moving from a Greek-oriental sound world to a Western European one. It is interesting to examine, in this context, the musicians’ quest for a ‘national’ sound and identity. Analysis of piano miniatures or vaudeville, the favourite genre of the Romanian audience in the first half of the century, shows eclectic combinations of urban folk music with sources of inspiration borrowed from popular foreign melodies. The second half of the century seems to be marked in modern scholarship by premieres: some composers are included in Romanian history just for the merit of writing the first Romanian symphony, the first string quartet, the first opera, and so forth. Their work led towards the constitution of a ‘national language’ adapted to genres borrowed from contemporary Western European music. In addition to demonstrating these ideas in the work of a number of Romanian composers (Josef Herfner, Ioan Andrei Wachmann, Anton Pann, Alexandru Flechtenmacher, Ludwig Anton Wiest, Carol Miculi, George Stephănescu, Constantin Dimitrescu, Gavriil Musicescu, Eduard Caudella, George Dima, Ciprian Porumbescu, Iacob Mureşianu, Dumitru Georgescu Kiriac, Alfonso Castaldi, Eduard Wachmann), the present article also encompasses two case studies. The first is Franz Liszt’s tour through the Romanian Countries, which offers a clearer image of the popular ideas circulating within the musical scene of the time. Liszt’s initiative to emphasize the national spirit through folk quotations reworked in rhapsodies should have inspired Romanian musicians; we will see whether this actually happened. The second case study concerns the musical life of Bucharest around 1900, when the directions of Romanian modern music were being traced, and cautious and selective steps were made toward harmonizing with Europe began.

Abstract:

Abstract:

Abstract:
This article analyzes the reflection on the myth of Faust in the work of Eduardas Mieželaitis (1919–1997). The author’s initial hypothesis that the interpretation of this myth can be used as another argument for revealing the poet’s dramatic feelings and self-reflection of inauthentic Soviet existence is tested. Together with other signs of existential self-abuse (such as the metaphors of “the ringed bird” and “the masked word”), this could help answer the question of whether the description of the 1920s generation (Alfonsas Nyka-Niliūnas, Bronius Krivickas, etc.) as “tragic” would fit Mieželaitis. Intertextuality is the best way to describe the nature of the analysis and socio-criticism would describe the interpretation of the results.E. Mieželaitis was (and still is) one of the most intertextual Lithuanian authors. The use of the “alien word” in his texts was a conceptual position of the poet’s creative consciousness. For him the world’s culture has become a central topic, and his texts have been occupied by the masters of arts and sciences of antiquity, Renaissance, or New Age, as well as modern European authors, Oriental classics, Bible quotations, paraphrases, allusions and other forms of culture. For the poet, Goethe’s Faust is one of such foundational texts, even if the Soviet era did not favor the “myths of the great contemplation” (Vytautas Kubilius). The forever relevant myth of Faust raises a fundamental question of person’s life choice – what one is prepared to sacrifice for one’s purpose, and what the results of that “sacrifice” are. But in the Soviet times Mieželaitis did not focus on this myth as a “symbol of the duality of human’s nature” – his human is rather one-dimensional and monolithic, full of positive energy directed towards technological progress and an absolute belief in the cognitive powers of man. This is the exact aspect that Faust embodies in Mieželaitis’ works, as if it was his “new man”, the product of the Enlightenment era. Mieželaitis did not care about the moment of negotiations between Faust and Mephistopheles – for him there could easily be no Mephistopheles. However, in late works (Postskriptumai, 1986; Gnomos, 1987; Laida, 1992), where the poet reflects on his life and creative path, there are many signs of disillusionment with the myth of human omnipotence as the results of his labor have turned against him – he wanted to create a heaven, but got a hell in the form of Hiroshima. It is at this point that Mephistopheles dominates the duet with Faust, but in general they have both become a “common place” and their function is limited by the text’s aesthetic figurativeness. My conclusion is that eventually the rich allusions to Faust in Mieželaitis’ works did not translate into reflections on the essence or meaning of being – he was not concerned with the problematics of human’s choices, duality, or contradictions. Thus, my initial expectation that the interpretation of the Faust myth may be another argument for revealing the poet’s dramatic feelings and self-reflection of inauthentic existence in the Soviet times was essentially not confirmed. And that could have been another argument for adding him to the social group of the 1920s.On the other hand, looking from the distant present, it is my opinion that Mieželaitis is still a part of the tragic generation, as the whole generation – whether they retreated to the West, or remained in Soviet Lithuania, collaborated or resisted – became hostages of the Second World War and the Soviet occupation.
