This chapter, found in the radiation exposure from medical imaging section of the book, provides a succinct synopsis of a key study examining low-dose ionizing radiation exposure and radiation-induced cancer risks. This summary outlines the study methodology and design, major results, limitations and criticisms, related studies and additional information, and clinical implications. The study reported that a substantial proportion of the nonelderly US population is exposed to medium to very high annual effective doses from medical imaging procedures. Strategies ensuring the appropriate use of medical imaging associated with ionizing radiation should be developed and adopted widely. In addition to outlining the most salient features of the study, a clinical vignette is included in order to provide relevant clinical context.

Abstract:
Decades of research have demonstrated that psychotherapy is generally effective: symptoms change for the better and most clients feel and perform better after talk therapy (Lambert, 2013). In this chapter, we examine the relationship between number of therapy sessions and symptom change. We will focus on the primary claims of the two competing views of this relationship. The dose–effect (DE) model proposes that sessions are like doses; more session-doses cause more improvement. The good-enough-improvement (GEI) model proposes that clients persist in therapy until they improve enough to meet their goals; symptom change controls session attendance. We compare these competing models by examining patterns in the treatment we have provided at our counseling center. Our primary goal was to answer what we consider the most important question about session totals and symptom change: Do session totals cause symptom change, as proposed by the DE model, or is the reverse true: does symptom change control session totals, as proposed by the GEI model? At our counseling center both models fit the data. Greater session totals are associated with more improvement for some clients and other clients leave treatment when they improve enough to meet their treatment goals.

Personal dose monitoring provides the important check and reassurance that radiation doses to those working with radiation are at a level that complies with dose constraints. The quantity which is relevant to external personal monitoring is the personal dose equivalent Hp(d) defined by the ICRU. Values at different depths d are equated to effective dose or doses to the skin or eye lens. This chapter contains the basic information that the radiation protection practitioner needs on personal monitoring, and the requirements for running a personal dose-monitoring service, including dosimeter calibration, performance testing, and record keeping. Techniques used in different types of dosemeters such as thermoluminescent dosimetry and optically stimulated luminescence, as well as neutron detection are explained, together with practical considerations for eye dose and extremity monitoring. Use of electronic personal dosemeters is discussed and techniques for monitoring internal exposure are described.

Abstract:
All medical exposures to ionizing radiation carry an increased risk of malignant disease. This chapter describes methods for evaluating the risks from exposures of varying magnitude from either the effective dose or doses to individual organs. Risk factors which can be utilized in assessments are included. Procedures should only be carried out if they will influence patient management and factors to be considered in the justification process are considered. In addition, techniques should be optimized so that doses to individual patients are as low as compatible with the medical requirements of each examination. The explanation of risks to patients is discussed for situations when a patient is to undergo a high dose procedure or has received an exposure greater than intended.
