Full textNär patienter får besked om att ett kirurgiskt ingrepp är nödvändigt så kan detta väcka olika känslor och skapa oro hos patienter. Syftet med denna litteraturstudie var att belysa vuxna patienters upplevelser i samband med elektiva kirurgiska ingrepp. Litteraturstudie valdes som metod och en sökning av vetenskapliga artiklar genomfördes. Sökningen resulterade i tio relevanta artiklar och dessa granskades och analyserades. Resultatet visade att det patienterna var mest oroliga för var att känna smärta, samt att tappa kontroll över sin kropp och vara utlämnad åt andra. Det framkom även om behovet av att få bra information med bra och tydlig information blev patienterna lugnare. Vidare framkom att de patienter som blev omhändertagna av en sjuksköterska som arbetade enligt den perioperativa dialogen upplevde att de fick en trygg relation och kände tillit till sjuksköterskan. Det behövs mera forskning i ämnet då morgondagens generation kommer att ha högre krav på sjukvården och vara mer upplysta om sina rättigheter i samband med elektiva kirurgiska ingrepp.

Full textBackground: The patient law, SFS 2014:821, was introduced in January the 1st of 2015, which has been developed to clarify and strengthen the position of patients and promote patient integrity, self-determination and participation in medical healthcare. The caregivers’ duty of giving information to the patients was clarified due to the introduction of the law. This means that the quality of the information should optimize the patient's ability to be self-determinated and involved in their care. Aim: To investigate whether chapter three and five in the patient law, SFS 2014:821, is followed and applied in the clinical practice with aim to promote patient involvement and self-determination. Method: Six patients were recruited from a thorax surgery ward at university hospital in Sweden. The participants were elective, thoracic surgical patients whose procedures were performed with extracorporeal circulation. A convenience sample was made where as the sample group included one woman and five men. The interviews were conducted qualitatively and semi- structured, on the surgical ward, 1-2 days before discharge. Data were analyzed by qualitative content analysis according to Hällberg-Graneheim & Lundman (2004). Findings: The caregiver provides information in a clear, respectful and considerate manner in purpose to promote patient participation, integrity and autonomy. Follow-ups were made to reduce the risk of misunderstandings. The majority of participants were satisfied with the information that was given during their care. They experienced reduced anxiety and increased sense of security due to good quality of information. The ward seems to follow regulations in regard to the new patient law. Conclusion: The patient law is followed in the surgical ward where the study was conducted. The collected data strongly indicated that the health staff was actively working to promote patients involvement in their health care. This by keeping the patient well informed and continuously invites patients to participate.

Full textBakgrund: Ett kirurgiskt ingrepp kan upplevas som en stressfylld situation av patienten och kan orsaka känslor av oro och ångest. Preoperativ oro kan påverka patienten fysiskt genom att bland annat ge förhöjd hjärtrytm och ökad smärta i det postoperativa skedet. Syfte: Syftet var att beskriva vuxna patienters upplevelser av preoperativ omvårdnad inför elektiv kirurgi på sjukhus. Metod: En allmän litteraturstudie valdes som metod och sökning av vetenskapliga artiklar genomfördes. Sökningen resulterade i elva relevanta artiklar, vilka granskades och analyserades. Resultat: Resultatet delades in i 3 olika kategorier: Individuell information, Kontinuitet samt Tillitsfull relation. I resultatet framgick att patienterna uppskattade ett preoperativt samtal med sjukvårdspersonalen då detta gav möjlighet till kommunikation. Tydlig och tillfredställande information hjälpte patienterna att känna sig lugnare samt lindrade oro och ångest En del patienter upplevde att det fanns brister i informationen och att den då kunde upplevas som skrämmande. Vidare framkom att patienter som blev omhändertagna enligt den ”perioperativa dialogen” upplevde att de fick en trygg relation och kände tillit till sjuksköterskan. Slutsats: Patienterna betonade vikten av kommunikation och dialog med sjuksköterskan och uttryckte en önskan att inte bli lämnade ensamma. För att uppfylla patienternas behov och önskemål krävs mer tid och resurser.

Full textBackground In 2018, 754 310 surgical procedures were performed on patients over 20 years old in the inpatient care in Sweden. For many patients, surgical procedures are associated with a feeling of anxiety. Studies have shown that 60-80 per cent of surgical patients suffer from anxiety. As a nurse, relieving the patients ́ anxiety prior to a surgical procedure and increasing patient participation requires time, knowledge and commitment. Aim The aim was to highlight nursing interventions that may encourage increased patient participation in preoperative care. Method A literature review where searches for scientific articles were made in PubMed and CINAHL. Eighteen scientific articles were selected and reviewed using an integrated analysis, examination of quality and then sorted by themes according to their findings. Result The results of the included scientific articles showed the importance of clear information and a person-centred approach from the nurses preoperatively to create participation and reduce anxiety. Patients who held adequate and clear information as well as when the nurse had a person-centred approach in both treatment and communication, entailed that the patients felt well prepared for the surgical procedure. Conclusion A person-centred approach and clear information to patients preoperatively are the nursing interventions shown to be most effective from the patient's perspective. This is the result of this literature review. As a nurse, establishing a relationship with the patient based on the patient's story and identifying the patient's need for care regarding the degree and content of information given prior to the surgical procedure, may be the key to increased patient participation.

Full textThe information the surgical nurse communicates to the patient before, during and after an elective surgical operation can reduce the patient's concern and the risk of operative complications. Correct information can also reduce the number of elective surgical procedures canceled, shorten the patient's care time, and reduce the number of medical injuries and the use of pain medication. Nevertheless, there is lacking research on what information patients want in conjunction with elective surgery. The purpose of this study was therefore to compile knowledge about what information patients who are to undergo elective surgery need. To meet the purpose, an integrative literature study was conducted on the basis of 15 scientific articles based on qualitative and quantitative method. The integrative literature study was analyzed using a qualitative method and structured according to two main themes called comprehensive information and person-centered information. The result showed that surgery teams and surgery nurses tried to meet patients' varied and individual needs for written and oral information during the preoperative, operative and postoperative period of the careprocess. During the preoperative period, patients primarily wanted more individualised verbal or written answers to their questions. During the postoperative period, patients wanted more comprehensive information than was provided in practice, and patients wanted as comprehensive, varied and individualized verbal or written information as possible. However, there were differences between the surgical team's and the patients' information preferences, where the patients could want a more person-centered and empathetic understanding as well as complete information about the long-term results of the operation. When such person-centered information was not fully provided, patients could feel insecure before the operation and uncertain in self-care after the operation. The conclusion was therefore that the surgical nurse needs to provide information that takes more account of the patients’ varied individual information needs and more consideration of the patients' sense of psychological security before/after the operation.
