Abstract:

Abstract:
Background:Tocilizumab (TCZ) showed trends for improving skin fibrosis and prevented progression of lung fibrosis in patients with systemic sclerosis (SSc) in placebo-controlled randomised clinical trials (RCTs). However, safety and effectiveness of TCZ beyond these selected and enriched clinical trial populations in SSc is still unknown. Objectives:To assess safety and effectiveness of TCZ treatment compared to standard of care in SSc patients from the large, multicentre, observational, real-life EUSTAR network/database using propensity score matching. Methods:SSc patients from the EUSTAR network/database, who fulfilled the ACR/EULAR 2013 classification criteria, with a baseline and a follow-up visit at 12±3 months, receiving TCZ or standard of care (controls), were selected. The following variables were used for the propensity score matching (1:1): age at diagnosis, gender, disease subtype, baseline modified Rodnan skin score (mRSS), forced vital capacity (FVC), and diffusing capacity for carbon monoxide (DLCO), co-therapy with immunosuppressives, disease duration, and year of treatment. Primary endpoints were mRSS and FVC at 12±3 months follow-up compared between the groups, using paired t-tests. Secondary endpoints were the percentage of progressive/regressive patients for skin and lung at 12±3 months follow-up according to standard definitions (1,2). Sensitivity analyses assessed pre-processing decisions (selection of most recent vs. random observation for control patients with multiple suitable time intervals), as well as the matching method (optimal vs. exact matching). Missing values were addressed with 100-fold multiple imputation using chained equations. Safety data were analysed in all patients. The study including the statistical analysis plan was pre-registered at www.drks.de (DRKS-ID: DRKS00015537).Results:We identified 93 SSc patients treated with TCZ and 2370 SSc patients with standard of care who fulfilled the inclusion criteria. Forty nine (57.7%) of the TCZ treated patients were diffuse, eight patients were not classified, disease duration was (mean±SD) 6.35±5.40 years, their baseline mRSS was 15.05±10.85, and 76 (81.7%) received immunosuppressive therapy in addition to TCZ.Through multiple imputation and propensity score matching, 100 imputed sets of 93 pairs of TCZ/controls were generated. Comparison between groups showed consistent effects of TCZ across all pre-defined primary and secondary endpoints: mRSS was lower in the TCZ group (mean difference (95% confidence interval (CI)) -1.8 (-4.79 to 1.19), p=0.24, Figure 1A). Similarly, FVC % predicted was higher in the TCZ group mean difference (2.25, 95% CI -4.57 to 9.06), p=0.51, Figure 1B). Considering secondary endpoints, the percentage of skin progressors as well as lung progressors at follow up was lower in the TCZ group (odds ratio OR 0.67 (95% CI 0.07 to 6.41), p=0.74 and OR 0.53 (95% CI 0.16 to 1.7); p=0.2, respectively. Consistently, the percentage of regressors for skin (OR 1.6 (95% CI 0.56 to 4.54), p=0.38) and for lung (OR 1.74 (95% CI 0.66 to 4.58), p=0.26) was higher in TCZ. These results were robust regarding the sensitivity analyses. Safety analysis confirmed previously reported adverse event profiles. Conclusion:In this large, observational, controlled, real-life EUSTAR study, effectiveness of TCZ did not reach statistical significance compared to standard of care treatment but showed consistent positive effects of TCZ on skin and lung fibrosis across all pre-defined primary and secondary endpoints confirming data from recent RCTs.References:[1]Prediction of improvement in skin fibrosis in diffuse cutaneous systemic sclerosis: a EUSTAR analysis. Abstract:
Abstract In Addio alla natura (2010), a provocative essay that has enflamed a lively debate in Italy, semiologist Gianfranco Marrone argues that taking leave from Nature is the only effective way to save the environment, our human lives, and our future. By contrast, in David Vann’s Sukkwan Island, the two male protagonists (a depressed absent father and his immature son) choose Alaska’s wilderness as their ideal place for personal escapes, mental regenerations, and new beginnings. What I intend to demonstrate in this essay is how both arguments - a world without Nature and a Nature without human beings - are inevitably doomed to fail. What I propose, instead, is an idea of ContaminAction, i.e. a gradual awareness of complex issues, which need to be faced from a multiplicity of perspectives and solved through the active participation of both the human and non-human agents of our societies. Eventually, the analysis of Vann’s novella perfectly illustrates how literary texts can affect behavioral models and lifestyles. By touching upon the level of the human conscience and individual responsibility, in fact, this literary work not only reflects cogent ecocritical issues but also their practical fallouts on the reality lying beyond the page. Finally, the interconnections of nature and madness demonstrate how the redemptive power of both nature and fiction cannot transcend past errors, familial dysfunctions, self-obsessions, and a haunting sense of doom.

Abstract:
The Hippo (Hpo) pathway regulates tissue growth in many animals. Multiple upstream components promote Hpo pathway activity, but the organization of these different inputs, the degree of crosstalk between them, and whether they are regulated in a distinct manner is not well understood. Kibra (Kib) activates the Hpo pathway by recruiting the core Hpo kinase cassette to the apical cortex. Here, we show that the Hpo pathway downregulates Drosophila Kib levels independently of Yorkie-mediated transcription. We find that Hpo signaling complex formation promotes Kib degradation via SCFSlimb-mediated ubiquitination, that this effect requires Merlin, Salvador, Hpo, and Warts, and that this mechanism functions independently of other upstream Hpo pathway activators. Moreover, Kib degradation appears patterned by differences in mechanical tension across the wing. We propose that Kib degradation mediated by Hpo pathway components and regulated by cytoskeletal tension serves to control Kib-driven Hpo pathway activation and ensure optimally scaled and patterned tissue growth.

Abstract:
ABSTRACT During 2015 to 2016, Brazil reported more Zika virus (ZIKV) cases than any other country, yet population exposure remains unknown. Serological studies of ZIKV are hampered by cross-reactive immune responses against heterologous viruses. We conducted serosurveys for ZIKV, dengue virus (DENV), and Chikungunya virus (CHIKV) in 633 individuals prospectively sampled during 2015 to 2016, including microcephaly and non-microcephaly pregnancies, HIV-infected patients, tuberculosis patients, and university staff in Salvador in northeastern Brazil using enzyme-linked immunosorbent assays (ELISAs) and plaque reduction neutralization tests. Sera sampled retrospectively during 2013 to 2015 from 277 HIV-infected patients were used to assess the spread of ZIKV over time. Individuals were georeferenced, and sociodemographic indicators were compared between ZIKV-positive and -negative areas and areas with and without microcephaly cases. Epidemiological key parameters were modeled in a Bayesian framework. ZIKV seroprevalence increased rapidly during 2015 to 2016, reaching 63.3% by 2016 (95% confidence interval [CI], 59.4 to 66.8%), comparable to the seroprevalence of DENV (75.7%; CI, 69.4 to 81.1%) and higher than that of CHIKV (7.4%; CI, 5.6 to 9.8%). Of 19 microcephaly pregnancies, 94.7% showed ZIKV IgG antibodies, compared to 69.3% of 257 non-microcephaly pregnancies (P = 0.017). Analyses of sociodemographic data revealed a higher ZIKV burden in low socioeconomic status (SES) areas. High seroprevalence, combined with case data dynamics allowed estimates of the basic reproduction number R 0 of 2.1 (CI, 1.8 to 2.5) at the onset of the outbreak and an effective reproductive number R eff of <1 in subsequent years. Our data corroborate ZIKV-associated congenital disease and an association of low SES and ZIKV infection and suggest that population immunity caused cessation of the outbreak. Similar studies from other areas will be required to determine the fate of the American ZIKV outbreak. IMPORTANCE The ongoing American Zika virus (ZIKV) outbreak involves millions of cases and has a major impact on maternal and child health. Knowledge of infection rates is crucial to project future epidemic patterns and determine the absolute risk of microcephaly upon maternal ZIKV infection during pregnancy. For unknown reasons, the vast majority of ZIKV-associated microcephaly cases are concentrated in northeastern Brazil. We analyzed different subpopulations from Salvador, a Brazilian metropolis representing one of the most affected areas during the American ZIKV outbreak. We demonstrate rapid spread of ZIKV in Salvador, Brazil, and infection rates exceeding 60%. We provide evidence for the link between ZIKV and microcephaly, report that ZIKV predominantly affects geographic areas with low socioeconomic status, and show that population immunity likely caused cessation of the outbreak. Our results enable stakeholders to identify target populations for vaccination and for trials on vaccine efficacy and allow refocusing of research efforts and intervention strategies. Abstract:

The Pampas is the starting point for any investigation which aims at working on issues related to identity in Rio Grande do Sul (RS) state, in the south of Brazil, and in the Prata region. Therefore, this study reviews others that are also concerned with the development of the theme pampa in the history of the literature in the south of RS and in the Prata region and with other issues related to the historical formation of the borders in the Pampas. It aims at outlining a diachronic, fictional and critical view which can place four storytellers – the Uruguayan Eliseo Salvador Porta and Juan Capagorry and the Brazilian Sergio Faraco and Aldyr Garcia Schlee – in the contemporaneity of the process of human and spatial identity construction, with their respective guiding forces. Their tales were read individually so that, afterwards, approximations and divergences regarding the representation of men and the Pampas, the relations between both and the connections between these proposals and tradition could be found among the authors.

"The threat of redistribution under democracy: explaining income redistribution based on intersectoral relationships of the elites. Abstract:
Despite the existence of a sizable body of literature claiming that democracy would tend towards income redistribution, even from a merely procedural perspective, there is evidence that points in the opposite direction: democratic regimes can maintain—even accentuate—inequality, more obviously so in poor or developing countries. This thesis wants to emphasize one important factor which strongly intervenes in the way income is allocated under democracy: the type of relationships the elites establish. This research aims to explore the extent to which elite’s relations affect income redistribution under contexts of democracy and of high income concentration. Three hypotheses guide the development of this thesis. Firstly, if the economic elites establish cohesive relationships with i) the political elites, on the one hand, and ii) the military, on the other hand, the less the likelihood of income redistribution. Moreover, iii) the more conflictive the relations between the landed elites and the bourgeoisie are, the lower the likelihood of redistribution. Taking countries during certain periods of time as units of analysis, this research will use comparative historical analysis (CHA) as its methodological approach. Using the most similar cases design, El Salvador and Honduras were selected as the cases to compare. Three ways to approach the cases were also considered. Firstly, we use process tracing in order to look for causal explanations in history. Secondly, we break each case in three different periods of time, coinciding such periods with critical junctures of the cases, a very frequent use in process tracing. Thirdly, we codify the preferences of the actors towards wide redistributive programs which were either implemented or planned during those junctures. Our hypotheses were confirmed. However, while the two first hypotheses are displayed as potential sufficient conditions, the third one seems to be only condition of a case.

"Between revolution and democracy: Business elites and the state in El Salvador during the 1980s." Tulane University, 1993.

Abstract:
The Salvadoran business elite has been historically identified as the most powerful and reactionary of political actors in the region due to its unbridled opposition to social and political reform. At the beginning of the 1980s it faced the twin threats of marxist revolution and democratic reform. Yet at the close of the decade, it was the principal beneficiary of a 12-year-long civil war costing 70,000 lives, and an electoral reform process, both of which were aimed at reducing its political power. This study explains how the business elite was transformed by the political and economic upheavals of the 1980s to re-emerge as the dominant political actor within this country in the 1990s Three questions guide the organization of this study: (1) what role did the business elite play in the emergence of revolutionary threat in the 1970s? (2) How did the political crisis reshape the interests and behavior of the business elite? and (3) What role has the business elite played in the long transition to democracy currently underway? To answer these questions, I focus on three dimensions of state-society relations: (1) the articulation of a state hegemonic project that attempts to overcome class differences by embodying national-popular goals and rhetoric in establishing legitimacy and a national economic plan; (2) the forms and degree of state intervention in the national economy; and (3) patterns of interest representation The emergence of a revolutionary threat is explained by showing how the hegemonic project, forms of state intervention, and patterns of representation that defined the traditional relationship between state and society each underwent a crisis before the emergence of broad-based lower class revolts. During the 1980s, these crises produced the political fragmentation of the traditional agrarian-based business elite and forced a redefinition of its relationship to the state. The traditional business elite was superseded politically by a neoliberal elite based in retail manufacturing and trade by the mid-1980s, as this latter group attained institutional and ideological autonomy through the creation of the Fundacion Salvadorena de Desarrollo Economico y Social. The ascendancy of the neoliberal elite was crucial in reducing the threat of revolution, ending the civil war, and giving the business elite a stake in perpetuating democratic institutions. Thus, whereas at the beginning of the 1980s, the business elite opposed democracy, by the end of the decade they fully supported it
acase@tulane.edu

Abstract:

Abstract:
This chapter considers how the armed forces declined in power throughout Latin America in the early 1990s, but the processes of demilitarization in El Salvador and Guatemala were unique. While demilitarization followed civil wars in El Salvador and Guatemala, these are the only two cases in Latin America in which the United Nations played a major role in brokering negotiated settlements to end the armed conflicts and in monitoring peace agreements that set in motion processes of demilitarization. In both countries political opposition to continued military domination, including armed insurgencies, was a constant feature from the 1960s onward. Moreover, economic elites who traditionally looked to the military to protect their business interests increasingly expressed concern about the liability of supporting a large, well-equipped military without a mission.

Abstract:
This book contributes to the anthropology of globalization by probing how people on the ground are negotiating global inequalities in their sexual practices and intimate lives. It has shown that, while top-down globalization in the form of the tourism industry still promises to spread the wealth to reach more Brazilian citizens, Bahian sex workers, tour guides, tourism industry workers, and cultural producers are enacting “insurgent cosmopolitanism” in the form of “counter-hegemonic solidarity, bottom-up globalization.” While the government, nongovernmental organizations, journalists, and abolitionist feminists focus on sex tourism as the problem of white Western elite men exploiting poor, marginalized, Third World women, sex workers in Salvador saw opportunities for cosmopolitanism, advancement, romance, intimacy, and potential transnational mobility through their ambiguous entanglements with foreigners. The book concludes by raising questions and implications for future research on issues of race, sexuality, and globalization within cultural anthropology.

Abstract:
This chapter evaluates how the counterinsurgency campaign during the Salvadoran civil war provides support for the compellence theory. In El Salvador from 1979 to 1992, the U.S.-backed government fought the Communist and nationalist insurgency to a draw, preserving the government from an insurgent takeover. Elite accommodation took place largely among civilian and military officers in the government as hard-liners and slightly more liberal political and military entrepreneurs jockeyed for influence. The Salvadoran government resisted U.S.-pressed reforms but accepted U.S. efforts to strengthen its security forces. It used its increased fighting ability to clear civilian areas, creating vast refugee flows that reduced provision of material support to the insurgency. It also used U.S.-provided air power to break down the insurgency's conventional formations but was never able to successfully pursue and destroy the smaller bands of insurgents or gain more popular support than it began the war with. Continued insurgent political and military strength, along with the end of the Cold War, forced the United States and the hard-liners within the military to accept peace talks and a political settlement to the war rather than the military victory they had pressed for.

Abstract:
Abstract: From the pages of L'Esprit Nouveau and inspired by films, Elie Faure coined the term "cineplástique" to refer to the combination betweenimages and music, similarly to theMethode Dalcroze in dancing. Le Corbusier, Albert Jeanneret and Pierre Chenal proposed two filmic experiences related to this new concept- "Batir" and "L'Architecture d'aujourd´hui". Several models were made specifically for these films and designed to be viewed on screen. This feature became more complex in the film of the proposal model for Palace of Soviets. Le Corbusier displayed a process consisting onthede-montageof the object, reversing thus the path to the actual building, which allows the viewer to become familiar from the start with the finished piece. Thereafter ,as if it was an "écorché", their insides are displayed, featuring an architectural anatomy lesson which ends with the empty site of Salvador, where the brightness of the large steel arch ("Stal" in Russian)is reflected in the Moskva River. This filming resource is related to Sergei Eisenstein's theories of " montage as conflict", illustrated by a scale model that also verifies the effectiveness of form with respect to acoustic waves, in line with those made by engineers such as Gustave Lyon. Therefore, the combination of movement, space and sound is achieved through the architectural model. That is the seed of acoustic architecture.
