Abstract:
Object To determine if physiologically based MRI sequences can be used to detect endolymphatic sac tumor (ELST)–associated hydrops, the authors performed contrast-enhanced delayed FLAIR imaging in consecutive ELST patients with clinical findings consistent with hydrops. Methods Consecutive patients with von Hippel-Lindau (VHL) disease and clinical findings of endolymphatic hydrops and ELSTs underwent contrast-enhanced delayed FLAIR MRI. Clinical, audiological, operative, and imaging findings were analyzed. Results Three patients (2 male, 1 female) with 4 ELSTs (1 patient had bilateral ELSTs) were identified who had clinical findings consistent with endolymphatic hydrops. Computed tomography and MRI evidence of an ELST was found in all patients. Their mean age at initial evaluation was 39.7 years (range 28–51 years). All patients demonstrated progressive sensorineural hearing loss that was associated with episodic vertigo and tinnitus. Contrast-enhanced delayed FLAIR MRI clearly demonstrated dilation of the membranous labyrinth consistent with hydrops in the affected ears but not the unaffected ears. Two patients underwent resection of the associated ELST that resulted in stabilization of progressive hearing loss, as well as amelioration of tinnitus and vertigo. Conclusions Contrast-enhanced delayed FLAIR MRI can be used to detect ELST-associated hydrops. Noninvasive MRI detection of hydrops can permit earlier detection of ELSTs in patients with VHL disease and provides direct insight into a mechanism that underlies ELST-associated audiovestibular morbidity.

Abstract:
Object. Von Hippel—Lindau (VHL) disease is characterized by multiple tumors in specific organs. The cell of origin and the reason for the particular organ distribution of the tumors remains unknown. Endolymphatic sac tumor (ELST) is one of the lesions associated with VHL disease. Data from previous studies of VHL disease—associated hemangioblastomas (HBs) and renal cell carcinomas (RCCs) have indicated that VHL gene deficiency causes coexpression of erythropoietin (Epo) and its receptor (Epo-R), which facilitates tumor growth. Methods. The authors studied ELSTs from five patients with VHL germline mutations. Analysis of the five ELST samples revealed loss of the wild-type allele, consistent with Knudson's two-hit hypothesis for tumorigenesis. All five ELST specimens were characterized microscopically and by immunohistochemical analysis. Coexpression of Epo and Epo-R was found in all five tumors on immunohistochemical studies and confirmed through reverse transcription—polymerase chain reaction and Western blot analysis. Conclusions. Expression of Epo appears to be a result of VHL gene deficiency, whereas the simultaneous coexpression of Epo-R may reflect a developmental mechanism of tumorigenesis. Coexpression of Epo and Epo-R in ELSTs together with the morphological and genetic similarities of these lesions with other VHL disease—associated tumors indicates that VHL disease—associated tumors in different organs share common pathogenetic pathways.

Patients generally present with symptoms referable to the lesion's location within the middle or posterior cranial fossa. Although well characterized as a distinct entity, ELSTs involved in metastatic dissemination have never been reported. In the present report the authors describe a case of ELST metastatic to the spine treated with resection.

We present a case of a papillary tumour of the petrous bone. The established terminology for this rare neoplasm is endolymphatic sac tumour (ELST) but the true origin remains controversial. ELSTs are associated with von Hippel-Lindau disease. They are locally invasive, highly vascular and often require endovascular embolization prior to surgery. Both radiologically and histologically ELSTs are easily mistaken for other more common tumours such as paragangliomas and renal or papillary thyroid carcinoma metastases. This is important because local excision is curative.

Objective. Endolymphatic sac tumor (ELST) is a rare neoplasm arising from the intrapetrous portion of the endolymphatic sac, either isolated or in association with the von Hippel-Lindau disease. We report a sporadic case of ELST with an overview of the literature and a discussion of clinic-radiological, histopathologic, and surgical findings.Case Report. A young woman presented with a progressive hearing loss in the left ear. Otoscopy showed a reddish, bleeding hypotympanic mass. CT demonstrated an expansile lytic mastoid lesion extending to the middle ear, with bone erosion. MRI confirmed a lesion of increased signal on T1-weighted sequences. The patient underwent a canal wall-down tympanoplasty with complete removal of the tumor. Histopathology was consistent with a papillary ELST. Immunohistochemistry was positive for cytokeratin and chromogranin A.Conclusion. This paper highlights the rarity of ELST, the need for an accurate neuroradiological and immunohistochemical study at the early stages, and the timeliness of surgical treatment.

✓ Endolymphatic sac tumors (ELSTs) are locally invasive neoplasms that arise in the posterior petrous bone and are associated with von Hippel–Lindau (VHL) disease. These tumors cause symptoms even when microscopic in size (below the threshold for detectability on imaging studies) and can lead to symptoms such as hearing loss, tinnitus, vertigo, and facial nerve dysfunction. While the mechanisms of audiovestibular dysfunction in patients harboring ELSTs are incompletely understood, they have critical implications for management. The authors present the case of a 33-year-old man with VHL disease and a 10-year history of progressive tinnitus, vertigo, and left-sided hearing loss. Serial T1-weighted magnetic resonance (MR) imaging and computed tomography scans revealed no evidence of tumor, but fluid attenuated inversion recovery (FLAIR) MR imaging sequences obtained after hearing loss demonstrated evidence of left intralabyrinthine hemorrhage. On the basis of progressive disabling audiovestibular dysfunction (tinnitus and vertigo), FLAIR imaging findings, and VHL disease status, the patient underwent surgical exploration of the posterior petrous region, and a small (2-mm) ELST was identified and completely resected. Postoperatively, the patient had improvement of the tinnitus and vertigo. Intralabyrinthine hemorrhage may be an early and the only neuroimaging sign of an ELST in patients with VHL disease and audiovestibular dysfunction. These findings support tumor-associated hemorrhage as a mechanism underlying the audiovestibular dysfunction associated with ELSTs.

Abstract:
My honors senior thesis, a creative project entitled Anything Else, is a collection of fourteen poems that reflects on trauma, loss, interpersonal relationships, and nature. Many of the poems are dramatic monologues, allowing me to portray a range of extreme voices, including a survivor of the bombing of Hiroshima, a U.S. veteran of the Iraq War, and murderer Perry Smith. Although I consider myself a free verse writer, preferring to work without regular meter or rhyme, one of the poems is written in iambic pentameter. In addition, I took material from the Yahoo! Answers website and composed it as a found poem, adding to the diversity of the manuscript. A number of questions are explored across the variety of speakers, themes, and forms of poems included here, often coming back to the question of whether or not there is anything else.

Instead, characters rise to complete tasks that require action of them, however small, and hope to make some change.

Projektet kommer att gå ut på att ta fram en brottarsko som inte är universal som dem som är ute på marknaden idag. Detta är en sko som är speciellt utvecklad för en viss typ brottarstil. Brottarskon kommer att rikta sig mot brottare på elitnivå där marginaler har en avgörande betydelse av matchens resultat. För att nå högsta möjliga resultat kommer projektet gå igenom designmetodikens tre huvuddelar: Analytisk fas, kreativ fas och en utförande fas. Efter sammanställandet av informationen som givits under den analytiska fasen, kommer olika tester att genomföras för att nå bästa resultat. Valeri Nikitin är en av dessa testpersoner som jag har använt mig av. Han har bland annat blivit placerad som 1:a i VM och 4:a på OS. Slutligen sammanställs projektets alla delar i en rapport och presenteras sedan muntligt den 18 maj. Projektet ställs sedan ut veckan efter presentationen där rapport med presentationsskisser och även en fysisk presentationsmodell ställs ut.Målet kommer vara att få fram ett färdigt koncept av en brottarsko som är redo att lanseras ut på marknaden. Visionen är att få ut produkten i produktion.

Abstract:
This thesis establishes a dialogue between neuroscience and contemporary poetry, based on Bakhtin’s principle of dialogicality (1981) and presents a novel approach to combining two disciplines usually regarded as separate. The contention of the thesis is that neuroscientists and poets are often concerned with the same questions about human consciousness and seek to explore the same ‘mysteries’ and that the perspective offered by each field can be greatly enriched by the other (Burke and Troscianko, 2013). I define and develop the approach I call ‘neuropoetics’, setting close thematic studies of key poets (including Norman MacCaig, Paul Muldoon and John Burnside) and qualitative data drawn from interviews with contemporary poets in dialogue with texts and ideas from the domain of neuroscience, notably McGilchrist (2009), Ramachandran (2011), Trimble (2007) and Shermer (2011), but also including Libet et al (1983), Turner and Poppel (1983), Rizzolati (2004), Seung (2013) and others. In this thesis I relate the work of each of the contemporary poets in my study to key issues in contemporary neuroscience (specifically metaphor, patternicity, negative tropes and free will) according to the dialogical principles established at the start. I then draw the work of all three poets together in relation to the themes of memory and self-identity. In conclusion, I evaluate this thesis alongside recent fMRI studies of creative writing and consider how the two approaches could be combined in a future enquiry that gives equal weight to poetry and neuroscience.

Elsa Triolet, née dans une famille intellectuelle juive de Moscou, apprend le français dès son enfance, comme son ami Roman Jakobson. Elle se lie des sa jeunesse avec Maïakovski, Chklovski, les futuristes et formalistes proches de L. Et O. Brik. Apres son mariage, elle séjourne à Tahiti, Londres, Berlin, Paris, où elle fréquente les milieux artistiques de Montparnasse. La rencontre d'Aragon en 1928, loin d'être facile, bouleverse le cours de sa vie. De 1930 (mort de Maïakovski) à la guerre, l'activité d'E. Triolet se diversifie. Ses premiers romans portent la marque de la culture russe, des grands écrivains classiques ainsi que d'auteurs français. Leurs procédés et leurs thèmes participent cependant de la littérature russe d'avant-garde. L'étude génétique des romans, manuscrits et journaux intimes montre le passage de l' autobiographique au romanesque, les processus d'écriture, l'évolution de la structure des œuvres; elle explique pourquoi et comment, après un bref renoncement, E. Triolet revient au roman, passant du russe au français avec Bonsoir, Thérèse, roman du possible. Œuvre continue, bilingue, féministe, les premiers romans d'Elsa triolet constituent un ensemble éminemment moderne
Elsa Triolet, born in an intellectual Jewish family from Moscow, learns French language from childhood, like her friend, Roman Jakobson. In her youth, she becomes fast friend with Maiakovski, Chlovski, the futurists and the formalists in relations with L. And O. Brik. After her marriage, she stays in Tahiti, London, Berlin and Paris where she frequents the artistic circles at Montparnasse. Her meeting with Aragon in 1928 is far from being easy and upsets the course of her life. From 1930 (death of Maiakovski) to the war, Elsa Triolet diversifies her activity. Her first novels bear the stamp of Russian culture, of great classic writers as well as French authors. Their processes and themes, however, have some of the characteristics of vanguard Russian literature. The genetic study of her novels, manuscripts and diaries shows how she passes from autobiography to novel, her writing processes and the evolution of her works structure ; it explains why and how, after a brief renouncement, Elsa Triolet comes back to novel, passing from Russian to French with Bonsoir, Thérèse, a novel of the eventual. As a continuous, bilingual and feminist work, Elsa Triolet 's first novels constitute an eminently modern whole

CyA administration isssociated with histological evidenceof vascular injury. Since vascularendothelial cell components are known to modify fibrinolytic activity, we have examined fibrinolytic changes in renal allograft recipients onCyA and, in addition, markers of vascular endothelial cell damage: plasma vWFRiCof activity and AT-III activity.Parameters of fibrinolytic activity have been assessed serially in 21 renal allograft recipients for 1 year post-transplantation by study of the following: euglobulin clot lysis time (ELT), plasminogen activator activity (PAA) by fibrin plate assay, tissue plasminogen activator (t-PA) by ELISA and fast-acting t-PA inhibitor (t-PAI) by chromogenic assay.From 1 month onwards, fibrinolytic activity (ELTs and fibrin plates) is significantly decreased, p<0.002 and p<0.002 respectively, compared to normal controls, and this decrease persists at oneyear post-transplantation. Mean ELTs (mins) and ranges at 1 and 6 months and 1 year are 363 (120-1740), 598 (135-1610) and 538(80-870) respectively. Mean PAA (% of pooled normal plasma) and ranges at these times are 104 (70-154), 79 (48-118) and 83 (65-108) respectively.In contrast, serial t-PA levels (%of pooled normal plasma) are significantly increased (p<0.02), 428 (86-3686), 297 (81-1130) and 198 ( 44-637 ) at 1 and 6 months and 1 year respectively.These results suggest that the decreased fibrinolytic activity is due to increased levels of t-PA inhibitor, too high to be surpassed by the amount of t-PA released. vWFRiCof activity and AT III activity aresimilarly increased.In conclusion, CyA-treated renal allograft recipients show evidence of chronic vascular endothelial damage and a prothromboticstate.
