Abstract:
Demand for the emergency medical technician (EMT) is 2 times greater than that of all other occupations. Sustainability of ambulance services (AS) personnel is dependent upon the recruitment of EMTs into the industry. The purpose of this multiple case study was the exploration of the recruiting strategies that AS administrators used to increase the number of EMTs. Herzberg's 2-€factor theory of motivation was the conceptual framework for this study. The data collection instrument included semistructured interviews with 6 AS company executives in southeastern New Mexico and southwestern Texas. Secondary data and document from each state's emergency medical services personnel and websites related to EMTs were reviewed. Data were analyzed using thematic analysis alignment between the recruiting strategies and the conceptual framework. Two key themes emerged: AS administrators have minimal data-€driven recruitment tracking mechanisms and recruiting strategies for EMTs must align with the motivational aspects of growth, advancement, recognition, and responsibility in the AS business to entice people into the industry. The implications of this study for social change include the potential for AS executives to identify recruiting strategies they might use to increase the recruitment of EMTs to meet patient and community needs for medical transport while reducing the demand for EMTs nationwide.

Abstract:
The material in current emergency medical services (EMS) curricula is insufficient to prepare prehospital emergency medical care personnel recognize the signs and symptoms of post-traumatic stress disorder (PTSD) within their workforce. Prehospital emergency textbooks focus on treating patients affected with PTSD, but there is very little included about how EMS professionals may also be affected. Moreover, supervisors and managers of EMS agencies receive very little education on workforce PTSD in their personnel. The purpose of this study was to understand the educational preparation of EMS supervisors in order to develop a PTSD-awareness course. The research question investigated the educational preparation that EMS supervisors receive. The conceptual framework of the study was Conti-O’Hare’s wounded healer theory. EMS professionals are wounded healers from frequent critical incident exposure. A qualitative approach featuring a case study design was used. The study included 9 participants. A focus group was used that consisted of three paramedics and three emergency medical technicians (EMTs). Separate interviews were conducted with three EMS supervisors. Data gained from the focus group and individual interviews were analyzed through coding with the goal of investigating the education received by EMS supervisors on PTSD. The themes that emerged were EMS supervisors do not receive enough education on workforce PTSD and a course specifically targeted on this subject is needed. Positive social change may be achieved through this study by enabling EMS managers to help paramedics and EMTs cope with a critical incident (CI) improving prehospital healthcare.

Abstract:
Psychiatric disorders are conditions which may afflict every one of us humans. Requirements of competence have been set for the healthcare service to be able to offer an effective emergency care to patients with psychiatric disorder. The aim is to describe the ambulance nurse’s experience by caring for patients with psychiatric disorders. The method is of a descriptive qualitative design and semi-structured interviews constitute the material for the qualitative content analysis, which has been carried out with a manifest basis. The result consist of three main categories as follows, ‘difficult encounters create fear and frustration and complicates interaction’, ‘difficult assessing conditions and appropriate measurements’ and ‘care build with autonomy, experience and inventiveness’. The conclusion is how the interaction between patients and nurses is built by evidence-based nursing, how it is in nurses’ assessments of conditions to be a lack of evidence and how the care most often provided is being strived to constitute of evidence-based healthcare while how it is lacking direct ways to come in contact with such a specialised healthcare. Thus has it been shown that it exist areas for amelioration and evolvement to be able to offer an effective emergency care for patients with a psychiatric disorder.

Abstract:
99
ABSTRACT Occupational back injuries are a major health problem in developed countries for workers who are required to have repeated forceful movement and heavy lifting. Emergency Medical Technician (EMT) is an occupational group at risk of back pain problems. There are many studies examined factors related to back pain among other health professionals, especially of nurses and hospital workers, but few studies have focus on factors related to back pain among EMT. The aims of this dissertation were to: 1) Describe the work-life and demographic factors of EMT; 2) describe the occupational stress, occupational satisfaction and social supports of EMT; 3) Identify the subsets of factors associated with back pain. In order to complete the above objectives, a non-experimental, correlational design was used in this study. This study utilized a sample of 428 male firefighters completed the Minnesota Satisfaction Questionnaire (MSQ) and Chinese version of Job Content Questionnaire (C-JCQ). A questionnaire was sent to a total of 540 EMT of Kaohsiung City, and 428 EMT (response rate 79.3%) completed the questionnaire, and two-third (65.2%) of EMT reported back pain. From the analysis of intrinsic job satisfaction utilizing MSQ, the most important factor for intrinsic satisfaction in these EMT is busy at work. The most important factor for extrinsic satisfaction in these EMT is inadequate work promotion. From C-JCQ, the difference between “skill discretion” and “decision authority” is significantly different as analyzed by paired t-test (t=13.72, p<0.001) From C-JCQ the difference between “skill discretion” and “decision authority” is significantly different as analyzed by paired t-test (t=13.72, p<0.001). The average score for each question of the dimension “psychological demand” is 2.88+0.48, which shows the psychological strain is high. According to the Karasek’s model of “demand-control”, high psychological demand belongs to high job strains work, Stepwise logistic regression shows that the set of factors related to back pain included years of work from 5-10 years, automatic electrical defibrillator use, manually lifting the patient, and decision authority (R2 =0.2630, p=0.023). No statistically significance with supervisor support, age, level of education and EMT levels.

Abstract:
92
Since 1996, to obey the government’s policy, National Defense Medical Bureau appointed the sanitation duty branch in Army Logistics School to recruit Emergency Medical Technician (EMT). Until now, it has already trained more than 4,600 persons of EMT-Ⅱ and EMT-Ⅰ. It is about time to evaluate the performance and effectiveness of the training program. Accordingly, purpose of this study was to explore factors of training effects of the Army of EMT-Ⅱ and EMT-Ⅰ. This study employed Kirkpatrick''s Four Levels of Evaluation to assess the training effects of the EMT-Ⅱ and EMT-Ⅰ. The training effects were evaluated by satisfactory levels in training, the degree of the ability improvement, and the degree of practice gained from the training program. Study subjects comprised the EMT-II (203) and EMT-I (1744) and their supervisors (455). The response rates were 40.4% (82/203) for EMT-II, 40.5% (706/1744) for EMT-I, and 56.3% (256/455) for their supervisors after excluding the incomplete and invalid ones. One Way Analysis of Variance, Sheffe’s method, t-test, Pearson’s Product-Moment Correlation Analysis and multiple regression were used for data analyses. Main findings of this study were as follows: 1. The training effectiveness of the EMT might be predicted by willingness to learn, the supervisors’ supporting, and the practices of EMS (Emergency Medical Service) technique. 2. Satisfaction with the designing of training program might have effects on the personal improvement and job performance. 3. The personal improvement was associated with the job performance. 4. There were positive correlations among satisfaction with the designing of training program, personal improvement, and job performance. 5. The first three leading causes of taking the training program were promoting skill, being required for work, developing personal relationship. There were several suggestions proposed by this study based on above findings: 1. Enhancing the willingness to learn of army’s accepting EMS training, focusing on planning of training courses, and improving the satisfactory degree in their training courses can be better in improving abilities and application for EMT. 2. Promoting the knowledge and information of EMS, especially the supervisors will increase the performance of EMT. 3. Promoting the relationship between EMS department and Fire Agency in communication, coordination, and teaching resources.

Due to face the complicated scenarios and illness, the E.M.T.s always need in-service training to enhance their ability. This study aimed at the study of the need and relative influent factors for E.M.T. in-service training, such as E.M.T. personal background, training modulus, and contents of training course. Our research design adopted cross-sectional survey method, and used overall interviews toward all E.M.T.-2(Emergency Medical Technician – Class 2)who are the main force nowadys in New Taipei City Fire Department as our research basis, and then used the self-constructed structured questionnaire as the study tool. This questionnaire has already been well tested and verified by face, expertise content validity and internal consistency reliability test. Through descriptive statistics - t-test, one-way ANOVA, and Chi-square test to analyze the variation for E.M.T. personal background, training modulus and contents of training course. The study revealled four major findings: Firstly, we found that E.M.T.s regard the time arrangement as the most important part for training course, and the highest Demand for teaching methods are practical operations. Moreover, they prefer case study as their training practice and they like emergency department doctors to be their instructors, and the most requirements for contents of training course are evaluation and treatment for patients. Secondly, for training modulus, E.M.T.s who serve in the ALS (advanced life support) unit or those BLS (basic life support) units which serve frequency is over 300 times per month expressed a significantly higher need for practical operation in the teaching method and specialty doctors for instructors’ backgrounds. Thirdly, in the aspect of contents of training course, E.M.T.s who serve in the ALS unit or those BLS (basic life support) units which serve frequency is over 300 times per month expressed a significantly higher need for advanced directive med ical processes, the relevant laws, ALS skills and emotional management ... etc. Finally, we hope the study results could contribute an evidence-based foundation toward E.M.T. in-service training program, and maybe take it as a reference to improve the quality of the E.M.T. in-service education by fire departments in the future.

Abstract:
107
For some areas facing increasing medical cases and lack of firefighters, the devotion of phoenix medical volunteers helps a lot for fire department. Not only do the phoenix medical volunteers greatly relieve the workload of emergency medical cases, they also improve the medical treatment qualities before arriving hospitals. However, because of the development of modern society, the public gradually arouse their awareness toward the medical treatment qualities. It can’t be effective to train volunteers just by past experience and their enthusiasm. As a result, by researching the phoenix medical volunteers’ training performances, we can explore emergency’s training methods nowadays and find out advantages and disadvantages to improve the medical treatment skills of emergency medical technician-1 in the first moments while they arrived. This study took phoenix medical volunteers in Nantou County as example. Questionnaires and literature reviews were used to explore the medical training performance. Main findings of this study are as follows: The findings showed only "the course of continuing educational training (review training)" and "elevating the volunteers’ enthusiasm" had significant relations. "The course of review training of new facilities" and "realizing the patient’s conditions" were significantly related. As a result, it’s inferred that the devotion of training resources(review training), the arrangements of training hours and the curriculum planning don’t meet the volunteers’ need, especially for the devotion of training resources (review training) and the arrangements of training hours. The findings also revealed that only "experienced instructors" and "elevating the volunteers’ enthusiasm" were significantly related. "Mode 4 (basic skills)" and "good for teamwork” were significantly related. As a result, it’s inferred that the devotion of training resources (regular training), the arrangements of training hours and the curriculum planning don’t meet the volunteers’ need, especially for the arrangements of training hours (regular training).

According to domestic and international studies, asphyxia lasting four to six minutes will result in brain death. Therefore, to activate “chain of survival” in the golden rescue period is critical in “out-of-hospital emergency care”, including early recognition and enable emergency care system, early CPR, early AED and early intervention of ALS. EMT who participates in the emergency care must be qualified in knowledge, technical as well as capability. To operate the related equipment quickly and effectively is the technical aspect and to be provided with the sufficient ability to determine the situation based on medical science on the instant and administrate properly is the capability aspect. To provide medical based thinking through official or unofficial training is the knowledge aspect. However, evaluating the quality of emergency care technique only by the survival rate of Out-of-Hospital Cardiopulmonary Arrest is considered insufficient. After all, to do right things is always more important than do more things. Therefore, regarding to the aspects described upon, an EMT must be equipped with the ability of determination, interpersonal interaction and technique to fit the eligibility of “out-of-hospital emergency care”. The area under Fire Bureau The 4th Rescue Brigade control range from ….to Pinglin is diverse in emergency care character because of rural-urban development divide and the special rescue including Water rescue、Hiker tracking and Car accident rescue in additional to common internal and surgical emergency care. This study, based on EMT training mode, EMT resources and Embulance、equipment planning of With Fire Bureau The 4th Rescue Brigade, New Taipei City Government, investigate the gap between EMT training and real-world practice, and also conduct a questionnaire survey within EMTs to analyze and determine which aspect of ability should be enhanced in emergency care practice through EMT training.

Abstract:
The expression ``motivation`` describes the fact that the human psyche is affected by internal driving forces, both conscious and subconscious. Holland´s vocational personality typology presents six basic types, namely Realistic, Investigative, Artistic, Social, Enterprising and Conventional. This thesis is aimed at finding and evaluating motivation factors which influence prospective emergency medical technicians in their professional choice. To get the research data, the combination of quantitative and qualitative techniques was used. The research was conducted among emergency medical technician students at the college and university and, as a supportive research material, among Czech and Scottish Emergency Medical Service professionals. The students´ primary motivating factor influencing their vocational choice to become an emergency medical technician is their interest in this field, sometimes supported by a healthcare worker in the family. Over 80% respondents prefer working in the field which they study. An emergency medical technician belongs to the most popular positions while an emergency dispatcher to the less popular ones. The fact that women respondents like working positions connected with driving a car was a surprising discovery of the research. Corresponding to the recent professional literature, the most frequent career types among professionals and students are Realistic and Social types. This thesis can contribute to further research into this topic (working position preferences, career types). Component findings can be introduced into personnel and human resources management of the Emergency Medical Service, especially searching for Investigative and Conventional career types of workers and their subsequent engagement in pedagogical and administrative work. The research can also be helpful in other issues, e.g. how to make a position of an emergency dispatcher more popular.

Abstract:
Problem: The paramedic’s role has been steadily evolving internationally. Elderly and chronically ill user’s demands have lead to an increase in prehospital non urgent interventions. Confronting the conflict between the perceived role of a paramedic and the shifting reality toward non urgent care may result in a role conflict that manifests as chronic-moderate organizational stress. Mechanisms of coping related to this stress may be adopted. Objectives: The objectives of this study are to depict the Quebec paramedic’s experience related to non urgent interventions and explore their role perception and resulting attitudes of cynicism and disengagement. Method: Qualitative data from semi-structured interviews have been codified and analyzed based on a model adapted from the literature on works in socio-psychology and health administration theories. (n=13, 3 administrative regions, less than 3 years to more than 20 years of career) Results: Paramedics acknowledge experiencing role conflict as they explain the formation of their work and field of practice as being exclusively oriented toward emergency response, contrary to their pervasive work reality. They demonstrate cynicism and disengagement, and specify it as being inherent to the professional experience and intimately linked with burnout, affecting quality of care despite fundamental professional intentions. Conclusion: Paramedics described different professional coping mechanisms aimed at acknowledging the conflicting duality of their practice, amongst them, showcasing their clinical capability to play an active role in primary health care in the prehospital, non urgent setting and adjusting and enhancing the training programs.

Abstract:
Emergency Medical Technicians are exposed to a large diversity of situations and have constantly to adapt their actions to the circumstances that they face, making this profession highly demanding in several aspects. This cross-sectional study focuses on the analysis of the risk of musculoskeletal disorders in patient handling tasks. The study encompassed two phases: The first, based on a questionnaire, answered by 51 professionals, that comprised socio-demographic characteristics, variables related with lifestyle, sleep and activity, the Portuguese medium version of COPSOQ II and the Portuguese adapted version of standardized Nordic Musculoskeletal Questionnaire; and the second, systematic observations of 340 postures, among 20 technicians, with subsequent application of the REBA method to four tasks (lifting the stretcher, descending the stretcher, placement of the chair in the ambulance and transport of the chair on stairs). The prevalence of musculoskeletal symptoms was higher in the lower back, upper back and shoulders. Statistically significant differences were identified in at least one segment, in 19 of the 32 COPSOQ II scales. These differences were found mainly in shoulders, neck and upper back, revealing, in these cases, an association between musculoskeletal symptoms and psychosocial risk factors. According to the REBA results, all tasks had a medium risk associated; the task of transportation of the chair on stairs, in the low position, obtained the most critical result, with 43.6% of the analyzed postures presenting a high risk. Ergonomics can have a decisive contribution in this context not only through the prevention of occupational risks but also ensuring greater comfort and satisfaction of the EMT’s, without neglecting their productivity.
