Nei giorni 10 e 11 maggio 2006 si è tenuto a Firenze, presso l'Aula Magna del Rettorato, il Convegno internazionale sul tema "Fiume, paesaggio, difesa del suolo. Superare le emergenze, cogliere le opportunità". L'input culturale e scientifico di tale iniziativa (promossa ed organizzata dal Dottorato di Ricerca in Progettazione Paesistica – prof. Giulio G. Rizzo, dott. Michele Ercolini – e dal Master in Paesaggistica – prof. Guido Ferrara – dell'Università degli Studi di Firenze), vale a dire il tentativo di trasformare un "sistema di esigenze" (riconducibile a necessità di difesa del suolo) in un "sistema di opportunità" per la progettazione di "nuovi paesaggi", ha contraddistinto la maggior parte delle relazioni presentate nelle due giornate di lavoro ed inserite nel presente volume.

This chapter considers the extent to which it is still meaningful to conceptualize pro-drop phenomena in parametric terms, introducing a three-factors model in which parameters are emergent, not UG-given. Within this model, it seems possible to distinguish macro, meso, and micro pro-drop systems. The attested systematic variation in even the most familiar instantiations of these putative types, however, raises questions about existing parametric accounts of the acquisition and typological relationship between these systems. Drawing on parallels with a neo-emergentist account of word-order variation, the chapter argues for an approach assuming interdependent parameters (a parameter-hierarchy) where the ‘size’ and precise formal specification of pro-drop in individual grammars is determined by the way the model’s three factors interact, with different formal features playing potentially parallel roles in different systems. The typological picture is thus more variation-rich than previously assumed, but this variation exhibits the kind of cross-linguistic systematicity a parametric approach predicts.

Emergent Israel’s position in the central Canaanite and Transjordanian highlands can be traced to an earlier culture of conflict between Egyptian forces and a precursor group identified in the Merneptah stele as “Israel” in the last decade of the thirteenth century BCE. This community eventually gave rise to Iron Age Israel, an agrarian population characterized by the leadership of chieftains who conceived of their patron deity (a form of the Canaanite El) as their own divine kinsman, protecting their settlements against threats from Egypt and pro-Egyptian Canaanite regents. The Song of the Sea (Exod 15:1b–18) bears early witness to the myth of group identity forged through these experiences, where Israel’s deity clears the sacred landscape of the Egyptian foe and then plants his people therein. This reflects the foundations upon which the Levites would come to build their own nascent ideology in subsequent decades.

This chapter introduces the ‘eventful sociology’ that characterizes the emergence of protest episodes in the four North African countries. Events are non-routine sequences of actions that reshape the routine forms of governance (and opposition) structuring everyday social and political life. Transformative events initiate a transformation of behaviors that is both strategic and reactive, and that reshapes social and political life first at the local level. This chapter qualifies the emergence of new causal processes and how they interact with preexisting practices of governance. The narrative places side by side the views and strategies of different pro- and anti-regime actors in the face of unexpected events and their consequences. The chapter outlines how sequences of events produced new practices, arenas and actors of contestations, often as unintended consequences of interactions. This event-centric account of protest episodes highlights the transformative role of protest in the construction of newly effective forms of political behaviors.

Neoliberalism: A Very Short Introduction explores the considerable variations of neoliberalism around the world, and discusses the origins, evolution, and core ideas of neoliberalism. Neoliberalism emerged as the world's dominant economic paradigm in the 1990s. The global financial crash of 2008 and the subsequent emergence of more nationalist ideologies have challenged both neoliberal assumptions and related financial systems—a development most spectacularly reflected in 2016's pro-Brexit referendum in the UK and the Trump election victory in the same year. This VSI asks whether changing versions of neoliberalism might succeed in drowning out the calls for a return to territorial sovereignty and national greatness.

In many Indo-European languages, pronouns (and other clitic-like elements) tend to appear in second position or near it. This phenomenon was first described by Jakob Wackernagel, after whom the position is named the Wackernagel position (WP). This chapter describes the emergence of the WP in German where it is the third position following SpecCP and C. Since subject clitics in the WP interact phonologically and morphologically with verbs and complementizers in C, three additional syntactic features (double agreement, complementizer agreement, partial pro-drop) are associated with the WP in German (forming the so-called Wackernagel complex). The chapter surveys the evidence for the existence of the WP in OHG and, to a lesser extent, in MHG, using the additional features as diagnostics. It also contains a new explanation of how complementizer agreement could have emerged.

This chapter outlines of the history of a secret press that appeared in London in 1640–1. Growing out of earlier radical propaganda networks, and created to provide an outlet for the dispersal of pro-Covenanter Scottish propaganda in England, the press distributed incendiary politico-religious tracts, which challenged Caroline policy at multiple levels. The chapter analyzes the ideas presented in these works, including contract theory, resistance theory, pleas for the demolition of the existing Church of England, extreme separatist propaganda, tolerationist arguments, and challenges to clerical monopolies. It traces personnel involved in the enterprise (including the future Leveller, Richard Overton) and assesses the impact of the propaganda dispersed by the press. The secret press provides a crucial vehicle for understanding the changing dynamics of print in the 1640s, the emergence of novel arguments against press censorship, and the later spread and development of radical political and religious ideas.

This chapter examines the productions of NFL Films that not only document but also exalt the National Football League (NFL). NFL Films productions document and provide historical records of NFL games. It creates the league's history by arranging exceptional moments into celebratory narratives, such as Greatest Moments in Dallas Cowboys History (1992), Era of Excellence: The 1980s,(1989), and the syndicated television program NFL Game of the Week. NFL Films' documentaries suggest the NFL's past is constituted by extraordinary moments—diving touchdown catches, punishing blocks, and graceful runs—that evidence the league's unique excitement and epic importance. This chapter discusses NFL Films' production practices that aim to glorify pro football and how its material is influenced by the media outlets for which the company produces content. It shows that NFL Films' highlights offer a model through which to explain the emergence of the contemporary sports highlight—perhaps the most powerful and prevalent genre in sports media.

This chapter analyzes the media's role in the containment of Kurdish ethnic nationalism and political Islam as undertaken by the military-led state in the 1990s. In this transformational decade, the emergence of new actors—such as commercial media, civil society organizations, Islamist networks, and Kurdish activists—created serious concerns for the Kemalist elite and the political economic order they had established decades ago. From the military-bureaucratic circles to the pro-state big capital owners, these power holders hoped to preserve their clout and sustain the central power of the state at a time when the country was encountering global, neoliberal currents. The chapter first discusses the reproduction of nationalist ethos in mainstream media and the state suppression of Kurdish media, both domestically and transnationally. It then investigates the state's attempts to rein in political Islam and the role mainstream media assumed in this process. Emphasis is placed on the political economic pacts between military-bureaucratic elite and media proprietors.

This chapter begins with an analysis of the shifts in global and local conjunctures that facilitated the Islamist AKP's (Adalet ve Kalkinma Partisi or Justice and Development Party) rise to power, followed by an overview of its neoliberal and pro-EU policies during its first term. It then explores how the anti-Western and anti-globalization currents became substantial elements of media, politics, and culture in the twenty-first century. Local and international developments—such as the EU accession process, the relative easing of restrictions on Kurdish cultural rights, the US invasion of Iraq, the emergence of a revisionist discourse on the Armenian genocide, and the entry of foreign media companies into the Turkish market—began to engender fears and anxieties among the nationalists about the decline of the Turkish state. Through the lens of these developments, the chapter discusses the tensions between globalizing and statist dynamics as well as the AKP's consolidation of the authoritarian neoliberal order.

This chapter explains how the outbreak of the civil war led to serious divisions within parliament’s camp. These divisions, over the nature of the war, the character and function of the king, and the propriety of negotiation and compromise with the royalists, reflected and further intensified shifts in the nature of political ideology. These emergent ideological shifts, first articulated in the petitions and pressure campaigns of hard-line supporters in the city, were bolstered by the publication of tracts and pamphlets, which revealed the increasingly radical tenor of the political thought gestating among pro-parliamentary militants. Examination of those tracts shows that, by early 1643, some parliamentarians were coming to reject Westminster’s official line on the war effort, along with fundamental features of the “ancient constitution” as it had been understood before 1642.

This book explores how NFL Films shaped the way Americans view football and paved the way for the emergence of cable television and Internet sports media. Baseball is traditionally recognized as America's favorite pastime, but the country's most popular and lucrative sports organization since the late 1960s has been the National Football League (NFL). NFL football's tremendous cultural and economic power is not simply a product of the games it provides for millions of live and mediated spectators, but also its cultural meanings. More than merely a game, the sport embodies and articulates characteristics, beliefs, attitudes, and values unique to American history, identity, and everyday life. This book examines the ways that NFL Films' productions changed how pro football, and sport in general, is represented and imagined while establishing a foundation from which the contemporary sports media landscape—an almost unavoidable facet of popular culture—developed. It discusses the institutional and cultural history of NFL Films as well as its circulating and archived productions, the discourses it generates, and the discourses surrounding the company.

Brains as Engines of Association seeks an operating principle of the human brain and is divided into four parts. The first part (“What Nervous Systems Do for Animals”) is intended to set the stage for understanding the emergence of neural systems as promoting what all organisms must accomplish: survival and reproduction. The second part (“Neural Systems as Engines of Association”) lays out the general argument that biological sensing systems face a daunting problem: they cannot measure the parameters of the world in the way physical instruments can. As a result, nervous systems must make and update associations (synaptic connections) on the basis of empirical success or failure over both evolutionary and individual time. The third part (“Evidence that Neural Systems Operate Empirically”) reviews evidence accumulated over the past 20 years that supports this interpretation in vision and audition, the sensory systems that have been most studied from this or any other perspective. Finally, the fourth part (“Alternative Concepts of Neural Function”) considers the pros and cons of other interpretations of how brains operate. The overarching theme is that the nervous systems of humans and every other animal operate on the basis associations between stimuli and behavior made by trial and error over species and lifetime experience.

This book offers a reception history of biblical Hagar. Despite the limited description of Hagar in biblical texts, she becomes an important figure in Judaism, Christianity, and Islam. While many interpreters often treat biblical characters as White or as non-Black, this book investigates how, when, and why some interpreters choose to identify biblical Hagar as a Black woman. It centers on treatments of Hagar by African American biblical interpreters and focuses attention on how professional and non-professional interpreters identify and engage issues of difference, including gender, race, ethnicity, and status. Building on the work of African American classicists and biblical scholars, Reimagining Hagar discusses the African presence in biblical texts and issues of race within biblical studies. It details the portrayal of Hagar in nineteenth century pro- and anti-slavery literature in the United States, in African American vernacular traditions, and in religious studies and biblical studies scholarship. Engaging an array of literary and artistic sources, Reimagining Hagar illustrates that interpretations of Hagar as Black emphasize elements of Hagar’s narrative in order to connect her with or disassociate her from particular groups. This book traces some of the key points within the emergence and development of this unique understanding of Hagar as a Black woman.

This book develops a political philosophic approach to restitution and repatriation of objects, by arguing that the development of restitutive norms in the West has been auxiliary to the emergence of modern state sovereignty. It draws on critiques of international law of cultural heritage return, and of its Western humanistic underpinnings, including the ontological binary distinction between things and persons. Rather than accept the restitutive goals of politics and law seeking to do justice for the past and to ‘undo’ the expropriations and dispossessions that have occurred, and are still occurring (be it in contexts of coloniality or war), this book looks at the limits and aporias of restitution in texts of philosophy, literature and social theory. As such, it identifies figures and objects situated beyond the possibility of restitution and repair. This includes analysis of the social fantasies and imaginaries that ‘prop’ our contemporary reparative politics—making the past ‘unhappen’, or cancelling out the occurrence of wrongs. What the analysed texts have in common is that they articulate restitution through the motifs of undoing and making-unhappen, as a reparative and curative procedure, and a prelapsarian return to a place, time or condition prior to the event of violence. Insofar as this reading uncovers the mythical-religious ‘substrate’ of the restitutive tradition, and illuminates the political and affective allures of prelapsarianism, this book also offers insights into Western secularism, not as disappearance of religious thought in the public domain, but as its ‘repression’ (in a psychoanalytic sense).

The European Union (EU) has been through almost two decades of near-constant constitutional crisis. The failure of the Constitutional Treaty was followed in quick succession by the struggles about the Lisbon Treaty, the Eurozone emergency, Brexit, and, recently, Corona-crisis-induced conflicts about financial solidarity. Over the course of these events, it has become clear that the EU’s constitutional development largely evades popular control. At the same time, the EU faces increasing politicization from below. While Eurosceptic forces seek to ‘take back control’ at the national level, pro-European citizens challenge the role of the states as the ‘masters of the treaties’. They reclaim what—in their view—has been illegitimately withheld from them: the right to shape the EU polity. This book advances the argument that these developments prompt the need for a theory of constituent power in the EU. The reason why European integration eludes citizen control and meets with growing discontent is that it allows constituted powers to operate as de facto constituent powers. Starting from claims to founding authority articulated in public narratives, the book explores competing models of constituent power in the EU—regional cosmopolitanism, demoi-cracy, split popular sovereignty, and destituent power—revealing their respective strengths and weaknesses. Rationally reconstructing established democratic practices of EU constitutional politics, it develops a new theory of constituent power in the EU. Addressing questions of the adequate conceptualization, allocation, agency, and institutionalization of constituent power, the book opens up the prospect of a more democratic mode of European integration.

Framed in debates about the crisis of democracy, the book analyzes one of the most influential social movements of recent times: Spain’s “Indignados” or “15-M” movement. In the wake of the global financial crisis and harsh austerity policies, 15-M movement activists occupied public squares across the country, mobilized millions of Spanish citizens, gave rise to new hybrid parties such as Podemos, and inspired pro-democracy movements around the world. Based on access to key participants in the 15-M movement and Podemos, and extensive participant observation, the book tells the story of this remarkable movement, its emergence, evolution, and impact. In so doing, it challenges some of the core arguments in social movement scholarship about the factors likely to lead to movement success. Instead, the book argues that movements organized around autonomous network logics can build and sustain strong movements in the absence of formal organizations, strong professionalized leadership, and the ability to attract external resources. The key to understanding its power lies in the shared political culture and collective identity that emerged following the occupation of Spain’s central squares. These protest camps sustained the movement by forging reciprocal ties of solidarity between diverse actors, and generating a shared set of critical master frames across a diverse set of actors and issues (e.g., housing, education, pensions, privatization of public services, corruption) that enabled the movement to effectively contest hegemonic narratives about the crisis, austerity, and democracy, influencing public debate and the political agenda.

Nos dias atuais, a administração (de empresas ou pública) tem assimilado conceitos e características de natureza empreendedora, que levam a mudanças de paradigmas dentro da própria área do conhecimento. Nesse limiar, a administração e o empreendedorismo se tornaram indissociáveis não apenas a nível acadêmico, mas na prática gerencial cotidiana. Com efeito, esse cenário chama a atenção dos pesquisadores e, cada vez mais, estudos são desenvolvidos com o foco em novas aplicações do conhecimento gerencial. É pensando nessa indissociabilidade, que este livro surge. Organizado em duas partes, o livro traz 15 capítulos que abordam temas extremamente atuais e emergentes que partem, ora de uma compreensão macro, ora de uma compreensão micro dos fenômenos que impactam diretamente as organizações, passando por temas, como: perfil empreendedor, liderança, pandemia, gestão estratégica, gestão de serviços, gestão de pessoas, gestão pública. O Capítulo 2, “Aversión Al Riesgo de Fracasar al Emprender un Negocio en México: Un Acercamiento con algunos Factores Educativos” (em tradução livre, Aversão ao Risco de Fracassar ao abrir um Negócio no México: Uma abordagem com alguns Fatores Educacionais), traz um estudo desenvolvido no México que relaciona a baixa escolaridade, a falta de acesso à educação e, consequentemente, a dificuldade de inserção no mercado de trabalho como condicionantes para a intenção de empreender. Trazendo dados demográficos e socioeconômicos, o estudo contribui com uma discussão extremamente pertinente para a compreensão do empreendedorismo por necessidade e seu impacto direto no desenvolvimento produtivo local – um cenário muito semelhante ao contexto brasileiro. O Capítulo 3, “A Importância da Gestão em Tempos de Crises: Uma Reflexão sobre a Pandemia da Covid-19”, trata de uma temática emergente e, por meio de uma interessante revisão de literatura, destaca práticas e ações a nível gerencial que impactam diretamente a vida nas organizações no momento pandêmico. Nos Capítulos 4 e 5, “Financiamento para Inovação: Pontos Positivos e Negativos do Crowdfunding” e “Financiamento da Inovação por Meio do Crowdfunding: Fatores de Sucesso”, descrevem os principais pontos (positivos e negativos) da prática de financiamentos coletivos ou crowdfunding, por meio de uma análise focada na inovação tecnológica. Os capítulos trazem contribuições principalmente para micro e pequenas empresas que atuam com inovação e necessitam superar dificuldades de acesso a recursos financeiros. No Capítulo 6, “Empresas Familiares: Gestão e Características Empreendedoras em Tempos de Pandemia da COVID-19”, tem-se um levantamento com gestores de empresas familiares, em que são discutidas as relações gerenciais e sociais com implicação direta no cotidiano organizacional, com um enfoque direto neste tipo de organização. Além disso, é um estudo extremamente atual, pois traz uma contextualização sobre os desafios e impactos da Pandemia da COVID-19 no mundo empresarial. O Capítulo 7, “A Liderança Feminina nas Organizações de Trabalho: Desafios e Construções Sociais”, traz uma importante discussão sobre a igualdade de gênero e os desafios que as mulheres enfrentam no mercado de trabalho e no mundo empresarial. Por meio de uma pesquisa empírica, o capítulo aponta para uma profunda reflexão sobre o enfrentamento à desigualdade que envolve, de maneira coletiva, diversas áreas do conhecimento, instâncias e organizações sociais e educacionais. Fechando a primeira parte do Livro, o Capítulo 8, “Emergencia de Nuevos Modelos de Gestión Estrategica” (em tradução livre, Surgimento de Novos Modelos de Gestão Estratégica), embarca em uma análise teórica visando responder à seguinte questão: quais novas perspectivas e modelos de gestão estratégica estão surgindo para dar suporte ao desenvolvimento das empresas na atualidade? No Capítulo 8, “Abordagem de Servicescape: Análise e Influência da Experiência do Consumidor em um Ambiente de Serviço”, analisa-se a influência da servicescape (ambiente físico onde acontece o serviço) na experiência do consumidor no processo de aquisição do serviço. Por meio de um estudo bibliométrico, o capítulo aprofunda sobre o servicescape e sobre o experienscape, levando em consideração também o que tem sido produzido academicamente na temática. O Capítulo 9, “A Importância do Treinamento como Estratégia de Potencialização do Desempenho dos Colaboradores nas Organizações”, faz uma breve análise sobre Treinamento, Desenvolvimento & Educação (TD&E), identificando os fatores que tornam esse processo tão relevante para as organizações, especialmente como estratégia de potencialização do desempenho dos colaboradores. Trazendo alguns conceitos gerais sobre a temática, o capítulo traça uma relação entre o processo de treinamento a nível de gestão de pessoas e o aumento da produtividade e a redução de custos. O Capítulo 11, “Cultura Organizacional: Uma Revisão Narrativa acerca das Tipologias e Influências na Gestão”, analisa os eventuais impactos que a cultura organizacional perfaz na gestão das organizações. Compreendendo as diversas tipologias culturais existentes, o capítulo oferece uma visão da gestão da cultura organizacional em prol da eficiência e do aumento da produtividade. No Capítulo 12, intitulado “Aplicação da Teoria das Filas em uma Hamburgueria: Um Estudo de Caso”, descreve uma pesquisa empírica realizada em um restaurante, do tipo hamburgueria, abordando-se a concepção do sistema de filas. O capítulo traz resultados interessantes para a compreensão dos fatores que envolve a gestão de filas dentro um ramo de negócio em extrema expansão no Brasil, que é o mercado de hambúrgueres e lanches. A partir deste momento, o livro se foca em estudos relacionados à gestão pública, trazendo 3 estudos que, individualmente, abordam a administração a nível municipal, estadual e federal. No Capítulo 13, “A Maturidade da Gestão do Conhecimento na SEAPI – Secretaria da Agricultura, Pecuária e Irrigação do Rio Grande do Sul (RS)”, tem-se um estudo que analisa como a gestão do conhecimento está inserida no ambiente organizacional de uma secretaria de estado, fazendo um paralelo teórico-empírico com o nível de maturidade gerencial. Este capítulo contribui especialmente para o mapeamento da maturidade em gestão do conhecimento em organizações públicas, apresentando ferramentas e técnicas para este tipo de análise. No Capítulo 14, intitulado “Avaliação dos Processos Gerenciais da Prefeitura de Luis Gomes/RN à Luz do Modelo de Excelência em Gestão Pública”, abordam-se os processos administrativos e de prestação de serviços de uma prefeitura. Neste capítulo, é utilizado um modelo padrão de análise baseado em excelência, cuja principal contribuição está em propor pontos de correção nos processos gerenciais cotidianos e ações estratégicas a serem realizadas para a melhoria da prestação de serviços aos cidadãos. Dando continuidade aos estudos relacionados à gestão pública, tem-se, por fim, o Capítulo 15, “Pregão Eletrônico nas Aquisições Públicas Federais: Economia e Impactos a Luz da Normativa 03/2011”, que trata do impacto causado na economia creditada ao uso do Pregão Eletrônico, após a implementação da Instrução Normativa nº 3/2011 do Ministério do Planejamento, Orçamento e Gestão (MPOG). Nesse sentido, o capítulo traz uma análise sobre a teoria dos leilões e a dinâmica do Pregão Eletrônico, à luz das demandas e necessidades da administração pública federal. Cabe ressaltar que este livro é fruto do esforço de cooperação desenvolvido por pesquisadores de diversas Instituições nacionais e estrangeiras, em que cada estudo apresentado perfaz uma particular contribuição para a área do conhecimento. E embora não se pretenda responder a todas as questões relacionadas à Administração e ao Empreendedorismo, esta obra traz algumas reflexões e discussões atuais que podem, de algum modo, contribuir positivamente com o avanço do conhecimento e incrementar a literatura na área.

En Medicina, el área de aplicación de Ginecología, la ciencia de la mujer condensa el estudio de las enfermedades frecuentes y graves, el diagnóstico, detección de los factores de riesgo y establecer mecanismos de prevención, prescribir los tratamientos médicos y quirúrgicos de las enfermedades del sis- tema reproductor femenino, entiéndase, todo lo relacionado con la vagina, las mamas, el útero y los ovarios. Durante el siglo XX, motivado por el crecimiento acelerado del conocimien- to científico y médico, se acrecienta la toma de conciencia del rol que le co- rresponde desempeñar a la medicina en el sector de la salud y la protección de la mujer embarazada. Los problemas del trato genital femenino cuando se asumen como responsabilidad de los ginecólogos, quienes incluyeron dentro del proceso de auscultación, diagnóstico y tratamiento aspectos fisiológicos y endocrinos. Las barreras de la formación académica se fueron difuminando y los ginecó- logos y obstetras comenzaron a estrechar su campo laboral y como resultante se constituyó la Ginecobstetricia. En el marco de estas reflexiones, surge la idea de la presentación de un tex- to titulado Ginecología – Obstetricia, mediante el cual se pretende hacer una contribución real a nivel teórico que permita apoyar a profesionales y estu- diantes en el área de salud humana, básicamente en algunas de las patologías o complicaciones médicas asociadas al embarazo, y tratadas por la especialidad obstétrica, así mismo, se abordan dos temas (1 y 2) de conocimiento general. Cabe indicar que el texto no pretende abordar la vasta información o literatura que sobre los temas se han tratado. El libro ha sido estructurado bajo el perfil de diez (10) temas que discurren estrictamente sobre contenidos específicos, a sa- ber: 1. El parto y sus fases, 2. Pruebas de Bienestar Fetal, 3. Amenaza de Parto Pretérmino, 4. Ruptura Prematura de Membranas, 5. Amenaza de aborto, 6. Desprendimiento de placenta, 7. Infecciones de vías urinarias en embarazadas, 8. Diabetes Gestacional, 9. Hipertension en las embarazadas y 10. Preeclamp- sia y eclampsia En el primer tema, el Parto y sus fases, se precisan diferentes nociones sobre 26 GINECOLOGIA - OBSTETRICIA el proceso y el resultado de parir (dar a luz). A lo largo de la historia ha evolu- cionado el conocimiento de este tema dando como resultado una terminología precisa sobre los diferentes tipos de parto: parto natural, parto normal, parto ins- trumental, parto pretérmino, parto humanizado, etc. Estas nociones obedecen a determinadas circunstancias específicas que lo circunscribe como el uso o no de instrumentos que ayuden al nacimiento de un feto. De manera general, el parto marca el final del embarazo y el nacimiento de la criatura que se engendraba en el útero de la madre. Este proceso por el que la mujer o la hembra de una especie vivípara expulsa el feto y la placenta al final de la gestación consta de tres fases: la fase de dilatación, la de expulsión y la placentaria o de alumbramiento. En el segundo tema titulado Pruebas de Bienestar Fetal, se destaca el desa- rrollo de diferentes pruebas para el control del bienestar fetal. Éstas constitu- yen las técnicas aplicadas a las madres que permiten predecir el posible riesgo fetal o hacer un pronóstico del estado actual del feto, es decir, que tratan de conseguir a través de una valoración del feto de forma sistemática, la identifi- cación de aquellos que están en peligro dentro del útero materno, para así to- mar las medidas apropiadas y prevenir un daño irreversible. Se abordan en este contexto las indicaciones y los métodos (clínicos, biofísicos y bioquímicos más utilizados para el control de bienestar fetal. En el tema tres (3) denominado Amenaza de Parto Pretérmino, el trabajo se centra, en el desarrollo de los siguientes ítems. La Definición de Parto Pretérmi- no, la Definición de amenaza de Parto Pretérmino, la Evaluación del riesgo, la etiología, la Clínica de la Amenaza de Parto Pretérmino, el Diagnóstico precoz de la Amenaza de Parto Pretérmino, la Evaluación de gestantes que acuden a emergencia por signos y síntomas de Amenaza de Parto Pretérmino y el trata- miento. El trabajo parte de la definición de Parto Pretérmino entendido como aquel que ocurre después de la semana 23 y antes de la semana 37 de gestación, para posteriormente, tratar lo relativo a la Amenaza de Parto Pretérmino (APP) definido como el proceso clínico sintomático (Aparición de dinámica uterina regular acompañado de modificaciones cervicales) que puede conducir a un parto pretérmino. Su etiología es compleja y multifactorial, en la que pueden intervenir de forma simultánea factores inflamatorios, isquémicos, inmunológi- cos, mecánicos y hormonales. 27 GINECOLOGIA - OBSTETRICIA Por otro parte, el tema cuatro (4) expone la Ruptura Prematura de Membra- nas, la cual constituye una complicación usual en la práctica obstétrica, esta puede aumentar la incidencia en la morbilidad y mortalidad materna – fetal. Múltiples estudios se están llevando a cabo para poder dilucidar completamente su fisiopatología, lo cual se hace cada vez más necesario para poder aplicar estos conceptos en la práctica clínica, la evidencia actual indica que la Ruptura Prematura de Membrana es un proceso que puede ser afectado por factores: bioquímicos, fisiológicos, patológicos y ambientales. El capítulo cinco (5) comprende la temática sobre la Amenaza de aborto. (AA) que es la complicación más común durante el embarazo, se define como el sangrado transvaginal antes de las 20 semanas de gestación (SDG) gestación o con un feto menor de 500g, con o sin contracciones uterinas, sin dilatación cervical y sin expulsión de productos de la concepción”. Es decir, se presenta hemorragia de origen intrauterino antes de la vigésima semana completa de ges- tación, con o sin contracciones uterinas, sin dilatación cervical y sin expulsión de los productos de la concepción. Los síntomas abarcan amenorrea secundaria, presencia de vitalidad fetal y cólica abdominales con o sin sangrado vaginal entre otros. Para el diagnóstico se puede hacer una ecografía abdominal o va- ginal, examen pélvico y de laboratorio. En un principio el tratamiento consiste en recomendar reposo en cama y reposo pélvico. La identificación de factores de riesgo, el Ultrasonido obstétrico y la medición de marcadores bioquímicos son de gran importancia para realizar un diagnóstico y establecer un pronóstico oportuno. Estos aspectos y otros relacionados con el tema como son: la clínica, el protocolo a seguir, el tratamiento y la prevención, son tratados en este capí- tulo. El tema Desprendimiento de placenta es desarrollado a lo largo del tema seis (6). Su contenido aborda los aspectos importes como los factores de riesgo, etiología, síntomas y signos, diagnóstico y tratamiento de esta complicación cuyo proceso se caracteriza por el desprendimiento total o parcial, antes del parto, de una placenta que esta insertada en su sitio normal. Este hecho que puede traer grandes consecuencias para el feto y para la madre, puede ocurrir en cualquier momento del embarazo. Los desprendimientos producidos antes de las 20 semanas, por su evolución, deberán ser tratados como abortos. Los que tienen lugar después de la semana 20 de gestación y antes del alumbramiento constituyen el cuadro conocido como desprendimiento prematuro de la placenta normalmente insertada. (abrptio plantae o accidente de Baudelocque). El pro- ceso ha tenido una variedad de denominaciones a lo largo del tiempo y son consecuencia de la diversidad de cuadros clínicos que pueden producirse, sien- do las más empleadas en la actualidad: abruptio placentae, ablatio placentae, desprendimiento prematuro de placenta normalmente inserta (DPPNI), junto con el término abreviado desprendimiento prematuro de placenta (DPP). Para hablar de otra importante complicación que aqueja a la gestante y su bebe por nacer se expone en el tema (7) relacionado con las Infecciones de vías urinarias en embarazadas. Los particulares cambios morfológicos y funcio- nales que se producen en el tracto urinario de la gestante hacen que la infec- ción del tracto urinario (ITU) sea la segunda patología médica más frecuente del embarazo, por detrás de la anemia. Las 3 entidades de mayor repercusión son: Bacteriuria asintomática (BA) (2-11%), cuya detección y tratamiento son fundamentales durante la gestación, pues se asocia a prematuridad, bajo peso y elevado riesgo de progresión a pielonefritis aguda (PA) y sepsis; la Cistitis aguda (CA) (1,5%) y la Pielonefritis aguda (1-2%), principal causa de ingreso no obstétrico en la gestante, que en el 10 al 20% de los casos supone alguna complicación grave que pone en riesgo la vida materna y la fetal. La Diabetes Gestacional se ubica y desarrolla en el tema ocho (8). Este tipo de diabetes que aparece o se diagnostica durante el embarazo ha aumentado su prevalencia y cobrado gran relevancia epidemiológica en los últimos años. La Diabetes Gestacional (DG) o Diabetes Mellitius Gestacional (DMG) se carac- teriza por una secreción de insulina insuficiente para compensar la resistencia a la hormona, propia del embarazo. Después del parto, los niveles de glucosa sanguínea suelen normalizarse; sin embargo, algunas mujeres desarrollan DM tipo 2 y se asocia con complicaciones graves en la madre y el hijo, incluso años después del nacimiento. La Hipertensión en las Embarazadas, tema tan tratado y controvertido en los últimos años por su significación a nivel de que es la complicación médica 29 GINECOLOGIA - OBSTETRICIA más frecuente de la gestación y ocurre según estudios comprobados en el 7% a 10% de los embarazos y constituye una causa importante de morbimortalidad materna y perinatal. De manera clásica, la HTA en el embarazo ha sido definida como el incremento, durante la gestación, de la presión arterial sistólica (PAS) en 30 mmHg o más y/o la presión arterial diastólica (PAD) en 15 mmHg o más comparado con el promedio de valores previos a la 20va. semana de gestación. Cuando no se conocen valores previos, una lectura de 140/90 mmHg o mayor es considerada como anormal. El tema desarrollado abarca una visión general sobre algunos aspectos relativos a la definición y su clasificación, los factores predisponentes, sintomatología, diagnóstico, tratamiento, etc. Por último, el tema 10 aborda dos alteraciones íntimamente ligadas a la hi- pertensión arterial en el embarazo: la preeclampsia y la eclampsia. Éstas son en ocasiones tratadas como componentes de un mismo síndrome ya que la pree- clampsia es la hipertensión de reciente comienzo con proteinuria después de las 20 semanas de gestación y la eclampsia es la presencia de convulsiones genera- lizadas inexplicables en pacientes con preeclampsia.
