AbstractAfrica continues to experience the highest infectious disease burden despite an increase in investments. These include investments in malaria, HIV/AIDS, tuberculosis, as well as in communicable diseases. The global targets are to reduce the burden of these diseases through improved surveillance, prevention of outbreaks, effective case management, elimination and eventually, eradication. Achieving these targets, however, is limited by the poor geographic descriptions of the disease burden. Of the big five infectious disease burdens, malaria is the most advanced in terms of mapping its distribution. Malaria cartography has since formed the evidence-base for the design of many national malaria control programmes. This chapter focuses on malaria as an example, demonstrating its geographical descriptions. The availability of georeferenced malaria case data whether based on prevalence or incidence indicators has been used extensively in the mapping of geographical extents at national and sub-national scales. However, routine surveillance data is emerging as a valuable methodology of tracking burden in sub-Saharan Africa. A particular focus of this chapter is the use of routine national health systems surveillance data to describe, at a fine-scale, the distribution of malaria. However, routine data can be applied to the cartographic description of other diseases beyond malaria. The methodological aspects of burden estimation from routine surveillance platforms and cartography are highlighted.

AbstractEndocrine-disrupting compounds (EDCs) as emerging pollutants and multi-target agents have accumulated in the environment at concentration levels inducing significant effects on planet and obviously on all living species so that public concern about the impact of EDCs is constantly growing.Moreover, there are many contaminants in the environment which have never been examined. Even low-level exposure to these chemicals can have significant effects, and the same dose response can have different effects on individuals. Furthermore, the cumulative effects of these chemicals are yet to be studied, thus the effect on human beings is not fully understood. Anyway the health consequences of these chemicals have been particularly studied on reproductive system. Male reproductive health, especially, has represented ideal target for analysing the effects and mechanisms of damage to health of these chemical compounds. This field of health is, indeed, critical for the future of society, not only for interdisciplinary approach of several specialists and institutions involved but also for the educational mission of new generations especially in the vulnerable adolescent period; a mission, about lifestyle, diet, behaviour, personal and social awareness to reduce the exposure to EDCs and prevent non-communicable diseases (NCDs). In this chapter, we will discuss policy Implication and Community Interventions to reduce EDCs Exposure for minimisation health damages in the frame of more recent knowledge on these contaminants and proposing how hazard-based approach to guide and reach the regulations should be preferred to the risk-based one. This approach is particularly important to safeguard the male and female reproductive system, which is the most exposed one to environmental stress.

AbstractThe increasing complexity and dynamicity of our society (and world of work) have meant that healthcare systems have and continue to change and consequently the state of healthcare systems continues to assume different characteristics. The causes of mortality are an excellent example of this rapid transformation: non-communicable diseases have become the leading cause of death, according to World Health Organization (WHO) data, but at the same time there are new problems emerging such as infectious diseases, like Ebola or some forms of influenza, which occur unexpectedly or without advanced warning. Many of these new diseases diffuse rapidly through the different parts of the globe due to the increasingly interconnected nature of the world. Another example of the healthcare transformation is the innovation associated with the introduction and development of advanced communication and technology systems (such as minimally invasive surgery and robotics, transplantation, automated antiblastic preparation) at all levels of care. Consequently, the social and technical dimensions of healthcare are becoming more and more complex and provide a significant challenge for all the stakeholders in the system to make sense of and ensure high quality healthcare. These stakeholders include but are not limited to patients and their families, caregivers, clinicians, managers, policymakers, regulators, and politicians. It is an inescapable truth that Humans are always going to be part of the healthcare systems, and it is these human, who by their very nature introduce variability and complexity to the system (we do not necessarily view this as a negative and this chapter will illustrate). A microlevel a central relationship in focus is that between the clinician and the patient, two human beings, making the health system a very peculiar organization compared to similarly high-risk organizations such as aviation or nuclear energy. This double human being system [1] requires significant effort (good design) in managing unpredictability through the development of personal and organization skills, such as the ability to react positively and rapidly to unexpected events and to adopt a resilient strategy for survival and advancement. In contrast to other similar industries, in terms of level of risk and system safety, healthcare settings are still plagued by numerous errors and negative events involving humans (and other elements) at various levels within the system. The emotional involvement is very high due to the exposure to social relationships daily and results in significant challenges to address both technical and non-technical issues simultaneously.

AbstractThe Arab World has sadly witnessed protracted conflict affecting several of its regions in recent years. Conflict-affected populations have been significantly impacted by chronic deficiencies in medical care including unprecedented numbers of refugees and migrants requiring ongoing support in neighbouring countries. Humanitarian response to crisis has historically focused on the supply of nutrition, water, and emergent trauma relief, followed by control and prevention of infections. Prevention and management of non-communicable diseases, specifically cancer, have been relatively neglected; however, with regional conflicts lasting for many years, cancer care has become an increasingly urgent issue to be addressed. The humanitarian community has stressed the need for situational assessment regarding disease prevalence and available resources, with identification of specific regional challenges. Lack of infrastructure, important diagnostic and treatment modalities, clinical experts and regulatory bodies are the main obstacles to cancer care at all stages from screening and prevention to therapeutics and palliation. The initial step to improving cancer care provision should be implementing needs-based priority assessment and policies within the context of governing bodies that control and monitor performance. Collaboration and coordination among national and international organizations and stakeholders are essential to improve data collection on which to base resource allocation and address growing disparities in cancer outcomes.

AbstractBiofilms are a community of microorganisms formed on both abiotic and biotic surfaces. These colonies play a vital role in the virulent life cycle of bacteria. Bacteria communicate intrinsically and extrinsically to grow and eventually disperse their virulent factors, ultimately leading to diseases. Biofilm dispersion is the last stage in this life cycle; at this stage, the biofilm has completed maturation. The microorganism then disperses as the biofilm ruptures and assumes a planktonic lifestyle until they find a new surface to attach to and repeat the cycle. This mechanism plays a vital role in the pathogenicity of the microorganism and can be triggered prematurely to disrupt the microorganism's virulent nature. In this mini-review, we have summarized biofilm dispersion, its mechanisms, and the factors influenced by, focusing on their effect on the pathogen's virulence. We have also discussed the significance of quorum sensing and the modern methods used to develop quorum sensing inhibitors through in-silico approaches.
