Based on these assumptions and in keeping with the theme of this book, this chapter will focus primarily on anesthetic considerations in homeland disasters likely to require the presence of the anesthesiologist in the out-of-hospital or pre-hospital environment. Although most anesthesiologists are adept at handling multiple trauma casualties in the familiar setting of the operating room, even during disasters, this fact does not necessarily apply to anesthesiologists’ expertise in the management of casualties of earthquakes or chemical or biological incidents outside the operating room.<sup>2</sup> In order to understand the context within which anesthesiologists might be asked to function in the out-of-operating room setting during disaster response, we will devote a part of this monograph to a brief review of the disaster management functions of prehospital emergency medical services (EMS)/trauma systems. We will also describe the reorganization of hospital and intensive care services necessary to handle a surge of incoming critically injured or ill casualties. Our focus will be the role of the anesthesiologist, working in partnership with community or local EMS/trauma system and its network of hospitals, since the local EMS/ambulance system constitutes the functional unit of disaster medical response in the United States. We will end with a brief description of the major challenges we face in the delivery of intensive care services in mass and catastrophic casualty disasters.

From birth to death, every human being leaves a long medical history consisting of laboratory exams, records of medical consultations, records, and hospitalizations, as well as any other important information that affects the patient's health. These are known today as Electronic Health Records (EHR) or Electronic Medical Records (EMR). However, because a person's lifestyle and health are continuously changing, most of this medical information is distributed among different institutions, cities, and even countries where the specific processes were undertaken, in possession of health insurance providers or even hidden inside a drawer of the patient's home. Therefore, aiming to enhance the availability of improved medical services at reduced costs, modern information technology is being increasingly used in the healthcare sector. Researchers, developers, and companies have made efforts to develop mobile, Web, desktop, and enterprise e-health applications raising the importance of interoperability and data exchange between e-health applications and Health Information Systems (HIS). In this regard, Cloud Computing (CC) promises low cost, high scalability, availability, and disaster recoverability, which can be a natural solution for some of the problems faced in storing and analyzing EMRs. However, CC, which is mainly defined to address the use of scalable and often virtualized resources, is still evolving. New, specific collaboration models among service providers are needed for enabling effective service collaboration, allowing the process of serving consumers to be more efficient. In this chapter, the current state and trends of CC in healthcare are presented as well as a detailed collaboration model based on intelligent agents focusing on the EHR sharing subject. This model for enabling effective service in cloud systems is based on a recent research proposal related to defining a collaboration mechanism by means of Scout Movement. The chapter also includes details on the way in which services and service providers are clearly defined in this particular system.

The use of electronic medical record (EMR) systems is transforming health care delivery in hospitals. Perioperative nurses work in a unique high-risk health setting, hence require specific considerations for EMR implementation. This research explored perioperative nurses’ perceptions of facilitators and barriers to the implementation of an EMR in their workplace to make context-specific recommendations about strategies to optimise EMR adoption. Using a qualitative exploratory descriptive design, focus group data were collected from 27 perioperative nurses across three hospital sites. Thematic analyses revealed three themes: 1) The world is going to change; 2) What does it mean for me? and 3) We can do it, but we have some reservations. Mapping coded data to the Theoretical Domains Framework identified prominent facilitators and barriers, and informed recommended implementation strategies for EMR adoption by perioperative nurses.

Today, enterprise systems are integrated across wired and wireless networks. Enterprise Mobile Service Computing (EMSC) is a recent development style in distributed computing, and Enterprise Mobile Service Architecture (EMSA) is a new enterprise architectural style for mobile system integration. This chapter introduces the concepts of EMSC, discusses the opportunities, and addresses mobile constraints and challenges in EMSC. The mobile constraints include aspects relating to mobile hardware, software, networking, and mobility. Many issues such as availability, performance, and security are encountered due to these constraints. To address these challenges in EMSC, the chapter proposes seven architectural views: Enterprise Mobile Service, Enterprise Mobile Service Consumer, Enterprise Mobile Service Data, Enterprise Mobile Service Process, Enterprise Mobile Service Infrastructure, Enterprise Mobile Service Management, and Enterprise Mobile Service Quality. Each is described with principles, design constraints, and emerging technologies. Early information systems were mainly built around secondary, administrative processes of the value chain (e.g., accounting). But since the Internet came into use, more and more primary processes have become accessible to automation: customer acquisition, ordering, billing and, in the case of intangible goods such as software, even delivery. To facilitate this complex task, we suggest that the relevant parts of the enterprise be modeled according to the MEMO (Multi-perspective Enterprise MOdeling) method. It allows for the description of an enterprise on three levels-strategy, organization and information system-and from four angles-process, structure, resources and goals. All partial models for the views are integrated via a common object-oriented core. In this framework we suggest a modeling language for the IS layer, the Event-driven Method Chain (EMC), a process-oriented language based on Event-driven Process Chains (EPCs), which we adapt to fit both the MEMO framework and the object-oriented paradigm, thus making it suitable for the development of Web-based applications in an object-oriented programming language. To illustrate this we use the example of a software trading company.
